Abstract:
This article examines changing labor regimes in the wake of the Nepalese Maoist revolution through a historical and ethnographic exploration of brick factory in the Kailali district, western Nepal. I argue that the Maoist rebellion has helped to produce a new young, mobile, and urbanizing working class that in the new political context feels increasingly secure in claiming new spaces for its own emerging pleasure, pastimes, and sociality vis-à-vis existing hierarchies and customs. I further show that there is a broad base for solidarities among more and less skilled people within this proletariat, though such solidarity does not yet seem to reach beyond “ethnic” and linguistic boundaries. Highlighting these facets of the transformation of everyday labor regimes in a sociopolitical context that is in a high state of flux and change, the article looks into the consequences of the Maoist revolution in the working lives of ordinary people.

Abstract:
La mobile-health propone una crescente gamma di tecnologie digitali, che offrono soluzioni nella cura della salute e nella promozione del benessere. Pur contribuendo in maniera vantaggiosa alla prevenzione delle malattie e alla gestione delle patologie croniche in particolare, tuttavia la varietà delle tecnologie disponibili e l’ampiezza dei fini perseguibili sollevano perplessità rispetto alla regolamentazione e alla valutazione morale della mobile-health. L’articolo analizza le implicazioni etiche e politiche della natura ibrida delle tecnologie digitali, al confine tra promozione della salute e gestione dello stile di vita, evidenziando come esse si appropriano della vita dell’utente. Attraverso un meccanismo di auto-sorveglianza, la mobile-health acquisisce dati ed elargisce raccomandazioni per una vita più salutare. Tale dispositivo, identificato nei processi di datificazione e modulazione delle condotte, è orientato all’ottimizzazione della vita, intesa come massimizzazione dei processi vitali, ovvero del loro funzionamento e del loro esito. L’ottimizzazione potrebbe comportare la riduzione della condotta umana a una moltiplicazione di prestazioni, attraverso le quali l’individuo cerca di migliorare se stesso ma, così facendo, egli stesso si rivela funzionale al profitto economico-produttivo, politico e organizzativo della società. L’ipotesi che si intende dimostrare è che l’auto-sorveglianza su cui si basa la mobile-health diviene una strategia di governo eteronomo delle condotte.

Nell’ottobre 2015 l’azienda 23andMe ha ottenuto da parte della Food and Drug Administration (FDA) l’approvazione a vendere direttamente ai consumatori alcuni test genetici, acquistabili quindi senza la supervisione di professionisti sanitari. I 36 test approvati forniscono informazioni sul rischio riproduttivo di malattie quali fibrosi cistica e Tay-Sachs. Al momento 23andMe non può commercializzare i test di suscettibilità genetica (GST), ma spera di ottenere l’approvazione della FDA per fornire informazioni anche sul rischio di insorgenza di malattia. L’idea è che, acquisendo tali informazioni, i consumatori possano intraprendere stili di vita più idonei per evitare che si sviluppi la patologia. “È in arrivo la sanità su misura dei consumatori”, ha esclamato Anne Wojcicki, co-fondatrice della 23andMe. Effettivamente, accanto ad altre modalità (quali mobile health, telemedicina, ecc.) di accesso online all’informazione sanitaria, i test genetici diretti ai consumatori incoraggiano un approccio consumistico alla salute. Tuttavia, questo è vantaggioso per la medicina e i consumatori? La promozione della salute richiede attitudini meramente consumistiche? Per discutere tali questioni, il paper affronterà innanzitutto i problemi di validità clinica e di utilità dei GST così come le loro implicazioni sul sistema sanitario: sebbene la ricerca continui, i GST hanno un potere predittivo limitato e le misure preventive o terapeutiche non sono sempre disponibili, comportando sovramedicalizzazione e minori risorse sanitarie per altri (i consumatori potrebbero richiedere ulteriori test per confermare o invalidare i risultati ottenuti). Verranno poi analizzate le questioni etiche relative alla commercializzazione della salute e al progressivo passaggio dal rapporto fiduciario medico-paziente al rapporto consumistico fornitore-fruitore. ---------- In October 2015 the company 23andMe gained the Food and Drug Administration (FDA)’s approval to market genetic tests directly to consumers, without supervision of healthcare professionals. The FDA approved 36 carrier tests to provide health information on inherited disorders, including cystic fibrosis and Tay-Sachs. 23andMe is still prohibited in marketing genetic susceptibility testing (GST), but the company hopes to gain the FDA’s approval to provide health risk information. The idea is that informing customers of their genetic susceptibility may motivate them to change their health-related behavior, reducing the risk of the disease’s onset. “Direct-to-consumer healthcare is coming!”, said Anne Wojcicki, co-founder of 23andMe, after the FDA’s approval. Actually, besides other ways (such as mobile health, telemedicine, etc.) of getting access to health information and services through internet, direct-to-consumer genetic tests fosters a consumerist approach to healthcare. Nevertheless, is such an approach advantageous for medicine and consumers? Can we consider health as a mere consumer good whose promotion requires consumerist attitudes? To discuss these issues, the paper will firstly address problems with clinical validity and utility of GST as well as their implications for healthcare systems: although research is ongoing, GST have limited predictive power and therapeutic or preventive measures are not always available, leading to overmedicalization and fewer healthcare resources for others (consumers could require follow-up tests to confirm or deny obtained results). Secondly, ethical concerns regarding putting health in the marketplace will be analyzed by examining the progressive shift from healthcare professional-patient fiduciary relationship to provider-user consumerist relationship.

