Esse estudo avaliou a performance de dois adesivos com condicionamento ácido prévio (All- Bond 2 e One-step Plus - Bisco) e dois sistemas adesivos autocondicionantes (Clearfil SE Bond- Kuraray e Xeno III- Dentsply) quanto à resistência de união à dentina coronária e do canal radicular, penetração do adesivo na dentina desmineralizada e influência sobre o grau de conversão do cimento resinoso. Também foi determinada, com mecânica da fratura, a tenacidade da interface pino/cimento/dentina formada com o adesivo All-Bond 2. O canal radicular de 80 dentes humanos anteriores superiores foi instrumentado e, em seguida, preparado com brocas do sistema de pinos FRC Postec (Ivoclar). A dentina foi tratada com um dos sistemas adesivos e o pino foi cimentado com cimento Duo-link (Bisco). Oito grupos foram formados de acordo com o sistema adesivo e o tempo de armazenagem em saliva artificial (7 d ou 4 m). Seis secções transversais (~1,0 mm) foram obtidas das regiões compreendidas entre 4 mm acima da JEC até 4mm do ápice radicular, isto é, dentina coronária e do canal radicular. Três destas seis secções foram submetidas ao teste de pushout em máquina de ensaio universal Instron (1 mm/min, 50 Kgf). Nas secções restantes foram avaliados o grau de conversão do cimento e a penetração do adesivo na dentina com μ-Espectroscopia Raman. As amostras de tenacidade consistiram de dois hemi-discos de resina reforçada por fibra (pino) cimentados a duas fatias de dentina da câmara pulpar, formando um sanduíche (espécime Brazil nut). Os resultados do teste Anova 3-fatores de medida repetida (p<0.05) indicam que, apenas, a “região” exerceu efeito significante sobre a resistência de união (MPa). This study evaluated the performance of two etch-and-rinse (All-Bond 2 andOne-step Plus- Bisco) and two self-etch adhesive systems (Clearfil SE Bond- Kuraray and XenoIII-Dentsply) concerning their bonding durability to coronal and root canal dentin, infiltration onto dentin and influence on the degree of cure of a dual-cure cement. It was also determined the interfacial fracture toughness of the post/cement/dentin interface forme with All-Bond 2. The root canals of 80 human incisors and canines were instrumented and prepared with burs (FRC Postec System, Ivoclar). Dentin was treated with one of the four adhesives and FRC Postec posts (Ivoclar) were cemented to the root canal with Duo-link dual-cure resin cement (Bisco). Eight groups were formed according to the adhesive system employed and the aging time in water (7 or 4 m). Six transverse sections (~1.0 mm) were cut from 4 mm above the CEJ up to 4 mm short of the root canal apex, comprising coronal and root canal dentin. Three out of the six sections were submitted to push-out testing in an Instron machine (1 mm/min, 50 Kgf). The degree of conversion of the cement and the infiltration of the adhesive were evaluated in the remaining sections through Raman μ-Spectroscopy. The fracture toughness specimens were made out of two half disks of fiber–reinforced composite (post) cemented to dentin pieces, forming the Brazil nut specimen. The analisys obtained with the 3-way Anova of repeated measures test (p<0.05) showed that only the “region” had statistically significant effects on the bond strengths (MPa). Comparison of the means achieved in each region (Tukey, p < 0.05) revealed that the coronal portion (4.20 MPa) was higher than the cervical (3.45 MPa) and middle portions (3.26 MPa) of the root, which were not statistically different. Resumo: Este trabalho foi dividido em 2 capítulos, sendo que no capítulo 1 o objetivo foi de avaliar a influência do etanol na rugosidade, na energia livre de superfície da dentina radicular e na interação entre o cimento AH Plus e a superfície dentinária. Foram selecionadas 100 raízes de dentes humanos anteriores superiores e padronizadas com comprimento de 18 mm. As raízes foram cortadas longitudinalmente, obtendo-se 200 amostras dentinárias, divididas aleatoriamente em 4 grupos (n = 50), de acordo com os métodos de secagem: (1) úmido: apenas aplicação de ponta aspiradora, (2) pontas de papel absorvente: ponta aspiradora + pontas de papel absorventes, (3) solução de etanol 70%: etanol 70% (1 minuto) + ponta aspiradora + pontas de papel absorvente, (4) solução de etanol 100%: etanol 100% (1 minuto) + ponta aspiradora + pontas de papel absorvente. As amostras dentinárias foram avaliadas com o auxílio do rugosímetro e goniômetro, dentro dos seguintes parâmetros de avaliação: (1) rugosidade superficial; (2) energia livre de superfície e (3) ângulo de contato formado entre o cimento AH Plus e superfície dentinária. No capítulo 2, o objetivo foi avaliar a influência do etanol no preenchimento dos túbulos dentinários. Foram selecionadas 40 raízes de dentes humanos anteriores superiores, com comprimento radicular padronizado em 18 mm, e divididas aleatoriamente nos mesmos 4 grupos descritos anteriormente. Após a instrumentação, as raízes foram obturadas pela técnica de Schilder, com guta-percha e cimento AH Plus misturado com o marcador fluorescente Rodamina B. Para a visualização em microscopia confocal de varredura a laser foram confeccionadas 4 secções transversais (1 mm de espessura), em cada terço radicular. Foram avaliados os seguintes parâmetros: (1) porcentagem do perímetro da parede do canal coberta com cimento; (2) máxima profundidade de penetração do cimento endodôntico no interior dos túbulos dentinários; (3) porcentagem da área da dentina penetrada pelo cimento e (4) valor da intensidade do marcador fluorescente Rodamina B. Os resultados encontrados foram analisados estatisticamente pelos testes ANOVA / Tukey, com nível de significância de 5%, em ambos os capítulos. No capítulo 1 observou-se que nos grupos em que se utilizou o etanol, quando comparados aos demais, foram encontrados menor rugosidade de superfície da dentina, maior energia livre de superfície e menor ângulo de contato formado entre o cimento AH Plus e a superfície dentinária. Já no capítulo 2, não foi verificada diferença significante entre os grupos, quanto à avaliação da porcentagem do perímetro da parede do canal coberta por cimento. Entretanto, maiores valores de profundidade de penetração e da porcentagem da área da dentina penetrada pelo cimento foram encontrados no Grupo etanol 100%, seguidos pelos Grupos etanol 70%, pontas de papel e úmido. O etanol 100% atingiu resultados significativamente maiores que a concentração 70%, exceto para a intensidade da Rodamina B no terço cervical. Além disso, os resultados do Grupo pontas de papel foram melhores que o Grupo úmido, com exceção da intensidade da Rodamina B no terço apical. Diante da análise dos resultados apresentados nos 2 capítulos, verificou-se que o etanol (70% e 100%) proporcionou melhor molhabilidade do cimento AH Plus na superfície dentinária. Da mesma forma, as soluções de etanol, especialmente na concentração de 100%, favoreceram a penetração do cimento endodôntico no interior dos túbulos dentinários
Abstract: This study was divided into 2 chapters. In chapter 1, the aim was to evaluate the influence of ethanol on dentin roughness, dentin surface free energy and interaction between dentin surface. A hundred roots of human maxillary teeth were selected with length standardized in 18 mm. The roots were split longitudinally yielding 200 dentin samples, which were divided randomly into 4 groups according to the drying method used: (1) wet: vaccum only (2) paper points: vaccum + absorbent paper points, (3) 70% alcohol: 70% alcohol (1min) + vaccum + absorbent paper points and (4) 100% alcohol group: 100% alcohol (1min) + vaccum + absorbent paper points. The specimens were measured with aid of goniometer and rugosimeter within the following evaluation parameters: (1) surface roughness, (2) surface free energy and (3) contact angle formed between AH Plus sealer and dentin surface. In Chapter 2, the aim was to evaluate the influence of ethanol on dentinal tubules filling. Forty roots of human maxillary teeth with length standardized in 18 mm were selected. The roots were randomly divided among the same 4 groups described above. After instrumentation, the roots were filled by Schilder's technique with gutta-percha and AH Plus mixed with rhodamine B fluorescent marker. The roots were cross-sectioned and dentin sections (1 mm thick) were analyzed by confocal laser scanning microscopy. The evaluation parameters were: (1) Percentage of root canal wall coverage, (2) maximum depth of sealer penetration into dentinal tubules, (3) percentage of penetrated area and (4) fluorescent rhodamine B intensity. The results were statistically analyzed by ANOVA and Tukey tests (?=5%). In Chapter 1 the groups in which ethanol was used lower dentin surface roughness, higher surface free energy and lower contact angle formed between AH Plus and dentin surface were found when compared to the control groups. In chapter 2, there was no significant difference between the groups regarding the percentage of root canal wall coverage. Higher values of maximum depth of sealer penetration into dentinal tubules and percentage of penetrated area were found in Group 100% ethanol, followed by 70% ethanol, paper points and wet groups. The results achieved by 100% ethanol were significantly higher than 70% ethanol, except for intensity of rhodamine B in cervical third. Furthermore, the paper points results were better than wet group, except for intensity of rhodamine B in apical third. After analyzing the results presented in the two chapters, it was verified that ethanol solutions (70% and 100%) provide better wettability of AH Plus sealer on dentin surface. Similarly, ethanol solutions, especially at 100% concentration, improve the penetration of the sealer into dentinal tubules

