Nonsuicidal self-injury (NSSI), the deliberate self-destruction of one’s own body tissue engaged in without associated suicidal intent, is a prevalent behavior among adolescents and young adults. The current study examined whether one aspect of cognitive control, inhibitory control in response to negative emotional stimuli, is associated with repetitive engagement in NSSI. It further sought to examine whether sleep deficiency/irregularity, stress, and reward sensitivity moderate this relationship. A multi-method approach (self-report, behavioral measures, actigraphy) was employed to sensitively probe these relationships among 114 late adolescents with and without a history of repetitive NSSI. Findings suggested no relationship between inhibitory control in response to negative emotional stimuli and NSSI, as measured by a behavioral measure, but a significant positive relationship as measured by self-report. Stress and sleep irregularity, but not sleep deficiency or reward sensitivity, were associated with NSSI group status. Interaction analyses suggested that sleep irregularity and stress moderated the relationship between inhibitory control in response to negative emotional stimuli and NSSI. Results are discussed in terms of conceptual and clinical implications. Findings highlight the necessity of examining the temporal dynamics between the study’s constructs and NSSI by employing an ecologically valid approach.

Full textTwenty percent of all people are highly sensitive (HSP = Highly Sensitive Person) which means that they are born with a sensitive nervous system. Many HSP's are experiencing negative consequences in their everyday lives that affect their quality of life. As the group HSP is large and the correlation between HSP, quality of life and self-compassion is not specifically is investigated, the present study was relevant. The examined questions are: Is there within the group of HSP regarding Quality of Life, Self-compassion, High sensitivity, BIS, BAS, Extraversion and Neuroticism (a) significant correlations between the variables? (b) To what extend do the variables explain Quality of Life and High Sensitivity? (c) Are there gender differences regarding the variables? (d) Is there a positive correlation between High Sensitivity and Age? The 111 participants were members of the Sveriges Förening för Högkänsliga and all were HSP according to self-assessment, which also was confirmed by the data collected. The group of participants consisted of 93 women (84%) and 18 men (16%) and the average age was 50, where the lowest age was 35 years and the highest 70. All questions were answered by these analytical methods: Pearson's correlation analysis, Standard Multiple Linear Regression and T-test. The main outcome was that the link between HSP and self-compassion contributed to new knowledge, which concluded that the practice of Self-compassion can help to actively increase the level of quality of life for HSP's. The strongest correlation in the study was between BIS and neuroticism, which means that at an increase of BIS, the degree of negative affect also increased and vice versa; the result was in line with previous research. Two unexpected results were that (a) the driving force (BASDrive) predicted high sensitivity most, unlike the literature suggesting that BIS and neuroticism would have the strongest connection and (b) the degree of high sensitivity decreased with increase in age; which contradicts earlier research. The essay is a relevant source of information for the HSP and all others who are interacting with HSP.

Full textKokvattenreaktorer av nordisk typ har en djup vattenbassäng under reaktorkärlet som kan utnyttjas för att kyla härdsmältan och de fragmenterade härdresterna vid ett svårt reaktorhaveri. För att bedöma effektiviteten av en sådan haverihantering har man föreslagit användande av en riskorienterad metodik för haverianalysen (ROAAM, från engelska ”Risk-Oriented Accident Analysis Methodology”). Föreliggande projekt fokuserar på kylbarhet hos härdresterna utanför reaktortanken och bidrar till den pågående vidareutvecklingen av ROAAM till ROAAM+. Höjden på och formen för den porösa ansamlingen av härdrester (här också kallad partikelbädd) är bland de viktigaste faktorerna som avgör om resteffekten kan kylas bort med hjälp av naturlig cirkulation av vattnet i bassängen. Ansamlingens geometriska form skapas under hela processen från utsläpp av härdsmältan via fragmentering och sedimentering i bassängens botten. Formen kan sedan förändras med tiden genom att partiklar rör sig och omfördelas i kylflödet. Detta fenomen kallas en självnivellerande process. I detta arbete studeras denna självnivellerande