Abstract:
A construction project requires a multitude of people with different skills and interests and the coordination of a wide range of disparate, yet interrelated, activities. Such complexity is further compounded by the unique characteristics of a project and many other external uncertainties. As a result, construction is subject to more risk than other business activities. In a risky situation, individuals or organizations perceive the situation in their own ways and behave differently to meet their own interests. Many researchers have asserted that divergent risk attitudes are sources of mismatched risk perceptions and inconsistent behaviors among project participants in different organizations, which can disturb proactive and consistent organizational activities. The research on risk attitude has, therefore, been advocated to exploring ways to consistently arouse people‘s cognition, affection, and behavior among stakeholders. However, previous research has been a widely misunderstood concept and remains a fragmented focus in the construction field. Evidence on the construction of risk attitude and how it manifests itself is unavailable. To date, prior researchers have suffered from an issue-oriented focus that has resulted in simplified models by studying single level of antecedents of risk attitude and consequences of management performance, rather than multi-level. Moreover, previous studies only focused on the direct relationship between risk attitude and management performance instead of providing a profound conceptualization of the indirect relationship between risk attitude and management performance or empirically exploring risk attitude‘s antecedents and consequences. The current study seeks to bridge this research gap. Triangulation research is employed as an appropriate research methodology in which both qualitative and quantitative data collection are used to test the research propositions. The research plan draws upon ontology and methodological pluralism. By adopting the Critical Incident Technique (CIT), coupled with an intensive literature review, one can explore the manifestation of risk attitude and its antecedents by analyzing critical incidents derived from preliminary interviews. Cognitive Motivation Theory (CMT) and Social Cognitive Theory (SCT) provide rationales to combine a processed view of risk attitude and the antecedents and management performance of individuals and organizations into a multi-level model of risk attitude. Responses to a questionnaire survey of 239 individuals nested in 61organizations were analyzed with a blend of Confirmatory Factor Analysis (CFA) and Hierarchical Linear Modeling (HLM) to establish and examine the hypothesized relationships in the theoretical model. To capture the practical manifestation of risk attitude and its influence on management performance, case studies of two ongoing construction projects were performed. The findings summarized from both qualitative and quantitative studies indicated that risk attitude diverged due to the multi-level influences of its antecedents on project participants, resulting in inconsistent risk perception and risk inclinations. Risk attitude has two levels of manifestation – an individual and organization level. Individual risk attitude manifests itself as cognition, affection, and behavioral inclination, while organizational risk attitude mainly shows up as managerial trust, formalization, an ambiguity of goals and objectives, and a merit system. The findings confirmed that motivated individuals tend to present more consistent risk attitude and be more willing to and capable of exhibiting good management performance. The motivation behind this study is beyond the traditional motivational means. It extends from internal motivation with its locus of control and self-efficacy to external motivation with its interpersonal exchanges, external controls, and observational learning. The risk attitudes of motivated people to evoke better management performance, especially in the process of integrating risk management into a safety management system and the outcome performance of a stakeholder‘s satisfaction and potential to organizations. The research attempts to advance risk attitude theory by re-conceptualizing the antecedents of risk attitude and the consequences of management performance make the underlying theorizing mechanism explicit and testable. This study also provides practical indications of concrete interventions by managers to make risk attitudes converge and then strengthen safety risk management. The thesis contributes to multi-level analysis in the management research field and differentiates the different levels of participants in construction projects. Methodological pluralism and blended qualitative and quantitative research methods will be addressed to demonstrate the different and complementary perspectives of research. Due to limited samples, the generalizability of the findings in the different project types or across other levels needs to be further verified.

Abstract:
Infrastructure such as transportation networks improves the condition of everyday lives by facilitating public services and systems necessary for economic activity and growth. However, constructing and maintaining transportation infrastructure poses safety hazards and risks to those working at the sharp end, leading to serious injuries and fatalities. Therefore, the identification of hazards and managing the risks they create is integral towards continually improving safety levels in Infrastructure Management. This work seeks to fully understand this problem and highlight past, present and future issues concerning safety in a comprehensive literature review. A decision support tool is proposed to improve the safety of transportation workers by facilitating hazard identification and management of associated control measures. This Tool facilitates the extraction of safety knowledge from real paper-based safety documents, capturing existing worker’s knowledge and experiences from industrial ‘corporate memory’. The Tool suggests the most appropriate control measures for new scenarios based on existing knowledge from previous work tasks. This is achieved by classifying work tasks using a new method based on unilateral UK legislation (Reporting of Injuries, Diseases and Dangerous Occurrences (1995) Regulations) and the innovative use of Artificial Intelligence method Case Based Reasoning. Case Based Reasoning (CBR) allows transparency in the Tool processes and has many benefits over other safety tools which may suffer from ‘black box’ stigmatism. The Tool is populated with knowledge extracted from a real transportation project and is hosted via the internet (www. Total-Safety.com). The end product of the Tool is the generation of bespoke method statements detailing appropriate control measures. These generated paper documents are shown to have financial and quality control benefits over traditional method statements. The Tool has undergone testing and analysis and is shown to be robust. Finally, the overall conclusions and opportunities for further research are presented and progress of the work against each of the five research objectives is assessed.

Abstract:
Ballast water has been identified as a major vector for the translocation of Non- Indigenous Invasive Species (NIS) and pathogens across zoogeographical regions and subsequent discharged into recipient port states/regions. This is bound to increase given factors like the globalization of trade and the economy of scale of the ship size. Established NIS has posed significant threat to the human health, economy, finances and marine bio-diversity of recipient regions and port states. The risks associated with the discharged NIS are uncertain and difficult to assess due to the stochastic nature of species assemblages and dispersal mechanism. The safest control measure advocated by the IMO is the conduct of ballast water exchange at sea while appropriate and effective proto-type treatment technologies are being developed and approved for future application. This study has been conducted while recognizing the inability of probabilistic approaches applied in ballast water risk management to addressing uncertainty and inadequacy of data. A qualitative approach using powerful multi-criteria decision making techniques and the safety principles of the Formal Safety Assessment framework have been utilized in this research to develop three generic models for ballast water hazard estimation, risk evaluation and decision-making analysis respectively. The models are capable of being modified and utilized in the industry to address the problems of uncertainty and inadequacy of data in ballast water management. This is particularly useful as an interim measure for port states in developing economies (with insufficient data and technology) to developed robust ballast water management plans. While recognising the huge impact of ballast water pollution in recipient regions this study recommends that ballast water management programmes be given due recognition as an important element of sustainable development programmes at national and international levels. The non-availability of a benchmark based on previous research on which to fully validate the research outcome was identified as a major limitation of this research study. The models developed will therefore be subject to modifications as new data become available.