Full textNiTi rotary instruments have shape memory and are highly flexible and super-elastic. These properties of the metal alloy allows for ease of root canal preparation to ultimately result in a root canal preparation that has a continuous taper, while canal shape and curvature is maintained. It must be noted that the NiTi rotary instrumentation may have an effect on root canal dentine, which may manifest as dentine damage. Different NiTi rotary systems on the market vary with regards to their design features and kinematics, which may influence dentine damage. The purpose of this in-vitro study was to compare the effect of four different NiTi rotary systems, as well as stainless steel files on root canal dentine. One hundred and eighty permanent human mandibular molar mesial roots were used for the study. The total samples were randomly divided into six groups, where one group (n=30) was left unprepared to serve as the control group. The remaining five groups were randomly assigned to a nickel-titanium rotary instrumentation system and one stainless steel hand file group. Group 1: Control group; Group 2: Stainless steel files group; Group 3: Wave One (Dentsply Maillefer) rotary group; Group 4: ProTaper NEXT (Dentsply Maillefer) rotary group; Group 5: iRaCe (FKG Dentaire) rotary group; Group 6: BT-Race (FKG Dentaire) rotary group.The root canal preparations were carried out according to the manufacturers' recommendations, after decoronation of the tooth crowns. Sodium hypochlorite (5,25%) and 17% EDTA was used as a root canal irrigant and a chelating agent during canal preparation. Each root segment was sectioned at 3mm, 6mm and 9mm from the apex. The root segments were observed under a stereomicroscope at 12x magnification and digital camera at 40 x magnification for the appearance of dentine damage. The images were observed by the author and an impartial second observer. Root segments were observed for the appearance of dentine damage (microcracks, craze lines or fractures), and samples were described as having 'dentine defect' or 'no dentine defect'. Data for the different groups were collected and results were calculated and the total incidence of dentine damage was as follows: Control group = 0% Stainless steel hand files group = 0% Wave One group = 56, 67% ProTaper NEXT group = 60% iRaCe group = 60% BT-Race group = 56, 67% There are no significant differences between the groups: WaveOne, ProTaper NEXT, iRACE and BT-Race. However, there is a significant difference between the four groups and the stainless steel group (p<0.0001).

O objetivo desse estudo é avaliar in vitro, por meio dos testes de push-out e tração, a adesão de diferentes cimentos endodônticos à dentina e aos componentes sólidos da obturação, guta-percha e resilon. Sessenta dentes humanos uniradiculares tiveram suas coroas removidas e foram divididos em três gupos de vinte dentes. Cada grupo foi instrumentado pela técnica rotatória até uma lima tipo Profile 40.04 e alargados com uma broca de preparo de pino com 1,5 mm de diâmetro, sob irrigação com hipoclorito de sódio 2,5% (NaOCl) seguido de irrigação final com EDTA 17%; gel de clorexidina 2%(CHX) alternado com soro fisiológico e seguido por EDTA 17% ou apenas com soro fisiológico, como controle. A seguir, foram obturados com os cimentos AH 26, Endo Fill, Endo Rez e Epiphany. Após 48 horas de incubação a 37 graus Celsius e 100% de umidade, os dentes foram cortados tranversalmente em discos de 2 mm de espessura. As amostras foram submetidas à força de compressão pelo teste de push-out. Para o teste de tração, discos de guta-percha e resilon foram criados e os cimentos AH 26, Endo Fill, Endo Rez e Epiphany foram colocados em contato com o material sólido da obturação por meio de anéis metálicos contendo os cimentos em seu interor. Após incubação as amostras foram submetidas ao teste de tração. Ambos os testes foram realizados em máquina de ensaios (1mm/min) e os resultados expressos em Mpa. O teste Anova e Newman-Keuls evidenciaram que cimento Epiphany apresentou maior adesão à dentina no teste de push-out, seguido pelos cimentos Endo Rez e AH 26 que foram semelhantes entre si e, por último o cimento Endo Fill. O uso da CHX aumentou a adesão à dentina de todos os cimentos testados. No teste de tração aos componentes sólidos, o cimento AH 26 mostrou ter uma maior adesão que os demais cimentos, que foram semelhantes entre si.
The aim of this in vitro study was to evaluate the bond strength of different root canal sealers to dentin and to the filling core material, using push-out and tensile tests. Sixty extracted single-rooted human teeth were divided into three groups and evenly instrumented with the crown-down technique up to a file #55 followed by a root canal enlargement using a 1,5 mm diameter post drill. Teeth were prepared under irrigation with 2.5% NaOCl + EDTA 17 %, 2% CHX associated to a saline solution + EDTA 17% or just saline solution, as control. All teeth were completely filled with AH 26, Endo Rez, Endo Fill and Epiphany sealers. After 48 hours of storage in 100% humidity at 37°C, all samples were sectioned transversally into 2mm thickness disks and the push-out tests were performed. For the tensile tests, gutta-percha and resilon disks were created and metallic rings with the sealers were positioned over the core material disks, in the same time/temperature conditions. Both push-out and tensile tests were performed using the universal testing machine (1 mm/min) and the maximum load at failure expressed in MPa. Two-way ANOVA and Newman-Keuls tests showed that Ephipany sealant presented the highest bond strength to dentin followed by Endo REZ and AH 26 that were similar to each other and Endo Fill with the lowest values of bonding to dentin. 2% CHX irrigation increased the bonding strength of all sealers to dentin. Tensile tests to the filling core materials showed that AH 26 presented the highest values of bonding. No statistical differences were found between the other sealers.

Full textTo get a better action of MIC, it is necessary to penetrate into the teeths ducts. So, this search brings an evaluation of teeths permeability by the penetration of MIC-M (intracanal medication with Rhodamine 1%), after biomechanical instrumentation with chemical solutions associated or not to irradiation with laser of high power of diode 810 nm. It has been used 72 daily pay-molar inferiors with only one root. The samples had been divided in three groups: I, II and III, composed by for 24 specimens each. These in turn, had been subdivided in three other groups, A, B and C in accordance with the chemical substance used in the biomechanical instrumentation. Group A used the sodium hypochlorite (NaOCl) 2,5%; group B used EDTA-T 17%; and group C used NaOCl 2.5% and final irrigation with EDTA-T 17% during three minutes. Group I was radiated after the biomechanical preparation immediately, in group II the diode laser was applied before the full filling of the canal with the MIC and group III was the group that has controlled. The teeth had been bisected longitudinally, typed and the images evaluated in the ImageLab program. The protocol that showed the best results were in group IB irrigation with EDTA-T, followed by MIC-M and irradiation of root canal walls. The results obtained were submitted to statistical analysis. Our conclusion for this study is that the experimental protocols caused alterations on penetration ability of the indicator agent.

