Full textBackground: Nursing theorist Florence Nightingale mentioned in the 1800s the idea of using animals in care. Research done in adults where the dog has been involved has shown positive effects on blood pressure, heart rate and apathy. Research also show that when a human pats an animal the hormone oxytocin is released, which is a soothing hormone that makes us feel good. Longitudinal studies have shown the positive effects of having a dog in long-term care facilities, both stationary and visiting dog. The psychological effects detected were improved mood and experiences of support and security. Purpose: The purpose of this literature review was to investigate the dog's effect on older people living in long-term care facilities. Method: A literary review where 12 articles were selected and reviewed according to Olsson & Sörensen review protocol. Result: The results of the analysis were divided in three main categories: Physical effects, Psychological effects and Social wellbeing. Conclusion: Dogs can have positive effects on the wellbeing of elderly people living in long-term care facilities.Keywords: Animal assisted intervention, dog, effect, elderly, long-term care facility.

Full textMedellivslängden har ökat i Sverige under hela 1900-talet och år 2010 förväntades de som är 65år eller äldre öka med 13 procent och de som är 85 år öka med 20 procent. Med dagens medicinska kunskap och effektivare behandlingar gör att människor lever längre med kroniska sjukdomar. Detta leder till att allt fler äldre kommer behöva vård. Syftet med denna studie är att belysa och sammanställa forskning som beskriver de äldres erfarenheter av att leva och hantera sina liv på särskilt boende. En litteraturöversikt på aktuella vetenskapliga studier har genomförts. Datamaterialet består av tio vetenskapliga artiklar. Resultatet har sammanställts under tre områden:Respekt och värdighet, Betydelser av möten och relationer och Hantera det dagliga livet . I resultatet framkom det att respekt och värdighet hörde samman med till vilken grad de boende kunde sköta sig själva. Det fanns många brister i mötena mellan de boende och vårdpersonalen. Möjlighet till ett socialt liv så som kontakt med familjen och vänner är begränsad genom deras fysiska begränsningar. För att kunna hantera vardagen vände de boende sig inåt och skapade en mening i sin tillvaro. För sjuksköterskan är det viktigt att vara medveten om att äldre befinner sig i en väldigt utsatt situation och utifrån det anpassa vård och omsorg. Det behövs mer kvalitativ forskning i hur de äldre upplever att leva på särskilt boende för att kunna förbättra deras levnadsvillkor, välmående och individanpassa äldrevården.

Full textThe number of elderly people in the world is increasing and the elderly population is getting healthier. Despite this, it is uncommon for elderly people to be able to manage on their own until the end of life. When it isn't possible to cater for the needs of the elderly at home, the society has a responsibility to offer them room at a nursing home. Dependency on others doesn't mean the quality of life for the elderly has to be reduced and it's been found that there is no connection between rising age and low quality of life. The aim of this study was to describe what quality of life is for elderly people living in nursing homes. This literature study is based on eleven scientific articles that were analysed using an inductive approach. Four areas that were important for quality of life for the elderly emerged from the results: relations, identity, activities and environment and organisation. The elderly wanted close relationships with the staff and wanted to be seen as unique individuals. They wanted to participate in meaningful activities on their own terms and live in a homelike environment. The nurse can strengthen quality of life for the elderly be taking these areas into account. More education in the matter for general nurses and more qualitative research on the subject of quality of life for the elderly is needed to increase the nurses knowledge and understanding of quality of life for elderly people in nursing homes.

