Abstract:
Supported employment is an established approach to community-based employment that has provided many individuals with significant disabilities an opportunity to choose between a lifetime of low wages in segregated sheltered workshop settings and the challenges of a real job in their local community. As a service-delivery strategy that embodies the principles of individualized, community-based support services and consumer empowerment, supported employment has become the preferred employment alternative for large numbers of individuals previously excluded from work opportunities (Rusch, Chadsey-Rusch, & Johnson, 1991; Wehman & Kregel, 1994). The purpose of this article is to summarize the present status of supported employment, discuss the major accomplishments of the initiative, and identify those issues that must be addressed in the future to allow the program to achieve its long-term goal of promoting the economic independence and community integration of individuals with significant disabilities.

Acute postpartum uterine inversion is a relatively rare complication. The uterus inverts and the uterine fundus prolapses to or through the dilated cervix. It is associated with major postpartum haemorrhage with or without shock. Shock is sometimes out of proportion to the haemorrhage. Minimal maternal morbidity and mortality can be achieved when uterine inversion is promptly and aggressively managed. We present this report of three cases of acute uterine inversion complicated with major postpartum haemorrhage and managed with Rusch balloon. The paper highlights the importance of early recognition and the safety of the use of intrauterine balloon to manage major postpartum haemorrhage in these cases.

We report a case of tracheal stenosis in which a migrated Polyflex (Rusch AG, Germany) stent entered the right main bronchus but could not be identified on the chest radiograph. The stent was identified at bronchoscopy and removed under general anaesthesia.Polyflex stents are radiopaque but may not always show up on radiograph. We recommend reliance on clinical symptoms rather than imaging to diagnose migration. If stent migration is suspected then imaging should include thoracic inlet films that incorporate a lateral view.

AbstractIntractable vaginal bleeding is a complication of gynecological tumors of the cervix and endometrium. The management of torrential bleeding usually requires laparotomy or laparoscopy, or less invasive techniques such as uterine artery embolization. This case report describes the use of an intrauterine Rusch balloon catheter, previously only used in the obstetric setting. This is a safe and inexpensive method of management, which does not need specific training. We also review all the current options for the management of severe bleeding as a complication of gynecological cancers.

Background: The three-way indwelling urinary catheter (IUC) is used for continuous bladder irrigation and is considered the cornerstone for clinical treatment of patients with macroscopic hematuria. Although there seems to be a logical relationship between catheter size and efficacy of irrigation and drainage, we often observe relevant variations in these parameters between different brands of catheters available on the market. The aim of this study was to compare the mechanical properties of different models of latex and silicone three-way catheters in an in vitro setting that resembles clinical use. Methods: Three different three-way catheters were evaluated: Gold Silicone-Coated Rusch® (Model A), 100% Silicone Rusch® (Model B) and X-Flow Coloplast® (Model C). Irrigation channel, drainage channel, and overall cross-sectional areas were all digitally measured. Irrigation and drainage channel flow rates were measured and correlated with their corresponding catheter cross-sectional area values. Results: Different catheter models of the same caliber have different internal irrigation port diameters, internal drainage port diameters and internal cuff port diameters. The Model C IUC internal irrigation port diameter is significantly larger than models A and B. When flows were evaluated, we found that in the same model, the increase in caliber of the IUC was related to an increased drainage flow, but not to an increased irrigation flow. Conclusion: Precise measurements of the internal architecture of the three-way catheter, rather than relying on the caliber itself, could assist surgeons in choosing the best product for each specific patient, while minimizing complications.

It has been estimated that 10% of ruptures of thoracic aortic aneurysm present as aortoesophageal fistula. They can present with mild esophageal bleeding giving warning of a fistula formation or as torrential exsanguinating hemorrhage. It is during the window of mild esophageal bleeding where it is possible to salvage the situation with prompt diagnosis and emergency surgical treatment. Once exsanguinating hemorrhage occurs, most patients are beyond salvage. We describe a patient who has torrential hemorrhage from an aortoesophageal fistula from a ruptured thoracic aneurysm. The hemorrhage was controlled promptly by the Sengstaken-Blakemore balloon compression tube (Willy-Rusch Ag, Kernen, Germany), which allowed stabilization of the patient's condition before definitive surgery was undertaken.

