Abstract:
Objetivo: Avaliar a eficácia dos monofilamentos de Semmes-Weinstein no diagnóstico e prognóstico do pé com neuropatia diabética. Método: Estudo prospectivo em 35 pacientes diabéticos tipo II. Os pacientes foram submetidos a um protocolo contendo anamnese, levantamento das queixas, exames clínicos, estudo neurofisiológico e pesquisa da sensibilidade cutânea nos pés. Resultado: Pôde-se constatar bom grau de concordância entre os monofilamentos de Semmes-Weinstein e o estudo neurofisiológico. Os monofilamentos de Semmes-Weinstein se revelaram sensíveis para detectar pacientes com algum tipo de alteração. Discussão: constatou-se bom grau de coincidência entre os monofilamentos de Semmes-Weinstein e o estudo neurofisiológico expresso pelo coeficiente de correlação de Spearman (r= 0,677). Os monofilamentos de Semmes-Weinstein demonstraram ser eficazes na detecção de alterações da sensibilidade cutânea, observando-se que 91% dos pacientes apresentaram variações entre os graus 2 e 5. O estudo neurofisiológico detectou 69% dos pacientes na faixa de 2 a 5 graus. A maior diferença entre os dois métodos ficou evidenciada nos pacientes sem comprometimento (grau 1), demonstrando uma sensibilidade mais elevada dos monofilamentos de Semmes-Weinstein. O estudo também mostrou uma boa correlação entre o comprometimento clínico dos pacientes com os déficits neurológicos medidos pelos monofilamentos de Semmes-Weinstein. Conclusão: os monofilamentos de Semmes-Weinstein são confiáveis para diagnosticar a neuropatia diabética dos pés. Há correlação entre os achados neurofisiológicos e os critérios clínicos obtidos com os monofilamentos. Os monofilamentos de Semmes-Weinstein ajudam na avaliação do prognóstico e evolução do pé diabético e podem ser utilizados com segurança na avaliação dos pés com neuropatia periférica.

Abstract:
Neurological surgery was defined as a separate surgical specialty by Harvey Cushing and a few other surgeons, most of whom were trained and influenced by Cushing. One of these, Raphael Eustace Semmes, became the first neurosurgeon in Memphis, Tennessee, in 1912. After World War II, Semmes and his first associate, Francis Murphey, incorporated the Semmes-Murphey Clinic, which has been primarily responsible for the growth of the Department of Neurosurgery at the University of Tennessee Health Science Center in Memphis, as well as the development of select neurosurgical subspecialties in Memphis area hospitals.