RiassuntoSono descritte, con la stessa metodologia impiegata in altre 4 aree italiane, sedi di registri psichiatrici dei casi (RPC), le caratteristiche della catchment-area, del RPC, della struttura e dei principi del Servizio di Salute Mentale di Arezzo. II monitoraggio della domanda su 4 anni evidenzia che i tassi di prevalenza annua (1537/100000 15 + ) e un giorno (714/100000 15 + ), specie per le psicosi funzionali, hanno valori vicini ai RCP europei. Con la incidenza (256/100000 15 + ), i tassi indicano un coverage e una presa in carico dei principali bisogni psicopatologici di popolazione di area. II monitoraggio dei servizi offerti evidenzia la chiusura dell'OP nel 1989 con dimissione dei lungodegenti e 1'espansione considerevole delle attivita e degli utenti in molteplici strutture e servizi a differente gradiente assistenziale, residenziali (66/100000 15 + ospitati nel 1990), diurni (205/100000 15 + utenti nel 1990), di inserimento lavorativo, domiciliari, ambulatoriali e ospedalieri, che ruotano intorno a un Centro con équipe in mobilita e aperto 24h/24 e 7 gg./7. II rapporto tra prevalenza annua non ospedaliera e ospedaliera e nel 1990 di 20:1; l'intervento precoce e alternativo consente un tasso medio di ammissioni ospedaliere di 86/100000 15 + con SPDC ma senza reparto e senza nuovi lungodegenti. II tasso di lungoassistiti (541/100000 15 + ) e il pattern di utilizzo dei servizi indicano una risposta continuativa, flessibile e integrata ad utenti per il 90% viventi nel proprio ambiente. The monitoring of the demand over 4 years shows that the mean year prevalence (1537/100000 15 +) and day prevalence (714/100000 15 +) rates, especially for functional psychoses, are statistically close to European PCRs. Together with incidence (256/100000 15 +), those rates indicate full coverage and response to the principal psychopathological needs of the population of the area. The monitoring of the service shows: the closure of the psychiatric hospital in 1989 with the discharging of long-stay patients; the considerable expansion of activities and the number of users in multiple structures and services, offering different degrees of care; i.e. residential (66/100000 15+ users in 1990), day-centres (205/100000 15+ users in 1990), work emplacement, domiciliary care, out and in-patients. Those services revolve around the Centre (open round the clock, 7 days a week) with a mobile crisis intervention team. The ratio between hospitalized and non-hospitalized users is 1:20, for 1990; early and alternative intervention gives an average rate of hospital admissions of 86/100000 15+ in clinical and psychiatric beds but with no psychiatric ward and no new long-stay patients. The rate of long-term patients (541/100000 15 +) and the pattern of use of the services indicate an ongoing, flexible and integrated response to users needs, 90% of whom live in their own home environment. The costs of the CMHS work out equal to the former expenses of the psychiatric hospital, in ratio to inflation and the cost of living.

The introduction of smartphones has revolutionized how late adolescents (aged 18-21 years) access and use the internet. Vast troves of health information are today just a tap or swipe away, with smartphones and internet connectivity becoming increasingly accessible. The need for eHealth literacy among late adolescents is now gaining importance as it ensures an effective use of health information. This study conducted a survey among 427 late adolescents in order to evaluate their eHealth literacy levels; their perceptions of the quality of online health information; their level of trust and credibility in online health and checked if acquiring health information through the online medium led to a change in their behavior intention. The results showed that most of the late adolescents preferred viewing multiple websites for their health information needs. Health information in the form of text and images were preferred over video content; and most preferred accessing online health information in their native language. Cancer and obesity are the common health issues of interest to both genders. Mobile applications (apps) were the least preferred mode of accessing heath information despite the high usage of smartphones. eHealth literacy and credibility positively predicted behavior intention while quality of health information did not predict behavior intention. La introducción de los smartphones (teléfonos inteligentes) ha revolucionado la forma en que los adolescentes tardíos (de entre 18 y 21 años) acceden y usan Internet. Hay una gran cantidad de información a solo un toque de distancia y los teléfonos móviles y la conectividad a Internet son cada vez más accesibles. La necesidad de aprender acerca de eSalud entre los adolescentes tardíos ahora está cobrando importancia, ya que garantiza un uso eficaz de la información de la salud. En este estudio se realiza una encuesta a 427 adolescentes tardíos para evaluar sus conocimientos en eSalud; sus percepciones de la calidad de la información de la eSalud; su nivel de confianza y credibilidad en eSalud y verificar si la adquisición de información de salud a través de este medio conduce a un cambio en su intención de comportamiento. Los resultados mostraron que la mayoría de los adolescentes tardíos preferían ver múltiples páginas web para sus necesidades de información de salud y la mayoría preferían acceder a información de eSalud en su idioma nativo. Las aplicaciones móviles (apps) eran el método menos usado para acceder a la información de salud a pesar del alto uso de smartphones. La alfabetización y la credibilidad de eSalud predijeron positivamente la intención de comportamiento, mientras que la calidad de la información de salud no predice la intención de comportamiento.

