Until the 1980s, the use of immunoglobulin as a therapeutic agent was confined to replacement therapy in patients with primary or secondary antibody deficiency. Its role as an effective immunomodulator was discovered serendipitously, when IV immunoglobulin (IVIg) was shown to consistently increase the platelet count in a child with antibody deficiency and immune thrombocytopenic purpura. Since then, the use of high-dose IVIg as an immunomodulator has become established as an important therapeutic option in many immune-mediated diseases. This chapter reviews the therapeutic indications for immunoglobulin, and its potential adverse effects.

Nurses are an integral part of the interdisciplinary team caring for patients with HIV/AIDS. Since the start of the epidemic nurses have provided care across the healthcare spectrum, serving as care coordinators, clinical nurse specialists, and nurse practitioners. To competently care for people with HIV or AIDS, nurses have to provide care that is advanced and specialized. Nurses working in the field of HIV or AIDS care require advanced knowledge of virology and infectious disease to address the clinical needs of persons with HIV-related illness. This chapter describes the historical engagement of nurses in the epidemic and the nursing strategies that support interprofessional work. It addresses the key aspects of nursing care of the HIV-infected adult throughout the spectrum of HIV illness. The chapter focuses on the nurse’s role in minimizing risk, preventing HIV transmission, caring for persons with HIV, helping individuals to cope with illness, and negotiating the healthcare system.

Buoyancy largely supports fish, reducing the role of the skeleton, which functions as an attachment for muscle involved in movement and in protection, as exoskeleton (scales, scutes, bony plates) and as endoskeleton (vertebral column, skull). The general organization of fish skeletons and their component parts are described, as well as bone and cartilage. The interesting occurrence of acellular bone, additional to cellular bone, in teleosts is considered. Fish show metameric segmentation with myotomes on either side of the vertebral column, the latter acting as a compression strut, preventing shortening. Myotome muscle is organized into linear units named sarcomeres which contract by means of protein fibres, myosin and actin, sliding past each other. Usually fish body wall muscles occur as a thin outer layer of aerobic red muscle, with an inner thick region of anaerobic white muscle. Interspecific variability in the relative roles of myotomes and fin musculature in swimming is discussed.

Abstract:
Anaesthetists have a central role in cardiopulmonary resuscitation (CPR). The incidence of treated out-of-hospital cardiopulmonary arrest is 40 per 100 000 population and is associated with a survival rate to hospital discharge of 8–10%. The incidence of in-hospital cardiac arrest (IHCA) is 1–5 per 1000 admissions and is associated with a survival rate to hospital discharge of 13–17%. The most effective strategy for reducing mortality from IHCA is to prevent it occurring by detecting and treating those at risk or to identify in advance those with no chance of survival and to make a decision not to attempt resuscitation. The European Resuscitation Council and the Resuscitation Council (UK) publish guidelines for CPR every 5 years and the evidence supporting these is described in the international consensus on CPR science. The advanced life support algorithm forms the core of the guidelines but the precise interventions depend on the circumstances of the cardiac arrest and the skills of the healthcare providers. High-quality CPR with minimal interruptions will optimize survival rates. Shockable rhythms are treated with defibrillation while minimizing the pause in chest compressions. Although adrenaline (epinephrine) is used in most cardiac arrests, no studies have shown that it improves long-term outcome. The post-cardiac arrest syndrome is common and requires multiple organ support in an intensive care unit. Therapy in this phase is aimed at improving neurological (e.g. targeted temperature management) and myocardial (e.g. percutaneous coronary intervention) outcomes. Based on standard outcome measurements (e.g. cerebral performance category), 75–80% of survivors will have a ‘good’ neurological outcome, but many of these will have subtle neurocognitive deficits.

Police Community Support Officers: Cultures and Identities within Pluralized Policing presents the first in-depth ethnographic study of Police Community Support Officers (PCSOs) since the creation of the role in 2002. Situated within the tradition of police ethnographies, this text examines the working worlds of uniformed patrol support staff in two English police forces. Based on over 350 hours of direct observation and thirty-three interviews with PCSOs and police constables in both urban and rural contexts, the book offers a detailed analysis of the operational and cultural realities of pluralized policing from within. Using a dramaturgic framework, the author finds that PCSOs have been undermined by their own organizations from the beginning, which has left a lasting legacy in terms of their relationships and interactions with police officer colleagues. The implications of this for police cultures, community policing approaches, and the success of pluralization are examined. The author argues that while PCSOs can have similar occupational experiences to those of constables, their particular circumstances have led to a unique occupational culture, one which has implications for existing police culture theories. The book considers these findings in light of budget reductions and police reforms occurring across the sector, processes in which PCSOs are particularly vulnerable.

