Abstract:
INTRODUCTION Low back pain effects up to 80% of the population at some time during their active life. Questionnaires are available to help measure pain and disability. The Oswestry Disability Index (ODI) is the most commonly used outcome measure for low back pain. The aim of this study was to see if training in completing the ODI forms improved the scoring accuracy. PATIENTS AND METHODS The last 100 ODI forms completed in a hospital's spinal clinic were reviewed retrospectively and errors in the scoring were identified. Staff members involved in scoring the questionnaire were made aware of the errors and the correct method of scoring explained. A chart was created with all possible scores to aid the staff with scoring. A prospective audit on 50 questionnaires was subsequently performed. RESULTS The retrospective study showed that 33 of the 100 forms had been incorrectly scored. All questionnaires where one or more sections were not completed by the patient were incorrectly scored. A scoring chart was developed and staff training was implemented. This reduced the error rate to 14% in the prospective audit. CONCLUSIONS Clinicians applying outcome measures should read the appropriate literature to ensure they understand the scoring system. Staff must then be given adequate training in the application of the questionnaires.

Abstract:
A variety of tools have been developed to assist in the quantification and analysis of naturalistic language samples. In recent years, computer technology has been employed in language sample analysis. This study compares a new automated index, Scoring Sentences Developmentally (SSD), to two existing measures. Eighty samples from three corpora were manually analyzed using DSS and MLU and the processed by the automated software. Results show all three indices to be highly correlated, with correlations ranging from .62 to .98. The high correlations among scores support further investigation of the psychometric characteristics of the SSD software to determine its clinical validity and reliability. Results of this study suggest that SSD has the potential to compliment other analysis procedures in assessing the language development of young children.

Abstract:
Distribution tables have become ubiquitous to the tax policy debates surrounding major legislative initiatives to change tax law at the federal level. The fairness of any proposed change to federal tax policy has become one of the most highlighted components of tax policy discussions. The presentation of tax data within distribution tables can hide or omit important information that is required in order to effectively evaluate the merits of any tax legislation. Many producers of distribution tables show only the information necessary to present their policy preferences in the best possible light. The different economic assumptions and presentations of data used by the various groups that release distribution tables have the inherent consequence of providing the public with numerous tables that are often used as political ammunition to influence and shape debate. The purpose of this research is to contribute to the tax policy research literature by exploring the limitations and biases inherent in specific designs of tax distribution tables and in specific methodological approaches to tax distribution analysis. This is done by means of a systematic examination of how different designs and methodologies provide an incomplete picture of a proposed change to federal tax policy. By comparing distribution tables as used by different groups to provide alternative perspectives of various tax proposals, the research shows how the use of tax distribution tables often provides misleading results about the impact of proposed tax legislation in order to influence and shape the issues surrounding a proposed change to federal tax policy. A method for evaluating tax distribution tables is proposed which highlights the deficiencies of design and methodology which characterize the present use of tax distribution tables. An index of questions is provided as part of this research project to serve as a new tool of policy analysis, an index termed the "Tax Distribution Table Scoring Index" (TDTSI). The TDTSI will assist in balancing the different perspectives presented via tax distribution tables by identifying the biases and limitations associated with different methodologies and presentations of data.<br>Ph. The growth of micro-credit along with the excellent conditions to carry out microfinance activity in the economy and financial system of the Republic of Peru are pushing for Microfinance Institutions (IMF) increased competition with banks in this segment business. Like in commercial banks, in microfinance questions such as: is this customer profitable?, What is the credit limit that I must accept to his/her application?, What interest rate should I charge to him/ her?, How I can reduce the risk default?, etc., are matters to be assessed properly. We propose a method that could facilitate improvement in customer qualification between failed and not failed. To this end, we propose a methodology that analyzes credit risk in the provision of microcredit through the design of a credit scoring model that we apply to a Development Agency for Small and Micro Enterprise (EDPYME), which is an IMF under the supervision by the Banking and Insurance Superintendency (SBS).<br>El crecimiento del número de microcréditos junto con las excelentes condiciones para llevar a cabo la actividad microfinanciera en la economía y sistema financiero de la República de Perú están impulsando a las instituciones de microfinanzas (IMF) a una mayor competencia con las entidades bancarias por este segmento de negocio. Al igual que en la banca comercial, en microfinanzas preguntas tales como: ¿conviene este cliente?, ¿cuál es el límite de crédito que debo aceptar a su solicitud?, ¿qué tasa de interés debo cobrar?, ¿cómo puedo reducir el riesgo de impago?, etc., son cuestiones que deben valorarse de forma adecuada. Este trabajo plantea un método que podría facilitar una mejora en la calificación de los clientes fallidos y no fallidos. Para ello, se propone una metodología que analiza el riesgo de crédito en la concesión de microcréditos mediante el diseño de un modelo de credit scoring aplicado a una entidad de desarrollo de la pequeña y micro empresa (EDPYME), IMF sometida a supervisión por la Superintendencia en Bancay Seguros (SBS).