Full textTidigare studier visar att människor som bor på särskilt boende ofta lider av depression och ensamhet. Personalen som jobbar på särskilt boende är ofta upptagna med de dagliga sysslorna och tiden för social interaktion med de boende är ofta begränsad. Detta kan resultera i en känsla av övergivenhet och ångest hos den boende. Genom århundradena, så har människan alltid använt sig av husdjur som ett sällskap och tröst. Studier visar att den generella hälsan förbättras och stärks genom umgänget med djur. Syftet med denna studie var att beskriva vårdpersonalens uppfattningar av vad ett husdjur på särskilt boende kan ha för inverkan eller betydelse för vården av de boende. En kvalitativ intervju studie genomfördes för att undersöka studiens syfte. Sju undersköterskor intervjuades. Resultatet visar att husdjur har en positiv påverkan på människan. Det stärker glädjen hos människor omkring och skapar en avslappnande atmosfär. Vårdtagarna på särskilt boende, där ett husdjur bor permanent på avdelningen har uppvisat en lugnande, positiv effekt. Hundar och katter är bland de mest populära sällskapsdjuren på grund av tillfredställelsen det ger att klappa och dess vänliga natur. Det finns dock även nackdelar med husdjur på särskilt boende. Allergier, rädsla för vissa djur och risken för att djuret kan orsaka skada eller att djuret själv kan ta skada förekommer. Författarna anser att det finns behov av fortsatt forskning om husdjur inom vården av äldre.
Former studies show that people living in nursinghomes are often suffering from feelings of depression and loneliness. The staff working at the nursing homes is often occuped with daily chores and the time for social interaction with the residents are limited. This results in feelings of desolations and anxiety. Troughout the centuries, man have always depended on pets as a company and comfort. Studies show that general health improves when interaction with animals occur. The purpose of this study was to describe the staff’s opinion of a pet in nursinghomes and what influence or meaning it may have on the care of the residents. A qualitative interview study was made to examen the purpose of the study. Seven assistant nurses were interviewed. Results show that pets have a positive affect on human beings. They often make people around them feel more joyful and relaxed. The residents in nursing homes, who have experienced a pet living with them at the unit, have showed a calming, positive effect in their behaviour. Dogs and cats are among the most popular animals as a company because of the satisfaction of stroking and its friendly nature. Allergy, fear for some animals and the potential risk for the animal to cause injury to it self or others occurs. The authors acknowledge the need for futher research about animals involved in caregiving.

Full textBackground: Risk of malnutrition and long night fasting for elderly people living in special housing is something that has been noted according to current research. The reason why the subject feels important to study further is that a night fast over 11 hours can be the first step to malnutrition. Malnutrition, in turn, can cause physical suffering and increases the risk of fall injuries, infections and severe wounds. Aim: The purpose of the study was to elucidate district nurses´ experiences of nutrition care in special housing for the elderly. Method: The study was conducted according to a qualitative inductive approach using semistructured interviews. Eight district nurses were interviewed. The data collection was analyzed based on a qualitative content analysis. Results: Analysis of the result generated three main categories and seven subcategories. Team collaboration plays an important role in the nursing work, both with regard to communication and the role of the district nurse. Management´s support or sometimes lack of support and organizational issues have an impact on how nutrition work is prioritized in the various health care professions. Participation includes both the elderly peolpe and the nursing staff. The person-centered care is reflected in how participation is utilized and used for a positive purpose. Conclusion: A well-functioning collaboration and good communication between all health care professionals involved is required to create good care for the elderly people and to avoid suffering in the form av malnutrition and long night fasting.

Full textBakgrund: Ofrivillig ensamhet har skadliga effekter på äldres fysiska och psykiska hälsa. Få intervjustudier har undersökt äldres upplevelse av ensamhet på särskilt boende (Säbo). Syfte: Syftet med studien var att belysa tankar om ensamhet hos individer över 70 år, på särskilt boende (Säbo) samt deras upplevelse av ensamhet relaterat till boendet. Metod: Studiedesignen var kvalitativ med en induktiv ansats. Semistrukturerade intervjuer med intervjuguide med 13 individer 70-98 år, genomfördes. Texten analyserades och tolkades med kvalitativ innehållsanalys. Resultat: Fem huvudkategorier framkom; Flytten till Säbo kopplat till ensamhet, Individuella aspekter av ensamhet, Ensamhet och den åldrande kroppen, Betydelsen av sociala kontakter för upplevelsen av ensamhet samt Individuella strategier att hantera ensamhet. Slutsats: En del äldre besvärades av ensamhet och efterfrågade en ökad gemenskap i samband med måltid, i form av gemensamma aktiviteter i grupp. Det var vanligt att de äldre med stigande ålder inte deltog i alla aktiviteter och istället valde att vara för sig själva, i en positiv ensamhet.

Full textToday older people’s health and life quality are current topics of conversation in society. The number of old people is increasing and their health is deteriorating, that is why there is a need of new ways of meeting their necessities. In this study we have tried to illustrate heads of districts reasoning concerning pets’ significance for old people in special housing. The purpose of the study is to investigate the management’s awareness and preparedness around and also management of pets in special housing in a county. We have also had a look at the practical work with pets in special housing. The material in the study comes from qualitative interviews. One geriatric care manager and six heads of districts with the responsibility of special housing for old people under municipal management have been interviewed. In our interpretation we have used Brülde’s theory (2003) of life quality called “a mixed theory of happy character”. Brülde combines elements from hedonism, the theory of fulfillment of wishes and from the objective pluralism to make his own theory. The result from the study shows that all heads of districts are positive to have pets in special housing and that all special housing has or has had pets in some form. The contact with pets gives many old people a better life quality with happiness, an increased well-being and decreases the sense of loneliness.

