Background: Mortality among people living with the human immunodeficiency virus (PLWH [HIV]) diagnosed with acquired immunodeficiency syndrome (AIDS)-defining cancers has greatly decreased with the advent of highly active antiretroviral therapy (HAART). Recent investigations exploring the impact of insurance coverage have concluded that PLWH with public insurance have greater mortality than their privately insured counterparts. One avenue to augment medical coverage is the Ryan White HIV/AIDS Program, first authorized by Congress in 1990 to provide primary care and support services to PLWH. Th

Abstract Background Ending the HIV Epidemic (EHE) requires prompt diagnosis and treatment of HIV to reduce transmission. Delayed HIV diagnosis and late entry into care remain challenging. Strategic deployment of testing resources may leverage both targeted and universal testing to accomplish the timely diagnosis of HIV infection. Methods We extracted data from the City of Philadelphia’s Enhanced HIV/AIDS Reporting System for 3,856 individuals diagnosed with HIV infection in Philadelphia, PA from 2012-2018, to determine characteristics associated with delayed diagnosis, defined as: AIDS diagnos

The development of highly active antiretroviral therapy (HAART) has shifted human immunodeficiency virus (HIV) and acquired immune deficiency syndrome (AIDS) from an acute to a chronic condition. Due to reduced fatality, approximately 1.1 million people living with HIV/AIDS (PLWHA) are faced with increased longevity in conjunction with functional consequences associated with chronic disability. Employment has been associated with increased treatment adherence, quality of life (QoL), and mental and physical health for people living with HIV/AIDS. The Ryan White HIV/AIDS Program (RWHAP) provides HIV-related medical and support services for uninsured and underinsured people living with HIV (PLWH) in the United States. In addition to HIV-related medical care, the program provides medical nutrition therapy and food assistance. The role of nutrition in the health of PLWH is well-documented, especially in resource poor areas; however, the role of medical nutrition therapy and food assistance provided through the RWHAP in resource rich areas is not well documented. The vulnerable populations model was utilized to identify appropriate variables for analysis as well as to provide a sequencing for the testing of models. This dissertation describes health care delivery in a Ryan White Program (RWP) HIV clinic, with a focus on medical home care, using the Bayesian Case Study Method (BCSM). The RWP funds medical care for uninsured HIV patients and Pappas and colleagues (2014) suggested enhanced HIV care build upon medical home models of care rooted in the RWP. However, little research describes how RWP clinics operate as medical homes. This study developed the BCSM to describe medical home care at a RWP clinic. The purpose of this study is to describe the organizational components and activities of three Ryan White CARE Act (RWCA) Title II consortia in Louisiana (referred to as Bayou Consortium, City Hall Consortium, and Farmstead Consortium). A historical analysis is conducted to determine how the consortia's membership characteristics and level of formalization affected their ability to successfully support services that address the needs of persons living with HIV/AIDS (PLWH). Intervening variables related to this outcome are also identified and analyzed. Drawing from the literature, the primary t

Upon completion of this chapter, the reader should be able to • Broadly understand the landscape of the US healthcare systems and payers as they relate to the provision of HIV care, treatment, prevention and medical coding. HIV-related healthcare services have historically been covered or provided by a patchwork of federal, state, and local programs, such as Medicare, Medicaid, the Ryan White Program, and state and local health programs. HIV/AIDS service organizations (H/ASOs) and private and charitable health organizations also provided services for HIV patients without coverage. These programs represent the long-standing pathways to insurance coverage, access to care, and access to treatment for people living with HIV (PLWH). Some of these programs also supply coverage for, or access to, medications that treat co-occurring conditions for PLWH....
