Recent advances in surgical and interventional procedures have led to a significant and increased demand for anesthesia services in locations distant from the traditional operating room. Special settings such as ophthalmologic surgery, interventional radiology, and the electrophysiology lab present unique challenges to the anesthesia provider. In addition to the remote location of the procedure rooms, the lack of familiarity with the equipment and distance from emergency back-up make for a challenging situation. Judicious preparation and set up of anesthesia equipment and materials as well as communication between the anesthesiologist, proceduralist, technicians, and nursing staff are key to performing these procedures in a safe fashion. Finally, procedures involving radiation exposure require awareness of occupational and patient safety concerns. This chapter discusses relevant anesthetic considerations for interventions performed in special settings including ophthalmologic surgery, gastrointestinal endoscopy, interventional radiology, cardiac diagnostic, and magnetic resonance imaging suites.

The definition of family as a conjugal group consisting of parents and children living in the same household is in the process of a profound reworking, one that includes the constellation of family life that exists around the world. Increased migration and mobility have challenged traditional notions of what constitutes a family, yet much mainstream research relies on past notions of a cohesive unit under one domicile. Many families today are separated across distance and maintain ties in a multitude of ways. And although researchers have increasingly paid attention to this new picture of the family, much of this work has focused on transnational families separated in the context of overseas economic migration. In fact, family separation and long-distance parenting result from a multitude of reasons undertaken in various circumstances. This volume presents work from scholars who collectively show reasons that motivate parenting across distance, how families cope with separation and maintain ties, the impact of separation on family members, and how family is redefined and reconfigured in these various settings. By better understanding how we parent from a distance, this volume synthesizes ideas of kinship, relationships, and bonding and helps readers broaden their own ideas of parenting and family life.

Abstract:
The book explores how diversity in United Nations’ peace mission composition affects peacekeeping effectiveness. It identifies four key dimensions of composition: Blue Helmets’ field diversity, top mission leadership diversity (between Force Commander and Special Representative of the Secretary General), vertical leadership distance (Leadership-Blue Helmets), and horizontal distance with the local population. Each dimension of diversity of mission is measured as linguistic, geographical, and religious distance. Our book conceptualizes original mechanisms—i. resolve commitment; ii. informative trust; iii. informative communicability; iv. skilled persuasion—through which diversity can shape mission effectiveness such as trust, communicability, deterrence, and persuasion. It then evaluates each dimension separately through three pathway case studies—the UN missions in Lebanon, in Mali, and in the Central African Republic—and quantitative analyses based on a global dataset of peacekeeping operations deployed since the end of the Cold War. The book finds that diversity of Blue Helmets and diversity of top leadership may increase the mission’s capacity to reduce battle-field violence and civilian victimization. At the same time, the effects of diversity are contextual and contingent. In fact, looking at the relation between peacekeepers and Force Commanders, proximity between them is generally associated with better performances. Furthermore, homogeneity between local populations and peacekeepers, or low distance between them, is also related to low levels of hostility and casualties. This book crucially demonstrates why diversity of mission composition is a key variable to consider when trying to enhance peacekeeping effectiveness.

By moving to the context of relativistic space-time structure, this chapter completes the argument of Chapter 4 that we can use quantum theory locally to explain correlations that violate Bell inequalities with no instantaneous action at a distance. Chance here must be relativized not just to time but to a space-time point, so that an event may have more than one chance at the same time—it may even be certain relative to one space-time point but ‘at the same time’ completely uncertain relative to another. This renders Bell’s principle of Local Causality either inapplicable or intuitively unmotivated. Counterfactual dependence between the outcomes of measurements on systems assigned an entangled state is not causal since neither outcome is subject to intervention: but it may still be appealed to in a non-causal explanation of one in terms of the other.

Abstract:
This essay explores the documentary and skeletal evidence for understanding the relationship between gender and population change in the Middle Ages by focusing on mortality, fertility, and migration. Although cemeteries and historical records both show high sex ratios that imply female supermortality, the explanations offered for this imbalance indicate little consensus, not least because of gender biases in the extant records and in the methods employed to exploit them. Studies of fertility throw a helpful light on gender and population change, even though lack of direct data has forced demographers to develop innovative, if often controversial, ways to understand how fertility worked, through such measures as female age at marriage, proportions of women married, and household size. New techniques such as mitochondrial DNA and isotope analysis show that women migrated over greater distances than did men, while documentary evidence for migration over short distances reveals that women did not always move for the same reasons as men.

The associahedron is a geometric object, a multidimensional solid, whose vertices can be understood to represent the possible temporal structures on a given number of events. We can use it to relate temporal structures, such as two temporal structures on the same events in different modalities. Comparison of disjunct tonal and formal structures can be understood, for instance, as structural appoggiaturas or anticipations depending on which direction they point within the associahedron. This is illustrated with a number of analytical examples from previous chapters. A more symmetrical solid, the permutohedron, is embedded within the associahedron, and distance from the center of the permutohedron can be used as a measure of evenness for a temporal structure.

Artists, designers, photographers, and visual scientists are routinely looking for ways to create, out of a single image, the feeling that there is more to see than what meets the eye. Many well-known visual illusions are dual in nature, causing the viewer to experience two different interpretations of the same image. Hybrid images illustrate a double-image illusion, where different images are perceived depending on viewing distance, viewing duration, or image size: one that appears when the image is viewed up-close (displaying high spatial frequencies) and another that appears from afar (showing low spatial frequencies). This method can be used to create compelling dual images in which the observer experiences different percepts when interacting with the image.

The inhabitants of most urban and rural communities in both Britain and Ireland during the later medieval period would have lived a relatively short distance from either a watermill or windmill. This chapter examines the most recent archaeological evidence for water- and wind-powered mills in later medieval Britain. The use of water power, in particular, was widespread in the later medieval period for a wide range of industrial activities. However, during this same period nearly all of the grain harvest was processed in either wind- or water-powered mills. The archaeological record also demonstrates a large degree of continuity, from the late Roman and early medieval periods, in the design of waterwheels and the mechanisms they actuated.