Introduzione: In Brasile, le reti di emergenza e di emergenza sono supportate e regolamentate dal sistema sanitario unificato, il professionista infermieristico è essenziale per le pratiche di assistenza svolte dai servizi di assistenza mobile di emergenza (SAMU ), non solo per quanto riguarda l'orientamento e il supporto medico, si nota che l'assistenza infermieristica ha un gran numero di funzioni relative alla sua assistenza in SAMU. Obiettivi: Identificare il lavoro degli infermieri in relazione al servizio di assistenza di emergenza mobile. Metodo: È stata condotta una revisione sistematica della letteratura, con l'analisi delle principali riviste infermieristiche nelle banche dati Scientific Electronic Library Online (SciELO), nella letteratura latinoamericana e caraibica nelle scienze della salute (Lilacs) e nel sistema online di ricerca e analisi della letteratura medica (Pubmed/MEDLINE). Un totale di 47 articoli sono stati raccolti con il tema dello studio. Risultati: È stato dimostrato che vi è una notevole quantità di articoli relativi alle competenze dell'infermiere nei confronti della SAMU e alla rilevanza della professione per il corretto funzionamento della SAMU. Conclusione: Si conclude che attraverso lo sviluppo scientifico e tecnico della professione, con la realizzazione di corsi di laurea e di laurea, le attribuzioni infermieristiche in relazione alla SAMU sono considerate importanti, ma che ulteriori ricerche sono ancora necessarie per studenti di questa categoria. Il suicidio è l’atto umano di porre fine alla propria vita. Gli atteggiamenti suicidi con l’intenzione di morire, ma senza portare alla morte, sono chiamati tentativi di suicidio, cioè sono atti intenzionali di autolesionismo senza che il risultato sia la morte. Questo studio ha analizzato, epidemiologicamente, le cure fornite dal Mobile Emergency Service – SAMU / AP relative ai tentativi di suicidio nel 2018 a Macapá / AP. È stata condotta una ricerca trasversale, descrittiva e quantitativa. I dati sono stati raccolti da 61 record di cure iniziali riferiti a eventi nel 2018, dal sistema di archiviazione di file SAMU / AP. Questo studio ha permesso di ampliare la conoscenza delle caratteristiche dei casi di tentativi di suicidio e suicidi consumati avvenuti a Macapá / AP, consentendo così al supporto di professionisti di lavorare su interventi con la popolazione alla ricerca della riduzione delle statistiche esistenti. È interessante notare che il problema della mancata o notifica di casi di tentato suicidio eseguiti in modo insufficiente è un aspetto importante per l’indebolimento delle informazioni epidemiologiche, delle azioni di assistenza in rete e per la realizzazione della territorializzazione e intersettoriale nella salute mentale. Azioni educative e politiche pubbliche che rafforzano al medico e alle squadre di pronto soccorso in generale l’importanza della corretta e completa registrazione delle informazioni.

Abstract The second and final phase of the EUVIS (EUrope Vaccines ICT Strategies) project, coordinated by the School of Public Health of the University Vita-Salute San Raffaele in Milan (Italy), aims at collecting best practices on the use and impact of Information and Communication Technologies (ICT) and digital tools to increase vaccine uptake and ultimately vaccination coverage in Europe. The field of ICT has flourished in recent years revolutionizing the processes of gathering, spreading and utilizing health information among healthcare providers, citizens and mass media. In particular, we are interested in any digital technology that can improve vaccine uptake supporting actions aimed at both increasing the demand of vaccines (i.e provide access to information through telecommunications, networks, the Internet, wireless, mobile devices), and the supply of effective and efficient life-course immunization services (i.e ICT-based interventions to support immunization programmes delivery and their monitoring). Within EUVIS we have previously conducted a series of systematic reviews to pool available evidence from experimental studies on the impact of selected ICT-based intervention (i.e. e-mail reminders, personal health records, among others) to improve vaccine uptake and other associated outcomes. In the current study, second and final phase of the EUVIS project, we are conducting a survey at the European level to gather original data on the use and impact of ICT and digital tools within immunization programmes in selected countries. The survey tool was developed on the basis of findings from EUVIS phase one and experts' consultation; it consisted on a 55-item questionnaire, distributed to public health professionals working in health agencies, institutions and the academia to build “ICT and immunization” countries' profile, as well as to gather, pool and critically appraise data on perceived potential and challenges of immunization programmes' digitalization in Europe.

Le tecnologie della comunicazione e dell’informazione influenzano sempre più le azioni per la salute, nonché la sicurezza dei pazienti. Così, questo studio mirava a sviluppare un’applicazione per la conferenza e il controllo di tutte le fasi della lista di controllo di Chirurgia sicura suggerita dall’Organizzazione Mondiale della Sanità per migliorare la sicurezza dei pazienti sottoposti a chirurgia. Il problema della ricerca risiede proprio nell’assenza di applicazioni mobili in grado di soddisfare la necessità di sicurezza del paziente. Questo studio viene applicato nello sviluppo tecnologico di un’applicazione con la possibilità di distribuzione in qualsiasi servizio sanitario e facile installazione su dispositivi mobili. L’applicazione è stata costruita sulla base della lista di controllo Safe Surgery stabilita dall’Organizzazione Mondiale della Sanità. L’applicazione consente l’identificazione del paziente attraverso tre identificatori. Successivamente contempla tutti gli elementi delle tre fasi della lista di controllo della chirurgia sicura: Prima dell’induzione anestetica, Prima dell’incisione chirurgica e prima che il paziente lasci la stanza. Al termine di tutte le fasi dell’applicazione, calcola il rischio per la sicurezza del paziente. L’applicazione sviluppata è uno strumento che può essere implementato negli istituti sanitari e utilizzato da professionisti che lavorano in sala operatoria.