This chapter brings together the numerous mechanisms and ideas that have the effect of supporting a payment obligation, in the sense that the use of one (or more) of these concepts makes it more likely that the obligation will be fulfilled. This objective can be achieved in a number of ways: a third party may accept liability alongside the obligor, as in the case of someone who assumes a joint liability; or the supporter may take a secondary role as a guarantor. The chapter then looks at the forms of support that are available on a commercial basis, in particular performance bonds and export credit facilities. It considers the mechanisms that provide support by a more indirect route, such as comfort letters and credit derivatives. It concludes by examining a form of support that reaches its destination from the opposite direction: subordination, both in the form of general subordination of the creditor to the claims of all other creditors, and as subordination to the claims of a particular creditor.

Our nation increasingly relies on family members or friends (i.e., informal caregivers) for needed care and support as we age. Family caregivers typically assume their caregiving role willingly and reap personal fulfilment from helping a family member, developing new skills, and strengthening family relationships. For these benefits, however, caregivers often sacrifice their own health and well-being. Depression, anxiety, poor physical health, and compromised immune function are more common among family caregivers than in adults not providing such care. The REACH OUT (Resources for Enhancing Alzheimer’s Caregiver Health: Offering Useful Treatments) program is a multicomponent, tailored, and flexible intervention for caregivers of people with dementia focused on the evidence-based therapeutic strategy of problem solving. This work is designed to guide clinicians through the process and provide them the necessary tools to share with caregivers with the goal of enhancing caregiver physical and mental health. Five common risk areas (home safety, caregiver health, social support, challenging behaviors, and emotional well-being) are described in the manual; and interventions are described that respect the nuances of each risk area. By beginning with an individualized risk assessment and being flexible to the needs and issues of the caregiver, the REACH OUT intervention helps clinicians identify risk areas and provide caregivers with tailored action plans to reduce risk and promote well-being.

The overall objective of the social worker in this setting is to support and equip the patient and those close to them to navigate and adjust to the impact of the disease on their lives. The social work perspective views living with cancer as an experience accompanied by a series of challenges as treatment decisions are made, side effects are endured, and relationships strained. For most patients, these challenges are managed with support from loved ones and their healthcare team. However, any one of these difficulties can develop into a crisis or a situation where customary methods of coping do not work and the person living with the disease feels overwhelmed. The focus of this chapter will be on the social work role during the crisis periods of the cancer experience.

We have various kinds of moral obligations to take care of those to whom we stand in intimate relationships, and, for many of us, some of those whom we consider family are among our most important intimates. These moral obligations have various grounds; some are unique to intimate relationships while others also occur in non-intimate relationships. Given the centrality of intimate relationships and their attendant moral obligations to our lives, we need to consider what role, if any, political and legal institutions ought to play in protecting, enforcing, and/or regulating intimacy, whether familial or not. I consider various approaches that the state might take to intimate familial relationships and their possible justifications. I then consider state regulation of spousal support after the dissolution of a marriage to see what approach the state seems to take and whether this has implications for how it ought to proceed in other cases.

Continuous positive airways pressure (CPAP) is a mechanical ventilation (MV) mode in which the patient breaths spontaneously at a higher than atmospheric pressure. CPAP increases transpulmonary pressure inducing an FRC increase and a WOB decrease in acute restrictive lung pathology, with improvement of gas exchange. The work of breathing (WOB) is also reduced in the resistive component and inspiratory effort can be reduced if the patient experiences airway collapse and flow limitation, where CPAP counteracts the inspiratory threshold load represented by intrinsic PEEP. CPAP has been proven to be useful in many clinical situation and the technique for administration has a pivotal role in clinical efficacy of the technique. It’s crucial to keep the positive pressure as constant as possible and to avoid any technical increase of WOB. These goals can be achieved by continuous or demand flow systems. The modern ventilators work well and have overcome the valve function problem, which made difficult to use CPAP with old-generation machines.

This chapter focuses on salient internal and external supports and challenges situated within a broader lifespan development framework for queer (e.g., queer, lesbian, gay, bisexual) emerging adults (EAs). The authors review indicators of, and disparities in, well-being, as well as the role of romantic and sexual partners, parents, family, and peer support in promoting healthy development. Challenges to positive development for queer EAs such as school harassment, lack of health-supporting education, and discriminatory laws and employment practices are reviewed alongside recent advances in rights. Research and practice recommendations for how to further improve educational and legal contexts to support the well-being of queer EAs are discussed. The developmentally situated and positive view of queer EA thriving taken in this chapter allows for the appraisal of experiences and opportunities to better support, and combat barriers to, healthy development for queer populations.