Abstract:
Syfte: Apné/hypopné index (AHI), antalet apnéer/hypopnéer per sömntimma, är den vanligaste parametern som används för att beskriva graden av sömnapné. Syftet med studien är att undersöka effekten av olika hypopnékriterier för AHI, med desaturationsnivå 3% eller 4% d.v.s. om det finns en signifikant skillnad i AHI vid användning av hypopnékriterier 3% jämfört med 4 %. Metod och material: Polygrafiska registreringar från 40 patienter analyserades retrospektivt. Hypopnéer definierades först som 30–90% minskning av luftflödet under &gt;10 sekunder förknippat med 3% desaturation. Därefter definierades hypopnéer med 4% desaturation. Patentregistreringarna tolkades manuellt på Fysiologkliniken vid Västmanlands sjukhus Västerås. Resultat och slutsats: En signifikant skillnad föreligger i AHI vid användning av de olika hypopnékriterierna, (p &lt;0,001). AHI varierade från medianvärdet 8,5 (2,9–17,6) för vid användning av tolkningskriterium 4% desaturation till 15,7 (6,5–24) vid användning av tolkningskriterium 3%. Det är viktig att följa de nya uppdaterade kriterierna från American Academy of Sleep Medicine och Svenska sömnapnéregistret för att kunna ställa rätt diagnos och behandling för patienten.<br>Purpose: The apnea/hypopnea index (AHI) is the most common parameter used to describe the severity of sleep apnea. The purpose of this study was to examine the effect of different oxygen desaturation levels (3% and 4%) on AHI i.e. if there is a significant difference in the AHI when using different hypopnea-criteria. Method and material: Polygraphic recordings of 40 patients were analyzed retrospectively. Hypopneas were first defined as 30-90% drop in airflow for &gt;10 s associated with 3% oxygen desaturation. Then hypopneas were defined as with 4% oxygen desaturation. Results and conclusion: There is a significant difference in AHI when using different hypopnea criteria, i.e. 4% or 3% desaturation. AHI varied from 8,5 (2,9–17,6) when using hypopnea criteria 4% to 15,7 (6,5–24) when using 3%. It is important to follow the new updated criteria from the American Academy of Sleep Medicine and the Swedish Sleep Apnea Register in order to provide the right diagnosis and treatment for the patient.

Tarptautinių atsiskaitymų banko (BIS) Bazelio II susitarimo nuostatos dėl bankų minimalaus kapitalo apibrėžia reikalavimus kredito rizikos skaičiavimui. Kredito rizikos vertinimo metodai leidžia naudoti vidines įmonių reitingavimo sistemas. Vienas svarbiausių reitingavimo modelio uždavinių – į modelį parinkti tokius finansinius ar nefinansinius rodiklius, kurie geriausiai klasifikuotų įmones pagal jų finansinio pajėgumo lygį. Populiariausias statistinis atrankos rodiklis yra tikslumo koeficientas dar vadinamas Gini indeksu. Tradicinis Gini indeksas buvo apibrėžtas 1914 m. ir iki šiol yra naudojamas pajamų nelygybei populiacijoje apskaičiuoti. 1995 m. Mosleris ir Koševojus pristatė k-matį Gini indekso analogą kaip zonoido tūrį. Šio darbo tikslas – naudojantis zonoidų teorijos idėjomis sukonstruoti apibendrintą reitingavimo modelių Gini indeksą. Pirmoje darbo dalyje pateiktos tradicinės Lorenco kreivės bei Gini indekso sąvokos ir Gini indekso apibendrinimai. Antroje darbo dalyje pagal BIS naudojamas reitingavimo modelio galios sąvokas, apibrėžtas reitingavimo modelio Gini indeksas. Be to, apibrėžti Lorenco kreivės apatinės ir viršutinės aproksimacijų Gini indeksai bei sudaryti šių indeksų apibendrinimai – normos bei tūrio daugiamačiai Gini indeksai. Pabaigoje analizuojamas atskirų finansinių rodiklių Gini indeksų stabilumas bei bendras Gini indeksų – vienamčio, normos ir tūrio – stabilumas ir pateikiamos išvados.<br>Bank for International Settlements (BIS) Basel II resolutions on banks regulatory capital include requirements for credit risk calculation. Credit risk evaluation methods define the possibility of using the internal rating system. One of the main tasks to build the powerful scoring model is to select financial and non-financial factors that appropriately classify companies according to their financial situation. The most popular statistical measure used for discriminatory analysis is the accuracy ratio or Gini index. General Gini index presented in 1914 is still widely applied to measure income inequality in the population. The k-dimensional analogue of Gini index as volume of zonoid was defined only in 1995 by Mosler and Koshevoy. The main purpose of this paper is to build the generalized Gini index of scoring model following the theory of zonoids. In the first part of the paper the usual Lorenz curve, traditional Gini index and its summary measures are presented. The second part presents the definition of the scoring models Gini index according to scoring model power measures applied in BIS resolutions. Furthermore the Gini indexes of Lorenz curve bottom and top approximations are defined and two its summary measures – norm and volume Gini indexes are constructed. Finally the stability of separate financial ratios Gini indexes and the general stability of univariate, norm and volume Gini indexes are analysed and final conclusions are presented.