Abstract:
The continued rise in accident and ill health statistics throughout the member states of the European Union indicate that the standards of occupational health, safety and environmental control require further improvement to minimise the current level of loss. Management systems are regarded as an effective means of reducing this loss by continuously improving standards. Whilst there is much discussion and debate about the possibilities of integrating management systems, at present, there are no national or international published integrated management standards, although some multi-national companies have introduced their own internal integrated standards. The research explored the development of an integrated health, safety and environmental (HSE) management system within a range of industrial organisations. This included the development of tools for successful implementation of integrated systems, specifically for significance review, risk assessment and auditing. Resources and accreditation constraints precluded exhaustive testing of all clauses within the proposed integrated management standard. However, analysis of key aspects of the standard revealed: 1. The introduction and use of separate health, safety and environmental (HSE) management systems improved the standards of risk control within organisations. 2. Organisations perceived that there were clear business advantages in some form of integration of existing standards. 3. The developed integrated HSE standard was technically possible in the area of policy development, process operations, working instructions and documentation. However, the integration of risk assessment and audit tools gave limited advantages compared to existing separate systems. 4. The proposed integrated HSE standard complied with both individual European member states national legislative requirements and European/World-wide management standard criteria. In summary this thesis represents an original contribution to the field of integrated management systems. The thesis also identifies areas of further work that will increase the knowledge base, scope of application of the work carried out.

This report is a summary of the activities developed during 9 months of curricular training at the Directorate of Risk Management for Medicines as part of the master degree in Pharmaceutical Medicine. The internship has the purpose of complement the theoretical knowledge previously acquired and was focused on the two areas of the Directorate: Safety Monitoring and Risk Minimization. The activities of the Directorate include the collection and validation of Individual Case Safety Report received from healthcare professionals, patients and pharmaceutical companies. The activities of Risk Minimization include the implementation of safety variations, management of periodic safety update report and risk management plan, validation of the direct healthcare professional communication and the evaluation of educational materials. The Directorate is also responsible to identify and manage safety signals.
Este relatório é o sumário das atividades realizadas durante 9 meses de estágio na Direção de Gestão do Risco de Medicamentos no âmbito do mestrado em Biomedicina Farmacêutica. This compilation thesis investigates risk and safety management at the Swedish local governmentallevel. It sets special focus on municipal implementation of overall international and nationalstrategies and objectives regarding holistic, cross-sectorial and multi-strategic risk and safetywork, and the prevention of accidents and injuries. The overall aim for this thesis have been to empirically map and provide an overview of theadministrative structures for, and the prevailing management perspectives applied in the societalrisk and safety management in Sweden, as well as to study the diffusion of strategic intentions forthis area down to the municipal level. Three different sources constitute the thesis empirical base: two different set of official planningdocuments, written by Swedish municipal officials on commission of the local politiciansand one set of data from a survey investigation with municipal officials representing differentdepartments/functions. The documents were investigated using content analysis, while datafrom the survey were analyzed using statistical measures. This thesis provides through its empirical works an outlook on the general characteristics andarchetypical features of the Swedish local level’s administrative structures and managementperspective on risk and safety management; and proposes a municipal typology and a set of valuecharacters for allocation and institutionalization of risk and safety-tasks. This thesis also suggestsa conceptual framework for overviewing risk and safety management’s systemic steering elementsin its framework report. Based on the findings made, it seems as if the local level’s compliance tothe strategic level’s ambitions for the risk and safety area is hindered by practical implementationdifficulties and that much work remains in order to reach stated strategic objectives regardingholistic, inter-sectorial and multi-strategic management approaches, and preventive risk andsafety work.

Abstract:
Petroleum refineries are important facilities for refining petroleum products that provide the primary source of energy for domestic and industrial consumption globally. Petroleum refinery operations provide significant contribution to global economic growth. Petroleum refineries are complex, multifaceted systems that perform multiple phase operations characterized by a high level of risk. Evidence based major accidents that have occurred within the last three decades in the petroleum refineries, around the world, indicates losses estimated in billions of US dollars. Many of these accidents are catastrophes, which have led to the disruption of petroleum refinery operations. These accidents have resulted in production loss, asset damage, environmental damage, fatalities and injuries. However, the foremost issue analysed in literatures in relation to major accidents in petroleum refineries, is the lack of robust risk assessment and resourceful risk management approaches to identify and assess major accident risks, in order to prevent or mitigate them from escalating to an accident. Thus, it is exceptionally critical to readdress the issue of petroleum refinery risk management with the development of a more dependable, adaptable and holistic risk modelling framework for major accident risks investigation. In this thesis, a proactive framework for advanced risk management to analyse and mitigate the disruption risks of petroleum refinery operations is presented. In this research, various risk elements and their attributes that can interact to cause the disruption of PRPU operations were identified and analysed, in order to determine their criticality levels. This thesis shows that the convergent effect of the interactions between the risk elements and their attributes can lead to the disruption of petroleum refinery operations. In the scheme of the study, Fuzzy Linguistic Preference Relation (FLPR), Fuzzy Evidential Reasoning (FER) and Fuzzy Bayesian Network (FBN) methodologies were proposed and implemented to evaluate the criticality of the risk elements and their attributes and to analyse the risk level of PRPU operations. Also, AHP-fuzzy VIKOR methodology was utilised for decision modelling to determine the optimal strategy for the risk management of the most significant risk elements’ attributes that can interact to cause the disruption of PRPU operations. The methodologies proposed and implemented in this research can be utilised in the petroleum refining industry, to analyse complex risk scenarios where there is incomplete information concerning risk events or where the probability of risk events is uncertain. The result of the analysis conducted in this research to determine the risk level of petroleum refinery operations can be utilised by risk assessors and decision makers as a threshold value for decision making in order to mitigate the disruption risk of PRPU operations. The decision strategies formulated in this thesis based on robust literature review and expert contributions, contributes to knowledge in terms of the risk management of petroleum refinery operations. The result of the evaluation and ranking of the risk elements and their attributes can provide salient risk information to duty holders and decision makers to improve their perceptions, in order to prioritise resources for risk management of the most critical attributes of the risk elements. Overall, the methodologies applied in this thesis, can be tailored to be utilised as a quantitative risk assessment tool, by risk managers and decision analysts in the petroleum refining industry for enhancement risk assessment processes where available information can sometimes be vague or incomplete for risk analysis.

ENGLISH ABSTRACT: Risk management is a widely used term meaning different things to people in different economic sectors. To some, it refers to auditing and, to others, it means Occupational Health and Safety (OHS), compliance or any risk management activity dominant in the user’s frame of reference. Risk management probably includes all these associations. Viewing this study field from different angles prevents the visualisation and grasping of the whole. Business schools do not lead the way in overcoming this fragmented study field, as an all-inclusive theory basis and course outline are not yet available. The whole is taken to be the complete answers to a list of very basic questions, namely who must do what, how, when and why, and what then. This introductory study starts with the what question. Risk management activities within a sample of companies from banking to mining and manufacturing were researched. The result is a proposed map of common risk types and sector or industry-unique risks dominant in a specific environment. Three risk management training programmes were evaluated to determine whether or not the basic questions are addressed: the UNISA Advanced Programme in Risk Management, the USB Programme in Risk Management and the Enterprise Risk Management Integrated Framework developed by COSO. These programmes were assessed to determine whether they address the practical risk management questions of who must do what, when and why, and what then. A combination of some of the strong points of the courses is proposed as a starting point for overcoming the theory gap.

