Background Systematic, large-scale RNA interference (RNAi) approaches are very valuable to systematically investigate biological processes in cell culture or in tissues of organisms such as Drosophila. A notorious pitfall of all RNAi technologies are potential false positives caused by unspecific knock-down of genes other than the intended target gene. The ultimate proof for RNAi specificity is a rescue by a construct immune to RNAi, typically originating from a related species. Methodology/Principal Findings We show that primary sequence divergence in areas targeted by Drosophila melanogaster RNAi hairpins in five non-melanogaster species is sufficient to identify orthologs for 81% of the genes that are predicted to be RNAi refractory. We use clones from a genomic fosmid library of Drosophila pseudoobscura to demonstrate the rescue of RNAi phenotypes in Drosophila melanogaster muscles. Four out of five fosmid clones we tested harbour cross-species functionality for the gene assayed, and three out of the four rescue a RNAi phenotype in Drosophila melanogaster. Conclusions/Significance The Drosophila pseudoobscura fosmid library is designed for seamless cross-species transgenesis and can be readily used to demonstrate specificity of RNAi phenotypes in a systematic manner.

Abstract:
Aim: The aim of this thesis was to investigate experiences of receiving notification of an abnormal Pap smear result and its impact on women’s health-related quality of life as well as to investigate women’s awareness of human papillomavirus.   Methods: In total, 176 women and 20 healthcare professionals participated. Data were collected through individual interviews (I, II) and a questionnaire (IV) including the instrument Functional Assessment of Chronic Illness Therapy – Cervical Dysplasia (FACIT-CD) and Hospital Anxiety and Depression Scale (HADS). For the translation and cross-cultural adaptation of the FACIT-CD, and for evaluation of its linguistic validity and reliability, cognitive debriefing interviews and a questionnaire consisting of the Swedish instrument FACIT-CD was used. Data were analysed by content analysis (I, II) and by using statistical analysis (III, IV), while one part (III) was analysed according to FACIT translation methodology.     Results: Women receiving notification of an abnormal Pap smear result have good overall HRQoL; they become anxious but not depressed. Reasons for anxiety were mainly that women misinterpreted the result as cancer, which could lead to lower attendance for further investigation, treatment and follow-up. To cope with the anxiety, women sought emotional support and information. They primarily used the Internet for information but also turned to healthcare professionals for information needs. Moreover, women had low awareness of HPV, its sexually transmitted nature, and its relationship to abnormal Pap smear results and cervical cancer. An awareness of HPV as a sexually transmitted infection did not lead to higher level of anxiety or more depression symptoms or worse HRQoL, compared to not being aware. Finally, the Swedish FACIT-CD is equivalent to the English version and linguistically valid and exhibited good internal consistency reliability.   Conclusion: Women have low awareness of HPV and abnormal Pap smear results, whereupon they misinterpret their test result as cancer. It is of importance that women understand their test result, in order to minimise anxiety as well as to maintain high attendance for investigation, treatment and follow-up of abnormalities.

Au cours de ces dernières décennies, des progrès considérables ont été réalisés dans le domaine de l'oncogénétique, activité médicale basée sur les avancées scientifiques de la biologie moléculaire permettant de prédire le risque de survenue d'un cancer. Néanmoins, dans 80% des cas, les analyses génétiques initiales, réalisées auprès de patientes atteintes d'un cancer du sein et/ou de l'ovaire, ne permettent pas d'identifier une mutation délétère sur l'un des deux gènes BRCA. Dans ce cas, le résultat est dit « négatif », alors qu'il existe toujours un facteur génétique résiduel et le risque de développer un autre cancer justifiant la poursuite de la surveillance personnelle et familiale. En collaboration avec des médecins oncogénéticiens de la région Rhône-Alpes, nous avons interrogé les modalités de transmission de l'information dans ce contexte, en évaluant d'une part l'apport d'un document écrit remis aux patientes à l'issue de la consultation et d'autre part la nécessité du cadre de la consultation. Nous avons également tenté de mieux discerner la manière dont les informations orales sont perçues par les patientes. Pour ce faire, nous avons interviewé 88 patientes au cours d'un entretien clinique de recherche et 172 patientes ont répondu à un questionnaire. Les résultats obtenus ont montré : - l'utilité de délivrer un document écrit résumant les informations orales transmises par le médecin ; - le cadre indispensable de la consultation pour délivrer le résultat négatif ; - l'existence d'un message paradoxal, dans la mesure où le résultat « non concluant » est annoncé comme négatif, rendant la compréhension de l'information plus difficile<br>Over the last decades, major progresses have been realized in oncogenetics. This medical activity based on scientific advances in molecular genetics predicts cancer risk. However, no deleterious mutation is detected by BRCA1/2 genetic testing for the majority (80 %) of women who suffer from either breast or ovarian cancer. Under this condition, the result is considered as “inconclusive” although there remains an increased risk for developing breast and/or ovarian cancer. The failure to identify a mutation in a diagnostic test does not rule out the existence of a predisposing factor. Surveillance should be pursued for both the patients and their families. In association with genetic counselors of the region Rhône-Alpes, we considered useful to address the question of information delivery in the context of an inconclusive result. We investigated on one hand the benefit from a written document delivered to patients upon the appointment, and on the other hand the usefulness of a disclosure session. We also investigated how patients experienced the oral information given during the counsel session. Therefore, we conducted semi-structured interviews with 88 women while 172 women answered a questionnaire. The observations demonstrate : - a written document, summarizing the oral information delivered by the oncologist, is useful for patients with an inconclusive result; - a disclosure session is absolutely necessary to announce the inconclusive result; - the presence of a “paradox” when the inconclusive result is delivered as a negative one, increasing the degree of comprehension of information more difficult

