A gonorréia é uma infecção bacteriana freqüente, causada pela Neisseria gonorrhoeae, um diplococo Gram-negativo de transmissão quase que exclusiva através de contato sexual ou perinatal. Primariamente afeta membranas mucosas do trato genital inferior, e mais raramente, as mucosas do reto, orofaringe e conjuntiva. A infecção genital ascendente na mulher leva a uma complicação séria, a salpingite aguda, uma das principais causas de infertilidade feminina. A partir dos anos 90, deu-se início a um novo tempo no que se refere a descobertas sobre a patogenia da gonorréia e seu agente etiológico. O controle da gonorréia tem sido difícil na maioria das populações, e essa permanece um exemplo da influência que os fatores sociais, comportamentais e demográficos exercem na epidemiologia de uma doença infecciosa. O manejo da gonorréia e de outras doenças sexualmente transmissíveis requer tanto o tratamento do paciente e de seu parceiro sexual como medidas de saúde pública para interromper a transmissão da infecção e evitar complicações a longo prazo.

Objetivo: Relatar a experiência de usuária do Sistema Único de Saúde em hospital situado em Piabetá/ Magé, distrito situado no Estado do Rio de Janeiro, na qual a cliente alega incapacitantes dores pélvicas. Método: Houve relato de experiência através de coleta de dados de prontuário, além de entrevista com a usuária. Resultados: A usuária com 48 anos de idade e funcionária de uma companhia de limpeza apresentava dores pélvicas paroxísticas, em meio a questões de ordem social e psicológicas desfavoráveis. Insistiu com a equipe que seu sofrimento advinha de afecções uterinas, em detrimento de mioma detectado em ultrassonografia e menorragia. Seu caso evoluiu à conduta cirúrgica com extirpação de útero e ovários. Conclusão: A análise histopatológica revelara mioma e salpingite, podendo esta ter motivado suas dores, devido às aderências. As dores, porém, persistiram com igual gravidade, tendo sido sanadas após procedimento urológico devido à litíase. Conclui-se que o singular, o sentimento da cliente relacionando a origem da dor à afecção do sistema reprodutor norteia seu primeiro tratamento cirúrgico, o que não lhe trouxe a almejada cura de imediato. O singular, assim, norteou a conduta do relato em tela. A resolutividade para a cliente poderia ter sido superior, caso a unidade hospitalar relevasse o enfermeiro em suas equipes multidisciplinares.

Se estudia histopatológicamente, en forma retrospectiva 96 casos diagnosticados como "embarazos ectópicos salpíngeos", pertenecientes a los Archivos del Instituto de Patología de la Facultad de Medicina U.N.M.S.M., en el Hospital Loayza. Sesenta de estos casos tenían historias clínicas utilizables. En el grupo de lesiones puras la más frecuente era la Salpingitis crónica folicular con 22 casos (22.9%) siguiéndole en orden de frecuencia la salpingitis crónica simple con 18 casos (18.8 %) y por último la salpingitis ístmica nodosa con 16 casos (16.6%). Entre las catalogadas como "mixtas" el mayor porcentaje correspondió a casos en los que se hallaba concomitantemente lesiones del tipo de la Salpingitis ístmica nodosa y la Salpingitis crónica folicular: 6 casos (6.3 %), siguiendo los casos de Salpingitis ístmica nodosa y Salpingitis crónica simple: 3 casos (3.1 %). Este trabajo muestra la importancia de las lesiones inflamatorias salpíngeas en la génesis del embarazo ectópico tubario y el importante pero poco conocido rol de la salpingitis ístmica nodosa en este proceso.

SUMMARYPelvic inflammatory disease (PID) and more specifically salpingitis (visually confirmed inflammation) is the primary cause of tubal factor infertility and is an important risk factor for ectopic pregnancy. The risk of these outcomes increases following repeated episodes of PID. We developed a homogenous discrete-time Markov model for the distribution of PID history in the UK. We used a Bayesian framework to fully propagate parameter uncertainty into the model outputs. We estimated the model parameters from routine data, prospective studies, and other sources. We estimated that for women aged 35–44 years, 33·6% and 16·1% have experienced at least one episode of PID and salpingitis, respectively (diagnosed or not) and 10·7% have experienced one salpingitis and no further PID episodes, 3·7% one salpingitis and one further PID episode, and 1·7% one salpingitis and ⩾2 further PID episodes. Results are consistent with numerous external data sources, but not all. Studies of the proportion of PID that is diagnosed, and the proportion of PIDs that are salpingitis together with the severity distribution in different diagnostic settings and of overlap between routine data sources of PID would be valuable.

