A desmineralização do esmalte dentário durante o tratamento ortodôntico é, ainda, um problema significante na Ortodontia. A presença de um aparato metálico retentivo sobre uma superfície dentária plana permite um sítio ideal para retenção de bactérias e restos alimentares, dificultando a higienização. O presente trabalho teve como objetivo avaliar através da lupa estereoscópica e da microscopia eletrônica de varredura, o efeito de um adesivo ortodôntico e um selante de fóssulas e fissuras na prevenção da desmineralização do esmalte ao redor dos bráquetes ortodônticos após exposição dos dentes à solução desmineralizadora. Foram utilizados 90 incisivos bovinos separados em três grupos de 30 dentes cada: grupo adesivo (G1), grupo selante de fóssulas (G2) e grupo controle (G3). Após a colagem dos bráquetes, os dentes foram submetidos a uma solução desmineralizadora durante 14 dias com monitoramento do pH através de um peagômetro. A análise por lupa estereoscópica e microscopia eletrônica de varredura mostrou que em todos os 90 dentes foram observados a ocorrência de desmineralização ao redor do bráquete ortodôntico. Entretanto, as áreas de perda de esmalte foram diferentes nos três grupos (ANOVA, p < 0,001). O grupo G2 apresentou uma redução significativa na incidência de desmineralização do esmalte (71,9%), enquanto esta redução no grupo G1 foi de 38%, quando comparados com grupo G3 (Tukey, p < 0,001). À microscopia eletrônica de varredura pode-se verificar a presença de crateras, poros e perda da camada aprismática do esmalte nos três grupos. O uso de selantes de fóssulas e fissuras para proteção da superfície vestibular dos dentes reduziu significativamente a desmineralização do esmalte in vitro.
The demineralization of dental enamel during orthodontic treatment is still a significant problem in Orthodontics. The presence of a retentive metallic apparatus on a dental flat surface allows an ideal site for retention of bacteria and food debris, making the cleaning difficult. This study aimed to evaluate through the stereoscopic microscope and scanning electron microscopy, the effect of an orthodontic adhesive and pit and fissure sealant preventing demineralization of enamel around orthodontic brackets after exposure of the teeth to demineralized solution. Ninety bovine incisors were used in three separate groups of 30 teeth each: adhesive group (G1), sealant of fossulae group (G2) and control group (G3). After bonding of brackets, the teeth were demineralized in a solution during 14 days with pH monitoring by a pH Meter. The analysis by stereoscopic microscope and scanning electron microscopy showed that in all 90 teeth was observed the occurrence of demineralization around orthodontic brackets. However, the areas of loss of enamel were different in the three groups (ANOVA, p < 0,001). The G2 group showed a significant reduction in the incidence of demineralization of enamel (71,9%), while the reduction in G1 was 38% when compared with group G3 (Tukey, p < 0,001). Scanning electron microscopy verified the presence of craters, pores and loss of the enamel aprismatic layer in the three groups. The use of pit and fissure sealant for protecting the labial surface of the teeth resulted in a significantly reduction of enamel demineralization in vitro.

Full textThe use of dental implants and prosthetic components have become very feasible resources to solve simple and complex cases in rehabilitation dentistry. However, the implant component junction may present problems, such as the presence of a microgap between these components parts.Therefore, several materials have been tested in an attempt to obstruct this space, with controversial results. The objective of this study was to evaluate in vitro the behavior of an anaerobic monocomponent adhesive (Loctite® 510, Henkel Ltda), high resistance and inert characteristics, in the microgap seal preventing bacterial passage from the external side to the inner side of the assembly. A total of 90 implants were used, with hexagonal external connection (n = 30), internal hexagonal connection (n = 30) and internal conical connection (n = 30) divided into Control Group (n = 45). Implants were opened in laminar flow cabinet and each unit received 3l of sterile BHI internally. On each unit a specific prosthetic component was screwed in for each brand and with torques recommended by the manufacturers with digital torque wrench. The sets of Test also received a thin layer of adhesive that was applied with microbrush between the parts; and all units of the Control and Test groups were sealed with provisional obturator material in their most coronal portion. Each set was immersed in 75 l of Enterococcus faecalis (ATCC - 29212) in mini-tubes to the limit between the implant / prosthetic component junction. All samples were incubated in a bacteriological incubator for 7, 14 and 28 days at 37oC before being reopened. After the proposed period, the internal contents of the implants were collected with sterile paper cones, diluted and seeded in Petri dishes, incubated for 48 hours. There was a significant difference between the test and control groups in the hexagonal external connection group at all times (Fischer and Chi-square test, p 0.05). The anaerobic adhesive tested works as a barrier, not allowing the migration of bacteria into the internal environment of the implant / prosthetic component assembly.

