Abstract:
The increase of psychosomatic disorders due to cultural changes requires enhanced therapeutic models. This study investigated a salutogenetic treatment concept for inpatient psychosomatic treatment, based on data from more than 11000 patients of a psychosomatic clinic in Germany. The clinic aims at supporting patients’ health improvement by fostering values such as humanity, community, and mindfulness. Most of patients found these values realized in the clinical environment. Self-assessment questionnaires addressing physical and mental health as well as symptom ratings were available for analysis of pre-post-treatment effects and long-term stability using one-year follow-up data, as well as for a comparison with other clinics. With respect to different diagnoses, symptoms improved in self-ratings with average effect sizes between 0.60 and 0.98. About 80% of positive changes could be sustained as determined in a 1-year follow-up survey. Patients with a lower concordance with the values of the clinic showed less health improvement. Compared to 14 other German psychosomatic clinics, the investigated treatment concept resulted in slightly higher decrease in symptoms (e.g., depression scale) and a higher self-rated mental and physical improvement in health. The data suggest that a successfully implemented salutogenetic clinical treatment concept not only has positive influence on treatment effects but also provides long-term stability.

In einer aktuellen Literaturreview konnte gezeigt werden, dass die Frauen nach der Entlassung aus der Klinik langfristig, salutogenetisch und kontinuierlich in ihrem Umfeld begleitet werden sollten, um die Spätfolgen für Mutter und Kind zu minimieren.

Als wichtige Schutzfaktoren gelten psycho-soziale Ressourcen wie z.B. Proaktivität, soziale Kompetenzen und Selbstwirksamkeitserwartung. Dabei ist Lebenskompetenz nicht nur als individuell trainierbar zu betrachten, sondern auch als durch gesellschaftliche Rahmenbedingungen beeinflussbar.

Background: Recent studies have proved the relevance of salutogenetic variables for fatigue management in breast cancer survivors with cancer-related fatigue (CRF). This comprehensive cohort design study is the first to examine the impact of 2 multimodal therapies, multimodal therapy (MT) and combined therapy (CT), compared with standard aerobic training (AT) on salutogenetic variables (self-regulation and internal coherence) and distress in breast cancer survivors with CRF. Methods: A total of 105 patients started the therapies and n = 84 completed the Self-regulation Scale, the Internal Coherence Scale, the Cancer Fatigue Scale, and the Hospital Anxiety and Depression Scale at baseline, 10 weeks after treatment (T1) and n = 81 after 6 months (T2). Patient satisfaction and qualitative feedback regarding therapy quality was assessed at T1. A general linear model including allocation type, therapy arm (MT/CT/AT), and bias-adjusting propensity scores tested the superiority of both multimodal therapies versus AT for all questionnaires at T1 and T2. Results: MT and CT were superior to AT to improve self-regulation and patients’ satisfaction at T1. Additionally, CT showed superiority for self-regulation at T2 (all P < .05). Compared with AT, internal coherence was significantly higher for patients in the MT arms at T2, respectively (all P < .01). Pearson’s correlations between self-regulation, internal coherence, and CRF improved from baseline to T2 (Mean r = −0.60). Qualitative feedback confirmed patients’ benefits in several health-related categories. Conclusions: Self-regulation and internal coherence are manipulable variables with relevant CRF associations. They can be positively affected by multimodal therapies. Alongside patients’ satisfaction and qualitative feedback they help refine treatment.

Background: Integrative medicine focuses on the human being as a whole—on the body, mind, and spirit—to achieve optimal health and healing. As a synthesis of conventional and complementary treatment options, integrative medicine combines the pathological with the salutogenetic approach of therapy. The aim is to create a holistic system of medicine for the individual. So far, little is known about its role in plastic surgery. Hypothesis: We hypothesize that integrative medicine based on a conventional therapy with additional anthroposophic therapies is very potent and beneficial for plastic surgery patients. Evaluation and consequence of the hypothesis: Additional anthroposophic pharmacological and non-pharmacological treatments are promising for all areas of plastic surgery. We are convinced that our specific approach will induce further clinical trials to underline its therapeutic potential.

