Le, Floch Bernard. "Quels facteurs positifs déterminent l'attrait vers la médecine générale et le maintien dans leur pratique clinique ?" Thesis, Brest, 2018. http://www.theses.fr/2018BRES0036/document.
Full textIntroduction: OECD countries, including France, face a shortage of general practitioners. Many research studies the negative sides of the specialty to explain it. The research project of the EGPRN was to highlight the factors of satisfaction of the general practitioner, in the participating countries. The research hypothesis of this project is that there must be well being and motivated GPs and that these positive factors could be used to promote attractiveness to General Medicine and maintenance in clinical practice.Method: The first step of this thesis was the realization of a systematic review of the literature to list the various factors of satisfaction of general practitioners already studied. For the second stage, qualitative studies by semi-structured interviews or focus groups were then conducted with general practitioners in the eight participating countries. The third step consisted of a consensus search by the Delphi method then hierarchization by nominal group. Results: Qualitative surveys by semi-structured interviews or focus groups conducted in the participating countries were used to interview 183 general practitioners interviewed in the eight countries.Codes from verbatim were created a "codebook" comprising 31 items that were classified into 6 themes: the general practitioner as a person, the skills specific to the profession of general practitioner, the positive organization of the profession of doctor generalist, the doctor-patient relationship, the teaching of general medicine and the factors that support the general practitioner. Conclusion: The results of the consensus study showed that to recruit in general practice it is necessary to develop care coordination, patient-centered approach, teaching and internship control and to pay attention to factors organization

Full textBy the representations it conveys and the solutions it encourages, the building of ageneral theory is never neutral in an axiological plan. Although its construction is based on knowledgeof Positive Law de lege lata, the resulting order meanwhile remains closely linked to the perceptionsand beliefs of the interpreter. It is in this sense that must be addressed the question of the contributionof labor law to the general theory of the legal act. This study highlights the protean nature of theconsidered phenomenon, without concealing the situations where the discipline resembles more acounter productive model than a true source of inspiration. Adopting a prospective point of view, thisresearch highlights aspects of the labour law likely to encourage the promotion of a theory of the legalact marked by pluralism. The material thus contributes to underline the variety of conditions in whichforms the willingness to engage oneself, just like the diversity of its modes of expression. It alsoshows a certain capacity to translate in the field of law, in order to understand them better, theotherness of the balances of power and the mutability of the factual data accompanying the executionof legal acts

Full textThe aim of this thesis is a comparative study of trade union approaches - reformist or revolutionary - in the field of culture, the two groupe being at that time organized in two trade union centres : the cgt and the cgtu. This cultural policy was emphasized in the congresses of the cgt and the cgtu. Minutes of those congresses, newspapers, "vie ouvriere", the weekly magazine of the cgtu. "le peuple", the daily paper of the cgt, constitute the majority of sources which were used. The results of the research, contained in two parts, with the thirties constituting the dividing line, lead to two essential conclusions. Cultural leisure activities coincide perfectly, whether they are organized by the cgt or the cgtu which have in common the popular education and culture inherited from the cgt, created in 1895. This identity of approaches disappears when cultural activities deal with the ideological basis of the cgt and the cgtu (as far workers'education is concerned in particular). The differences derive from the contents - reformist or revolutionary - of the changes for which each trade union is fighting, in its own way

Full textThe first part develops the conceptual framework by defining the following key concepts : job satisfaction, pay satisfaction and wage equity. In fact, the objective was to cope with the confusion, existing in the literature, between wage equity and pay satisfaction. The implementation of equivalence test provides support for the independence of both constructs. Then, we set up the research model. The second part presents the empirical study integrating construction, validation and confirmation of wage equity, pay satisfaction and job satisfaction measurement scales using SPSS and AMOS. Then, the data collection was made within executives from private and public organizations in France. .

Full textThis PhD dissertation has a twofold objective: to theoretically analyse, by using a workleisure trade-off model, the sensibility of physician’s working time to different payment systems when only the number of consultations and patients are contractible; and to empirically observe how French GP’s working time would change if consultation fee increased, in the specific case where different range of human motives are considered. The first part is dedicated to the presentation of the main properties of the basic model we use to study the labour supply behaviour of self-employed GPs in various contexts (chapter 1). Then, we show how this model can be ‘fitted’ to deal with the issue of health inequalities through physician’s choice in terms of consultation length (chapter 2). Finally, we theoretically investigate how physicians’ labour supply reacts to a population ageing shock, according to the effective payment scheme - fee-for service or capitation (chapter 3). The second part presents a survey on the emergence of the concept of intrinsic motivations in health economics aiming at specifying the theoretical background with which the remainder of our work is in line (chapter 4). Next, we explore how standard labour supply theory can integrate intrinsic motivations as a key determinant of human behaviour and, more especially, the effect of extrinsic incentives on the former in the field of general practice (chapter 5). Lastly, we intend to check, from an empirical perspective, if economic incentives can have a detrimental effect on intrinsic motivations in the particular field of health promotion, by using two different strategies (chapter 6)

