Abstract:
The objective of the present study was to analyze the role of the arterial chemoreceptors in arterial pressure alterations produced by sinoaortic denervation (SAD) in rats. The mean arterial pressure during 2 h of continuous computerized beat-to-beat recordings was higher after aortic denervation (AD; 130 +/- 2 and 124 +/- 3 mmHg, respectively), lower after sinus denervation (SD; 101 +/- 1 and 101 +/- 3 mmHg), and remained unaltered after SAD (121 +/- 3 and 108 +/- 2 mmHg) 1 and 20 days after denervation compared with control rats (114 +/- 1 mmHg). Hypotensive effect of SD was confirmed when arterial pressure was recorded in the same animal before and after SD (from 112 +/- 2 to 103 +/- 2 mmHg). A similar effect was observed after isolated carotid body artery (CBA) ligation (from 114 +/- 3 to 104 +/- 3 mmHg). Furthermore, CBA ligation attenuated by 13% the hypertension after AD (from 136 +/- 2 to 118 +/- 3 mmHg). Bradycardic response to phenylephrine and arterial pressure variability were markedly altered by SAD and AD but remained normal after SD. In contrast, the chemoreflex (intravenous KCN) was abolished after SAD, SD, and CBA ligation but was preserved after AD. These data suggest that the arterial pressure alteration produced by SAD in rats represents the net effect of the abolition of inhibitory (baroreceptor deafferentation) and excitatory (chemoreceptor deafferentation) influences on the arterial pressure.

Abstract:
Latar belakang. Peran serta dan penerimaan ibu mengenai imunisasi ulangan difteria-tetanus diperlukan untuk menunjang upaya pencegahan penyakit tersebut.Tujuan. Mengetahui pengaruh pengetahuan terhadap sikap ibu murid SD kelas I mengenai imunisasi ulangan difteria-tetanus.Metode. Subjek penelitian adalah ibu murid SD kelas I di Kotamadya Bandung, menggunakan penelitian kuantitatif dan kualitatif. Rancangan penelitian kuantitatif berupa survei cross-sectional dimulai bulan November sampai Desember 2007. Sampel diambil secara cluster random sampling. Analisis statistik dilakukan untuk melihat pengaruh pengetahuan ibu murid SD kelas 1 terhadap sikap imunisasi ulangan difteria-tetanus dengan menggunakan analisis jalur. Pengetahuan adalah informasi yang dimiliki oleh ibu mengenai penyebab, gejala, pengobatan, penularan, dan pencegahan tentang penyakit difteria-tetanus. Sikap adalah respons ibu terhadap penyakit difteria-tetanus mengenai penyebab, gejala, pengobatan, penularan, dan pencegahannya. Rancangan penelitian kualitatif berupa focus group discussion dengan analisis deskriptif, untuk menilai pengetahuan dan sikap ibu mengenai imunisasi ulangan difteria-tetanus yang tidak tergali dengan penelitian kuantitatif. Sampel diambil sebanyak 5-10 orang dari ibu yang telah ikut dalam penelitian kuantitatif.Hasil. Didapatkan 226 ibu dengan rata-rata usia 35 tahun. Hasil analisis jalur, didapatkan pengetahuan secara signifikan dipengaruhi oleh pendidikan ibu dan jumlah anak. Sikap signifikan dipengaruhi oleh pendidikan ibu dan pengetahuan (95%CI: -t<+1,96>t). Hasil analisis jalur didapatkan pengaruh total pendidikan ibu dan pengetahuan terhadap sikap adalah cukup. Pengaruh total pendidikan ibu dan jumlah anak terhadap pengetahuan adalah kurang. Analisis focus group discussion tergambarkan bahwa ibu yang mempunyai pengetahuan tinggi lebih bersikap mandiri terhadap pelaksanaan imunisasi ulangan difteria-tetanus.Kesimpulan. Pengetahuan ibu berpengaruh positif terhadap sikap ibu mengenai imunisasi ulangan difteria-tetanus

Abstract:
Aims The purpose of this study is to examine the adductus impact on the second metatarsal by the nonosteotomy nonarthrodesis syndesmosis procedure for the hallux valgus deformity correction, and how it would affect the mechanical function of the forefoot in walking. For correcting the metatarsus primus varus deformity of hallux valgus feet, the syndesmosis procedure binds first metatarsal to the second metatarsal with intermetatarsal cerclage sutures. Methods We reviewed clinical records of a single surgical practice from its entire 2014 calendar year. In total, 71 patients (121 surgical feet) qualified for the study with a mean follow-up of 20.3 months (SD 6.2). We measured their metatarsus adductus angle with the Sgarlato’s method (SMAA), and the intermetatarsal angle (IMA) and metatarsophalangeal angle (MPA) with Hardy’s mid axial method. We also assessed their American Orthopaedic Foot & Ankle Society (AOFAS) clinical scale score, and photographic and pedobarographic images for clinical function results. Results SMAA increased from preoperative 15.9° (SD 4.9°) to 17.2° (5.0°) (p < 0.001). IMA and MPA corrected from 14.6° (SD 3.3°) and 31.9° (SD 8.0°) to 7.2° (SD 2.2°) and 18.8° (SD 6.4°) (p < 0.001), respectively. AOFAS score improved from 66.8 (SD 12.0) to 96.1 (SD 8.0) points (p < 0.001). Overall, 98% (119/121) of feet with preoperative plantar calluses had them disappeared or noticeably subsided, and 93% (113/121) of feet demonstrated pedobarographic medialization of forefoot force in walking. We reported all complications. Conclusion This study, for the first time, reported the previously unknown metatarsus adductus side-effect of the syndesmosis procedure. However, it did not compromise function restoration of the forefoot by evidence of our patients' plantar callus and pedobarographic findings. Abstract:
This study examined the validity and incremental validity of the Constructive Thinking Inventory in a sample of Iranian managers. These 159 men were 39.9 yr. old ( SD = 2.5) and volunteered to participate in a project in which they responded to the Constructive Thinking Inventory, the Big Five Factors, the Costello and Comrey Depression and Anxiety Scales, and the Perceived Stress Scale. Numerous findings confirmed the validity of the Constructive Thinking Inventory, and the Global Constructive Thinking subscale displayed incremental validity. These data supported the validity of the Constructive Thinking Inventory and its associated theoretical assumptions in a sample of Iranian managers.

AbstrakPenelitian ini difokuskan pada penerapan pelatihan metode pembelajaran Gambar Pikiran Plus untuk guru pada pembelajaran di SD Inklusi yang bertujuan untuk mengetahui apakah pelatihan metode Gambar Pikiran Plus yang diberikan kepada guru dapat meningkatkan kinerja guru dalam mengatasi hambatan verbal yang dialami siswa tunarungu, autis dan tunagrahita sehingga prestasi belajar siswa meningkat. Pada siklus II diketahui bahwa pelatihan penggunaan metode Gambar Pikiran Plus terhadap guru dapat meningkatkan kualitas mengajar guru dan meningkatkan prestasi siswa. Abstract:
Penggunaan backpack dan posisi duduk merupakan bagian dari keseharian anak usia sekolah. Jika penggunaan backpack tidak sesuai dan posisi duduk tidak ergonomis, maka hal ini menjadi pencetus terjadinya nyeri punggung pada anak usia sekolah. Penelitian ini bertujuan untuk mengetahui hubungan penggunaan backpack dan posisi duduk terhadap keluhan nyeri punggung pada anak usia sekolah. Penelitian ini merupakan penelitian kuantitatif dengan pendekatan cross sectional. Sampel pada penelitian ini berjumlah 121 anak usia sekolah di SD Negeri 111 Kota Pekanbaru. Pengambilan sampel pada penelitian ini menggunakan teknik stratified random sampling. Pengambilan data dilakukan menggunakan kuisioner penggunaan backpack dan lembar observasi REBA (Rapid Entire Body Assesment). Penelitian ini menggunakan analisis bivariat yaitu uji Chi-Square. Hasil penelitian menunjukkan, dari 121 siswa, dijumpai 97 siswa (80,2%) mengeluh nyeri punggung ringan. Nyeri ringan banyak dikeluhkan oleh siswa dengan penggunaan backpack yang tidak sesuai sebanyak 78 siswa (86,7%), dan siswa dengan posisi duduk yang kurang ergonomis sebanyak 58 siswa (80,6%). Terdapat hubungan antara penggunaan backpack dengan keluhan nyeri punggung dengan nilai P value 0,011(P<0,05), dan terdapat hubungan antara posisi duduk dengan keluhan nyeri punggung dengan nilai P value 0,042 (P<0,05) pada siswa kelas 4 dan 5 SD Negeri 111 Kota Pekanbaru. Kesadaran untuk memperhatikan tata cara penggunaan backpack dan posisi duduk anak di sekolah penting untuk diterapkan sebagai upaya pencegahan guna menurunkan timbulnya keluhan nyeri punggung pada anak usia sekolah.