In un mondo sempre più propenso all'uso delle nuove tecnologie, si osservano i vantaggi ottenuti dalla digitalizzazione sanitaria e dalla diffusione della mHealth, "salute mobile", pratica per la quale i dispositivi mobili fanno da tramite per gestire dati inerenti alla medicina e alla salute. In questo elaborato vengono analizzate le fasi di progettazione, sviluppo, rilascio e testing di TalApp, un'applicazione italiana per dispositivi Android, dedicata a soddisfare le necessità di persone a�ette da talassemia. Essendo quest'ultima una malattia che prevede un regime stretto di analisi e controlli, viene studiata al meglio la patologia tramite consulto di medici e pazienti, per trovare il modo adeguato di fornire ausilio alle loro esigenze. TalApp permette di annotare dati clinici, consentendo al paziente di creare uno storico delle trasfusioni, esami e terapie pregresse e future. Inoltre, fornisce un sistema di monitoraggio basato su notifiche e allarmi che sollecita gli utenti a seguire al meglio le terapie in corso e gli impegni sanitari calendarizzati. Tramite un sondaggio è stato effettuato un test di usabilità, sono state analizzate le funzionalità e il grado di soddisfazione degli utenti. L'app è stata poi rilasciata sul Play Store.

This thesis focuses on the development and adaptation of psychometric scales to evaluate the problematic use of Internet and the mobile phone in adolescents in Barcelona and London. It aims to describe the characteristics and patterns of this use, to categorize problematic users of either technology and to test category validity by developing the “Problematic Internet Entertainment Use Scale for Adolescents” and adapting the “Mobile Phone Problematic Use Scalefor Adolescents” (here after, PIEUSA and MPPUSA, respectively). It also proposes a statistical model to obtain predictive factors and estimates the prevalence of problematic users of both technologies so as to more effectively describe their psychological and social profile and examine the addictive nature of their use. The thesis takes the findings of five selective studies using convenience samples of more than 1000 secondary school students in Barcelona and a similar sample size in London, who completed questionnaires. Although the study methods were mainly quantitative and used statistical analysis, one study adopted a mixed methods approach. The results allowed the researchers to validate the scales, which demonstrated excellent internal consistency and adequate factorial and construct validity, in as much as both scales were unidimensional. The participants were categorized and the prevalence of problematic users was estimated. The cut-off points were satisfactorily tested for sensitivity, specificity and diagnostic accuracy, and the results demonstrated the presence of addictive behaviour in users of both technologies, where user profiles were also defined according to characteristics, patterns of use and addictive symptomatology. In conclusion, the thesis examined the border area between non-problematic and problematic Internet and mobile phone adolescent use, which international institutions like the American Psychiatric Association may begin to classify as mental disorders if the scientific and clinical evidence supports this.
La presente tesis doctoral se centra en la elaboración y adaptación de escalas psicométricas para evaluar el uso problemático de Internet (conocido como Problematic Internet Use o PIU) y del teléfono móvil (Problematic Mobile Phone Useo PMPU) en adolescentes barceloneses y londinenses. Se pretende diversos objetivos específicos: describir las características y patrones de uso de ambas tecnologías en estudiantes de secundaria, elaborar la Problematic Internet Entertainment Use Scale for Adolescents (PIEUSA) y adaptar la Mobile Phone Problematic Use Scale for Adolescents (MPPUSA)para ambas culturas, establecer las categorías de usuarios en ambas tecnologías y comprobar la precisión en la clasificación propuesta, así como proponer un modelo estadístico para obtener los factores predictivos y, por último, estimar la prevalencia de los adolescentes problemáticos de ambas tecnologías para poder estudiar su perfil psicosocial y tratar de abordar la sintomatología subyacente de carácter adictivo. No obstant això, el creixement d'aquestes solucions mHealth s'ha produït sense cap tipus de control o regulació específica. L’objectiu principal d’aquesta tesi era crear una nova guia per ajudar en el desenvolupament d'apps de qualitat i facilitar l'avaluació de les disponibles. La Tesi inclou tres estudis. En el primer s'han identificat els criteris més importants per a les apps de salut utilitzant diferents fonts d'informació rellevants en el camp. En el segon estudi, s’ha realitzat un procés de validació per tal de determinar els criteris realment imprescindibles i que s’integrarien a la guia. Un total de 48 criteris s’han inclòs finalment a la guia relacionats amb les temàtiques de Privacitat, Ciberseguretat, Usabilitat, Seguretat, Tecnologia, Adequació i idoneïtat, Transparència i contingut, i Suport tècnic i actualitzacions. En el tercer estudi, es va contrastar la fiabilitat (específicament la fiabilitat entre jutges) per avaluar les apps existents, i es va comparar amb la guia més utilitzada actualment. El objetivo principal de esta tesis era crear una nueva guía para ayudar en el desarrollo de apps de calidad y facilitar la evaluación de las disponibles. La Tesis incluye tres estudios. En el primero se han identificado los criterios más importantes para las apps de salud utilizando diferentes fuentes de información relevantes en el campo. En el segundo estudio, se ha realizado un proceso de validación para determinar los criterios realmente imprescindibles y que se integrarían en la guía. Un total de 48 criterios se han incluido finalmente en la guía relacionados con las temáticas de Privacidad, Ciberseguridad, Usabilidad, Seguridad, Tecnología, Adecuación e idoneidad, Transparencia y contenido, y Soporte técnico y actualizaciones. In recent years, a huge number of mobile health-related applications have been developed. However, the growth of these mHealth solutions has occurred without any specific control or regulation. The main aim of this thesis was to create a new guide to help in the development of quality apps and facilitate the evaluation of those available. The thesis includes three studies. In the first one, the most important criteria for health apps have been identified using different relevant sources of information in the field. In the second study, a validation process was carried out to determine the essential criteria that would be integrated into the guide. A total of 48 criteria have finally been included in the guide related to the topics of Privacy, Security, Usability, Safety, Technology, Appropriateness and suitability, Transparency and content, and Technical support and updates. In the third study, reliability (specifically, interrater reliability) was studied to evaluate existing apps, and results were compared with those provided when using the most used guide. The guide created: Mobile App Development and Assessment Guide (MAG), is a valid tool to both evaluate and guide the development of new quality health apps, and has proven to be more reliable than the most used guide in the area (Mobile App Rating Scale).