Abstract:

Safety is an important topic for a wide range of disciplines, such as engineering, economics, sociology, psychology, political science and philosophy, and plays a central role in risk analysis and risk management. The aim of this thesis is to develop a concept of safety that is relevant for decision-making, and to elucidate its consequences for risk and safety research and practices.

Essay I provides a conceptual analysis of safety in the context of societal decision-making, focusing on some fundamental distinctions and aspects, and argues for a more complex notion than what is commonly given. This concept of safety explicitly includes epistemic uncertainty, the degree to which we are uncertain of our knowledge of the situation at hand. It is discussed the extent to which such a concept may be considered an objective concept, and concluded that it is better seen as an intersubjective concept. Some formal versions of a comparative safety concept are also proposed.

Essay II explores some consequences of epistemic uncertainty. It is commonly claimed that the public is irrational in its acceptance of risks. An underlying presumption in such a claim is that the public should follow the experts’ advice in recommending an activity whenever the experts have better knowledge of the risk involved. This position is criticised based on considerations from epistemic uncertainty and the goal of safety. Furthermore, it is shown that the scope of the objection covers the entire field of risk research, risk assessment as well as risk management.

Essay III analyses the role of epistemic uncertainty for principles of achieving safety in an engineering context. The aim is to show that to account for common engineering principles we need the understanding of safety that has been argued for in Essays I-II. Several important principles in engineering safety are analysed, and it is argued that we cannot fully account for them on a narrow interpretation of safety as the reduction of risk (understanding risk as the combination of probability and severity of harm). An adequate concept of safety must include not only the reduction of risk but also the reduction of uncertainty.

The onshore tank farm operations has become more useful and handy, as a result of increased international sea-borne trade, particularly, the unprecedented higher volume of petroleum products and hazardous chemicals traffic globally. The onshore tank farm is a facility used for safe discharge, loading and storage of petroleum products and other hazardous chemicals at the ports. It has become an important element in the supply chain system because of the increased universal energy demand and the fact that large number of modern tanker vessel is busy and efficiently moving cargo to different destinations around the world. The tank farm serves as a back-up facility to the ports. However, it has high degree of system-wide challenges of potential major incidents/accidents, as evidenced in various tank farm recorded accidents, which occurred at different times with estimated losses valued in millions of US dollars. The accidents could be catastrophic, leading to deaths, extensive damages and adverse impact on environment. To eliminate or minimize the risk of major incident/accidents, as well as minimize the magnitude and severity, it is acutely urgent to uncover and assess all potential hazards, with a view to adopt the best preventive/mitigative policy direction in the management of this strategic facility. This thesis presents multiple safety/risk assessment approaches, uncertainties treatments and decision making techniques that are capable of finding optimal solutions that will ensure safety of tank farm operations. The standard tools of analysis employed in this tank farm operational risk assessment are Failure Mode Effect Analysis (FMEA), Faulty Tree Analysis (FTA), fuzzy logic, Analytic Hierarchy Process (AHP) and Technique for Order Preference by Similarity to the Ideal Solution (TOPSIS). Firstly, the FMEA-Fuzzy Rule Based (FRB) is applied in Hazard Identification (HAZID) and risk evaluation of tank farm operations. The methodology is utilized to discover five possible causes of catastrophic accidents in tank farm operations. The causes/hazards are described as the automatic shut-down oil safety valve failure, pipe corrosion protection system failure, automatic tank gauge system failure, leak detection device system failure, and secondary containment monitoring system failure. In the risk assessment conducted, the leak detection system failure was identified as the riskiest hazard using the Expected Utility Theory. Consequent upon the need for further investigation, another technique, Fuzzy Fault Tree (FFT), as novel model is used successfully to investigate and understand the causes of the leak detection system failure. The main aim of these two exercises is to assess risks and facilitate proper manage of these risks in tank farm operations, in order to forestall accidents that could cause damage to the facility, workers and the port environment. Nevertheless, the tank farm operations need to be optimized by ensuring the efficiency and safety of all systems and sub-systems through the adoption of best safety management decisions, which is achieved by employing AHP-TOPSIS model. This method is used to solve a complex multi-criteria decision-making problem such as selection of best Safety Control Design (SCD) among various SCDs identified. Finally, the results produced from the developed models and frameworks are summarized and other areas where they can effectively make impacts in HAZID, risk assessment and safety improvement are defined.

User-friendly systems of human factors (HF) analysis are not presently available to the managers of small to medium sized enterprises (SMEs). It is therefore difficult for such professionals to assess the safety culture within their own workplaces without the assistance of externally sourced experts. Large companies have implemented methods of HF analysis with a significant degree of success using HF experts. The aim of this research project was to confirm that SMEs could also benefit from these methods using in-house personnel with a specially-created HF assessment tool. Human error is often cited as the cause of accidents and incidents. A system of HF analysis was created as part of this research project to allow the technique to be implemented by non-experts within SMEs to identify human-related risks and thereby to assist in improving safety culture and safety performance by implementing measures to minimise those risks through HF methods. This research project found that potential collaboration partners that were initially keen to take part soon withdrew from the research project after realising what was involved in terms of required resources. For those companies that participated, the workforce was surveyed to determine the workplace safety culture. Some positive results were obtained but the overriding findings of this research project were that, of the majority of SMEs that were keen to collaborate, they did not actually want to change their safety culture; rather, they were content to continue to implement safety by enforcement of rules & regulations (antecedents) with little scope for implementation of behaviour-based safety systems of control. Although most companies approached knew of the potential benefits it was clear that they had no desire to allocate the resources necessary to achieve those benefits.

