Full textBackground: Physical and cognitive disabilities may affect patients’ ability to cope with oral health and increase the risk of suffering from illness in the mouth. Studies show that staff in municipality provided care have poor knowledge in oral health and that oral health is less prioritized than other nursing interventions. Registered Nurses (RN) have the ultimate responsibility for providing care but there are few studies that describe how nurses experience working with oral health. Aim: To examine the experience of RN’s with regards to oral health and oral care in municipal health care. Method: A qualitative content analysis with an inductive approach including one focus group discussion, with five RN’s participating, and seven patientual interviews. Results: Three categories were identified: Oral health promotes general health, Works with oral health when self-care deficit and Agents in the nursing system. Conclusion: The RN’s believed that oral health is self-evident and that it is important to safeguard the patient's self-care and integrity. When working in quality registers, the RN participation in oral health work increase, but they primarily work with oral health when a problem arises. Great focus was placed on oral health and oral care in palliative patients, but the work with oral health could be suffering in patients who to some extent manage themselves. To improve oral health, the study showed that there is a need for more education for health care workers providing municipality care and RN’s, there is also a need for better routines and better collaboration with the dental care.

Full textBackground: Heart failure is a global health problem that is constantly increasing. Symptoms of heart failure affects the performance in activities of daily living and the ability to perform self-care. Proper self-care can decrease symptoms, increase self-esteem and well-being. Orem's theory Self-care deficit nursing theory was used as a theoretical framework. Aim: was to describe chronic heart failure patients experiences of daily self-care. Method: The literature review had scientific articles with a qualitative method which was analyzed by Friberg's (2017). Results: A total of 13 scientific articles were selected for the results and presented in three main categories with respective subcategories: Emotional challenges (changed emotions), Physical limitations (dyspnea & fatigue), Support through self-care (resources). Conclusion: Patients experienced that negative emotions affected their ability to perform self-care. Social support from the family had both a positive and negative impact. Conversations with various professions were valuable for patients as they gained increased knowledge about heart failure to manage their self-care. Inadequacy in healthcare staff resulted in incorrect information being communicated to patients with heart failure and causing inefficient self-care.

Full textMany women with type 2 diabetes fail in their efforts with self care management. The consequence are medical complications. The purpose of this paper is to look at a number of research articles describing women’s experiences relating to their diabetes diagnosis. These experiences affect the woman's ability to self care but also guides the nurse in her profession on how to best support the woman. Orem's self care deficit theory is tied to the diabetic woman and her self care but also to the role of the nurse. There is also an analysis of the sampling methods used in each of the articles. This study was done as a descriptive litterature study based on ten qualitative research articles. The result from the analysis of the articles shows that the experiences of the diabetic women can be sorted into five categories. 1. Self-care and lifestyle change 2. Coping with ones own feelings and emotions 3. Motivation and engagement 4. The surrounding support 5. The interaction with the caregiver/medical expertise. The conclusion that can be made from this study is that women with type 2 diabetes have a lot of emotional experiences relating to the diagnosis that affects their self care management. The analysis of the sampling methods used in each of the articles show that it is critical for the result how the sampling was done and care should be exercised before drawing any conclusions from the results.

Full textBackground: Diabetes mellitus is a chronic disease which affects more and more people around the world. Studies have shown a close relationship between lifestyle and diabetes type 2 and the importance of having a good self-care treatment to reduce the risk of complications. The diabetes nurse has an important responsibility to work with both health promotion and disease prevention through guidance in self-care. Purpose: The purpose of the study was to describe the diabetes nurse’s experiences of guiding people with diabetes in self-care. Method: 16 semi-structured interviews was performed during January – March 2016 with diabetes nurses who worked in primary care. The interviews were transcribed and analyzed using a qualitative content analysis. Result: The content analysis resulted in three categories; the ground for commitment, Knowledge transfer and Organizational conditions. The four main results in the study was: The diabetes nurse’s possibilities to create participation, knowledge that both the diabetes nurse and the person possessed, group educations health promoting impacts and the team’s impact on guidance. Conclusion: The result highlights that guidance in self-care should be built on knowledge and lead to a higher level of engagement. A well-functioning communication is crucial to achieve common goals and that the team around the person with diabetes is working together towards the same goal.