This thesis focuses on the development and adaptation of psychometric scales to evaluate the problematic use of Internet and the mobile phone in adolescents in Barcelona and London. It aims to describe the characteristics and patterns of this use, to categorize problematic users of either technology and to test category validity by developing the “Problematic Internet Entertainment Use Scale for Adolescents” and adapting the “Mobile Phone Problematic Use Scalefor Adolescents” (here after, PIEUSA and MPPUSA, respectively). It also proposes a statistical model to obtain predictive factors and estimates the prevalence of problematic users of both technologies so as to more effectively describe their psychological and social profile and examine the addictive nature of their use. The thesis takes the findings of five selective studies using convenience samples of more than 1000 secondary school students in Barcelona and a similar sample size in London, who completed questionnaires. Although the study methods were mainly quantitative and used statistical analysis, one study adopted a mixed methods approach. Cada vez son más las personas que buscan tener un buen estado de salud, y según la definición elaborada por la Organización Mundial de la Salud (OMS), “la salud es un estado completo de bienestar físico, mental y social, y no solamente la ausencia de afecciones o enfermedades”. Para lograr este fin, las personas deberían realizar diferentes actividades como tener una dieta saludable, una rutina de ejercicios, evitar el consumo de sustancias tóxicas, hacerse revisiones médicas periódicas, etc. Además, generalmente, una persona que ha pasado por operaciones o se ha realizado diversas pruebas médicas no lleva el control de estas. Por ello, cuando acude a una consulta médica y no cuenta con información de sus atenciones, podrían pasar por alto cuestiones relevantes para responder ante una urgencia y no se apliquen en primera instancia los cuidados adecuados. La importancia de esto se incrementa sobre todo para los padres de familia, quienes llevan control y seguimiento de las atenciones de sus hijos menores de edad.  Considerando que el cuidado de la salud cobra relevancia, se ha identificado una oportunidad de negocio orientada a contribuir en el cuidado de la salud. La alternativa que planteamos permite el registro médico personal y familiar, separación de citas y cobro de estas, ranking de médicos, alertas sobre la toma de medicamentos y citas programadas, publicidad de clínicas, farmacias y ubicación de estas. Todo ello, a través de una aplicación administrada por el paciente, quien se encargaría de registrar la información de las atenciones médicas a través de imágenes, grabaciones de voz o de manera escrita. Con la propuesta de valor planteada se busca facilitar el cuidado de salud de las personas y de sus familias.
More and more people are looking for a good state of health, and according to the definition developed by the World Health Organization (WHO), "health is a complete state of physical, mental and social well-being, and not only the absence of conditions or diseases. All this, through an application administered by the patient, who would be responsible for recording information on medical care through images, voice or written recordings. With the proposed value proposition seeks to facilitate the health care of people and their families.

Este plan de negocios busca aportar en una problemática a nivel mundial, la cual va aumentando en el número de afectados cada año, la salud mental. La OMS afirma que las funciones mentales están interconectadas con el funcionamiento físico, mental y con el estado de salud, donde por ejemplo la depresión constituye un factor de riesgo en el cáncer o en los trastornos cardiovasculares, la ansiedad puede afectar un tratamiento físico de manera sustancial. La ausencia de salud mental ha traído grandes consecuencias como suicidios en jóvenes y adultos, debido a que no fueron tratados a tiempo ya sea por el poco conocimiento de su relevancia o por el costo alto que esta enfermedad conlleva. La salud es un estado de completo bienestar físico, mental y social y no solamente la ausencia de afecciones o enfermedades. Es por todo ello que a través de este trabajo se propone un aplicativo de ayuda para la prevención, alivio y mejora de estados emocionales que pueden perjudicar nuestra salud. ¨Un día nuevo¨ app es una propuesta con programas especializados que brindará herramientas de manera integral, que acompañarán diariamente al usuario, abarcando los temas de alimentación, deporte y mente, pensamientos. Ofreciendo también un Chatbot las 24 horas para que el usuario puede encontrar alivio cuando lo necesite, a través de actividades adecuadas a las necesidades que este requiera. Para este plan de negocios nos hemos enfocado en las mujeres de 15 años a 55 años de edad, de niveles socioeconómicos A, B y C. Debido a que existe una diferencia entre géneros en cuanto a la salud mental, las mujeres son más propensas en padecer trastornos de ansiedad o depresión debido a la internalización de estos estados emocionales y como responde el cuerpo ante estas emociones a diferencia de los hombres. Con una inversión de S/128,090 proyectado a 5 años en un escenario esperado, muestra indicadores de rentabilidad con un valor neto actual (VAN) de S/740401.48 y una tasa interna de retorno (TIR) de 48% estos valores concluyen que a nivel económico el proyecto es viable y rentable.