Large dams are critical infrastructures whose failure could produce high economic and social consequences. For this reason, in recent years, the application of quantitative risk analysis to inform dam safety governance has risen significantly worldwide. This thesis is focused in how computed quantitative risk results can be useful to inform dam safety management. It proposes different methods and metrics to deal with the two key issues identified in this process: how risk results can be managed to prioritize potential investments and how uncertainty should be considered in quantitative risk models to inform decision making. Firstly, it is demonstrated that risk reduction indicators are a useful tool to obtain prioritization sequences of potential safety investments, especially in portfolios with a high number of dams. Different indicators for dam safety are assessed, analyzing their relation with equity and efficiency principles. Secondly, it is proposed to consider explicitly and independently natural and epistemic uncertainty in quantitative risk models for dams, following the recommendations developed by other industries. Specifically, a procedure is developed to separate both types of uncertainty in the fragility analysis for the sliding failure mode of gravity dams. Finally, both issues are combined to propose different metrics that analyze the effect of epistemic uncertainty in the prioritization of investments based on risk results. These metrics allow considering the convenience of conducting additional uncertainty reduction actions, like site tests, surveys or more detailed analysis.
Las grandes presas son infraestructuras críticas cuyo fallo puede producir importantes consecuencias económicas y sociales. Por este motivo, en los últimos años la aplicación de técnicas de análisis de riesgos para informar a la gobernanza de la seguridad de presas se ha extendido por todo el mundo. La presente tesis se centra en analizar cómo los resultados calculados de riesgo pueden ser útiles para la toma de decisiones en seguridad de presas. Para ello, se proponen diferentes métodos e indicadores que tratan los dos principales problemas identificados en este proceso: cómo gestionar los resultados de riesgo para priorizar potenciales inversiones en seguridad y cómo debe ser considerada la incertidumbre en los modelos de riesgo para orientar a la toma de decisiones. En primer lugar, se muestra como los indicadores de reducción de riesgo son una herramienta útil y eficaz para obtener secuencias de priorización de potenciales medidas de reducción de riesgo, especialmente en la gestión conjunta de grandes grupos de presas. Por ello, los diferentes indicadores para la gestión de la seguridad de presas son evaluados, analizando su relación con los principios de eficiencia y equidad. En segundo lugar, se propone considerar la incertidumbre epistémica y la incertidumbre natural de forma independiente dentro de los modelos de riesgo cuantitativos para presas, siguiendo las recomendaciones de otras industrias. En particular, se propone un procedimiento para separar ambos tipos de incertidumbre en el análisis del modo de fallo por deslizamiento en presas de gravedad. Per a això, es proposen diferents mètodes i indicadors que tracten el dos principals problemes identificats en aquest procés: com gestionar els resultats de risc per a prioritzar potencials inversions en seguretat i com el models de risc han de considerar la incertesa per a orientar a la presa de decisions. This thesis considers the subject of crowd safety and investigates how the application of risk assessment can provide support for decision making in crowd safety management and planning. The focus is on major public venues and events where large crowds arc a normal part of the operation. Conventional methods of assessment tend to be ad hoc, reactive and rely on individual experiences. The risk assessment approach, which is comprehensive, systematic and pro-active, can help to overcome these shortfalls. Risk assessments have already been successfully applied in many workplaces, ranging from high hazard industrial plants to the office environment. However, this thesis argues that for it to be of benefit, the risk assessment must be appropriate to the nature of the operation and the nature and the extent of the hazards involved. The existing risk assessments are inappropriate to crowd safety in this respect and a more suitable methodology is required. In order to identify the methods and tools that could provide the potential solutions to the problems of assessing crowd and behaviour related hazards, a review of other risk assessments was conducted. It has highlighted a number of techniques and tools that could be applied to assess crowd safety risks. Based on the criteria mentioned above and the findings of this review, a prototype crowd safety risk assessment methodology was developed. A series of trials were conducted to evaluate the validity and usability of the prototype. Revisions were made accordingly to produce the final draft. Experiments and a questionnaire survey were then carried out on the final draft to test and verify the methodology. In general, they show that the methodology has led to an improvement in most aspects of crowd safety risk assessment. In the experiments that compared the methodology against methods representing the existing risk assessments and the conventional way of assessing crowd safety, subjects using the methodology tend to perform better in most areas. More hazards were identified. In the evaluation of risks, better consistency was achieved between individuals using the methodology. However, their judgements appeared to be less consistent over time. The use of a larger rating scheme with more choices available in the methodology could have an impact on consistency in risk evaluation. Another key factor could be that the subjects who took part in the experiments were all novice assessors. Possible learning effect may have occurred in between experiments, which could have resulted in a change of mind over time. If this is the case, this result could be an indication that the methodology is more sensitive to changes in risks or risk perception. It will be interesting to find out if experienced assessors can achieve better consistency. By and large, the experiments and questionnaire survey have served to verify, at least in part, the arguments that risk assessment is better than the conventional assessment method and that there are more benefits to be gained when the risk assessment is more appropriate to the nature and the extent of the crowd safety hazards that could arise in major public venues. Nevertheless, it is important to recognise that the research work presented in this thesis is merely the first step towards a crowd safety risk assessment methodology. There are outstanding issues yet to be resolved, not least the issue of the apparent lack of consistency over time in risk evaluation. This thesis has identified the research and development work that is required to resolve these issues and to further the benefits that risk assessment could bring to crowd safety.

The continuous desire to improve health and safety in UK construction has in recent years been challenged to adopt offsite strategies in order to address the poor health and safety record of construction. Despite the benefits of using offsite there has been little research on the actual benefits and disadvantages of the effect of offsite on occupational health and safety. This is important given that the UK government has promoted the use of offsite to improve health and safety performance. This thesis provides a strategy for the management of offsite risk and a risk management tool has been developed. The study investigated offsite manufacturers views on offsite activities and risks in comparison with insitu activities and risks. This was achieved through three phases: phase I comprised two expert group interviews, phase II involved ergonomic audits and phase III consisted of three semi-structured interviews with three offsite manufacturers. The thesis identified that there are significant health and safety benefits of offsite. The benefits relate to specific activities within the offsite categories and context studied. Examples include the elimination of work at height, reduction in noise, reduction in work in confined space, reduction in congested work with trade overlap and greater control over work in the factory. The research revealed that there are still potential health and safety risks with offsite. Examples include; transportation and delivery of units of large size and weight with associated high consequence craneage and handling risks (unit fall and hand injury), whole body vibration, cuts, MSDs, RSIs, fumes and slips trips and falls. There appears to be little in the literature to support the identification of offsite risk issues. The study identified strategies to eliminate and reduce offsite residual risks. The case study investigated solutions to further reduce residual risks, which were further explored in phase III the semi-structured interviews. The solutions are grouped into four approaches: process change, workplace environment designing out risks, automation and the use of tools. An offsite risk management tool was developed which transfers knowledge from the study to provide awareness and management of offsite risk. The thesis provides a contribution to knowledge by providing a better understanding of offsite risks, offsite residual risks and strategies used to reduce residual risks.

The hotel industry prides itself in its welcoming attitude towards its guests by offering hospitality, comfort and privacy. At the same guest safety and security has been argued to be of primary concern in the industry. The aim of this thesis is a) to study how hoteliers develop and implement risk management plans and procedures to ensure safe hotel operations, and b) to study how it is possible to gap the bridge between providing hospitable services, which at the same time are safe and secure. A qualitative, cross-case study has been applied. The study builds on theories composed primarily of established step-by-step risk management processes. In-depth interviews were carried out with four managers at different hotels across Sweden. The study found that the hotel industry in Sweden lacks a common risk management framework for which to implement consistently and coherently. A common framework would enable hotels to embrace risk management and be able to utilize it in their operation. Furthermore, it was found that risk management ought to be integrated in all organizational activities and processes. It should not be an isolated event, but rather implemented on an on-going basis. It is imperative for hoteliers to realize that hospitality is not only related to offering qualitative services, but also that ensuring guests of their safety/security is a major part of the overall service. In addition, providing hospitable services that are safe at the same time necessitates that risk management is implemented consistently by regularly aiming to raise current standard. Hence, continuous improvement ought to be of fundamental importance in any organization that is fully committed to risk management. This thesis, therefore, suggests a model that interlinks risk- and quality management.

