Abstract:
OBJECTIVESyringobulbia (SB) is a rare entity, with few cases associated with Chiari malformation type I (CM-I) in the pediatric population. The authors reviewed all pediatric cases of CM-I–associated SB managed at their institution in order to better understand the presentation, treatment, and surgical outcomes of this condition.METHODSA prospectively maintained institutional database of craniovertebral junction abnormalities was analyzed to identify all cases of CM-I and SB from the MRI era (i.e., after 1984). The authors recorded presenting symptoms, physical examination findings, radiological findings, surgical treatment strategy, intraoperative findings, and outcomes. SB cases associated with tumors, infections, or type II Chiari malformations were excluded.RESULTSThe authors identified 326 pediatric patients with CM-I who were surgically treated. SB was identified in 13 (4%) of these 326 patients. Headache and neck pain were noted in all 13 cases. Cranial nerve abnormalities were common: vagus and glossopharyngeal nerve dysfunction was the most frequent observation. Other cranial nerves affected included the trigeminal, abducens, and hypoglossal nerves. Several patients exhibited multiple cranial nerve palsies at presentation. Central sleep apnea was present in 6 patients.Syringomyelia (SM) was present in all 13 patients. SB involved the medulla in all cases, and extended rostrally into the pons and midbrain in 2 patients; in 1 of these 2 cases the cavity extended further rostrally to the cerebrum (syringocephaly). SB communicated with the fourth ventricle in 7 of the 13 cases.All 13 patients were treated with posterior fossa decompression with intradural exploration to ensure CSF egress out of the fourth ventricle and through the foramen magnum. The foramen of Magendie was found to be occluded by an arachnoid veil in 9 cases. Follow-up evaluation revealed that SB improved before SM. Cranial nerve palsies regressed in 11 of the 13 patients, and SB improved in all 13.CONCLUSIONSThe incidence of SB in our surgical series of pediatric patients with CM-I was 4%, and all of these patients had accompanying SM. The SB cavity involved the medulla in all cases and was found to communicate with the fourth ventricle in 54% of cases. Posterior fossa decompression with intradural exploration and duraplasty is an effective treatment for these patients.

The asymmetric unit of the title complex, [Sb(C44H28N4)Cl2][Sb2Cl8]0.5·CH2Cl2, is composed of a SbV complex cation wherein the Sb atom is hexacoordinated by four N atoms of the pyrrole rings of the tetraphenylporphyrinate (TPP) ligands and two chloride ions, a half di-μ-chlorido-bis[trichloridoantimonate(III)] counter-anion and a dichloromethane solvent molecule. In the cation, the average Sb—N distance is 2.066 (2) Å, while the Sb—Cl distances are 2.3410 (11) and 2.3639 (12) Å. The central unit of the cation, SbN4C20, is far from being planar, with deviations of atoms from the least-squares plane ranging from −0.110 (4) to 0.124 (4) Å. The Sb—Cl distances in the anion, which is located about an inversion center, lie in the wide range 2.3715 (13)–2.7489 (13) Å, the longest distances being between the Sb and bridging Cl atoms. The crystal structure is stabilized by intermolecular C—H...Cl interactions involving the cations, the anions and the solvent molecules. The solvent molecule is disordered over two orientations in a 0.901 (13):0.099 (13) ratio.

Background:To examine which school- and home-based factors at age 11 to 12 (6th grade, elementary school) predict physical activity (PA) and sedentary behavior (SB) at age 13 to 14 (8th grade, secondary school).Methods:Data at both time points were collected from 472 children (mean age baseline = 10.97, SD age = 0.41) and their parents. Children and parents completed self-reported questionnaires. Children’s height and weight were measured and children wore a pedometer for 7 days.Results:Hierarchical regression analyses revealed that predominantly boys (β = −.11), children with higher levels of pedometer-determined PA (β = .44) and more parental logistic support (β = .11) at age 11 to 12 displayed higher levels of pedometer-determined PA at age 13 to 14 (R2 = 39.1%). Similar results emerged for self-reported moderate-to-vigorous PA (R2 = 36.7%). Finally, lower levels of screen-based SB at age 13 to 14 (R2 = 32.5%) were most strongly related to lower levels of screen-based SB (β = .41), a medium/high socioeconomic status (β = −.18), and higher levels of parental PA explicit modeling (β = −.18) at age 11 to 12.Conclusions:Children’s PA/SB and the supportive role of parents at age 11 to 12 are strong predictors of PA and screen-based SB at age 13 to 14.

