Abstract:
Mining equipment are becoming very sophisticated and complex. At the same time, mining companies modernise its operation by increased mechanisation and automation (Dhillon, 2008). That requires well-developed and planned maintenance strategy. A proper maintenance strategy and appropriate maintenance actions, are essential to keep equipment safe & reliable, decrease the overall costs, and it helps to eliminate workplace hazards. Maintenance strategies and maintenance actions, in turn, are changed and developed more than any technology (Moubray, 1997). Changing and developing of the maintenance can be due to many factors, perhaps due to the complexity of the systems or due to the development of the technology. One of the best research (developed technology) that has been done in this area is the research by Nowlan and Heap about Reliability Centered Maintenance (RCM) in 1978. RCM is defined as, “a zero-based, structured process used to identify the failure management strategies required to ensure an asset meets its mission requirements in its operational environment in the safest and cost – effective manner” (Regan, 2012). The RCM is one of the best powerful asset management strategies. RCM can be applied to any asset, nuclear power plant, aeroplane, or truck ship. According to (Nowlan & Heap, 1978), the main goal of implementation an RCM process is to provide the stated function of the facility with required reliability and availability at the lowest cost. In this master thesis, an RCM analysis process has been described in details. Moreover, two types of implementation approaches of an RCM analysis process have also been described. The intention of describing these two approaches is to see what the differences between them are and which one can be recommended for using in mining. The first approach is according to (Hinchcliffe & Smith, 2004) and the second approach is according to (Regan, 2012). Moreover, in this master thesis, there are also two case studies that have been analysed. The intention of analysing these two case-studies is to see how the RCM has been implemented. As a result of the research carried out in this master thesis, some basic streamline RCM analysis process has been recommended for using in mining. This recommended RCM analysis process could be applicable to be used in every industry section. The result of this work can be summarised that RCM is an applicable tool which can be used in mining for selecting the right maintenance approach for different equipment, systems or subsystem. RCM analysis process is discussed from different perspective. In the discussion, the misconceptions about the implementation of an RCM analysis process have been discussed. Altogether, Reliability Centered Maintenance is one of the most powerful asset management strategies that can be used in every industry section. If the RCM is applied by right people and by the right way, the result can be profitable for the mining companies.

Abstract:
Increased demands on productivity, quality and cost-efficiency affecting manufacturing trends towards increased complexity and higher degree of process automation. A major break down in such a manufacturing system may generate severe damage on productivity, environment and personnel. Using risk assessment to identify serve risks within a plant, in combination with different maintenance strategies, is one course of action to prioritising maintenance activities needed. A methodology for executing a so-called risk-based maintenance is reliability-centred maintenance, RCM. There are several benefits generated from RCM, for example, improved safety and maintenance cost-effectiveness. Though, several companies have problems to make it work. The problems that occur are many times within management and organisational (M&O) aspects, such as lack of communication and management support. Some M&O aspects of importance when introducing improvement methods as TQM and TPM are similar with the ones valid for RCM. Though, differences seem to depend on the use of RCM in a more technology environment, overshadow the affects M&O aspects really bring about when introducing it. That is probably the main reason why obstacles occur when introducing RCM. A structured step model has been developed, focusing on the preparation and planning activities when introducing RCM.

Abstract:
"Operational costs are of central importance for the economic health and sustainability of any organization. There are many contributors to these costs; some are industry specific, some are not. Factors such as organizational makeup and structure or general management and/or leadership practices all play a part, albeit, challenging to measure in terms of direct dollar correlation. Others, such as payroll, capital purchases, and asset management costs, to name a few have a more direct operational cost clearly linked to dollars and become the most practical place to look when trying to minimize operational costs. One of the critical aforementioned contributors to operational costs is that of capital asset management; in particular the issue of maintenance and repair of a company’s capital assets. More specifically, one can try to determine the best maintenance practice and schedule to use on varying systems in hopes of lowering maintenance costs and ultimately operational costs. One approach is to formulate a maintenance cost equation given the specific data and constraints available to solve a probabilistic problem through simulation. This study proposes a methodology that could be used as a tool to determine what maintenance practices to use on varying systems, sub-systems, and components. The focal point of this methodology is to formulate viable simulation logic. The logic takes into account maintenance costs that must be identified and defined. Additionally, coupled with this is the need to create a Weibull Distribution, which helps predict the next failure based on historical data. By matching the maintenance cost with the Weibull Distribution of each system, sub-system or component the simulation logic or equation is created. With this model in hand, simulations are run using Monte Carlo Simulation. In the end, an optimal schedule is determined based on the input. This thesis has three main deliverables. First, a maintenance methodology which assists in determining optimal component change out schedule based on historical data is created. Secondly, from the information gathered from Industrial Support Command Alameda*, ideal systems to target are identified. Thirdly, an implementation strategy is offered. Lastly, though not a primary deliverable, this study also offers some other maintenance related miscellaneous findings and/or recommendations. "

Abstract:
Process plants come with a wide variety of engineering challenges. Each process has uniqueoperating conditions that require careful operation and maintenance programs to keep the process equipment in a condition that is safe for operation. A pilot-scale plant was built in Luleå to demonstrate the possibility of carbon dioxide removal from blast furnace gas usingthe Sorption Enhanced Water Gas Shift (SEWGS) technology. One of the challenges facingthe SEWGS pilot plant was the lack of a maintenance plan which was the motivation for this thesis. The aim of this thesis was to create a maintenance plan for the SEWGS pilot plant. Studying the process conditions of the pilot plant was the starting point since an understanding of the unique operating conditions was necessary for creating a maintenance plan for the pilot plant’s equipment. The Reliability-centered maintenance (RCM) method was chosen to create the maintenance plan. The study began with determining the objectives and contents of the analysis. The next step was a functional failure analysis in the form of a Failure Mode and Effect Criticality Analysis (FMECA) covering all the equipment in the plant. Afterwards the failure consequences were assessed based on a criticality assessment matrix to determine which equipment necessitate the performance of maintenance activities. The maintenance plan was created based upon the criticality of the equipment and the selected maintenance activities were based on the recommendations of manufacturers. The failure analysis found that the blast furnace gas compressor is the most critical for the process. The boiler and superheater have been found to be highly critical due to the dangerous safety consequences of their failure. In addition, all pressure safety valves have been found to be highly critical due to the hidden nature of their failure. The maintenance plan addresses these critical items and other less critical items while some non-critical items have been removed from the plan due to the negligible consequences of their failure.

