Full textThis thesis proposes an effort to better characterize alexithymia in people with recent-onset schizophrenia (SZ) compared to non clinical controls through three major steps. The first step is to critically review the previous studies on alexithymia in SZ (n = 10) which, despite important conceptual variability and methodological caveats, highlight high severity of alexithymic features that are relatively independent from major SZ symptoms and associated to symptoms of general psychopathology, and that can be reliably measured with the TAS-20. The second step involves the comparison of two groups (SZ n = 33, Controls n = 31) regarding alexithymic features and the relationships between alexithymia, mentalizing and social cognition. It reveals that despite a higher severity of alexithymia and a mentalizing impairment in people with SZ, there is no association between both constructs in this group, thus infirming our hypothesis. However there is a significant negative association between them in the control group. The third step, as an attempt to find an alternative measure to the TAS-20 questionnaire in people who are known to show poor insight and emotional awareness, involves the study of psychometric properties of the Observer Alexithymia Scale (OAS) when it is rated by mental health professionals among people with SZ (n = 33). It shows that the OAS displays good psychometric properties in general, though it is moderately to strongly correlated to negative symptoms of SZ. This is an important limit to its use in this clinical population, at least when it is rated by a mental health professional. Results could be more accurate if the OAS was rated by a close relative, as suggested for future research. Overall, the results from the current thesis support the existence of important difficulties in emotion processing in people with SZ, principally characterized by the alexithymic feature “difficulty identifying emotions”, that are not related to mentalizing nor to symptoms but associated to a diminished social functioning. Some explanations, such as the specificity of alexithymia measures and the possible presence of a particular type of alexithymia in people with SZ, are proposed.

Full textOnly clozapine, a second generation agent, presents an interest for the effective treatment of schizophrenia cases that are rebel to be treated with conventional neuroleptics, and also the only one that reduces the risk of suicide. Its use is limited because of the monitoring it requires. Thus, a monitoring associated with a clozapine therapy becomes a clinical essential tool. This monitoring helps to optimize the efficiency of the therapeutics, prevents an excessive toxicity of clozapine and reduces the risk of relapse. Nevertheless, numerous interindividual variations have been widely reported, this could be attributed to environmental factors, such as smoking, or physiological, such as gender or age, or still genetic, such as polymorphisms on the gene encoding for CYP1A2, the main enzyme that catabolise the clozapine. The aims for this study were: to provide a efficient and standard clinical tool with a precise tracking of pharmacokinetics of clozapine, to acquire a better understanding of interindividual variations in relation to the fact of smoking on the therapeutic effects of clozapine, explain some genetic polymorphisms at the level of the gene encoding the CYP1A2 and at the level of the glutathione S-transferases (GST), enzymes of the metabolism of the phase II, which may promote the development of schizophrenia. Thus, the thresholds of toxicity and therapeutic efficacy of clozapine must be considered individually. In addition, the clozapine serum level is an important parameter in the periodical medical monitoring. The ratio "CYP1A2 metabolic index" to "plasma clearance of clozapine" could be considered as a plasmatic marker adaptable to a collective level. The SNP -3860G>A, the deactivated form of CYP1A2, could be a genetic risk factor. Furthermore, our study confirms the role of GSTT1 as a factor predisposing to schizophrenia

Full textWe developed a self-rated instrument for evaluating subjective quality of life in patients suffering from schizophrenia : the S-QoL. The S-QoL includes 41 questions and 8 dimensions (psychological well-being, self esteem, physical well-being, relations with family, relations with frieds, sentimental life, autonomy, resilience), with a global index. We present the results of validation studies showing that the S-QoL is valid, reliable and has good responsiveness. The S-QoL has been compared with two standard instruments : the QoLI and the SF-36. In regression analysis studies, explanatory factors of the S-QoL are : paranoid symptoms, hostility, depression, age, previous comorbidity with alcohol abuse and depression, daily activities, and the number of weekly contacts with family and friends. Preliminary results show that self-evaluation with the S-QoL is not influenced by memory and executive function deficits but has statistical relationships with insight. The interest of self-rating has been verified by comparing evaluations of patients and referent health professionals. A preliminary study describes quality of life from the caregiver's point of view

Full textThis doctoral research interests the field of psychosocial rehabilitation, promoting the fight of stigma in severe mental disorders. The objective is to better understand the schizophrenia stigma through the study of stereotype components and its effect on current clinical neuropsychological practice. The review of stigma scientific publishing underlines a great methodological heterogeneity regarding schizophrenia stereotype exploration, that can be explain by the scientific partitioning of Social psychology and Psychiatry research. Consequently our doctoral research was aimed at integrating these two bodies of research to enhance the operationalization of schizophrenia stereotype content and its potential effects on cognitive performances.We conducted two studies aimed at: i) exploring schizophrenia content stereotype in families and the general population in France; ii) investigating the impact of stereotype threat on cognitive performances in schizophrenia.The first study shows that schizophrenia stereotype is more strongly rejected by families, especially in its “Dangerousness” dimension. Results also suggest that “Incompetency” is a core dimension of schizophrenia stereotype, both in families and the general population. The integration of our results with the models of Social psychology leads us to propose a model of schizophrenia stereotype content in which the Dangerousness and Incompetency dimensions would both predict specific patterns of emotions and discriminatory behaviors.The second experimental study failed to show any negative effect of schizophrenia stereotype threat on executive performances. Finally, this failure leads us to discuss the methodological issues of experimental designs, especially regarding their application in schizophrenia. Thus, several factors of interest, like the identification domain, the choice of cognitive measures and the consideration for various potential threats appear to be relevant for future research