L'arrivée de Hitler au pouvoir et la mise en œuvre de la politique du IIIème Reich provoquent l'arrivée en France de plusieurs vagues successives de réfugiés provenant d'Allemagne et de tous les pays nouvellement conquis par le nazisme. Lyon, véritablecitadelle contre l'obscurantisme, allait s'élever comme un rempart pour les protéger et venir en aide aux nouveaux arrivants, parmi lesquels les Juifs.Le 26 août 1942, 1016 juifs étrangers, entrés en France après le 1er janvier 1936, des dix départements de la Région de Lyon, situés en zone dite libre, avaient été arrêtés et dirigés au camp de Vénissieux, situé dans la proche banlieue de Lyon, par les forces de police et de gendarmerie françaises afin d'être livrés par Vichy à l'occupant nazi. En utilisant la couverture de l'Amitié Chrétienne et grâce au soutien des membres résistants des services de Vichy, des combattants sans armes et de toutes obédiences, qui avaient déjà entrepris d'autres actions de sauvetage et qui se connaissaient, participèrent au travail de la commission de criblage de Vénissieux afin de plaider chaque dossier des internés dans un sens favorables aux exemptions.Trois jours plus tard, le 29 août 1942, 545 d'entre eux ont été conduits à la gare de St Priest pour être livrés à l'occupant en vue de leur déportation à Auschwitz tandis que les 470 restants viennent d'être libérés dont une centaine d'enfants, et prennent le chemin de la liberté. Three days later, on August 29th, 1942, 545 prisoners where led to the train station of St Priest, handed over to the occupant and deported to Auschwitz, while 470 others, among which 100 children, and set off to freedom.The survivors we could find after the war, gave us the account of this epic. Still, the emblematic case of Vénissieux, will remain in the history of the Shoah in France. For employers, resume serves as one of the screening tools before interviewing applicants. Besides, the brief autobiography has been taken seriously by employers, due to the characteristics of the brief autobiography such as showing applicants’ personality, logic, and writing skills. Nowadays, employers screen applicants’ resumes and their brief autobiography as well. Previous research on the congruence of individuals’ gender and gender traits, reveal that individuals suffered from disadvantaged outcomes in incongruent conditions, such as being less likely to be chosen during interviews or being teased by peers. In this study, the author will examine the congruence of applicants’ gender on the resume and gender traits on the brief autobiography, to explore the effect of congruence between their gender and gender traits. This study is to investigate the relationships between congruence of applicants’ gender-gender traits and the screening results, and how evaluators’ seniority and HR experience affect on this relationship. The questionnaires were collected from 160 employees from different companies, and 140 of questionnaires were valid. The results of this study included: 1. Incongruence of applicants’ gender-gender traits has positive effect on applicants’ possibility of being interviewed further and possibility of being hired. 2. Evaluators’ seniority has no moderating effect on relationships between congruence of applicants’ gender-gender traits and the screening results. 3. Evaluators’ HR experience has no moderating effect on relationships between congruence of applicants’ gender-gender traits and the screening results.

Moreover, in every kind of network, interpersonal relationship is the most important. This relationship could be people’s social capital. This study extend the study of Nguyen, Allen, and Godkin (2006)which focused on the how do the “social capital” (friendship tie) and “human capital” affect applicants’ resumes. However, this study adopted the Balance Theory of Heider (1958) to propose the hypothesis of the resume filter process, and also using P,O and X instead of recruiter, applicant and job. The main purpose is to understand the influence of “social capital” and “human capital” on decision making when recruiters screen resumes. Furthermore, this paper will consider the influence of recruiter’s sex and self-monitoring on result of the screening of resumes. This research is based on the scenario description. We asked participants to assume themselves being the manager(recruiter) of the human resource department in on well-known business. The samples of this study are the Business Administration Department and Human Resource Department students of one university. This study used four applicant resumes including friendship with the applicant (with/without) and if the applicant matches the job requirements(Yes/No) to process resume filter.