Almost all traditional hazard analysis or risk assessment techniques, such as failure modes and effect analysis (FMEA), fault tree analysis (FTA), and probabilistic risk analysis (PRA) rely on a chain-of-event paradigm of accident causation. Event-based techniques have some limitations for the study of modem engineering systems. Specifically, they are not suited to handle complex software-intensive systems, complex human-machine interactions, and systems-of-systems with distributed decision-making that cut across both physical and organizational boundaries. STAMP (System-Theoretic Accident Model and Processes) is a comprehensive accident model created by Nancy Leveson that is based on systems theory. It draws on concepts from engineering, mathematics, cognitive and social psychology, organizational theory, political science, and economics. The general notion in STAMP is that accidents result from inadequate enforcement of safety constraints in design, development, and operation. STAMP includes traditional failure-based models as a subset, but goes beyond physical failures to include causal factors involving dysfunctional interactions among non-failing components; software and logic design errors; errors in complex human decision-making; various organizational characteristics such as workforce, safety processes and standards, contracting; and other managerial, social, organizational, and cultural factors. The main contribution of this thesis is the augmentation of STAMP with a dynamic executable modeling framework in order to further improve safety in the development and operation of complex engineering systems. This executable modeling framework: 1) enables the dynamic analysis of safety-related decision-making in complex systems, 2) assists with the design and testing of non-intuitive policies and processes to better mitigate risks and prevent time-dependent risk increase, and 3) enables the identification of technical and organizational factors to detect and monitor states of increasing risk before an accident occurs.
(cont.) The modeling framework is created by combining STAMP safety control structures with system dynamic modeling principles. A component-based model-building methodology is proposed to facilitate the building of customized STAMP-based dynamic risk management models and make them accessible to managers and engineers with limited simulation experience. A library of generic executable components is provided as a basis for model creation, refinement, and validation. A toolset is assembled to identify risk increase patterns, analyze time-dependent risks, assist engineers and managers in safety-related decision-making, create and test risk mitigation actions and policies, and monitor the system for states of increasing risk. The usefulness of the new framework is demonstrated in two independent projects: 1) A risk analysis of the NASA Independent Technical Authority (ITA), an organization mandated by the Columbia Accident Investigation Board (CAIB) to provide independent safety oversight of space shuttle operations, and 2) A risk management study for the Exploration Systems Mission Directorate (ESMD) at NASA. For these two projects, model refinement, validation and analysis required extensive data collection and interactions with NASA workforce. Over 45 interviews were conducted at five NASA centers (HQ, MSFC, KSC, JSC, and LaRC). Interviewees included representatives from the Office of the Administrator, the Office of the Chief Engineer, the Office of Safety and Mission Assurance, ESMD Directorate Offices, Program/Project Offices, and many others. Among other data sources, 200 pages of interview transcripts were compiled and used for model creation and validation activities. Specific risks analyzed include: 1) NASA workforce and knowledge management issues, 2) the impact of various levels of outsourcing, 3) the impact of safety priority on design, and 4) the impact of requirements change on safety and schedule during development.

BACKGROUND: Formalized risk management (RM) is an internationally accepted process for reducing hazards in the workplace, with defined steps including hazard scoping, risk assessment, and implementation of controls, all within an iterative process. While required for all industry in the European Union and widely used elsewhere, the United States maintains a compliance-based regulatory structure, rather than one based on systematic, risk-based methodologies. Firefighting is a hazardous profession, with high injury, illness, and fatality rates compared with other occupations, and implementation of RM programs has the potential to greatly improve firefighter safety and health; however, no descriptions of RM implementation are in the peer-reviewed literature for the North American fire service. METHODS: In this paper we describe the steps used to design and implement the RM process in a moderately-sized fire department, with particular focus on prioritizing and managing injury hazards during patient transport, fireground, and physical exercise procedures. Hazard scoping and formalized risk assessments are described, in addition to the identification of participatory-led injury control strategies. Process evaluation methods were conducted to primarily assess the feasibility of voluntarily instituting the RM approach within the fire service setting. RESULTS: The RM process was well accepted by the fire department and led to development of 45 hazard specific-interventions. Qualitative data documenting the implementation of the RM process revealed that participants emphasized the: value of the RM process, especially the participatory bottom-up approach; usefulness of the RM process for breaking down tasks to identify potential risks; and potential of RM for reducing firefighter injury. CONCLUSIONS: As implemented, this risk-based approach used to identify and manage occupational hazards and risks was successful and is deemed feasible for U.S. (and other) fire services. While several barriers and challenges do exist in the implementation of any intervention such as this, recommendations for adopting the process are provided. Additional work will be performed to determine the effectiveness of select controls strategies that were implemented; however participants throughout the organizational structure perceived the RM process to be of high utility while researchers also found the process improved the awareness and engagement in actively enhancing worker safety and health.

Suborbital flights will soon take flight as a viable commercial operation. Operators such as Virgin Galactic, along with their designer Scaled Composites, will be responsible for safety of the flight crew, Spaceflight Participants and indeed the uninvolved public beneath their flight trajectories. Within the United States, the Federal Aviation Authority’s Office of Commercial Transportation (FAA-AST) has provided Launch License Regulations and Guidelines for prospective design organisations and operators alike. The aim of this thesis is to analyse suborbital spaceflight approaches to safety management and to determine whether effective safety management is being or could be applied to influence vehicle design and subsequent operation. The thesis provides a review of current safety-related information on suborbital spaceflight, existing space safety information and also existing aviation safety information. The findings of the review concern two main areas; firstly that a gap exists within suborbital safety management criteria, and secondly that a gap exists in existing aviation-based safety guidelines. In the first case, the research concluded that FAA-AST safety management criteria did not present sufficiently explicit and rationalised guidelines for this new industry. Indeed, the thesis argues that the scope of the FAA-AST regulations (covering both orbital and suborbital aspects) is too broad, and that regulations and guidelines should be split into distinct orbital and suborbital sections so as to provide more effective directives. In Europe, no such regulations or guidelines exist as there has until now been no requirement (a ‘customer’) for the European Aviation Safety Agency (EASA) to implement such a framework. This thesis sought to address this gap by using a safety tool (Goal Structuring Notation) to construct a goal-based regulatory approach, which was included in a draft EASA suborbital Policy. Secondly, the main significant finding of this research is that a gap (literally) exists between current aviation-based design organisation safety guidelines and operator safety risk management guidelines. This absence of communication means operators are not managing their safety risks as effectively as they could. The thesis argues that the suborbital domain should take heed, as most vehicles are based on aircraft designs and therefore suborbital operators will, no doubt, apply ‘best practice’ either from the aviation or commercial space domains. Neither is appropriate or effective. As a result of the main finding a contiguous safety model has been developed which employs a ‘key (platform) hazard’ to join the design organisation analysis to the operator safety risk management, therefore completing an explicit sequence from the initiating causal event to the accident. The model is demonstrated using case studies from space disasters (Space Shuttle) and also from aviation accidents (Air France flight AF447); the model details the explicit accident sequence and shows missing or failed controls leading up to the accident. The research enabled models to be constructed and also proposed additional and explicit guidelines for the suborbital industry such as medical and training standards and separate safety criteria for vertical launch vehicles; these are included as recommendations and need to be ratified by recognised bodies such as the International Association for the Advancement of Space Safety’s Suborbital Space Safety Technical Committee for inclusion in their Space Safety Standards Manual. In the latter case these recommendations are already agenda items for the Technical Committee to address.