Resumo: Conceitos da odontologia restauradora adesiva vêm sendo empregados em endodontia na cimentação de pinos intrarradiculares e na obturação dos condutos. O primeiro estudo que compõe este trabalho teve como objetivo comparar a qualidade e durabilidade do selamento promovido por materiais obturadores endodônticos que se aderem à dentina (Epiphany/ Resilon) àquelas obtidas com materiais convencionais (AH Plus/ guta-percha) e verificar a influência da existência de uma barreira coronária neste selamento. Dentes humanos unirradiculares receberam tratamento endodôntico e obturação com uma das combinações de materiais em estudo (Epiphany/ Resilon ou AH Plus/ guta-percha). Os condutos obturados receberam selamento coronário com Coltosol, Clearfil SE Bond/ Filtek Z-250 ou foram deixados sem barreira coronária. A qualidade do selamento proporcionada por cada combinação de material foi avaliada por um método de transporte de fluidos em dois períodos: após a presa do cimento endodôntico (imediato) e depois de 180 dias de armazenamento. Espécimes obturados com Epiphany/Resilon apresentaram maior permeabilidade (selamento menos efetivo) independentemente do tipo de selamento coronário e do período de avaliação. Ambos materiais utilizados como barreira coronária (Coltosol ou Clearfil SE Bond/ Filtek Z-250) aumentaram significativamente o selamento dos espécimes. Após o armazenamento, observou-se redução nos níveis de permeabilidade dos espécimes obturados com AH Plus/guta-percha sem selamento coronário ou selados com Coltosol e dos espécimes obturados com Epiphany/Resilon que não receberam selamento coronário. Concluiu-se que a capacidade de selamento de materiais obturadores adesivos é inferior àquela dos materiais obturadores convencionais e que o armazenamento por 180 dias não interferiu negativamente nesse selamento. A presença de uma barreira coronária contribuiu para a efetividade do selamento radicular. O segundo estudo teve como objetivo investigar a expressão de enzimas metaloproteinases da matriz (MMPs) na dentina radicular uma vez que tais enzimas têm sido implicadas na perda da estabilidade da união dente-material restaurador. Fragmentos de dentina humana coronária e radicular foram triturados e transformados em pó e as proteínas dentinárias extraídas com o uso de hidrocloreto de guanidina e EDTA. A análise por zimografia revelou atividade gelatinolítica na dentina coronária e radicular correspondente à MMP-2 e MMP-9. A metaloproteinase 2 foi mais intensamente detectada na dentina radicular desmineralizada e a MMP-9 foi, em sua maioria, recuperada do compartimento mineralizado do tecido, antes do uso do EDTA. Através do ensaio de Western Blot detectou-se ainda MMP-8 em ambos os substratos. Concluiu-se que metaloproteinases são expressas em dentina radicular de maneira semelhante à dentina coronária. No terceiro estudo a mesma metodologia foi empregada para a detecção de metaloproteinases no substrato dentinário bovino, uma vez que este é frequentemente utilizado como um substituto à dentina humana em ensaios de resistência de união. As proteínas da dentina bovina coronária apresentaram atividade gelatinolítica em zimografia correspondente à MMP-2 e MMP-9, enquanto a dentina radicular apresentou atividade compatível com MMP-2 somente. Pode-se concluir que o substrato dentinário bovino expressa metaloproteinases com atividade gelatinolítica e que existem diferenças no perfil de expressão entre coroa e raiz.
Abstract: Restorative strategies involving dentin bonding have been increasingly applied to endodontics. The first study evaluated the immediate and long-term sealing ability of adhesive versus conventional endodontic filling materials and investigated the influence of a coronal seal on leakage rates. After chemo-mechanical preparation/irrigation, human roots were filled with either AH Plus/gutta-percha or Epiphany/Resilon. Coronal sealing was performed with Coltosol or Clearfil SE Bond/Filtek Z250, and some samples received no material. The quality of root canal sealing was assessed by a fluid transport method, performed after sealer setting and 180 days of storage. Specimens filled with Epiphany/Resilon exhibited higher leakage than specimens filled with AH Plus/gutta-percha regardless of experimental condition. Coronal sealing reduced leakage significantly, but no difference was detected between Coltosol and Clearfil/Filtek Z250. After storage, significant decrease in leakage was observed in roots filled with AH Plus/gutta-percha either without coronal sealing or sealed with Coltosol, as well as in roots filled with Epiphany/Resilon without coronal sealing. It was concluded that adhesive filling materials did not provide better sealing than the conventional materials and that this sealing was not disturbed by storage of root filled specimens The presence of a coronary seal contributed to reduce leakage. Matrix metalloproteinases (MMPs) found in coronary dentin may play a role in the loss of stability of bonded restorations. The second study aimed to investigate whether these enzymes can also be detected in root dentin. Crown and root sections of human teeth were pulverized into powder and dentin proteins were extracted by using guanidine-HCl and EDTA. Zymography revealed gelatinolytic activity in both crown and root dentin samples, with bands corresponding to MMP-2 and MMP-9. MMP-2 activity was more evident in demineralized root dentin whereas MMP-9 was mostly extracted from the mineralized compartment of dentin. Western Blot analysis was able to detect MMP-8 equally distributed in crown and root dentin. In conclusion, MMPs are expressed in radicular dentin similarly to crown dentin. Using the same methodology, the third study assessed whether metalloproteinases are expressed in bovine dentin since this substrate has been frequently used as a substitute to human dentin in bond strength evaluations. Protein extracts obtained from bovine crown dentin exhibited gelatinolytic activity in zymography corresponding to MMP-2 and MMP-9. A similar pattern could be observed in root dentin proteins regarding the expression of MMP-2 isoforms, but no gelatinolytic activity was observed in the molecular weight area corresponding to MMP-9. It could be concluded that bovine sound dentin can express MMPs with gelatinolytic activity, but differences may occur in the expression profile of crown and root dentin substrates.

Resumo: O objetivo desse estudo é avaliar "in vitro", por meio dos testes de push-out e tração, a adesão de diferentes cimentos endodônticos à dentina e aos componentes sólidos da obturação, guta-percha e resilon. Sessenta dentes humanos uniradiculares tiveram suas coroas removidas e foram divididos em três gupos de vinte dentes. Cada grupo foi instrumentado pela técnica rotatória até uma lima tipo Profile 40.04 e alargados com uma broca de preparo de pino com 1,5 mm de diâmetro, sob irrigação com hipoclorito de sódio 2,5% (NaOCl) seguido de irrigação final com EDTA 17%; gel de clorexidina 2%(CHX) alternado com soro fisiológico e seguido por EDTA 17% ou apenas com soro fisiológico, como controle. A seguir, foram obturados com os cimentos AH 26, Endo Fill, Endo Rez e Epiphany. Após 48 horas de incubação a 37 graus Celsius e 100% de umidade, os dentes foram cortados tranversalmente em discos de 2 mm de espessura. As amostras foram submetidas à força de compressão pelo teste de push-out. Para o teste de tração, discos de guta-percha e resilon foram criados e os cimentos AH 26, Endo Fill, Endo Rez e Epiphany foram colocados em contato com o material sólido da obturação por meio de anéis metálicos contendo os cimentos em seu interor. Após incubação as amostras foram submetidas ao teste de tração. Ambos os testes foram realizados em máquina de ensaios (1mm/min) e os resultados expressos em Mpa. O teste Anova e Newman-Keuls evidenciaram que cimento Epiphany apresentou maior adesão à dentina no teste de push-out, seguido pelos cimentos Endo Rez e AH 26 que foram semelhantes entre si e, por último o cimento Endo Fill. O uso da CHX aumentou a adesão à dentina de todos os cimentos testados. No teste de tração aos componentes sólidos, o cimento AH 26 mostrou ter uma maior adesão que os demais cimentos, que foram semelhantes entre si.
Abstract: The aim of this in vitro study was to evaluate the bond strength of different root canal sealers to dentin and to the filling core material, using push-out and tensile tests. Sixty extracted single-rooted human teeth were divided into three groups and evenly instrumented with the crown-down technique up to a file #55 followed by a root canal enlargement using a 1,5 mm diameter post drill. Teeth were prepared under irrigation with 2.5% NaOCl + EDTA 17 %, 2% CHX associated to a saline solution + EDTA 17% or just saline solution, as control. All teeth were completely filled with AH 26, Endo Rez, Endo Fill and Epiphany sealers. After 48 hours of storage in 100% humidity at 37°C, all samples were sectioned transversally into 2mm thickness disks and the push-out tests were performed. For the tensile tests, gutta-percha and resilon disks were created and metallic rings with the sealers were positioned over the core material disks, in the same time/temperature conditions. Both push-out and tensile tests were performed using the universal testing machine (1 mm/min) and the maximum load at failure expressed in MPa. Two-way ANOVA and Newman-Keuls tests showed that Ephipany sealant presented the highest bond strength to dentin followed by Endo REZ and AH 26 that were similar to each other and Endo Fill with the lowest values of bonding to dentin. 2% CHX irrigation increased the bonding strength of all sealers to dentin. Tensile tests to the filling core materials showed that AH 26 presented the highest values of bonding. No statistical differences were found between the other sealers.