ABSTRACTThere is lack of research on old indigenous women's experiences. The aim of this study was to explore how old women narrate their experiences of wellbeing and lack of wellbeing using the salutogenetic concept of resilience. Interviews from nine old Sami women were analysed according to grounded theory with the following themes identified: contributing to resilience and wellbeing built up from the categories feeling connected, feeling independent and creating meaning; and contributing to lack of lack of resilience and wellbeing built up from the category experiencing lack of connectedness. The old Sami women's narratives showed that they were to a high extent resilient and experienced wellbeing. They felt both connected and independent and they were able to create meaning of being an old Sami woman. Having access to economic and cultural capital were for the old Sami women valuable for experiencing resilience. Lack of resilience was expressed as experiences of discrimination, lack of connectedness and living on the border of the dominant society. Analysis of the Sami women's narratives can give wider perspectives on women's health and deepen the perspectives on human resilience and increase the understanding of minority groups in a multicultural world.

Nonspecific low back pain (NSLBP) is an important health issue of our time. Personal as well as economic factors, like suffering pain and experiencing disability on the one hand and enormous and still increasing costs to the economy and society on the other hand, display the importance of the matter. Tremendous research has been conducted in the last few decades on NSLBP. A PubMed search (June 17, 2013) on “low back pain” provided 22,980 hits, and when specifying for “low back pain, systematic review,” 3,134 hits were still generated. Most research has been done examining the development, risk factors, or therapeutic measures of NSLBP, but hardly any literature exists on resources related to NSLBP. The aims of this review are twofold. In order to shade light on the salutogenetic approach of NSLBP, and thus to focus on health instead of illness, the first aim is to facilitate the understanding of which therapeutic measures enhance the ability to cope with chronic NSLBP and enable (more) normal functioning in life. The second aim is to stimulate the understanding of resources protecting against the onset of NSLBP or against the development of chronic NSLBP and its resulting work absence.

In this aspect the study of both risk and protective factors for human health (in a salutogenetic perspective) becomes increasingly fundamental. The issue of population at an advanced age has definitely been considered as an independent subject since the beginning of the 21st century. Moreover, the society perceives it as an essential basis for further progress and flourishing of the mankind. Pursuing your life mission is often very difficult, and many frustrations are experienced along the way. Major failures to bring out our potential can cause us considerable emotional pain. When this pain is unbearable, we are induced to shift from one intention and talent to another that better allows us to adapt and survive. Thus, we become set on a course that brings out a secondary or tertiary talent instead of the primary talent. This talent displacement may be expressed as a loss of our true nature or true self. The new purpose in life now functions as the core of a new personality: the ego. The ego has a structure similar to that of the true self. It is anchored in a talent and it draws on subtalents. But the person who is centered in his or her ego is not as powerful or talented as the person he or she originally was, living the primary purpose of life. This is because the original personality (the true self or “higher self”) is still there, active and alive, behind the ego. Symptoms, disorders, and diseases may be explained by the loss of energy, joy in life, and intuitive competence because of inner conflicts, which may be alleviated or cured in the salutogenetic process of Antonovsky that helps patients find their sense of coherence or their primary purpose in life. Many cases of reduced ability to function, physically as well as psychologically, socially or sexually, can also be explained and alleviated in this way. When a person discovers his true talent and begins to use it with dedication, privately as well as professionally, his life will flourish and he may overcome even serious disease and great adversity in life. The salutogenetic process can also be called personal development or “quality of life as medicine”. It is important to note that the plan for personal development laid out by this theory is a plan not for the elimination of the ego, but for its cultivation. An existentially sound person still has a mental ego of course, but it is centered on the optimal verbal expression of the life mission. Such an ego is not in conflict with one�s true self, but supports the life and wholeness of the person, although in an invisible and seamless way. The more developed the person, the more talents are taken into use. So although the core of existence remains the same throughout life, the healthy person continues to grow. As the number of talents we can call on is unlimited, the journey ends only at death. Understanding the concept of the ego, it is very easy for the physician to motivate the patient to go through a lot of difficulties in order to grow and develop, and when the patient fully understands the concept of the ego and the true self (higher self), the patient gets a strong feeling of direction in personal development, and a motivation to fight the internal obstacles for quality of life, health, and the ability to function.