Full textBackground: The World population is getting older. Today is 1 of 11 persons over the age 65 but in 2050 it will be 1 of 6 persons aged over 65. The number of people over 80 years old requires efforts in the form of home service, housing for elderly, day care or short-term housing are increasing. This will be one of the biggest challenges for social support, health care and welfare in the future. Aim: To describe how older persons experience what it is like living in housing for elderly.Method: A descriptive literature study based on 12 scientific articles with a qualitative approach. The searches for articles have been done in the database PubMed. Main results: The move to housing for elderly had an impact on their feelings for freedom. Living in the housing for elderly felt like a long wait for death and many felt lonely and abandoned. The elderly felt like they were limited and experienced that no one listened to their opinions. Locked doors in the environment perceived as a limitation and the elderly felt like they were in prison. Activities seemed like important for the wellbeing, both together with others and alone. To be able to move freely, being close to nature, outdoor life and being able to take a walk outside was important. Relatives being a part of the elderly´s life was important for their wellbeing. The way the nurse treated them had a big impact on how they enjoyed living in the housing for elderly. Conclusion: The elderly experienced the activities or training sessions together with others or on their own was a positive experience. Relatives being a part of the elderly´s life was highly important. The experience of closed doors made the elderly feel like they were locked, and they had a wish of be able to be outside more often. Many had a feeling of loneliness in the housing for elderly and that it was just a long wait for death. Even If they had a lot of people around them in the housing for elderly. It is important for the elderly that the nurse takes her leadership role and guides the staff to have a good attendance in all situations. For the nurse to get a better sight of what is affecting the elderly´s experience of living in nursing homes, more research is needed.

Full textUppsatsen syftade på att ge en bild av hur personalen arbetade och bemötte de äldre som
bodde på ett särskilt boende. Intresset för bemötande väcktes när jag under min praktik som
enhetschef kom i nära kontakt med en Lex Sarah anmälan. Jag ville undersöka om de äldre
hade möjligheter att påverka sin vardag eller om de måste rätta sig efter hur personalen
styrdes av rutinarbetet, samt att se hur de äldre blev bemötta på det särskilda boendet. När en
person behöver bo på ett särskilt boende, är de trots vad socialtjänstlagen säger, i en beroende
ställning till personalen som sköter om dem. För att uppnå mitt syfte blev valet av metod att
göra en öppen observationsstudie. Resultatet blev analyserat utifrån mina teorier, som var
symbolisk interaktionism, organisation samt till relevant litteratur. Huvudresultatet var min
observationsstudie på ett särskilt boende för äldre som hade behov att få hjälp att klara sin
vardag. Frågeställningen var: Hur arbetar personalen? Kan den äldre påverka sin vardag? och
Styr rutiner/schema arbetet med den äldre? Min observation visade på hur personalen på det
särskilda boendet arbetade och bemötte den äldre, samt att vissa delar i vardagen kan den
äldre bestämma över. Rutiner och schema styr det mesta av vad som sker under dagen.

Sometimes an individual has to move to a nursing home, related to special needs. To some people this can be a hard thing to adjust to and may lead to loneliness and depression. Services offered to the elderly should provide for the physical, psychological and social needs. This is why it’s in our interest to increase the knowledge, to be able to provide a positive change, which could be a dog. Objective: The aim of this study was to investigate if and how dogs affect elderly people when they are living in a nursinghome. Method: A literature study based on the 13 scientific papers. Results: The result presented three categories of Social impact, Psychological impact and Physical impact. Conclusion: The scientific studies showed different results, but assumptions can be made regarding dogs impact on people needs in their everyday life.