Study Objective. To identify sensitive and specific histological criteria for endometritis in women with laparoscopically-confirmed acute salpingitis.Methods. Women, age 18–40 years of age presenting with complaints of lower abdominal pain ≤2 weeks and no antibiotics use in past two weeks, were enrolled. They underwent clinical examination, screening for HIV; other sexually transmitted infections plus endometrial biopsy sampling for histopathology. Diagnostic laparoscopy confirmed the diagnosis of acute salpingitis. Controls were women undergoing tubal ligation and HIV-1 infected women asymptomatic for genital tract infection.Results. Of 125 women with laparoscopically-confirmed salpingitis, 38% were HIV-1 seropositive. Nineteen HIV-1 negative controls were recruited. For the diagnosis of endometritis, ≥1 plasma cells (PC) and ≥3 polymorphonuclear lymphocytes (PMN) per HPF in the endometrium had a sensitivity of 74% for HIV-1-seropositive, 63% for HIV-1-seronegative women with a specificity of 75% and positive predictive value of 85% regardless of HIV-1-infection for predicting moderate to severe salpingitis. For HIV-1-seronegative women with mild salpingitis, ≥1 PC and ≥3 PMN had a sensitivity of 16% and a PPV of 57%.Conclusion. Endometrial histology, did not perform well as a surrogate marker for moderate to severe salpingitis, and failed as a surrogate marker for mild salpingitis.

Abstract:
Salpingitis is manifested as hemorrhagic follicular inflammation exudations and peritonitis, leading to reduced egg production and high culling of breeder flocks. From 2018 to 2021, increasing salpingitis during egg peak is threatening the poultry industry post-artificial insemination, both in breeder layers and breeder ducks across China. In our study, Escherichia coli (E. coli), Enterococcus faecalis(E. faecalis) and Chlamydia psittaci (C. psittaci) were isolated and identified from the diseased oviducts using biochemical tests and PCR. To identify and isolate pathogenicity, we inoculated the isolates into laying hens via an intravaginal route. Later, laying hens developed typical salpingitis after receiving the combination of the aforementioned three isolates (1 × 105 IFU/mL of C. psittaci and 1 × 106 CFU/mL of E. faecalis and E. coli, respectively), while less oviduct inflammation was observed in the layers inoculated with the above isolate alone. Furthermore, 56 breeder ducks were divided into seven groups, eight ducks per group. The birds received the combination of three isolates, synergic infection of E. coli and E. faecalis, and C. psittaci alone via vaginal tract, while the remaining ducks were inoculated with physiological saline as the control group. Egg production was monitored daily and lesions of oviducts and follicles were determined post-infection on day 6. Interestingly, typical salpingitis, degenerated follicles and yolk peritonitis were obviously found in the synergic infection of three isolates and the birds inoculated with C. psittaci alone developed hemorrhagic follicles and white exudates in oviducts, while birds with E. faecalis or E. coli alone did not develop typical salpingitis. Finally, higher E. coli loads were determined in the oviducts as compared to E. faecalis and C. psittaci infection. Taken together, the combination of E. coli and E. faecalis, and C. psittaci could induce typical salpingitis and yolk peritonitis both in laying hens and breeder ducks. Secondary infection of E. coli and E. faecalis via artificial insemination is urgently needed for investigation against salpingitis.

Objective: To compare the epidemiology and hospital course of patients with acute salpingitis with and without coincident human immunodeficiency virus (HIV) seropositivity.Methods: Patients admitted to the UMDNJ-University Hospital in Newark, New Jersey from January 1, 1991, to December 31, 1991, with acute salpingitis were studied.Results: Eight percent of all hospitalized patients with acute salpingitis were HIV-positive. The mean age of the HIV-negative group was 25.4 compared with 29.6 years in the HIV-positive group. Gonorrhea and chlamydia were present in 49% and 22%, respectively, in HIV-negatives and in 40% and 20% of HIV-positives. Two of 5 (40%) HIV-positive patients had tuboovarian abscesses compared with 12 of 59 (20%) HIV-negative patients. Three of 5 (60%) HIV-positive patients had admission WBC counts fewer than 10,000/mm3 compared to 6 of 59 (12%) of HIV-negatives (P = 0.024). The hospital stay was 5.4 days for HIV-positives and 5.8 days for HIV-negatives.Conclusions: Eight percent of hospitalized patients with acute salpingitis were HIV-seropositive. Neisseria gonorrhoeae and chlamydia were commonly found organisms in both groups. The initial WBC count was lower for HIV-positive patients. The hospital course of both groups was similar.