Abstract:
Globally, construction is considered a hazardous industry with a disproportionate amount of fatal and non-fatal injuries as compared to other industries. Electrocution is named as one of the "fatal four" causes for construction injuries by the Occupational Safety and Health Administration (OSHA). In the United States, an average of 47.9% electrical fatalities occurred in the construction industry from 2003 to 2012, according to the U.S. Department of Labor. These fatalities include both electrical workers and non-electrical workers. Such a disproportionate rate suggests a need of research to improve construction safety and reduce injuries due to electrocution. However, there is a lack of understanding of causation mechanisms surrounding fatal accidents by electrocution using a systems approach; and there is a disconnection between the mechanism of fatal electrocution accidents and the associated control measures, which may lead to less effective prevention in construction. This dissertation has three objectives, including: (a) establishing a sociotechnical system model that reflects the electrocution occurrence in the U.S. construction industry and identify the associations among its internal subsystems; (b) determining specific electrocution patterns and associated mechanism constraints; and (c) examining hierarchy of control (HOC) measures and determining their appropriateness. Findings from his research include: (a) the identification of three system patterns of electrocution in construction work systems and the associations between personnel, technological, organizational/managerial subsystems, and the internal and external environment for each of the three patterns, using a macroergonomics framework; (b) the identification of five features of work, and map out their decision-making chains, critical decision-making points and constraints, as an interpretation of electrocution mechanisms in the workplace; and (c) revealing that behavioral controls remain prevalent in electrical hazard mitigation even though the knowledge of construction safety and health has increased in the past decades, and that the effectiveness of controls is not statistically different by construction type nor occupation. Based on these findings, the research also suggests corresponding mitigation recommendations that construction managers shall strictly follow HOC rules by giving priority to higher level of controls and upgrading the industry's prevention strategy by introducing more technological innovations and encouraging prevention through design (PtD) strategies.

This research has first established that it is based on multiple methodologies developed to tackle the areas of engineering cargo handling systems, both at port and on-board vessels, as well as in the area of organisational self-assessment. It continued in reviewing the current status and future aspects of marine safety assessment together with an examination of a few major accidents. The major problems identified in marine safety assessment in this research are associated with inappropriate treatment of uncertainty in data and human error issues during the risk modelling estimation process and the calculation of failure probabilities. Following the identification of the research needs, this thesis has developed several analytical models for the safety assessment of cargo handling systems and organisational assessment structure. Such models can be effectively integrated into a risk-based framework using the marine formal safety assessment, safety case concepts. Bayesian network (BN) and evidential reasoning (ER) approaches applicable to cargo handling engineering systems have been proposed for systematically and effectively addressing uncertainty due to randomness and vagueness in data respectively. ER test cases for both a vessel selection process and a comparison of the safety maturity of different organisations in terms of self-assessment have been produced within a domain in which main and sub criteria have been developed for assessment reasons a long with the combination of the proposed model with existing organisational models. BN test case for a Liquefied Petroleum Gas (LPG) reliquefaction plant has been produced within a cause-effect domain in which Bayes' theorem is the focal mechanism of inference processing. A methodology aiming in finding the probability of failure when having variables ruled by uncertainty is established using certain variable transformation methods through the First and Second order reliability methodologies. Form/Sorm produces a most likely failure point, which is demonstrated through the application at a port cargo handling crane system. The outcomes have the potential to facilitate the decision-making process in a risk-based framework. Finally, the results of the research are summarised and areas where further research is required to improve the developed methodologies are outlined.

Recent high profile accidents involving environmental damage caused by structural failures in ageing oil tankers and bulk carriers has highlighted the importance of structural integrity issues involving these types of ships. Between 1980 and 1996, there were 186 total losses of bulk and combination carriers and 1,278 lives lost. These events have led to concerns from the public, media and within the international maritime community, about deteriorating ship structural safety standards and the environmental impact. Evidence suggests that structural failure may account for more ship losses than hitherto believed. Industry critics have complained that the quality of designs for new tonnage and effectiveness of the present control mechanisms governing structural condition for vessels in service, are inadequate. Due to the relatively low safety margins inherent in modern commercial ship structural designs, a buyer beware policy prevails in ship procurement. A weakness in current ship design practice appears to be the difficulty of incorporating an owner's individual preferences. Recognising that to be effective, improvements in ship structural design must be implemented at the design stage, this study addresses the challenge of further improving the structural safety and performance of large bulk ships through exercising specific options related to the structural design of the ship within the remit of the buyer. A broad comprehensive literature survey was conducted to cast a wide net around the problem. The complex web of regulatory controls affecting the design and operation of bulk ship hull structures was analysed and problems involving design, construction and maintenance of these vessels were uncovered to build evidence to justify proposing an improved method. An analysis of recent high profile tanker and bulk carrier accidents involving structural failure was performed, to determine root causes. These findings formed the basis for a proposed novel risk-based "design for safety" framework The core of the method is the new evidential reasoning (ER) algorithm developed on the basis of a MCDA evaluation framework and the evidence combination rule of the Dempster-Shafer (D-S) Theory. A number of structural design options focused on the cargo tank mid body area of a typical double hull VLCC were evaluated. A set of quantitative and qualitative criteria were identified and articulated, leading to a structural evaluation framework for eliciting preferences for competing options. The MCDAlER model provides a risk-based, rational, transparent methodology for rapid techno-economic evaluation of alternative structural designs, putting buyers in a stronger position to balance risks and determine the expected structural safety outcomes of different designs. The ER modelling is performed using the Intelligent Decision System (IDS) software program developed by Yang and Xu. The method was tested with an example and validated through a sensitivity study. Finally, the evidence necessary for constructing and demonstrating the MCDAlER structural evaluation framework was used to build the arguments for a safety case approach to hull structures using the Australian Offshore safety case model. The safety case for hull structures is built upon a foundation of existing prescriptive statutory and classification society structural regulatory requirements. The advantages of the safety case applied to oil tankers were explained, including suggestions for a new regulatory approach. The application of new technology and tools was discussed.