Full textEnvironmental satisfaction is a key concept in environmental psychology. Surprisingly however, it has been little explored in the French-speaking organisational framework. Our research defines environmental satisfaction at work as an individual’s overall assessment of the workplace, regardless of the spatial scale (workstation, office, organization, neighbourhood). We have constructed a scale in order to better understand this concept and its links with concepts from both environmental psychology and occupational psychology. Our study has three objectives. The first is based on Churchill’s paradigm (1979) and involves the construction (study 1), and exploratory (study 2) and confirmatory (study 3) validation of the Environmental Satisfaction at Work Scale. Our second objective involves examining environmental satisfaction at work in relation to variables from environmental psychology (study 1) and occupational psychology (study 2). Finally, we examine the ESET from an intercultural perspective by comparing the environmental satisfaction of French and Italian office workers taking an emic (study 1) and an etic (study 2) approach. The ESET possesses satisfying psychometric characteristics that can be used both by researchers in social sciences and by human resource managers. This scale can help improve the quality of life in work environments

Full textWe study in our dissertation the mediator effect of the well-being at work on the relationship between satisfaction at work and commitment. Because there no satisfying explanation on this link we used Saari and Judge (2004) et Riggio (2008) to introduce well-being at work in this relation. First, we made a review of literature about Satisfaction and commitment to show the lack about this relation and to highlight the concept of well-being. Then we have defined and delimit the concept of well-being at work. No scale was found in France, so we had to build one (EPBET: échelle positive du bien-être au travail) in the first time. To do it correctly, we used the Churchill paradigm and two sample n=313 and n=865 were collected. This scale was used in the second time test our principal hypothesis: the mediator effect of the well-being at work on the relation between satisfaction at work and commitment. We used Baron and Kenny test and Preacher and Hayes to show it. Then we change our analysis level with a person-centered approach whereas a variable-centered approach. We found five profiles of well-being using the factor mixture models and especially latent profiles analysis and factor mixture analyses. Those profiles are qualitatively and quantitatively distinct from each other. There is a profile of complete well-being, a profile of collective well-being, a profile of non-time well-being, a profile of relational well-being and a profile of low well-being. All this results are discussed in the last part of our dissertation. We also describe our principal findings in this final part

Full textRecent studies on trade unionism reveal the CGT extending its action, to a certain degree, towards less secure sectors of the employment market, which bear the marks of absent unionist traditions (large retailers, people care, industrial subcontracting...). Within its organisational structure, Local Unions (UL) are supposed to help this extension process. They are designed to welcome employees who work in companies bereft of trade-union institution, faced with their employers, and to help them stand up for their rights. Based on an ethnographic inquiry within two ULs, the present thesis intends to study the ways of commitment and militancy of low and precarious working-class employees (checkout assistants, store shelf stockers, cleaners, nursing auxilaries, non-skilled workers…).Intersecting sociology of social movements and sociology of the working classes' relation to politics, the present research sheds light on the circumstances in which trade unionsim is made possible for members of dominated social groups – a priori characterized by weak resources for protesting and collective action. The ways of trade-union action analyzed herein inform us about these groups' ordinary relation to militancy, but also about how political and protesting skills are passed on, and maintained by the ULs. Lastly, this research gives a report on how a union organization offers a variety of comittments for such employees, who work in conditions hostile to the development of collective action (shattered groups, precarious work and employment conditions, weak social rights, repressive measures). It is also a study on the layman's uses of trade unionism

