Abstract:
Due to the increase in dual-income families, work-family conflict has become a more prevalent phenomenon in today's society. Home healthcare workers have been previously identified as an employment group that is susceptible to high levels of burnout and low levels of job satisfaction, yet work-family conflict concerns have yet to be examined. Particularly because of the great deal of care being provided within a home, both at work and in life, this population is of particular interest for examining work-family conflict. The purpose of this study is to empirically investigate the relationships among work-family conflict, job satisfaction, affectivity, and burnout within a sample of rural, home healthcare employees. More specifically, four distinct models are proposed which include the following variables: positive and negative affectivity, number of hours providing care for others outside of work, number of hours worked per week, family-interference with work conflict, work-interference with family conflict, job satisfaction and three facets of burnout (personal accomplishment, depersonalization, and emotional exhaustion). While models predicting job satisfaction and emotional exhaustion accounted for the most variance, all four models provided information regarding the direct, indirect and mediating relationships of the aforementioned variables. More specifically, the findings suggest that the two types of work-family conflict uniquely mediate the proposed outcome variables highlighting the importance of examining work-family conflict from a more refined perspective. Exploratory group differences are also examined. This study contributes to a gap in the literature examining individuals' experiences of work-family conflict, job satisfaction, and burnout who are employed in a specific career field. Practical, research, and theoretical implications are discussed.

Abstract:
Home visits are a required training component of many Family Medicine residency programs in the United States. However, they are becoming less popular due to such factors as increasing resident responsibilities, decreasing reimbursement, and a decline in resident intention to incorporate home visits into future practice. This study’s aims are: (1) to evaluate the current practices of one Family Medicine residency training program’s time and resource expenditure to conduct home visits, and (2) to evaluate resident and faculty experiences of home visits. Residents and faculty in a Family Medicine training program were provided with a 12- question survey immediately after completing a home visit. A total of 19 surveys from residents and faculty were collected and analyzed. Average reported time spent per home visit was 90 minutes (range = 50-180 minutes), and the home visit teams included an average of 4 members (range = 2-6 members). The providers felt that they knew their patients and the patients’ circumstances better after the home visit with a score of 4.1 (on a 1-5 scale with 5 being a positively framed statement). Resident opinions were neutral (average score 3.1 on a 1-5 scale) regarding whether they found home visits to be educational to their residency training in Family Medicine. Residents also had mixed feelings (average score 2.9) regarding whether they would perform more home visits during their residency training if given the opportunity. Most faculty members (5/7) indicated they had done home visits during their residency training and all faculty (7/7) felt that home visits added value to their training in Family Medicine. Finally, qualitative recommendations were collected from respondents which may allow this training program to improve home visits in the future. Overall, significant time is currently being spent conducting home visits, with a difference in perceived efficacy between residents and faculty. Future research may include a cost analysis to quantify financial value, as well as expanding data collection to other Family Medicine residency training programs to improve generalizability.

Abstract:
The intent of this thesis was to write a memoir of my five month trip to Libya that explores cultural differences through my experiences as an American with Western ideals. This memoir is focused on the cultural norms of marriage in the rural town of Msalata, in the central rural farming belt north of the ever expanding Sahara Desert of North Africa. My goal was to produce a work that is informational while showing the humanity of the local people through my perceptions as an outsider with different expectations. It was a time of discovery for me about the value of my upbringing and the positive aspects of American and Libyan culture. Our five months in Libya proved our strength and weakness. Libya was not what I expected. The people were hospitable beyond my experience. The customs at times were primitive and required an open mind. My children and I were the token Americans that summer who were invited to every wedding and birth. I was expected to attend many social events from circumcision celebrations to giving condolences along the side of my brother-in-law’s wife. Due to my American Christian upbringing I shared the moral values of Islam, which made it easy for me to become Muslim and live an Islamic life. At the same time, I could not fully accept all aspects of Libyan culture nor did my husband. Hadi rejected many things about his culture because it conflicted with Islam. My thesis did not come out the way I expected. It took a different direction from what I had original planned. It became focused on wedding traditions rather than on broader cultural contrasts.