Sarapan merupakan hal penting juga untuk memenuhi gizi bagi setiap orang untuk mengawali aktivitasnya. Sarapan adalah kegiatan makan dan minum yang dilakukan antara bangun pagi sampai jam 9 untuk memenuhi sebagian yaitu 15-30% kebutuhan gizi harian (Hardinsyah, 2012). Tujuan dari penelitian ini untuk mengetahui gambaran kebiasaan sarapan pada siswa sekolah dasar di Kabupaten Badung. Jenis penelitian ini adalah deskriptif, yakni menggambarkan kebiasaan sarapan pada siswa sekolah dasar di Kabupaten Badung. Lokasi penelitian ini akan dilaksanakan pada empat sekolah dasar di Kabupaten badung yaitu SD 1 Kuta, SD 3 Kuta, SD 2 Kuta dan SD 1 Jagapati menggunakan 154 Siswa SD yang diambil dengan purposive sampling. Dari 154 siswa yang diteliti didapatkan bahwa sebagian besar siswa Sekolah Dasar di Kabupaten Badung terbiasa sarapan yaitu sebanyak 120 orang (77,9%), sebagian besar sarapan setiap pagi hari yaitu sebanyak 109 orang (70,8%), sebagian besar sarapan pada jam 6-7 pagi yaitu sebanyak 149 orang (96,8%), sebagian besar sarapan di rumah yaitu sebanyak 152 orang (98,7%) dan sebagian besar dengan menu yang berganti-ganti yaitu sebanyak 121 orang (78,6%). Disarankan petugas kesehatan memberikan penyuluhan kesehatan tentang pentingnya sarapan yang sehat baik pada siswa dan kepada orangtua karena selain untuk kesehatan juga sangat bermanfaat untuk meningkatkan konsentrasi dan prestasi siswa.

Este artigo analisou enunciados relativos ao trabalho e inclusão para pessoas com Síndrome de Down (SD) queparticipam de atividades produtivas. Propôs-se uma reflexão genealógica sobre trabalho e sua articulação com inclusão eSD, entendendo que essas marcas funcionam como herança para as possibilidades de inserção nas instâncias do trabalho.A pesquisa foi realizada numa instituição de apoio à pessoa com deficiência, utilizando como metodologias: entrevistascom pessoas que possuem SD e profissionais da instituição, além do acompanhamento das oficinas e confecção dediário de campo. A partir da análise, podemos sintetizar que os enunciados são diversos e apontam para a complexidadedo campo. Os enunciados nos apontaram para a importância de três temáticas na forma como as pessoas com SD serelacionam com o trabalho e a inclusão: Construção de vínculos; tempo e ritmo de trabalho; e família e remuneração.Consideramos que o estudo questiona significados atribuídos ao trabalho para funcionários com ou sem Síndrome deDown, possibilitando pensar nas singularidades que constroem formas de inclusão e exclusão no âmbito profissional.

Abstract:
Education is one of the key goals of sustainable development (SD), which establishes the basis for the improvement of the people’s living conditions. In this logic a special role is played by universities that create an institutional framework for educating citizens on sustainable development, offering a new understanding of social problems. On the one hand, universities can create and promote knowledge about SD by their educational, expert and research activities, hence developing relevant values among people. On the other hand, universities can become an active agent in implementing the concept of SD by introducing it into its own academic activities. The article considers stakeholder approach as one of the approaches to the implementation of the concept of education for sustainable development (ESD). Therefore based on this approach, the article explores the benefits of the key stakeholders of the sustainable university. Low awareness among key stakeholders is one of the significant factors that hindering the implementation of the SD concept. Due to the lack of a sufficient research focused on studying the interests of the main stakeholders in the framework of ESD, this article is an attempt to narrow this gap.

Abstract:
Aims The purpose is to determine the non-inferiority of a smartphone-based exercise educational care management system after primary knee arthroplasty compared with a traditional in-person physiotherapy rehabilitation model. Methods A multicentre prospective randomized controlled trial was conducted evaluating the use of a smartphone-based care management system for primary total knee arthroplasty (TKA) and partial knee arthroplasty (PKA). Patients in the control group (n = 244) received the respective institution’s standard of care with formal physiotherapy. The treatment group (n = 208) were provided a smartwatch and smartphone application. Early outcomes assessed included 90-day knee range of movement, EuroQoL five-dimension five-level score, Knee Injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS JR) score, 30-day single leg stance (SLS) time, Time up and Go (TUG) time, and need for manipulation under anaesthesia (MUA). Results Overall, 90-day mean flexion was not significantly different between the control (121° (SD 11.7°)) and treatment groups (121o; p = 0.559); 90-day mean SLS was 22.7 seconds (SD 9.8) in controls and 24.3 seconds (SD 20.8) in treatment (p = 0.519); 90-day mean TUG times were 10.1 seconds (SD 4.8) in control and 9.3 seconds (SD 3.3) in treatment (p = 0.139). Mean KOOS JR scores were significantly different between control group (73.6 (SD 13.4)) and treatment group (70.4 (SD 12.6); p = 0.026). MUAs were performed in nine (3.7%) patients in the control group and four (1.9%) in the treatment group (p = 0.398). Physiotherapy was performed by 230 (94.4%) of control group and 123 (59.3%) of treatment group (p < 0.001). There were no significant differences between groups in postoperative urgent care visits, or readmissions within 90 days, with significantly fewer emergency department visits in the treatment group (16 (8.2%) vs five (2.5%), p = 0.014). Conclusion The use of the smartwatch/smartphone care platform demonstrated non-inferiority of clinically significant outcomes to traditional care models, while requiring significantly less postoperative physiotherapy and fewer emergency department visits. This pilot study evaluated a high-energy, high-protein, peptide-based, (medium-chain triglycerides) MCT-containing enteral tube feed (Nutrison Peptisorb Plus HEHP®, Nutricia Ltd., Trowbridge, BA14 0XQ, UK.) containing 1.5 kcal/mL and 7.5 g protein/100 mL. Fifteen community-based, enterally tube-fed adults (42 (SD 16.3) years) received the intervention feed daily for 28 days, with gastrointestinal tolerance, compliance and nutrient intake assessed at baseline and after the intervention period. Incidence and intensity of constipation (p = 0.496), nausea (p = 1.000), abdominal pain (p = 0.366) and bloating (p = 0.250) remained statistically unchanged, yet the incidence and intensity of diarrhoea improved significantly after receiving the intervention feed (Z = −2.271, p = 0.023). Compliance with the intervention feed was significantly greater compared to the patient’s baseline regimens (99% vs. 87%, p = 0.038). Compared to baseline, use of the intervention feed enabled patients to significantly increase total energy (1676 kcal/day (SD 449) to 1884 kcal/day (SD 537), p = 0.039) and protein intake (73 g/day (SD 17) to 89 g/day (SD 23), p = 0.001), allowing patients to better achieve energy (from 88% to 99%, p = 0.038) and protein (from 101% to 121%, p < 0.001) requirements. This pilot study demonstrates that a high-energy, high-protein, peptide-based, MCT-containing enteral tube feed maintains gastrointestinal tolerance and improves compliance, energy and protein intake in complex, enterally tube-fed, community-based adult patients, though more work is recommended to confirm this.