Full textWe study in this thesis the subjective formation of job satisfaction and the impact of organization on employees satisfaction. First, we work on the modeling of psychological process of job satisfaction, defined as an attitude at work. Then, we propose a model to study the impact of work organization on work activity. Our research aims to study job satisfaction using an innovative methodological and theoretical approach in the domain of social simulation: based on major theories in the fields of organizational psychology and economy of well-being, our goal is to produce an agent-based model of the cognitive aspects of job satisfaction. Our purpose is to provide a theoretically as well as empirically grounded model of job satisfaction that fosters the understanding of organizational impact on employees' satisfaction. To do so, we tested our model on an ideal scenario of work conditions improvement policy. The results revealed that this policy do not systematically causes a global improvement on job satisfaction: agent's cognitive profil, that is a typical way to evaluate job satisfaction, have a great impact on policy outcomes. Next, we tested our model using a scenario based on a panel survey of work conditions conducted in a big french organization during 2013/14. We were able to reproduce declared individual levels of job satisfaction. Furthemore, in order to provide a more realistic representation of work improvement policy, we have also developed a work activity model. Firstly, we analyzed the impact of various work organizations on organizational performance, then we conducted an analysis of the impact of work activity on the formation of burnout episodes. This last study helped us to bring up the importance of work activity and organization on employees' well-being; the aim being eventually to propose a coupled model of work activity and job satisfaction

Full textOne may wonder if the new forms of contracts such as the back-to-work assistance plan, the parental responsability contract, the host and integration contracts, the work placement and personalized social support contracts, can be considered as such, if one sticks to the traditional definition of the word contract, namely an agreement of wills which aims at creating obligations. The purpose of the thesis is, through their study, a new analysis of the notion of contract. A revised definition, shared by private law and administrative contracts, can be suggested. A contract is the meeting of mutual consents which aims at creating new right effects. Besides, contract guidelines can emerge. Finally, the peculiarity of new forms of contract can be explained by the emergence of new contract functions : the guidelines for problem resolution. Like other contracts, they belong in the revealed category of unequal contracts which are characterized by two elements : the existence of a power held by one of the contractors and the consequential vulnerability of the other. A sketch of a common scheme seems to appear because of the need to protect vulnerable contractors, and more generally, to maintain the contract as a social link. Such a scheme is difficult to imagine, in view of the specific characteristics of each unequal contract. This standardization is desirable to facilitate the effectiveness of the desired result

Full textThe issue of mental suffering is ubiquitous at work as evidenced by the recent controversies related to suicides int the workplace. Physical suffering is moving towards a psychological suffering. In this thesis we ask the question: how is mental suffering, and how does it appear ? Our hypothesis is: Organizational changes generate mental suffering manifested by particular reactions in Morocco and France. There are many difference in how to experiment that, according to the personality and its impact on the self-image. Our research focuses on the administrative staff of two universities in France and Morroco. Analysis of the data to a questionnaire, interviews and NEO PI-R shows that suffering is little expressed at work in Morocco, also there is individual and personal problems related to intimate situation of each one next to the dissatisfaction, whereas mental suffering is manifested by the devaluation of the self-image in France

Full textThe balance between personal and professional life is a recurring theme in our societies as well as the spillover phenomena, as well work on the personal life and the personal life on work, were seized by HR managers. As such, operational solutions have been proposed through, the researchers admit that family-friendly practices auch as flexible hours, childcare services on site, company conciergeries etc. However, these practices being relatively undeveloped in France so we can ask on which basis can they be implemented, and especially what their real role on employees’ work-life balance? This question requires an examination of perceptions of work-life balance and its possible effects on job satisfaction. A research model integrating perceptions of work-life balance, intentions to use harmonization practices, and job satisfaction, we. proposed. ln support of an empirical study on two samples of 210 and 224 French employees in the private sector, we have shown that these practices are considered as a real measure supporting work-life balance, and may be, in this sense, a lever for job satisfaction. It also appears that these practices and balance itself are subject to different assessments according to employees' profiles, and finally, while the these practices are in favor of work-life balance, the work-life balance may be a determinant of job satisfaction

Full textOur main preoccupations were to study the relationship between work and live satisfaction from the intercultural point of view and determine work values maintained in France and Zaïre. First, work values seem to be affected by culture as well as intra-cultural and inter-cultural variations of live satisfaction. However, these two effects are not systematic. Rather, they occur according to the method used and or socio-professional category. As for the relationship between work and outside work satisfaction, the operationalization of Iris and Barrett of the spillover appeared to be more general than that of the compensation. However, that verification depends on the socio-professional category, uni-dimensional criteria (consideration of the median of distribution of raw scores) and or multi-dimensional criteria (the utilization of scores for the first and second factors) for the identification of workers who are satisfied or dissatisfied at work, also our psychological explanation of spillover and compensation reveals itself as cultural. In other words, whatever the socio-professional category, the spillover and the compensation are explained either by passivity in France or by activity in Zaire. Finally, methodological implications are discussed