Full textThis study aimed to evaluate in vitro the efficacy of the application of infiltrant, compared to sealant and two different adhesive systems on prevention (phase 1 - sound enamel) and inhibition of the progression of erosive lesions associated or no the enamel surface conditioning prior to application of the material (phase 2). The specimens were prepared from bovine enamel. In phase 1 (prevention), the sound enamel surface (H) were randomly divided into 5 groups according to the material used: HCN untreated - negative control, HH application of resin sealant - Helioseal Clear®, HA application of self-etching adhesive system - AdheSE®, HT application of phosphoric acid-etching adhesive system - Tetric Nbond ®, HI application of infiltrant - Icon®. In phase 2, all specimens were eroded by immersion in 0.01 M HCl, pH 2,3 for 30 s to simulate initial erosion lesion, and then randomized into 9 groups (E). In groups ECn, EH, EA, ET and EI, the specimens were treated with the same materials, respectively, following the manufacturers recommendations and the groups, EHs, EAs, ETs e EIs no enamel surface conditioning was performed. For both phases, all blocks were subjected to erosive cycling by immersion of the blocks in hydrochloric acid (0.01 M, pH 2.3) for 2 minutes, followed by immersion in artificial saliva for 120 minutes 4x/day for 5 days. The enamel alteration was evaluated by profilometry and the results subjected to Kruskal-Wallis and the Dunns test (p <0.05). In phase 1, the infiltrant (HI), sealant (HH) and self-etching adhesive (HA) protected the enamel against erosion and were significantly different from control. In phase 2, the groups EH, EA, EI (sealant, selfetching adhesive, infiltrant), ETs and EIs (phosphoric acid-etching adhesive and infiltrant, both without surface conditioning) were similar and showed no enamel alteration after erosion. Greater enamel wear was observed in groups ECn (untreated - negative control), ET (phosphoric acid-etching adhesive), EHs (resin sealant without surface conditioning), AEs (self-etching adhesive without surface conditioning) and these were statistically similar. Taking into consideration the results, it is concluded that the application of infiltrant (with or without prior enamel conditioning) and sealants on sound and initially eroded enamel, prevented enamel alterations when subjected to erosive challenge.

