Full textThe number of connected devices and present in the lives of users is currently expressive and performing activities using multiple devices is already a reality. However, the integrated and synergistic use of these resources, which is called distributed interaction, has found obstacles in the limitations of current systems to provide and attend user interactions that cross the boundaries of the device. A variety of platforms, also involved research areas are contrary forces to the distributed interaction too. The aim of this research is therefore to define an architecture that facilitates the building of applications observing aspects of a distributed interaction, where user and device does not only bind in the interaction model 1 user:1 device. A service-oriented architecture has been defined, after exploratory and systematic research about frameworks and middlewares that promote integration among devices, and its feasibility has been verified by proofs of concept. As a result, we have obtained a pattern of transitions necessary for the construction of various usage scenarios and a P2P event-driven service-oriented architecture that allows implementation and execution of these scenarios.

Full textAdverse events (AEs), defined as unintended injuries caused by medical care, are recognized as a major health problem. Although most of them lead to minimal impairments, a considerable proportion is related to patients death. Urgent care is considered an important AE risk factor. No related Brazilian studies were published so far. The present study aimed to identify the occurrence of AEs in patients admitted for stroke to the medical emergency department of the Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP) and to determine the AEs categories associated to death. This paired case-control study enrolled 468 patients admitted for stroke to the HCFMUSP medical emergency department from March 1996 to September 1999. The cases comprised 234 consecutive deaths and the controls 234 discharged patients, matched for primary diagnosis and admission period. AEs, detected by retrospective chart review, were classified with regard to their severity, immediate causes, affected systems and professional involved in patient care. The association with death was analyzed by multivariate conditional regression including variables related to demographic aspects, clinical severity on admission and care characteristics. A total of 1,218 AEs were identified in 468 patients: 932 AEs (76.5%) in 170 cases and 286 AEs (23.5%) in 125 controls. Major AEs corresponded to 54.1% of all AEs, with 659 episodes: 538 events in 143 cases and 121 in 65 controls. Diagnostic and therapeutic procedures and nursing activities accounted together for 55.2% of all events. Concerning the affected system, 46.0% of the identified AEs lead to general manifestations. Nursing and medical AEs represented the most frequent professional categories involved (38.4% and 31.0% of all events). A significant association with death was found regarding major AEs, medical AEs and nosocomial infections, with adjusted OR estimates of 3.72 (95% IC = 1.63-8.48), 3.69 (95% IC = 1.60-8.50) and 3.20 (95% IC = 1.20-8.51), respectively. In summary, adverse events, most of them severe, were frequent in cases and controls, leading mainly to general manifestations. Diagnostic and therapeutic procedures and nursing activities corresponded to the main AEs immediate causes. Regarding the professional involved, AEs related to nurses and physicians predominated. Major AEs, medical AEs and nosocomial infectious were significantly associated to death in stroke patients admitted to the medical emergency department of the Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo

Resumo: A segurança na utilização de medicamentos tornou-se um tema altamente discutido nas políticas de saúde, em parte devido a pesquisas que demonstram a ocorrência de importantes eventos adversos aos medicamentos (EAM) causados por fármacos comumente utilizados. Poucos estudos em pediatria quantificam os EAM, o que demonstra a necessidade da condução de mais estudos com esta faixa etária. O objetivo deste trabalho foi realizar uma análise prospectiva sobre a incidência de EAM e estabelecer o perfil de adesão à farmacoterapia de uso contínuo em pacientes pediátricos admitidos em uma unidade de emergência. Trata-se, portanto, de um estudo transversal, com duração de um ano (julho/2011 a junho/2012), conduzido na Unidade de Emergência Referenciada Pediátrica do Hospital de Clínicas da (HC) da Universidade Estadual de Campinas (UNICAMP). Os EAM foram identificados, quantificados e caracterizados em 7 categorias (reação adversa ao medicamento; não adesão; inefetividade terapêutica; uso inadequado do medicamento; interação medicamentosa; intoxicação e queixa técnica) e classificados segundo a gravidade obedecendo aos critérios do CTCAE (Common Terminology Criteria for Adverse Events). As reações adversas aos medicamentos foram classificadas pela causalidade, segundo Algoritmo de Naranjo. Para os pacientes em tratamento medicamentoso em regime contínuo, foi avaliado o grau de adesão à terapia proposta utilizando o Teste de Morisky-Green. No período de estudo foram admitidos na unidade de emergência 20.441 pacientes e 1723 (8,4%) foram atendidos pela farmacêutica responsável pelo projeto. Destes, 199 (11,5%) foram admitidos em decorrência de pelo menos um EAM e fazem parte da "População EAM" e 226 (13,2%) relataram o uso de pelo menos um medicamento em regime contínuo e por isso foram incluídos na "População Adesão". O evento de maior frequência foi uso inadequado do medicamento, identificado em 39,3% dos casos. O principal medicamento envolvido nos casos de reação adversa ao medicamento, não adesão e uso inadequado foi a Amoxicilina (26,0%). Nos casos em que mais de um medicamento estava envolvido em um ou mais eventos, notou-se que a maioria dos medicamentos causadores dos EAM provinha de medicamentos prescritos (85,8%). Foi observada maior ocorrência de EAM de gravidade leve (62,9%) e, ao estudo da causalidade das reações adversas, apenas 1 caso foi classificado como definido (2,4%). Quanto às características do perfil de adesão à farmacoterapia prescrita, houve predomínio de alta adesão (62,2%) principalmente entre crianças de 0 a 9 anos. Este trabalho demonstra uma alta incidência de EAM relacionados à admissão de pacientes pediátricos em unidade de emergência. A carência de informações e de estudos publicados na área demonstra a necessidade de estímulo a pesquisas nos grandes pólos de saúde no Brasil os quais poderão demonstrar o real problema de eventos adversos aos medicamentos na pediatria, podendo com isso estabelecer propostas de ações mais efetivas para o uso racional de medicamentos nesta faixa etária o que ressalta a importância da atuação do farmacêutico clínico através da atenção farmacêutica, para orientar e intervir no uso correto dos medicamentos
Abstract: The safety drug utilization has became a highly visible topic in health politics, due in part to research suggesting that there are important adverse drug events (ADE) caused by commonly used medications. Few pediatric studies quantify the ADE; this fact demonstrates the necessity of conducting further research with this age group. The objective of this study was to realize a prospective analysis on the incidence of ADE and to establish the profile of adherence to pharmacotherapy of continuous use in pediatric patients admitted to an emergency unit. This study was transversal with lasting one year (July/2011 to June/2012), conducted at the Emergency Pediatric Unit, Hospital de Clinicas (HC), State University of Campinas (UNICAMP). The ADE were identified, quantified and characterized in seven categories: (adverse drug reaction, non-compliance, and treatment ineffectiveness, inappropriate use of medication, drug interactions, toxicity, and technical defect) and classified by severity according to the criteria of the CTCAE (Common Terminology Criteria for Adverse Events). The adverse drug reactions were classified by causality, second to Naranjo algorithm, and type (organ involved). For patients under treatment in continuous, the degree of adherence to therapy proposal was evaluated using the Morisky-Green test. During the study period 20,441 patients were admitted and 1,723 (8.4%) were attended by the pharmaceutical responsible for the project. Of these, 199 (11.5%) were admitted with at least one ADE and form part of the "Population ADE"; and 226 (13.2%) were evaluated because of administering at least one drug in continuous and so were included in the "Population Adherence". The most frequent event was inappropriate use of the drug, identified in 39.3% of cases. The main drug involved in cases of adverse reaction to medication, non-compliance and inappropriate use was Amoxicillin (26.0%). In cases where more than one drug was involved in one or more events, it was noted that most of the drugs that cause ADE came from prescription drugs (85.8%). There was higher occurrence of ADE mild severity (62.9%), and the study of causality of adverse events, only 1 case was classified as definite (2.4%). Regarding the characteristics of the profile of adherence to pharmacotherapy prescribed, were higher in adherence (62.2%) mainly children aged 0 to 9 years. This work demonstrates a high incidence of ADE related to admission of pediatric patients in emergency unit. The lack of information and studies published in the area demonstrates the need to stimulate research in major health centers in Brazil which will demonstrate the real problem of adverse drug events in pediatric, and this may establish proposals for more effective actions for rational drug use in this age group which emphasizes the importance of the role of the clinical pharmacist through pharmaceutical care to guide and intervene in the correct use of medicines