Abstract:
Families of children with special health care needs (CSHCN) and disabilities who lived in rural communities faced a variety of economic, social, and environmental challenges. Bronfenbrenner (1979, 2005) in his Bioecological Theory of Human Development offered an insightful lens for understanding the nested environments in which these families interact. This model was used as the overarching framework for this dissertation. The three manuscripts contained in this dissertation have included analyses of the involvement and participation of families of CSHCN and disabilities in the creation of a medical home located in a rural southwestern border community. These studies were critical for the medical home professionals - family involvement was at the core of the medical home philosophy. The overarching purpose of this dissertation was to set the foundation for successful family participation and feedback in the medical home. Within the first manuscript, I used the Medical Home Family Index to discover families’ (a) perceptions of interactions with medical professionals and (b) ratings of the quality of care within the medical home. Families of CSHCN (N = 92) completed the Medical Home Family Index and descriptive statistics as well as Chi-Square analyses were completed. Significant associations between families’ home languages and the amount of time the CSHCN had been receiving services at the medical home and the families’ responses were found. No associations between the children’s ages and the families’ responses were found. Recommendations for medical home professionals to complete the partner index, the Medical Home Index, were included. The purpose of the second manuscript was to examine the support needs of families of CSHCN and disabilities. Relationships between the severity of the children’s special health care needs and disabilities and the potential services they required were explored. For this study, a small sample (N = 25) of families of CSHCN completed the Family Needs section of the Center for Medical Home Improvement Family Survey, an in-depth, five-part survey designed to have families report on the services and supports that their CSHCN actually received. Families of CSHCN identified the need for therapies to be provided within the community. Findings from this study supported the creation of a pilot program in which parents were active participants in an intensive summer program designed to address the language and communication needs of their children. The purposes of the third manuscript were to (a) conduct research in two settings - a rural medical home and the families’ natural environments, (b) identify families’ perceptions of a targeted summer language intervention program, and (c) determine what changes in young children’s communication skills could be measured when parents were active members in a targeted summer language intervention program. For the third study, fourteen children with language delays and their families participated. Children were assessed using the Battelle Developmental Inventory-2nd Edition or the Preschool Language Scale-4th Edition. Intervention was conducted within a pediatric medical home and families continued the intervention at home. For both test results, the treatment had a significant effect. All families indicated a strong desire to continue the program and families who were most concerned with their children’s language were most satisfied with the program. This pilot program model was an example of ways that interventions could be extended successfully beyond traditional settings.

Abstract:
Standard economic models often emphasize inputs, outputs and an examination of the structures in order to conduct an economic performance evaluation. This study applies the Institutional and Development Framework (IAD) in the broader context of New Institutional Economics (NIE) in order to examine the transaction costs of delivering Community and Home Based Care and Support (CHBC) to HIV/AIDS sufferers. For purposes of unveiling the empirical reality guiding decision making processes in the CHBC service delivery, comparative qualitative research techniques of normative variable and concept formation have been adopted to draw out the relative institutional influences from the HIV/AIDS national response partnerships. The study identifies the conflict between the predominantly standardized and more rigid formal management techniques adopted by key members of the national response and the informal cultural techniques familiar to the rural communities, and a lack of motivational incentives in the CHBC structures as the key factors against CHBC capacities to draw external funding for service delivery. CHBCs are also weakened by incoherent governance structures at the district level for facilitation of funding and information flow exacerbating the community vulnerability. Rationalization of the institutional arrangements and a clarification of roles from district to community levels, a shift of focus to facilitation of informal techniques and an integration of performance enhancing incentives are the critical policy insights envisaged to spur CHBCs to work better.

