Full textThe emergence of cooperatives providing services in the field of health and social services is a rather marginal phenomenon, although their number has increased over the last decades. The SABSA Multi-Stakeholder Cooperative in Quebec City’s Lower Town has been created by nurses and social workers whose aim was to facilitate access to adapted primary care for a population unreached by conventional structures. The objective of this study is to understand the context which has led to the foundation of the SABSA Multi-Stakeholder Cooperative. In order to achieve this objective, a qualitative single case study with embedded units of analysis adopting a methodological perspective inspired by an epistemology of the particular was conducted. Interviews with founding members (n=5) as well as data from interview transcripts conducted with these same individuals for a previous study were used. The results of this research provide details regarding the socio-economic, political, institutional and individual contexts that are liable to contribute to the understanding of the emergence of this model in the province of Quebec. In addition, the results suggest that SABSA's founding members aim to use the cooperative model as an innovation tool with the intention to offer services that were previously missing, inadequate, incomplete or fragmented, in the absence of any possibility to improve the services offered by the current structures. Moreover, the founding members of SABSA use the cooperative model to involve all those concerned in the decision-making process relating to services offered and their organization. Ultimately, the emergence of the SABSA Multi-Stakeholder Cooperative highlights the need and the challenge of organizing dynamic spaces conducive to innovation and collective participation to governance for current health care systems. Full textThis qualitative study aims to extend the knowledge about social work in third line (i.e., hospital-based and specialized) psychiatric services. The outlook of social workers is central in our study as we asked these professionals about the meaning of their duties at work. Interviews were conducted with mental health social workers from the Mental Health Institute of Quebec City. From April to June 2015, 10 social workers were interviewed. They answered a semi-structured questionnaire divided in two sections: (1) social work practice in third line psychiatry; and (2) a practice narrative centered on an actual clinical example. Thematic content analysis based on a codification table with predetermined and emerging categories was conducted. Results and discussion: The central theme of our analysis pertains to the notion of linking, which is viewed as central to social work practices: ties created, preserved, or re-established by social workers, and involving users, families, other practitioners, and other various partners. Social workers enact various roles in third line psychiatric settings. Significantly, the way their role is understood in interdisciplinary teams influences the continuity of the collaboration itself, as well as the achievement of the goals established with clients. The case-centered practice narratives underline that clinical management in third line psychiatric settings need interventions from different collaborators and demand the use of new intervention approaches. Constructive and effective collaborations along with the addition of families in complex interventions provide great support in the realization of the users’ intervention plan or discharge plan. For social workers, the diverse forms of clinical supports, training accessibility and work flexibility facilitate their tasks. Keywords: social work, hospital practice, mental health, psychiatry.

Full textThe origins, experiences and backgrounds of newcomers to the province of Quebec have been increasingly diverse since the 1960s. The challenges of this new reality has led to the use of intervention approaches in an intercultural context that favour the development of a greater cultural sensitivity and the appropriation of strategies to overcome obstacles to understanding and collaborating between social stakeholders and those assisted. However, these approaches do little or do not address the religious or spiritual aspect, which can sometimes be a major part of the newcomer’s identity. While Canadian policies emphasize the need to consider the religious dimension, the current dominant view of this reality is yet to marginalize this aspect of the identity of newcomers. This characterizes the reality of the Muslim population who is held in fear because of public prejudice. Considering the previous findings and given the limited data available on this subject in the city of Quebec, this qualitative research seeks to explore and obtain a better understanding of the perspectives of eight Muslim immigrants regarding whether or not their religious identity is taken into consideration during services they receive or have received in the past by social workers. Throughout the use of the intercultural systemic approach, the data collected during the interviews made it possible to highlight diverse experiences regarding the way in which the religious is received in context of the aid relationship. It also made it possible to describe the social and political context in which this project is situated and the impact that this context has on the daily experience of the respondents. Full textIn 2003, public health authorities of the Province of Quebec, Canada, published their first national public health program in which a strategy known as the “Support for the development of communities” was included. In order to enrich the knowledge on the implementation of the strategy and its influence on the practice of the professionals mandated to update same, community organizers from the local Health and Social Services Centers, this thesis had three main objectives which were investigated through three different research processes. The first part had for objective to study a community development process. An ethnographic study revealed the complexity of the strategy, underlined the role of the socio-cultural and historic factors and targeted elements to be considered for practice. It also highlighted the importance to adopt a health equity perspective. The second part aimed at describing how the strategy influenced community organizing practices. The analysis of a reflexive practice group, composed of community organizers, helped identify key challenges related to the strategy: the need to clarify the role of community organizers in community development, the difficulties to stimulate and support citizen participation and finally the difficulties associated with this function within the organizational context of Health and Social Services Centres. Finally, the third part sought to identify favourable and unfavourable elements to the integration of the strategy in the practice of community organizers and the organizations that employ them. In addition to the interviews realised in the ethnographic study and the reflexive practice group, nine semi-structured interviews with key actors were realised. The participants indicated that the strategy brings a paradigm shift in favour of health equity, which cannot be achieved without important changes to professional and organizational practices. This thesis has thus contributed to provide new knowledge on the development of communities for the field of public health and the results indicate that it still has to be refined and that its implementation could be improved. In this regard, a heuristic model to generate a conversation between community organizers and public health practitioners, is offered in conclusion.