This thesis is an effort to implement Reliability Centered Maintenance “RCM” for C20trains at Tunnelbanan Teknik Stockholm AB. In depth study is performed along withmaintenance team of TBT at center of excellence, a facility for maintenance workshop. RCM is a philosophy or framework to ensure efficient working of assets according todefined and required operating context. In general, the successful implementation ofRCM can achieve improvements through lesser maintenance, operating techniquechanges, and generating inclusive maintenance plan. RCM is also one of the best toolsavailable to identify and prevent equipment failures. Therefore TBT has decided tointroduce RCM to ensure effective services with great confidence and precision. The selection of critical system of C20 train was obligatory to initiate implementation ofRCM, and the door system was considered a very complicated system with maximumfailures that results in traffic disturbances and stoppages. Through literature study,depots & workshop visits, brainstorming engineers & train drivers etc, the requiredtasks to be performed were lay down. A template that illustrates details such as FMECA,risk analysis, current maintenance strategy and the recommended actions wasprepared and new maintenance regime along with the changes in work instructionswere proposed. The cost effectiveness as well as safety assessment was carried out.Moreover, the efficacy of RCM analysis is directly proportional to quality and accuracyof available failure data, and the proposed time scale for thesis was underestimated andthe reason behind was the convolution of CMMS as well as poor quality of failure data. The study results are officially approved by all stakeholders, i.e. SL, MTR Stockholm,MTR HK and TBT. This work can guide the officials to take imperative decisions foroptimization of maintenance practices, also the study for C20 door system can be theroadmap for comprehensive implementation of RCM at TBT Stockholm AB.

Asset management is getting increasingly important in nearly all fields, especially inthe electric power engineering. It is mainly due to the following two reasons. First isthe high investment cost include the design cost, construction cost, equipment costand the high maintenance cost. Another reason is that there is always a high penaltyfee for the system operator if an interruption happened in the system. Besides, due tothe deregulation of electricity market in these years, the electricity utilities are payingmore attentions to the investment and maintenance cost. And one of their main goalsis to maximize the maintenance performance. So the challenge for the systems is toprovide high-reliability power to the customs and meanwhile be cost-effective for thesuppliers. Reliability Centered Asset Management (RCAM) is one of the bestmethods to solve this problem.The basic RCAM method is introduced first in this thesis. The model includes themaintenance strategy definition, the maintenance cost calculation and an optimizationmodel. Based on the basic model some improvements are added and a new model isproposed. The improvements include the new improvement maintenance strategy,increasing failure rate and a new objective function. The new model is also able toprovide a time-based maintenance plan.The simulation is done to a Swedish distribution system-Birka system by GAMS. Theresults and a sensitivity analysis is presented. A maintenance strategy for 58components and in 120 months is finally found. The impact on the changing failurerate is also shown for the whole peroid.

The current economic climate in South Africa requires organisations to optimise available resources – human and otherwise – to successfully sustain business operations. This is especially true for the growing SMME sector in South Africa. Organisations utilising hydrocarbon based products for input products into their respective process - specifically fuels and lubricants – face an even greater challenge in optimising resource usage as fuel and lubricant prices have increased substantially over the last decade. Automated and advanced technological solutions to properly and effectively manage these fuel and lubricant resources must be employed within organisations. This is critical as fuel and lubricants can constitute a large part of the annual expenditure within an organisation. Such organisations can include: • Mining operations • Transport operations • Agricultural operations • Maritime operations Newcom Fluid Management has developed a Fuel & Fluid Management Solution which consists of an electronic control system and various other elements to assist organisations in managing these fuel and lubricant resources. The Newcom FMS makes use of physical hardware on the client’s site as well as an internet based software system to control, monitor and report on all fuel and lubricant usages. There is also a large human resource element behind the system which continuously maintains these remote systems such that clients can enjoy the availability of fuels and lubricants when desired. The Newcom FMS must be properly maintained and resources optimised to allow Newcom to not only make a profit, but to stay competitive in the market place by providing clients with a sustainable and available solution. Therefore a properly researched maintenance management strategy must be developed for Newcom and the Newcom FMS solution to ensure that not only the client’s resources are optimised, but also Newcom’s resources in order to maintain the Newcom FMS. The aim of this research was to: • Research the theory behind maintenance management; • Identify and develop a sustainable maintenance strategy for the Newcom FMS solution taking into account the success factors as required by Newcom; • Test the experimental strategy and the current maintenance program at current Newcom clients and capture data on the two strategies employed; • Analyse and compare the experimental data to determine the effectiveness of the experimental maintenance strategy versus the corrective strategy; • Provide the experimental maintenance strategy “product” to Newcom along with the data obtained in the experiment as well as the recommendations on the way forward with the data obtained from the experiment serving as inputs. The parameters which were measured in the experiment were: • System availability; • Strategy expenditure and • Resources usage. The parameters were selected by Newcom as being the most pertinent to their current operational environment. O atual ambiente competitivo, onde baixo custo, alta qualidade e um grau crescente de customização são exigências para a sobrevivência das empresas, tem influenciado as ações dos profissionais de manutenção com vistas ao aprimoramento das políticas de manutenção. Como resultado, tem-se na área de manutenção industrial, uma grande quantidade de métodos, software, modelos e ferramentas de gestão disponíveis. Dentre eles, dois métodos se destacam: o RCM (Reliability Centered Maintenance ou Manutenção Centrada em Confiabilidade), de origem norte-americana, e a TPM (Total Productive Maintenance ou Manutenção Produtiva Total), de origem japonesa. A TPM promove a integração total entre homem, máquina e empresa, onde a manutenção dos meios de produção passa a constituir uma responsabilidade de todos. A utilização da TPM contempla a implementação de pilares de sustentação, sendo um deles, a manutenção planejada. Entretanto, a TPM não especifica a estratégia a ser adotada pela manutenção planejada. O RCM é uma metodologia lógica de procedimentos que objetiva estabelecer uma manutenção preditiva e preventiva para alcançar, de maneira efetiva e eficiente, os níveis de segurança e confiabilidade requeridas para cada equipamento. Considerando um ambiente com a TPM já implementada, o objetivo deste trabalho é a melhoria do pilar Manutenção Planejada da TPM através da utilização do RCM para nortear as estratégias de manutenção em empresas industriais. A sistemática proposta é ilustrada através de um estudo de caso na indústria de conformação de metais.
Today’s competitive business environment, in which low costs, high quality and an ever increasing degree of customization are essential for companies’ survival has influenced maintenance professionals’ courses of action, aiming at improving maintenance policies. As a result, in the industrial maintenance sector there is a great number of methods, software and management tools available. Among them, two stand out: the RCM (Reliability Centered Maintenance), a North American approach, and the TPM (Total productive Maintenance), a Japanese approach. TPM promotes total integration among manpower, machines and the company itself, and the maintenance of the means of production is everybody’s responsibility. The introduction of TPM demands the implementation of the so called sustaining pillars, and planned maintenance is one of them. However, TPM does not specify the strategy to be followed by planned maintenance. RCM is a set of logical procedures aiming at setting up predictive and preventive maintenance in order to effectively and efficiently achieve the security and reliability standards required for each equipment. Considering an environment where TPM has already been implemented, this paper aims at optimizing Planned Maintenance of the TPM by using RCM to guide maintenance strategies in industries. The system proposed is illustrated through a case study in a metal cold roll forming factory.