This work presents the relation between permanence in the field and the qualification of family farmers. In order to have this permanence and continuety in rural activity, the qualification allows, while social construction, an interaction between the elements involved, namely, the environment and people. Qualify means not only prepare them to cope with new technologies, but, educate them, teach them how to search for knowledge, in order to improve their action in activity and the environment in which they live. For the remaining it is needed the use of the qualification of the young farmer, to aggregate value to their activities, clearly and with the knowledge. An instrument for this knowledge occurs, is the Rural Family House (Casa familiar Rural) a helper in the decision to remain, the theoretical knowledge taught in addition to the practice helps the young farmer to learn accordingly to his own reality. In this way, to continue their activity, it is necessary, to learn and chase the activity diversification. For this learning becomes a qualification, the young farmer needs to be always constantly novitiate, that is, be looking for knowledge that promotes the activity and prepares for the work in rural property. With the qualification, the permanence becomes an activity able to bring benefits for the young farmers and also for their families, as well as enhances the family agriculture, therefore, the sector is responsible for food security in the country, that is, the basic food consumed by Brazilians. Denoting with it, the family farming shows a capacity of generation of income, and represents 75% of hand-to-work used in the field. Therefore, for this analysis, family farming, the permanence of the young farmer in the field and its relationship with the qualification, reflects the reality of the process.
O presente trabalho apresenta a relação permanência no campo e qualificação de agricultores familiares. Para que haja esta permanência e continuar na atividade rural, a qualificação possibilita, enquanto construção social, uma interação entre os elementos envolvidos, ou seja, ambiente e pessoas. Qualificar significa não apenas prepará-lo para lidar com as novas tecnologias, mas, educá-lo, ensinando-o, a aprender a buscar o conhecimento, a fim de melhorar sua atuação na atividade e no meio em que vive. Para a permanência faz se uso da qualificação do jovem agricultor, no sentido de agregar valor às suas atividades, de forma clara e com conhecimento. Um instrumento para que este conhecimento ocorra, é a Casa Familiar Rural uma auxiliadora na decisão de permanecer, pois além dos conhecimentos teóricos ensinados, a prática dos mesmos, favorece um aprendizado condizente com a realidade que o jovem agricultor presencia. Dessa forma, para continuar na atividade, faz-se necessário, aprender e buscar a diversificação da atividade. Para que este aprendizado se torne uma qualificação, o jovem agricultor precisa estar sempre em constante aprendizado, ou seja, estar buscando conhecimentos que alavanquem a atividade e o preparem para o trabalho que desenvolvem na propriedade rural. Com a qualificação, a permanência se torna uma atividade capaz de trazer benefícios para o jovem agricultor e também para a sua família, bem como valoriza a agricultura familiar, pois, o setor é responsável pela segurança alimentar do País, ou seja, dos alimentos da cesta básica consumidos pelos brasileiros. Denotando com isso, que a agricultura familiar apresenta capacidade de geração de renda, e representa 75% da mão-de-obra utilizada no campo. Portanto, para esta análise, a agricultura familiar, a permanência do jovem agricultor no campo e sua relação com a qualificação, traduz a realidade do processo.

Losing a spouse is as devastating an experience anyone will ever deal with in his or her life. Research, however, shows that men have a harder and longer time coping with the loss of a spouse than women. When the widower’s profession is farming, there are no resources to specifically help that individual with their loss. The purpose of this research was to gain insight into the lives of widowed farmers with particular focus on transitions in their farming operation and their family life. Through their stories, we learn what is happening before the loss of the female spouse on the farm, during the bereavement period, and how the farmer handles the situation after the bereavement period has ended. Findings indicate that the Cooperative Extension Service can play a major role in supporting our widowed farmers as they seek a new normal for their farm operation and family life. Also, farming widowers have more stress following the loss of a spouse due to the added aspect of managing the farm and the home simultaneously after the passing of their spouse. Recommendations include further expanding spousal bereavement materials to include a section for males and in particular farming widowers.