Full textThe aim of this thesis was to explore, describe and understand elderly people's experience of well-being, based on interviews and narratives and told in situ at nursing homes. This thesis is based on two studies, I) Elderly people’s descriptions of becoming and being respite care recipients. II) Elderly women’s subjective sense of well-being from their course of life perspective. Methods used were interviews conducted with 20 older residents whose experiences and stories form the basis of the results. The method was qualitative and the interviews were conducted in the form of casual conversations (Study I), and was analyzed by content analysis, and narrative method (Study II), which was analyzed using dialogic performative analysis. In substantial, this thesis contributes to an understanding of how elderly people living in nursing homes experience and describe their well-being. They did this by specifying different qualitative values for well-being. The first value indicates well-being that is (i) to retain their autonomy through self-determination and participation in everyday life decisions regardless of accommodation. The second value indicates (ii) the need for continuity of one's identity, and the third one (iii) dealt with being an individual along with others, i.e. the notion of the ego strength is important when to assert their individuality, but this notion is simultaneously dependent on the interaction with others. This identity is based on the experience in life and the individual experiences of relationships with other people, where relationships with family members are the relationship's innermost core. At the nursing home, identity-based experience is based on the relationship with the nursing aids. Well-being is described as a total experience regardless of physical ability, in the context of residential care which constitutes both a home and a care institution. From a theoretical viewpoint this thesis contributes to knowledge of a caring scientific perspective on well-being, with a foundation in the elderly’s experiences of well-being, autonomy and continuity of identity. The study contributes to knowledge here called Interactionistic Caring Science, and it provides insights into the complexity of the experience of well-being in the nursing home described by these elderly people as interactively and situated produced (in situ). Practically, this thesis contributes knowledge about how the well-being of elderly people can be supported in caring situations at nursing home, which are both a home and a health care institution. The conclusion show that the elderly are becoming very dependent on other people to experience their wellness. This, as the elderly often have weak intrinsic activity and health and suffer from lack of their own ability to act, this makes relations and interactions an important part of nursing and nursing acts, and even more important for old people. This in turn requires increased knowledge by caregivers about this interactive meaning of caring, i.e. how people interacts in a dynamic relationship with their environment, both physiologically and psychologically, despite weak intrinsic activity and health. The old person’s dependence in their nursing aides makes those relations and interactions of even more importance in the nursing caring and in the nursing acts of the elderly.

Full textAn important part of preventive work to maintain health is to prevent healthcare-associated infections. Nursing home for the elderly is an arena for health and care. Healthcare-associated infections refer to infections among persons in care as an outcome of an action of care. A central strategy for maintaining health and promoting public health is to prevent infectious diseases in the healthcare facilities. Factors that affect the adherence of staff to the hygiene routines in municipal care are resources, management, personnel and external factors. Aim of the study was to describe what infection control in a nursing home for the elderly meant from a managerial perspective. The method was a qualitative interview study. Seven managers for nursing homes were interviewed with semi-structured questions. The result was analyzed with a qualitative content analysis. Results and main findings in the study showed that managers in nursing home for the elderly were very aware of that work with infection control is of great relevance to minimize and break the spread of infection. They also highlighted needs such as working more centrally and structured with infection control and hygiene issues. Factors such as knowledge, premises, attitude, and finances sometimes fail and could be more elaborated in work. The conclusion of this study shows that managers in special housing for the elderly are very aware that health care is of great relevance to minimize and break the spread of infection. There is also a need to work more centrally and structured with the health-hygienic issue. Identified difficulties with the health-hygienic work since conditions such as knowledge, premises, attitudes, attitudes and finances sometimes fail.

Man vet idag att vissa äldre medspeciella bakgrundsfaktorer har en större risk att drabbas av sjukdom och skada.En av de fyra största bakgrundsfaktorerna som medför hög risk för sjukdom ochskada är fallbenägenhet. Fall är mest rapporterade inom särskilda boenden, cirka60 % av vårdtagarna inom särskilda boenden drabbas årligen av ett eller flera fall.Tidigare studier har visat att fysisk aktivitet kan leda till en förbättrad balans ochminskad falltendens. Litteraturstudien ämnade att undersöka lämpliga former avfysisk aktivitet för att minska fall hos äldre i särskilt boende, genom att belysafysisk aktivitets effekt på inre riskfaktorer samt fallfrekvensen. En litteraturstudieav kvantitativa studier gjordes. Resultatet som framkom föreslår att ett och sammaträningsprogram förmodligen inte passar alla äldre. Förenklad Tai Chi passarmöjligen de äldre som är mer mobila och Sun-style Tai Chi passar de som harnågon riskfaktor för att falla. En del program, som exempelvis att utföraträningsprogrammet sittande, är möjligen mer fördelaktigt för de som är i behovav assistans vid förflyttning.