The absence of mental health has brought great consequences such as suicides in young people and adults, since they were not treated on time either because of poor knowledge of its relevance or its high cost. Health is a state of complete physical, mental and social well-being and not merely the absence of diseases or illnesses. Therefore, through this plan, an application is proposed to help prevent, relieve and improve any emotional state that may be harmful to our health. "Un día nuevo" app is a proposal with specialized programs that will provide tools in a comprehensive manner, which will accompany the user daily, addressing the topics of food, sports and mind, and thoughts. It also offers a 24-hour chatbot so that the user can find relief when needed through activities that meet their needs. For this business plan, our focus is on women from 15 to 55 years old from A, B and C socioeconomic levels. Since there is a gender difference in mental health, women are more likely to suffer from anxiety disorders or depression due to the internalization of these emotions and the way their body responds to these emotions unlike men. With an investment of S/128,090 planned for 5 years in an expected scenario, it shows indicators of profitability with a net present value (NPV) of S/740401.48 and an internal rate of return (IRR) of 48%. These values conclude that, at an economic level, the project is viable and profitable.

En el presente trabajo, se ejecutó un plan de negocios sobre una aplicación móvil para realizar consultas psicológicas de forma virtual, creado por cinco estudiantes de la facultad de Negocios de la Universidad Peruana de Ciencias Aplicadas (UPC). El proyecto se desarrolló en base al aumento de afecciones psicológicas vistas durante el periodo del 2020 -2021 a causa de la pandemia del Covid-19 que ha afectado a la población peruana de forma relevante, llegando así a generar cambios significativos en el desarrollo de su día a día, cambio que se ve reforzado por la falta de herramientas para acceder a una consulta psicológica siguiendo las reglas de prevención contra esta enfermedad. Al analizar este sector podemos ver que existe un alto potencial de crecimiento debido a la tendencia de uso de teleconsultas en el sector salud, en especial, consultas psicológicas. Luego de evaluar las necesidades y requerimientos de nuestro público objetivo, desarrollamos una aplicación móvil que sirve como nexo entre el psicólogo y el paciente, que permite obtener una comunicación más fluida y accesible al brindar una plataforma exclusiva para realizar psicoterapia. Al finalizar nuestro proyecto de ventas, se analizaron los diferentes planes presupuestales del área de RR.HH., de operaciones, marketing y ventas para tener una proyección, en términos monetarios, de lo que necesitamos para llevar a cabo nuestra idea de negocio. Con ello, seremos capaces de saber si es aceptable invertir en él o en todo caso, aprender y crear nuevas ideas de negocio.
In the present work, a business plan was executed on a mobile application to carry out psychological consultations in a virtual way, created by five students from the Business School of the Peruvian University of Applied Sciences (UPC). The project was developed based on the increase in psychological conditions seen during the 2020 -2021 period due to the Covid-19 pandemic that has affected the Peruvian population in a relevant way, thus generating significant changes in the development of their day by day, change that is reinforced by the lack of tools to access a psychological consultation following the parameters of prevention against this disease. When analyzing this sector, we can see that there is a high growth potential due to the trend of teleconsultations in the health sector, especially psychological consultations. After evaluating the needs and requirements of our target audience, we developed a mobile application that serves as a link between the psychologist and the patient, which allows for more fluid and accessible communication by providing an exclusive platform for psychotherapy. At the end of our sales project, the different budget plans of the HR, operations, marketing, and sales areas were analyzed to have a projection, in monetary terms, of what we need to carry out our business idea. With this, we will be able to know if it is acceptable to invest in it or in any case, learn and create new business ideas.