Full textThe present research is aimed at studying different dimensions of job satisfaction in Brazilien and French workers. In each country, subjects were randomly sampled out from similar domains of activity. In addition, the percepion that foremen have about the worker's satisfaction was explored. The job descriptive index, designed by Smith, Kendall and Hulin (French adaptation by J. -L. Mogenet, university Paris X) was used in the study. The foremen received a modified version of the questionnaire. When interpreting the results, the respective socio-economical situation in each country was taken into account, in order to control for actual conditions of living of the workers. The general hypotheses bear on the comparison of the different satisfaction dimensions in each sub-sample. They were tested at a descriptive and correlational level. A principal components factor analysis permitted to elicit the structure of the relationships between dimensions of satisfaction. A structural variance-covariance model (Lisrel) was applied to test for causal relationships between latent variables extracted from the factor analysis

Full textAs in other European countries, the french unionism is in a deep crisis from the 80's. A range of analysis have been provided in order to understand and explain that crisis. Unionism institutionalization could be the main factor, as argued by one of these hypothesis. Correlated to this institutionalization, the nature of trade union representatives is presumed to have evolved. In the last years unions activists as main figure of french unionism would have been replaced by unions officials. This research is based on the analysis of this trade unions professionalization process. After a review of literature in a first section, the project aims to address some points. First, the making up of union representative position is analysed. Some key historical turning point of the process are investigated : the eve of the century, the 1921 CGT's split, the making of work councils in 1945-47, the may 68 disruption. The second part is based on fieldwork evidences on two unions federations of two branches. Union's representatives career is explored and its examination is pursued. This career is analytically reconstructed from its key steps : the first commitments, the making up of an expertise and skills achievement, the coming up to professional status and finally the move into a new occupation. As a result of the research, the postulated growing professionalization of unions representatives is questioned. A large range of practical and material issues is hindering the stabilization of an union interests representative body. Furthermore, the professionalization concept itself is denying the representation embodyed by observed unions officials

Full textIn France, ever since the early 2000s, the discourse on medical shortages has become an issue and the geographical accessibility to primary health care has become a matter of public policy and regional planning. As a result of the gradual tightening of the numerus clausus and the attractiveness of the various forms of salaried activities, the decrease in the supply of primary care raises the question of private practitioners turnover. In addition, the fresh graduates in general medicine, who nevertheless chose a private practice, are turning away from vacant surgeries left by their predecessors, and show new expectations in terms of work organization. This thesis focuses on the issue of the liberal general practitioners turnover. It questions the newcomers' loss of interest for traditional exercise pattern from the new-comers, and shows that their generational affiliation is a possible interpretive key to understand the changes at work in the private practice of general medicine. The comparison between two generations of liberal General Practitioners, the incomers and the leavers, has helped to contextualize the choice of exercise of each group from their respective period of installation. While making a distinction between the socio-historical changes that affect the work sphere plus the family and personal world and those related to the medical field, particularly demographic changes, this perspective has shown how anchoring, in a particular context, shapes a specific link to work for a specific generation, and determines how to practice general medicine. Finally, inequalities in primary care available in Franche-Comté have raised issues that are discussed in this thesis as well as the tools given by the government to solve them. It provided the opportunity to question the representations that both generations of General Practitioners have about collective work, especially the balance between the incomers’ expectations who are taking up the craft and the concept of collective work which is supposed to be tomorrow’s mainstream

Full textThe moderating role of career stages has never ceased to attract the attention of researchers in this time when a career concept is the subject of much debate in the middle of scientists. Our goal in this study is to investigate the moderating role of career stages in relationships of a group of variables, with a view to clarifying these relationships and to identify variables that may contribute to the improvement of job satisfaction and organizational commitment. To achieve our goal, we developed a research model with four variables levels. The first is related to stress factors at work, the second is job satisfaction, the third deals with the organizational commitment and the last level deals with the stages of career. This model is tested on a sample of 242 university teachers in Angola. After various analyzes (Multi-Group Analysis, and others), our results indicate that career stages play a moderating role in the overall relations between our variables. This moderation is manifested specifically in direct and indirect relationships of our variables in career steps included in our study. These results have allowed us to present the contributions and recommendations of managerial types in line to help professionals and institutional managers to improve their policies on the well-being of employees