The present study applies a salutogenetic approach to psycho-social stress and wellbeing at work and for the first time analyzes the relation of an extended model of four work-related behavior and experience patterns to work related perceptions, like work ability, job satisfaction and turnover intention, or engagement. Employees of an international financial services company (N = 182) completed the questionnaire Work-related behavior and experience pattern (Arbeitsbezogenes Verhaltens- und Erlebensmuster; AVEM). The AVEM has oftentimes been used for research in helping professions, but research in non-helping professions is scarce. In addition to the AVEM, measures of job satisfaction, work ability, work engagement, presenteeism, and turnover intention were included in this study. Almost half (46.2%) of the sample showed a rather unambitious attitude towards work, followed by a burnout-related risk pattern (22.0%), a healthy pattern (19.8%), and a pattern at risk for overexertion (12.1%). Significantly more favorable scores were found for all work-related perceptions in participants with the healthy pattern compared to those with the burnout-related risk pattern, except for turnover intention where no significant differences were found. For work ability and vigor, those with a healthy pattern also had significantly higher scores than those with an unambitious pattern and a pattern at risk for overexertion. Being at risk for burnout not only affects job-related wellbeing and coping resources, but also work ability and work engagement. A need for personnel and organizational development and health promotion is indicated by a high number of individuals with reduced working motivation and risk patterns for overexertion or burnout.

Abstract:
The original aim of this study was a follow-up assessment of a recreational program running for six months (September 2019–February 2020) within controlled conditions. Following the arrival of the COVID-19 pandemic, the survey acquired a new goal: how do the subjects of the follow-up sampling experience this severe stress situation, and in this experience, what role does physical activity and a salutogenetic sense of coherence play. Austrian women (N = 53) took part in the training program, whose physical condition was assessed before the start of the program, then reassessed after three months and after six months; the organizers also had them fill out the sense of coherence questionnaire (SOC) as well as the Regensburger insomnia scale. After the lifting of the lockdown introduced due to the pandemic, participants completed an online survey relating to their changed life conditions, physical activities, sense of coherence and sleep quality. Results: After the first three months of the training, no significant changes were detected. After six months, the participants SOC and sleep quality improved (Friedman test: p = 0.005 and p < 0.001). During the lockdown, sleep quality generally deteriorated (W-rank test: p = 0.001), while SOC did not change. The women in possession of a relatively stronger SOC continued the training (OR = 3.6, CI 95% = 1.2–12.2), and their sleep quality deteriorated to a lesser degree. (OR = 1.7, CI 95% = 1.1–2.8). Conclusion: The data reinforce the interdependency between physical exercise (PE) and SOC; furthermore, the personal training that the authors formulated for middle-aged women proved to be successful in strengthening their sense of coherence, and it also reduced the deterioration in sleep quality due to stress.

Abstract Background Positive mental health (PMH) is a factor of far-reaching salutogenetic importance. The present study aimed at validating the Persian version of the Positive Mental Health Scale (PMH-Scale). Methods Reliability and validity of the Persian version of the PMH-Scale were established in an Iranian student sample (N = 573). Internal consistency, convergent and discriminant validity were investigated, and exploratory factor analysis was conducted. Furthermore, it was assessed how PMH scores moderate the association between depressive symptoms and suicide ideation/behavior. Results The Persian version of the PMH-Scale was shown to have a unidimensional structure with excellent internal consistency, as well as good convergent and divergent validity. PMH differentiated between participants with higher vs. lower suicide risk. Furthermore, PMH proved to moderate the association between depressive symptoms and suicide ideation/behavior. Conclusions The results suggest that the PMH-Scale is a brief, reliable, and valid measure of subjective and psychological well-being that can be used in Iranian student samples and research settings.