En toda la etapa de vida, las mujeres pasan por determinadas experiencias físicas para la conformación de su femineidad como la menstruación, el parto, lactancia, embarazo y menopausia. Los estudios sobre el tema explican que las mujeres enfrentan enfermedades complejas y otras menos severas, algunas enfermedades son: cáncer de ovarios, miomas, ovarios poliquísticos, infecciones, entre otros. Siendo las infecciones la principal causa que lleva a las mujeres al ginecólogo, según el Inem, además, se sabe que mensualmente casi 7 mil mujeres acuden a una consulta ginecológica, de ellos cerca del 70% acuden solo por infecciones (Álvarez et al., 2017). Por ello, se creó Ammy, una idea de negocio que busca solucionar los problemas relacionados con la salud reproductiva femenina y que funciona a través de una aplicación móvil. El servicio consiste en brindar un monitoreo menstrual con alertador de normalidad o anormalidad del ciclo menstrual, con opción para ingresar los síntomas, de esta manera alertar a la usuaria cuando sus molestias requieran acudir a un ginecólogo, luego de ello, sugerirle un listado de médicos para realizar sus citas médicas por la aplicación de Ammy. Si bien, en el mercado existen varias aplicaciones extranjeras, lo cierto es que ninguna de ellas permite realizar las consultas médicas virtuales. Además, existen miles de mujeres en Lima que están dispuestas a pagar por este servicio y por las suscripciones para el monitoreo de su ciclo menstrual. Sin duda este negocio es innovador y muy rentable demostrado en sus indicadores de rentabilidad del presente trabajo.
Throughout the life stage, women go through certain physical experiences to shape their femininity, such as menstruation, childbirth, lactation, pregnancy and menopause. Studies on the subject explain that women face complex diseases and others less severe, some diseases are: ovarian cancer, fibroids, polycystic ovaries, infections, among others. Being infections the main cause that leads women to the gynecologist, according to the Inem, in addition, it is known that almost 7 thousand women go to a gynecological consultation every month, of them about 70% go only for infections (Álvarez et al., 2017). For this reason, Ammy was created, a business idea that seeks to solve problems related to female reproductive health and that works through a mobile application. The service consists of providing menstrual monitoring with an alert of normality or abnormality of the menstrual cycle, with the option to enter the symptoms, thus alerting the user when her discomfort requires going to a gynecologist, after which, suggesting a list of doctors to make your medical appointments by the Ammy app. Although there are several foreign applications on the market, the truth is that none of them allow virtual medical consultations. In addition, there are thousands of women in Lima who are willing to pay for this service and for subscriptions to monitor their menstrual cycle. Without a doubt this business is innovative and very profitable, demonstrated in its profitability indicators of this work.

La cultura de prevención médica ante posibles enfermedades a la salud es un tema en el que la población peruana todavía no toma conciencia. Muchos peruanos sólo acuden a hospitales o clínicas cuando les duele una parte del cuerpo o sienten que ya deberían de tratarse. Por otro lado, el obtener una cita médica con el especialista suele demorar varios días o incluso meses, en donde el paciente no tiene una solución rápida para calmar ese mal que le aqueja. A raíz de la pandemia actual, la obtención de estas citas se ha dilatado o incluso se han priorizado especialidades médicas por otras, esto ha generado la saturación en el sistema de salud. Se ha encontrado como oportunidad de negocio la creación de un aplicativo móvil denominado “Mi Doc” que cumpla la función de ser un intermediario entre el paciente y consultorios independientes (doctores), con la finalidad de reservar citas médicas en donde el usuario podrá elegir la programación de la cita según su disponibilidad, cercanía y precio.
The culture of medical prevention in the face of possible diseases is a topic that the Peruvian population is not yet aware of. Many Peruvians only go to hospitals or clinics when they are in pain or feel they should be treated. On the other hand, obtaining a medical appointment with a specialist usually takes several days or even months, where the patient does not have a quick solution to calm this disease that afflicts him. Due to the current pandemic, obtaining these appointments has been delayed or even prioritized by other medical specialties, which has generated saturation in the health system. In less than three decades, Nokia emerged from Finland to lead the mobile phone revolution. It grew to have one of the most recognizable and valuable brands in the world and then fell into decline, leading to the sale of its mobile phone business to Microsoft. This book explores and analyzes that journey and distills observations and lessons for anyone keen to understand what drove Nokia’s amazing success and sudden downfall. It is tempting to lay the blame for Nokia’s demise at the doors of Apple, Google, and Samsung, but this would be to ignore one very important fact: Nokia had begun to collapse from within well before any of these companies entered the mobile communications market, and this makes Nokia’s story all the more interesting. Observing from the position of privileged outsiders (with access to Nokia’s senior managers over the last twenty years and a more recent, concerted research agenda), this book describes and analyzes the various stages in Nokia’s journey. This is an inside story: one of leaders making strategic and organizational decisions, of their behavior and interactions, and of how they succeeded and failed to inspire and engage their employees. Perhaps most intriguingly, it is a story that opens the proverbial “black box” of why and how things actually happen at the top of organizations. Why did things fall apart? This chapter describes the impact of giant multimedia conglomerates on internet services and applications. It first considers the media conglomerates that had dominated the political economy of communications and that now continued to help structure its capital logic. In particular, it looks at the competition presented not only by broadband and mobile internet operators but also by well-financed outsiders and upstarts such as Google and Facebook. It then examines three possible forms of revenue generation for communications and media beyond financing by venture capital and the sale of stock: direct payments, advertising, and noncommercial support, either through governmental or philanthropic finance or voluntary donations. The chapter concludes with a discussion of changes in specific media industries such as the book publishing industry, e-book industry, television industry, music industry, and film industry.

AbstractCO2 geological sequestration has been proposed as a climate change mitigation strategy that can contribute towards meeting the Paris Agreement. A key process on which successful injection of CO2 into deep saline aquifer relies on is the dissolution of CO2 in brine. CO2 dissolution improves storage security and reduces risk of leakage by (i) removing the CO2 from a highly mobile fluid phase and (ii) triggering gravity-induced convective instability which accelerates the downward migration of dissolved CO2. Our understanding of CO2 density-driven convection in geologic media is limited. Studies on transient convective instability are mostly in homogeneous systems or in systems with heterogeneity in the form of random permeability distribution or dispersed impermeable barriers. However, layering which exist naturally in sedimentary geological formations has not received much research attention on transient convection. Therefore, we investigate the role of layering on the onset time of convective instability and on the flow pattern beyond the onset time during CO2 storage. We find that while layering has no significant effect on the onset time, it has an impact on the CO2 flux. Our findings suggest that detailed reservoir characterisation is required to forecast the ability of a formation to sequester CO2.