Background: Total knee replacement (TKR) is a major orthopedic surgery that is considered high risk for the development of venous thromboembolism (VTE).
 Objective: The aim of this study is to evaluate the clinical outcomes that resulted from the use of a new proposed VTE risk stratification protocol for selecting a suitable extended VTE prophylaxis for post TKR surgery patients administered in conjunction with patient education programs.
 Method: A randomized controlled trial was conducted in two medical centers in Saudi Arabia. A total of 242 patients were enrolled in the study, 121 patients in each group. The experimental group (A) was assessed by using the proposed VTE risk stratification protocol and also took part in patient education programs about TKR and its complications. The control group (B) was assessed by using the 2005 Caprini risk assessment tool and no education programs were given to this group. Both groups were followed for 35 days post operation.
 Results: The mean age of the participants was 65.86 (SD 8.67) and the majority of them were female 137 (56.6%). The mean body mass index of the study sample was 32.46 (SD 5.51). There were no significant differences between the two groups except for surgery type; the proportion of bilateral TKR in group A was higher than in group B (69/121 (28.5%) vs. 40/121(16.5%), p˂0.05). There were no confirmed pulmonary embolism cases in the study sample and diagnosis of deep-vein thrombosis was confirmed in 12/242 (5.0%) of patients: 1/121 (0.8%) in group A and 11/121 (9.1%) in group B (p˂0.05). The readmission rate for all patients was 2.5% (6/242), all of whom were in group B (p˂0.05).
 Conclusion: The proposed VTE risk stratification protocol that was applied in conjunction with patient education programs reduced VTE complications and readmission events, post TKR surgery.

Pendekatan saintifik (5M) yang meliputi keterampilan proses mengamati, menanya, mengumpulkan informasi, mengasosiasi dan mengkomunikasikan perlu disinergikan dengan standar proses eksplorasi, elaborasi, dan konfirmasi (EEK). Kedua keterampilan (5M dan EK) inilah yang menjadi pemandu bagi guru untuk memilih model (dan metode) pembelajaran serta alat penilaian otentik yang sesuai. Dalam rangka membantu para guru untuk mengimplemantasikan kurikulum 2013, Kementerian Pendidikan dan Kebudayaan menyiapkan buku guru dan buku siswa. Namun demikian para guru dapat memodifikasi dan mengembangkan sendiri. Pengembangan instrumen pembelajaran disesuaikan dengan karakteristik para siswa dan lingkungan belajar-nya, sehingga mutu proses dan hasil pembelajaran dapat ditingkatkan. Pengem-bangan mutu pembelajaran seperti ini sekaligus menjadi sarana bagi para guru untuk mengembangkan profesinya secara berkelanjutan. Abstract Background Standard dose ganciclovir (SD-GCV) for treatment of CMV infection/disease is 5 mg/kg every 12 hours, although higher doses (7.5–10 mg/kg every 12 hours) may be considered for resistant CMV. Literature on safety/efficacy of high dose GCV (HD-GCV) is limited. We sought to evaluate safety and clinical outcomes of SD-GCV vs HD-GCV strategies. Methods Retrospective single center study of adult SOT recipients with CMV viremia from 1/1/2017-1/31/2019 who received IV GCV therapy. Primary objective was to compare incidence of cytopenias between SD-GCV and HD-GCV. Secondary outcomes compared CMV viremia clearance, incidence of CMV disease and recurrent viremia within 30 days, granulocyte colony stimulating factor (G-CSF) use, and antiviral resistance testing rates. Results We evaluated 121 patients: 74 received SD-GCV, 47 received HD-GCV. Baseline characteristics were similar between groups. Most patients received a liver transplant (46% SD vs 36% HD) and had D+/R- CMV serostatus (55.4% SD vs 68% HD). Induction immunosuppression occurred in 75%, mostly with anti-thymocyte globulin. Median baseline CMV viral loads were similar (4620 IU/mL SD vs 7770 IU/mL HD, p=0.25). Incidence of cytopenias was similar between groups: leukopenia (43% SD vs 43% HD, p=0.96), neutropenia (15% SD vs 13% HD, p= 0.75), thrombocytopenia (24% SD vs 31% HD, p=0.62). HD-GCV did not significantly impact CMV clearance (HR: 0.79 [95% CI 0.52–1.21], p=0.27). There was no difference in incidence of CMV disease (35% SD vs 38% HD, p=0.72) or incidence of recurrent CMV viremia (15% SD vs 28% HD, p=0.098). G-CSF requirement was not different (23.7% SD vs 14.3% HD, p=0.295), however, patients on HD-GCV received more doses of G-CSF (median 2 SD vs 5 HD, p=0.001). More patients in HD-GCV group were tested for antiviral resistance: 15 (21%) SD vs 20 (43%) HD, p=0.01. Of these, there was no difference in rate of resistance detection (7/15 (47%) SD vs 11/20 (55%) HD, p=0.95). Conclusion Therapy with HD-GCV did not demonstrate increased incidence of cytopenias compared to SD-GCV, nor did we observe improved time to CMV clearance or incidence of CMV disease between groups. Penelitian dilaksanakan selama 1 bulan yaitu bulan September 2014 sampai dengan bulan Oktober 2014. Subjek dalam penelitian adalah proses dan hasil belajar PKn pada siswa kelas VI SD Negeri 1 Mongkrong, Wonosegoro semester I tahun pelajaran 2014/2015 yang terdiri atas 19 siswa yaitu 10 laki-laki dan 9 perempuan. Prosedur penelitian yang digunakan yaitu prosedur jenis penelitian tindakan kelas yang dilaksanakan dalam 2 siklus, setiap siklus terdiri dari 4 tahapan yaitu perencanaan, pelaksanaan, pengamatan, dan refleksi. Analisis data, data kualitatif hasil pengamatan proses belajar dianalisis menggunakan analisis diskriptif kualitatif dengan membandingkan siklus I dengan siklus II, sedangkan data yang berupa angka (data kuantitatif) dari hasil belajar siswa dianalisis menggunakan deskriftif komparatif yaitu membandingkan nilai tes kondisi awal, nilai tes siklus I dan nilai tes siklus II, kemudian direfleksi. Hasil penelitian menunjukkan bahwa melalui metode <em>Problem Based Learning</em> dapat meningkatkan prestasi hasil belajar PKn pada siswa Kelas VI SD Negeri 1 Mongkrong, Wonosegoro Semester I tahun pelajaran 2014/2015. Proses belajar PKn materi Proses Perumusan Pancasila dari siklus I ke siklus II terdapat peningkatan: hasil belajar siswa dari siklus I ke siklus II mengalami peningkatan, yaitu dari 8 siswa (72,7%) yang mendapat nilai tuntas menjadi 11 siswa (100%). Terjadi peningkatan sebanyak 3 siswa (27,3%) dan nilai rata-rata kelas dari 77,8 menjadi 83,5 meningkat sebesar 5,7.

121 Background: More than 362,000 female cancer survivors under 40 years old live in the US. Research consistently shows no increased cancer risk among offspring of cancer survivors in the absence of a hereditary cancer. Still survivors voice concerns over the health of future children. This analysis explores young adult female cancer survivors’ (YAFCS) perceptions of genetic cancer risk and related reproductive concerns. Methods: Eligible females were aged 18-35, at least one year post-treatment. Participants were recruited using social media posts from 17 young adult cancer advocacy groups and directed to an anonymous web-based survey that included the Reproductive Concerns after Cancer Scale (RCACS), with a 3-item “Concerns about Future Child Health” subscale. Results: 346 YAFCS completed the full survey, and based on logic-based branching, 192 respondents could have completed the RCACS: 177 (92%) completed the entire scale and 187 (97%) completed the child’s health subscale. Mean RCACS score was 58.5 (sd = 11.6; range 22-86); mean child’s health score was 11.3 (sd = 3.3; range 3-15), with a negative skew. At the item-level, 65% of the sample worried about passing along cancer risk to offspring; 71% worried about family health history affecting future children; and 60% were afraid their children would have a high chance of getting cancer. RCACS and child’s health subscale scores did not vary by age, race, education, income, or diagnosis. Further, mean scores did not vary between survivors of family cancer syndromes (breast, ovarian, colorectal) and those with other diagnoses (RCACS: t = 1.4, df = 175, p = .14; child’s health: t = 0.02, df = 185, p = .99). Conclusions: Despite studies showing minimal genetic cancer risk in offspring of survivors, many YAFCS have concerns about the health of future children. This may be related to mistaken beliefs that genetic mutations within tumor cells are transmissible or unawareness of availability of pre-implantation genetic diagnosis for embryo selection in those with transmissible genetic mutations. There is a need to educate patients about these issues to enable them to make family building choices based on accurate information about genetic risk.