Only a handful of studies focuses on the relationship between the legal origin and the content of CSR reports, based on the institutional differences. The previous studies have contradicting results. The paper aims to add to the body of research by analyzing the relationship between the legal origin and its effect on the content of CSR disclosure. To analyse the content a scoring index is developed following Clarkson, Li, Richardson, &amp; Vasvari, 2008 and Ong 2016. 45 CSR reports from 8 different countries are analysed and the research period is 2018 or FY 2019.  The findings suggest that companies from code law countries do publish more in-depth CSR reports. The key findings are that code law countries disclose more employment related information and that institutional regulations lead to better disclosure. Overall, this study extends the discussion on the effects of the legal origin.

Seven hundred eighty nine Peruvian students (362 men and 427 women, in ages between 16 and 27 years) answered a 6 - items Scale of Perception of Contempt in Peru. Although there is no significative statistical differences between the total scores in men and women, item- analysis showed these difference in three items. The subjects wich saw themselves as of white race had a stronger perception of contempt in the Peruvian society than the other people. The validation and other psychometric qualities of the Scale of Perception of Contempt in Peru are commented in this communication.<br>Setecientos ochenta y nueve universitarios peruanos (362 varones y 427 mujeres, en edades entre 16 y 27 años) respondieron una escala de 6 ítems que evaluaba la percepción de desprecio en el Perú. Aunque no se dieran  diferencias estadísticamente significativas entre los puntajes totales de hombres y mujeres, el análisis de ítems puso en evidencia esas diferencias en tres de ellos. Los sujetos que se vieron a sí mismos como de raza blanca tuvieron una percepción más fuerte de desprecio en la sociedad peruana que otras personas. La validación y otras cualidades psicométricas de la Escala de Percepción de Desprecio  en el Perú son comentadas en esta comunicación.

The narrow therapeutic index associated with most analgesic opioids creates a high degree of risk, obliging caution in patient selection; this chapter describes screening practices. More attention is due to the use of opioid therapy for chronic non-cancer pain as episodes of respiratory arrest increase. Universal precautions are recommended for all patients. Before and throughout treatment, selected tools assessing risk and misuse should be employed. Increased access to the opioid antagonist naloxone has followed the increase in opioid poisoning deaths; the Risk Index for Overdose or Serious Opioid-induced Respiratory Depression (RIOSORD) helps determine the likelihood of an opioid overdose, serving to select candidates for at-home naloxone access. Urine drug testing aids treatment plan adherence. Tables include: a comparison of screening tools for risk stratification; a comparison of tools for assessment of opioid misuse; the scoring procedure for the RIOSORD; and a list of risk classes with predicted probabilities.

Imaging tools in preventive cardiology can be divided into imaging modalities to assess pre-clinical and clinical atherosclerosis and functional assessments of vascular function or vascular inflammation. To calculate the likelihood of pre-clinical atherosclerosis intima-media thickness as well as coronary calcium scoring are most frequently used. However, beyond these two there are other parameters derived by ultrasound and multi-detector computed tomography as well as magnetic resonance imaging and nuclear/molecular imaging which are discussed in the chapter. Functional tests include flow-mediated dilatation, pulse wave analysis, and the ankle-brachial index. In clinical research other invasive measurements such as intravascular ultrasound/virtual histology/elastography, optical coherence tomography as well as thermography are being used. However, their value in clinical prevention still needs to be established.

Abstract:
Differentiating life-threatening from benign causes of chest pain in the critical care setting is a challenge when the symptoms and signs overlap, and patients are unable to communicate fully. A high index of suspicion is required for occult disease. Once the clinician has ensured the patient is haemodynamically stable, it is imperative to rule out myocardial infarction in the first instance. Where possible, a thorough history and a full examination should be undertaken. Electrocardiogram, chest X-ray, and routine observations are often diagnostic. Targeted investigation such as computed tomography, or transthoracic or transoesophageal ultrasonography may be required to confirm these diagnoses. Timely intervention optimizes survival benefit. Treatment may be necessary prior to confirmation of diagnosis, based on high clinical suspicion and risk scoring. Other causes of chest pain should be considered once the immediately life-threatening conditions are excluded.