Forty-nine consecutive subjects with established diabetic peripheral neuropathy were treated with monochromatic near-infrared photo energy (MIRE) to determine if there was an improvement of sensation. Loss of protective sensation characterized by Semmes-Weinstein monofilament values of 4.56 and above was present in 100% of subjects (range, 4.56 to 6.45), and 42 subjects (86%) had Semmes-Weinstein values of 5.07 or higher. The ability to discriminate between hot and cold sensation was absent (54%) or impaired (46%) in both groups prior to the initiation of MIRE treatment. On the basis of Semmes-Weinstein monofilament values, 48 subjects (98%) exhibited improved sensation after 6 treatments, and all subjects had improved sensation after 12 treatments. Therefore, MIRE may be a safe, drug-free, noninvasive treatment for the consistent and predictable improvement of sensation in diabetic patients with peripheral neuropathy of the feet. Abstract:
This transversal and comparative study investigated the use of the Semmes-Weinstein Monofilaments (SWM) and the Antineoplastic Induced Neurotoxicity Questionnaire (AINQ) to detect sensory neuropathy in persons receiving neurotoxic chemotherapy. A sample of 117 persons were divided into 02 groups: patient and control. The patient group included 87 persons who had completed at least one chemotherapy cycle with oxaliplatin or taxanes (paclitaxel or docetaxel). The control group included 30 people who had never received any antineoplastic treatment. The project had IRB approval and met the Resolution 196/96 recommendations. The data were collected using the following tools: a Sociodemographic Form, a Clinical Data Form, the ECOG Performance Scale, the AINQ, and a SWM Kit. Data were entered into the SPSS version 15.0, cleared up and analyzed using descriptive, correlational and comparative statistics, with a significance level of 5%. The mean age of the patients was 54 years (SD=11.9 years) and of the control group was 44 years (SD=12.7 years). Both groups were composed mainly by married women, with low socioeconomic and educational level. Functional performance was significantly different between the two groups (p=0,000). Breast cancer was the most frequent type of cancer in the patients, followed by colorectal and gynecologic tumors. The majority were in paclitaxel (68.9%; n=60) and oxaliplatin (28.73%; n=25) protocols. The AINQ was validated and showed good reliability, with a Cronbach alpha of 0.863. The results of the paired samples T- test confirmed good internal consistence. There were statistically significant differences (p<0.001) for all symptoms between the groups. Chemotherapy Induced Peripheral Neuropathy (CIPN) symptoms were not related to the type of the chemotherapy drug (p=0.478). In the patients group, the hand esthesiometry showed higher sensory changes frequency in all points of the ulnar, median and radial nerves (p<0.05). The patients hand sensory changes varied from the blue monofilament (0.2g) to the red x (4g). In the control group, the worst sensory change was detected by the violet monofilament (2g). Results of the feet estesiometry showed significant differences between the groups (p<0.05). Sensory changes were more severe in the patients, ranging from the violet monofilament (2g) to complete sensory loss in certain points. A comparison between the tools had Kappa values of 0.193 in the patients group, 0.204 in the control group, and of 0.320 for the total sample (p<0.05). The correlation between the AINQ and the Esthesiometer was positive and moderate (=0.357, p<0.001). The study showed that both the AINQ and the Esthesiometer can be useful tools for CIPN detection in oncology.
Estudo de corte transversal, comparativo, que investigou o uso dos Monofilamentos de Semmes-Weinstein (MSW) e do Questionário de Neurotoxidade Induzida por Antineoplásicos(QNIA) na detecção de neuropatia periférica sensorial em pessoas tratadas com quimioterápicos potencialmente neurotóxicos. A amostra foi composta por 117 indivíduos, subdividos em 02 grupos: pacientes e controles. O grupo pacientes foi composto por 87 pessoas em tratamento com oxaliplatina ou taxanes (paclitaxel ou docetaxel) e que haviam realizado no mínimo um ciclo de quimioterapia. O grupo controle foi composto por 30 pessoas jamais submetidas a qualquer tratamento antineoplásico. O projeto foi aprovado em todas as instâncias necessárias e atendeu às recomendações da Resolução 196/96. Para a coleta de dados foram utililizados os seguintes instrumentos: Formulário para dados sociodemográficos; Formulário para dados clínicos e Escala de Performance de ECOG; QNIA e Kit Estesiômetro Sorri®. Os dados foram inseridos no programa SPSS, versão 15.0, onde foram tratados e analisados, utilizando-se estatística descritiva, correlacional e comparativa, considerando-se um nível de significância de 5%. A média de idade dos pacientes foi de 54 anos (DP=11,9anos) e 44 anos para o grupo controle (DP=12,7 anos). Ambos os grupos foram compostos por mulheres casadas, com baixos níveis de escolaridade e socioeconômicos. A capacidade funcional dos grupos evidenciou significativa diferença entre eles (p=0,000). No grupo pacientes, o câncer de mama foi o mais frequente seguido pelos tumores de colon/reto e ginecológicos. Houve predomínio de protocolos envolvendo paclitaxel (68,9%; n=60) e oxaliplatina (28,73%; n=25).Na validação do QNIA, evidenciou-se boa confiabilidade do instrumento, com um alfa de Cronbach igual a 0,863. O teste de amostras pareadas evidenciou boa consistência interna. Na comparação entre os grupos foram encontradas diferenças estatisticamente significativas entre eles (p<0,001), para todos os sintomas avaliados. Os sintomas de NPIQ (Neuropatia Periférica Induzida por Quimioterapia) não estiveram relacionados ao tipo de quimioterápico utilizado (p=0,478). Os resultados da estesiometria nas mãos demonstraram que em todos os pontos a frequência de alterações de sensibilidade foi superior nos pacientes do que nos controles (p<0,05). Nas mãos, a perda sensorial no grupo de pacientes variou desde o monofilamento azul (0,2g) até o vermelho x (4g). No grupo controle, a pior alteração de sensibilidade foi apenas até o monofilamento violeta (2g). Na estesiometria dos pés, também verificamos diferenças estatisticamente significativas entre os grupos em todos os pontos avaliados (p<0,05). Para o grupo de pacientes, a alteração de sensibilidade foi mais grave, variando de violeta (2g) até a não percepção de nenhum dos filamentos. Na comparação dos resultados obtidos entre os instrumentos, verficamos que os valores de Kappa para o grupo pacientes foi de 0,193; 0,204 para o grupo controle e para a amostra total de 0,320, todos eles com p<0,05. Apesar dos baixos valores de kappa, os resultados obtidos com o QNIA e Estesiômetro no grupo de pacientes tiveram correlação positiva e moderada( = 0,357; p<0,001). Concluiu-se que tanto o QNIA quanto o Estesiômetro podem ser instrumentos usados na prática oncológica para detecção da NPIQ.