process experimentellt och analytiskt. Experimenten utfördes i en särskild experimentuppställning utformad för att att samla in data och parametrar som behövs för att simulera fenomenet och utveckla en beräkningsmodell som sluts empiriskt. Denna modell kopplades sedan till en modell för beräkning av dryout i partikelbädden. Genom denna koppling av de två beräkningsprogrammen är det är möjligt att beräkna tiden för partikelbädden att nå en kylbar konfiguration. Inverkan av variationer i modellens indata studeras med hjälp av känslighetsanalys. Härigenom identifierades de minst inflytelserika parametrarna såsom effektiv drifttid, partikeldensitet, experimentell ovisshet i de empiriska samband som används för att sluta modellen, samt omlokaliseringstid efter det att reaktorn snabbstoppats (SCRAM). Dessa parametrar avfördes sedan från den fortsatta känslighetsanalysen. Ett artificiellt neuralt nätverk tränades för att användas i stället för den kopplade koden och möjliggöra den beräkningseffektivitet som krävs för att studera hur osäkerheter i indata förs vidare i riskanalysen. Resultaten är presenterade i form av komplementära, kumulativa fördelningsfunktioner för den betingade sannolikheten för brott på reaktorinneslutningen (CCFP, från engelska ”conditional containment failure probability”). Det visas att CCFP kan variera inom ett brett område beroende på de valda kombinationerna av frekvensfunktioner för ingångsparametrarna. Resultaten visar att effektiv partikeldiameter och hög porositet är de två parametrar som ger de största bidragen till osäkerheten i CCFP. Vi har också funnit att fenomenet självnivellering har en gynnsam inverkan på CCFP och leder till lägre utsläppsrisk. Det vore värdefullt att förfina de modeller som beskriver bildandet av den initiala partikelbädden. Detta är särskilt viktigt i de scenarier där det finns kort tid för självnivellering innan partikelbädden börjar smälta igen, dvs när man har relativt hög initial temperatur i partikelbädden och/eller hög specifik värmeeffekt.

Full textNickelallergi är vanligt förekommande. Prevalensen i Skandinavien är 15--25% hos kvinnor och cirka 3% hos män. Sambandet mellan nickelallergi och uppkomst av handeksem har tidigare diskuterats och i vissa studier anges att 30--45% av alla individer med nickelallergi får handeksem. Det finns dock endast ett fåtal publicerade studier där personer ur normalbefolkningen har lapptestats för nickel. Handeksem ärvanligt och har ofta flera olika kombinerade orsaker. Det övergripande syftet med avhandlingen var att studera nickelallergins betydelse för uppkomst av handeksem. Detfinns ett intresse av att följa förekomsten av nickelallergi över tid, speciellt sedan det i början av 2000-talet infördes ett EU-direktiv som begränsar nickelinnehåll i klockor,smycken, metallknappar etc. Ytterligare ett syfte med avhandlingen var att utvärderaepidemiologiska metoder för att följa förekomsten av nickelallergi.Den första studien var en uppföljningsstudie av 908 flickor ur normalbefolkningen,vilka i skolåldern lapptestats med nickel. Tjugo år senare skickades en enkät till dessa kvinnor, svarsfrekvensen var hög (81%). Förekomsten av självrapporterat handeksemefter 15 års ålder var 17.6%. Det förelåg ingen signifikant skillnad i förekomst avhandeksem mellan de kvinnor som var nickelallergiska som barn jämfört med dem som inte var nickelallergiska. År 2006 utfördes ytterligare en studie, som inkluderade de kvinnor som fortfarande bodde i Örebro län. Studien omfattade en klinisk undersökning av händerna samt ett lapptest. 30% av kvinnorna var positiva för nickel.Det förelåg ingen signifikant skillnad i förekomst av handeksem mellan de som var positiva för nickel och de som var negativa. Vid separat analys av de kvinnor som angav tidigare barneksem jämfört med dem som aldrig hade haft barneksem visade det sig att risken för handeksem var dubbelt så stor hos nickelallergiker i den gruppen som aldrig hade haft barneksem. Båda studierna visade att barneksem var den största riskfaktorn för att få handeksem som vuxen, med en 3-4 gånger ökad risk. Den tredje studien var en validering av självrapporterad nickelallergi. Överensstämmelsen var låg mellan enkätfrågor gällande nickelallergi och lapptestverifierad nickelallergi. Av dem som själva bedömde sig vara nickelallergiska var endast 59% positiva enligt lapptest. För att följa förekomsten av nickelallergi i befolkningen behövs därför andra metoder. I den fjärde studien utvärderades ett självtest för nickelallergi. 191 patienter från tre olika hudkliniker i Sverige deltog i studien. Validiteten för metoden självtest var tillfredsställande, sensitiviteten var 72%och graden av överensstämmelse var 86%.