Abstract:
This thesis gives an experiential account of notions of the home in contemporary rural China. Based on a year of fieldwork in a mountain village in rural Shanxi Province, the thesis explores everyday and ritual practices to investigate how people make themselves at home under conditions of political economic transformation. Villagers accommodate and resist conflicts of interest by negotiating boundaries of insiders and outsiders through the home. Differences of gender and generation come to the fore as people compromise between aspiration and pragmatism within the home under conditions of resurgent market competition. The theoretical concern of the thesis lies in connecting wider social processes to personal life projects through the intimate sphere of the home. The rhythm of the seasons patterns the thesis into spring, summer, autumn and winter chapters, as the seasons were pivotal in ordering people’s everyday practices and ritual activities within a shared social and ecological environment. The opening chapter on the autumn harvest coincided with my arrival in the village. The chapter explores how labour, and particularly women’s labour, transforms the earth into affective belonging, and how women negotiate conflicts over food consumption between the agricultural and market economy. The winter chapter parallels tales of personal life history with wider kinship networks across various generations, while simultaneously tracing bodily pathways from the domain of the hot stove in the home to the cold grave in the fields. The next chapter begins with the celebratory periods of springtime during the New Year Festival, a time of ritual renewal in the home when women partook in a local domestic ritual of propitiating the little spirits of the house. At Qingming Festival villagers’ practices of worshipping the ancestors in the fields were juxtaposed with a tour company’s staging of an elaborate ritual revival of star worship in the village. Conflicting aspirations over the future of the past thereby tore fissures into the emerging ritual terrain between outside spectacle and inside convergence. The last ethnographic chapter looks at the summer as a time for regenerating life, particularly through marriage and children. Reciprocal caring cycles between different generations of women are central to balancing domestic and occupational aspirations in negotiation with the local implementation of the family planning policy. House-based rituals at children’s birthday parties and bridal farewell ceremonies formally celebrate the roles of matrilateral relatives.

The valuation of family farming by Brazilian society constitutes an important tool for local development, since it uses scarce resources like land, labor force, efficiently. The rural homegardens are important production systems to complement other forms of land use, which besides providing food security for farmers, they perform functions aesthetic, recreational and conservation of local biodiversity. Thus, the study aimed to examine the total economic value (TEV) of the backyards of the universe of rural farmers involved in the Cooperative Family Farming of Itapuranga, noting its main economic functions, environmental and cultural. By means of the contingent valuation method, using the technique "payment card", tried to capture also the values of these non-market yards, as the perception and preferences of the farmers interviewed. Thus, 40 questionnaires were applied and visited 35 farms, where it was possible to observe directly the interaction of families with that environment. For economic functions, there was a survey of the main items (gross) consumed and sold during October 2007 and October 2008, reaching an average value of direct use of R$ 420.70 monthly. Other benefits from the yards were measured by DAP of the farmers interviewed, obtained by the MAC, with an average value of R $ 493.75 monthly. These values also correspond to the sociocultural aspects related to the tradition of those families where the woman responds by management and by the choices of crops, according to the tastes and habits of the family group. Regarding environmental aspects, the yards are considered important agroecological systems, since in most cases are not used chemical or toxic substances in their production. In recent years, faced with environmental problems in general, and the reopening of the sugarcane agro-industry in the city of Itapuranga, so does the importance of gardens as conservative biodiversity and balance of the fauna and flora.
A valorização da agricultura familiar pela sociedade brasileira constitui-se uma importante ferramenta de desenvolvimento local, uma vez que esta utiliza os recursos escassos, como a terra e a mão-de-obra, de forma eficiente. Os quintais rurais são importantes sistemas de produção complementar às outras formas de utilização da terra, que além de possibilitar a segurança alimentar dos agricultores familiares, desempenham funções estéticas, de lazer e de preservação da biodiversidade local. Dessa forma, o trabalho buscou analisar o valor econômico total (VET) dos quintais rurais do universo dos agricultores familiares associados à Cooperativa de Agricultura Familiar de Itapuranga, observando as suas principais funções econômicas, ambientais e culturais. Por meio do Método de Avaliação Contingente, utilizando a técnica cartão de pagamento , procurou-se captar também os valores de extramercado desses quintais, conforme a percepção e as preferências dos agricultores entrevistados. Com isso, foram aplicados 40 questionários e visitadas 35 propriedades rurais, onde foi possível observar diretamente a interação das famílias com esse ambiente. Para as funções econômicas, fez-se o levantamento dos principais itens (brutos) consumidos e vendidos durante outubro de 2007 e setembro de 2008, chegando-se a um valor médio de uso direto de R$ 420,70 mensais. Os demais benefícios provenientes dos quintais foram mensurados pela DAP dos agricultores entrevistados, obtida através do MAC, apresentando um valor médio de R$ 493,75 mensais. Esses valores correspondem também aos aspectos socioculturais ligados à tradição dessas famílias, onde a mulher responde pelo manejo e pelas escolhas dos cultivos, conforme os gostos e costumes do grupo familiar. Quanto aos aspectos ambientais, os quintais são considerados importantes sistemas agroecológicos, já que na maior parte das vezes não são utilizadas substâncias tóxicas ou químicas em sua produção. Nos últimos anos, frente à problemática ambiental de uma forma geral e com a reabertura da agroindústria canavieira no município de Itapuranga, cresce também a importância dos quintais enquanto conservador da biodiversidade e do equilíbrio da fauna e da flora local.