Previous scholars think the main reasons which influence recruiters’ decision result are “Sex Role Congruency” and “Fit Model”. Thus, researchers started to focus on the factors such as physical attractiveness of photos on resumes and impression management tactics. They tried to discuss how those factors affect applicants’ gender traits perceived by recruiters and the reasons causing differences in employment. However, this paper adopts experimental method to discuss the matter that “Recruiters in Different Genders” perceive “Applicants in Different Genders” possess different “Gender Traits” and cause the differences in employment when applicants choose different “Sex Occupations”. This experiment makes applicants’ “Resumes” and “Job Description” to manipulate. The results show that male recruiters would perceive female applicants possess more “Warmth” than males but female recruiters wouldn’t perceive this way. Nevertheless, female recruiters would perceive male applicants possess more “Competency” than females but male recruiters wouldn’t perceive this way either. Lastly, no matter which occupation is applied for, once the applicants are perceived highly “Warm” and highly “Competent” by recruiters, they would get better appraisal.

By taking the highest incidence male cancer - oral cancer as an example, this study probed into the relationahip between the social demography, unhealthy-behaviors, health services utilization and the screening result of oral cancer in a county in northern Taiwan. The relationship between the sites of screening, doctors’ expertise and the diagnostic results were also investigated. By Logistic Regression Analysis, we found that gender, age, betel nut chewing and smoking are all related to positive oral cancer screening result. The screening sites and doctors’ expertise also linked to the result. In gender, the oral abnormality ratio of male is 2.024 times of female. In age, the oral abnormality ratio increases 1.004 times with one-year increasing. In smoking and betel nut chewing, the oral abnormality ratio increases 1.126 and 1.393 times separately with one-level rising of severity. In screening site, the positive screened ratio in hospital is 2.011 times of screened in community or workplace medical station. In doctors’ expertise, the positive screened ratio ranked from high to low are dentist , E.N.T doctors, and other expertise. By cross-table analysis, there is significant relationship between screening doctors’ expertise and confirming diagnosis of oral cancer screening. The accuracy of initial oral cancer screening in hospital is higher than screening in community and workplace. The screening outcomes by different expertise doctors are also different. The accuracy of confirming diagnosis by dentists and E.N.T doctors are both much higher than by the other expertise doctors. This can provide health institution the reference evidence of screening doctors’on-job training and practical education to enhance the quality of oral cancer screening and the capability of differential diagnosis. Eventually, it could benefit to achieve the goals of the health promotion and cancer medical expense reduction.

Colorectal cancer has become the third place of the top ten cancer mortality ranking in the past 16 years. Therefore, the object of this study is to understand the relationship between participant’s health behavior and test results of immunoassay fecal occult blood test(iFOBT) for colorectal cancer screening. Method: The sample’s age was between 50 and 74 years old, and data collected throughout the country from June 2011 to May 2013 who receive iFOBT by Health Promotion Administration's inspection database. SPSS Statistics 17.0 software was used to analyze the database. Result: A total of 114,715 who completed iFOBT. And 7.8% sample of the screening is a positive rate, male (9.7%) and female (6.5%) respectively. In the age group of "60-69", the number of positive rate were the hightest. Moreover, "Eastern region" has the highest positive rate(9%). Gender, age, geography, exercise, intake of fruits and vegetables etc, have a statistical significant differences with screening results. Conclusion: (1) The research object’s health behavior significant differences in gender, age, geography and family history.(2) The study of gender, age, and geography with positive results are statistically significant differences.(3) Those who screened positive results with health behaviors of "Exercise every time"(2=7.901,p=.019) and "Vegetable and fruit intake"(2=19.91,p=.000) all having relevance.(4)No matter result of screening with positive and family history(2=0.008,p=.918) having no relevance. Health behaviors associated with result screening for colorectal cancer. Recommended diet education seminars in the community advocate. Prevention of colorectal cancer.