Abstract:
The COVID-19 pandemic is also called a crisis of uncertainty because of so many unforeseeable events like canceled qualification competitions, loss of training facilities, and postponement of the Olympic games. Athletes and their entourage experience this uncertainty as stressful. Sport psychology practitioners (SPPs) are in a key position to support athletes in coping with these unforeseeable stressors. However, SPPs are similarly affected by the COVID-19 pandemic and simultaneously have to cope with stress. Salutogenesis, which describes how to manage stress and to stay well, provides a theoretical approach to how to cope with uncertainty. The salutogenetic approach aims at strengthening individuals’ sense of coherence (SoC) and consists of three components, namely comprehensibility, manageability, and meaningfulness. Although it is known that the SoC can be enhanced via psychological skills training, so far, this approach has not been systematically applied to the elite sport context. Athletes have been advised to see SPPs for help; thus, the question of how SPPs handle the time of uncertainty while supporting others emerges. The aim of this contribution was to outline how the salutogenetic approach can be applied to strengthening SPPs’ SoC via a single-day four-part workshop. Additionally, we applied the workshop to N = 26 volleyball coaches and evaluated the workshop’s effects on participants’ psychological aspects [i.e., the Sense of Coherence—Leipziger short version (SoC-L9), resilience (RS-13): coping with uncertainty, affective response, and stress via semantic differentials] and the workshop’s quality ratings (i.e., Quality Questionnaire for Sport Psychological Coaching, QS-17). The evaluation provides results that show a positive impact on a descriptive level of the participants’ SoC, uncertainty, affect, and stress perception; however, the results show no significant main effect of time [F(8, 10) = 1.04, p = 0.467, ηp2 = 0.454]. Workshop quality (on average, 3.60 ± 0.35 out of 4.00) and skill acquisition (on average, 3.00 ± 0.64 out of 4.00) were positively evaluated; 82.00% of the participants would use the learned tools in the future. Thus, we outline how this workshop might help strengthen SPPs’ SoC and at the same time empower them to strengthen their athletes’ SoC. Overall, we add a theoretical (i.e., salutogenesis in sports) and a practical perspective (i.e., coping techniques based on salutogenesis) on how to cope with the COVID-19 pandemic for SPPS, athletes, and their support network.

Nella teoria bio-psico-sociale, la visione salutogenetica (Antonowsky, 1979) è volta a promuovere il benessere. La promozione della salute indica un processo di cambiamento e sviluppo adattivo diretto alla prevenzione primaria, secondaria e terziaria attraverso la diffusione di stili di vita sani. Nel presente lavoro si propone un modello di salutogenesi: “I Salotti del Ben-Essere” (S. Diamare, 2015), sperimentato dall'ASL NA 1 Centro UOC. Qualità e Umanizzazione, che prevede lo gv. sviluppo di tematiche di salutogenesi in laboratori esperienziali di consapevolezza psico-corporea, implementato in Campania attraverso la rete OMS di Promozione della Salute: Health Promoting Hospitals & Health Service (HPH & HS). Quale approccio olistico di Health Advocacy, I Salotti del Ben-Essere© rappresentano uno spazio multicentrico e multiprofessionale di assistenza partecipativa, rivolta a piccoli gruppi ed a comunità, in cui un team specializzato realizza un programma multidisciplinare di educazione sanitaria prevalentemente in un setting di riabilitazione. L’educazione alla salute trova, durante un trattamento riabilitativo, la collocazione più proficua per sostenere la motivazione al cambiamento consapevole di stili di vita perniciosi. Il metodo "Salotti del Ben-Essere" può essere utilizzato, come Peer Education, anche nel sistema universitario. Questa metodologia è stata testata, infatti, con gli studenti di Scienze dell'Educazione presso l’Università Suor Orsola Benincasa. I risultati sono stati soddisfacenti in tutti i contesti.

AbstractBackgroundA broad range of studies show that medical students often have a significantly deteriorated mental health status. Although starting medical school with values comparable to the population average, only a few semesters later, medical undergraduates show increased rates of psychological risk states and some manifest mental illnesses, such as burnout and depression. In our survey we intentionally assessed mental health parameters from a salutogenetic, i.e. resource-oriented point of view.MethodsWe examined first-year medical students in a cross-sectional study and assessed sense of coherence (SOC) and resilience as parameters from the salutogenesis model by Antonovsky in a structured way using validated, self-administered questionnaires. In total, we examined 236 students of human medicine, dentistry and molecular medicine at the Friedrich-Alexander-University Erlangen-Nürnberg (FAU).ResultsOur analyses showed significantly higher values of resilience among male students compared to female students (p < 0.01). In contrast, even though a significant correlation between resilience and SOC was observed, only a non-significantly lower value of SOC was found in female students. Compared to the reference sample our medical students in their first year of study showed significantly lower values for resilience (p < 0.01) and SOC (p < 0.01).ConclusionResilience and SOC are known to correlate with psychological stress (burnout parameters) and depression. In order to keep protective factors like SOC and resilience in medical students at a good and healthy level we see the necessity to address that problem proactively and educationally. Integrating training focused on the preservation of the students´ own mental health into the medical curriculum from the beginning of university courses, and throughout the whole medical study course, is essential and should be an obligatory training goal. Based on our study results, we also deem it necessary to think about ways to adapt the measures for the gender-specific needs of our students, e.g. dependent on their biological gender.