Full textSyftet med studien var att undersöka om individuella omvårdnadsplaner i nutrition kunde förbättra nutritionsstatus och som en följd därav förbättra den hälsorelaterade livskvalitén hos omsorgstagarna. Studien var en jämförande kvantitativ undersökning. Den utfördes på ett kommunalt äldreboende för 36 omsorgstagare, deras medelålder var 87 år. Som riskbedömningsinstrument för undernärnäring användes MNA-SF, värdet därav sammanvägdes med bedömningen av sjuksköterskan vid ställningstagande till omvårdnadsplan i nutrition. Nattfastans längd togs också i beaktande vid upprättandet av omvårdnadsplanerna. EQ-5D användes för att mäta omsorgstagarnas hälsorelaterade livskvalité och därefter verkställdes omsorgstagarnas omvårdnadsplaner i nutrition. Omsorgstagarna följde sina omvårdnadsplaner för nutrition i fyra veckor. Därefter gjordes nya mätningar inklusive EQ-5D. Resultatet visade att det i denna studie inte fanns något samband mellan förändring i nutritionsstatus och den hälsorelaterade livskvalitén och att fyra veckor är för kort tid för att se någon skillnad i en studie som den här.
The aim for this study was to see if care plans in nutrition could improve nutritional status and thus even improve the quality of life of the caretakers. The study’s design was a comparative quantitative investigation. The study was performed in a block of municipal service flats where 36 caretakers were living; their average ages were 87 years old. Nutritional assessment was made by using MNA-SF as a screening tool for malnutrition. The result from the MNA-SF screening was combined with nursing assessment to do the care plan considering nutrition. The time for overnight fasting was also measured to be considered making the plan. EQ-5D was used to measure the caretakers’ quality of life, care plans for their nutrition were made and they were realized. The caretakers used their care plans for four weeks. Then new assessments were made inclusive the EQ-5D.The result was that there were no significant correlation between changes in nutritional status and the caretakers’ quality of life in this study and those four weeks is too short time to see any difference in a study like this.

Full textSyftet var att beskriva hur äldre människor som bor på särskilt boende upplever sin nattsömn, dess kvalité och behov av omvårdnad på natten. Designen var en beskrivande design med kvalitativ ansats. Efter godkännande från forskningsetisk kommitté och verksamhetschef på ett särskilt boende utfördes halvstrukturerade intervjuer med åtta boende. Intervjuerna spelades in på dator och analyserades sedan med en manifest innehållsanalys. Huvudresultatet visade att de boende upplevde nätterna som lugna och rofyllda och att den enda yttre störningen som förekom var från byggarbetet som ibland förde oljud tidigt på morgonen. Andra faktorer som kunde påverka nattsömnen negativt var inre störningar, som tankar, sjukdom, smärta och nattliga toalettbesök. Det framkom också faktorer som kunde främja sömnen, som vilostunder på dagen, att ha mörkt på natten, att få en sömntablett och att få bibehålla sina egna rutiner på kvällen. De boende uttryckte att de var nöjda med omvårdnaden på natten och personalen var uppskattad och gav en stor trygghet. Slutsatsen av examensarbetet är att deltagarna upplevde lugna och rofyllda nätter på det särskilda boendet om det inte fanns några yttre eller inre störningar vilket sällan förekom. Deltagarna var nöjda med nätterna och den omvårdnad de fick.

Full textNutritionsproblem är vanligt hos äldre på särskilt boende. Trots att problemet varit känt sedan länge, fortsätter de äldre att minska ofrivilligt i vikt. Nattfastan på särskilt boende är längre än riktlinjerna från Livsmedelsverket och Socialstyrelsen, vilket är 11 timmar.
Syftet med studien var att belysa betydelsen av nattfastans längd i relation till de äldres viktdifferens före och efter intervention som genomfördes i två steg. Interventionen utmynnade i en ökad tillgänglighet av dryck/mat för de äldre under kväll och natt.
Den långa nattfastan minskade efter intervention 1, de äldre blev mer viktstabila. Intervention 2 medförde ytterligare förkortning av nattfastans längd, fler äldre ökade i vikt. Intervention 2 hade effekt över tid.
Urvalet är selektivt och litet i förhållande till kvantitativ ansats, studien ska därför ses som en förberedande pilotstudie som i liten skala prövade en arbetsrutin. Konklusion: Studien har genererat hypotesen att en förkortad nattfasta kan minimera ofrivillig viktförlust hos äldre på särskilt boende.