Full textThis research focuses on the use of health and social services by the Aboriginal peoples living in urban areas. Studies on the subject show that there are social inequalities in this population, which results, among other things, in a difficulty in accessing these services. The lack of culturally appropriate services and the presence of discrimination among providers are the two main factors raised by the scientific literature about this issue. In a culturalist approach, the cultural peculiarities of First Nations, Métis and Inuit are put forward to explain it. This study offers a different take on the issue by examining the experience of First Nations people in terms of their relationships with professionals in the health and social services. In what ways do they feel perceived by them? What are the impacts of these impressions on their experience and use of services? The results show that First Nations people experience different experiences, ranging from satisfaction to loss of confidence in the services received. However, the negative elements spontaneously raised by the participants are more often organizational than relational. The First Nations people who participated in this thesis are looking for empathic professionals and egalitarian relationships.

Full textObjectives: This master’s thesis explores the learning gained from an internship experience within an "alternative" model of organization of front-line services and its potential application in subsequent medical practice. Method: The exploratory qualitative study is based on twelve interviews with practicing doctors who completed an internship at La Maison Bleue, our research environment; and three interviews conducted with key informants from La Maison Bleue, and from the health and social services network and academic institutions, involved in the management of internships. Results: The main lessons learned from the internship experience are related to: (1) taking into account people in their globality and in their context; (2) a reflexive perspective favorable to the deconstruction of prejudices and the questioning of norms, leading to a less stigmatizing approach; (3) the richness and scope of enhanced practice from an interdisciplinary perspective; (4) the definition of professional identity. Organizational barriers to the application of learning, based on the philosophy of the health and social services system and characteristics specific to the practice environment, have been identified. Facilitating factors have, for their part, been more identified on the human level but seem to have, ultimately, only a marginal effect on the real capacity of physicians to implement learning into their practice. Conclusions: This study highlighted the presence, within the Quebec health and social services system, of primary care physicians who express a desire to practice medicine differently in order to meet the needs and expectations of populations in vulnerable situations. The results of our study suggest the need for organizational awareness of the barriers that these physicians experience in implementing a practice in accordance with their values, ideals and identities.

Full textThe results of the correlation analysis revealed a positive relationship between public spending and volunteer rates. In addition, a positive correlation was found to exist between levels of coordination activity and rates of volunteerism, lending strength to the contention that organizational infrastructure is related to volunteer rates. In general, the results highlight the integral role of public monies to the sustainability of Quebec's voluntary sector.

Full textMethod. This study consists of two methods: a document review of relevant policies and reforms of Quebec's health and social services. Secondly, qualitative interview methods were used to explore the experiences of seven managers within a CSSS and their attitudes towards the reform.
Results. Managers described the following: a new window of opportunity, importance of leadership in the reform process, distorted communication, environment of trust, and culture of the organisation.
Conclusion. Quebec's health and social services is an ongoing discourse. Distorted communication creates an atmosphere of uncertainty which hinders the reform process. Implications include the need for further research in understanding healthcare professionals and other actors in the reform process.