The objective of this thesis is to design and develop a knowledge base model to support the maintenance system structure. The aim of this model is to identify the failure modes which are the heart of maintenance system through the functional analysis and then serves as adecision support system to define the maintenance tasks and finally to implement a preventive maintenance task. This knowledge base management system is suitable to design and develop maintenance system since it encompasses all necessary steps in maintenance area. Moreover, it is capable of being integrated with other knowledge base systems. The structure and the environment of this knowledge base system is flexible to allow users to deploy different kinds of tools which they will. It is also a well structured approach to develop, debug, upgrade and trace. In this thesis, the CommonKADS methodology is used as the knowledge base methodology. At the first step, a knowledge base system is developed to create the maintenance system infrastructure. To implement this, Reliability-Centered Maintenance (RCM) has been chosen as the method to design a maintenance system. In order to make it more specified, a Spindlesubsystem is taken as a case study to make the model clearer. Secondly, another knowledge base system is developed for decision making process to select the suitable maintenance task and finally, a general knowledge base model is developed for condition-based monitoring onSpindle. In chapter 1, background, previous works and gap analysis have been surveyed. Then in chapter 2 the methodology and tools have been discussed and described. Design and development the knowledge base for maintenance system is described in detail in chapter 3 and finally in chapter 4, the conclusion and the future works are presented.

In the surveyed published literature, there is not a single model which incorporates all maintenance cost components to analyze the effect of different maintenance strategies for generator companies (GENCOs). The work enclosed in this thesis demonstrates that there is a considerable requirement for accurately modelling cost components of the maintenance model, to be used in maintenance scheduling for deregulated power system, in order to attain a superior schedule with major financial and operational impact. This research investigates and models most cost factors that affect the maintenance activities of the deregulated GENCOs, and demonstrates the utilization of the developed cost models in maintenance scheduling. It also presents the data gathering process for the developed maintenance cost model. A generator maintenance scheduling model that considers direct and indirect maintenance costs, opportunity costs (i.e. loss of customer goodwill), effective maintenance strategies, failures, and interruptions is developed. A Genetic Algorithm (GA) based approach is employed to achieve maintenance schedules to various generators maintenance scenarios. An Analytical Hierarchy Process (AHP) approach is proposed for modelling customer goodwill. The maintenance model was redeveloped under the Reliability Centred Maintenance (RCM) strategy to analyze the effect of a maintenance strategy on maintenance costs. Case studies are presented to demonstrate the utilisation of the developed models.The investigation shows that the market prices, opportunity costs and maintenance strategy have an effect on the final maintenance schedule. The research demonstrates that the cost components are critical factors to achieve an effective maintenance schedule, and they must be considered and carefully modelled in order to reflect more realistic situation for maintenance scheduling of generator units in deregulation environment.

A generator maintenance scheduling model that considers direct and indirect maintenance costs, opportunity costs (i.e. loss of customer goodwill), effective maintenance strategies, failures, and interruptions is developed. A Genetic Algorithm (GA) based approach is employed to achieve maintenance schedules to various generators maintenance scenarios. An Analytical Hierarchy Process (AHP) approach is proposed for modelling customer goodwill. The maintenance model was redeveloped under the Reliability Centred Maintenance (RCM) strategy to analyze the effect of a maintenance strategy on maintenance costs. Case studies are presented to demonstrate the utilisation of the developed models.The investigation shows that the market prices, opportunity costs and maintenance strategy have an effect on the final maintenance schedule. The research demonstrates that the cost components are critical factors to achieve an effective maintenance schedule, and they must be considered and carefully modelled in order to reflect more realistic situation for maintenance scheduling of generator units in deregulation environment.

Abstract:

The Swedish electricity market was de-regulated in 1996, followed by new laws and a new regulation applied to the natural monopolies of electrical distribution systems (EDS). These circumstances have motivated distribution systems operators (DSOs) to introduce more comprehensive analysis methods. The laws, the regulation and additional incentives have been investigated within this work and results from this study can be valuable when developing risk methods or other quantitative methods applied to EDS. This tendency is not unique for Sweden, the results from a comparative study of customer outage compensation laws between Sweden and UK is for example included.

As a part of investigating these incentives, studies of the Swedish regulation of customer network tariffs have been performed which provide valuable learning when developing regulation models in different countries. The Swedish regulatory model, referred to as the Network Performance Assessment Model (NPAM), was created for one of the first de-regulated electricity markets in the world and has a unique and novel approach. For the first time, an overall presentation of the NPAM has been performed including description of the underlying theory as a part of this work. However, the model has been met by difficulties and the future usage of the model is uncertain. Furthermore, the robustness of the NPAM has been evaluated in two studies with the main conclusion that the NPAM is sensitive toward small variations in input data. Results from these studies are explained theoretically investigating algorithms of the NPAM.

A pre-study of a project on developing international test systems is presented and this ongoing project aims to be a useful input when developing risk methods. An application study is included with the approach to systematically describe the overall risk management process at a DSO including an evaluation and ideas of future developments. The main objective is to support DSOs in the development of risk management, and to give academic reference material to utilize industry experience. An idea of a risk management classification has been concluded from this application study. The study provides an input to the final objective of a quantitative risk method.

En el capítulo 1 se presenta una revisión bibliográfica de los temas principales sobre los que se fundamenta la propuesta de mejora: el sector papelero en el Perú, la evolución del mantenimiento, definición de términos, tipos de mantenimiento, técnicas de mantenimiento avanzado (RCM, TPM), costos de mantenimiento, indicadores, herramientas de mejora continua y casos de éxitos en la implementación del RCM. En el capítulo 2 se realiza una descripción general de la empresa (reseña histórica, misión, visión, valores, organigrama, productos, clientes, ventas y el proceso productivo) para finalmente llegar al detalle de la situación actual. Luego se describe y cuantifica el principal problema que afecta al cumplimiento del plan de producción anual, posteriormente se hace el análisis de causa raíz para identificar las principales causas que están generando el problema. En el capítulo 3 se evalúa tres posibles soluciones para el problema (RCM, TPM, CBM), para la elección de la metodología a aplicar se hace uso del ranking de factores y se desarrolla cada una de las fases del RCM. En el capítulo 4 se valida la propuesta para lo cual se hace un modelamiento del proceso actual y propuesto utilizando el Software de simulación ARENA, también se hace una evaluación económica-financiera para determinar la viabilidad del proyecto. El cierre de la tesis se da mediante la presentación de las conclusiones y recomendaciones.
This document develops a proposal to increase the production of notebooks in the R17 line of the Panasa company, applying the RCM methodology. Chapter 1 presents a bibliographic review of the main topics on which the proposal for improvement is based: the paper industry in Peru, the evolution of maintenance, definition of terms, types of maintenance, advanced maintenance techniques (RCM, TPM), maintenance costs, indicators, continuous improvement tools and success stories in the implementation of the RCM. In Chapter 2 a general description of the company (historical review, mission, vision, values, organization chart, products, customers, sales and the production process) is made to finally get to the detail of the current situation. Then the main problem that affects the fulfillment of the annual production plan is described and quantified, then the root cause analysis is done to identify the main causes that are generating the problem. Chapter 3 evaluates three possible solutions to the problem (RCM, TPM, CBM), for the choice of the methodology to be applied, the ranking of factors is used and each phase of the RCM is developed. Chapter 4 validates the proposal for which a modeling of the current and proposed process is done using the ARENA simulation software, an economic-financial evaluation is also made to determine the viability of the project. The thesis is closed by presenting the conclusions and recommendations.