Abstract:
La diabetes mellitus tiene una serie de complicaciones, entre las que tenemos la neuropatía diabética que predispone a la amputación de las extremidades inferiores. El presente estudio tiene la finalidad de comparar las respuestas obtenidas a través de la técnica de prueba de sensibilidad presora con monofilamento Semmes Weinstein en relación con estudio electrofisiológico en la identificación temprana de neuropatía diabética subclínica en un grupo de pacientes del Hospital Central de la Policía que acudieron desde enero del 2000 hasta mayo del 2000. Se solicitaron los permisos correspondientes tanto a los servicios como a los pacientes antes de iniciar el estudio, se acudió puntualmente a las consultas para captar el número más real posible de pacientes teniendo en cuenta las características de los mismos. Además, se elaboró una lista de asignación de recursos (tanto humanos como materiales), un presupuesto y un cronograma de actividades. Durante el periódo de estudio que comprendió los meses de enero a mayo del 2000 se ingresaron 59 pacientes al estudio ya que cumplían los criterios de inclusión, 4 pacientes no concluyeron el estudio ya que faltaron a sus citas para los exámenes respectivos. El mayor porcentaje de pacientes se encuentra entre los 46 y 50 años. El estudio concluye que el método del monofilamento Semmens Weinstein es una prueba que guarda una relación significativa con respecto a la velocidad de conducción sensitiva, se obtuvo respuesta anormal al monofilamento en todos los pacientes con velocidad de conducción sensitiva alterada. Sin embargo, se deben incluir dentro de los pacientes de riesgo para neuropatía a los pacientes con diabetes subclínica, es decir a los que tienen antecedentes familiares y han tenido un examen de glucosa en más de 110mg/dl.

Die altpreussische Sprache ist sehr wichtig für die Baltistik und Indogermanistik. Die spärlichen Sprachdenkmäler und die reichen Angaben des altpreussischen Namengutes sind eine wichtige Quelle der lexikalischen Forschungen. Die Forschungen der Eigennamen, die eine beständige Schicht der Sprache bilden, geben wichtige Auskunft über die Eigenschaften der toten Sprache, über ihre Phonetik, Lexik und Morphologie, die in Eigennamen fixiert sind, aber in den Apellativa nicht mehr vorkommen. In der vorliegenden Magisterarbeit werden 567 samländische Personenamen, aufgeschrieben im 13.–16. Jahrhundert, analysiert. Man kann über die eingliedrige Personenbezeichnung sprechen. Es ist zu bemerken, dass man die Anfänge der zweigliedrigen Personenbezeichnung feststellen kann. Das Ziel der vorliegenden Arbeit ist die samländischen Personennamen, die in der Studie von R. Trautmann Die altpreussischen Personennamen verzeichnet sind, zu untersuchen in Hinsicht auf Bildung und Herkunft. Es werden ostbaltische Parallelen und, wenn nötig, deutsche und slawische, angeführt. Die altpreussischen Personennamen werden nach dem Alphabet beschrieben, weil die Bildung und Herkunft vieler Personennamen unklar ist. Alle vorhandenen Etymologien werden angeführt. Es wird versucht auch eigene Etymologie vorzuschlagen.

The Internet was a catalyst for refiguring the previous models of media relationships. For many, the Internet is a medium that liberates individuals from the centralised and asymmetrical power structures of traditional mass media and other social institutions in particular, the boundaries set by the nation and the state. For other people, the Internet increases the capacity for surveillance and control. This dissertation argues for a fluid conception of the operations of power and resistance on the Internet that takes into account the various discourses which play a part in determining agency and subjectivity. It examines and balances the narratives of liberation and oppression against each other: for, just as the developments in Internet technology contribute to changes in discourse, so too existing or prior discursive limits and relations of power affect Internet culture and technology. In the process of analysing the interplay of different discourses on the Internet, this dissertation takes into account transnational and national cultural flows and the insights that conceptual work on globalisation, transnationalism, and cosmopolitanism can provide. The case studies are concerned with change and centre on the use of the Internet to effect this change; they include: the Singaporean Internet, a ‘thread’ about Asian culture and Australia, the representation of oppression and the formation of Chinese diasporic collectivities, and anti capitalist networks. Through these case studies, the dissertation examines the degree to which the nation-state can regulate and affect the discourses at play on the Internet as well as the agency of participants in countering and maintaining these discourses. This dissertation also analyses activists’ use of the Internet to form transnational networks. It discusses the limitations of their work including problems with representation.