Abstract Functional performance measures (grip strength, Short Physical Performance Battery (SPPB), and 3-meter gait speed) represent underlying disease progression and predict mortality. However, there is little information regarding whether these measures assessed at 2-months post-hip fracture predict long-term mortality (10-year follow-up). To address this gap, a longitudinal analysis of Baltimore Hip Studies-7 cohort, with mortality verified by National Death Index, was conducted. Mean difference in 2-month functional performance measures (n=242, men n=121, female n=121) among those who survived and did not survive over 10 years was determined using t-test. Prediction of mortality by these measures, overall and by sex, was estimated using cox proportional hazard models, for which Hazard ratios (HR) with 95% confidence intervals (CI) were estimated. We found that, gait speed [0.47(standard deviation,SD=0.39) versus 0.31(SD=0.27)] and SPPB score [4.89(SD=3.31) versus 2.83(SD=2.24)] were significantly higher at 2 months among those surviving compared to those who did not. Adjusting for covariates, functional performance predicted long-term mortality in men and women. Increase in gait speed by 0.1m/s predicted 15% decrease in mortality for men [HR=0.85(0.55-0.96)] and 17% for women [HR=0.83 (0.74-0.93)]. Increase in SPPB by 1 unit predicted decrease in mortality by 14% for men [HR=0.86(0.77-0.95)] and 17% for women [HR=0.83(0.74-0.93). Increase in grip strength by 1 kg predicted 5% decrease in mortality for men [HR=0.94(0.92-0.97)] and 9% for women [HR=0.90(0.86-0.95)]. Functional performance measured at 2-months post-hip fracture predicted long-term mortality. Those with poor functional performance at 2-months can be referred for further assessment to optimize their care to promote survival.

Abstract Pediatric burn injuries are stressful for parents, yet few burn clinics report screening caregivers. We evaluated psychometric properties of a two-item depression screener administered to parents of children with burns during outpatient clinic visits. We also examined associations between parent depression symptoms and child characteristics. We used a retrospective review of pediatric patients with burn injuries (n = 496, age range: 0–21 years; M = 5.0 years, SD = 4.4 years) from an outpatient specialty burn clinic. Sample was 54.8% male; ethnicity was 42.4% Black/African American and 42.2% White. Most children (94.7%) had a burn TBSA of 10% or less and partial thickness burns (87%). Depression measure was administered at two time points as part of routine care: T1 (n = 496) and T2 (n = 121). Score range was 0 to 8. The means were 1.17 (SD = 1.74) at T1 and 0.81 (SD = 1.40) at T2. The majority scored ≤3 (89.9% caregivers) at T1. The measure demonstrated satisfactory internal consistency at T1 (Cronbach α = .74) and T2 (α = .82). Scores at T1 and T2 for a subsample (n = 121) were related (r = .61, p < .001). Parents of non-White children tended to report higher depression scores at T1. At T2, being female and greater burn degree were associated with higher depression scores. This brief two-item scale used with caregivers of pediatric burn patients is a reasonable method for screening parental depression in this setting. Given the association between parental depression and child characteristics, further studies are needed, including examination of predictive validity of parental depression with pediatric outcomes.

The inaccuracy of measuring human bronchial artery blood flow has previously been considerable. En bloc double-lung transplantation with bronchial artery revascularization (BAR) using a single conduit offers the unique opportunity of direct measurement of the total bronchial artery blood flow. In eight en bloc double-lung-transplanted patients with complete BAR, the basal blood flow was measured by using a 0.014-in. Doppler guide wire and arteriography. The average peak velocity in the conduit was 12–73 cm/s [±2.1 (SD) cm/s], and the conduit diameter was 1.7–3.1 mm [±0.10 (SD) mm], giving individual basal flow values between 19 and 67 ml/min [±5 (SD) ml/min], or 0.2–1.9% of estimated cardiac output. In three patients basal measurements were followed by injection of nitroglycerin and verapamil into the conduit. This increased the bronchial artery flow to 121–262% of basal values (31–89 ml/min). The measured values appear more physiologically plausible than previous bronchial artery blood flow measurements in humans.

The purpose of this study was to assess tarsal BMD associated with acute inflammation (ie, inflammatory osteolysis) in individuals with chronic diabetes mellitus (DM), peripheral neuropathy (PN), and recent-onset neuropathic (Charcot) arthropathy (NCA) of the foot.Research Design and MethodsThis was a case-control study of 32 people (11 men, 21 women) with DM, PN, and NCA of the foot or ankle. The subjects with DM, PN, and NCA were compared with 64 age-, sex-, and race-matched control subjects (24 men, 40 women) without DM, PN or NCA. Within the first 3 weeks of cast immobilization, BMD was estimated in both calcanei using quantitative ultrasonometry. Acute inflammation was confirmed by comparing skin temperature differences between the feet of the subjects with DM, PN, and NCA and the feet of the control subjects.ResultsSkin temperature differences averaged 6.7°F (SD=4.0°F) (involved foot minus noninvolved foot) in the feet of the subjects with DM, PN, and NCA compared with 0.0°F (SD=1.3°F) in the feet of the control subjects. Calcaneal BMD averaged 384 mg/cm2 (SD=110) in the involved feet and 467 mg/cm2 (SD=123) in the noninvolved feet of the subjects with DM, PN, and NCA and 545 mg/cm2 (SD=121) in combined right and left feet of the control subjects.ConclusionsInflammation in individuals with DM, PN, and NCA may contribute to or exacerbate a rapid loss of BMD. Inflammatory osteolysis may be a prominent factor responsible for both the spontaneous onset of neuropathic fracture and the insidious and progressive foot deformity that is the hallmark of the chronic Charcot foot.

AbstractBoth maternal Fe deficiency (ID) and being overweight or obese (Ow/Ob, BMI≥25 kg/m2) may negatively affect offspring brain development. However, the two risk factors correlate and their independent effects on infant neurodevelopment are unclear. PREOBE is a prospective observational study that included 331 pregnant Spanish women, of whom 166 had pre-gestational Ow/Ob. Fe status was analysed at 34 weeks and at delivery, and babies were assessed using Bayley III scales of neurodevelopment at 18 months. In confounder-adjusted analyses, maternal ID at 34 weeks was associated with lower composite motor scores at 18 months (mean 113·3 (sd 9·9) v. 117·1 (sd 9·2), P=0·039). Further, the offspring of mothers with ID at delivery had lower cognitive scores (114·0 (sd 9·7) v. 121·5 (sd 10·9), P=0·039) and lower receptive, expressive and composite (99·5 (sd 8·6) v. 107·6 (sd 8·3), P=0·004) language scores. The negative associations between maternal ID at delivery and Bayley scores remained even when adjusting for maternal Ow/Ob and gestational diabetes. Similarly, maternal Ow/Ob correlated with lower gross motor scores in the offspring (12·3 (sd 2·0) v. 13·0 (sd 2·1), P=0·037), a correlation that remained when adjusting for maternal ID. In conclusion, maternal ID and pre-gestational Ow/Ob are both negatively associated with Bayley scores at 18 months, but independently and on different subscales. These results should be taken into account when considering Fe supplementation for pregnant women.

While acid or papain digestion resulted in the loss of antimutagenic activity of casein, trypsin digestion had little effect although the digestion of casein by each proteinase was over 75%. β-Casein, egg albumin, and bovine serum albumin also had a similar antimutagenic activity to that of whole casein.