Full textGutta-percha, in combination with a variety of sealers, is the most commonly used material for root canal filling. This association presents no adhesiveness to the root structure. Resilon cones and resinous cement (Epiphany) were introduced on the market that, in association with a self-conditioning primer, would alow a solid monoblock to be obtained. The Resilon/Epiphany system forms a monoblock within the canals that bonds to the dentin walls. It has potential to strengthen the walls against fracture. The material was proposed as an alternative to gutta-percha. The purpose of this in vitro study was to evaluate the fracture resistance of root canals filled with gutta-percha/ AH Plus and root canals filled with Resilon/Epipanhy system. Sixty extracted single rooted teeth were selected. The teeth were randomly categorized into 6 groups of 10 teeth each as follows: Group I: teeth without endodontic treatment; Group II: teeth were instrumented but without endodontic obturation; Group III: teeth were instrumented and obturated with gutta-percha in conjunction with AH Plus sealer and obturated with lateral condensation technique; Group IV: teeth were instrumented and obturated with gutta-percha in conjunction with AH Plus sealer and obturated with hybrid Tagger`s techinique; Group V: teeth were instrumented and obturated with Resilon/Epiphany system and obturated with lateral condensation technique; Group VI: teeth were instrumented and obturated with Resilon/Epiphany system and obturated with hybrid Tagger`s technique. The roots were submited to the compressive test. A Kratos Universal Testing Machine was used to apply a load to each specimen at a crosshead speed of 0,5mm per minute until the root fracture. The results were submited to ANOVA test ( 5%) and Tukey test. With respect to the fracture resistance there was no statiscally significant difference between the group filled with gutta-percha and the Resilon/Epiphany system. However, the group VI showed statistically significant difference when compared with group I and group II. The presence of the endodontic material is very important to maintenance the good fracture resistance, because the group II showed the worst results. The thermoplastic technique presented better results, but within statistically significant differences.

Resumo: O objetivo deste estudo in vitro foi avaliar o selamento apical após obturação endodôntica, em dentes humanos preparados com e sem ampliação do forame apical, com o Sistema Rotatório Mtwo® (VDW, Munique, Alemanha). Vinte prémolares inferiores foram divididos em dois grupos. No Grupo I, os canais radiculares foram preparados 1mm além do forame apical e, no Grupo II, 1 mm aquém do CRC. Os dentes foram preparados para avaliação no MEV (JEOL, Modelo JSM 5600LV, Tókio, Japão) e as imagens do forame apical obtidas em aumento de 75x, em posições padronizadas, permitindo a mensuração da área foraminal antes, após o preparo e após a obturação. Por meio do processo do Sistema de Subtração de Imagens (Emago® Sistema de Diagnóstico Oral, Louwesweg, Amsterdam, Holanda), foi possível identificar as variações de forma e transporte do forame apical assim como o nível de preenchimento de sua circunferência pelo cimento obturador Pulp Canal Sealer (Sybron Endo, USA) após o preparo. Os resultados encontrados, submetidos à análise estatística, permitiram concluir que os dentes do Grupo I apresentaram maior diâmetro foraminal que os do Grupo II (Teste Anova, p<0,05) e que houve diferença estatisticamente significante quanto ao transporte do forame (Teste Exato de Fisher, p=0,0108); quanto ao selamento apical houve diferença estatisticamente significante entre os grupos I e II (Testes t de Student e Exato de Fisher, p=0,0007) sendo que, no Grupo I, ocorreram selamento em 100% dos espécimes analisados
Abstract: The aim of this in vitro study was to evaluate the apical root sealing after endodontic filling, in human teeth that were prepared with and without apical foramen enlargement by Mtwo® Rotary System (VDW, Munich, Germany). Twenty mandibular premolars were divided in two groups. Group I had root canals prepared 1mm beyond apical foramen, and in Group II, 1mm below RCL. Teeth were prepared for an electronic microscopy analysis (JEOL, Model JSM 5600LV, Tokyo, Japan). Apical foramen images were 75 times magnified, at standardized positions, allowing measurements from apical foramen area before and after root canal preparation, and after root filling. By using the Image Subtraction System (Emago®, Oral Diagnostic System, Louwesweg, Amsterdam, Netherlands) apical foramen shape modifications and transportation as well as the level of its circumferential filling by Pulp Canal Sealer cement (Sybron Endo, USA) after root canal preparation were accessed. After statistical analysis, results allowed to conclude that Group I teeth showed larger foraminal diameter than Group II (Anova Test, p<0,05) and that foramen transportation difference was statistically significant (Fisher Exact Test, p=0,0108); apical foramen sealing also differed statistically between groups I and II (Student t Test and Fisher Exact Test, p=0,0007) and that 100% of Group I specimens showed apical root sealing

Resumo: O objetivo deste estudo foi avaliar a microbiota presente no sistema canais radiculares antes e após o preparo biomecânico; e as condições histopatológicas da reparação apical e periapical após preparo biomecânico dos canais radiculares de dentes com reação periapical crônica. Foram utilizados 78 canais radiculares de dentes de cães, os quais, após indução de lesão periapical, foram submetidos ao preparo biomecânico utilizando as seguintes soluções irrigadoras: solução de digluconato de clorexidina a 2%, de hipoclorito de sódio a 2,5% ou soro fisiológico. Um grupo controle não recebeu preparo biomecânico. Foram realizadas culturas microbiológicas antes e após 30 dias do preparo biomecânico. Após este período os animais foram mortos para processamento e análise histopatológica. O estudo microbiológico demonstrou que houve redução dos microrganismos nos grupos que utilizaram soluções antimicrobianas (p<0,05), com melhor resultado para a solução de clorexidina. Os grupos que utilizaram solução fisiológica controle apresentaram aumento de microrganismos. No estudo histopatológico, foi observado, de um modo geral, infiltrado inflamatório periapical severo e em grande extensão, severo espessamento do ligamento periodontal e grandes áreas de reabsorção óssea e apical em todos os grupos (p>0,05). Concluiu-se que o emprego de soluções irrigadoras antimicrobianas durante o preparo biomecânico promove redução da microbiota endodôntica, sem a sua eliminação do sistema de canais radiculares em dentes de cães com lesão periapical. A solução de clorexidina a 2% proporcionou maior redução da microbiota que a solução de hipoclorito de sódio a 2,5% (p<0,05). Porém, os resultados histopatológicos demonstram que somente o preparo biomecânico não foi capaz de criar condições para reparação dos tecidos apicais e periapicais.
Abstract: The aim of this study was to evaluate the microorganisms in the root canal system before and after biomechanical preparation and the apical and periapical repair after this biomechanical preparation. The study was performed in 78 root canals of dog's teeth with induced chronic periradicular lesion. There were used the following endodontic irrigating solutions: 2% chlorhexidine digluconate solution; 2.5% sodium hypochlorite solution, saline solution and a control group without biomechanical preparation. The microbiological sampling were performed before and 30 days after the biomechanical preparation. After this period the animals were killed to histological analysis. In the microbiologic study, the results showed that the antimicrobial solutions used reduced the number of microorganisms (p<0.05). The chlorhexidine solution showed better performance. The group that used saline solution and the control group presented an increased number of microorganisms. The histological results showed a severe periapical inflammation, severe thickness of periodontal ligament, apical and osseous resorption in all groups (p>0.05). It was concluded that the antimicrobial endodontic solution reduced but not eliminated the microorganisms present in root canals of dogþs teeth with necrosis and periapical lesion. The chlorhexidine solution was more effective in reducing the number of microorganisms than sodium hypochlorite solution (p<0.05). Although, the histopathological results showed that the biomechanical preparation alone was not capable to promote an adequate condition for the apical and periapical tissues repair.

