Full textNeo-Malthusianism is a line of thought which gathers education specialists,doctors, anthropologists and women's and anarchist activists. Its reflexion and its actionhave a political, economic and scientific dimension. Its project is to deeply transformhuman societies through birth control in order to reach a better social organisation, likelyto meet human beings’ legitimate aspiration for happiness. The means implemented toreach this goal are the generalisation of contraceptive prophylaxis, which includescontraceptive methods and abortion. The neo-Malthusian movement was particularlyactive in France, from the 1890ies to the eve of World War II. It has also met with strongopposition and was actively fought, noticeably after World War I. Observing the number ofbirths, neo-Malthusianism points how incompatible the exponential increase of births iswith the possibility for the members of a given community to have what they need to reachhappiness. Hence, French neo-Malthusianism is inseparable from eugenics, which is acentral theme in the history of this movement, from its emergence to its last developmentsin the 1930ies. The objective of the present work is to draw a new and previouslyunpublished synthetic portrait of this movement, focussing on its link to bio-medicalsciences and scientific culture. [...]

Full textLa résistance aux antibiotiques et leur utilisation inappropriée constituent un problème majeur de santé publique qui affecte particulièrement les pays à revenus faibles et intermédiaires. Les données concernant cette problématique sont très limitées au Pérou. Nous avons interrogé 256 médecins de deux hôpitaux à Lima afin d’évaluer leurs connaissances et leurs pratiques vis-à-vis de l’utilisation des antibiotiques et de la résistance aux antibiotiques. La plupart des médecins étaient conscientisés à la problématique de la multirésistance chez les entérobactéries, mais seulement 20% d’entre eux estimaient correctement le taux résistance de Klebsiella pneumoniae aux céphalosporines à large spectre au sein de leurs institutions. Au cours de ce travail, un réseau entre neuf hôpitaux de Lima a été implémenté afin de suivre la résistance aux antimicrobiens des souches isolées d’hémocultures. Au cours des années 2008-2009, le Staphylococcus aureus était la bactérie Gram-positive la plus fréquente (22%) ;K. pneumoniae et Escherichia coli représentaient les bactéries Gram-négatives les plus souvent isolées d’hémocultures. Parmi ces dernières entérobactéries, 75% produisaient des β-lactamases à spectre étendu (BLSE) avec un taux élevé de co- résistance à la ciprofloxacine et à la gentamicine. Les souches de S. aureus isolées d’hémocultures chez les patients hospitalisés et de dépistage chez les professionnels de santé (PS) ont été caractérisées pour leur profil de résistance aux antibiotiques et génotypées par biologie moléculaire. La moitié des 338 souches isolées d’hémocultures étaient résistantes à la méticilline (« MRSA ») et appartenaient à un clone prédominant disséminés dans ces différents hôpitaux, appelé le ST 5- spa t149-SCCmec I, ou le clone Cordobes-Chilien. Le taux de portage nasal de MRSA parmi les PS était à 8,7% ;les deux génotypes les plus fréquemment observés chez les PS appartenaient aux mêmes clones que ceux retrouvés majoritairement chez les patients hospitalisés. Cette observation suggère une transmission horizontale. L’implémentation de politiques de contrôle de l'infection est primordiale dans les établissements de santé au Pérou afin de réduire la transmission de micro-organismes multi-résistants.

Full textThe biological materiality of the human body has become an object of unprecedented interventions through “new biomedical technologies” as medically assisted procreation, genetic tests, or hormonal contraception. This thesis interrogates the difficulties inherent to anti-naturalist approaches in order to address the biological dimension of sexed bodies. “One is not born a woman, one becomes one”, but is this also true for the body? The analytical frames of materialist or deconstructivist feminism cannot cease the modalities through which biomedical technologies invest the flesh. Do biomedical technologies make constructivist approaches obsolete through the revenge of a biological – hormonal, genetic, molecular – ground that tops the anatomical effects of socialization? Partisans of a feminist “material turn” seem to think so. After analyzing how the molecular biology revolution changes the very concept of nature in defining it as “life itself”, I offer an alternative interpretation by defining biomedical technologies as technologies of power that stem from a molecular biopolitics of gender. Instead of overturning constructivist perspectives, these new social mediations (residing on a new epistemic paradigm) help understanding a shift in what has been seen as the limits of gender. This shift creates unprecedented identities, experiences and subjectivities of gender. In exposing the coordinates of the biomedical apparatus, this comparative study between disciplinary techniques and molecular biopolitics of gender pleads for a renewed anti-naturalist critique that takes the form of a critique of technology in order to allow for a collective appropriation of biomedical technologies.