Full textO objetivo deste trabalho, dividido em três estudos, foi investigar (1) a reparabilidade de um cimento de ionômero de vidro modificado por resina, (2) o efeito cariostático e (3) a retenção de selantes ionoméricos aderidos ao esmalte não desgastado com sistemas adesivos autocondicionantes simplificados. Materiais e Métodos: Para o estudo 1 foram confeccionados 60 espécimes de cada material, Vitremer (VT) e resina Z250 (RC). Após envelhecimento químico e térmico, eles foram divididos em 3 grupos de acordo com o tratamento mecânico da superfície: ponta diamantada em alta rotação, ponta em ultra-som e sem desgaste. Cada espécime foi reparado com o mesmo material utilizado inicialmente. Aplicou-se na superfície do VT o Vitremer Primer (VP), e para a resina, utilizou-se ácido fosfórico seguido de adesivo Single Bond. Os espécimes foram isolados deixando exposta a interface de união. Após novo envelhecimento, os mesmos foram impregnados por nitrato de prata, seccionados e avaliados quanto à infiltração e formação de fendas. Os dados foram analisados pelos testes de Kruskal-Wallis e Mann-Whitney (α=0,05). No segundo estudo 64 incisivos bovinos foram distribuídos em 4 grupos segundo o tratamento da superfície de esmalte: VP, Prompt L-Pop (Pr), Xeno III (Xe) ou sem tratamento. Um cilindro de VT foi construído em metade dos dentes, enquanto que a RC foi utilizada na outra metade. Os corpos de prova foram 19 submetidos a desafio cariogênico e seccionados, para realização de testes de dureza em três regiões: externa, união e interna, até a profundidade de 300 μm. Os dados transformados em porcentagem de perda mineral foram analisados pelo teste de ANOVA complementado pelo teste de Tukey ao nível de significância de 5%. The purpose of this work, divided into three studies, was to investigate (1) the reparability of a resin-modified glass-ionomer cement, as well as (2) the cariostatic effect and (3) retention of glass-ionomer sealants bonded to enamel with one-step self-etching adhesive systems. Materials and Methods: For the first study, 60 specimens were prepared with Vitremer (VT) and Z250 resin. After chemical and thermal aging, the specimens were divided into 3 groups according to the mechanical treatment of the surface: diamond bur, ultrasound coupled bur or no treatment. Each specimen was repaired with the original restorative material. For VT repairs, Vitremer Primer (VP) was first applied while for composite resin the surface was phosphoric acid etched followed by the application of Single Bond. The specimens were coated with acid resistant varnish except for 1 mm around de interface. After additional aging, the specimens were impregnated by silver nitrate and sectioned medialy for the evaluation of silver infiltration and gap formation in SEM. Data were submitted to Kruskal-Wallis and Mann-Whitney tests (α=0.05). In the second study, 64 bovine teeth were randomly assigned to 4 groups according to the enamel surface treatment: VP, Prompt L-Pop (Pr), Xeno III (Xe) or no treatment. A cylinder of VT was built up in half of the teeth while a composite resin was used in the other half. The teeth were then submitted to a cariogenic challenge after which they 23 were medialy cut. Transversal hardeness evaluation was performed in three regions, external, interface and internal, up to the depth of 300μm. Hardness numbers were converted to percentage of mineral loss and the data were submitted to ANOVA and Tukey tests at the level of significance of 5%. Full textThe use of resin-based materials over the dental tissue is an alternative for erosion progression prevention, however there is little information regarding the effect of resin infiltration into erosive lesion. The aim of this study was to evaluate the effect of resin-based materials over (without excess removal) and into (with excess removal) previously eroded enamel, subjected to erosive challenge. Bovine enamel blocks were immersed in HCl 0.01 M, pH 2.3 for 30 seconds for the formation of softened erosion lesion. Then the blocks were random divided and treated according to 8 groups (n=12): Cc and Cs- control without treatment, Hc and Hs- fissure resin sealant (Helioseal Clear®), Ac and As- self-etch adhesive (Adhese®), Ic and Isinfiltrant (Icon®); being c- with excess removal and s- without excess removal of the material. The materials were applied according to manufacture instructions. The groups with excess received the same materials, however before light curing the excess of material over enamel was gently removed with a cotton roll. After treatment, the blocks were immersed in HCl for 2 min, followed by immersion in artificial saliva for 120 min, 4 times per day, during 5 days (erosive cycling). Enamel wear and material thickness were analyzed using profilometry and the results submitted to two-way ANOVA and Tukeys test (p<0.05). Groups Hs, As, Is, resulted in the formation of a layer of material over enamel, being effective in inhibiting erosion progression. Groups in which the material over enamel was removed (Hc, Ac, Ic), resulted in enamel loss after application. In addition, they (Hc, Ac, Ic) were not able to protect enamel, since after the erosive challenge enamel wear of these groups was similar to control. Based on the results of this study, resin-based materials are able to protect enamel against erosion only when they are present over enamel, forming a mechanical barrier.