Resumo: Este trabalho avaliou in vitro a resistência à fratura de raízes de incisivos bovinos com 3 espessuras de dentina, nas quais, foram cimentados pinos de fibra de vidro com e sem reembasamento com resina composta. 180 raízes bovinas foram padronizadas em 14 mm. Os espécimes foram divididos em 3 grupos de acordo com a espessura da parede dentinária: Grupo1 (G1) = 1 milímetro de espessura (n=60); Grupo 2 (G2): 2 milímetros de espessura (n=60) e Grupo Controle (G3)= raízes sem fragilização (n=60). Os 4 mm apicais dos canais radiculares foram selados com Coltosol® e as raízes foram inseridas em um material de moldagem a base de poliéter e resina acrílica para simulação ligamento periodontal. Em seguida, cada grupo foi subdividido em 2 sub-grupos (n=30): raízes que receberam cimentação de pino de fibra de vidro somente (P) e raízes que receberam cimentação com pino reembasado com resina composta (PR). O cimento Relyx ARC ® foi utilizado para cimentação dos retentores intrarradiculares. Os corpos-de-prova foram submetidos ao teste de resistência à fratura em máquina de ensaio com aplicação de carga em ângulo de 135º com o longo eixo do espécime por lingual (n= 90) e por vestibular (n=90). Os valores foram registrados em Newtons (N) submetidos à Análise de variância e teste de Tukey. As fraturas foram avaliadas em microscópio clínico e classificadas quanto à possibilidade de reabilitação pós-fratura em favoráveis e desfavoráveis. O dado obtido deste estudo mostrou não haver diferença estatística significativa entre os grupos avaliados (p>0,05) com relação ao teste de resistência a fratura com forças aplicadas por vestibular. Por lingual, o grupo G1PR apresentou diferença estatística quando comparado aos grupos G2P e G3P (p<0,05). No teste por vestibular, os espécimes do grupo PR que tiveram fratura radicular, tiveram a incidência de fraturas favoráveis em G1: 87,5%, G2: 80% e G3: 83,3%. Por Lingual, o grupo G3PR apresentou fraturas apenas nos núcleos de preenchimento, não havendo fraturas em raiz. O G1PR e o G1P apresentaram 80% dos espécimes com fraturas radiculares, das quais 91,67% e 66,67% foram classificadas como favoráveis. As diferentes espessuras de dentina não interferiram na resistência à fratura, sugerindo que dentes com paredes fragilizadas são passíveis de serem restaurados. No ensaio realizado por Lingual G1PR apresentou maior resistência a fratura. A maioria das fraturas radiculares, ocorridas nas raízes restauradas com pinos de fibra de vidro e núcleos de preenchimento em resina composta, foram consideradas favoráveis
Abstract: The aim of this is study was to evaluate in vitro fracture strength of weakened roots with 3 thicknesses of dentin, in which were cemented fiberglass posts and anatomic fiber posts (fiberglass post with composite resin). 180 freshly extracted bovine incisors were used. The crowns and apices of were sectioned to create the samples with 14 mm of root. The samples were divided into 3 groups: Group 1 (G1) = dentin walls with 1 mm thickness (n = 60), Group 2 (G2): dentin walls with 2 mm thickness (n = 60) and Control Group (G3) = roots without weakening (n = 60). 4mm apical root canal was sealed with Coltosol ®, and the samples were inserted into artificial periodontal ligament with an impression material and acrylic resin. Then, each group was divided into two subgroups (n = 30) that received roots luting of glass fiber post only (P) and roots receiving cementing anatomic fiber post (PR). The cement Relyx ARC ® was used for cementation of intracanal retainers. The samples were tested for fracture strength in a universal testing machine with load application angle of 135 degrees with the long axis of the specimen by palate (n = 90) and vestibular (n = 90). The values were recorded in Newtons (N) and statistically analyzed by ANOVA. The fractures were evaluated in clinical microscope and classified after fracture as favorable and unfavorable. Results from this study showed no statistically significant difference among the groups (p > 0.05) to the fracture strength test with forces applied by vestibular. On the lingual face there was statistically significant difference between groups G1PR and G1P when compared to CPR (p< 0.05). On the vestibular face the specimens restored with anatomic post that had root fracture from groups G1, G2 and G3 had the incidence of favorable fractures in 87.5%, 80% and 83.3% of the cases. By lingual the CPR group had fractures only in filling cores, with no root fractures. The G1PR and G1P showed 80% of specimens with root fractures, of which 91.67% and 66.67% were classified as favorable. The different thicknesses of dentin did not affect the fracture strength, suggesting that teeth with weakened walls are easily restorable. By Lingual. O objetivo deste estudo foi avaliar a microbiota presente no sistema canais radiculares antes e após o preparo biomecânico; e as condições histopatológicas da reparação apical e periapical após preparo biomecânico dos canais radiculares de dentes com reação periapical crônica. Foram utilizados 78 canais radiculares de dentes de cães, os quais, após indução de lesão periapical, foram submetidos ao preparo biomecânico utilizando as seguintes soluções irrigadoras: solução de digluconato de clorexidina a 2%, de hipoclorito de sódio a 2,5% ou soro fisiológico. Um grupo controle não recebeu preparo biomecânico. Foram realizadas culturas microbiológicas antes e após 30 dias do preparo biomecânico. Após este período os animais foram mortos para processamento e análise histopatológica. O estudo microbiológico demonstrou que houve redução dos microrganismos nos grupos que utilizaram soluções antimicrobianas (p<0,05), com melhor resultado para a solução de clorexidina. Os grupos que utilizaram solução fisiológica controle apresentaram aumento de microrganismos. No estudo histopatológico, foi observado, de um modo geral, infiltrado inflamatório periapical severo e em grande extensão, severo espessamento do ligamento periodontal e grandes áreas de reabsorção óssea e apical em todos os grupos (p>0,05). Concluiu-se que o emprego de soluções irrigadoras antimicrobianas durante o preparo biomecânico promove redução da microbiota endodôntica, sem a sua eliminação do sistema de canais radiculares em dentes de cães com lesão periapical. A solução de clorexidina a 2% proporcionou maior redução da microbiota que a solução de hipoclorito de sódio a 2,5% (p<0,05). Porém, os resultados histopatológicos demonstram que somente o preparo biomecânico não foi capaz de criar condições para reparação dos tecidos apicais e periapicais.
The aim of this study was to evaluate the microorganisms in the root canal system before and after biomechanical preparation and the apical and periapical repair after this biomechanical preparation. The study was performed in 78 root canals of dog's teeth with induced chronic periradicular lesion. There were used the following endodontic irrigating solutions: 2% chlorhexidine digluconate solution; 2.5% sodium hypochlorite solution, saline solution and a control group without biomechanical preparation. The microbiological sampling were performed before and 30 days after the biomechanical preparation. After this period the animals were killed to histological analysis. In the microbiologic study, the results showed that the antimicrobial solutions used reduced the number of microorganisms (p<0.05). The chlorhexidine solution showed better performance. The group that used saline solution and the control group presented an increased number of microorganisms. The histological results showed a severe periapical inflammation, severe thickness of periodontal ligament, apical and osseous resorption in all groups (p>0.05). It was concluded that the antimicrobial endodontic solution reduced but not eliminated the microorganisms present in root canals of dogþs teeth with necrosis and periapical lesion. The chlorhexidine solution was more effective in reducing the number of microorganisms than sodium hypochlorite solution (p<0.05). Although, the histopathological results showed that the biomechanical preparation alone was not capable to promote an adequate condition for the apical and periapical tissues repair.

Full textThis prospective university-based study aimed to evaluate the success rate and factors associated to failure of pulpectomies performed in primary teeth by undergraduate students. The convenience sample comprised patients aged 3 to 10 years who were under care in the institution and underwent primary tooth pulpectomies. Treatment-related data, such as type of tooth involved (anterior or posterior teeth), presence of dental pain, tooth status, pulp diagnosis, EDTA treatment after chemic-mechanical preparation, use of intra-canal medicament, technique used for endodontic filling, quality of endodontic obturation, type of restorative material used (CR; RMGIC), number of appointments and consult time were collected by a single evaluator on a specific chart for endodontic procedure. Also, Factors potentially associated with treatment failure were also investigated, including gender (male; female), patient age, Visible Plaque (VPi) and Gingival Bleeding (GBi) index, decayed, missed and filled teeth (dmft), and child behaviour. Kaplan–Meier survival curve with log-rank test was used to analyze the longevity of pulpectomies (p<0.005). Lastly, 81pulpectomies placed in 62 children (5.6 1.5 years) were included in the analysis. The survival of restorations reached 62.9% up to 12 months of follow-up, with overall annual failure rate (AFR) of 28.9%, and the majority of the failures occurred in the first 3 months (p<0.001). Teeth restored with composite resin presented more longevity than those teeth restored with RMGIC (p=0.006). The use of two or more sessions to perform the endodontic treatment resulted in more failures (p=0.028). Patients presenting gingivitis (GBI>20%) experienced more failures in their pulpectomies (p=0.006). There were no significant differences for the others individual and clinical parameters tested. The pulpectomies of primary teeth performed by undergraduate students presented low success rates.