Full textEste relatório surge no âmbito do Curso de Mestrado em enfermagem Médico-Cirúrgica (EMC) e tem como finalidade apresentar reflexivamente o percurso conducente ao desenvolvimento de competências de enfermeiro especialista (comuns e específicas) e de Mestre. Para isso, foi utilizada a metodologia de trabalho de projeto, integrando dois projetos distintos: projeto de intervenção em serviço (PIS) e projeto de aprendizagem clínica (PAC), desenvolvidos ao longo dos estágios realizados num serviço de cirurgia geral (SCG). O PIS, formulado sob a premissa de que uma eficaz gestão dos cuidados está numa das bases da qualidade dos mesmos, incidiu sobre a necessidade de uniformizar procedimentos relativamente à avaliação funcional das pessoas internadas no SCG. Este, veio permitir, através de formação e criação de documentos normativos e orientadores desta prática, promover a qualidade dos cuidados de enfermagem através da otimização da intervenção dos enfermeiros no domínio da avaliação funcional da pessoa internada no SCG. Foi nosso referencial teórico a Teoria do Défice do Auto-Cuidado de Orem, com enfoque na importância de direcionar os cuidados prestados às necessidades de cada pessoa, partindo de uma avaliação funcional, dotada de objetividade através do uso de escalas validadas, como a escala modificada de Barthel. Por sua vez, no PAC projetaram-se atividades específicas, das quais se destacam: participação num exercício de simulação de inundação numa unidade de cuidados intermédios e realização de respetivo poster de divulgação; realização de ação de sensibilização aos familiares das pessoas internadas no SCG, sobre importância da higienização das mãos na visita; e realização de um poster sobre hipodermoclise, classificado em 2º lugar num encontro de enfermagem. Neste documento analisamos crítica e reflexivamente ambos os projetos, e os contributos dos mesmos no desenvolvimento das competências supra-citadas.
This report is set within the context of the Medical Surgical Nursing (MSN) Master’s Degree, in order to reflexively present the route which led us to the development of specialist (either common or MSN specific) and Master nurse skills. Therefore, we used the project methodology, integrating two distinct projects: intervention project in service (IPS) and clinical learning project (CLP), both developed throughout the internship in a general surgery service (GSS). The IPS was formulated under the premise that effective care management is one of the bases of its quality, and focused on the need to standardize procedures for the functional evaluation of people admitted to the GSS. Through training and creation of normative and guideline documents, the IPS has promoted the quality of nursing care by optimizing the nurses’ involvement in the functional evaluation of the hospitalized patient in GSS. Our theoretical framework was the Orem’s Self-Care Deficit Theory, focusing on the importance of care directed to the needs of each person, and based on a functional evaluation, endowed with objectivity through the use of validated scales such as the modified Barthel index. In turn, on the CAP we designed specific activities, among which the following are highlighted: participation in an exercise of flood simulation in an intermediate care unit and the development and disclosure of the respective poster; performing of an awareness session to the relatives of those admitted to the GSS, about the importance of hand hygiene in the visit; and creating a poster on hypodermoclysis, that was ranked 2nd in a nursing meeting. In this document we analyze critically and reflexively both projects, and their contributions to the development of the above-mentioned skills.

Abstract “Self-care deficit / bath and hygiene” is a common health problem in schizophrenia inpatients. Nurses play the primary role in dealing with this problem. However, there is no valid instrument for nurses to apply these patients. The purposes of this study were (1) to establish the care plan of “self-care deficit / bath and hygiene”, (2) to establish the reliability and validity for the defining characteristics (DC) of “Self-care deficit / bath and hygiene”, (3) to establish the reliability and validity for the related factors of “Self-care deficit / bath and hygiene”, (4) to establish the reliability and validity for the nursing interventions of “Self-care deficit / bath and hygiene”, (5) to establish the reliability and validity for the expected outcomes of “Self-care deficit / bath and hygiene”, (6) to explore the affected factors of the nursing care plan of “Self-care deficit / bath and hygiene”. Reviewing literature, 7 itemsof nursing care plan of “Self-care deficit / bath and hygiene” were constructed. Twelve nursing experts and thirty-six nursing staffs appraised the applicability of each item. One hundred and twenty schizophrenia inpatients were recruited from acute and chronic wards in teaching hospital. All the data were collected by investigator during one and half month period. Clinical assessments were carried out by observation, interview, physical examination and chart review. Results of this study showed that 22 DC, 18 related factors, 70 nursing interventions and 28 expected outcomes had high Cronbach’s alpha coefficient of 0.87, 0.89, 0.99 and 0.99, and a significant average inter-item correlation of 0.35, 0.36, 0.74 and 0.89. Correct item / total correlation of all items were over 0.30. It presented a good internal consistency perspectively. Based on the criterion of factor loading >=0.40, two and three factors were extracted from 22 DC and 12 related factors in terms of orthogonal factor analysis that accounted for the 65.63﹪and 66.71﹪of the variance. Two subconcepts of DC and and three subconcepts of related factor can be used as a reliable instrument to help nurses to make the diagnosis of Self-care deficit / bath and hygiene in schizophrenia inpatients. Moreover 70 nursing interventions and 28 expected outcomes can providenurses to offer the nursing activity for these patients.