Abstract:
Shave biopsy (SB) is used for the diagnosis of suspicious skin lesions, including melanoma. Its accuracy for melanoma has not been confirmed. We examined our experience with SB to determine its ability to predict true Breslow depth (BD). We performed a retrospective review of the tumor registry for all patients diagnosed with melanoma by SB from 1995 to 2004. Site and depth of lesion, tumor stage, correlation of BD between SB and wide local excision (WLE), and changes in surgical management due to discordance were examined. Melanoma-in-situ was defined as a depth of 0 for this analysis. One hundred thirty-nine patients were diagnosed with melanoma by SB. Pathology after WLE were as follows: 54 (39%) patients had no residual disease, 67 (48%) had a BD equal to or less than the SB, and 18 (13%) had a thicker BD compared with the SB. For these 18 patients, the median BD by SB and WLE was 1.1 mm (range 0-6.5) and 3.5 mm (range 0.5-20.5), respectively ( P = 0.0017). Upstaging of final BD from SB to WLE was significantly associated with increasing tumor depth and higher stage of melanoma ( P < 0.0001). Only seven of the 139 patients (5%) required further surgery because of the increased depth of the WLE. SB underestimated the final BD of melanoma in 13 per cent of patients, but changed the management of few patients. SB is a valuable tool for practitioners in the diagnosis of melanoma. Nevertheless, patients diagnosed with melanoma by SB should be counseled about the rare need for additional surgery.

Abstract:
Thermal and mechanical properties of neat linear low-density polyethylene (LLDPE) hybrid biocomposite with sugarcane bagasse and eggshell as a reinforcing filler were investigated. Hybrid biocomposites with sugarcane bagasse/eggshell was varied in the range of 6/4, 12/8, 17/13, 20/20, 13/17, 8/12, and 4/6 wt.%, were prepared and examined. The sugarcane bagasse fiber (Sb) was surface-treated using a silane coupling agent (3-(trimethoxysilyl) propyl methacrylate) and eggshell (Es) was treated using titanium (IV) isopropoxide. The results showed that high calcium oxide content was 99 % by weight in eggshell and silicon dioxide content was 50 % by weight in sugarcane bagasse. The tensile and flexural modulus and hardness of the composites with treated Sb/Es were higher than that of the untreated. These values of the LLDPE/Sb/Es composites increased with Sb/Es content. Differential scanning calorimetry (DSC) results show interesting trends in term of the influence of Sb/Es content, both with untreated and treated Sb/Es on the crystallization behavior of the composites but the melting and cold crystallization temperature of its composites did not obviously change. Thermogravimetric analysis (TGA) indicated their thermal stability of the composites, both untreated and surface-treated Sb/Es were lower than that of neat LLDPE. The sample molded from the biocomposite with surface-treated Sb/Es particles showed better dispersion of the materials inside.

Abstract:
The crosstalk between central nervous system (CNS) and gut microbiota plays key roles in neuroinflammation and chronic immune activation that are common features of all neurodegenerative diseases. Imbalance in the microbiota can lead to an increase in the intestinal permeability allowing toxins to diffuse and reach the CNS, as well as impairing the production of neuroprotective metabolites such as sodium butyrate (SB) and indole-3-propionic acid (IPA). The aim of the present study was to evaluate the effect of SB and IPA on LPS-induced production of cytokines and tryptophan metabolites in human astrocytes. Primary cultures of human astrocytes were pre-incubated with SB or IPA for 1 hour before treatment with LPS. Cell viability was not affected at 24, 48 or 72 hours after pre-treatment with SB, IPA or LPS treatment. SB was able to significantly prevent the increase of GM-CSF, MCP-1, IL-6 IL-12, and IL-13 triggered by LPS. SB and IPA also prevented inflammation indicated by the increase in kynurenine and kynurenine/tryptophan ratio induced by LPS treatment. IPA pre-treatment prevented the LPS-induced increase in MCP-1, IL-12, IL-13, and TNF-α levels 24 hours after pre-treatment, but had no effect on tryptophan metabolites. The present study showed for the first time that bacterial metabolites SB and IPA have potential anti-inflammatory effect on primary human astrocytes with potential therapeutic benefit in neurodegenerative disease characterized by the presence of chronic low-grade inflammation.

Abstract:
The paper describes a modified and optimised conventional wet ashing method for the determination of antimony (Sb) in polyethylene terephthalate (PET) bottles used for bottling sparkling and still water. The modified optimum conventional wet ashing method was applied as the digestion method to extract Sb from PET bottles, and the extracted Sb analysed using flame atomic absorption spectrometry (F-AAS), and in bottled water, Sb was analysed with hydride vapour generator atomic absorption spectrometry (HVG-AAS). Microwave digestion method was also used as described above for comparison. The concentrations of Sb obtained using modified optimum conventional wet ashing and microwave digestion protocols were from 169 ± 13 to 285 ± 17 μg g−1 and from 123 ± 0.5 to 249 ± 18 μg g−1, respectively. The average concentration of Sb in bottled water samples using the HVG-AAS was 0.183 ± 0.013 μg L−1, with a range of 0.012 ± 0.001 to 0.358 ± 0.006 μg L−1. Sb concentration increased marginally with longevity of storage, but still remained well below the various guidelines for Sb levels in drinking water.