However, there are very few studies longitudinally following up the same subjects who received repeated fecal occult blood test so the optimal interval for repeated fecal screening that is tailored to individual risk factors remains elusive. Aim: The purpose of this study was to assess the risk factors associating with a positive immunochemical fecal occult blood test following a baseline negative result. Methods: A retrospective cohort of asymptomatic adults who underwent health screening program from January 2007 through December 2011 was evaluated in the present study. Subjects with initial negative fecal tests were recruited as our study population and we followed up these subjects who received subsequent fecal tests in the 5-year study period. Risk factor analyses were based on the computerized dataset with anthropometric measures, social habits, medical histories, family histories, laboratory findings, symptomatic evaluation, and endoscopic screening. The Kaplan-Meier survival curves estimation and log-rank test and Cox proportional hazards regression analysis were performed to identify risk factors associated with the occurrence of a positive fecal test result following an initial negative test result. Results: A total 6532 of 26624 subjects were enrolled in the study with initial and following fecal tests, including 4065 men and 2467 women (mean age ± SD, 52.4 ± 10.8 years). The sensitivity, specificity of the fecal test for colonic neoplasm were 17.2% (95% CI: 16.8%-17.7%), 92.5% (95% CI: 92.2%-92.8%), respectively. Among 6532 study subjects, 448 had positive results of following fecal tests, including 68.3% male subjects. The Kaplan-Meier survival curves estimation and log-rank test and Cox proportional hazards regression analysis identified that elder age (hazard ratio= 1.026, 95% CI: 1.017-1.035) and male gender (hazard ratio= 1.29, 95% CI: 1.057-1.576) were significantly associated with the risk of positive fecal testing in the follow-up screening. Conclusions: Elder age and male were risk factors for the future occurrence of positive immunochemical fecal occult blood test results.

碩士<br>國立成功大學<br>護理學系碩博士班<br>94<br>The purposes of this study were (1) to explore parents’ experience about encountering the positive result of newborn screening on congenital hypothyroidism (CHT); (2) to generate knowledge about the parents’ uncertainty experience and coping process since they received positive CHT screening result till being disclosed diagnosis period. Eighteen subjects (10 mothers and 8 Fathers) were recruited from a referral medical center in southern Taiwan. Data was collected through field observation and semi-structure interviewing over 5 months in 2004 .The comparative method (grounded theory) was used to analyze data. Five categories were revealed which included “Panic-stricken: due to unexpectedly news”, “Anticipation versus fear: unendurable waiting“,”Articulating by situation changes: calculating and planing”, “Stay calm: preparation for receving diagnosis” and” Relief but worring: the uncertain future”. Finally, “Anchor finding within uncertainty experience “was generated as a core issue. This theory of described how parents cope wth uncertainty and interacte with health care provider. It also explains the process of decreasing parents’ distress and increasing their bearing capacity before being disclosed the diagnosis. In conclusion, the relevent interventions need to be established, especially in waiting time before CHT diagnosis is made. Implications for future studies in examining communication process between parents and health provider are suggested.

Abstract:
The diploma thesis deals with problems of depression in older people. Mainly the work is focused on identifying and analyzing the role of nurses in screening for depression in older people in primary care, acute care, long-term care and home care. This thesis was focused on theoretical direction and was used the method of design and demonstration. In this thesis was set one main goals with five research questions. The main goal was to identify and analyze the role of nurses in screening for depression in the elderly. RQ 1: What is the role of the nurse in screening for depression in the elderly? RQ 2: What is the role of the nurse in the primary care in screening for depression in the elderly? RQ 3: What is the role of the nurse in screening for depression in hospitalized patients in acute care? RQ 4: What is the role of the nurse in screening for depression in seniors in long-term and home care? RQ 5: What rating scales and methods are used in screening for depression in the elderly? The thesis introduce the concept of depression. The following are specified the causes of and the important factors that affect depression in the elderly. It also deals the differences in the clinical symptomatology of depression in old age. It explains possibilities and various barriers in the diagnosis of depression. Another chapter introduces complete geriatric examination, diagnostic classification systems, possible screening methods and scales for detection of depression in the elderly population. It also deals methods of pharmacological and non-pharmacological treatment and its possible complications associated with older age. By reason of increased suicide rate caused by depressive disorder the issue of suicidal behavior in the elderly is introduced. The next chapter deals with the nursing process, which is used by nurses in practice. It consists of the evaluation of the patient's health condition, making nursing diagnosis, creating nursing plan and subsequent implementation and evaluation. The nursing process is also needy for providing quality care. The nursing process in the stage of nursing diagnosis, introduces possible nursing diagnosis for a patient suffering from depression, which are based on the latest classification. Finally is described the role of nurses in screening for depression in the elderly in different health facilities and their contribution to the timely evaluation of depression in the elderly. This chapter introduces the role of nurses, nursing screening and collaboration with a physician. The role of nurses in screening for depression in different medical facilities is based on the first phase of the nursing process of assessment. On the basis of objective and subjective information, the nurse will assess the overall health and mental condition of the patient. Primarily, it was investigated what is the role of the nurse in screening for depression. On the basis of content analysis and synthesis it was necessary to used and processed domestic and foreign literature. A number of relevant sources are the results of various studies and Meta-analyzes, mostly from abroad, but also from the Czech Republic. The thesis can serve as a basis for nurses. The result of this thesis is to create e-learning material available for students in the Faculty of Health and Social Sciences of South Bohemia in Ceske Budejovice in the tutorial called Moodle.