The result of investigation during September and October 2015 and review of previous studies showed that 224 stony coral species, 232 coral reef fish species, 88 Mollusca and Echinoderm were found in Ly Son Marine Protected Area. The biodiversity of coral reef communities was at medium level compared with other MPAs. The coverage of stony coral and soft coral was 6.1% ± 4.2 SD and 5.6% ± 5.0 SD, respectively. The density of coral reef fishes averaged 121 ± 74.4 SD Ind./100 m2 and this density mainly focused on small size group (< 10 cm) which ocupied 69.6% of total density. The average density of macro-invertebrates was 45 ± 3.0 SD Ind./100 m2 that depended on the Echinoderm group (more than 85% of total density). The coverage of living coral was considered the lowest meanwhile the density of coral reef fishes and macro-invertebrates was at medium level compared with other MPAs. Our result showed that there were two distinguished assemblages of coral reef communities. The charateristics of coral reef fishes and macro-invertebrates communities showed the resemblance in terms of distribution in most of the study sites meanwhile the coral community revealed the separation between two types of communities.

The purpose of this study is to evaluate the accuracy of an oscillometric blood pressure monitor in anesthetized pigs. Invasive blood pressure (IBP) and noninvasive blood pressure (NIBP) measurements were taken using a DRE Waveline Pro multiparameter monitor at four different time points in 17 pigs undergoing injectable anesthesia. NIBP measurements were taken on both the thoracic and pelvic limbs. Bland Altman analysis was used to assess agreement between methods and a linear mixed-effects model was used to evaluate the effect of cuff position and blood pressure on bias. Invasive systolic arterial pressure (SAP) ranged between 112 and 161 mmHg (mean ± SD: 138.8 ± 13.3; median: 139.5). Invasive diastolic arterial pressure (DAP) ranged between 60 and 104 mmHg (mean ± SD: 86.0 ± 9.1; median: 87.0). Invasive mean arterial pressure (MAP) ranged between 79 and 121 mmHg (mean ± SD: 103.2 ± 9.3; median 103.0). Only the diastolic and mean measurements obtained from the pelvic limb met criteria outlined by the American College of Internal Medicine for required accuracy of NIBP monitors. Bias was significantly higher in the thoracic limb in comparison to the pelvic limb and was significantly higher at blood pressures above median. In general, NIBP measurements underestimated IBP measurements. In conclusion, the use of the DRE Waveline Pro to assess NIBP in anesthetized pigs may be useful in monitoring trends in mean and diastolic blood pressure and is most accurate when used on the pelvic limb.

Sampel penelitian sebanyak 121 orang yang diambil dengan teknik purposive sampling. Hasil penelitian pada analisa bivariat menunjukan bahwa ada hubungan antara pola makan dan aktivitas fisik (P-Value=0,000), dengan kejadian obesitas pada anak di SD Labratorium Universitas Negeri Gorontalo (UNG). Diharapkan pada pihak sekolah agar memperhatikan status gizi siswa/siswi, karena status gizi yang buruk akan berdampak negatif bagi kondisi kesehatan anak.

7556 Background: Erlotinib is effective in NSCLCs with wild-type EGFR and shows enhanced benefit in EGFR mutation-positive cancers. However, resistance invariably develops, often involving persistent ErbB3 signaling and activation of the PI3K/AKT survival pathway. We present the full results of the Phase 1 study evaluating the safety and tolerability of erlotinib plus MM-121 a fully human IgG2 monoclonal antibody (mAb) to ErbB3. Methods: Eligible patients had advanced stage NSCLC, and ECOG 0-2. Seven cohorts were enrolled, evaluating varying levels of the MM-121 and erlotinib, as well as alternate MM-121 infusion schedules. Tumor response was assessed every 8 weeks. Dose levels were determined by safety and pharmacokinetic (PK) data, and immunogenicity, efficacy endpoints and exploratory biomarker evaluations were performed. Results: From February 2010 – July 2011 32pts entered the study (median age 63y; 45% male; 18% ECOG 0, 82% ECOG 1. 56% had adenocarcinoma and 30% pts received 3 or more lines of prior therapies (range 0-7) with 91% having had prior platinum. 65% had wild-type EGFR status and were never treated or never responded to EGFR-TKIs (EGFRwt) and 28% had acquired resistance to erlotinib treatment (EGFRresist). The most common toxicities (any grade) observed were diarrhea (82%), rash (64%), and fatigue (64%). DLTs observed in different cohorts were diarrhea, mucositis, rash and failure to thrive. Clinical activity was observed including 1 PR (an EGFR TKI naïve EGFR mutant) and 14 SD. Average duration of disease stabilization was 21.6 wks (range 7.1 -89.3 wks). Median PFS is 8.1 weeks and the 16 week PFS rate was 41% in the overall population. For EGFRwt and EGFRresist pts the median PFS were 7.7 weeks and 15.1 weeks, and the 16 week PFS rates were 32% and 44%, respectively. 7/20 EGFRwt pts and 5/9 EGFRresist pts achieved SD. Conclusions: This study suggests that the combination of erlotinib and MM-121 has an acceptable safety profile in heavily pre-treated NSCLC patients with and without EGFR mutations. Early signals of patient benefit were seen and a large randomized phase II study is ongoing.

The present study was designed to determine the apparent absorption and retention of the inorganic Se compounds SeO32- and SeO42-, which are commonly used for Se fortification of clinical nutrition products and infant formulas. Ten healthy men were fed a milk-based formula labelled with 40 μg Se as 74SeO32- or 76SeO42- on two consecutive days using a randomised crossover design. Se stable-isotope analysis of 9 d complete collections of urine and faeces was used to calculate apparent Se absorption and retention. Se retention from 74SeO32- (41·0 (SD 8·4) %) AND FROM 76SEO42- (46·0 (sd 7·9) %) was not significantly different (P>0·05). However, Se absorption was significantly higher from SeO42- than from SeO32- (91·3 (sd 1·4) % v. 50·2 (sd 7·8) %, P<0·05). Urinary excretion of the administered dose was 9·2 (sd 1·8) % for 74SeO32- and 45y3 (sd 8·2) % for 76SeO42- (P<0·05). Urinary Se excretion kinetics differed significantly for the two Se compounds; 90 % of the total urinary Se was excreted after 121 h for 74SeO32- and after 40 h for 76SeO42- (P<0·05). These results suggest that although Se absorption and urinary excretion differ for SeO32- and SeO42-, both Se compounds are equally well retained when administered at a relatively low dose (40 μg Se). The nutritional impact of Se fortification of foods would thus be expected to be similar when SeO42- or SeO32- are used.

Background Face masks, also referred to as half masks, are essential to protect healthcare professionals working in close contact with patients with COVID-19-related symptoms. Because of the Corona material shortages, healthcare institutions sought an approach to reuse face masks or to purchase new, imported masks. The filter quality of these masks remained unclear. Therefore, the aim of this study was to assess the quality of sterilized and imported FFP2/KN95 face masks. Methods A 48-minute steam sterilization process of single-use FFP2/KN95 face masks with a 15 minute holding time at 121°C was developed, validated and implemented in the Central Sterilization Departments (CSSD) of 19 different hospitals. Masks sterilized by steam and H2O2 plasma as well as new, imported masks were tested for particle filtration efficiency (PFE) and pressure drop in a custom-made test setup. Results The results of 84 masks tested on the PFE dry particle test setup showed differences of 2.3±2% (mean±SD). Test data showed that the mean PFE values of 444 sterilized FFP2 face masks from the 19 CSSDs were 90±11% (mean±SD), and those of 474 new, imported KN95/FFP2 face masks were 83±16% (mean±SD). Differences in PFE of masks received from different sterilization departments were found. Conclusion Face masks can be reprocessed with 121 °C steam or H2O2 plasma sterilization with a minimal reduction in PFE. PFE comparison between filter material of sterilized masks and new, imported masks indicates that the filter material of most reprocessed masks of high quality brands can outperform new, imported face masks of unknown brands. Although the PFE of tested face masks from different sterilization departments remained efficient, using different types of sterilization equipment, can result in different PFE outcomes.