AbstractIn order to assess new and often costly treatments, the need for objective scar measurement tools has become increasingly important. A combination of subjective and objective measures should be the aim of every researcher. Quantitative assessment of scars requires devices to measure their physical and physiological properties. Physiological scar parameters are scar characteristics relevant to pathological scarring which cannot be seen with the bare eye. This also means that they can only be assessed with objective assessment tools.Skin hydration is defined as the water content of the epidermis and the dermis, and the ability of the skin to retain water is important to avoid dry appearance of the skin and prolonged inflammation in scarring. Many approaches exist to measure skin water content. One single method is often not enough to capture all the relevant information. Trans-epidermal water loss, stratum corneum water content, and dermal water content are equally important and related to each other.Scar maturation has been related to transcutaneous oxygen tension, and it is hypothesized that low levels of transcutaneous oxygen pressure in evolving scars result from low oxygen diffusibility through scar tissue.Tactile sensitivity of the skin can be measured by esthesiometers. The Semmes-Weinstein monofilament test is found to be a feasible and reliable outcome measure to evaluate touch perception threshold in older upper extremities burn scars.

Abstract:
Sememes are defined as the minimum semantic units of human languages. People have manually annotated lexical sememes for words and form linguistic knowledge bases. However, manual construction is time-consuming and labor-intensive, with significant annotation inconsistency and noise. In this paper, we for the first time explore to automatically predict lexical sememes based on semantic meanings of words encoded by word embeddings. Moreover, we apply matrix factorization to learn semantic relations between sememes and words. In experiments, we take a real-world sememe knowledge base HowNet for training and evaluation, and the results reveal the effectiveness of our method for lexical sememe prediction. Our method will be of great use for annotation verification of existing noisy sememe knowledge bases and annotation suggestion of new words and phrases.

Régóta ismert tény, hogy Anatóliában találunk madzsar („magyar”) elemet tartalmazó földrajzi neveket, Madzsar vagy Madzsaroglu nevű családokat és madzsar nevű nomád nemzetségeket. Származásukról két elképzelés fogalmazódott meg: az egyik szerint a közép­ vagy a kora újkorban Magyarországról származó kitelepülők, menekültek, esetleg felszabadított rabszolgáktól erednének, a másik szerint Magyarországhoz semmi közük, hanem a nomád törökség részét képeznék. A fellelhető adatok elemzése a következő eredményekre vezetett: Anatólia számos területén találunk madzsarokat, jóval több helyen annál, hogy kései magyarországi bevándorlóktól származtathatnánk őket. Kifejezetten gyakoriak a nomádokkal is kapcsolatba hozható adatok, és elég nehéz olyan történeti modellt felvázolni, amely alapján magyar menekültek török nomád nemzetségek névadójává válhatnának. Ráadásul már a 15. századból is vannak rájuk vonatkozó adataink: ők egészen biztosan nem Magyarországról származnak. A madzsarokra vonatkozó adatok jól megfeleltethetőek a többi anatóliai nomád törzs és nemzetség adatainak. Összességében tehát nem kételkedhetünk benne, hogy valamikor Anatóliába bevándorolt egy vagy több madzsar nevet viselő nomád nemzetség. Ugyanakkor ez a szó a török nyelvekben nem jelent semmit, e név a törzsi­nemzetségi névadásnak a régies rétegéhez tartozik. A nomád népek történetének ismeretében semmi akadálya nincs annak, hogy a hajdan szétszóródott korai magyarság kései leszármazottait lássuk bennük. Ennek egyébként történeti lehetősége is van: az Anatóliába költöző oguzok éppen onnan, az Urál vidékéről is sodortak magukkal néptöredékeket (például az alajontlugokat és a besenyőket), ahol a korai magyarok is élhettek. Így elképzelhető, hogy az anatóliai madzsaroknak mégis van közük a magyarokhoz, ám ez a kapcsolat jóval régebbi, mint ahogy sokáig gondolták.

Aspect extraction relies on identifying aspects by discovering coherence among words, which is challenging when word meanings are diversified and processing on short texts. To enhance the performance on aspect extraction, leveraging lexical semantic resources is a possible solution to such challenge. In this paper, we present an unsupervised neural framework that leverages sememes to enhance lexical semantics. The overall framework is analogous to an autoenoder which reconstructs sentence representations and learns aspects by latent variables. Two models that form sentence representations are proposed by exploiting sememes via (1) a hierarchical attention; (2) a context-enhanced attention. Experiments on two real-world datasets demonstrate the validity and the effectiveness of our models, which significantly outperforms existing baselines.

This work which began on January 14 at New London, Conn. and ended on February 14 at Key West, Fla. included a week of operations with the East Coast Sound School out of Key West. The bathythermograph, an instrument for measuring the sea water temperature continuously from the surface down to 75 fathoms, was used for oceanographic observations.