Full textThis in situ study evaluated on demineralized bovine enamel, submitted or not to a cariogenic challenge (dental biofilm), the effects of a high viscosity glass ionomer cement (HV-GIC) and a resin sealant in comparison with a control group (without sealing). Twelve volunteers wore a palatal appliance containing 12 blocks of demineralized bovine enamel mounted (2 by 2) to simulate a fissure in a V shape and sealed with: HV-GIC (Ketac? Molar Easymix), resin sealant (Delton) or maintained with no sealing material (control). Half of specimens was submitted to a cariogenic challenge by dripping a 20% sucrose solution 8 times/day besides a fluoride dentifrice solution (1 g : 3 mL) 3 times/day. The others specimens only received a dentifrice solution. After 14 days, the concentration of fluoride in dental biofilm and enamel was evaluated as well as the mineral content (cross-sectional microhardness) of the enamel. The concentration of fluoride in dental biofilm was higher with HV-GIC (Friedman ANOVA, p<0.05) in comparison with resin sealant and control group. The concentration of loosely-bound fluoride on enamel was significant (ANOVA and Tukey test, p< 0.05) specially without a cariogenic challenge for HVGIC, while it was similar for the resin sealant and control group. Concerning the mineral content, ANOVA and Tukey test (p<0.05) detected differences among the groups on the superficial layers of the enamel. Under the dental biofilm influence the mineral content was always higher on the enamel associated with HV-GIC than with resin sealant and the control group presented intermediary results. Without dental biofilm the control group presented a tendency of a higher mineral content. The results suggest that HV-GIC applied on a demineralized surface and under the dental biofilm influence has shown a cariostatic potential by its ability to give fluoride to the dental structure and biofilm reducing the demineralization effects.

Resumo: O objetivo deste estudo foi avaliar clinicamente os materiais híbridos de ionômero de vidro/resina composta, Vitremer (3M) e Dyract (Dentsply), quando utilizados como sei antes de fóssulas e fissuras. Para tanto, foram utilizados os primeiros molares permanentes inferiores de cem crianças na faixa etária de sete e oito anos, sendo um dos materiais utilizado no elemento esquerdo e o outro no direito, alternadamente. Decorridos seis e doze meses do selamento, foi efetuada a avaliação clínica e a análise comparativa dos materiais, quanto à retenção e à capacidade de prevenção de cárie em relação a um grupo controle não selado. Os resultados da análise estatística demonstraram a semelhança de desempenho entre os materiais no período de seis meses.. quanto à retenção. Aos doze meses as avaliações clínicas demonstraram índices de 95,9% e 85,7% de retenção para o Dyract e o Vitremer respectivamente, implicando em uma diferença estatisticamente significativa entre os materiais (OR > 1). Os materiais híbridos demonstraram efeito protetor contra a cárie estatisticamente significante em relação ao grupo controle, aos seis e doze meses (OR < 1)
Abstract:The aim of this study was to evaluate the clinical performance of two glass ionomer/resin composite hybrid materiais, when applied as pits and fissures sealants. The mandibular first permanent molars on one side of dental arch in 100 children were sealed with Vitremer (3M), and the same teeth on the other side were sealed with Dyract (Dentsply). Six and twelve months after placement, examiners documented sealant retention and caries prevention in relation to an unsealed contrai group. The results, supported by statistical analysis, showed similarity betwen the material's perfomance afier six months. At one year, the retention of the Vitremer (3M) Was significantly less (OR) than the Dyract (Dentsply), the rates of complete retention were 95.9% and 85.7% fqr Dyract and Vitremer respectively. The caries preventive effect of hybrid materiais in sealed group was significantly different (OR<1) in comparison with the unsealed group, at six and twelve months