Category: Ankle, Ankle Arthritis Introduction/Purpose: The laterally placed trabecular metal ankle replacement is placed using a jig and a fibular osteotomy. The tibia and talus are reamed to match the curved contour of the replacement. The lateral approach was advocated to reduce the wound complications of the anterior approach. Additional fixation is achieved using two rails on the tibial and talar component. This study outlines the outcomes of 121 ankles followed prospectively with minimum 2 years follow up. Methods: 121 total ankles were consented to enrollment in a prospective study on outcomes. 10 centers were involved in the prospective study. Enrollment was from 2014 to 2016. 3 patients underwent revision and 5 withdrew (1 death, 2 lost to follow up and 2 requested discontinuation). Demographics were obtained at baseline. The average age was 59.9 at the time of surgery. 48% were male and BMI averaged 29.2. 12.4% had inflammatory arthropathy. EQ5D, AOFAS, AOS scores were obtained preoperatively and at 6 weeks, 6 months, and yearly there after. Revision of metal components for infection or loosening were secondary end points. Statistical significance was tested by the Wilcoxon Signed Rank Test 26 and set at p= 0.05. 95% confidence intervals are given. Of 113 cases, 113 were followed at 6 weeks, 113 at 1 year, and 100 at 2 years and 67 at 3 years. Results: AOS Outcomes improved from 59.7 (SD 20.6) to 22 (SD 18) at 6 months, 18.6 (SD 19.6) at 1 year, 19.8 (SD 21) at 2 years and 21.3 (SD 25) at 3 years. AOFAS scores and EQ5 D scores showed similar improvements at all time points. The improvements were significant compared to baseline at all time points (see figure). Survivorship was .98 at 1 year (CI .92 to .99), and .96 at 2 and 3 years (CI .90 to .99). 3 components were revised, a tibial component (loosening), a talar component (instability), and a polyethylene liner (infection). Conclusion: The laterally placed trabecular metal total ankle arthroplasty has demonstrated reliable improvements in outcome and an excellent survivorship at minimum 2 years follow up. Longer term follow up and larger series will be required to clearly outline its outcome. This early outcome demonstrates very promising outcomes at 3 years and low revision rates.

The measurements of subtalar joint neutral position and hindfoot range of motion have been shown to be unreliable. The first step in making these measurements is to determine the calcaneal bisection. This study examines the reliability of bisecting the calcaneus with digital linear calipers. Five trials on each of six cadavers resulted in a mean absolute angular difference of 0.60° (SD ±1.17°). These results were then compared with results from the typical visual method used clinically. Three raters each performed five trials on six cadavers. Visual bisection was more variable, with a mean absolute error of 3.61° (±3.13°). A mean error of 6° (±1°) is certainly possible when the heel is visually bisected. Anterior tibial muscle protein synthesis in seven healthy postabsorptive men was determined from increases in muscle protein bound leucine enrichment during a primed continuous infusion of l-[1−13C]leucine. Biopsies were taken 30 min after the beginning of leucine infusion (when plasma 13C enrichment was steady), 240 min later during continued fasting and again after 240 min of infusion of a mixed amino acid solution which increased plasma total amino acid concentrations by 37%. The mean enrichment of 13C in plasma α-ketoisocaproate was used as an index of the enrichment of the precursor pool for leucine metabolism. 2. Anterior tibial muscle mixed protein synthetic rate during fasting was 0.055 (sd 0.008) %/h and this increased by an average of 35% during infusion of mixed amino acid to 0.074 (sd 0.021) %/h (P < 0.05). 3. Whole-body protein breakdown (expressed as the rate of endogenous leucine appearance in plasma) was 121 (sd 8) μmol h−1 kg−1 during fasting and decreased (P < 0.01) by an average of 12% during amino acid infusion. Leucine oxidation was 18 (sd 3) μmol h−1 kg−1 during fasting and increased (P < 0.001) by 89% during amino acid infusion. Whole-body protein synthesis (non-oxidative leucine disappearance) was 104 (sd 6) μmol h−1 kg−1 during fasting and rose by 13% (P < 0.001) during mixed amino acid infusion. 4. 13C enrichment of muscle free leucine was only 61 (sd 19) % of that in plasma α-ketoisocaproate and this increased to 74 (sd 16) % (P < 0.02) during mixed amino acid infusion. 5. The results suggest that increased availability of amino acids reverses whole-body protein balance from negative to positive and a major component of this is the increase in muscle protein synthesis.

Abstract We compared concentrations of antithrombin III (AT-III) in plasma, as determined by an immunological method and by a functional thrombin inhibition method, in the presence of heparin in 160 blood samples from Type I diabetics. Although the correlation was highly significant (P less than 0.001) between the results obtained by the two methods, our data demonstrated that results by the thrombin inhibition assay, 121 (SD 15)%, expressed as percentages of the results for a normal plasma pool, were significantly (P less than 0.001) higher than by the immunoreactive method, 104 (SD 15)%, indicating an overestimation of functionally active AT-III. Concentrations of functionally active AT-III determined by a factor Xa inhibition assay, 105 (SD 13)%, were in the same range as immunoreactive AT-III. Addition of IgG antiserum to normal pooled plasma quenched only about 90% of the AT-III activity determined by the thrombin inhibition assay, but all of the AT-III activity determined by a factor Xa inhibition assay. These results demonstrate that the factor Xa inhibition assay is more specific for the determination of AT-III than the thrombin inhibition assay. We suggest that the high concentrations of heparin cofactor II, 117 (SD 17)%, might have caused an overestimation of AT III in this group of patients with diabetes Type I, and should not be overlooked in other clinical situations.

Endometrial caruncles were excised from sheep (caruncle sheep) before pregnancy. The effect of this on umbilical and uterine blood flows in a subsequent pregnancy was examined. Thirteen caruncle and twelve control sheep with indwelling vascular catheters were studied at 121 and 130 days pregnancy. In caruncle sheep, fetal, placental, and total uterine content weights were significantly lower than in control sheep. Six caruncle sheep carried normal-sized fetuses (weight within +/- 2 SD of mean weight for control fetuses) and seven carried small fetuses (weight greater than +/- 2 SD below mean weight for control fetuses). Mean weights of placentas in these groups were 0.290 +/- 0.067 and 0.156 +/- 0.069 kg, respectively, compared with 0.459 +/- 0.157 kg in control sheep. In small caruncle fetuses, umbilical and uterine blood flows and placental antipyrine clearance were significantly lower than in controls at 121 and 130 days gestation. Only umbilical blood flow was reduced in normal-sized caruncle fetuses. Umbilical blood flow and placental antipyrine clearance increased with gestational age in control sheep but not in sheep with normal-sized or small caruncle fetuses. In all sheep, umbilical and uterine blood flows and antipyrine clearance correlated with placental weight. Umbilical blood flow per kilogram of placenta but not uterine blood flow per kilogram of placenta correlated inversely with placental weight. Fetal weight at 130 days generally correlated with placental weight, umbilical and uterine blood flows, and antipyrine clearance in a curvilinear fashion such that fetal weight was not greatly restricted until these variables were less than or equal to 65% of control values.

ABSTRACTWe offer the first sub-seasonal view of glacial age archives from the Siple Dome-A (SDMA) ice core using the ultra-high resolution capabilities of a newly developed laser ablation-inductively coupled plasma-mass spectrometry (LA-ICP-MS; 121 µm sampling resolution) system capable of conducting multi-element glaciochemical analysis. Our ultra-high resolution data demonstrates that: (1) the SDMA ice core record can be annually dated based on seasonality in chemical inputs at a depth not previously possible using previous glaciochemical sampling methods, (2) winter accumulation at the SD site was greater than summer accumulation during the three late glacial periods selected (~15.3, 17.3, 21.4 Ka ago) in this study and (3) resulting annual layer thicknesses results show greater variability than the current SD ice core depth/age model (Brook and others, 2005), possibly due to depositional effects such as wind scouring and/or decadal variability in snow accumulation that is not captured by the resolution of the current depth/age model.