Full textA Arquitetura orientada a serviço (SOA) é um estilo arquitetural para estruturar sistemas de software de modo que exista um baixo grau de acoplamento entre as aplicações e essas possam ser facilmente integradas de forma dinâmica. A incorporação de SOA e serviços Web em sistemas que modelam processos de negócios grandes e complexos contribui para a necessidade de testes mais formais e sistemáticos. Além disso, características próprias dessa nova classe de software fazem com que técnicas de teste tradicionais não possam ser diretamente aplicadas. O teste baseado em modelo (TBM) apresenta-se como uma abordagem promissora que busca a resolução desses problemas. Esta tese investiga como duas técnicas de modelagem, Máquina de Estados Finitos (MEF) e Grafo de Sequência de Eventos (GSE), podem ser utilizadas para apoiar o TBM de aplicações orientadas a serviço. Essas técnicas modelam diferentes aspectos e podem ser aplicadas de forma complementar. Inicialmente, é definido um processo de TBM para aplicações orientadas a serviço que emprega MEFs. Com base na experiência adquirida, é proposta uma abordagem baseada em modelo para o teste de serviços compostos usando GSEs. Essa abordagem é holística uma vez que conjuntos de teste são gerados para cobrir tanto situações desejadas (teste positivo) quanto comportamentos inesperados (teste negativo). Três estudos experimentais avaliam a abordagem proposta: (i) um estudo de caso, (ii) uma análise de custo e (ii) um estudo na indústria. Ferramentas de teste também são apresentadas para apoiar o uso prático da abordagem proposta

Full textFeatures of software exposed as Web services are becoming more common and their forms of composition and coordination are increasingly essential. Orchestration and choreography, traditional approaches for Web service compositions, are provided by tools that manage business processes with different approaches. Despite the success of these approaches, there are still challenges to be overcome such as the difficulty of maintaining flows in existing control, the communication cost associated with Web service interactions, knowledge of the business process by the services and even their compatibility in service compositions. As an alternative to traditional approaches, this paper proposes the use of WED-flow approach for Web services composition, so that the execution of business processes is driven by changes in data states. In our approach, the control flow is not a requirement but a consequence of the Web service execution, which provides greater flexibility for the development and maintenance of applications. More specifically, the first contribution of this work is to propose and evaluate possible scenarios of orchestration and choreography according to predefined criteria. The second contribution of this work is the implementation of WED-flow approach for Web service compositions, as well as its validation in the choreography and orchestration scenarios.

Full textA progressiva introdução das Tecnologias da Informação e Comunicação na Administração Pública (AP) provocou uma grande evolução na prestação de serviços ao cidadão. Transitou-se de um paradigma de prestação de serviços baseado nas competências de cada instituição, resultado da organização da AP em silos, para um paradigma de prestação de serviços integrados, que pode envolver a participação de serviços de várias instituições, que para o efeito trocam informação entre si, porventura sem que o cidadão disso se aperceba. Um dos objetivos da integração de serviços é a prestação de serviços que visam satisfazer situações do dia-a-dia do cidadão que implicam a interação com serviços da AP, i.e., serviços orientados a eventos da vida (serviços OEV). No entanto, apesar da bondade do objetivo, a integração de serviços é complexa e tem o potencial para criar situações desfavoráveis para o cidadão, nomeadamente para sua privacidade. Com efeito, o cidadão deixa de ter o controlo sobre a difusão da sua informação pelas várias instituições, uma vez que são estas que comunicam entre si para obter a informação necessária para a prestação dos respetivos serviços. Nesta tese propomos um modelo de prestação de serviços OEV, o modelo CHAPAS, que pretende: (i) desincentivar a comunicação direta entre instituições para a obtenção de informação do cidadão, (ii) colocar o cidadão no controlo da disseminação da sua informação pelas várias instituições e (iii) fomentar a minimização da informação que o cidadão tem de fornecer às várias instituições para obter os serviços que pretende. Para cumprir esses objetivos, transferimos para o cidadão a responsabilidade pela obtenção de todos os serviços que compõem um serviço OEV, e dotámos o cidadão de uma aplicação, o Chappie, que lhe permite: (i) compor o serviço OEV que pretende obter, (ii) verificar que informação tem de fornecer para obter cada um dos serviços que compõem o serviço OEV e, caso o cidadão assim o decida, (iii) proceder à obtenção desses vários serviços. Como o cidadão pode fornecer a cada instituição toda a informação necessária para que esta lhe preste o serviço pretendido, mesmo que tenha de os obter previamente de outras instituições, estas deixam de ter necessidade de comunicar entre si para obter a informação que necessitam para a prestação dos respetivos serviços, o que permite limitar a difusão de informação do cidadão e dessa forma proteger a sua privacidade. Para a avaliação do modelo usámos o evento da vida de compra de casa, que envolve interações do cidadão com serviços de várias instituições da AP e particulares e cujas características nos permitem explorar as várias vertentes do modelo. Com base nele, desenvolvemos um cenário de exploração e protótipos do Chappie e dos vários serviços, que nos permitiram concluir da viabilidade do modelo CHAPAS, com algumas vantagens e com algumas limitações, para ser uma alternativa viável para a prestação de serviços de governo eletrónico ao cidadão.