A função manutenção vem sendo considerada como fator estratégico para as empresas, pois através do alinhamento de suas políticas corporativas e integração de seus programas de gestão de ativos, de riscos e de ciclo de vida de suas unidades de negócios, as empresas vêm buscando a constante redução de custos e a melhoria de seus resultados operacionais. E, assim, obtendo maior competitividade. A Manutenção Centrada em Confiabilidade (RCM) é um método já bem disseminado por todo o mundo e que, ao longo dos anos, vem promovendo estes diferenciais estratégicos através de preceitos que possibilitam a elaboração de Programas de Manutenção Preventiva de custo-eficaz, através de um método para a definição de políticas de manutenção mais adequadas, com o foco na manutenção da função dos ativos em seu contexto operacional. Ao longo dos anos, o método RCM vem sendo aplicado em inúmeros estudos de casos em diferentes empresas de diversos seguimentos, onde podem ser observadas novas adaptações ou simplificações do método RCM clássico. Estas adaptações buscam uma maior adequação as particularidades destas empresas e/ou um retorno mais rápido de resultados. O setor nuclear de geração de energia foi um dos pioneiros na adoção e disseminação do RCM, e vem desenvolvendo processos simplificados de aplicação do RCM, como o “Streamlinned RCM” e o “método PMO” (Otimização do Programa de Manutenção, do inglês: Preventive Maintenance Optimization). Estes estudos mostram que o método PMO apresenta uma maior flexibilidade, o que permite a adoção de diferentes estratégias de aplicação que, por sua vez, têm trazido resultados expressivos para as empresas, através da otimização dos Programas de Manutenção já existentes. Com base na literatura, neste trabalho são abordadas questões referentes ao RCM e sua contextualização na área nuclear, estudos sobre os métodos simplificados do RCM e o desenvolvimento do método PMO. Por fim, é realizada uma aplicação prática do método PMO sobre os sistemas relacionados e subsistemas do Sistema de Remoção de Calor Residual (JN), mais especificamente, sobre as Bombas de injeção de segurança do Sistema de Injeção de Alta Pressão (JND) da Usina Nuclear Angra 2. Através dos resultados obtidos com esta aplicação, pretende-se otimizar o Programa de Manutenção da Planta (PMP) referente a estes equipamentos e, assim, validar o método PMO como ferramenta para a melhoria contínua do Programa de Gestão de Ativos e Ciclo de Vida da Usina Nuclear Angra 2.
Corporations tend to consider maintenance work a strategic element. It is through maintenance—especially the alignment between corporate policies and integration of their programs to manage assets, risks, and life cycles of business units—that corporations try to continuously improve their results, reduce their operational costs, and, therefore, increase their competitiveness. Reliability Centered Maintenance (RCM) is a popular method across the world; it has been promoting competitiveness through concepts that allow the design of cost-effective Preventive Maintenance Programs These programs are effective because they entail appropriate maintenance policies that are focused on the preservation of the assets functions in their operational context and on the formation of technical knowledge basis supported by hard data. Throughout the years, RCM has been applied in numerous case studies and in different companies engaged in a variety of market segments. Such diversity in the application of RCM allowed us to observe new adaptations and variations of the classic method. Such adaptations aim to better respond to specific operational contexts in and within production units, as well as achieve faster results. The nuclear power sector has pioneered regarding the adoption and dissemination of RCM; it has been developing simplified versions of RCM, such as the “Streamlined RCM” and the PMO (Preventive Maintenance Optimization). These studies demonstrate that the PMO presents enhanced flexibility, which allows the adoption of different strategies; such enhanced flexibility brings expressive results to corporations as pre-existing maintenance programs are optimized. Based on the currently available literature, this dissertation addresses numerous questions regarding RCM and its application to nuclear power segments. It also addresses studies about simplified versions of RCM and the development of the PMO method. The discussion is supplemented with a practical application of the PMO method regarding auxiliary systems and sub-systems of Removal of Residual Heat System (JN), especially those regarding the security injection pumps of the High Pressure Injection System (JND) at Angra II Nuclear Plant. Through the results obtained from this application, it is possible to optimize the Maintenance Program of these equipments, and therefore, validate the PMO method as a tool of continuous improvement of the Assets and Life Cycle Program of Angra II.

La propuesta es el diseño de una propuesta para mejorar la implementación del plan de gestión de mantenimiento en el proceso de moldeo por inyección de polímeros basado en la filosofía de las metodologías Lean Office y RCM. La aplicación se lleva a cabo en una empresa peruana de inyección de plástico donde se identificó el uso inadecuado del plan de Gestión de Mantenimiento, causado por la baja disponibilidad de repuestos y personal, el porcentaje de materia prima reprocesada y el deterioro de las Máquinas de moldeo. La importancia de resolver este problema es reducir la baja confiabilidad del equipo, la demora en la entrega de pedidos y la pérdida de producción. Los resultados esperados validado por la simulación de sistemas mostraron que la disponibilidad del personal aumentó a 83.07% y las paradas no planificadas de la máquina de inyección se redujeron en un 30%.
This professional project for the title of Industrial Engineer, design of a proposal to improve the implementation of the maintenance management plan in the polymer injection molding process based on the philosophy of the Lean Office and RCM methodologies. The application is carried out in a Peruvian plastic injection company where the damaged use of the Maintenance Management plan, the risk due to the low availability of spare parts and personnel, the percentage of raw material reprocessed and the treatment of the Machines were identified Molding The importance of solving this problem is to reduce the low reliability of the equipment, the delay in the delivery of orders and the loss of production. The expected results validated by the simulation of controlled systems that the availability of the personnel had an 83.07% and the unplanned stops of the injection machine were reduced by 30%.

En la actualidad, la importancia de la gestión de mantenimiento ha tomado mayor interés debido a la necesidad de las plantas industriales de mantener la capacidad de los equipos y los niveles de los costos operativos en los que se incurren por mantenimiento. Por consiguiente, la selección de la estrategia de mantenimiento se convierte en una labor importante, para este caso se eligió al mantenimiento centrado en la confiabilidad (RCM) como una de las herramientas de ingeniería industrial para mejorar la disponibilidad de los equipos. Éste articulo propone una visión general de la implementación del mantenimiento centrado en la confiabilidad en la línea de chancado primario en una empresa minera. Dicha implementación requiere la recopilación y el análisis de datos históricos de fallas y mantenimientos para determinar la condición actual de los equipos. Estos datos se tomaron en una mina de tajo abierto con capacidad de 117 200 tonelada/día de procesamiento de mineral. Finalmente, la implementación de RCM se justifica por los resultados que se obtuvo en la simulación de la situación mejorada de la línea de chancado primario en la cual se observa la reducción del tiempo que toman las reparaciones de mantenimiento.
Nowadays, the importance of maintenance management has increased due the needs of industrial plants to its in keeping and improving the availability of equipment and operating costs incurred for maintenance. Accordingly, the selection of the maintenance strategy becomes an important task, for this case reliability centred maintenance (RCM) was chosen as one of the industrial engineering tools to enhance the availability of equipment. This article offers an overview of the implementation of maintenance focused on the reliability of the primary crushing line in a mining company. This implementation requires the collection and analysis of historical data of failures and maintenance to determine the current condition of the equipment. These data were taken in an open-pit mine with a capacity of 117,200 tons / day of ore processing. Finally, the implementation of RCM is justified by the results obtained in the simulation of the improved situation of the primary crushing line in which the reduction in the time taken for maintenance repairs is observed.

