Resumo: As características físicas, químicas e morfológicas da dentina remanescente foram analisadas após a remoção químico-mecânica e mecânica do tecido cariado. Molares decíduos cariados extraídos foram seccionados no centro da lesão de cárie, no sentido mésio-distal, em duas metades semelhantes e os espécimes gerados foram divididos em 3 grupos de acordo com o método utilizado para remoção do tecido cariado: grupo CS (Carisolv™), grupo PC (Papacárie®) e grupo BR (Broca). Foram realizadas as análises em microscopia confocal (MC), microscopia óptica de luz (MO), microscopia eletrônica de varredura (MEV), espectrometria de energia dispersiva (EDS) e microdureza (MD). Na análise da MC observou-se que o método químico-mecânico removeu menos tecido cariado e a análise histológica evidenciou maior preservação da estrutura dentinária. No MEV/EDS, após a remoção químico-mecânica do tecido cariado, a dentina remanescente apresentou reduzido conteúdo de cálcio, porém a superfície dentinária apresentava-se mais irregular e livre de lama dentinária. A microdureza da dentina após a remoção do tecido cariado foi semelhante, independente do método utilizado. Os resultados deste estudo mostraram que o método químico-mecânico é mais conservador na remoção do tecido cariado
Abstract: The physical, chemical and morphological characteristics of the remaining dentine were analysed after chemomechanical and mechanical caries removal. Extracted carious deciduous molars were sectioned mesiodistally through the center of the carious lesion into two similar halves. The specimens were divided into three groups according to the method used for caries removal: CS group (Carisolv™), PC group (Papacárie®) and CB group (conventional bur). The specimes were analysed under confocal laser scanning microscopy (CLSM), light microscopy (LM), scanning electron microscopy (SEM), energy dispersive analysis of x-rays (EDS) and microhardness (MH). After chemomechanical caries removal, under CLSM the remaining dentine showed less carious tissue and more dentine structure preserved in the histological analysis. SEM/EDS analysis showed the remaining dentine after chemomechanical carious removal presented reduced calcium content with irregular surface and smear layer free. Regardless the method used for caries removal the microhardness of the dentine was similar. The results showed that chemomechanical method was more conservative in the removal of carious tissue

Full textThe purpose of this in vitro study was to analyze the residual dentinal surfaces following caries removal using rotatory instruments and two chemomechanical methods (Papacárie® and Carisolv®), by laser fluorescence (DIAGNOdent), microhardness test and scanning electron microscopy (SEM). Thirty primary incisives, from the Human Bank Teeth of FOUSP, with carious cavities on one of the proximal surfaces were evaluated, before and after caries removal, by laser fluorescence, and then, prepared for the microhardness test. Microhardness was measured in different distances bellow the carious cavity side (50, 100, 150, 200, 300, 400 e 500µm) and on sound dentine of the other proximal side (100, 150, 200, 300, 400, 500, 1000 e 1500 µm). Next, other 30 primary incisives were divided in three groups, according to the caries removal method used, and their residual dentine was examined under SEM (15). After caries removal, 15 of these teeth were restored with Single Bond (3M) adhesive system and Z100 Filtek composite resin (3M). The tags of the replicas were observed under SEM. In conclusion, the laser fluorescence measurements observed before and after caries removal showed statistically significant difference in all groups; however, there was no difference among the caries removal methods used. The microhardness values of the residual dentinal surfaces were significantly lower than the ones found on sound dentine. No difference was found among the three caries removal methods regarding the microhardness values of residual dentine. Under SEM, the chemomechanical caries removal methods (Papacárie® and Carisolv®) formed an amorphous layer, similar to the smear layer and few exposed dentinal tubules; the conventional caries removal method produced a smooth and regular dentinal surface, with typical smear layer and exposed dentinal tubules. Full textThe minimally invasive treatment has been increasingly used in the treatment of dental caries, particularly in young children. Thus, the removal of carious tissue by chemical-mechanical method allows greater conservation of healthy tooth structure. The Papacárie Duo® is an easily applied material, and has bactericidal and anti-inflammatory properties. This material is applied over the carious dentin in order to promote a softening thereof, facilitating its removal. The cellular effects of Papacárie Duo® are little known, therefore this research was to evaluate the effect of Papacárie Duo® in undifferentiated cells from dental pulp (Chapter 1) and the capacity of Papacárie Duo® to induce macrophage activation and synthesis inflammatory mediators (Chapter 2). The Papacárie Duo® was prepared at concentrations of 0.5 and 5% by serial dilution from the gel obtained commercially. OD-21 cells and J774.1 macrophages were maintained in culture with the different treatments for a period of 24 hours to perform the cytotoxicity assay (LDH assay) and for 36 hours for evaluation of cell proliferation (MTT colorimetric assay). The following, was carried out the assessment of the relative gene expression of Ibsp, Runx2 and Spp1 genes in OD-21 cells, and Il10, Mmp9, Tnf and Ptgs2 in J774.1 cells by reverse transcription method and reaction in real time polymerase chain reaction (qRT-PCR) using the TaqMan system after the stimulation period of 24 hours. The 5% Papacárie Duo® was cytotoxic to cells of dental pulp and inhibited cell proliferation and the expression of Runx2 and Ibsp. However, in both concentrations stimulated the expression of Spp1, which was higher at a concentration of 5%. In macrophages, the Papacárie Duo® was cytotoxic at concentrations of 5%, but did not affect the cell proliferation in any of the concentrations (0.5 and 5%). LPS inhibited cell proliferation in the presence or absence of Papacárie Duo® without giving cytotoxicity. The Papacárie Duo® induced the expression of Ptgs2 and Il10, without changing Tnf and Mmp9. Therefore, the Papacárie Duo® was cytotoxic, depending on the concentration, and showed inhibitory effect on the differentiation of pulp cells (OD-21), but without influencing cell proliferation. Papacárie Duo® not prevent macrophage proliferation, but was cytotoxic at a concentration of 5% but not more than 0.5%. Additionally, Papacárie Duo® modulated the activation of macrophages by inducing Il10 and Ptgs2 expression without altering the expression of ,Tnf, and Mmp9.