Risk is an increasingly important factor of our modern society, but that does not mean that risks have become higher today versus any other epoch of human history. Especially in the developed world, risks have been progressively lowered over time. In opposition to the declining risk in everyday life of our modern society, there seems to be an apparent increasing prevalence of risk-taking in our leisure pursuits. More people are willing to take unenforced risks and the fast growing development of adventure tourism is a good example of this. The main purpose of this thesis is to create an understanding how service providers of dangerous activities manage the balance risk and safety in interaction with their customer. Our research objective is to obtain an understanding of risk management practice in a context with the potential physical risks inherent in adventure tourism. The organizational structure for a service provider in extreme environments can be similar to a temporary organizational structure and can therefore be seen from a project management perspective. This research is conducted through a qualitative case-study, which was carried out in the context of a mountaineering expedition in the attempt to summit Mount Everest. Our empirical data are gathered from a database created by the research project Extreme Environments – Everyday Decisions (Triple ED). This database contains numerous transcribed interviews, tracking records, and field notes that was used in order to interpret and analyze the expedition and encounters happening during the process of this mountaineering expedition. Our empirical findings suggest that management of a balance between risk and safety is highly dependent on honest communication. Educated personnel that have the experience and knowledge necessary to manage an expedition or group in a similar setting are another key feature. Furthermore, managing a balance between physical risk and safety is an on-going process in the context of adventure tourism with a large amount interaction between the service provider’s representatives and their customers. For these reasons, the building of a well-functioning relationship between the incorporated strong stakeholders is of importance when balancing risk and safety in a dangerous activity with inherent risks for physical harm.

The proposed FL model incorporating fuzzy set theory and an evidential reasoning synthesis has been demonstrated on the FPSO-shuttle tanker collision scenario. The FL and BN models have been combined via mass assignment theory into a fuzzy-Bayesian network (FBN) in which the advantages of both are incorporated. This FBN model has then been demonstrated by addressing human error issues in a ship evacuation study using performance-shaping factors. It is concluded that the developed FL, BN and FBN models provide a flexible and transparent way of improving safety knowledge, assessments and practices in the marine and offshore applications. The outcomes have the potential to facilitate the decision-making process in a risk-based framework. Finally, the results of the research are summarised and areas where further research is required to improve the developed methodologies are outlined.

Safety culture may be conceptualised as an interpretive device that mediates between organisational safety rhetoric and safety programs on the one hand, and local workplace cultures on the other. More recently, risk-awareness has emerged as a cultural approach to safety. Front line workers are encouraged to become risk-aware through programs designed to prompt them to undertake mental or informal risk assessments before commencing work. The problem is that risk-awareness programs have not been the subject of systematic research and the impact of these programs on the culture of safety and the resultant level of risk is unknown. Therefore, this ethnographic study of two sites within a large contract maintenance organisation in Australia explored what impact risk-awareness programs have upon the culture of safety and the resultant level of risk. The researcher spent two months in the field and data was collected through participant observation, semistructured interviews and through a review of organisational documents. This study found that managers focused upon collecting the paperwork associated with the program as proof that workers had a safer workplace, whereas workers preferred to rely upon their common sense rather than the paperwork to keep them safe. As a consequence, the riskawareness program resulted in a culture of paperwork and varying levels of risk reduction because the paperwork associated with the program created an illusion of safety for managers as much as common sense did for workers. The results of this study have implications for safety culture, risk-awareness programs and for organisational learning. They also have implications for organisations wishing to improve their safety culture by encouraging risk-awareness in front-line workers.

This demand an ongoing management review for continual improvement through regular audits. Effective leadership is the key to a good health and safety. Risk management is equal to good corporate governance as it helps in dealing with uncertain future events that could influence the achievement of the organisation’s objectives. Therefore risk management must be embedded in all levels of management and in the planning stage so that it must form part of the daily activities of the business.

Tradicionalmente, la gestión del riesgo y la definición de estrategias de adaptación en la toma de decisiones han asumido la invariabilidad de las condiciones climáticas, incluida la persistencia de patrones históricos de variabilidad natural y la frecuencia de eventos extremos. Sin embargo, se espera que el cambio climático afecte de forma importante a los sistemas hídricos y comprometa la seguridad de las presas, lo que puede acarrear posibles impactos negativos en términos de costes económicos, sociales y ambientales. Los propietarios y operadores de presas deben por tanto adaptar sus estrategias de gestión y adaptación a medio y largo plazo a los nuevos escenarios climáticos. En la presente tesis se ha desarrollado una metodología integral para incorporar los impactos del cambio climático en la gestión de la seguridad de presas y en el apoyo a la toma de decisiones. El objetivo es plantear estrategias de adaptación que incorporen la variabilidad de los futuros riesgos, así como la incertidumbre asociada a los nuevos escenarios climáticos. El impacto del cambio climático en la seguridad de presas se ha estructurado utilizando modelos de riesgo y mediante una revisión bibliográfica interdisciplinaria sobre sus potenciales efectos. Esto ha permitido establecer un enfoque dependiente del tiempo que incorpore la evolución futura del riesgo, para lo cual se ha definido un nuevo indicador que evalúa cuantitativamente la eficiencia a largo plazo de las medidas de reducción de riesgo. Además, para integrar la incertidumbre de los escenarios futuros en la toma de decisiones, la metodología propone una estrategia robusta que permite establecer secuencias optimizadas de implementación de medidas correctoras para la adaptación al cambio climático. A pesar de las dificultades para asignar probabilidades a eventos específicos, esta metodología permite un análisis sistemático y objetivo, reduciendo considerablemente la subjetividad. Esta metodología se ha aplicado al caso real de una presa española susceptible a los efectos del cambio climático. El análisis se centra en el escenario hidrológico, donde las avenidas son la principal carga a la que está sometida la presa. Respecto de análisis previos de la presa, los resultados obtenidos proporcionan nueva y valiosa información sobre la evolución de los riesgos futuros y sobre cómo abordarlos. En general, se espera un aumento del riesgo con el tiempo; esto ha llevado a plantear nuevas medidas de adaptación que no están justificadas en la situación actual. Esta es la primera aplicación documentada de un análisis exhaustivo de los impactos del cambio climático sobre el riesgo de rotura de una presa que sirve como marco de referencia para la definición de estrategias de adaptación a largo plazo y la evaluación de su eficiencia.