Full textThis in situ study, developed in a 14-day period, aimed to evaluate the effect of two glass ionomer cements (GIC) compared to a resin sealant, employed for pit and fissure sealing on intact bovine enamel, with accumulation of dental biofilm, by analysis of the fluoride content on the dental biofilm and longitudinal microhardness. Eighty-eight enamel blocks (4mm x 4mm) of bovine incisors were employed, selected according to their initial microhardness value. The blocks formed a V-shaped artificial fissure that was sealed, constituting the following groups: GI - high-viscosity conventional GIC (Ketac? Molar Easymix); GII - resin-modified GIC (Vitremer?); GIII - resin sealant without fluoride (Helioseal? Clear); and GIV - control, without sealing. The artificial fissures were fixated on a palatal plate (PP), in two distinct rows, which were covered with a plastic screen to favor the accumulation of dental biofilm. Eleven volunteers employed the PP for 24h/day, dripped a 20% sucrose solution 8 times a day, and used a fluoridated dentifrice solution (3ml of deionized water : 1g of fluoridated dentifrice) on all fissures 3 times a day. After the study period, the accumulated biofilm was collected for evaluation of the fluoride content. The fissures were sectioned through the middle for evaluation of longitudinal microhardness from the enamel surface, at 4 depths (10, 30, 50, 70 µm). The enamel/material limit at the occlusal region was the zero position (material margin); from this point, indents were sequentially performed at 50, 150 and 500 µm internally and externally to the material margin. Data were statistically analyzed (a = 5%). The Friedman test revealed that the fluoride concentration in the biofilm was not statistically significantly different among groups (p<0.07), yet there was a tendency of better outcomes for the high-viscosity conventional GIC. With regard to the mineral content, the ANOVA and Tukey test revealed significance (p<0.05) for all groups individually, at the most superficial enamel layer (10 µm). Resumo: Embora se observe uma diminuição da prevalência da doença cárie, a superfície oclusal permanece tendo a maior incidência, devido, principalmente, à sua morfologia complexa que dificulta a limpeza mecânica e permite o acúmulo de placa dental. A utilização de selantes de fissuras tem tido comprovada efetividade na prevenção da cárie nessa superfície, no entanto, existe uma busca contínua por um material que- apresente- alta retenção à estrutura dental e efeito anticariogênico mesmo com sua perda macroscópica. Assim, esse trabalho composto por quatro artigos tem como objetivos avaliar a retenção e efetividade na proteção da cárie de: 1) um cimento de ionômero convencional e um cimento de ionôrnero modificado por resina após 2 anos de aplicação clínica como selantes; 2) um cimento de ionômero de vidro modificado por resina, uma resina composta de alto escoamento e uma resina modificada por poliácido após 6 e 12 meses de aplicação clínica como selantes; 3) dois materiais ionoméricos utilizados como selantes após 5-anos de aplicação clínica; 4) uma resina modificada por poliácido após 6 anos de aplicação como selante. Os resultados encontrados mostram que os materiais avaliados por períodos superiores à dois anos, apresentaram uma baixa retenção, no entanto, foram efetivos na prevenção da cárie oclusaL Já os materiais avaliados após 6 e 12 meses apresentaram taxa de retenção total satisfatória. Em relação à cárie, os materiais ionoméricos apresentaram-se efetivos na prevenção de lesões cariosas nas fissuras oclusais
Abstract: Although a decrease of cavities prevalence are observed, occlusal surface remains as a major incidence rate, because of its complex morphology that difficult the mechanical cleaning and allows the accumulation of dental plaque. The use of fissure sealant had proved its effectiveness on cavities prevention on this surface, however there is a continue quest for a material that presents high retention rate to enamel and a anticariogenic effect even after its macrocospic loss. This study is composed of four manuscripts, which main goals are to evaluate retention and effectiveness on cavities prevention of:: 1) two íonomer materiais after two years of clinical use; 2) a ionomer glass cement modified by resin, a flowable composite and a resin modified by a polyacid, after a clínical use as a occlusal sealant during six and twelve months, 3) two ionomer cements used as físsure sealant after five years of clinical use; 4) a polyacid modified resin after six years used as fissure sealant. The finding results show that the materiais evaluated for periods longer than two years present a low retentton rate, however they showed effectiveness on occlusal cavity prevention. Materiais evaluated after six and twelve months presented a satisfactory retention rate. Regarding cavities, the ionomeric material showed its effectiveness on occlusal caries prevention

