Abstract:
Objective: This study was a retrospective cross-sectional examination. Methods: The study enrolled 301 patients in 2018 who were referred to dialysis clinic. Data is collected through a series of questions about demographics (gender, age, education, employment, housing, marital status) and a checklist for causes of chronic renal failure (hypertension, diabetes, kidney disease, etc.). The Chi-square test, Fisher's exact test, analysis of variance (ANOVA), and SPSS version 24 were utilized to carry out inferential statistics. Results: Patients on hemodialysis typically develop chronic renal failure due to high blood pressure, although kidney disease and diabetes also play a role. Among the many potential factors that led to ESRD in these patients, the most common were: other/unspecified causes (15.3%), surgical history (6.3%), anemia (5%), trauma (4.3%), shock (7.7%) (2%), viral diseases (2%), drug use and congenital causes (1.3%), and poisoning during pregnancy and lupus (0.3%). Conclusions: According to the results of this study, promoting screening procedures for patients who are at risk may help to lower the prevalence of end-stage renal failure.

Condition being studied: In clinical field, aggressive fluid resuscitation therapy is administered to prevent the progression of multiple organ failures by maintaining tissue and organ perfusion. Normal saline is frequently used, but it has been some concerns. Although large-scale studies with balanced crystalloids have been conducted, they couldn’t reach significant conclusions due to the diversity of disease severity. Therefore, we aims to evaluate and identify the best fluid for patients at high risk of multiple organ failure by comparing the effects of normal saline and balanced crystalloids.

Review question / Objective: To systematically evaluate the efficacy and safety of cordyceps preparations as a complementary preventive therapy for Contrast-induced nephropathy (CIN). Condition being studied: At present, contrast agents are widely used in diagnostic and interventional radiology examinations worldwide. However, they can affect kidney function and cause a risk of renal impairment. Contrast-induced nephropathy (CIN) is defined as a rise in serum creatinine (SCr) levels by ≥ 25% of baseline or 44 µmol/l from the pre-contrast value within 72 h of intravascular administration of a contrast agent in the absence of an alternative etiology. The incidence of CIN varies widely among studies depending on study population and baseline risk factors, as for high-risk groups such as pre-existing renal insufficiency, diabetes, advanced age, or receiving nephrotoxic agents, the incidence is up to 30–50%. To date, CIN has been the third most common cause of hospital-acquired renal failure, after impaired renal perfusion and nephrotoxic medications, which can lead to longer hospital stay, increased costs and higher mortality.

Review question / Objective: 1) In humans, how does environmental and/or occupational exposure to uranium affect the risk of mortality due to primary kidney disease compared to unexposed individuals? (2) In humans, how does environmental and/or occupational exposure to uranium affect the risk of developing kidney failure compared to unexposed individuals? Eligibility criteria: We included cohort studies that evaluate the risk of CKD/ESKD due to uranium exposure. We also included cohort studies that evaluate standardized mortality due to all-cause mortality, kidney cancer, chronic kidney disease, diabetes, and cardiovascular disease in humans with exposure to uranium. We also included cross sectional studies that evaluate renal function in humans exposed to uranium via biomarkers and hard clinical measures (such as creatinine clearance) compared to humans with low/no uranium exposure. In order to not include the same cohort multiple times in the statistical analyses, we selected studies that evaluated an outcome of interest for a given cohort for the longest follow-up period. When this was not possible (due to multiple studies using different combinations of cohorts with varying lengths of follow up), the study with the largest study population size was selected.