Els propietaris i operadors de preses deuen per tant adaptar les seues estratègies de gestió i adaptació a mitjà i llarg termini als nous escenaris climàtics. En la present tesi s'ha desenvolupat una metodologia integral per a incorporar els impactes del canvi climàtic en la gestió de la seguretat de preses i en el suport a la presa de decisions. L'objectiu és plantejar estratègies d'adaptació que incorporen la variabilitat dels futurs riscos, així com la incertesa associada als nous escenaris climàtics. L'impacte del canvi climàtic en la seguretat de preses s'ha estructurat utilitzant models de risc i mitjançant una revisió bibliogràfica interdisciplinària sobre els seus potencials efectes. Això ha permès establir un enfocament dependent del temps que incorpori l'evolució futura del risc, per a això s'ha definit un nou indicador que avalua quantitativament l'eficiència a llarg termini de les mesures de reducció de risc. A més, per a integrar la incertesa dels escenaris futurs en la presa de decisions, la metodologia proposa una estratègia robusta que permet establir seqüències optimitzades d'implementació de mesures correctores per a l'adaptació al canvi climàtic. A pesar de les dificultats per a assignar probabilitats a esdeveniments específics, esta metodologia permet una anàlisi sistemàtica i objectiva, reduint considerablement la subjectivitat. Aquesta metodologia s'ha aplicat al cas real d'una presa espanyola susceptible a l'efecte del canvi climàtic. L'anàlisi se centra en l'escenari hidrològic, on les avingudes són la principal càrrega a la qual està sotmesa la presa. Respecte d'anàlisis prèvies de la presa, els resultats obtinguts proporcionen nova i valuosa informació sobre l'evolució dels riscos futurs i sobre com abordar-los. En general, s'espera un augment del risc amb el temps; això ha portat a plantejar noves mesures d'adaptació que no estarien justificades en la situació actual. [EN] Large dams as well as protective dikes and levees are critical infrastructures whose failure has major economic and social consequences. Risk assessment approaches and decision-making strategies have traditionally assumed the stationarity of climatic conditions, including the persistence of historical patterns of natural variability and the likelihood of extreme events. However, climate change has a major impact on the world's water systems and is endangering dam safety, leading to potentially damaging impacts in terms of economic, social and environmental costs. Owners and operators of dams must adapt their mid- and long-term management and adaptation strategies to new climate scenarios. This thesis proposes a comprehensive approach to incorporate climate change impacts on dam safety management and decision-making support. The goal is to design adaptation strategies that incorporate the non-stationarity of future risks as well as the uncertainties associated with new climate scenarios. Based on an interdisciplinary review of the state-of-the-art research on its potential effects, the global impact of climate change on dam safety is structured using risk models. This allows a time-dependent approach to be established to consider the potential evolution of risk with time. Consequently, a new indicator is defined to support the quantitative assessment of the long-term efficiency of risk reduction measures. Additionally, in order to integrate the uncertainty of future scenarios, the approach is enhanced with a robust decision-making strategy that helps establish the consensus sequence of measures to be implemented for climate change adaptation. Despite the difficulties to allocate probabilities to specific events, such framework allows for a systematic and objective analysis, reducing considerably the subjectivity. Such a methodology is applied to a real case study of a Spanish dam subjected to the effects of climate change. The analysis focus on hydrological scenarios, where floods are the main load to which the dam is subjected. The results provide valuable new information with respect to the previously existing analysis of the dam regarding the evolution of future risks and how to cope with it. In general, risks are expected to increase with time and, as a result, new adaptation measures that are not justifiable for the present situation are recommended. This is the first documented application of a comprehensive analysis of climate change impacts on dam failure risk and serves as a reference benchmark for the definition of long-term adaptation strategies and the evaluation of their efficiency.

Abstract:
This research examines the impact of certain bank-specific variables on bank stability in Ghana, in the context of the existing regulatory structures. The thesis examines this issue along two main themes. The first part of this study examines whether two of the commonly used measures of banking stability, the CAMELS and the Z-Score, provide similar or different results in assessing the stability of banks in Ghana. The results of this study show that the use of the CAMELS and the Z-score measures could lead to different outcomes in terms of bank stability in Ghana. This suggests that the traditional micro-prudential CAMELS framework should be complemented with the Z-score which inherently has both micro and macro-prudential characteristics of signaling weaknesses in bank stability, and to enhance the management of bank stability. The second part of the study examines the impact of some bank-specific variables on bank stability. Using the panel data approach, the results show that while bank size, regulatory governance, regulatory independence and origin impact significantly on the stability score, there was no significant impact in terms of interbank borrowing and non-performing loans. Further analysis using the Blinder –Oaxaca decomposition also suggests that foreign banks in Ghana exhibit relatively higher levels of stability compared to local banks. The policy implications of these findings suggest that the liberalisation of the banking sector should be accompanied by an effective micro- and macro-prudential supervisory regime in order to manage the stability of the constituent banks and the banking sector as a whole.

Abstract:
This research has evaluated the rules, guidelines and regulations related to ship vibrations. A historical failure data analysis is carried out to identify associated components, equipment and the areas of defects related to ship vibration problems. Ship Hull Vibration (SHV) is recognised as a major problem onboard ships and the propulsion system is identified as the major contributor to SHV. The current status of ship vibrations is reviewed and possible sources which create SHV are recognised. The major problems identified in this research are associated with risk modelling under circumstances where high levels of uncertainty exist. Following the identification of research needs, this PhD thesis has developed several analytical models for the application of Formal Safety Assessment (FSA). Such mode quently demonstrated by their corresponding case sti vith regard to application of FSA for SHV modelling. Firstly, in this research a generic SHV model is constructed for the purpose of risk estimation based on the identified hazards. The hazards include the SHV effects induced by ship design criteria, failure of components, and different patterns associated with the ship propulsion system (propeller system and machinery) as the major contributors to SHV. Then risk estimation bn is carried out utilising Evidential Reasoning (ER) and a fuzzy rule base. Secondly, ship selection (decision making) is investigated to to select the best ship design based on the risk estimation results of SHV. The risk estimation is carried out using ER, a fuzzy rule base and continuous fuzzy sets. The best ship design is selected by taking into account an ER-based utility ranking approach. Thirdly, combining discrete fuzzy sets and an Analytical Hierarchy Process (AHP) risk estimation is conducted in terms of four risk parameters to select the major causes of component failure and then SHV.

Abstract:
Self Supply Systems (SSS) can be defined as privately owned household level water sources. The research focus is on urban self supply hand dug wells in Abeokuta, Nigeria. Self supply wells serve an estimated 45% of Abeokuta’s population. SSS can be gradually upgraded to improve water quality, but water quality can also be improved through effective risk management. The World Health Organization (WHO) has developed a risk management tool known as Water Safety Plans (WSP), but the tool has not been tried for SSS. This research focuses on the relevance of the generic WHO water safety plans tool to SSS, with the aim to develop an appropriate water safety framework for self supply sources to ensure acceptable household water. Water from self supply wells is used for both ingestion and non-ingestion household activities. The water quality of the sources is poor and not safe for consumption with faecal coliform counts in excess of 100 cfu/100 ml of water. Self supply wells in Abeokuta are plagued by four main water safety threats; style of source operation – primarily through bucket and rope -, construction problems, proximity to sources of contamination, and user’s hygiene practices. Users are in denial of the health consequences of unsafe water. There is a predominantly reactive attitude to water safety management. The main source management approaches include access and hygiene management. To appropriate existing WSP to SSS, source and water safety control measures require user acceptability to be sustainable in terms of adoption and compliance. Incentives are needed for the adoption of SSS water safety plans. A two- phase supporting program is necessary: awareness and enlightenment campaigns and relevant training activities. Water safety development for self supply wells need to be initiated and coordinated by an established institution other than the source owners. This research suggests the Department of Public Health as the institution to facilitate the development of water safety plans for SSS in Abeokuta, Nigeria.