In this paper the authors propose a Secure and Optimized Proximity Mobile Payment (SOPMP) Framework using NFC (Near Field Communication) technology, WPKI (Wireless Public Key Infrastructure), UICC (Universal Integrated Circuit Card). The novelty of this proposed mobile payment framework is messages are exchanged in the form of Digital Signature with Message Recovery (DSMR) and merchant sends Invoice in the form of Digital Invoice Certificate (DIC) (which is digitally signed by the merchant). The communication link between mobile phone and merchant POS (Point Of Sale) is NFC. Digital Signature with Message Recovery based on ECDSA eliminates the need of adopting PKI cryptosystems thereby reducing the consumption of resources i.e. it consumes less computational and communication cost. DSMR eliminates the need of certificates validation and removes the hurdle of PKI thereby reducing storage space, communication cost and computational cost. The authors proposed protocol ensures Authentication, Integrity, Confidentiality and Non Repudiation, achieves Identity protection from merchant and Eavesdropper, achieves Transaction privacy from Eavesdropper and Payment Gateway, achieves Payment Secrecy, Order Secrecy, forward secrecy, and prevents Double Spending, Overspending and Money laundering. In addition to these our proposed protocol withstands Replay, Man in the Middle and Impersonation attacks. The security properties of the proposed protocol have been verified using AVISPA and Scyther Tools and presented with results.

An old saying coming from the telecom world states that nothing can be really considered as a service unless you are able to charge for it. The last several years have seen a boom in interest in mobile commerce, mainly due to the high penetration rates of mobile phones. Furthermore, there is evident the need for a real-time, open, and trusted payment service that can be used any time, anywhere, and that can handle any transaction in any currency. Such a service would promote not only content creating activities but would empower the electronic and mobile commerce area and kick-start new innovative services. The time is right for such a mobile payment service, because the infrastructure, the business models, and other conditions that favor its existence are realistic and in place (Vilmos & Karnouskos, 2004). Up to now, we have witnessed the rise and fall of several efforts in the area, ranging from realizing simple intangible good purchases, up to interaction with real points of sale (POS) and person-to-person (P2P) transactions. Day by day, new trials are initiated, targeting different sections in the MP area; however, there is still no solution that is open and widely accepted. In this article, we first introduce the reader to the mobile payment area, present the guiding forces behind it, and subsequently examine such an open, secure mobile payment approach that has been successfully designed, implemented, and tested. Furthermore we identify some midterm future trends that we consider will be of high importance to the further development of the area.

The purpose of this research is to analyse penetration and use of mobile payments in Baltic countries and Poland in point of sale (POS) segment. Mobile payment or m-payment (MP) is referred to the transfer of money (in digital form) from one party (e.g., consumer) to another party (e.g., seller or merchant) using a mobile device. Mobile payments allows to pay for goods or services with mobile devices instead of paying with cash or physical credit cards. The statistical data helps us to know information about mobile payments growth. The article concerns analysis of mobile payment use in POS systems. The penetration of mobile payments in countries is different. Lack of scientific information and novelty of this article provided by this research include methods for multi criteria decision support by applying SAW and TOPSIS methods. In this study case we demonstrate the evaluation of mobile payment use in Lithuania with comparison with other Baltic countries and Poland. The SAW and TOPSIS methods supply the structure of decision making which can help us to evaluate the penetration of mobile payments.

Abstract In this paper, we describe an approach to designing monitoring schemes for carbon dioxide sequestration in saline aquifers. Changes in key parameters are investigated over timescales of up to a thousand years. The study addresses movement of the CO2 plume, possible locations for observation wells and the period for which a storage location should be monitored. For the initial sensitivity analysis, we use a simple homogeneous reservoir simulation model to understand how reservoir, operational and model parameters affect the amount of mobile CO2 remaining at different times over the storage period. The parameters with the greatest impact are taken forward to uncertainty studies, which are conducted on two reservoir models with more realistic geological characteristics: one with lateral extensive baffles and one with sand channels. For these cases, we investigate the movement of the CO2 plume and its arrival at possible locations for an observation well. Results from the sensitivity analysis indicate that the most influential parameters are horizontal permeability, dipping angle, critical gas saturation, salinity, the period of injection and the capillary pressure curve. The results from the uncertainty studies indicate that for the two heterogeneous models, a reasonable monitoring period is in the range of 60 to 150 years and that the movement of the plume probably stops after approximately 100 years. The arrival time of CO2 at the observation well can be predicted with greater confidence when the well is in close proximity to the injector and in the direction in which CO2 will preferably move. A correlation analysis on the uncertain parameters shows that the main contributor affecting the amount of mobile CO2 is critical gas saturation, followed by dipping angle and the period of injection. While previous studies focus on how different parameters affect immobilization of CO2, this study aims to develop a methodology to plan long-term monitoring of mobile CO2. Prediction of the expected plume movement can help to determine suitable observation well locations and reasonable timescales for the monitoring process.