Full textAntalet äldre ökar i Sverige. Åldrandet medför ökad risk att drabbas av sjukdomar vilket kan påverka munhälsan negativt. Äldre med stort vård- och omsorgsbehov flyttar ofta till särskilt boende. Många av dem behöver hjälp med munhygienen. Munvården till äldre i särskilt boende är ofta bristfällig med dålig munhälsa som följd. Syftet var att beskriva vårdpersonalens attityder och beteenden avseende den egna samt de äldres munhälsa och munvård, samt undersöka om munvårdsutbildning kan relateras till munvårdsinsatser på särskilt boende. En tvärsnittsstudie med kvantitativ ansats genomfördes och datainsamlingen gjordes med hjälp av ett frågeformulär. Resultatet visade att de flesta respondenterna ansåg munhälsan som viktig. Majoriteten tyckte att de boende bör få hjälp med munvård två gånger per dag men få hjälpte de äldre med munvård så ofta. Munhygienen ansågs vara den svåraste delen i omvårdnadsarbetet. Flertalet hade fått munvårdsutbildning och få önskade mer kunskaper. Munvården upplevdes fungera bra på avdelningarna och sågs som en del av god omvårdnad. Flera faktorer, som att de äldre gjorde motstånd och tidsbrist, uppgavs som svårigheter. Konklusionen var att attityden till munvård var positiv, men hindrande faktorer som motstånd från de äldre och tidsbrist bidrog till att den prioriterades lågt. Endast en liten del av vårdpersonalen önskade mer munvårdsutbildning.

Full textObjective: How nurses in residential care reflect on quality follow-ups and its possible consequences for nursing care. Method: Semi-structured interviews with six nurses. Qualitative content analysis according to Graneheim and Lundman's method. Main Results: To witch degree quality follow-ups and national quality registers are integrated in Nursing care and its development is central to if these are seen as support or as obstacle to good nursing care quality. Double documentation contributes to nurses working more consultative and administrative. This reduces the time for nursing observations and guiding the nursing staff as well as contributes to national quality registers being perceived as hindrance. The nurses used their professional expertise and clinical experience to a greater degree than registry data in nursing care assessments. These were seen as too complex to be captured in questionnaires. Conclusion: The nurses perceive that they work under severe time constraints. Time is specified as essential for quality of nursing care. In decisions about quality indicators registrations nurses’ total workload should be taken into account. Registrations should be integrated into medical record used so nurses’ re-prioritizations not have negative consequences for nursing care quality.

Full textOlder people living at nursing homes often need help with their oral care. Earlier studies and reports describe a lack of knowledge among the nursing staff about caring about and the attitude towards oral health and oral care. The purpose of this study is to describe nurses’ perceptions of the work with old people’s oral care in nursing homes in a municipality. A qualitative method with a phenomenographic approach was used. Fifteen informants from eleven nursing homes were interviewed. The result from the informants’ different perceptions of the work with old people’s oral health comprised seven qualitative divided description categories: responsibilities, management of oral care, organization and leadership, access, autonomy and integrity, knowledge and competence, information and cooperation. The responsibility was divided between several trade categories, the management of oral care varied and organization and work management functioned well. Accessibility to the Dental service facilitated the work, while autonomy and integrity could be an obstruction. There was a need of education about dementia for Dental service’s staff. Nursing home staff needed education about oral health and oral care. Information and collaboration between the nurses and the Dental service varied. Conclusion: Routines and guidelines need to be improved and clarified regarding nurses’ responsibilities for the work with older people’s oral care in nursing homes. Nurses also need to improve their knowledge and assessment of older people’s oral care. Nursing staff also need to improve their knowledge of older people’s oral care, which often was deemed inadequately, but was adequately done in well-organized establishments. Even though Dental service’s availability supposedly improves the work with older people’s oral care, the cooperation between nurses and Dental service occasionally did not functioned, which need to be improved.