Full text355091\u Cette étude fait état de la situation des gestionnaires intermédiaires du système de santé et de services sociaux québécois à la suite de l’adoption, en 2015, de la Loi modifiant l’organisation et la gouvernance du réseau de la santé et des services sociaux notamment par l’abolition des agences régionales (L.Q., 2015, c. 1.). Cette loi prévoit, l’abolition de plusieurs postes de gestion, la fusion de divers établissements du réseau ainsi que des changements majeurs dans les conditions de réalisation du travail des gestionnaires intermédiaires. Il s’agit donc de déterminer quel est l’impact de ces changements sur la qualité de vie au travail des gestionnaires intermédiaires et quels sont leurs désirs en matière de représentation collective. Plus précisément, cela consiste à déterminer si les changements ont engendré une dégradation de leur qualité de vie au travail et si cette dégradation a révélé un désirde représentation plus grand. Nous avons réalisé que les gestionnaires intermédiaires vivent une réelle dégradation de leur qualité de vie au travail. Nous arrivons également à la conclusion que les gestionnaires intermédiaires éprouvent un désir de représentation plus fort que la simple consultation sans remettre en cause la structure actuelle puisqu’ils ne souhaiteraient pas une syndicalisation conventionnelle et surtout un grand besoin de reconnaissance de la part de leur employeur.

Full textThe Saint-Roch district of Quebec City is marked by a process of revitalization, a societal mix,and diverse social challenges. This qualitative research aims to explore and understand a local intervention which is deployed there and aims to improve cohabitation between the various social groups. This is the job of the agent.e de mixité. This thesis aims to answer the following research question : How is the function of the agent.e de mixité negotiated at the intersection of the perceptions of social actors concerned by the challenges brought about by the context of revitalization of the Saint-Roch district and their expectations for this function? The theoretical framework of the research is based on constructivism and is more precisely inscribed in symbolic interactionism. Data collection was carried out with seven participants from five social groups: citizens, citizens living in poverty, community workers, traders, and workers in an institution. The results allow us to understand their perceptions of the challenges present in the context of revitalization and social mix of the district such as cohabitation, gentrification, marginalization, and judiciarisation. It also allows understanding of their expectations regarding the function, position, and skills of the agent.e de mixité. Subsequently, the results of these individual interviews were presented to a group of 12 people involved with the Projet Vie de parvis. The discussions during this meeting shed light on the dimensions of the function of the agent.e de mixité which are negotiated at the intersection of these different expectations and perceptions such as temporality, outlines, scope, and extent of function. All content has been compiled and analyzed using Dedoose.

Full textThrough sustained efforts to establish a professional role and to develop its own psychosocial and ecological perspectives, meaningful contributions from social work are likely to improve the quality of life of people with Alzheimer's disease and related conditions and their families. However, the role of the social worker in the interprofessional teams is not sufficiently clarified and this seems to cause confusion among the clientele and other professionals. This qualitative study, which is exploratory and descriptive in nature, aims to better understand social workers' perception of their roles in people with Alzheimer's disease or a related condition and the challenges of these roles in interprofessional collaboration. The analysis of the semi-structured individual interviews with 10 participants demonstrated that the definition and contribution of social workers within interprofessional teams remains to be clarified. It was possible to document the role of social workers and to identify three role issues: ambiguity, conflict and role overload, despite a limited number of participants.