Aujourd’hui les réseaux électriques sont exploités dans un marché dérégulé. Les gestionnaires des réseaux électriques sont tenus d’assurer un certain nombre de critères de fiabilité et de continuité du service, tout en minimisant le coût total consacré aux efforts effectués pour maintenir la fiabilité des installations. Il s’agit de trouver une stratégie, qui répond à plusieurs exigences, comme :le coût, les performances, la législation, les exigences du régulateur, etc. Cependant, le processus de prise de décision est subjectif, car chaque participant ramène sa contribution sur base de sa propre expérience. Bien que ce processus permette de trouver la « meilleure » stratégie, cette dernière n’est pas forcément la stratégie « optimale ». Ce compromis technico-économique a sensibilisé les gestionnaires des réseaux électriques à la nécessité d’un recours à des outils d’aide à la décision, qui doivent se baser sur des nouvelles approches quantitatives et une modélisation plus proches de la réalité physique.

Cette thèse rentre dans le cadre d’un projet de recherche lancé par ELIA, et dénommé COMPRIMa (Cost-Optimization Models for the Planning of the Renewal, Inspection, and Maintenance of Belgian power system facilities). Ce projet vise à développer une méthodologie qui permet de modéliser une partie du réseau électrique de transport (par les réseaux de Petri stochastiques) et de simuler son comportement dynamique sur un horizon donné (simulation de Monte Carlo). L’évaluation des indices de fiabilité permet de comparer les différents scénarios qui tentent d’améliorer les performances de l’installation. L’approche proposée est basée sur la stratégie RCM (Reliability-Centered Maintenance).

La méthodologie développée dans cette thèse permet une modélisation plus réaliste du réseau qui tient compte, entre autres, des aspects suivants :

- La corrélation quantitative entre le processus de maintenance et le processus de vieillissement des composants (par un modèle d’âge virtuel) ;

- Les dépendances liées à l’aspect multi-composant du système, qui tient compte des modes de défaillance spécifiques des systèmes de protection ;

- L’aspect économique lié à la stratégie de maintenance (inspection, entretien, réparation, remplacement), aux coupures (programmées et forcées) et aux événements à risque (refus disjoncteur, perte d’un client, perte d’un jeu de barres, perte d’une sous-station, etc.).

Abstract:
Reliability Centred Maintenance (RCM) started a new chapter in the history of preventive maintenance strategy setting. It was now possible to develop a scientifically based, highly successful maintenance program for complex systems. It developed as a result of the reliability problems and cost of maintenance of aircraft during the late 50’s and early 60’s. The result was a methodology called MSG-1, followed by the improved MSG-2. When MSG-2 was used contractually for the United States Department of Defence, it led to the present definition of RCM. In academic circles there developed a growing dissatisfaction with the technique [Pintelon et al (1999], of which part stems from watering down its scientific basis to make RCM more marketable [Moubray (2000)], while at least part is based on perceived inherent scientific weaknesses in the methodology itself. This thesis, in setting out to solve these limitations, makes several important contributions to the RCM methodology. The first of these is a method of concentrating the RCM analysis effort on the most important failure modes encountered by the organisation. Secondly, it introduces a Quality Improvement task in the RCM task selection tree, based on a limitation identified by Harris (1985). The third contribution is the addition of a formal task packaging methodology, following Gits (1984). The thesisalso combines the use of RCM for the most important failure modes with conventional maintenance tasks for the remaining failure modes, to form a total methodology for the typical industrial concern. It furthermore introduces the application of sound management principles in the implementation of RCM and lastly, blends concepts from different RCM authors, together with the innovations listed above, into one logical whole. In summary, the proposed revised methodology can play a very important part to achieve the goal of World Class manufacturing standards, including ensuring that the organisation’s maintenance effort is as proactive as possible. Statistical methods are nowadays being increasingly useful in industrial engineering. From plant design reliability to equipment analysis, there is much to cover with statistical models in order to improve efficiency of the systems. A real challenge is put forward to maintenance managers due to high requirements demand in this competitive world where changes come fast and a good support in decision is then essential, which leads us to the Reliability Centered Management (RCM) philosophy, that guides institutions in a reliability sense of option. The present work refers to a PhD made within a business context, namely, industrial context, aiming to be practical and fulfil the needs of Sines’ refinery concerning RCM requirements. A non-controlled environment was at stake, once real data was collected and no possibility to follow and focus on a certain subject was possible, against traditionally theoretical models with produced data usually used for this kind of studies. For this purpose, a collection of real failure data for rotating equipment served as support to statistical analysis using survival statistical models, which are most of times used in medicine or biology, but are being increasingly used in engineering. Survival models with random components as well as mixtures of two distributions were used and a collection of reliability data was constructed for rotating equipment. In this kind of industry, impact of failure rates is financially measured, especially insurance related, as a higher reliability of systems usually implies lower insurance rates. Our goal was to contribute to a reliability databook compilation so they could be able to benchmark with standards of the same industry type, as OREDA (2009), which results have been effective. The main innovative aspect was concerned to the root cause analysis for Turbo-Expander (TE) turbine to have shutdowns due to vibration. Beyond its large economic impact, this equipment is quite rare within its configuration and that is the reason why it is very special. Despite there already exist some studies about TE vibration causes, chemical and mechanical related, we’ve tried to understand TE failure problem using parametric survival models, using physical and mechanical variables involved in the process. Even though no absolute conclusion was found, results match and reinforce previous studies on the subject and highlight that composition of the feedstock is highly related with TE vibration; RESUMO: Modelos de risco aplicados a equipamento industrial utilizando distribuições de fiabilidade e implementação de RCM Os métodos estatísticos são cada vez mais úteis na engenharia industrial. Um verdadeiro desafio é então colocado à gestão de manutenção devido aos elevados requisitos necessários e normas a cumprir, neste mundo competitivo onde as mundanças aconteçem rápido e um bom suporte à decisão é absolutamente necessário. Isto leva-nos à filosofia do RCM (Reliability Centered Management), a qual guia as empresas no sentido da gestão pela fiabilidade. O presente trabalho refere-se a um doutoramento realizado em contexto empresarial/industrial com um sentido prático e de resposta a questões colocadas pela refinaria de Sines no que respeita à gestão pela fiabilidade. Em jogo estava um ambiente não controlado e dados reais ao contrário do que é habitual num projecto desta natureza, com dados simulados. Uma primeira parte deste estudo foi realizada com base em dados de equipamento rotativo da refinaria de Sines e serviu de base ao nosso estudo estatístico que se centrou nos modelos de sobrevivência, muitas vezes utilizados em medicina ou biologia, e que são cada vez mais utilizados em engenharia. Modelos de sobrevivência com componentes aleatórias ou modelos com mistura de duas distribuições, são alguns exemplos dos modelos utilizados. Contribuir para a construção de uma compilação de dados de fiabilidade era o nosso objectivo, de forma a que a refinaria pudesse ter a possibilidade de realizar benchmarking com indústrias do mesmo tipo, como é o caso do OREDA (2009). O principal aspecto inovador deste trabalho está relacionado um equipamento raro na sua configuração, o Turbo-Expander, cujas vibrações elevadas levam a paragens intempestivas. O elevado impacto económico deste problema é motivo para que vários artigos sobre o tema já tenham sido escritos, quer relacionados com váriáveis físicas como químicas ou processuais. Apesar de não existir um absoluto resultado concreto, o nosso trabalho permitiu reforçar os estudos já existentes sobre o problema e realçar a importância da composição da matéria-prima, com forte probabilidade de produzir componentes capazes de levar a elevadas vibrações.