Abstract. Surtsey, the youngest of the islands of Vestmannaeyjar, is an oceanic volcano created by explosive basaltic eruptions during 1963–1967 off the southern coast of Iceland. The subsurface deposits of the volcano were first sampled by a cored borehole in 1979. In summer 2017, three cored boreholes were drilled through the active hydrothermal system of the volcano by the International Continental Scientific Drilling Program (ICDP) SUSTAIN Expedition 5059. These cores are expected to provide the first glimpse of microbial life in very young and native basaltic tuff of the oceanic crust. To reduce the contamination of the subsurface environment, seawater circulating fluid was filtered and passed through two UV-sterilizing treatments. One of the boreholes has been equipped with a subsurface observatory dedicated in situ experiments for monitoring water–rock interactions and microbial processes in sterile, artificial basaltic glass and in olivine granules. With temperatures ranging from 25 to 125 ∘C, the subsurface observatory provides a precise geothermal window into an active hydrothermal system and thus represents an exceptional natural laboratory for studying fluid–rock–microbe interactions at different temperature regimes and facilitates experimental validation of active submarine microbial processes at the limit of functional life, about 121 ∘C. Comparisons with the 1979 and 2019 drill cores will provide time-lapse observations of hydrothermal processes over a 50-year timescale. Here, we present the technical design of the observatory and the incubation chamber experiments deployed from September 2017 to summer 2019.

The two formulas had similar Mn concentrations (77 and 99 μg/1) but had different Fe, Cu (121 and 619 μg/1), Zn and I contents. The mean Cu concentration in mother's milk was 833 μg/1.3. The following mean daily Mn intakes and retentions (μg/kg) respectively were measured: breast-fed fullterm 1·06 (sd 0·43) and 0·43 (sd 0·65), formula-fed full-term 14·2 (sd 3·1) and 2·8 (sd 4·8), formula-fed preterm 15·0 (sd 2·2) and 0·06 (sd 5·87). The results for Cu were 114·5 (sd 22·3) and 88·0 (sd 46·5) μg/kg in breast-fed, 19–8 (sd 4·2) and 4·6 (-11·5–9·6) in the unsupplemented formula-fed and 106·4 (sd 18·9) and 55·5 (sd 20·3) in the supplemented formula full-term infant group. No significant influence of the trace element contents of the formulas on the relative retention of Mn or Cu was found.4. Young preterm infants, and to some degree young full-term infants, often had negative Mn balances caused by a high faccal excretion. The formulas with a Mn concentration below 100 μg/l gave a sufficient supply of Mn. Preterm infants fed on the unsupplemented formula had a marginal Cu supply and their first balances were negative (-3·8 (sd 1·8) μg/kg).5. In accordance with the estimated safe and adequate daily dietary intakes (recommended dietary allowances), formula-fed infants receive much more Mn than breast-fed infants and their absolute retention is higher.6. Cu from breast-milk had a significantly better biological availability than that from cow's milk formula. If retentions similar to those in breast-fed infants are intended, we conclude, therefore, that cow's milk formula should be fortified with Cu up to a level of at least 600 μg/l.

ABSTRACT Introduction The posterior inclination of the tibial plateau relative to the longitudinal axis of tibia is referred to as the posterior tibial slope (PTS). There is paucity of data regarding PTS in Indian population. Metaphysio-diaphyseal angle (MDA) is the angle between longitudinal axis of tibia and proximal tibial metaphysis, a new entity with a possible clinical significance. This study was performed to determine the mean PTS and mean MDA to study the correlation of PTS and MDA changes with osteoarthritic degeneration in Indian population and to assess the sensitivity and specificity of PTS and MDA in detecting osteoarthritis. A descriptive, cross-sectional study design was followed. Materials and methods A total of 173 X-rays with true PA and lateral views were examined from 121 individuals using standardized technique. Osteoarthritis was classified based on Ahlback grading system. Posterior tibial slope was defined as the angle formed by two lines in the lateral knee radiograph. Metaphysio-diaphyseal angle is a new entity defined in this study, formed between two lines—first line is the proximal anatomical axis of the tibia and the second is the axis of the proximal tibial metaphysis. All the observations and measurements of PTS and MDA were statistically analyzed using MedCalc software. Results There were 121 individuals in the study with 91 osteoarthritic knees and 82 normal knees. The mean PTS among normal group is 9.69° [range 5–13° with standard deviation (SD) 1.81] and among arthritic group is 14.05° (range 10–24° with SD 2.38). The mean MDA among normal group is 19.87° (range 15–30° with SD 2.70) and among arthritic group is 25.03° (range 19–34° with SD 3.05). There is a moderate correlation between PTS and MDA (r = 0.64). Sensitivity and specificity in detecting osteoarthritis with PTS is 96.7% and 85.4% and by MDA is 90.1% and 84.7%, respectively. Conclusion Our study finds that native PTS is similar to that of oriental population but higher than that of Caucasians. There is moderate linear correlation between PTS and MDA. To evaluate the frequency of Helicobacter pylori in individuals with functional dyspepsia. Study Design: Cross sectional descriptive study. Period: Six months from 01-May-2014 to 31-10-2014. Setting: Liaquat National Hospital, Karachi. Materials and Methods: All the patients between 18 to 70 years of age diagnosed as functional dyspepsia for more than 03 months duration were admitted and evaluated for Helicobacter pylori infection through histopathological examination. Results: Total 121 subjects with functional dyspepsia were screened for Helicobacter pylori infection. The mean ±SD for age of subjects with functional dyspepsia was 41.74±08.87. The mean age ±SD of Helicobacter pylori infected individuals was 40.70±7.82 while the mean ±SD of age in helicobacter pylori negative patients was 40.55±10.63. Majority of the subjects were out-patients, 30-39 years of age with male predominance. The most common duration of disease observed was 6-9 months with epigastric pain and burning the predominant symptoms. The Helicobacter pylori infection was observed in 78(64.4%) individuals, of which 55 were males and 23 were males (p=<0.01). Conclusion: The functional dyspeptic patients are prone to acquire Helicobacter pylori infection therefore present study reported 64.4% prevalence for H. pylori infection with male gender predominance.

Objective: The aim of this study was to evaluate the feasibility and safety of multimodal therapy for patients with non-superacute vertebral basilar artery occlusion. Method: We performed a retrospective analysis of multimodal therapy for patients with vertebral basilar artery occlusion. All patients who were beyond the time window to receive intravenous thrombolysis and who had deterioration of symptoms after drug treatment received small-balloon dilatation of the occlusive artery to estimate vascular occlusion aetiology. Thrombectomy with a Solitaire AB system was applied to the thrombus, and angioplasty with intracranial stents was used to treat stenosis. During the 3-month follow-up, National Institute of Health Stroke Scale (NIHSS) and modified Rankin scale (mRS) scores were recorded regularly. Results: We included 12 patients with a mean age of 60.4 (SD: 12.9) years. The average score on the NIHSS was 16.6 (SD: 11.6), and the average time from onset to admission was 95 h (SD: 121). The arteries were recanalized for all patients, but the degree of residual stenosis in the parent artery was 17.5% (SD: 20.1). During the follow-up period, one patient died of pulmonary haemorrhage and infection, and another patient died from haemorrhage related to high perfusion. After 3 months of follow-up, the 10 surviving patients showed an average NIHSS score of 7.9 (SD: 8.7) and an average mRS score below 2 (1.3 ± 1.4). Conclusion: For patients with posterior circulation stroke due to basilar artery or vertebral artery occlusion who present to the hospital 6 h after symptom onset and who exhibit deterioration of symptoms after drug treatment, multimodal recanalization of the occluded artery may be a feasible and safe therapy.

Findings were compared with students admitted via the general (usual) admission pathway.DesignRetrospective observational study using secondary data.Setting 6-year medical programme (MBChB), University of Auckland, Aotearoa New Zealand. Students are selected and admitted into Year 2 following a first year (undergraduate) or prior degree (graduate).Participants1676 domestic students admitted into Year 2 between 2002 and 2012 via three pathways: GENERAL admission (1167), Māori and Pacific Admission Scheme—MAPAS (317) or Rural Origin Medical Preferential Entry—ROMPE (192). Of these, 1082 students completed the programme in the study period.Main outcome measuresGraduated from medical programme (yes/no), academic scores in Years 2–3 (Grade Point Average (GPA), scored 0–9).Results735/778 (95%) of GENERAL, 111/121 (92%) of ROMPE and 146/183 (80%) of MAPAS students graduated from intended programme. The graduation rate was significantly lower in the MAPAS students (p<0.0001). The average Year 2–3 GPA was 6.35 (SD 1.52) for GENERAL, which was higher than 5.82 (SD 1.65, p=0.0013) for ROMPE and 4.33 (SD 1.56, p<0.0001) for MAPAS. Multiple regression analyses identified three key predictors of better academic outcomes: bridging programme attendance, admission as an undergraduate and admission GPA/Grade Point Equivalent (GPE). Attending local urban schools and higher school deciles were also associated with a greater likelihood of graduation. All regression models have controlled for predefined baseline confounders (gender, age and year of admission).ConclusionsThere were varied associations between admission variables and academic outcomes across the three admission pathways. Equity-targeted admission programmes inclusive of variations in academic threshold for entry may support a widening participation agenda, however, additional academic and pastoral supports are recommended.