Resumo: Um dos principais objetivos do tratamento endodôntico é o debridamento do sistema de canais radiculares através da remoção dos restos pulpares, da smear layer e smear plugs, para eliminar a contaminação bacteriana e subprodutos da mesma, os quais podem se tornar reservatórios de irritantes no canal tratado. Além disso, as substâncias irrigadoras e os métodos de irrigação utilizados durante o preparo químico-mecânico, podem também contribuir para a remoção da smear layer e consequentemente alterar a permeabilidade dentinária. Dessa forma, os objetivos desta Dissertação foram: (1) discutir através da revisão da literatura, a eficácia de substâncias irrigadoras, bem como a influência de dois métodos de irrigação (irrigação manual ou ativação ultrasônica) utilizados durante o preparo químico-mecânico de dentes decíduos e permanentes, considerando a permeabilidade da dentina radicular; (2) correlacionar o índice de permeabilidade da dentina radicular de dentes decíduos sob a influência de substâncias irrigadoras associadas ou não a auxiliares de instrumentação (líquido de Dakin, líquido de Dakin associado ao peróxido de hidrogênio, solução salina associada à clorexidina gel e solução salina) e diferentes métodos de irrigação (irrigação manual ou ativação ultrasônica) aos seus aspectos morfológicos (presença ou ausência de smear layer). Baseando-se na revisão de literatura pôde-se concluir que as substâncias irrigadoras e associações utilizadas durante o tratamento endodôntico promoveram aumento da permeabilidade dentinária radicular de dentes permanentes e que não houve diferenças quanto à permeabilidade dentinária, quando comparada a irrigação realizada manualmente ou através de vibração ultrasônica. Não há estudos relacionados à permeabilidade radicular de dentes decíduos, quando estes são submetidos a substâncias irrigadoras e diferentes métodos de irrigação. A técnica de irrigação manual foi considerada mais efetiva do que a técnica de irrigação através de sistema de vibração ultrasônica, quanto à permeabilidade dentinária radicular de dentes decíduos. A presença da smear layer nas paredes radiculares foi um fator limitante à penetração do corante (índice de permeabilidade) para o terço médio quando dentes decíduos foram irrigados manualmente. O líquido de Dakin apresentou os maiores valores de índice de permeabilidade, nos três terços radiculares, sugerindo que essa solução irrigadora seja indicada para o tratamento endodôntico de dentes decíduos.
Abstract: One of the main goals of root canal treatment is the cleaning of the entire root canal system, through removal of pulpal debris, smear layer and smear plugs in order to avoid harbor bacteria or bacterial by-products, providing a reservoir of potential irritants in treated root canal. In addition, the cleansers and irrigation methods used in the endodontic treatment, can also contribute with the smear layer removal and with permeability alterations. Thus, the aims of this Thesis was: (1) to discuss the efficacy of some cleansers, as well as the influence of two irrigation systems used during endodontic preparation of primary and permanent teeth (manual or ultrasonic vibration) with regard to root dentin permeability, based on a literature reviewing; (2) to correlate the dye permeability to morphological aspect (presence or absence of smear layer) of the primary root dentin wall, using scanning electron microscopy (SEM), regarding the endodontic preparation and irrigation methods. Based on literature review (1) it can be concluded that among the solutions used for permanent teeth, all solutions and associations studied increased the dentin permeability. There was no difference in the dentin permeability when comparing manual and ultrasonic irrigation. There are not enough papers focusing primary teeth on this subject to definitely establish the patterns of increased dentin permeability in tooth root canal treatment regarding the irrigating solutions and irrigation systems. The results of the second study showed that manual irrigation technique was better than ultrasonic one and that Dakin's liquid, Dakin's liquid associated with hydrogen peroxide, and saline solution showed the highest permeability index averages values. Regarding the correlation study, it could be concluded that the smear layer presence on the root dentin walls was a limiting factor to dye penetration (Permeability Index), in middle third for manual irrigation.

Full textElectronic foramen locators are becoming more popular in the endodontic therapy for the determination of the working length. The working length delimits the deepest point in the root canal that instruments may reach during root canal treatment, and its accurate determination is a key factor for the success of endodontic therapy. The radiographic method is traditionally used in this surgical step, but electronic methods have proved to be faster, more precise and safer. Several electronic methods have been proposed and studies assessing the accuracy of these devices were published. The evaluation of the impedance between the two test probes used by the electronic devices, the endodontic file and the hook shape electrode that is attached to the oral mucosa membrane, has also been the subject of several studies. However, there is a lack of in vivo evaluations of the root canal impedance in the literature. The present work tries to fulfill this gap with in vivo investigative studies of the root canal impedance. The results of these studies were used to model the frequency response of the root canal impedance and to find equivalent circuits that represent the inclination of the frequency response in logarithmic scale. They were also employed in the development of an innovative electronic foramen locator based on the spectral attenuation coefficient of the root canal impedance. With the prototype used in the in vivo characterization of the root canal impedance, another study was conducted to assess the ratio method, which is one of the electronic techniques used to locate the apical foramen.

Os sistemas de único instrumento usados em cinemática reciprocante e novos instrumentos com tratamento térmico das ligas devem ser estudados quanto ao efeito sobre o preparo do canal e dentina radicular. A anatomia do canal, as técnicas obturadoras e o emprego de diferentes materiais podem influenciar na qualidade da obturação. O objetivo do estudo foi avaliar o preparo de canais achatados de incisivos inferiores e canais mesiais de molares inferiores com instrumento rotatório e/ou reciprocante, bem como a qualidade da obturação usando diferentes técnicas ou cimento de silicato de cálcio. Noventa incisivos inferiores apresentando canais achatados foram preparados com sistema Mtwo em rotação contínua (Mtwo rot) até 40/.06; Mtwo 40/.06 instrumento único em cinemática reciprocante horária (Mtwo rec) e Reciproc (R40) (n=30). Os dentes foram escaneados por microtomografia computadorizada (Micro-CT) antes e após o preparo. Abstract: The nickel-titanium instruments used during the preparation of root canals can present different treatment processes in alloy during manufacturing. Mechanized instrumentation systems present instruments with individual formats, being suggested various kinematics during the action on the dentin. Single instrument systems used in reciprocating kinematics and the new instruments with thermal treatment of alloys should be studied as to the effect on the preparation of the root canal and dentin. The anatomy of the root canal, the filling techniques and the use of different materials can influence in the quality of filling. The objective of this study was to evaluate the flattened root canal preparation of mandibular incisors and mesial root canals mandibular molars using rotatory and/or reciprocating instruments, as well the quality of root canal obturation using different techniques or calcium silicate sealer. Ninety mandibular incisors presenting flattened canals were prepared using Mtwo system in continuous rotation (Mtwo rot) up to 40/.06; Mtwo 40/.06 single instrument in reciprocating motion (Mtwo rec) and Reciproc (R40) (n = 30). The teeth were scanned by computed microtomography (Micro-CT) before and after the preparation. Data were expressed as a percentage of the increase in volume of root canal (% V), reduction of the thickness dentin (% DTR), uninstrumented surface area (% S) and remaining debris (% D). Resumo: Os objetivos do presente estudo foram analisar a microbiota endodôntica em dentes que se apresentavam com necrose pulpar e abscesso periapical; investigar a presença de enterococos, fungos e enterobactérias na saliva e nos canais radiculares; e, determinar in vitro a suscetibilidade antimicrobiana de bactérias anaeróbias estritas isoladas com método de E-test e produção de _-Lactamases. Coletas microbiológicas de 30 canais radiculares e 30 amostras de saliva dos mesmos pacientes foram processadas e identificadas empregando-se métodos de cultura microbiana e testes bioquímicos. Análise estatística foi realizada através dos testes Qui-quadrado de Pearson ou de Fisher. Dos 159 microrganismos isolados nos canais radiculares, predominaram anaeróbios estritos e bactérias Gram-positivas, sendo Peptostreptococcus micros a espécie mais freqüentemente isolada. Fungos, enterobactérias e enterococos foram pouco isolados dos canais radiculares, sendo mais freqüentemente observados em amostras de saliva. Associações de espécies e sinais e sintomas de origem endodôntica foram encontrados. Penicilina G, amoxicilina, amoxicilina + ácido clavulânico, metronidazol foram bastante efetivos frente aos microrganismos testados. Todas as cepas de P. buccae, P. intermedia/nigrescens, P. disiens, P. micros e P. propionicum foram sensíveis à amoxicilina e ao metronidazol. F. nucleatum demonstrou ser bastante resistente aos antibióticos lactâmicos. Observou-se proporção variável de cepas capazes de produzir lactamases. Os resultados sugerem que a infecção endodôntica primária associada a casos sintomáticos é mista, com predomínio de anaeróbios estritos. A presença de fungos, enterococos e enterobactérias não foi significativa tanto nos canais radiculares quanto nas amostras de saliva. Os antibióticos testados mostraram-se efetivos, no entanto resistência microbiana foi detectada em taxas variáveis de acordo com a espécie estudada.
Abstract: The aim of the present study was to analyze the endodontic microbiota of teeth with pulp necrosis and spontaneous pain; detect the presence of enterococci, fungi and enteric bacteria in saliva and root canals; and, to determine, in vitro the antimicrobial sensitivity of anaerobic isolates by E-test and _-lactamasis production. Thirty samples were collected from both root canals and saliva, and processed using strict microbiological techniques. Statistical analysis was performed with Persons X2 test or Fisher's Exact Test, as appropriated. A hundred and fifty nine strains were isolated from the root canals, with a predominance of strict anaerobes and gram-positive bacteria, and Peptostreptococcus micros was the most frequently isolated species. Fungi, enterobacteria and enterococci were seldom found in root canals. There were no Enterobacteria in both places. Positive associations between specific species and signs and symptoms of endodontic origin were present. Penicillin G, amoxicillin, amoxicilin + clavulanate, and metronidazole were effective against the tested microorganisms. All strains of P. buccae, P. intermedia/nigrescens, P. disiens, P. micros e P. propionicum were susceptible to amoxicillin and metronidazole. About 12% to 33% of the isolates were able to produce beta-lactamasis. The present results suggested that the primary endodontic infection in symptomatic teeth was mixed, and was predominantly formed by strict anaerobic strains. Fungi, enterococci and enterobacteria were not significantly found in root canals and saliva. The tested antibiotics were effective, however resistance was detected in several clinical isolates.