It is the author's experience that too many times operators have attempted to carry out the function of maintenance engineering with out full understanding of the reasons and implication of their actions, thus ending up with an ineffective and non applicable collection of maintenance activities that can only add to the expense of the technical operation and even at times contribute to the possibility of occurrences. Esta investigación desarrolla un modelo de gestión para el área de mantenimiento de una empresa de panadería. Las técnicas utilizadas son los 3 pilares del Total Productive Maintenance (TPM), mantenimiento autónomo, mantenimiento planificado y mejora continua, integrado con la metodología Reliability Centered Maintenance (RCM) para proponer un plan de mantenimiento preventivo. El objetivo es aumentar la disponibilidad de una línea de producción y reducir los costos de mantenimiento. El caso de estudio analiza principalmente una línea de panadería cuyos equipos tiene una alta tasa de paradas. Para validar los efectos del modelo se desarrolla un sistema de simulación de la línea de producción que incluye el porcentaje de producción perdida por fallas y los tiempos de parada de la máquina. Los resultados mostraron un aumento en la disponibilidad de línea en un 9,72% y un ahorro del 35% en los costos anuales de mantenimiento.
This research develops a management model for the maintenance area of ​​a bakery company. The techniques used are the 3 pillars of Total Productive Maintenance (TPM); autonomous maintenance, planned maintenance and continuous improvement, integrated with the Reliability Centered Maintenance (RCM) methodology to propose a preventive maintenance plan. The goal is to increase the availability of a production line and reduce maintenance costs. The case study mainly analyzes a bakery line whose equipment has a high stop rate. To validate the effects of the model, a simulation system of the production line is developed that includes the percentage of production lost due to failures and machine downtime. The results showed an increase in line availability by 9.72% and a saving of 35% in annual maintenance costs.

El sector textil y confecciones es uno de los sectores con mayores expectativas de crecimiento sostenible de la industria y uno de los que mayor competencia acumula con otros países del sector internacional. Este sector abarca diferentes tipos de procesos, que van desde tratamiento de fibras, hilatura, tejido y confección. El presente análisis recae sobre la baja disponibilidad que presenta los equipos en el proceso de teñido. Este proceso se mide a partir de los reprocesos que existen debido a que se ejecuta mantenimientos correctivos, el cual afecta la producción en el teñido de telas. Asimismo, se logra evaluar el tiempo promedio entre fallas (MTBF) y el tiempo promedio de reparación (MTTR), lo que indica la variabilidad por debajo del objetivo esperado por la organización. Por otro lado, en el tercer capítulo se desarrolla le propuesta de mejora a partir del desarrollo del estado del arte del primer capítulo. Para ello se propone la metodología da seguir de acuerdo con una combinación de RCM y TPM. Bajo esta metodología se desarrolla diferentes actividades y estrategias que permitirán detectar equipos críticos e implementar planes de mantenimiento preventivo mecánico y eléctrico. Además, se incluye como parte de este sistema la implementación de un mantenimiento basado en condición (predictivo) y autónomo. Finalmente, el tercer capítulo finaliza con el soporte de la metodología 5 ‘’S’’ y la evaluación económica a partir de la estructura de costos que tiene prevista el proyecto. Finalmente, en el capítulo cuatro se presenta la validación de la propuesta de mejora a partir de la validación de los resultados en el software Arena. Ello permite concluir que los indicadores MTTR y MTBF ha logrado alcanzar los objetivos presentados en la propuesta.
The textile and garment sector is one of the sectors with the highest expectations of sustainable growth in the industry and one of the sectors with the highest competition with other countries in the international sector. This sector covers different types of processes, ranging from fiber treatment, spinning, weaving and confection. The present analysis falls on the low availability that presents the equipment in the process of dyeing. This process is measured from the reprocesses that exist due to the execution of corrective maintenance, which affects the production of fabric dyeing. Likewise, it is possible to evaluate the average time between failures (MTBF) and the average repair time (MTTR), which indicates the variability below the target expected by the organization. On the other hand, in the third chapter the improvement proposal is developed from the development of the state of art of the first chapter. For this, the methodology to be followed is proposed according to a combination of RCM and TPM. Under this methodology, different activities and strategies are developed to detect critical equipment and implement mechanical and electrical preventive maintenance plans. In addition, the implementation of a maintenance based on (predictive) and autonomous condition is included as part of this system. Finally, the third chapter ends with the support of the 5 '' S '' methodology and the economic evaluation based on the cost structure planned by the project. Finally, chapter four presents the validation of the improvement proposal based on the validation of the results in the Arena software. This allows concluding that the MTTR and MTBF indicators have achieved the objectives presented in the proposal.