A case of Xanthogranulomatous salpingitis in 55yrs old women with sigmoid diverticulitis is presented. Xanthogranulomatous inflammation is an uncommon form of chronic inflammation which affects various organs and is destructive in nature. This inflammatory process is characterized by massive infiltration of the tissues by lipid laden histiocytes admixed with lymphocytes, plasma cells and polymorphonuclear leucocytes. Pelvic inflammatory disease is the main etiology. This could have possibly resulted in xanthomatous process in our case leading to xanthogranulomatous salpingitis.

Abstract A case of xanthogranulomatous salpingitis and oophoritis in a 47-year-old woman is presented. Xanthogranulomatous inflammation is an uncommon form of chronic inflammation that is destructive to affected organs; it is characterized by the presence of lipid-filled macrophages with admixed lymphocytes, plasma cells, and neutrophils. Only a few cases of xanthogranulomatous salpingitis and oophoritis have been reported to date. The case presented here is associated with Escherichia coli infection, endometriosis, and an intrauterine device.

Objective: To investigate the diagnostic value of color Doppler ultrasonography applied to infertile patients with chronic salpingitis and mycoplasma infection. Methods: 116 infertile patients with chronic salpingitis and mycoplasma infection were selected in this investigation. After these patients were admitted to hospital, they all underwent color Doppler ultrasound guided hysterosalpingogram and X-ray hysterosalpingography, in order to make definitive diagnoses, and the X-ray hysterosalpingography was considered as the gold standard. The diagnostic value and rates of adverse reaction between color Doppler ultrasound guided hysterosalpingogram and X-ray hysterosalpingography were evaluated. Results: A total of 116 patients with chronic salpingitis and mycoplasma infection received color Doppler ultrasound guided hysterosalpingogram and X-ray hysterosalpingography. Diagnostic coincidence rates were equal, and no statistical difference could be observed (P > 0.05), with respect to the tubal patency in double sides, tubal obstruction in double sides, tubal patency in single side, and tubal obstruction in single side. When concerning adverse reactions, the incidence rates of allergy and colporrhagia between two groups were statistically significantly different (P = 0.025 and P = 0.040, respectively). In addition, the total incidence rate of adverse reactions was also statistically significant different (P = 0.003). Conclusion: The color Doppler ultrasound aided hysterosalpingogram could make a diagnosis intuitively and clearly for patients with chronic salpingitis and mycoplasma infection, and can be used as the preferred method for diagnosing tubal infertility.

Presentamos el caso de una paciente de 45 años de edad con el diagnostico de carcinoma primario de salpinge siendo que la paciente acude a consulta por dolor abdominal y presencia de flujo vaginal, con reporte ecográfico de masa anexial derecha, es ingresada a cirugía con el diagnóstico pre-operatorio de “tumor de ovario derecho”, se realiza estudio trans-operatorio mismo que descarta lesión en ovario y postula lesión neoplásica primaria de salpinge , llegando al diagnóstico definitivo por histopatología e inmunohistoquimica de carcinoma seroso de alto grado, afecta capa muscular sin rebasarla. Asociado a focos de TIC (carcinoma intratubario). En ovario se evidenció: ocarcinoma seroso de alto grado metástasico de salpinge.

AbstractBased on external and internal structural features of larvae and adults, the phylogeny of Trictenotomidae, Salpingidae, Pythidae, Boridae, Tydessa Peacock (included previously in Pilipalpinae), Pilipalpinae, Pyrochroinae, and Pedilinae is reconstructed as: (Trictenotomidae + Salpingidae + Pythidae) + (Boridae + {Tydessa + [Pilipalpinae + (Pyrochroinae + Pedilinae)]}). The genus Tydessa is placed in its own monobasic subfamily, Tydessinae. Both Tydessinae and Pilipalpinae are included in Pyrochroidae, along with Pyrochroinae, Pedilinae, and possibly Cononotus and Agnathus. A historical account of the classification of Pilipalpinae is presented, along with a revised subfamilial classification of Pyrochroidae and Pythidae.