Full textCONTEXT. Neglect is the most common form of maltreatment in children under 6 years of age. Given the lack of warm, well-adjusted parent-child interactions that characterizes it, this context of adversity greatly compromises their development, particularly language. Their language difficulties, found in research in an English-speaking context, are manifested mainly in the morphosyntactic component of language. However, the morpshosyntactic development of French-speaking neglected children remains unknown, hence the interest of conducting work in this language. In addition to deepening our understanding of the challenges of developing the morphosyntactic component, it is essential to focus on the educational contexts in which neglected children evolve and that are likely to have a positive influence on their language skills, particularly early childhood educations (ECE) settings. More specifically, the quality of the interactions in the group is the component of ECE that is most strongly associated with language. AIMS. The thesis aims to 1- describe the level of morphosyntactic development of neglected children aged 4 years and compare it to that of non-neglected peers; 2- to quantify the prevalence of morphosyntactic difficulties (MD) in neglected children, approximated by the mean length of utterances in morphemes (MLUm); and 3- to study the relationship between the quality of interactions in the group in ECE settings and the presence of MD in these children. METHODS. A sample of 136 children, including 44 neglected children, was recruted. A spontaneous language sample was collected using semi-structured standardized play to measure the participants' level of morphosyntactic development. The analysis was performed using the Systematic Analysis of Language Transcripts software. A sub-sample of 18 neglected children who attended an ECE setting was then created. The quality of interactions was estimated by the Classroom Assessment Scoring System Pre-K. RESULTS. Neglected children have a level of development of MLUm up to seven times lower than their non-neglected peers. Their MDs also translate into less frequent verbal inflections and more frequent word omissions. The results suggest trends between four dimensions of the quality of interactions in the group and the MLUm of neglected children with MDs, two positive and two negative. CONCLUSION. The results confirm for the first time the great MDs of French-speaking neglected children. The innovative aspect of the thesis is also reflected in the demonstration of a relationship between the quality of interactions in the ECE setting and the morphosyntactic skills of neglected children with MDs. The findings from this study illustrate, particularly through the high prevalence of MD, the high need for individual services in speech-language therapy for neglected children. The results also argue for support to maintain and improve the level of quality of interactions in the ECEs attended by neglected children.

Full textWithin a context where weight-related problems continually increase, a major public health concern consists of limiting its associated consequences. Amongst some actions put in place towards this goal, the Québec government supports a widespread dissemination of the program called “Choisir de Maigrir?” (CdeM?). Since a program can also become inefficient or harmful if implemented inadequately, the success of such initiative depends especially on the efficiency of its implementation process. The efficiency of this process requires that the program implemented is faithful to the original. However, when faced with the specific culture as well as the type of clientele, adaptations seem to be inevitably performed when a program is transferred into different settings. This work is devoted to the process evaluation of the CdeM? program in terms of adaptations performed by the providers who present it in order to explore the way this issue (fidelity-adaptations debate) matters throughout an implementation process. As reported in the actual literature, every provider’s dyads from the project’s sample have performed adaptations during the implementation of the program. From the program instigators’ perspective, most of those adaptations might have compromised its core components (unacceptable adaptations). In that regards, further analyses could be realized in order to verify if those adaptations will have an influence on the program efficiency. All in all, documentation of adaptations that were realized within a natural context will be able to support certain decisions that pertain to the continual improvement of the program or training and assistance that are currently offer to providers. Full textThe aim of this qualitative study is to contribute to empirical knowledge about compulsive hoarding behavior for social workers, educators, psychoeducators and other professionals involved with hoarders in order to improve services offered to these people. We tried to know how users of CSSS, who received social services for compulsive hoarding behavior, perceive and assess interventions conducted by social professionals. Semi-structured interviews were conducted with six users of Centre de santé et de services sociaux of Quebec City area (CSSS Alphonse- Desjardins, Portneuf, Québec-Nord and Vieille-Capitale). They also answered the French version of the questionnaire Working Alliance Inventory – Short Version (WAI-SR). Focus group were conducted with eleven professional implicated with hoarders to complete and contextualize data. Results: Compulsive hoarding disorder is a life condition before being a psychiatric condition, and that hoarding behaviors are part of the perception that the users have of themselves, of their priorities, objectives and conditions in which they want to live. Users also have predominantly a favorable opinion about social services in CSSS, which can be linked at a good recognition of the problem. Finally, both users and social professionals perceived a lack of resources in the Quebec City area to answer the specifics needs of people with problems inherent to their compulsive hoarding behaviors. Full textIn Quebec City, gentrification is increasingly observable in the Saint-Sauveur neighbourhood. This qualitative study provides an opportunity for participants to talk about a subject that concerns them, namely gentrification, and to cross-reference various perceptions of this phenomenon. The research aims to answer the following question: what are the perceptions three groups of social actors have of gentrification in the Saint-Sauveur neighbourhood of Quebec City? The analytical framework of the research is based on critical and constructivist epistemological postures but also on the theory of symbolic interactionism. This case study was conducted through 15 semi-directed individual interviews that were conducted with three groups of social actors: recent and long-term residents, community workers and volunteers, and business owners. The content analysis was performed using NVivo 11 software. After the analysis was done, participants were gathered for a final group meeting to validate its results. The results of this study allow for a better understanding of participants’ perceptions. Their perceptions of the gentrification process are very nuanced; they describe it as a mechanism where it is important to preserve a certain balance. These perceptions have been subdivided into six categories: population, housing, physical space, culture, local businesses and services, and security. These factors influence each other and combine to present a portrait of the different aspects of gentrification. The research participants expressed their perceptions regarding the roles of different social actors within the neighbourhood, revealing the full complexity of the gentrification phenomenon. The perspectives for research, for interventions and for the future of the neighbourhood are explained in order to offer some avenues for reflection on the topic of gentrification.