I denna fallstudie har SeventyOne Consulting analyserats som ett exempel på en organisation som verkar under det kommande paradigmet inom kvalitetsutvecklingen. Studien kopplar samman teori från kvalitetfältet med design thinking, paradigmteori och teori om människocentrerad företagskultur. Syftet var att bidra med kunskap gällande vilken roll design thinking och människocentrerad företagskultur kan spela i ett kommande paradigm inom kvalitetsutveckling. Frågeställningarna var: 1. Hur kan det kommande paradigmet i kvalitetsutveckling förstås? 2. Vilken roll kan design thinking och en människocentrerad företagskultur spela i det kommande paradigmet? Metoden inkluderade semi-deltagande observationer av organisationen, icke-deltagande observationer av arbetet med kunder, intervjuer med företagets medlemmar samt dokumentationsanalys. Studien har huvudsakligen genomförts online. Dess resultat har organiserats i den metaforiska och hypotetiska analysmodellen bröllopstårtan, vilken illustrerar hur en människocentrerad företagskultur baserad på psykologisk trygghet, Teal-principer och glädje utgör grunden för att hantera kunders komplexa problem utifrån de metodologiska strategierna relatera och samskapa, eklektisk metod och ett design thinking mindset. Design kapabilitet har analyserats som förmågan att sammankoppla och arbeta utifrån flera olika kunskapstraditioner samtidigt, samt integrera företagskulturer utifrån en människocentrerad bas. Människocentrerad kultur har därmed förståtts som en förutsättning för att kunna möta komplexiteten och innovationskraven som präglar det nya paradigmet, medan design thinking förståtts som en potentiellt användbar metod, förutsatt att design kapabilitet utvecklats.
In this case study, SeventyOne Consulting was analysed as an example of an organisation operating under the coming paradigm in quality management. The study connected theory from quality management, design thinking, paradigm theory and human-centred culture theory. The purpose was to contribute with knowledge concerning what role design thinking and a human-centred culture can play in the coming paradigm of quality management. The research questions were: 1. How can the coming paradigm of Quality Management be understood? 2. Which roles can design thinking and a human-centred culture play in the coming paradigm? The method included semi-participatory observations of the organisation, non-participatory observations with its customers, interviews with its members and document analysis. The study has mainly been performed online. Its result were organised into the metaphorical and hypothetical analytic model of the wedding cake, illustrating how a human-centred culture based on psychological safety, Teal principles and happiness gives the foundation for handling customers’ complex problems through the methodological strategies relate and co-create, eclectic methodology and a design thinking mindset. Design capability was analysed as the ability to connect and work simultaneously with different kinds of knowledge and integrating cultures, while also coming from a human-centredness. Human-centred cultures were thereby understood as an important prerequisite for being able to meet the complexity and innovative demands of a new paradigm, while design thinking was understood as a potentially suitable method, provided that design-capability has been developed.

In accordance with the Taiwan Society of Nephrology,hemodialysis patients keeps increasing from 1997 to 2004 in Taiwan,requests issue of hemodialysis patients approximately 52,000 people to 95 year in March . The outpatients of dialysis to expend the health insurance is astonishing,however hypertension, cardiopulmonary, diabetes and other chronic diseases in hemodialysis patients is very common. Therefore, the safety of the hemodialysis process is the first concern to hemodialysis patients. In addition, national health insurance payment is decreasing; effectively lower costs of maintenance will help hospital best less cost. RCM(Reliability Centered Maintenance) is a maintenance approach which can both manage systematic risk and lower the cost of maintenance. From many experiences of factories applying RCM, Rausand【1998】 finds that RCM can effectively lower PM costs and also increase the systematic usefulness. The purpose of this study is to investigate Reliability-Centered Maintenance (RCM) Approach. Through collecting the hemodialysis machine maintenance data of one regional hospital and using the Fault Tree Analysis (FTA) and Failure Mode Effect Analysis (FMEA), it is to find significant failure modes by focusing on sifting the criticality of important equipment. By means of Genetic Algorithm(GA) to develop the best interval time of preventive maintenance, it is to further provide the preventive maintenance strategy from the RCM point of view to the related decision making units.

Maintenance determines not only the safety performance but also the operation Cost for railway transportation. Traditionally, the facility maintenance has been considered for preventive purpose for the railway industry in Taiwan. Preventive maintenance is thought to be an ineffective strategy in the viewpoint of resource management. This research tries to study the potential of applying the concept of Reliability-Centered Maintenance (RCM) to maintain the facilities of railway system. For empirical study purpose, Ridit analysis, the fault tree analysis (FTA) and the failure modes, effects and criticality analysis (FMECA) are conducted to find out the maintenance significant item (MSI) of the Electric Multiple Unit (EMU) system of Taipei Rapid Transit Company. It is expected to help determining the maintenance priority among items through the criticality matrix. Finally, the Petri Net simulation is conducted to develop appropriate maintenance strategy of EMU system. The results of this study will provide valuable reference for the maintenance management of railway system in Taiwan. Our study shows that there are five MSIs with higher priority in EMU system:Brake Caliper, Brake Activator, Air Supply Equipment, Conical Rubber Axle Spring, Anchorage (This MSI doesn’t conduct simulation). The simulation result of the maintenance strategy shows that the replace interval of these MSIs should increase; the checking interval of Brake Activator should increase but that of the others should decrease. Finally, this study points out that RCM is applicable in analyzing the railway system safety and it can lower the risk effectively to satisfy the safety requirement of railway system. This will help to upgrade the maintenance concept of railway system in the future.

O mercado de energia elétrica em Portugal evoluiu de totalmente regulado para liberalizado, passando por várias etapas. Atualmente com o MIBEL, a produção de energia elétrica caracteriza-se por um ambiente competitivo com diversos fatores como a entrada das renováveis, as condições hidrológicas, a descarbonização do parque electroprodutor, com o respetivo contrabalanço da variabilidade do consumo assente na base económica com as conhecidas flutuações.Apresentando estes pressupostos do mercado, uma central de ciclo combinado a gás natural, que inicialmente, em projeto, incorporava a base do diagrama, procura agora flexibilizar-se e entrar nas oportunidades de mercado, por forma a maximizar o retorno do investimento. Um pilar extremamente importante nesta posição perante o mercado é a função manutenção e a sua estratégia.A manutenção procura exponenciar a disponibilidade dos equipamentos com elevados padrões de fiabilidade. Para conseguir este objetivo, a estratégia da manutenção passa por utilizar ferramentas que permitam a otimização dos seus planos de manutenção, como o Plano de manutenção Preventiva. A metodologia Reability Centered in Maintenance produz resultados no sentido da gestão da manutenção, do equipamento, com base na sua criticidade no processo e custos (de corretiva e de preventiva).Enquadrando a metodologia RCM e a voracidade e velocidade de resultados em mercado, as empresas do sector elétrico, optam pela aplicação do SRCM Streamlined Reability Centered in Maintenance, que produz resultados mais rápidos, e comprovados pela EPRI, fiáveis como o RCM. O objetivo desta dissertação é expor a aplicação do SRCM a um sistema da central de ciclo combinado – sistema elétrico de muita alta tensão (subestação). The electricity market in Portugal has developed from fully regulated to liberalized, going through several stages. Currently with MIBEL, the production of electric energy is characterized by a competitive environment with several factors such as the entry of renewables, hydrological conditions, decarbonization of the generation portfolio, with the respective other side of the coin with the variability of the demand, based on the variations of the economic.With these market assumptions, a natural gas combined cycle plant, which initially design to be working in the basis of the load profile, now seeks to become more flexible and to be ready for market opportunities, in order to maximize the return on investment. An extremely important support of this position is the maintenance function and its strategy.Maintenance seeks to expose the availability of equipment with high standards of reliability. To achieve this goal, the maintenance strategy is to use tools/methods that allow the optimization of the maintenance plans, such as the Preventive Maintenance Plan. The Reliability Centered in Maintenance is one of the methodology that produces results in the management of maintenance, based on equipment criticality in the process and costs (corrective and preventive maintenance).Because of the voracity and speed of results in the market, companies in the electricity sector had made the option for implement SRCM Streamlined Reliability Centered in Maintenance, which produces faster results and proven by EPRI, with the same quality of the RCM. The objective of this thesis is to revel the implementation of the SRCM to a system of combined cycle power plant – substation system.The present work was developed under the scope of the curricular unit Masters’ thesis in Electrical and Computer Engineering, taught in the Department of Electrical and Computer Engineering of the Faculty of Science and Technology, Coimbra University, in the school year 2017/2018.