Full textThe aim of this study was to evaluate quantitatively, in vivo, the response of apical and periapical tissues of dog\'s teeth with experimentally induced chronic periapical lesion, after endodontic treatment and root canal filling using the following sealers: EndoREZ, Real Seal and Sealapex. Non-filled root canals (intracanal dressing) were used as control. After induction of the experimental periapical lesions, the teeth were subjected to coronal access, neutralization of the necrotic content, biomechanical preparation, placement of an intracanal dressing with a calcium hydroxide paste (Calen), and root canal filling with the different endodontic sealers mentioned above. After 30 and 90 days, the animals were euthanized and the material was subjected to histotechnical processing. A quantitative microscopic analysis was performed using an Axio Imager.M1 (Zeiss) microscope coupled to an AxioCam MRc5 (Zeiss) camera. A complete description of the characteristics of the apical and periapical tissues was performed and the number of inflammatory cells, size of the periapical lesions (in mm2), presence of bone resorption and presence of sealing of the apical opening with mineralized tissue deposition was determined on microscopic images obtained from hematoxylin and eosin-stained histological sections, using the Software AxioVision Rel.4.6 (Automatic Measurements Programs Program Wizard). Data were examined for their distribution and the compared using analysis of variance (ANOVA) followed by Tukeys post-test with a 5% significance level. Presence and extension of an inflammatory infiltrate, predominantly mononuclear, beyond half of the periodontal ligament was observed for all sealers, at both 30-day and 90-day periods. There was no modulation of the inflammatory infiltrate for any of the sealers because the number of inflammatory cells was similar at both 30-day and 90-day periods (p>0.05). Bone and radicular resorption areas were found in approximately 50% of the cases at 90 days for EndoREZ and Real Seal, and in 30% of the cases at the 30-day and 90-day periods for Sealapex. The periapical lesion size was similar at both experimental periods (30 and 90 days) for all evaluated sealers. Finally, sealing of the apical opening with mineralized tissue deposition was observed at 90 days in approximately 20%, 30% and 50% of the cases for EndoREZ, Real Seal and Sealapex, respectively; and in 15% dos cases, at the 30-day period, only for Sealapex. Within the limitations of this experiment, it may be concluded that the resin-based sealers EndoREZ and Real Seal were not well tolerated by the apical and periapical tissues. Periapical tissue repair, with cementum deposition, was found in the teeth filled with Sealapex, although complete repair of the periapical lesions was not observed.

Full textNinety extracted human teeth were analysed in this work to verify the influence of citric acid application under three concentrations, EDTA-T and EDTA-C in the apical marginal sealing of root canals. Chemical and surgical root canal preparations were performed by Paiva & Antoniazzi technique, being these teeth randomly distributed in six groups. Each group had specific chemical treatment of filling the root canal for 3 minutes with one of the following substances: saline solution, 10% citric acid, 15% citric acid, 20% citric acid, 17% EDTA-T and 17% EDTA-C. All root canals were filled with gutta-percha cones and N-Rickert cement by the multiple cone technique with vertical condensation. A crescent increase of methylene blue dye penetration could be observed in the root canals treated with citric acid equivalent to the increase in acid concentration. The highest penetration mean was shown in the root canals treated with EDTA-C and the lowest in those treated wint saline solution or EDTA-T. The differences between the apical penetration means in the six experimental groups that had been together analysed showed no statistical significance. When individually compared, there was a significant difference (p<0,05) between group G6 (EDTA-C), group G5 (EDTA-T) and group G1 (saline solution).

Full textIntroduction: The evolution of NiTi instruments can be divided in generations, according to changes like in the NiTi alloy, heat treatment, cross-section format, and in the movement used for root preparation that goes form continuous rotation to asymmetric oscillation. These changes aim to provide greater flexibility to the instruments, allowing a better shaping, with less transportation and risk of perforation. Other aspect is the increased fracture resistance of the alloys with heat treatment, which is a big concern about NiTi instruments. Aim: This study compared the cyclic fatigue and the canal transportation promotedby TF Adaptive and Navigator EVO systems when used with two different motions. Methods: Forty mesiobuccal roots of maxillary molars were scanned using micro-CT imaging before and after root canal preparation with the two instrument systems two motions (adaptive and continuous rotation). Samples were divided into four groups: TFA – TF Adaptive instruments under adaptive motion; TFC - TF Adaptive instruments under continuous motion; NA - Navigator instruments under adaptive motion; and NC – Navigator instruments under continuous motion. Root canals were prepared until 35.04 instruments. Apical transportation, centering ability and volume were analyzed using micro- CT at three levels: 3, 6 and 9 mm from the apex. The cyclic fatigue tests were performed using a custom-made device. Ten instruments of each brand were activated by using a 6:1 reduction handpiece powered by a torque-controlled motor using the preset programs ‘‘custom mode'' and ‘‘TF Adaptive'' to activate25.06 and 35.04 instruments. Data were analyzed with Kruskal-Wallis and Dunn tests were used to assess canal transportation, centering ability and canal volume. Student-T test was used to evaluate cyclic fatigue (P = .05). Conclusions: Both systems can be used under adaptive or continuous rotation. However; the lifespan of the instruments was higher when used under continuous rotation. Little canal transportation occurred when mesiobuccal root canals from maxillary molars were prepared until 35.04instruments.