Full textThis thesis focuses on job satisfaction processes and assessment. To date, imprecision persist in the job satisfaction assessment as well as in the understanding of the job satisfaction processes. Our aim is to analyse variations between assessment tools in order to question the validity of the job satisfaction models on which these tools are based. It follows a double issue. The first concerns the relation between global and facets satisfaction and the way in which facets satisfactions rise in a global attitude. We focused on composite scales (where the global level of satisfaction is inferred from the facets scores) putting into light their lack of validity. We tested two explanatory factors : the lack of facets exhaustiveness and weighting. These factors are not enough to explain the difficulty to predict global satisfaction from facets scores. Indeed, the complementary analyses we carried out highlight the existence of a moderator in the facets-global satisfaction relation. The second issue concerns the relation between satisfaction and person-job fit. We undertook to specify up to what point job satisfaction could be regarded as resulting from the fit between an individual and his environment. We showed thet the low empirical links observed in previous studies did not reflect really the satisfaction-fit relation but arise from operationalization choices, and highlighted that if indirect scales of fit did not make it possible to strongly predict job staisfaction it was possible when using a perceived fit scale on which individuals directly declare up to what point they consider their job as fulfilling their expectation

It was devoted on the urgency of the planet awareness on environmental problems. But beyond this struggle of protecting the planet is a big political, diplomatic, economic and social games at stakes. The francophones countries having been aware of this new games at stakes, took advantage of the world mobilisation to formulate a principle of cooperation in the domain of environment. This should have double objectives. For them it is a question of taking an active part in the international community leadership role in favour of environment all in assuring their political, economical and cultural interests. It is also a question of defining a common strategy to fight against environmental problems that are daily confronted with by francophone countries, the most urgent of which are deforestation, water, pollution, desertification and industrial pollution. In the last analysis, it is a question of offering a frame of life environmentally healthy for their population that are more and more becoming numerous, in creating the term of a durable development linking the objectives of ecological economics in francophone states

It proposes a new interpretation framework of professional transition from a historical and relational viewpoint. This perspective shows the way the trade unionists increase their experience in militancy, it reveals how they move up through different professional contexts and it finally explains how the support facilities for retraining evolve. From three fields – the CGT national office, some dedicated courses at university and a specific entrance exam at the National School of Administration (ENA) – this study is concerned with the output of the CGT’s unionist reputation as a distinguishing feature, sometimes infamous or highly sought, built during the professional transition. In order to understand the retraining process without individualist and pro-active explanations, this research pays attention to the historical background of social mobility and to the social inequalities between unionists. The professional retraining is considered as a social experience depending on collective negotiations and institutional policies applied by the union. The career paths of these retrained unionists must be understood as the result of the effects produced by institutions – educational, professional or even political – that contribute to create them

Full textThis research appraises the efficiency level of three components of compensation on both motivation and job satisfaction. These three components, e. G. Salary, merit-pay incentives, and benefits, have been outlined after conducting three different studies (studies 1,2, and 3) among employees in both private and public sectors. Discrepancy theory for job satisfaction, and expectancy theory for motivation, provided the theoretical background of the research. Three different questionnaires were used: the compensation satisfaction questionnaire (csq), the work motivation & compensation questionnaire (wmc), as well as the Minnesota satisfaction questionnaire (msq). The first two questionnaires are original constructions. Causal analysis was conducted following structural equation modelling (lisrel VII), and using results obtained in studies 1 and 3 (n =579). Main results suggest that salary is positively associated to both motivation and satisfaction. On the contrary, merit-pay incentives are of no influence on motivation or satisfaction. Employee benefits have a positive influence on job satisfaction and a negative one on motivation. These latter results, however, owe their ambivalence to inconclusive statistical figures

Full textSome education policies are structured in response to the dropping-out problem, in order to develop actions towards early-school leavers, or youth at risk of early-school leaving. This thesis aims to analyse the unusual position of the dropping-out treatment in the French education policy, and the effects of this policy for the youth. It is based on a field survey of local managers of a Mission générale d’insertion (MGI), and about young dropouts who undergo the MGI programs. In order to understand the construction of this form of policy, an institutionalist theoretical frame is used, considering that the school system is embedded in a societal pattern, and analysing the justification of social action based on stable principles, called conventions. These conventions direct education policy in a certain way. In France, the battle against dropping-out is not obvious, because of the domination of the academic convention, in which students are competing for academic achievement. This explains the marginal position of the MGI in the French school system, and their difficult integration within the schools themselves. The MGI aims to orientate educational principles toward a relationship between training and the labour market, as others youth school-to-work programs, according to a vocational convention. Nevertheless, its real action is bring ing dropouts back to school, and access to certification. The MGI has a positive effect on both these goals, to the exception of academically disadvantaged students