Ever since United Nations’ declaration of 1994 as the Year of the Family, the study and understanding of families has taken center stage, albeit with constant references to ‘normal’ versus ‘deviant’ families based on structural functionalism theory’s rigid definitions of what a ‘normal’ family should be. On the other hand HIV/AIDS has attracted much attention too because of its life threatening traits, especially in Sub-Saharan Africa where Zimbabwe lies. Efforts to fight the epidemic have seen the mushrooming of innovative programs, which include home–based care (HBC) services for those infected with HIV. However, although HIV and AIDS has clearly had adverse impacts on families, it is rarely discussed within perspectives that integrate family dynamics. To fill this gap this study has explored how HBC, as a response to HIV and AIDS, may change our understanding of families. Research data was gathered within the participatory action research design through methods such as focus group discussions by 35 HBC volunteers and patients, interviews of 26 of these, review of relevant family policies and other interactive participatory exercises by which research participants expressed their opinions through drawings. The evaluation showed that households were not always synonymous with families, although a thin line divided the two. Further, survival considerations are the leading priority why people find themselves in families within HBC settings. However, survival options available to women are exploitative and this has kept those in HBC settings reeling under the burden of demanding but unrewarded care work and domestic household jobs. The study teaches that families cannot be understood in aggregated terms and that individuals dictate what families become, not the other way round. The research essentially recommends policy revisions to reflect unique realities found in HBC settings, and among female HBC volunteers. This should be coupled with awareness campaigns in communities and further research on families.

Abstract:
The purpose of this study was to explore the role of family members in support of their relatives who are mentally ill and to develop guidelines for the support of mentally ill relatives by their families in a rural community. The research design was qualitative, explorative, descriptive and contextual. The research population consisted of families of mentally ill relatives collecting their monthly medications at the Jane Furse Gateway Clinic. Purposive sampling was used to draw the sample. Data was collected by individual in-depth semi-structured interviews that were tape-recorded. The study revealed that families provide physical, psychosocial, financial and developmental support to their relatives. Furthermore, families are faced with frustrations when providing support to their mentally ill relatives