Full textBackground: About 50 million people suffer from dementia, most of them are over 65 years. Dementia is an umbrella concept for various diseases that affect the brain and cause impaired memory and thought ability. People with dementia shows different symptoms depending on where in the brain the injury is. In connection with the development of the disease, a move to a nursing home may become necessary as the need for nursing increases. The task of health care is to contribute to the best quality of life for the demented. Achieving a good quality of life for those with dementia places large demands on the care environment.Main: The purpose of the literature study was to explore the importance of the care environment in regards to the quality of life for people with dementia in nursing homes with focus on general spaces, based on staff and relatives experiences.Method: The literature study is based on twelve scientific studies with qualitative study design. Seven of the studies were based on individual focus group interviews and the remaining five studies were based on observational- and interview studies. The audit was based on SBU:s (2014) quality assessment template and SBU:s (2017) criteria for quality assessment. Six of the studies achieved a high quality and the remaining six studies achieved a medium quality.Results: According to staff and relatives, there are three different themes with eight associated subthemes that have an impact on the importance of the care environment for the quality of life for people with dementia; All three themes are: The perceptual, the social interaction and the psychosocial.Conclusion: Small changes in the environment were found to reduce disorientation and agitation, which in turn increased the independence, communication and the quality of life of the dementia sufferers.

Full textBackground: There is no information on how many people are suffering from hard-to-heal wounds in Sweden today. Hard-to-heal wounds is a major health problem, and it affects many older people. The risk of wounds increases with age, and the number of older people in our society increases. In nursing homes, these wounds are common, and nurse is a key person who leads and plans the nursing. Aim: The aim of the study was to illustrate nurses' experiences of working with older people with hard-to-heal wounds in a nursing home. Method: A qualitative interview study was conducted. Thirteen nurses in nursing home in one municipality were personally interviewed using a semi-structured interview method. A qualitative content analysis with inductive reasoning was applied. Result: The results of the study showed that the nurses mainly based their nursing interventions on hard-to-heal wounds in the older people on an experience-based knowledge. The old in a nursing home were fragile or in a palliative condition; many times, it was not realistic to heal the wounds because of the older people’s conditions. The nurses in a nursing home were alone, and they were independent in their assessments of the wounds and the old. The nurses' cooperation with the nursing staff was hampered by the nursing staff's unequal competence and thus, a lack of insight into the implementation of the nursing intervention for the older people. The doctor usually relied on nurses' assessments of wounds and lacked interest and knowledge of wounds in the old, according to the nurses. Wounds were an extensive and challenging, area; more in-depth knowledge of wounds was called for. Conclusion: Working with the old with wounds in a nursing home was an educational challenge for the nurses. The nurses needed to have a great deal of expertise in geriatric diseases, older people with hard-to-heal wounds needed to be assessed from a holistic perspective. The nurses needed to work more according to evidence-based knowledge in the area of hard-to-heal wounds in the old. Full textBackground: Caring for a person with dementia in special care facilities requires resources like personnel, facilities and medicine. It takes adequate educated staff with personal qualities to create a long-lasting relationship with relatives and residents. Aim: The aim of this study was to examine the experience of being a relative to a person with dementia living in a nursing home. Method: A literature review was conducted in which ten scientific studies were examined and analyzed according to Friberg (2006). Results: Five themes emerged from the analyses describing relatives’ experiences of nursing home care, activities, surroundings and integrity as well as coping strategies. Conclusions: Personal relations are of outmost importance for the experience of being a relative to a person with dementia living in a nursing home. Education and guiding of staff and relatives might prohibit misunderstandings and may help to avoid conflicts.