Botanical medicines can play an important role in helping men and women with sexual dysfunction. Marketing hype has distorted their role, and illegal adulteration of herbal products with unlabeled phosphodiesterase type 5-inhibiting drugs is a rampant problem in the marketplace. Research most validates the traditional use of Panax ginseng (Asian ginseng) root, Pausinystalia yohimbe (yohimbe) bark, Epimedium spp (horny goat weed) leaf and stem, and Crocus sativus (saffron) stamen for enhancing libido and erectile function. Withania somnifera (ashwagandha) root, Turnera diffusa (damiana) leaf, and Tribulus terrestris (caltrop vine) fruit are less well studied but still have a strong reputation in traditional medicine as aphrodisiacs. Clinical use of these herbs is discussed with case examples. The problem with biopiracy of traditional aphrodisiacs from the Global South is also reviewed.

This chapter provides information about music therapy practice with children receiving care in a hospital, including information about music therapy service development in new settings. Music therapy offers opportunities for children and their families to be supported while undergoing difficult experiences following an injury or during an illness. Children with life-limiting conditions, and with chronic illness are also treated in hospitals, and can benefit from specialized support and help to cope with their circumstances. Music therapy can be provided to address treatment goals relating to psychosocial care, rehabilitation, pain management, developmental skill attainment, palliative care, and family issues. Music therapists use music improvisation, song singing, instrumental playing, music listening, and music composition, particularly song writing, to support young patients and their families. There is an emerging evidence base to support the role of music therapy as helpful to children and families needing support to cope with hospitalization and treatments.

Abstract:
This chapter demonstrates how anti-Communism became a means to debate the proper role of government on the issue of race rights. Liberals and their leftist allies supported the wartime New Deal's vision of an expanded role for government in both fair housing and fair employment for African Americans. They embraced the inclusive, democratic nationalism of the New Deal. Opponents of this view argued that racial advances would come at the expense of white workers and homeowners. These groups supported a far more limited conception of the New Deal, one that shied away from racial equality while providing federal support for white homeownership. They often equated racial egalitarianism with Socialism and Communism.

Chapter 11 considers the supportive care needs of people affected by cancer – not just those who are told that they have a cancer diagnosis, but also their family members and friends, who are also often profoundly affected. The critical role of health care professionals in meeting the needs of all people affected by cancer is underscored. The many and varied sources of additional support provided outside the clinical setting are then described. The review focuses upon the needs of adults with cancer, though many of the issues and examples are also applicable to children, adolescents, and young adults with cancer. A critical issue is how to best integrate clinical programmes with the variety of supports outside the clinical setting.

Abstract:
This chapter evaluates the claim that the decline of legitimacy is due to a decline of social capital. The idea that voluntary associations play an important role in establishing social cohesion and political support is a traditional insight in the field of political sociology. The basic assumption is that voluntary associations function as a training ground for democracy, where citizens involved acquire democratic norms and skills that they subsequently apply in their relation with the political system. If this argument is correct, political support should be at least partly influenced by citizens’ participation in civil society organizations. Using European Social Survey data from 2006 and 2012 the authors demonstrate that there is a clear and significant correlation between social involvement on the one hand and satisfaction with the working of democracy and political trust on the other, which largely survives the introduction of a range of control variables.

The World Health Organization supports the global application of the principles of palliative care to every child with a life-limiting illness, irrespective of geographical location whilst acknowledging that treatment options are limited in some countries. The care of the child with advanced and progressive illness and their family requires a holistic approach to the often complex physical, social, psychological, and spiritual needs as well as support in bereavement. This inevitably requires a paediatric palliative care team with diverse skills and abilities to meet these needs. This chapter discusses the global perspective of paediatric palliative care and the role of the team, nursing, and volunteers in that care. The significant physical and psychosocial issues for the child with advanced illness are discussed, as well as bereavement support of families. The necessity of quality improvement and strategies for setting up such a programme are outlined.

Abstract:
This article examines scientifically supported writing practices (SSWPs) and what instructional research tells us about the teaching of writing to school-aged students. It first explains why teachers should apply SSWPs before considering writing practices based on the insights and experience of professional writers and teachers, along with their limitations. It then describes writing practices based on scientific evidence and outlines three reasons why SSWPs are important to writing instruction and should become central to everyday writing practices. Twenty SSWPs that provide teachers with a variety of tools for improving students’ writing are analyzed; these SSWPs also provide a framework for a writing program that addresses six principles. The article concludes with a discussion of two basic theories that have dominated much of the discussion about how writing develops: The first takes into account how context shapes writing development, and the second emphasizes the role of cognition and motivation.
