Traditional cost accounting lost its relevance, and those who use it for decision-making may cause damages to their organizations. Traditional cost accounting is no longer valid because of the changes in healthcare cost structures and changes in the competitive environment. The chapter shows that the assumptions of traditional cost accounting that were valid at the beginning of the 20th century are no longer valid for analyzing costs of healthcare service organizations. Reliance on the cost of the product or the cost of the service may lead to the loss of business opportunities on the one hand and interference in the decision-making processes on the other hand. Chapter 16 will present alternatives for traditional cost accounting that are appropriate for healthcare organization in the modern business environment.

More than any other branch of medicine, the practice of neurology depends on the classical methods of intuitive conversation, structured examination, and selective investigation. We teach the importance of eliciting an accurate neurological history. The key symptoms at onset are identified and their subsequent course defined. For the experienced clinician, this process becomes routine, efficient, and quick. The competent neurologist is the one who instinctively senses relevant components of the history, appreciates the most likely underlying disease mechanisms, reliably elicits the relevant physical signs, knows which investigations are necessary and assesses their relevance in the clinical context, provides a sensible clinical formulation, and communicates the situation accurately and sensitively to the patient and relatives. Rather than slavishly collecting an encyclopaedia of facts, in which the key issues may be lost in a surfeit of redundant information, the critical components are sifted and the subsequent conversation steered down an algorithm that seeks anatomical, physiological, and pathological explanations for what the patient describes.

This chapter examines the Chinese Cultural Revolution not so much to “explain” why it happened. Rather, it demonstrates the relevance of traditional values and certain child-rearing practices to psychosocial explanations of how mass atrocities were perpetrated. It focuses on the genocide of the Maoist revolution and its dystopian project of peasant collectivization. The Cultural Revolution was political in its imposition of the Communist Party structure throughout China; but it was also socioeconomic and thus essentially about food, its production and distribution at macrolevels of social organization and mobilization. During the Maoist revolution, macabresque transgressions had to be displayed, that is, performed before audiences comprised not only of perpetrators but also of community witnesses. Communal desire for vengeance over whatever was dramatized as “lost” represents a form of motivation relevant to explanations of sadism and its executions in the movement from filial piety to revolutionary shame and rage.

This chapter traces the Bible’s path through the history of American public education beginning in the colonial period, where it was central to the project of education, through the Common School movement, where its relevance was challenged as Enlightenment and scientific reasoning took hold. By the turn of the twentieth century, the Bible had lost its stronghold on public schools and the contentious relationship was cemented through a series of court cases that continue to impact policy and curriculum to the present time. The chapter concludes by highlighting several contemporary policies implemented to try to return the Bible, in some fashion, to public schools.

With reportedly over USD50 billion lost annually through graft and illicit practices, combating corruption in Africa has been challenging. However, laws and policies at the continental, regional and national levels have been promulgated and enacted by African leaders. These initiatives have included the establishment of anti-corruption agencies mandated to tackle graft at national level, as well as coordinate bodies at regional and continental levels to ensure the harmonisation of normative standards and the adoption of best practices in the fight against corruption. Yet, given the disparity between the apparent impunity enjoyed by public servants and the anti-corruption rhetoric of governments in the region, the effectiveness of these agencies is viewed with scepticism. This continent-wide study of anti-corruption agencies aims to gauge their relevance and effectiveness by assessing their independence, mandate, available resources, national ownership, capacities and strategic positioning. These surveys include evidence-based recommendations calling for stronger, more relevant and effective institutions that are directly aligned to regional and continental anti-corruption frameworks, such as the African Union Convention on Preventing and Combating Corruption (AUCPCC), which the three countries in this current report Kenya, Tanzania and Uganda have all ratified.

Against a generalized loss of meaning in society, social scientists find it hard to undertake relevant research that addresses problems facing our world. Science has turned from a vocation aimed at improving the lot of humanity to a careerist game dominated by publishing hits in starred journals. Instrumental rewards replace the passion for discovery and the intrinsic quest for knowledge. Competition among academics and academic institutions, such as journals, universities, and professional bodies, is not intrinsically harmful. Competition in the social sciences, however, is currently resulting in large quantities of formulaic publications, increasing specialization, faddishness, opportunism, and a general ironing out of originality and relevance. Academic authorship and the voice of individual scholars is wiped out as most papers are co-authored by several researchers, each a specialist in his or her area. The result is a devaluation of scholarship and a privileging of technical expertise in narrow disciplinary areas.

Bull argued that in European international society, the ‘diplomatic culture’, the common stock of ideas and values the representatives of states shared had been strengthened by an ‘international political culture’, the intellectual and moral culture that determined societal attitudes towards the states-system. With the expansion of international society, he contended, the diplomatic culture had lost much of its earlier foundation in the normative commitments specific to European international society. It was conceivable, Bull argued, that a new ‘cosmopolitan culture’ will succeed in binding peoples together in the first universal society of states. To consider those issues further, this essay draws on Elias’s writings to show how conceptions of ‘civilized manners’ and ‘civilized’ statehood linked the international political and diplomatic cultures. The discussion also considers some recent writings on the civilizational dynamics of world politics that have special relevance for Bull’s reflections on how those two cultures might develop in future.