Syntheses along the Tl<sub>5</sub>(As,Sb)<sub>13</sub>S<sub>22</sub> compositional line in the Tl<sub>2</sub>S-As<sub>2</sub>S<sub>3</sub>-Sb<sub>2</sub>S<sub>3</sub> phase system showed that the compositional range of rebulite extends from Tl<sub>5</sub>As<sub>9.5</sub>Sb<sub>3.5</sub>S<sub>22</sub> to Tl<sub>5</sub>As<sub>7.75</sub>Sb<sub>5.25</sub>S<sub>22</sub>. The Sb-rich end-member is in equilibrium with jankovićite of ideal composition Tl<sub>5</sub>Sb<sub>7.5</sub>As<sub>5.5</sub>S<sub>22</sub>. It is considered to be the As-rich end-member of the jankovićite solid solution. The crystal structure analyses of crystals from the As and Sb end-members of rebulite show that the Sb/As substitution is present in Sb3, Sb4, Sb5, As1 and As2 structural sites. Of them, Sb3 is always Sb dominated whereas other four vary from As- to Sb-dominated over the range of the solid solution. The change of the structural topology from jankovićite to rebulite, the closely related but not identical structures, is explained through necessity to accommodate the smaller volumes of the As coordination polyhedra and is accomplished through unit-cell twinning over the periodic (001)<sub>reb</sub> twin boundaries. The As end-member of the rebulite solid solution is in equilibrium with the phase of Tl<sub>2.4</sub>Sb<sub>0.68</sub>As<sub>7.18</sub>S<sub>13</sub> ideal composition, interpreted as imhofite.

Utilization of the potential of marginal land with adequate water and nutrients is an important factor in increasing corn production. However, high porosity levels and low nutrient levels cause growth and production of corn is not optimal. The aim of this research is to obtain an irrigation technique combined with efficient fertilization in the sweet corn cultivation system in marginal land. The field research was carried out at Faculty of Agricultural Research Wedomartani, Depok Sleman Yogyakarta. The study was conducted from September 2019 to February 2020. The research aims to get best sweet corn line at various concentrations of liquid organic fertilizer in marginal land with a drip irrigation system.The research was arranged in a completely randomized design (CRD) with 3 replications. The first factor is sweet corn lines, which consists of 9 levels, namely, G1 (5042B), G2 (KD-11), G3 (KD-13), G4 (SB-11), G5 (SB-13), G6 (SB-14), G7 (SB-16), G8 (SB-21), and G9 (SB-22). While the second factor is the irrigation system combined with fertilization (fertigation) which consists of 3 levels, namely the drip irrigation system with a concentration of liquid supplement fertilizer 2 cc / L, 3 cc / L and 4 cc / L. The results showed that the drip fertigation system with a concentration of POC 3 cc / L was able to give a better effect on plant height and leaf length in sweet corn. The crystal structure and powder X-ray diffraction (XRD) patterns for three skutterudite samples, Fe(Sb2+xTe1−x), x = 0.05, 0.10, 0.20, have been determined. These compounds crystallize in the cubic space group $Im\bar 3$. Te was found to randomly substitute in the Sb site. Because of the fact the covalent radius of Sb is greater than that of Te, a trend of increasing lattice parameter has been observed as the x value in Fe(Sb2+xTe1−x) increases [cell parameters range from 9.10432(4) to 9.11120(3) Å for x = 0.0 to 0.2, respectively]. The Fe–Sb/Te bond distance also increases progressively [from 2.5358(4) to 2.5388(4) Å] as the Te content decreases. While average Sb/Te–Sb/Te distances in the four-membered rings are similar in these three compounds, the average Sb/Te–Sb/Te edge distances in the octahedral framework increase progressively from 3.5845(12) to 3.5900(13) Å. Reference XRD patterns of these three phases have been prepared to be included in the Powder Diffraction File (PDF).

Purpose: Many children attend family child care homes (FCCHs), an important setting to influence children’s physical activity (PA) and sedentary behavior (SB). This study assessed children’s PA and SB while in FCCHs, characteristics of the FCCH PA environment, and relationships between the environment and child PA and SB. Method: Children ages 1.5–4.0 years (n = 495) were recruited from 165 FCCHs in North Carolina. Children’s moderate to vigorous PA and SB were measured via accelerometry for 3 days. FCCH PA environments were assessed over 2 days using the Environment and Policy Assessment and Observation for FCCHs. Ten subscores and an overall PA environment score (possible range: 0–30) were calculated; higher scores indicate better quality. Results: Children accumulated 30 (13) minutes of moderate to vigorous PA and 143 (42) minutes of SB in FCCHs daily. FCCHs scored low on the Environment and Policy Assessment and Observation for FCCHs, with an average overall score of 13 (2). FCCHs scored highest on screen time and screen time practices subscores, and lowest on PA education/professional development and PA policy subscores. Although no statistically significant associations were observed, some large Cohen d effect sizes were noted (eg, outdoor playtime subscore and moderate to vigorous PA). Conclusions: This study highlights opportunities to improve FCCHs and increase children’s behaviors (eg, providing adequate time and outdoor play spaces).