The authors treated a patient for what appears to be the first reported occurrence of acute peritonitis and salpingitis associated with streptococcal toxic shock syndrome (STSS). This was caused by Lancefield group G α-haemolytic Streptococcus dysgalactiae subsp. equisimilis TKCH2004-001. The isolate showed M protein type stc36 and carried the spegg gene. To the best of the authors' knowledge, the present report represents the first case of STSS complicating acute peritonitis and salpingitis caused by Lancefield group G α-haemolytic S. dysgalactiae subsp. equisimilis.

Se informa la presencia de Poophylax falklandica en dos islas del archipiélago Diego Ramírez, siendo el primer registro de esta especie en territorio chileno. Se establecen las semejanzas y leves diferencias con respecto a las descripciones previas de la especie y se dan datos morfológicos no considerados previamente. Se reconoce su similitud con el género Nawelbuta Solervicens que comparte con Poophylax la organización de los tarsos, situación particular entre los Salpinginae. Sobre la base de la asociación de la especie con coironales de Poa flabellata se estima su probable presencia en islas del archipiélago del Cabo de Hornos donde hay registros de esta especie de gramínea.

ABSTRACT Infection of the Fallopian tubes (FT) by Neisseria gonorrhoeae can lead to acute salpingitis, an inflammatory condition, which is a major cause of infertility. Challenge of explants of human FT with gonococci induced mRNA expression and protein secretion for the proinflammatory cytokines interleukin (IL)-1α, IL-1β, and tumor necrosis factor alpha (TNF-α) but not for granulocyte-macrophage colony-stimulating factor. In contrast, FT expression of IL-6 and of the cytokine receptors IL-6R, TNF receptor I (TNF-RI), and TNF-RII was constitutive and was not increased by gonococcal challenge. These studies suggest that several proinflammatory cytokines are likely to contribute to the cell and tissue damage observed in gonococcal salpingitis.

ZusammenfassungDie Endosalpingiose der Harnblase ist eine sehr seltene benigne Erkrankung der Harnblase gekennzeichnet durch das ektope Auftreten von endosalpingialem Gewebe. Das zusätzliche Vorliegen von endozervikalem oder endometrialem Gewebe wird als Müllerianose bezeichnet. In der englischsprachigen Literatur sind ca. 5 reine Endosalpingiose- und 17 Müllerianosefälle beschrieben. Über die Entstehung der Erkrankung existieren zwei Theorien. Während die Implantationstheorie von der Versprengung ektopen Gewebes durch chirurgische Eingriffe ausgeht, propagiert die Metaplasie-Theorie die Neuentstehung durch einen vermeintlichen hormonellen Einfluss. Die Symptomatik der Erkrankung ist ebenso vielfältig wie unspezifisch und kann eine Drangsymptomatik, suprapubische Schmerzen, aber auch Dysurie oder Makrohämaturie beinhalten. Die Beschwerden können zudem zyklisch und zyklusabhängig auftreten.Die Diagnosestellung wird aufgrund der Seltenheit weitestgehend ein Zufallsbefund im Rahmen der zystoskopischen Abklärung dieser Beschwerden bleiben. Eine adäquate Therapie gelingt in der Regel mittels transurethraler Resektion. Allerdings kann es auch bei scheinbar vollständiger Resektion zu Rezidiven kommen, welche zum Teil eventuell einen invasiveren Eingriff nach sich ziehen können.Wir präsentieren die Kasuistik einer 40-jährigen Patientin mit schmerzhafter Makrohämaturie, bei der zufallsbefundlich eine intravesikale Salpingiose nachgewiesen und die in unserer Klinik mittels transurethraler Resektion behandelt wurde. Zudem geben wir einen Überblick über die vorliegende Literatur dieser seltenen intravesikalen Raumforderung.

Background. The incidence of tubal ectopic gestation caused by schistosomiasis induced tubal pathology is undocumented in this environment, which may be due to rarity of this pathology. Bilateral tubal gestation is common in patients that have undergone in vitro fertilization. We report a hitherto undocumented case of spontaneous bilateral ectopic gestation following tubal schistosomiasis.Case Report. Mrs. OB was a 32-year-old G4P3+0(3 alive) woman who complained of abdominal pain and bleeding per vaginam of 4 and 2 days’ duration respectively following 8 weeks of amenorrhea. A clinical impression of ruptured ectopic gestation was confirmed by ultrasound scanning. She had bilateral salpingectomy with histology of specimens showing bilateral ectopic gestation withSchistosoma haematobiuminduced salpingitis (findings ofSchistosoma haematobiumova noted on slide).Conclusion.Schistosomainduced salpingitis is a rare but possible cause of bilateral tubal gestation.