The International Fruit Company case is rightly famous for two reasons: (1) the succession of the European Community to its member states in the General Agreement on Tariffs and Trade (GATT) and (2) the lack of direct effect of art. XI (the prohibition of quantitative restrictions) and of the GATT in general. On point (2) the case has largely lost its initial relevance, since the Court of Justice (ECJ), though still of the same view with respect to the GATT’s successor, the WTO, has considerably developed and refined its initial reasoning. It is on point (1) that the case has continued importance, because it sheds light on the comparable, but different situation in which the EU effectively follows the rules of international organizations or of treaty bodies, to which all member states are parties, but without the possibility for the EU to accede to these organizations and treaty bodies.

This chapter highlights the historical evolution and peculiarities of Italian translations of Milton’s epics. The homeland of classicism and stronghold of Catholicism often subordinated aesthetic judgement to moral judgement, and Milton appeared too unorthodox and irregular to suit the cultural and religious expectations of Italian readers. Thus, despite its relevance in Milton’s own cultural development, Italy has been a major contributor to the ‘conspiracy of silence’ noted by Waldock half a century ago. The common practice of Italian translators has been to self-censor or ‘improve’ Paradise Lost drastically. Borgogni describes Rolli’s longed-for fidelity yielding to contemporary religio-cultural pressures and Papi’s domesticating adaptations and expurgations. He devotes special attention to the ‘conspiracy within the conspiracy’—namely, the neglect of Paradise Regained—with an analysis of the few nineteenth- and twentieth-century Italian versions and a discussion of the linguistic and theoretical challenges he faced in his own 2007 translation of the brief epic.

Chapter 5 reflects on Coleridge’s famous fancy–imagination distinction, which inspired the author’s own distinction between fantasy and imagination. The continuing relevance of Coleridge’s distinction lies in recognizing imagination as essentially truth-directed. Importantly, we can venture into the unreal with two quite different intentions—to become lost there, or to find ourselves. We can see the unreal world as a place of escape, fulfilling dreams in cost-free ways that set up channels of reward which so often lead to addiction and psychological enslavement (as in pornography). Or, we can see the world of the unreal as an imaginative construct for deeper epistemological purposes, to know through sympathy the varieties of human life, as life that could be ours. The term ‘imagination’ is reserved for this second approach. This imaginary is the unreal called to judgement by the real, in contrast with the pretence of reality in clichéd, sentimental, or kitsch fantasy.

Falls in the elderly are very common, and their frequency increases with aging. At a personal level, falls are associated with a subsequent fear of falling, a decline in function, increased nursing home placement, and increased use of medical services, and complications resulting from falls represent the leading cause of death from injury in geriatric population. At the more global level, falls in the elderly are associated with increased use of medical services and increased cost directly to the patient and also indirectly, if we add the number of hours of work lost by caregivers who will assume care of that elderly person after the fall. This chapter covers the definition and relevance of falls in the elderly population; etiology and risk factors for falls; evaluation and management; and assessment for and correction of risk factors. Integrative management approaches discussed in this chapter are movement and exercise, nutrition and supplements, and hormone replacement.

Neurobiological substrates for the positive reinforcement of drug abuse involve activation of the brain’s incentive salience and reward systems, such as dopamine and opioid peptides, and neurobiological substrates for the negative reinforcement of drug addiction involve the brain stress systems, such as corticotropin-releasing factor (CRF), norepinephrine, and dynorphin in the ventral striatum and extended amygdala. Decreased function of brain antistress systems further contributes to motivational dysfunction. A brain stress response system is hypothesized to be dysregulated by acute excessive drug intake, sensitized during repeated withdrawal, persist into protracted abstinence, and contribute to the allostatic changes in motivation associated with addiction. From a theoretical perspective, the combination of loss of reward function and recruitment of brain stress systems drives the compulsivity of addiction and provides a rich neurobiological substrate for therapeutic interventions.

Abstract:
In 2015, one hundred years passed since Robert Park penned his seminal article “The City: Suggestions for the investigation of human behaviour in the city environment” in the American Journal of Sociology. It provided an agenda for the Chicago school of urban sociology, which came to shape urban research for decades to come. Since 1915 much has changed, both in the urban world itself and in the urban research that reflects on those transformations. In today’s world of global cities, cities around the world have undergone dramatic development, and nowhere as dramatic as in China. In the world of urban research, Park’s human ecology approach has lost the appeal that it once had. Against this background, in this book specialists on urban China reflect on the relevance of Park’s article on “The City” – for cities in China, for urban research, and for questions about studying the social life of the city. The aim of the book is to take Park’s article as a point of departure for critical reflection on both the research on urban China and on the issues that Chinese cities face. The book offers readers a timely respite from the eruption of urban China research, to reflect on what the city in China contributes to urban studies more generally. Despite the shared starting point, the contributors represent a range of perspectives that would disrupt any notion of monolithic “Chinese school” while also pointing the way towards recurrent challenges, topics and approaches relevant for a contemporary urbanism.

Abstract:
Scholars from several different fields have studied the individual consequences of job loss and unemployment since the beginning of the twentieth century. This chapter provides a review of some important results of this research with special emphasis on the effects of unemployment on mental health. Among the topics reviewed are the size and practical relevance of these effects and the question of causality (i.e., does job loss really cause distress or are people with distress symptoms more likely to lose their jobs and remain unemployed for a longer time?) Results of moderator variables are also reviewed, as well as those concerning coping mechanisms. Then theories concerning the question of why unemployment impairs mental health (mediating mechanisms) are described, along with empirical results. Finally, some selected research findings for other outcome dimensions, ranging from physical health to political attitudes, are reported. The chapter ends with a discussion of the practical consequences of the reviewed research findings and the identification of research gaps.