e18140 Background: Crizotinib can result in SB as a part of its dose-dependent pharmacodynamic property. We analyzed the correlation between SB and tumor response in ALK or MET+ NSCLC patients (pts). Methods: Single institution retrospectively analysis of SB incidence (HR < 55) of ALK or MET+ NSCLC patients enrolled in 2 phase II crizotinib trials. Comparison performed between pt characteristics, HR changes, treatment duration, tumor response and shrinkage according to SB status. Results: 42 ALK or MET+ NSCLC pts (30 from A8081001, 12 from PROFILE1005) rx’d with crizotinib 250 mg po bid were analyzed. 90% of pts had a HR decrease of > 10 beats per minutes (bpm). 19 (45%) patients developed at least 1 SB episode at anytime during crizotinib treatment including 8 (19%) pts with profound SB (HR =< 45). All pts with SB were asymptomatic. There was no difference in the median age of diagnosis between pts (57.6 yrs [SB] vs. 49.7 yrs [no SB]; p = 0.073) by SB status. Median time to SB was 3 wks (mean: 7.2 wks, range 2-32 wks). There was significant difference in the mean pre-Rx baseline HR by SB status (74.5 bpm [SB] vs. 92.6 bpm [no SB]; p = 0.005), Rx duration (57.3 wks [SB] vs. 33.3 wks [no SB]; p = 0.021). The average maximum HR decrease (MHRD) from baseline for all pts were 26.3 bpm. There was no significant difference in the mean MHRD from baseline according to SB status (28.2 bpm [SB] vs. 24.7 bpm [no SB]; p = 0.598). 68% (13/19) of SB pts compared to 35% (8/23) of pts without SB achieved a CR/PR (p = 0.030). The mean maximum decrease in tumor measurements (MDTM) (RECIST) was 46.2% for pts with SB compared to 32.0% for pts without SB (p = 0.095). There was no difference in the mean MDTM in pts who achieved CR/PR without SB (62.5%) and with SB (63.5%) (p = 0.913) nor the mean response duration (RD) in CR/PR pts by SB status (43.3 wks [SB] vs. 51.5 wks [no SB]; p = 0.426). Conclusions: HR decrease is common in pts treated with crizotinib. Pts who developed asymptomatic SB had significantly lower pre-Rx baseline HR, longer Rx duration, higher response rates. However there was no difference in the mean MDTM or RD between pts who achieved CR/PR by SB status.

The purpose of this study is to evaluate the effects of botulinum toxin type A (BoNT-A) for managing sleep bruxism (SB) in a randomized, placebo-controlled trial. Thirty SB subjects were randomly assigned into two groups evenly. The placebo group received saline injections into each masseter muscle, and the treatment group received BoNT-A injections into each masseter muscle. Audio–video–polysomnographic recordings in the sleep laboratory were made before, at four weeks after, and at 12 weeks after injection. Sleep and SB parameters were scored according to the diagnostic and coding manual of American Academy of Sleep Medicine. The change of sleep and SB parameters were investigated using repeated measures analysis of variance (RM-ANOVA). Twenty-three subjects completed the study (placebo group 10, treatment group 13). None of the SB episode variables showed a significant time and group interaction (p > 0.05) except for electromyography (EMG) variables. The peak amplitude of EMG bursts during SB showed a significant time and group interaction (p = 0.001). The injection decreased the peak amplitude of EMG bursts during SB only in the treatment group for 12 weeks (p < 0.0001). A single BoNT-A injection cannot reduce the genesis of SB. However, it can be an effective management option for SB by reducing the intensity of the masseter muscle.

Responses of past peak lactating Iranian Holstein cows to feeding roasted whole soybean (SB), raw SB or soybean meal (SBM) supplements were evaluated. Treatments consisted of a total mixed ration (TMR) with 34% forage (21% alfalfa hay and 13% corn silage) supplemented with 11.9% SBM or 13.3% roasted SB or raw SB. Diets were offered to 18 multiparous cows assigned randomly to one of three experimental diets for a 49-d trial. Dry matter intake was not significantly different, but total and fat-corrected milk yields were higher for cows fed the roasted SB diet than the other treatments. Milk fat concentration was not significantly different among treatments, but milk fat yield was significantly higher for cows fed the roasted SB and SBM diets. Milk protein concentration was significantly decreased by the raw and roasted SB diets, but milk protein yield was unaffected. Feeding roasted SB significantly reduced rumen ammonia N, plasma urea N and milk urea N concentrations. Ruminal pH, plasma glucose and beta-hydroxybutyrate concentrations were not affected. Compared with the other treatments, roasted SB increased plasma concentration of most essential amino acids, except leucine and phenylalanine. Although caution must be taken in the interpretation of the results due to the limited number of observations in the experiment, feeding roasted SB in a diet with alfalfa hay as the primary forage was beneficial. ABSTRACT In experimental visceral leishmaniasis, inhibition of interleukin 10 (IL-10) signaling enhances Th1-cell-associated responses, promoting gamma interferon (IFN-γ) secretion, granuloma assembly, macrophage activation with substantial liver parasite killing, and synergy with pentavalent antimony (Sb) chemotherapy. To determine if inhibiting other suppressive cytokines has similar therapeutic potential, Leishmania donovani-infected BALB/c mice were injected with anti-IL-4 monoclonal antibody or receptor fusion antagonists of IL-13 or transforming growth factor β (TGF-β). Targeting IL-13 or TGF-β enabled inhibition of L. donovani replication but little parasite killing; anti-IL-4 had no effect. None of the three antagonists promoted IFN-γ production, granuloma maturation, or Sb efficacy. Excess IL-13 and TGF-β exacerbated liver infection; however, effects were transient. Among IL-10, IL-4, IL-13, and TGF-β, cytokines capable of disabling Th1-cell mechanisms (including those which support chemotherapy), IL-10 appears to be the appropriate target for therapeutic inhibition in visceral L. donovani infection.