Full textBackground: People with learning disabilities communicate in various ways depending on their abilities. Communication difficulties affect interactions between people with learning disabilities and their professional carers’ and this places a high demand on the carers to interpret and understand each individual’s wishes and needs. At the same time, there is a risk that the carers will be misunderstood. If carers fail to understand what the residents are trying to communicate, and vice versa, situations with challenging behaviour such as an unwillingness to interact or verbal and physical aggression may occur. Aim: The overall aim of this thesis was to explore the interactions between adults with learning disabilities and carers in special accommodations as well as to evaluate the effects of an intervention created for carers working with people with learning disabilities at special accommodations. Method: The setting for the study was at special accommodations in one county in northern Sweden. In study I, information regarding 556 adults with learning disabilities was collected. In both study II & III, 11 people with learning disabilities and 16 of their carers participated. In study IV, 7 carers and one person with learning disabilities participated. Data in study I consist of carers’ estimations of the residents disabilities and frequency of restraint use. Video-recorded observations and individual narrative interviews were used in study II and study III, and in study IV multiple methods of data collection were used in a mixed method design with both carers’ estimations using instruments and with semi-structured interviews. Methods for analyses in study II, III and IV were qualitative content analysis and in study I and IV statistical analyses were used. Results: Study I revealed that of the 556 residents studied, 99 (18%) had been subjected to physical restraint use over the previous week. Of these participants, almost all were subjected to more than one type of restraint. The most commonly used physical restraint was a belt in chair (74%). Use of physical restraint can be related to both physical disabilities and challenging behaviours in the interaction between people with learning disabilities and their carers’. In interviews for study II, the carers reflected on both successful and unsuccessful interactions after viewing video of situations where they have been participating. Examples of successful interactions included understanding cues, satisfying needs, and managing situations with challenging behaviours. The carers’ reflected on how successful interactions could influence their feeling of security, confidence, and satisfaction among the residents. The carers also reflected on the consequences of unsuccessful interactions, which included causing irritation, aggression and violence. In study III, interactions between 11 residents with learning disabilities and 16 of their caregivers were recorded on video. Verbal and non-verbal interaction skills used among the carers were identified. Four ‘caring situations’ were chosen as examples of skilled interactions. The results showed that skilled interactions between the carers and their clients were based on being confirming, sharing daily life experiences, giving time and space, and using congruent and distinct language. Study IV evaluated the effects of a web-based learning intervention, based on a theoretical model, and the results showed an increased ability among the carers to handle situations with challenging behaviour and a decreased frequency of challenging behaviour. Conclusion: Physical restraint is commonly used in special accommodations for people with learning disabilities. Use of physical restraint can be related to both physical disabilities and challenging behaviours in the interaction between people with learning disabilities and their carers. Carers find the use of physical restraint difficult and exhausting. A break in communication between people with learning disabilities and their carers could lead to impaired interaction that creates situations of challenging behaviour. When carers are able to interact successfully with people who have limited ability to communicate verbally, it seems to give satisfaction to both the residents and their carers in group homes. Carers often experience helplessness and uncertainty in situations with challenging behaviour and express a need for support. The carers considered the web-based training program to be useful. Further, a decrease of situations of challenging behaviour was observed after the web-based program was implemented. This indicates that the web-based training program may be useful in training for carers, both novices and more experienced carers in special accommodations for people with learning disabilities

Full textBackground: The conditions for the physiotherapists’ work in housing for the elderly vary depending on number of care recipients, the availability of the physiotherapists and the resources provided at the county level in Sweden. Previous studies have shown that the lack of resources makes the work difficult for the physiotherapists. Moreover, it is hard to meet the needs of individual care recipients when physiotherapists have the medical rehabilitation responsibility for a large number of care recipients. Rehabilitation is carried out on a specific level and a general level and often delegated to care assistants. Purpose: The aim of this study was to examine physiotherapists’ perceptions of their work in housing for the elderly. Method: Seven physiotherapists working in houses for the elderly were interviewed with the help of a semi-structured interview guide. The interviews were analyzed with qualitative content analysis. Results: The analysis revealed five categories: controlling factors, priorities, cooperation around the care recipient, the breadth of the work and self-image and status. Conclusion: The participants’ perceptions of work in housing for the elderly were that the job was fun and varied but influenced by stress and lack of resources. The work was perceived as consisting of a lot of prioritizing; however, the grounds on which the decisions were made varied as did the perception of support from guidelines. The participants reported that they desired a well-functioning working relationship with the care assistants because the physiotherapy inputs were often delegated to them.

Full textBackground Equal and good care is a right for every person in Sweden according to National Board of Sweden. Previous studies show that patients in geriatric care are exposed to abuses of various shapes. The most common form of abuse is neglect of the resident. To give good care the care workers need to respect and maintain the integrity of the resident. Aim The aim of the study was to investigate if care workers in nursing homes observed any abuse against the elderly and what kind of abuse. The aim was also to find out what possible causes of the abuse could be and investigate the care workers knowledge of Lex Sarah. Method Study-specific questionnaire with quantitative approach. Result The result shows that 69 of the 80 respondents had noticed any kind of abuse or neglect in the last six months in their nursing home. The most common abuse or neglect was inadequate care of the teeth and hygiene among the elderly. The respondents think that lack of education and stressful situations may be the cause of the abuse and neglect. Conclusion The study shows that abuse and neglect does exist in Swedish nursing homes. Causes of the current abuse is ignorance and lack of time among careworkers. The knowledge of Lex Sarah was high among the careworkers.