Full textIn radiation therapy, the accuracy of the dose delivered to the patient during the course of treatment is of great importance to progress towards improved quality and coherence of the outcome data. One of the first steps to evolve towards a Clinical-Decision Support System (CDSS) is to be able to accurately reconstruct that delivered dose, taking into account the range of factors that can potentially generate significant differences between the planned dose visualized on the screen of the dosimetrist, and the actually delivered dose accumulated during the treatment sessions. These factors include accelerator output variations, commissioning uncertainties, dose computation errors, patient and organ movement, etc.The objective of this work is to implement and test a Monte Carlo platform for the validation of the Cyberknife and Tomotherapy systems installed at Centre Oscar Lambret. A study of a small field-dedicated detector (the microLion ionization chamber) is also included, this detector being particularly suited for measurements on the Cyberknife system.The context and theoretical concepts are introduced in the first two chapters. In the third chapter, the Monte Carlo modelling of the Cyberknife and microLion detector is detailed. The fourth part includes the description of the Monte Carlo platform Moderato and its evaluation module. In the final chapter, the modelling of the latest MLC-equipped Cyberknife model (the M6) is described. A new technique is also introduced to accelerate the optimization of the beam electron parameters of a Monte Carlo model, thus allowing for an easier and more automated use of the Moderato system.

Full textAbstract:Regorafenib is one of the last treatment options for patients with chemo-refractory metastatic colorectal cancer (mCRC), associated with some efficacy but with important toxicities impairing its use in patients with poor general condition. As a multi-targeted tyrosine kinase inhibitor, its mechanism of action remains largely unknown, challenging the identification of clinically useful predictive biomarkers. The ultimate aim of our work was to contribute to the identification of mCRC patients unlikely to benefit from regorafenib. To achieve this objective, we moved from translational to mechanistic studies.Our translational studies focused on therapy response monitoring using liquid biopsies obtained from a prospective clinical trial in mCRC patients treated with regorafenib (RegARd-C trial). We first, confirmed the prognostic value of cell-free DNA (cfDNA) level before treatment. Similarly, we found that high level of pre-treatment metabolically active tumor volume was associated with poor prognosis. When combined, these markers remained predictors of patients’ progression-free (PFS) and overall survival (OS).Then, we measured circulating tumor DNA (ctDNA) based on tumor-specific mutations already identified in patients’ tumor tissue by NGS. We demonstrated that an increasing ctDNA level more than 50% after 14 days of treatment, either based on one or on multiple mutations, is correlated with patients’ clinical outcome in terms of PFS and OS. Furthermore, our data strongly suggested that both baseline cfDNA and ctDNA dynamics are strong complementary predictors of both PFS and OS. Also, we highlighted the leading role of Carbohydrate Antigen 19-9 as a prognostic and early predictive biomarker of mCRC patients’ outcome.Having observed an overall resistance to regorafenib in the majority of patients, we investigated the potential related intrinsic and acquired resistance mechanisms in representative CRC cell lines and found rather different degrees of intrinsic resistance to regorafenib in these cell lines. We then explored potential mechanisms of resistance after short and long-term exposure to regorafenib. The investigation of MAPK and PI3K/AKT pathways pointed to the latter as a major player in acquired resistance.Another observation based on particular cell morphological changes led us to investigate in deep a drug-initiated senescence-like phenotype that is also known to cause drug resistance. SW480 cells were able to acquire stable senescent-like properties, also promoted by a specific senescence-associated secretome, and cell cycle arrest. In line with tumor cell dormancy this phenotype may have a positive impact on patient’s outcome as long as he is under treatment. However, dormant cells contribute to disease recurrence after drug withdrawal. In contrast, HCT-116 cells undergo senescent properties after short exposure and develop acquired resistance triggering EMT, which is also associated with drug resistance. This latter mechanism could be deleterious for the patient as the appearance of more aggressive tumor subclones may induce disease progression and worsen clinical outcome.We also investigated Multi-Drug Resistant protein 1 (MDR1) as a possible additional mechanism of resistance and we found that regorafenib seems to overcome MDR in SW480 treated cells while in HCT-116, an increase of MDR1 expression was observed after short and long exposure compared to parental cells.In conclusion, we contributed to a better understanding of different mechanisms of resistance to regorafenib in CRC. We pointed to cell plasticity such as senescence and EMT in addition to possible MDR as major players; and certainly highlighted the high heterogeneity of the disease. Will regorafenib gain a better place as an efficient drug in CRC? Our work provides some insights for answering this question.