The reliability for the power system of rolling stocks is determined by the effectiveness and efficiency of its maintenance strategies. Recently, the increasing failures of the Electric Multiple Unit (EMU) power subsystem of Taiwan Railway Administration (TRA) have reduced the reliability of their service as well as the confidence of passengers. This study aims to analyze the potential of applying Reliability-Centered Maintenance (RCM) approach to inspect the system reliability of TRA rolling stocks. RCM is a systematic approach to develop a focused and cost-effective maintenance plan. Based on the quantitative reliability measure, RCM can help to explore the maintenance significant items (MSI) and distribute the maintenance resource efficiently. By applying RCM, both the system safety and expense saving can be achieved through such efficient maintenance procedure. This study shows that Pantograph is the first-priority MSI in the power subsystem. Applying the “Replacement at Predetermined Age” maintenance policy, the replacement interval with the minimum cost can be calculated. Furthermore, the minimum cost replacement interval executed in intermediate level (I level) is considered better cost-effective than that in depot level (D level) via the analysis of repair level. Results of this study point out that RCM is applicable in analyzing the railway rolling stock maintenance planning and provide valuable reference for the maintenance management of railway system in Taiwan.

A presente dissertação foi realizada no âmbito da conclusão do Mestrado Integrado em Engenharia e Gestão Industrial pela Universidade do Minho e teve como principal intuito apresentar uma metodologia de planeamento da manutenção construída com base na metodologia RCM. O caso de estudo apresentado dirige-se à empresa Bosch Car Multimedia Portugal, na qual foi realizado o estágio curricular no departamento de manutenção da produção. O departamento rege-se pela filosofia de gestão da manutenção TPM. A referida metodologia inicia-se pela aplicação do método AHP, por forma a priorizar os equipamentos mais críticos do ponto de vista de três critérios: MTBF, custo de spare parts e tempo de manutenção preventiva. De seguida recorre-se ao diagrama funcional da máquina para percecionar os subsistemas e componentes que a constituem. O terceiro passo consiste no estudo das funções de cada subsistema e das suas falhas funcionais. Para cada falha funcional relevante analisam-se de seguida os modos de falha através da FMEA, que permite realçar quais os mais críticos que devem ser combatidos. Uma árvore de decisão com três questões permite perceber quais as ações de manutenção que podem ser alocadas a cada modo de falha. Para decidir de entre as várias opções, efetua-se uma avaliação económica das propostas. Neste seguimento, propõe-se ainda um estudo de fiabilidade baseado na distribuição de Weibull e no gráfico de Glasser, cujo fim é definir a periodicidade ótima de substituição preventiva. Por fim, a metodologia, que pretende ser aplicável a qualquer equipamento mecânico e eletromecânico, é validada com a aplicação num equipamento da fábrica. Através da aplicação desta metodologia a um equipamento obtiveram-se os seguintes resultados: redução do número de avarias em 33%, redução do número de perdas de produção em 33%, melhoria do MTBF em 15% e um incremento no rendimento da máquina em 1,8%.
This dissertation was done within the scope of conclusion of MSc degree in Industrial Engineering and Management at University of Minho and the main goal was to present a methodology of maintenance planning developed based on RCM methodology. This case study is related to Bosch Car Multimedia Portugal company, in which a curricular internship was done in the production maintenance department. The department follows the TPM maintenance management philosophy. The mentioned methodology begins with the application of AHP method to prioritize the critical machines using three criteria: MTBF, spare parts cost and preventive maintenance time. Then, the functional structure of the machine is used to see the subsystems and parts of it. The third step consists in studying the functions of each system and their functional failures. To each functional failure, failure modes by the FMEA is analysed, which allow to highlight the more critical failure modes that should be mitigated. A decision tree with three questions allows identifying which maintenance actions can be allocated to each failure mode. To decide between various actions, it is made an economic evaluation of the proposals. In this scope, a reliability study is proposed based on Weibull distribution and Glasser method, whose purpose is to define the optimal preventive replacement periodicity. Lastly, the methodology, which pretends to be applicable in any mechanical or electromechanically equipment, is validated in one machine of the plant. By using this methodology in an equipment, the following results were obtained: reduction of failures number in 33%, reduction of production losses in 33%, MTBF improvement in 15% and increase machine yield in 1,8%.

O objectivo da presente dissertação é de criar a base teórica para a implementação de uma metodologia de gestão da manutenção de dispositivos médicos em estabelecimentos de prestação de cuidados de saúde, com ênfase na redução de custos e melhoria da qualidade do serviço. Esta metodologia é baseada na utilização do processo RCM e da distribuição de probabilidade de falhas Weibull. É apresentado um caso de estudo como exemplo de aplicação do método proposto.
It is generally accepted among health care establishments that the cost of medical equipment maintenance is too high. There is also a lack of strict procedures for the accreditation of calibration and proper performance of the measuring instruments, thus compromising the safety of healthcare professionals and even patients. The maintenance management of medical devices presents itself as an essential tool to ensure high levels of reliability, availability, security, and technical and economic efficiency. The purpose of this dissertation is to establish the theoretical basis for the implementation of a methodology for managing the maintenance of medical devices in health care establishments, with emphasis on reducing costs and improving service quality. This methodology is based on the use of the RCM process and the Weibull probability distribution function. A case study is presented to exemplify the proposed method.

A research report submitted to the Faculty of Engineering and the Built Environment, University of the Witwatersrand, in partial fulfillment of the requirements for the degree of Master of Science in Engineering in Mechanical Engineering by course work and research report. 25 October 2016
The objective of this report is to compare the existing maintenance methods in Eskom to RCM and then test the applicability of RCM in improving boiler reliability. Firstly, a comparative study was conducted on different RCM methods and the RCM method to apply selected and compared to the Eskom’s initiatives. The RCM method is piloted to a sample system and the results are compared with those from the current Eskom’s initiatives. The biggest change due to the RCM analysis was the way to document the results of the RCM process as compared to the current practice. It was also found that intervals between maintenance tasks proposed by the RCM study are different from the intervals currently used. In conclusion, the report recommends that the RCM results be used as a guide for continuous improvement in order to fill in gaps that are crucial in determining reliability goals.

The implementation of a maintenance strategy without taking the uniqueness of the specific business into account, will not necessarily result in a high level of performance. The maintenance function should be developed specifically to support the particular business processes and technology in use. There is presently a large gap between the results obtained through the implementation of the maintenance strategy at a major coal-mining complex and the desired targets to be met. To close the gap, this dissertation suggests that support systems should be implemented to sustain growth and stability. The dissertation specifically proposes the creation of a Central Support Function to support the maintenance strategy, which has been implemented together with the Enterprise Management System (EMS) with the System Application Product (SAP) as an enabler.