Full textThis qualitative study of exploratory type allowed us to answer the research question: what are the perceptions of Cameroonian immigrant of the issues related to existing intra communal in Quebec City? The objective behind this study was the comprehension of organizational reasons in association of new Quebec citizen coming from Sub-Saharan Africa in Quebec City. The study is particularly focused on the Cameroonian immigrants in Quebec City. The analysis of the participants’ speeches in this study indicates that their idealism of immigrants, before they leave their origin’s country, is a country where they would fulfill themselves and thus they would offer better living conditions not only to them, but also to their families. This self-achievement from their idealism necessarily involves their contribution to the development and good functioning of the society which hosts them and which becomes their new home. This self-achievement made impossible or delayed by difficulties in land of immigration, the organization in association by ethnicity and or by the country of origin, is the way to live a solidarity that allows them to overcome their difficulties but also, to remember who they are, where they come from. Thus, their original cultures that have forged their identities, allows them to integrate the culture of the country of immigration.

Full textIn Quebec City, there is a low attendance of African immigrant women in the services for victims of sexual violence. To understand this phenomenon, we developed this mixed study with a constructivist perspective on social representations of sexual violence and the available assistance. Its aims were to find out how African immigrant women in Quebec City represent sexual violence and the available assistance. To answer these questions, we used three data collection instruments: the sociodemographic questionnaire, the semi-directive interview guide, and the free association technique. A total of eleven immigrant women aged between 18 and 30 years participated in this study. They came from eight francophone countries in Africa and lived in Quebec for less than ten years. The results showed that rape was the main form of sexual violence for the participants. In addition to this, they enumerated other forms of violence more specific to their environment, such as female genital cutting and forced marriage. They mainly explained this violence by social factors. Thus, according to them, in African society, the victims are more blamed than the perpetrator, whether he is known or not. In addition to that, they considered as obstacles to their denunciation: the fear of breaking up the family, if the perpetrator is a close member or the husband, as well as the trivialization of certain forms of sexual violence. Regarding their perception of the psychological assistance, participants believed that it is reserved for «white» or «crazy people» and not very useful in African society. The assistance from the judicial system is presented as ineffective due to stereotypes about victims and subjectivity in the work of the police officers. We discussed all the study results with both our literature review and analysis framework. We also suggested recommendations for both research and social work intervention.

Full textThe financial exemption program for domestic help services in Quebec aims to encourage house holds with needs for domestic help to obtain their services from social economic enterprises domestic help while promoting their development. This program mainly targets people who are losing their independence due to their age, as well as those who have special needs due to a condition of incapacity or vulnerability. Several types of aid are then offered, such as compensatory aid granted to organizations providing services, fixed aid and variable aid granted to users. To this end, investment shave been made since 2009 in this program to improve variable aid for vulnerable clients in order to allow users to afford more services. The purpose of this study is to assess the effect of a increase in variable assistance per hour of domestic help which happened in October 2016. We use this almost natural experience and data on active users 18 months before and 18 months after the increase. We use a Tobit type 1 model in double differences to evaluate the impact of this reform on users’ consumption of domestic help services. Monthly, we find that the improvement in variable assistance in 2016 had the effect of increasing the average consumption of services by 0.19 per hours (approximately 11 minutes). This figure corresponds to a monthly increase of approximately 4% in the average consumption of hours of services. In addition, we distinguish two types of effect : a contribution at the extensive margin and at the intensive margin. The results reveal a greater response at extensive margin. These results suggest that the improvement in variable assistance had a positive and significant effect on the average consumption of hours of home help services in Quebec.