Resumo: Selantes de fissuras têm sido considerados eficazes na prevenção de cárie oclusal, entretanto há controvérsias na literatura quanto ao material de escolha para o procedimento de selamento. Desse modo, o presente trabalho composto por 4 artigos tem como objetivos: I) descrever o desempenho dos selantes de fóssulas e fissuras ao longo dos anos e discutir sua importância como método preventivo; 2) avaliar clinicamente 3 diferentes materiais aplicados como seiantes de fissuras (Vitremer, Dyract Flow e Revolution) após 2 anos de aplicação clínica; 3) comparar, in vitro, o desgaste e mudanças na rugosidade superficial de 4 materiais odontológicos (Delton, Filtek Flow, Dyract Flow e Vitremer) aplicados como selantes após abrasão por escovação e dentifrício; 4) avaliar, in vitro, a infiltração marginal em seiantes de fissuras realizados com diferentes materiais odontológicos (Delton, Filtek Flow, Dyract Flow e Vitremer). Em relação à revisão sobre selantes, foi concluído que eles são eficazes na prevenção de cárie e suas indicações e contra-indicações devem ser cuidadosamente avaliadas pelos profissionais. Os resultados após dois anos de aplicação clínica permitiram concluir que a resina composta de alto escoamento Revolution apresentou a melhor taxa de retenção, embora não tenha havido diferença estatisticamente significante em relação à presença de cárie entre os grupos. Quanto à resistência à abrasão, não houve diferença estatisticamente significante entre os materiais Vitremer e Dyract Flow e nem entre Filtek Flow e Delton, entretanto os primeiros apresentaram maior taxa de desgaste. O material Vitremer apresentou o maior valor de rugosidade, enquanto o material Delton apresentou o menor valor após o teste de abrasão. Em relação ao teste de microinfiltração, embora todos os materiais estudados tenham apresentado um certo grau de microinfiltração, não houve diferença estatisticamente significante entre os mesmos. O contínuo estudo de materiais odontológicos para a aplicação como selantes de fissuras é recomendado, uma vez que avanços tecnológicos podem contribuir para a melhora das propriedades físicas dos materiais e, conseqüentemente, para a melhora de seu comportamento clínico
Abstract: Fissure sealants had been considered efficacious on occlusal caries prevention, however there are controversies in the literature with regard to the material to apply. This present study, composed by four articles, has the following aims: a) to describe the performance of the fissure sealants and to discuss their relevance as a preventive method; b) to evaluate three different materiaIs (Vitremer, Dyract Flow and Revolution) used as an occlusal sealant after 2-year clinical application; c) to compare the in vitro wear and changes on the superficial roughness of different materiaIs (Delton, Filtek Flow, Dyract Flow e Vitremer) used as fissure sealant after abrasion with toothbrush and dentifrice and d) to evaluate the in vitro microleakage of different materiaIs used as fissure sealants (Delton, Filtek Flow, Dyract Flow e Vitremer). Regarding the review (a), it was concluded that the fissure sealant is efficacious on caries prevention and the indications and contraindications of this proceeding should be carefully evaluated by professionals. In the 2-year clinical evaluation (b) was verified that Revolution showed the best retention rate, however there was no statistically significant difference with regard to caries presence after the evaluated period among the groups (Revolution, Dyract Flow and Vitremer). With reference to the abrasion test (c), there were no statistically significant difference either between Vitremer and Dyract Flow or Filtek Flow and Delton, however the former showed the highest wear before the abrasion test. Vitremer showed the highest roughness measurement, while Delton showed the lowest measurement after the abrasion test. Regarding the microleakage test (d), although all the materiaIs had exhibited some degree of microleakage, there were no statistically significant difference among them. A continuous study of dental materiaIs to apply as fissure sealants is recommended, since that technological advances can contribute to the improvement of physical properties of dental materiaIs and consequent1yto the improvement of their clinical behavior