Full textObjetivos: comparar os achados do auto-exame das mamas com o seu exame clÃnico no rastreamento de anormalidades mamÃrias. Verificar a freqÃÃncia dos conhecimentos e prÃticas relacionadas com o auto-exame das mamas. Estimar a sensibilidade e especificidade do auto-exame das mamas em relaÃÃo ao seu exame clÃnico. Averiguar se a sensibilidade à influenciada pelas variÃveis antropomÃtricas e sociodemogrÃficas ou pelos conhecimentos e prÃticas do auto-exame. Metodologia: estudo transversal de validaÃÃo de teste diagnÃstico, envolvendo 505 funcionÃrias de um hospital universitÃrio. Foi aplicado um questionÃrio sobre conhecimentos e prÃticas relacionadas com o auto-exame das mamas e ministrada instruÃÃo sobre o auto-exame a todas as participantes que posteriormente realizaram o auto-exame e foram submetidas a exame clÃnico por Ãnico examinador. Os intervalos de confianÃa de 95% foram calculados pelo mÃtodo de aproximaÃÃo pela distribuiÃÃo normal. ProporÃÃes foram comparadas por meio do teste do qui-quadrado e foram consideradas significativamente diferentes (teste bicaudado) quando a probabilidade de estas serem semelhantes foi menor ou igual a 0,05. Resultados: 94,06% tinham recebido informaÃÃo sobre o auto-exame, 29,90% pelo mÃdico. 32,28% realizavam o auto-exame mensalmente, 55,95% apÃs a menstruaÃÃo e o procedimento mais realizado era a palpaÃÃo durante o banho. A sensibilidade para qualquer achado foi de 37,21 % com IC 95% de 32,77%-41,82%. A sensibilidade, excluindo-se a assimetria, foi de 44,85% com IC 95% de 39,59-50,26%. A sensibilidade exclusiva para nÃdulo e espessamento (massa) foi de 52,07% com IC 95% de 43, 17%-60,87%. A especificidade para qualquer achado foi de 71,64% com IC 95% de 60,00%Â81,44%. Full textThis is a thesis about anti-paternalism – the liberal doctrine that we may not interfere with a person’s liberty for her own good. Empirical circumstances and moral values may certainly give us reason to avoid benevolent interference. Anti-paternalism as a normative doctrine should, however, be rejected.
Essay I concerns the definitions of paternalism and anti-paternalism. It is argued that only a definition of paternalism in terms of compound reason-actions can accommodate its special moral properties. Definitions in terms of actions, common in the literature, cannot. It is argued, furthermore, that in specifying the reason-actions in further detail, the notion of what is self-regarding, as opposed to other-regarding, is irrelevant, contrary to received opinion.
Essay II starts out with the definition of paternalism defended in essay I and claims that however this very general definition is specified, anti-paternalism is unreasonable and should be rejected. Anti-paternalism is the position that certain reasons – referring one way or the other to the good of a person, give no valid normative support to certain actions – some kind of interferences with the same person. Since the reasons in question are normally quite legitimate and important reasons for action, a convincing argument for anti-paternalism must explain why they are invalid in cases of interference. A closer look at the reasons and actions in question provides no basis for such an explanation.
Essay III considers a concrete case of benevolent interference – the withholding of information concerning uncertain threats to public health in the public’s best interest. Such a policy has been suggested in relation to the European Commission’s proposed new system for the Registration, Evaluation, and Authorisation of Chemicals (REACH). Information about uncertain threats to health from chemicals would allegedly spread anxiety and depression and thus do more harm than good. The avoidance of negative health effects is accepted as a legitimate and good reason for withholding of information, thus respecting the conclusion of essay II, that anti-paternalism should be rejected. Other reasons, however, tip the balance in favour of making the information available. These reasons include the net effects on knowledge, psychological effects, effects on private decisions and effects on political decisions.