Nanoparticles of antimony-doped tin oxide (ATO) were characterized for 0–33.3% Sb doping, both in aqueous dispersion and as dried powder. Antimony is incorporated in the cassiterite SnO2 structure of the ATO nanoparticles (d ≈ 7 nm) up to the highest doping levels, mainly as SbV, but with increasing Sb doping the SbIII content increases. We found adsorption of NH3 at the particle surface and evidence for the incorporation of nitrogen in the crystal lattice of the particles. The total nitrogen content increases with increasing Sb doping of the particles. Compact powder conductivity measurements show an increase in conductivity of ATO powder up to 13% Sb and a small decrease for higher Sb contents. Furthermore, we show that these particles can be used to prepare highly transparent conductive cross-linked ATO/acrylate nanocomposites with a continuous fractal particle network through the polymer matrix and a very low percolation threshold (ϕc ≈ 0.3 vol%).

The strong association between penetrating colon injuries and intra-abdominal abscess (IAA) formation is well established and attributed to high colon bacterial counts. Since trauma patients are rarely fasting at injury, stomach and small bowel colony counts are also elevated. We hypothesized that there is a synergistic effect of increased IAA formation with concomitant stomach and/or colon injuries when compared to small bowel injuries alone. Consecutive patients at a level one trauma center with penetrating small bowel (SB), stomach (S), and/or colon (C) injuries from 1996 to 2012 were reviewed. Logistic regression determined associations with IAA, adjusting for age, gender, Injury Severity Score (ISS), admission Glasgow Coma Score, transfusions, and concurrent pancreas or liver injury. A total of 1518 patients (91% male, ISS = 15.9 ± 8.4) were identified: 496 (33%) SB, 231 (15%) S, 288 (19%) C, 40 (3%) S + SB, 69 (5%) S + C, 338 (22%) C + SB, and 56 (4%) S + C + SB. 148 (10%) patients developed IAA: 4 per cent SB, 9 per cent S, 10 per cent C, 5 per cent S + SB, 22 per cent S + C, 13 per cent C + SB, and 25 per cent S + C + SB. Multiple logistic regression demonstrated that ISS, 24 hour blood transfusions, and concomitant pancreatic or liver injuries were associated with IAA. Compared with reference SB, S or S + SB injuries were no more likely to develop IAA. However, S + C, SB + C, and S + C + SB injuries were significantly more likely to have IAA. In conclusion, combined stomach + colon, small bowel + colon, and stomach, colon, + small bowel injuries have a synergistic effect leading to increased IAA formation after penetrating injuries. Heightened clinical suspicion for IAA formation is necessary in these combined hollow viscus injury patients.

Agro-wastes are abundant in both rural and urban areas and when carelessly disposed off to the environment by dumping or burning, they lead to environmental pollution, and consequently to health hazards. Growing of edible mushroom on these wastes can contribute to decrease in environment pollution. This work investigated the growth and yield of Pleurotus ostreatus on different agro-wastes: plantain midrib (PM), corn straw (CS), sugarcane bagasse (SB) and their combinations (PM+CS, PM+SB, CS+SB, CS+PM+SB) with a view to finding the best substrate for the growth of P. ostreatus. The experiment was carried out using completely randomized design with seven treatments and three replications. Results revealed that mycelia growth period and days to pinhead formation were faster in corn straw (7 and 11 days, respectively from the day of inoculation), this was followed by CS+SB (8 and 12 days, respectively) while CS+PM+SB recorded the longest days (13 and 17, respectively). Stipe length ranged from 2.6 cm in PM to 3.3 cm in CS+PM while the pileus diameter was between 3.3 cm in CS and PM and 4.3 cm in PM+SB. Number of fruiting bodies, fresh and dry weights of the mushroom were significantly higher in CS than in the other substrates. The least number of fruiting bodies, fresh and dry weights were recorded in CS+PM+SB.

Background: Sedentary behaviour (SB) can delay hospitalised older adults’ recovery from acute illness and injuries. Currently, there is no synthesis of evidence on SB among hospitalised older people. This scoping review aimed to identify and map existing literature on key aspects of SB among hospitalised older adults, including the prevalence, measurement and intervention strategies for SB and sedentary behaviour bouts (SBBs) as well as healthcare professionals, patients and carers’ perspectives on interventions. Methods and analysis: Several electronic databases were searched between January 2001 and September 2020. The Joanna Briggs Institute (JBI) framework was used to conduct this scoping review. Results: Out of 1824 articles, 21 were included comprising 16 observational studies, 3 randomised controlled trials, 1 comparative study, and 1 phase-1 dose-response study. The sample size ranged from 13 to 393, with all 1435 participants community-dwelling before hospitalisation. Only two studies focused on measuring SB and SBBs as a primary outcome, with others (n = 19) reporting SB and SBB as a sub-set of physical activity (PA). Older adults spent an average of 86.5%/day (20.8 h) sedentary. Most studies (n = 15 out of 21) measured SB and SBB using objective tools. Conclusion: Hospitalised older people spent most of their waking hours sedentary. Studies explicitly focused on SB and SBB are lacking, and the perspectives of patients, carers and healthcare professionals are not clarified. Future hospital-based studies should focus on interventions to reduce SB and SBB, and the perspectives of healthcare professionals, patients and carers’ taken into account.