The role of negative social exchanges that amplify the association between stressor and depressive symptoms was examined. Data collected from 121 female college students (mean age = 18.9, SD = 0.7) were analyzed to determine whether the effect of life event stress on depression differs depending on participants' negative exchanges with members of their social network. The results indicated that the association between the total number of experienced life events and depressive symptoms was not amplified by negative exchanges. However, analyses testing life events individually indicated that negative exchanges significantly amplified the association of depressive symptoms with life events that were related to participants' financial strain or salient social roles.

121 Background: In the home hospice setting, family caregivers (CGs) often assume medication administration responsibilities traditionally performed by licensed nurses. Little is known about how to assess and support CG medication management skills. As part of an ongoing study of nurse-caregiver interactions in home hospice, we gathered data on CG medication management needs. Methods: A longitudinal, multicenter study of home hospice nurse visits captured audiorecorded communication between nurses and CGs. Participants included patients with cancer and their self-identified CGs who were recruited upon home hospice enrollment. The current sub-study included participants aged ≥65 and their family CGs from 7 hospice agencies. Two investigators independently coded transcriptions of the first audio recorded nurse home visit using a directive content analytic approach to map conversations to a previously published, interview-derived, framework for hospice medication management by CGs. Results: A total of 18 patients (mean age 76.5 [SD 10.7], 56% female) and their CGs (mean age 59.6 [SD13.4], 78% female) were included. Content analysis revealed that CG skills needed for medication management are not limited to drug knowledge. Complicated organizational skills are needed to track medication acquisition and dosing, record the use of short- and long-acting drugs with similar modes of action, and coordinate medication administration by multiple CGs. Teamwork skills are needed to help coordinate medication prescribing between specialist, primary, and hospice physicians. CGs also need symptom management skills regarding the proper selection of medications, as well as skills to manage side effects, inadvertent errors, and possible medication related-emergencies. CGs play a vital role in patient advocacy, alerting providers to the burden and quality of life issues related to medication use, including whether medications have intended or unintended effects, or are potentially unnecessary or causing harm. Conclusions: CGs must have multiple skills to effectively manage medications in home hospice. A systematic approach and intervention is needed to support CGs’ medication management skills.

Metode pengumpulandata dalam penelitian ini adalah dengan menggunakan tes. Untuk menguji hipotesis penelitian dengan menghitung koefisienantara X dan Y dengan menggunakan rumus Korelasi Product Moment. Dari hasil perhitungan uji hipotesis diperoleh nilai rxyhitung = 0,866 kemudian dibandingkan dengan rtabel pada taraf signifikan 0,05 diperoleh nilai rtabel untuk dk = 91 dan α =0,05 adalah = 0,207, oleh karena itu rxy hitung > rtabel maka H1 diterima. Besarnya hubungan atau korelasi antara kecerdasanlogis matematis dengan hasil belajar matematika setelah dianalisis diperoleh korelasi r = 0,866 dengan koefisien determinasisebesar 75%. Dengan demikian disimpulkan bahwa terdapat hubungan yang signifikan antara kecerdasan logis matematisdengan hasil belajar matematika pada siswa Kelas V SD Gugus bajawa I Tahun Ajaran 2016/2017.

Information on habitual consumption of soya foods and soya milk was obtained by personal interview. The intakes of isoflavones were then estimated using the US Department of Agriculture nutrient database. Logistic regression analyses were performed to assess the association between soya consumption, isoflavone intakes and oesophageal cancer risk.SettingUrumqi and Shihezi, Xinjiang Uyghur Autonomous Region, China.SubjectsParticipants were 359 incident oesophageal cancer patients and 380 hospital-based controls.ResultsThe oesophageal cancer patients consumed significantly less (P < 0·001) total soya foods (mean 57·2 (sd 119·0) g/d) and soya milk (mean 18·8 (sd 51·7) ml/d) than the controls (mean 93·3 (sd 121·5) g/d and mean 35·7 (sd 73·0) ml/d). Logistic regression analyses showed an inverse association between intake of soya products and the risk of oesophageal cancer. The adjusted odds were OR = 0·33 (95 % CI 0·22, 0·49) and OR = 0·48 (95 % CI 0·31, 0·74) for consuming at least 97 g of soya foods and 60 ml of soya milk daily (the highest tertiles of consumption), respectively, relative to the lowest tertiles of consumption. Similarly, inverse associations with apparent dose–response relationships were found between isoflavone intakes and oesophageal cancer risk.ConclusionsHabitual consumption of soya products appears to be associated with reduced risk of oesophageal cancer in north-west China.

Abstract Introduction: Improper hip joint development may lead to numerous unfavourable changes in the musculoskeletal system. The aim of this research was to determine how often adolescents with idiopathic scoliosis and their healthy counterparts experienced anomalies of the hip in their infancy period and to examine the correlation between the occurrence of hip anomalies and idiopathic scoliosis. Material and methods: The research was conducted in medical centres as well as in schools. The parents of adolescents with idiopathic scoliosis and parents of healthy adolescents completed a questionnaire on the basis of their child’s development history included in the medical records book and other medical documentation. Results: 533 questionnaires were taken into consideration, included 145 questionnaires completed by parents of adolescents with scoliosis (121 girls - 13.8 years, SD 1.9; 24 boys - 12.9 years, SD 2.5) and 388 questionnaires from the group of adolescents without scoliosis (194 girls - 13.5 years, SD 2.0; 194 boys - 13.4 years, SD 2.1). No significant differences were noted in the incidence of hip anomalies between the groups of girls and boys with and without scoliosis, no correlations between anomalies of the hip and scoliosis were found (girls χ2=0.840; Cramer V=0.052; p=0.36; boys χ2=1.205; Cramer V=0.074; p=0.27). Conclusions: Hip anomalies such as hip dysplasia, movement asymmetry or range of motion limitations diagnosed in the infancy period did not correlate with idiopathic scoliosis. Further research aimed at a separate analysis of the influence of hip dysplasia on the occurrence of idiopathic scoliosis should be carried out.

Relationships between common lifestyle practices important to sleep hygiene (e.g., smoking cigarettes, drinking alcohol, ingesting caffeine, exercising, bedtimes, getting-up times) and nocturnal sleep have not been documented for women with insomnia in their home environments. This community-based sample of 121 women, ages 40 to 55 years, included 92 women who had experienced insomnia for at least 3 months and 29womenwith good-quality sleep. Women recorded lifestyle practices and sleep perceptions (time to fall asleep, awakenings during sleep, feeling rested after sleeping, and overall sleep quality) in diaries while undergoing 6 nights of somnographic sleep monitoring at home. Compared to women with good-quality sleep, women with insomnia reported greater nightto-night variation in perceived sleep variables, poorer overall sleep quality (M = 2.8,SD = 0.7 vs.M = 1.9,SD = 0.5,P < 0.05), and longer times to fall asleep (M = 25 min,SD = 14.2 vs.M = 12.9 min,SD = 5.8,P < 0.05). Correlations between mean individual lifestyle practice scores and mean perceived or somnographic sleep variables were low, ranging from 0 to 0.20. An aggregated sleep hygiene practice score was not associated with either perceived or somnographic sleep variables. Regression analysis using dummy variables showed that combinations of alcohol, caffeine, exercise, smoking, and history of physical disease explained 9% to 19% of variance in perceived or somnographic sleep variables. Lifestyle practices, and combinations thereof, do warrant consideration when assessing or treating insomnia, but these data fail to support a dominant relationship between lifestyle practices and either perceived or somnographic sleep variables.