Neste artigo discute-se a aquisição e a mudança da língua como uma consequência da percepção de sua materialidade. Para isso, foram considerados os modelos da family resemblance e do perceptual magnet effect propostos por Elianor Rusch, a partir de Wittgenstein, e por Patricia Kuhl respectivamente, complementados pela hipótese do motherese como estímulo. Tais propostas foram tomadas como ponto de partida para a aquisição. A mudança, por sua vez, partiu dos modelos da Teoria Matemática da Comunicação, de Claude Shannon, tal como foi reformulado por Roman Jakobson e por Niklas Luhmann. Dados do Finlandês e do Alemão foram considerados como argumentos que justificassem a mudança do Latim para o Português como decorrência da percepção segundo os modelos propostos. Com base nesses argumentos, concluiu-se que a mudança decorreria de reações cumulativas, caracterizadas como aquisição pela formação inicial de formas prototípicas que sofreriam mudanças subsequentes conforme o sistema reaja aos estímulos externos.

Major obstetric haemorrhage remains a significant cause of maternal morbidity and mortality. Previous case reports suggest the potential benefit of recombinant activated factor VII (rFVIIa: ) as a haemostatic agent. We performed a retrospective review of the use of rVIIa in major obstetric haemorrhage in the Northern Region between July 2004 and February 2007. Fifteen women received rFVIIa. The median patient age was 34 years. Major haemorrhage occurred antepartum (5 patients), intrapartum (1), and postpartum (9). All women received an initial dose of 90 mcg/kg rFVIIa and one received 2 further doses. Bleeding stopped or decreased in 12 patients (80%). Additional measures included antifibrinolytic and uterotonic agents, Rusch balloon insertion, uterine curettage/packing, and vessel embolisation. Eight patients required hysterectomy. All women survived to discharge from hospital. No adverse events, including thrombosis, were recorded. This study provides further support for the safety and efficacy of rFVIIa as adjunct therapy in major obstetric haemorrhage.

The behavior of concrete is highly nonlinear, even at very low loading levels. Steel, on the other hand, exhibits a relatively linear behavior up till yielding. The synergy between the two materials and their compatibility has long been the subject of research. While the failure criterion for steel is straight forward, concrete can be approached by various theories. The most prominent are the Kupfer-Hilsdorf-Rusch and the Möhr failure envelope. The behavior of material under bi-axial stresses subsequent to cracking can be assumed isotropic or orthotropic, resulting in a differentiation in the material constitutive matrix formulation. This work covers the finite element modeling of reinforced concrete elements, based on the two failure envelopes, while assessing the isotropic and orthotropic methodology. The Finite Element smeared crack approach is used to analyze stresses and the propagation of cracking pattern for the element. The resulting load – displacement curves are validated with identical laboratory tested specimens.

Abstract:
A new, simple, hypoelastic, and anisotropic stress–strain relationship is proposed for modeling the behaviour of concrete subjected to a general state of monotonic loading. The proposed model reduces to an incremental orthotropic model if the direction of the principal stress axes remains constant. It also eliminates the inability of the orthotropic constitutive relationships to model the behaviour of concrete under a general state of monotonic loading when the principal stress axes rotate during the loading process.The capability and reliability of the proposed model for the analysis of structures in which the principal stress direction does not rotate is verified. A finite element computer program is formulated and used for the nonlinear analysis of planar concrete specimens tested by Kupfer, Hilsdorf, and Rusch and by Tasuji and reinforced concrete panels tested by Vecchio and Collins. The results obtained from the analytical models are in good agreement with the corresponding experimental results. Abstract:
We report the successful treatment of a postpartum pyomyoma, a rare but serious complication of uterine leiomyomata in a 28-year-old primigravida. The patient was treated for anEscherichia Coli(E. Coli) urinary tract infection (UTI) at 16 weeks of gestation. She had asymptomatic short cervical length on ultrasound scan at 20 weeks that was managed conservatively due to the presence of further UTI and received antibiotics. She was known to have a left sided intramural leiomyoma. She presented with abdominal pain and vaginal bleeding at 23+1weeks of gestation and the next day she had spontaneous vaginal delivery and collapsed withE. Coliseptic shock, massive postpartum haemorrhage, and disseminated intravascular coagulation and was successfully treated with oxytocic drugs, a Rusch intrauterine balloon, and intravenous antibiotics. Eleven days postnatally she re-presented with systemic sepsis and was treated for retained products of conception. Sepsis persisted and investigations showed a postpartum pyomyoma that was initially managed with intravenous antibiotics to avoid surgery. Ultimately she required laparotomy, drainage of pyomyoma, and myomectomy. Postoperative recovery was good and the patient had a successful pregnancy two years later.