Kloramfenikol merupakan obat pilihan lini pertama untuk terapi demam tifoid tanpa komplikasi pada anak sampai saat ini. Antibiotik lain yang dipergunakan adalah tiamfenikol, sefiksim dan azitromisin. Tujuan.Melakukan evaluasi respon antibiotik yang digunakan dalam terapi demam tifoid tanpa komplikasi di Bagian Ilmu Kesehatan Anak RSU Prof.Dr.R.D.Kandou, Manado.Metode.Penelitian kohort retrospektif dilakukan di Bagian Ilmu Kesehatan Anak RSU Prof.Dr.R.D.Kandou, Manado, Juli 2007-Juni 2012 pada anak usia 6 bulan-13 tahun dengan diagnosis demam tifoid. Data diperoleh dari rekam medik pasien. Waktu bebas demam dan lama rawat pada tiap kelompok antibiotik di data. Analisis statistik dilakukan dengan menggunakan ANOVA dengan uji F dengan program SPSS 17.Hasil. Didapatkan 161 kasus yang memenuhi kriteria inklusi. Antibiotik terbanyak dipakai adalah kloramfenikol (31,1%), tiamfenikol (27,3%), sefiksim (23%), dan azitromisin (18,6%). Waktu bebas demam paling pendek dicapai pada kasus yang diberikan azitromisin yaitu 37,9 (SB 32,8) jam, diikuti oleh kloramfenikol 40,3 (SB 28,3), tiamfenikol 45,3 (SB 38,1) dan sefiksim 50,8 (SB 32,3). Rerata lama rawat paling cepat ditemukan pada kelompok kloramfenikol yaitu 4,4 (SB 1,3) hari, diikuti dengan azitromisin 4,6 (SB 1,3), tiamfenikol 4,8 (SB 1,7) dan sefiksim 4,8 (SB 1,6) Tidak terdapat perbedaan bermakna rerata waktu bebas demam dan lama rawat keempat jenis antibiotik.Kesimpulan.Pemberian antibiotik kloramfenikol, tiamfenikol, sefiksim, dan azitromisin pada demam tifoid anak tidak terdapat perbedaan bermakna pada rerata waktu bebas demam dan lama rawat inap.

Introductions: Stillbirth (SB) is one of the most common adverse outcomes of pregnancy. The aim of this study was to determine the SB rate and to identify the likely causes contributing to SB. Methods: This cross-sectional study was conducted at Patan Hospital from 15th June 2014 to 14th June 2017 for all the cases of SBs, at or after 22 weeks, birth weight of 500 gm or more. The perinatal outcome, demographic profile, fetal characteristics, causes and contributing factors were analyzed. Results: There were 262 SB out of total 23069 deliveries, (11.24 per 1000) and 119 (46.12%) had antenatal check-up (ANC) at Patan Hospital. The 214 (82.95%) SB were among 20-34 years mothers, 133 (51.55%) being multigravida. Antepartum SB were 234 (89.31%), macerated 213 (81.30%), birth weight <1000gm 86 (32.82%) and male 156 (59.54%). The intrauterine growth restriction (IUGR) was present in 60 (22.90%), unexplained casue in 43 (16.41%), prematurity 28 (10.69%), congenital anomalies 26 (9.92%), pre-eclampsia 19 (7.25%), gestational diabetes, and abruptio placenta each 13 (4.96%). Delay in seeking care in 202 (78.30%) was a potential contributing factor. Conclusions: The SB was 11.24/1000 births. The causes in descending order were IUGR, unexplained, prematurity, congenital anomalies, pre-eclampsia, gestational diabetes and abruptio placenta. Delay in seeking care was found as a potential contributing factor.

AbstractReaction of SbCl3 with KPcp (Pep = pentamethylcyclopentadienyl) in etheral solution at -78 °C affords dimeric pentamethylcyclopentadiene and orange-red (PcpSb)4 in high yield. 1H, 13 C NMR, IR, UV and mass spectra are reported. The tetrameric structure was found by a crystal structure determination carried out with X-ray diffraction data collected at -50 °C (928 independent observed reflexions. R = 0.055). Crystal data: tetragonal, space group I41/amd, a = 2400.0(5), c = 781.7(5) pm, Z = 4. The (PcpSb)4 molecules have a non-planar Sb4 ring with Sb -Sb bond lengths of 285.6(1) pm, one η1-Pcp ring is attached to each Sb atom. Contrary to the solid state structure, monomeric molecules are found in benzene and hexane solutions.