Abstract:
Esta dissertação engloba duas vertentes: uma projetual e outra teórica. Na primeira, os temas foram desenvolvidos em torno do conceito de “Mundo Novo”, imagem ancorada nos pressupostos concetuais de A. Huxley (1932), onde se recolhe um referencial passível de contextualizar um futuro a médio prazo, dada a dinâmica paradoxal que opõe os pressupostos da cultura moderna e industrial à perspetiva de organização neo-ruralista. Congregados nesses fundamentos, foram problematizados fatores como a influência da organização económica e política nos modos de vida e na relação do indivíduo com a sua comunidade, a influência da tecnologia na organização social, bem como o modo como os recursos naturais poderão influenciar as ações sobre o território, a localização e a organização do espaço doméstico. A reflexão e dimensionamento desta problemática visaram a definição de um perfil social que se previsse possível, num futuro de duas décadas. Neste contexto, foram desenvolvidos, em grupo, workshops: |marca, texto e espaço; Guiné-Bissau - Bafatá; temas II,III e perfil social. Individualmente foi desenvolvido um projeto que relaciona a proposta tipológica e habitacional com o excerto urbano que caracteriza a sua envolvente próxima, através do estabelecimento da relação entre a macro e a micro escala, para Lisboa, no eixo entre o Largo do Rato e a Colina das Amoreiras. Este consistiu na elaboração de projetos para 4 habitações, contextualizadas e operacionalizadas ainda como tipologias associadas ao universo social definido. Foi desenvolvida ainda, uma proposta de reflexão individual (tema IV), que resultou na criação de uma “Caixa/projeto”, simbolicamente representativa das experiências e vivências condicionadoras da forma como o estudante de arquitetura interpreta e explicita a realidade. Na vertente teórica, desenvolveu-se a reflexão em torno da arquitetura popular, enquanto campo dotado de autonomia própria, onde se consubstancia um vasto campo de investigação e de práticas, que tornam obsoleto o senso comum e os ensaios dispersos até então existentes. O conceito de arquitetura popular como objeto de estudo permite considerar o seu inerente valor patrimonial que há que preservar e continuar a analisar. Assim, foi abordada a problemática da arquitetura habitacional rural madeirense edificada, na sua dimensão contextual, particularmente, nas freguesias de Quinta Grande e de Jardim da Serra, do Concelho de Câmara de Lobos, e a sua relação com as gentes, os costumes, o trabalho de campo e a vida doméstica, trabalho centrado no estudo das habitações, de três gerações de uma mesma família, proposta radicada nos ideias neo-ruralistas.
This dissertation consists of two parts: one theoretical and one projective. In the first, the themes were developed around the concept of “New World”, anchored in assumptions conceptual image of A. Huxley (1932), which collects a framework capable of contextualizing a future in the medium term, given the paradoxical dynamic that opposes the assumptions of modern culture and the industrial organization perspective to the neo-rural caucus. Gathered into these grounds, were problematized factors as the influence of economic and political organization in the ways of life and the individual’s relationship with its community, the influence of technology on social organization, as well as how natural resources can influence the actions of territory, the location and organization of domestic space. The reflection of this issue and sizing aimed at defining a social profile that foresee possible, a future two decades. In this context, were developed, in group workshops: | mark, text and space; Guiné-Bissau - Bafatá; themes II, III, and social profile. Individually, a project was developed that relates the proposed typological beteween urban housing with the excerpt that characterizes his immediate surroundings, by establishing the relationship between macro and micro scale, to Lisbon, the axis between Largo do Rato and Hill of Amoreiras . This consisted of developing projects for 4 dwellings, contextualized and operationalized as yet associated with the social world typologies defined. It was developed yet, a proposal for individual reflection (theme IV), which resulted in the creation of a “Box / project”, symbolically representative of the experiences and life conditioners how the architecture student interprets and explains the reality. In the theoretical model, it was developed a reflection on folk architecture as a field endowed with its own autonomy, which embodies a vast field of research and practices that make obsolete common sense and essays scattered hitherto existing. The popular concept of architecture as an object of study considers its inherent asset value that must be preserved and continue to analyze. Thus, addressed the issue of rural housing of Madeira architecture built in its contextual dimension, particularly in the parishes of Quinta Grande and Jardim da Serra, in the municipality of Câmara de Lobos, and their relationship with the people, the customs, the work field and home life, work focused on the study of housing of three generations of the same family, proposal rooted in neo-ruralists assumptions.