Resumo: Este estudo analisou a interface de união e a força de adesão dos pinos de fibra de vidro Rebilda Post (RP) (VOCO) e Rebilda Post GT (GT) (VOCO) após cimentação em dentes com Reabsorção Radicular Interna (RRI) simulada. Quarenta e cinco raízes de pré-molares inferiores humanos foram padronizadas em 16 mm e incluídas em resina acrílica. Os canais radiculares (CR) foram preparados biomecanicamente, as raízes foram seccionadas em sua metade e a RRI foi simulada. Os segmentos foram retornados à posição original com auxílio de parafusos. Uma pasta de Ca(OH)2 permaneceu como MIC por 15 dias. A remoção foi feita por PUI e a qualidade da limpeza foi avaliada por estereomicroscopia. Os CR das amostras foram obturados e o preparo foi feito de forma padronizada. Os espécimes foram divididos em 2 grupos (n=20), de acordo com o tipo de pino cimentado. A resistência de união foi avaliada aos níveis cervical e da RRI pelo teste push-out. Para a análise estatística só foram incluídas falhas adesivas e predominantemente adesivas. Os resultados foram submetidos ao teste de Kruskal-Wallis, Mann Whitney e teste de Dunn (α = 0,05). Foi realizada MEV para análise da interface de união entre cimento e dentina radicular (n=1) e análise por microtomografia computadorizada (micro-CT) do pino GT (n=3). PUI não foi capaz de limpar completamente a maioria das RRI (62,5%). Houve alteração da resistência adesiva entre todos os grupos (p <0,05). ABSTRACT : This study analyzed the adhesion interface and bond strength of Rebilda Post (RP) (VOCO) and Rebilda Post GT (GT) (VOCO) glass fibers after cementation in teeth with simulated Internal Root Resorption (IRR). Forty-five roots of lower human premolars were standardized at 16 mm and included in acrylic resin.The root canals (RC) were prepared biomechanically, the roots were sectioned and the IRR was simulated.The segments were returned to original position with the aid of screws. A Ca(OH)2 paste remained as MIC for 15 days.The removal was done by PUI and the quality of the cleaning was evaluated by stereomicroscopy. The RC of the samples were filled and the preparation was done in standardized way. The specimens were divided into 2 groups (n=20), according to the type of cemented post. The bond strength was evaluated at the cervical and RRI levels by the push-out test. For the statistical analysis only adhesive and predominantly adhesive failures were included. The results were submitted to Kruskal-Wallis test, Mann Whitney test and Dunn test (α =.05). SEM was used to analyze the bond interface between cement and dentin root (n=1) and Microcomputed tomography (micro-CT) analysis of the GT post (n=3). PUI was not able to clear completely the majority of RRI (62.5%). There was alteration of the adhesive resistance between all groups (p <.05). The values of bond strength for both groups in the cervical region were higher in relation to the groups in the RRI region.The SEM showed formation of resin tags at the bond interface between resin cement and root dentin for both groups. In micro-CT analysis, the presence of bubbles in the cement and spaces in the bond interface between cement are visible. The adhesion of both posts was affected negatively by the level of cementation, post adaptation in the root canal and presence of IRR. Full textThe aim of this ex vivo study was to evaluate the chemical and morphological changes in root dentin treated with different high power lasers. Fifty maxillary canines were selected and had their crowns sectioned transversely and the root canals prepared with NiTi rotary instruments to the instrument # 50.02.After biomechanical preparation, the teeth were distributed into five groups (n=10) according to the surface treatment: GI - water, GII - NaOCl + EDTA, GIII - NaOCl + EDTA + Diode (980nm), GIV - NaOCl + EDTA + Nd:YAG (1064nm) and GV - NaOCl + EDTA + Er,Cr:YSGG (2780nm). The groups subjected to laser treatment were irradiated for 20 seconds by performing helical motion along the rootwith a speed of approximately 1.5 mm/s. Samples were bisected, exposing the treated surface and were subjected to the analysis of organic and inorganic composition by Raman spectroscopy (cts), elements quantification by energy-dispersive x-ray spectroscopy (EDS) (%), and had the morphology evaluated by scanning electron microscope (SEM).Data were submitted to statistical analysis (ANOVA, p <0.05), considering the variables surface treatment and root third. None of the evaluated surface treatments alter the mineral peak intensity (cts) (p = 0.183).The group irradiated with Er,Cr:YSGG significantly reduced the intensity of collagen peak (cts) (290.7 ± 41.7) compared with the watertreated group (328.3 ± 63.5) and the group treated with NaOCl and EDTA (333.9 ± 55.8) (p <0.05). The group irradiated with Er,Cr:YSGG also showed higher mineral/collagen ratio (9.5 ± 1.1)compared to the group treated with water (7.7 ± 1.5), NaOCl and EDTA (8.0 ± 1.4) and diode 980 nm (8.2 ± 1.6). Both collagen and mineral peak intensities increased from cervical to apical root thirds (p<0.05)in all groups evaluated, resulting in lower mineral/collagen ratio to the cervical third when compared to middle and apical thirds. EDS analysis found no difference between the chemical elements as well as the Ca/P ratio between the studied groups (p <0.05). SEM showed that the group irradiated with 980 nm diode had an amorphous organic matrix surface, while the Er,Cr:YSGG provided greater removal ofintertubular dentin forming craters, and the group irradiated with Nd:YAG promoted fusion of dentin, sealing the dentinal tubules. It is concluded that none of the surface treatments evaluated were able to promote changes in the inorganic content of the root dentin; treatment with NaOCl, EDTA, and irradiated withEr,Cr:YSGG alter the collagenstructure. Full textIntroduction: The root canal system has a complex anatomy with main root canal and many lateral, secondary, accessory canals, apical isthmuses and deltas, as well as numerous branched dentinal tubules. Due to its complexity, the root canal system cannot be completely cleaned, shaped and disinfected using available techniques of instrumentation, irrigation and intracanal medication. The aim of root canal filling is to achieve a three-dimensional obturation of the root canal space, entomb residual bacteria and prevent their penetration into the periradicular tissue. Defining and understanding the rheological properties of endodontic sealers in different conditions contributes to the proper handling of the material according to the obturation technique applied. The aim: testing the viscoelastic properties of three different endodontic sealers, including the flow / sealer penetration into dentinal tubules in combination with different obturation techniques. Materials and Methods: The total sample consisted of 132 mesiobuccal and mesiolingual root canals of first mandibular molars. After root canal preparation using ProTaper endodontic instruments, in crown-down manner, the sample is divided into three groups depending on the tested sealers (AH Plus, Sealapex and EndoREZ) and additionally into four subgroups according to the obturation technique applied (cold lateral compaction, single-cone technique, carrier – based obturation and warm vertical compaction), making a total of 12 groups of 11 canals. After root canal obturation, the samples were sectioned horizontally at 3, 5 and 8mm from the anatomical foramen, in order to get the sections that correspond to the apical, middle and coronary third of the canal. The coronal surfaces of the sections were analyzed using scanning electron microscopy (SEM). The maximum depth of sealer penetration was measured and expressed in micrometers and as a percent of the total distance: canal dentinal wall - the outer surface of the root. The viscoelastic properties of endodontic sealers were measured using frequency sweep test on dynamic oscillatory rheometer, at four different temperatures: 25°C, 35°C, 40°C and 65°C. Results: Obturation techniques with AH Plus that applied heat and pressure produced a significantly higher depth of sealer penetration into dentinal tubules as compared to techniques that did not apply. The application of heat and pressure during obturation with Sealapex had no a significant role in the depth of sealer penetration into dentinal tubules. Obturation techniques with EndoREZ that did not apply heat and pressure produced a significantly higher depth of sealer penetration into dentinal tubules as compared to techniques that applied. The depth of penetration of all three endodontic sealers into dentinal tubules was the highest in the coronary third, followed by the second and was significantly lower in the apical third of the root canal. AH Plus, Sealapex and EndoREZ exhibited viscoelastic character with dominant values of elastic over viscous modulus, at all specified temperatures, in the range of applied frequencies and shear stresses. Complex viscosity of all sealers decreased as the frequency increased at all temperatures. AH Plus showed decreased viscosity as the temperature increased, while Sealapex and EndoREZ demonstrated opposite behavior. Correlation analysis has established a strong negative correlation between the complex viscosity of the tested sealers and the depth of penetration into dentinal tubules: the decrease of complex viscosity increase the depth of sealer penetration into dentinal tubules. Conclusion: The choice of endodontic sealer should be based on the obturation technique applied.

Full textA morfologia da raiz dos incisivos inferiores possui um sulco longitudinal mesial e distal que pode condicionar a bifurcação de seu canal radicular. A proposta neste trabalho é estudar in vivo e in vitro a incidência de bifurcação do canal radicular dos incisivos inferiores com a utilização do radiovisiography. No estudo in vivo analisamos quatrocentos dentes, sendo realizadas radiovisiografias ortorradiais da região de incisivos e da região de caninos, onde visualizamos os incisivos com desvio de 20° distalmente. No estudo in vitro foram utilizados duzentos incisivos inferiores posicionados em um modelo, simulando o arco dental inferior, sendo radiovisiografados no sentido vestíbulo-lingual e posteriormente no sentido mésio-distal. As radiovisiografias foram visualizadas no monitor do aparelho onde analisamos a presença de canal único, canal bifurcado e canal com características indicativas de bifurcação, que correspondem à diminuição da radiolucidez cérvico-apical e à presença de linhas radiolúcidas dispostas longitudinalmente à raiz. Os resultados mostraram que 17,5% dos dentes do estudo in vivo e 15% dos dentes do estudo in vitro, que foram radiovisiografados no sentido vestíbulo-lingual, apresentaram bifurcação ou indicativos de bifurcação. No estudo in vitro, no sentido mésio-distal, encontramos 20% de bifurcação. Concluímos que a utilização da técnica de distalização do feixe de raios-X permite detectar grande percentagem dos casos de canais radiculares bifurcados ou com indicativos de bifurcação dos incisivos inferiores com a utilização do radiovisiography
The morphology of the root of the inferior incisors have a longitudinal, meslal and distal furrow that can condition the bifurcation of its root canal. The objetive of this work is to study in vivo and in vitro the incidence of the bifurcation of the root canal of the inferior incisors using radiovisiography. On the in vivo study, four hundred teeth were analyzed and orthoradial radiovisiographles of the incisors region and canines region were done where incisors with a 20º distal deviation could be seen. On the in vitro study, two hundred inferior incisors were used positioned in a mould, simulating the inferior dental arch and were radiovisiographed on the bucco-lingual direction and later, on the mesio-distal direction. The raradiovisiographies were visualized on the device monitor where the presence of an only canal, a bifurcated canal and a canal with bifurcation characteristics were analyzed. They correspond to the decrease in radiolucity and to the presence of radiolucidity lines disposed longitudinally to the root. The results show that 17,5% of the teeth on the in vivo study and 15% of the teeth on the in vitro study that were radiovisiographed on the bucco-lingual direction presented bifurcation or signs of bifurcation. On the in vitro study, on the mesiodistal direction, we found 20% of bifurcation. We can conclude that the use of the dislalization of the X-rays shaft technique allows us to delect a great percentage of cases of bifurcated root canals or cases with signs of bifurcation of the inferior incisors with the use of radiovisiography