Purpose: Increases in maximal oxygen uptake () and running economy improve performance in long-distance runners. Nevertheless, long-distance runners require sprinting ability to win, especially in the final phase of competitions. The authors determined the relationships between performance and sprinting ability, as well as other abilities in elite long-distance runners. Methods: The subjects were 12 elite long-distance runners. Mean official seasonal best times in 5000-m (5000 m-SB) and 10,000-m (10,000 m-SB) races within 1 year before or after the examination were 13:58.5 (0:18.7) and 28:37.9 (0:25.2) (mean [SD]), respectively. The authors measured 100-m and 400-m sprint times as the index of sprinting ability. They also measured and running economy ( at 300 m·min−1 of running velocity). They used a single correlation analysis to assess relationships between 5000 m-SB or 10,000 m-SB and other elements. Results: There were significant correlations between 5000 m-SB was significantly correlated with 100-m sprint time (13.3 [0.7] s; r = .68, P = .014), 400-m sprint time (56.6 [2.7] s; r = .69, P = .013), and running economy (55.5 [3.9] mL·kg−1·min−1; r = .59, P = .045). There were significant correlations between 10,000 m-SB and 100-m sprint time (r = .72, P = .009) and 400-m sprint time (r = .85, P < .001). However, there was no significant correlation between 5000 m-SB or 10,000 m-SB and (72.0 [3.8] mL·kg−1·min−1). Conclusions: The authors' data suggest that sprinting ability is an important indicator of performance in elite long-distance runners.

Aim. Causes, clinical features, and diagnostic approaches for small bowel (SB) bleeding were analyzed to derive recommendations in dealing with this clinical condition. Methods. We included 54 patients undergoing surgical treatment for SB bleeding, from January 2009 to December 2019. Detailed clinical data, diagnosis procedures, and causes of bleeding were collected. Results. Among 54 cases with SB bleeding, the most common causes were tumors (64.8%), followed by angiopathy (14.8%), ulcers (9.3%), diverticula (5.6%), tuberculosis (3.7%), and enteritis (1.9%). Most tumors (32/35 cases, 91.4%) and vascular lesions (8/8 cases, 100%) were located in the jejunum. The incidence of tumors was higher in the older (30/41 cases, 73.1%) than that in patients younger than 40 years of age (5/13 cases, 38.5%, P < 0.01 ). Common initial findings were melena (68.5%) and hematochezia (31.5%). The overall diagnostic yield of computed tomographic enterography (CTE) was 57.4% (31/54 cases), with the figures for tumors, vascular lesions, and inflammatory lesions being 71.4% (25/35 cases), 62.5% (5/8 cases), and 12.5% (1/8 cases), respectively. Double-balloon enteroscopy (DBE) definitively identified SB bleeding sources in 16/22 (72.7%) patients. Conclusion. Tumors, angiopathy, ulcers, and diverticula were the most common causes of SB bleeding in Northern Vietnamese population. CTE has a high detection rate for tumors in patients with SB bleeding. CTE as a triage tool may identify patients before double-balloon enteroscopy because of the high prevalence of SB tumors.

Measures to contain the spread of coronavirus disease 2019 (Covid-19) imposed by governments have undoubtedly impacted on preventing its spread but may have also produced longer periods of sedentary living across all segments of society. To examine this phenomenon, this study compared the sedentary behavior (SB) of Thai adults before and during the Covid-19 pandemic. The 2019 and 2020 datasets of Thailand’s Surveillance on Physical Activity (SPA) were employed. A total of 5379 (SPA2019) and 6531 (SPA202020) persons age 18–64 years who had access to the Internet were included in the analysis. Measures imposed to contain the spread of Covid-19 infection were significantly associated with lower opportunity of Thai adults for work-related physical movement, and that increased their SB, particularly with the shift from onsite to online working platforms. Cumulative SB increased from 824 (before the pandemic) to 875 min/day during the pandemic. The odds of accumulating >13 h/day of SB was highest among females, young adults, those who completed post-secondary education, unemployed or working in the non-agriculture sector, having a chronic disease/condition, residing in an urban area, and living in a ‘higher-risk’ pandemic zone. The insignificant association of physical activity (PA) and the Fit from Home (FFH) intervention in reducing SB during the pandemic suggests that PA is not directly associated with SB, and that the FFH intervention was insufficient to prevent SB.

Abstract Background Economy is an important part of chronic pain. Aim To describe the economy in chronic non-malignant pain patients attending a 13-week Rehabilitation Program (RP). Methods All patients participating in the RP 2006–2008 were evaluated at baseline (BL) and at follow-up (FU) after an observation period of mean … month in relation to: (1) work-income (WI) or (2) transfer income (TI), comprised by: (a) sick-leave (SL), (b) sick pension (SP), (c) social benefit (SB) and (d) rehabilitation benefit (RB). The economic impact on state and county and the time to age pension was calculated. Results 117 patients attended the RP. At BL 23 patients had WI and 19 maintained this at FU (3 were on SP and 1 on RB). 90 patients were on TI at BL: 58 on SL at BL changed to 20 on SP + 23 on WI + 6 on SB + 1 on RB + 6 maintained SL, 12 on SB at BL changed to 6 on WI+ 1 on SP + 5 still on SB. 7 on RB at BL changed to 6 on WI + 1 on RB. The economic situation was concluded for 97 patients (83%). State expenses were increased by 540,000 Euro and county savings was 698,000 Euro. The societal savings were 158,000 Euro. The total costs for the RP was 421,000 Euro. Costs balanced savings after 2.7 years. The average time to age pension for the participating patients was 25 years. The potential accumulated savings thus amounted to 3.5 million Euros. Conclusions The 13-week Rehabilitation Program was highly cost effective: expenses for the program balanced savings after 3 years and the time to age pension for the participating patients was 25 years. The potential accumulated saving per patient was 30,000 Euro.