Full textCette recherche vise à comprendre comment les besoins perçus chez les familles immigrantes par les travailleuses sociales des CLSC de Québec et Chaudière-Appalaches influencent l’intervention sociale auprès d’elles. Dix entretiens semi-dirigés ont été réalisés avec des travailleuses sociales. La perspective socioconstructiviste, le cadre interculturel-systémique, puis les concepts de besoins perçus et d'intervention sociale ont orienté la recherche. Les résultats montrent que des caractéristiques des immigrants, les types de besoins perçus chez eux ainsi que des caractéristiques des participantes et de leur mandat influencent la perception des besoins des immigrants et l’intervention auprès d'eux. Des liens entre les demandes de formation pour intervenir auprès de familles immigrantes et les difficultés rencontrées dans l’intervention auprès d’elles ont aussi été relevés. Des pistes de réflexion et d’action sont portées à l’attention des milieux de recherche, de pratique et de planification sociale. Full textIn the context of the internationalization of higher education in OECD countries, Canada welcomes a significant number of international students each year. Some of them have applied for permanent residence after graduation. They are based on Canada's immigration policies that facilitate the transition of international students to permanent residence. The results of this study show that the majority of participants did not intend to immigrate to Canada, but the immigration project has profiled during their studies due to several factors. Other participants put forward their study project in order to carry out their permanent immigration project. In this context, this exploratory study aims to understand the perception of former international students who become immigrants with respect to their career path. Analysis of the interviews shows that the majority of participants expressed a sense of satisfaction with their professional integration. An analysis of the speeches of some participants shows that this satisfaction cannot be explained by the quality of the jobs held. They have put forward the passion that drives them in their work even if they do not have jobs that match their skills. However, it is revealed that in this study other participants are very dissatisfied with their professional integration. Regarding the factors that contribute to their integration, all participants agreed that having a network of contacts in professional environments is a key element. For the factors that hinder professional integration, participants in this research from minority groups mention that discrimination and prejudice are challenges.

Full textThe data for this qualitative study were obtained through indepth interviews with the program staff and through an examination of agency files. The findings suggest that service is limited to those families who are assessed to be motivated--that is compliant with the objectives of the program and accepting of the intensive nature of the service. The interventions focus on individual parenting, most often the mother's parenting. For the workers, the dual role of support and scrutiny is managed within a relationship of trust.
The findings further indicate that families who are experiencing severe problems, often related to alcoholism, family violence and extreme poverty are not served by this program.

Full textHundreds of people with a severe mental health disorder live in intermediate and family-type resources in the Quebec region. The main purpose of these structures is to integrate vulnerable people into the community. The aim of this research is to determine the impact of intermediate and family-type resources owners’ as well as their employees’ interventions on the community integration of their residents. This study uses Segal and Aviram’s (1978) community integration model as a conceptual framework. Their model proposes to divide the community integration into two concepts: internal and external integration. Internal integration refers to the evolution of residents in the housing environment, whereas external integration concerns the engagement of residents outside the residence. Semi-structured interviews were conducted with residents, owners, and employees of intermediate and family-type resources in the Quebec region. The sample was comprised of 12 actors involved in the field of mental health housing resources including five residents, four owners, and three employees. The data collected were analyzed using a thematic content analysis. The main interventions carried out by owners and employees to ensure the residents’ community integration are grouped into six categories: the establishment of rules, group activities, management of the consumption of goods and services, the formation of a residents’ committee, support for family relationships, and the sustainment of activities independently carried out by residents. It was shown that owners’ and employees’ interventions focus on two objectives: developing residents’ autonomy and ensuring their protection. Owners’ and employees’ authority status is used as one of the main tools to intervene with their customers. Results from this research confirm, but also contradict, previous findings regarding the tension between residents’ need for protection and autonomy, owners’ and employees’ authority status, and the impact of these individuals on the community integration of residents.