The introduction of Information and Communication Technologies in the Public Administration (PA) gave rise to a huge evolution in the provisioning of public services to the citizen. It evolved from a service provisioning paradigm based in the competences of each PA department, which directly results from the PA siloed organization, into an integrated service provisioning paradigm, that may involve multiple PA departments that exchange information with each other, possibly without the citizen being aware of those exchanges. One of the reasons for this service integration is the provisioning of better services targeted to the satisfaction of citizens’ everyday situations that require interaction with many PA services, i.e., life-event services. Despite the goodness in the goal of improving the citizen interaction with PA, the integration of PA services is complex and may create adverse situations for the citizens, namely regarding their privacy. The citizen loses the control over the dissemination of his personal information throughout the many PA departments, as they communicate with each other to gather the information required for service provisioning. In this thesis, we propose a model for life-event service provision, the CHAPAS model, with the following goals: (i) to discourage direct communication between PA departments to exchange citizens’ information; (ii) to place the citizen in control of the dissemination of his information throughout the many PA departments; and (iii) to promote the minimization of the disclosure of citizens’ information to PA departments when obtaining the wanted services. To fulfill these goals, we transfer the responsibility for obtaining all the partial services that composes a life event service to the citizen that is empowered with an application, the Chappie, which enables him to: (i) compose the life-event service he wants; (ii) verify the information he must disclose to obtain each and every partial service that composes the life-event service he wants; and upon a citizen decision, (iii) obtain all those partial services. As the citizen is able to supply all the information that a PA department needs to provide the service the citizen wants, departments no longer need to communicate with each other to gather the information they need for service provisioning. This enhances the protection of citizens’ privacy as we avoid the dissemination of citizens’ information without his control. For CHAPAS model validation, we used the Buying a Home life event, which requires citizen interactions with services from several PA departments and private institutions, and whose characteristics allows for a full exploration of the model. We developed an exploitation scenario and prototypes for the citizen Chappie and for the several services from which we concluded that the CHAPAS model, with some advantages and some disadvantages, might be a viable alternative for the provisioning of e-government services to citizens.

Full textPatient identification has been acknowledged as a crucial an essential component in patients\' safety and the absence of a standard identification process in healthcare services can contribute to failures. This study aimed to investigate the theme around patient identification in healthcare services. To reach such objectives, research was conducted in three phases: elaboration of an integrative review and the conduction of two descriptive studies. Search for primary studies to be included in the integrative review was done on the following databases: PubMed, CINHAL, LILACS, SciELO, Scopus and Embase. The integrative review\'s sample concerned of 17 primary studies in six categories: identification in the healthcare teams\' and patients\' (or the patients\' tutors\') perspective (eis); identification in newborn and pediatric patients; identification in administration of chemotherapy and medication; identification in blood transfusion and sample collecting; identification in surgical patients and implementation of patient identification policies. The identification wristband strategy in an individualized manner with regards to both the patients\' and the institutions\' needs has been acclaimed by authors. Phase 2\'s descriptive study was conducted in 17 healthcare institutions, 12 hospitals and 5 emergency care units, selected with participation of 17 tech-nurses in charge. For data collection, an instrument that underwent apparent and content validation by five judges was created. Nurses answered about strategies used by healthcare services for patient identification. Every hospital reported having identification protocols; and while wristbands usage stands out in hospitals, no independent emergency care unit uses that strategy. Patient admission represents the stage during which wristbands are placed. In 75% of the researched institutions, lack of standardization in wristband use was pointed out as an issue as well as adoption of non-standardized methods; variety in color for the sake of categorization of patients sticks out in this matter. In 33% of the hospitals, more than one wristband is placed on a single patient. For identification of allergies, 50% of the hospitals use red wristbands. A patient wristband must contain a patient\'s full name, hospital record and date of birth. The identification wristband has been acknowledged as the best form of identification for patients in healthcare facilities, but apart from this method, it is important that all who are carrying out patient care check the identification, confirming the preconized identification. Phase 3\'s descriptive study was conducted in a selected hospital facility for this investigation aimed to analyze the influence of patient identification on adverse events (EA) as to causes, locale, frequency, if identification was a barrier, damage caused, consequences, and corrective and preventive measures. In the time period from August 2012 and August 2014, a total of 1673 occurrences in the institution\'s identification system was notified; among those, 38 concerned patient identification, representing 2,3% of the total. Patient identification acted as a barrier to identify a failure in an EA-related process in 45% of institutions. Protocol breach was behind 86% of the cause of EA occurrences that were connected to patient identification. Existence of protocols and procedures written alone does not guarantee failures occur, especially when it comes down to patient identification; which brings out the need for implementation of measures of evaluation that assure that these protocols are being followed by the staff. This will allow the enhancement of this process and with it a betterment in quality and safety in patient care