Objective: This study was designed to compare the sensitivity, specificity, efficiency, positive and negative predictive values, and ease of use for 2 commercially available hybridization kits for detecting human papillomavirus (HPV) DNA: Oncor Southern blot (SB) (Oncor, Inc., Gaithersburg, MD) and Digene ViraType dot blot (DB) (Digene Diagnostics, Inc., Silver Spring, MD).Methods: A total of 179 specimens (172 cervical and 7 penile biopsies) were assessed for acceptability based on the presence of epithelial cells and tested for HPV by DB and SB. The results were evaluated based on Papanicolaou-stained cervical specimens and selected risk factors.Results: One hundred six (97.2%) of 109 results were concordant, i.e., 93 negative (85.3%) and 13 positive (11.9%). Using SB as the gold standard, we found the sensitivity, specificity, efficiency, and positive and negative predictive values for the ViraType DB to be 100%, 96.9%, 97.3%, 81.3%, and 100%, respectively. Comparing the Papanicolaou smear to SB and DB, we found the sensitivity, specificity, efficiency, and positive and negative predictive values to be 33.3% (SB) vs. 44.4% (DB), 89.5% vs. 87.6%, 87.3% vs. 84.2%, 11.8% vs. 23.5%, and 97.0% vs. 94.9%, respectively. The only significant risk factor for predicting an HPV infection was the number of sexual partners.Conclusions: Although SB has been considered the standard model, DB is an acceptable method for detecting and identifying HPV infections.

Abstract Context Time spent in moderate-to-vigorous physical activity (MVPA), but not in sedentary behavior (SB), is related to cardiometabolic risk among non-Hispanic white youth. Objective Examine associations of SB and MVPA with cardiometabolic risk factors among Hispanic/Latino youth. Design Cross-sectional analysis. Setting Four US communities. Participants Hispanic/Latino youth (N = 1,426) ages 8 to 16 years. Measurements Associations of MVPA and SB, measured using 7-day accelerometer data (independent variables), with markers of glucose and lipid metabolism, inflammation, and endothelial function (dependent variables), were assessed in multivariable linear regression models while adjusting for sociodemographic characteristics and accelerometer wear time. Additional models controlled for obesity measures. Results SB comprised a mean (SD) of 75% (13%) of accelerometer wear time; mean (SD) time of MVPA was 35 min/d (22 min/d). Deleterious levels of high-density lipoprotein–cholesterol (HDL-C), triglycerides, insulin resistance, C-reactive protein, and plasminogen activator inhibitor-1 were associated with lower levels of MVPA and higher levels of SB (all P &lt; 0.05). Associations of MVPA with log-transformed triglyceride concentrations (β per 15-min/d increment, −0.039; SE, 0.018; P = 0.037) and SB with HDL-C (β per 30-min/d increment, −0.63; SE, 0.26; P = 0.018), but not those with other markers, remained significant after adjusting for MVPA or SB and further adjustment for body mass index and waist circumference. Higher SB tertiles were associated with lower soluble receptor for advanced glycation end products in fully adjusted models (P for trend = 0.037). Conclusions Physiological precursors of diabetes and cardiovascular disease were associated with MVPA and SB among US Hispanic/Latino youth, a group that bears a disproportionate burden of metabolic disorders.

Abstract OBJECTIVE The purpose of this study was to assess the predictive value of response to sympathetic blockade (SB) on the success rate of spinal cord stimulation (SCS) in patients with complex regional pain syndrome. METHODS We performed a retrospective study on 23 patients with complex regional pain syndrome who underwent both SB and subsequent SCS trials in the past 3 years at the Massachusetts General Hospital Pain Center, Boston, MA, and Walter Reed Army Medical Center, Washington, DC. Fifteen of these patients underwent permanent placement of an SCS device, and pain relief at 1- and 9-month follow-up was recorded. RESULTS Among the 23 patients included in the study, those having transient pain relief with SB were more likely to have a positive SCS trial: all 13 with positive SB had good pain relief during the trial, compared with only 3 of the 10 with negative SB (100% versus 30%, P &lt; 0.001). Among the 10 patients with negative SB, 7 noted poor pain relief during the trial despite adequate coverage, and they did not undergo placement of a permanent device. Among the patients who underwent permanent placement of an SCS device, those who received good pain relief with SB were more likely to have greater than 50% pain relief at 1-month follow-up (100% versus 33%, P = 0.029) and 9-month follow-up (87.5% versus 33.3%, P = 0.15). CONCLUSION We conclude that patients with good response to SB before SCS are more likely to have a positive response during their SCS trial and long-term pain relief after placement of permanent SCS device.
