Abstract:
The results of the 2018 Riskesdas show that some health problems that are still major health problems in Indonesia are the high prevalence of stunting and the prevalence of cavities or dental caries. Therefore, it is necessary to further study the relationship between the two diseases to find out their pathogenesis. In general, this study aims to determine the pattern or pattern of dental caries in stunting sufferers based on the division of sextants. While the specific goal is to determine the close relationship of risk factors for dental caries index in stunting sufferers. This type of research is a non-experimental research with cross sectional design. The study was conducted in the city of Pontianak in 2019 with the research subjects being stunting toddlers. The number of research samples is 30 children, taken randomly. Retrieval of data is done by checking and interviewing his parents. Data analysis was performed by means of analysis using descriptive analysis and Pearson correlation analysis with a significance level of α = 0.05. The results of this study indicate that the sequence of sextan in the occurrence of caries in patients with stunting in a sequence are sextant II, sextant VI, sextant, IV, sextant III, sextant I, and finally sextant V. In addition, the results of the study also showed a significant relationship between risk factors for the def-T index, namely salivary pH factors (r = -0.444, Sig = 0.013), eating habits cariogenic foods (r = -0.444, Sig = 0.013), milk type factors (r = 0.520, Sig = 0.003), toothbrush factors (r = 0.654, Sig = 0.000), and debris index factors (r = 0.447, Sig = 0.013) with a degree of significance α = 0.05. The conclusion of the study was the existence of a certain pattern of dental caries in stunting sufferers and a significant relationship between risk factors and caries index in stunting sufferers.

Abstract:
Abstract Periodontal Screening and Recording™ (PSR) is a diagnostic screening tool for the early detection of periodontal disease. The purposes of this study are to utilize PSR to estimate the periodontal health status of a representative military population and to compare the results with other studies of varying populations. When used to evaluate the periodontal health of a randomly-selected military population, PSR demonstrated the following: (1) males and females had a similiar prevalence of being designated PSR+ (having PSR Code 3 score in two or more sextants or a PSR Code 4 score in at least one sextant), (2) Blacks and Hispanics had a similiar prevalence of PSR+, and (3) both groups were twice as likely to be PSR+ as were Caucasians. Although income did not appear to be a significant predictor of PSR+, PSR+ did appear to be inversely proportional to education levels. When comparing PSR scores by sextant, the following was noted: (1) the maxillary central sextant was the most disease-free, (2) the mandibular central sextant most often presented with calculus, (3) mucogingival defects were observed more frequently in maxillary posterior sextants, and (4) the maxillary right sextant demonstrated the most destruction from periodontal disease. Objective: To evaluate whether teeth close to the cleft area present higher prevalence and severity of periodontal disease than teeth in other regions. Design: Cross-sectional. Setting: Hospital for Rehabilitation of Craniofacial Anomalies, University of São Paulo. Patients: There were 400 individuals with complete unilateral or bilateral cleft lip and palate, aged 15 to 49 years, without any previous periodontal treatment. Main Outcome Measures: All clinical parameters were evaluated in six sites for each tooth. The arithmetic means were calculated for each sextant. Results: Of the sextants, 86.75% presented means of probing depth smaller than or equal to 3 mm. No sextant exhibited means of probing depth greater than or equal to 6 mm. There was a statistically significant difference (p < .001) in probing depth according to age, types of cleft, and sextant; 95.87% of sextants presented mean attachment levels smaller than or equal to 3 mm. The sextant with cleft did not present higher means of probing depth, clinical attachment level, plaque index, and gingival index. There was gingival bleeding in 99.08% of the sample and plaque in 97.40%. The type of cleft was not an important factor influencing the prevalence of periodontal disease. Age seems to be an important factor influencing the prevalence and severity of periodontal disease for all aspects investigated. Conclusions: Periodontal disease in individuals with clefts occurred in a similar manner as observed in other populations. The presence of the cleft does not seem to increase the prevalence of the disease.

Abstract:
The aim of this cross-sectional study was to explore the eye tracking (ET) performance of postgraduate pediatric dental students in correctly detecting abnormalities in different sets of panoramic radiographs. This observational study recruited postgraduate pediatric dental students to evaluate seven panoramic radiographs. RED-m® SMI software (Sensomotoric Instruments, Teltow, Germany) was used to track the participants’ eye movements as they looked at the radiographs. The data collected for areas of interest (AOIs) included revisit counts, fixation counts, fixation times, entry times, and dwell times. Univariate and bivariate analyses were conducted to summarize the participants’ characteristics and ET measures. The overall percentage of correctly located AOIs was 71.7%. The residents had significantly more revisits and fixation counts in AOIs located in one sextant than in multiple sextants (p < 0.001). Similar patterns were observed for fixation and dwell times (p < 0.001), but not for entry time. Heatmaps showed that the highest density of fixations was on the AOIs and the residents fixated more on dentition than on bony structures. In single-sextant radiographs, residents had significantly more revisits and fixation counts for AOIs compared to those of multiple sextants. Residents had slower entry times and dwelled less on AOIs located in multiple sextant(s). The reported findings can direct dental educators to develop a standardized scan scheme of panoramic radiographs to minimize misdiagnosis.

It is still controversial which prostatic biopsy technique can provide the highest positive rate, complications, prostatic volume and PSA levels being equal. 656 consecutive patients (mean age 62.2 years) were submitted to biopsies using two similar transrectal techniques differing only in the number of biopsies: the “classic” six-sextant and a tensextant technique (adding 4 paraurethral biopsies). Every suspicious or hypoechoic area was also biopsied. Nine per cent (8/89) and 14% (6/42) of the patients subjected to 6-sextant and 10-sextant biopsies respectively, with a PSA level between 4.0-10.0 ng/ml had prostatic cancer. The positive rate increased to 24% (12/49) with sextant plus directed biopsies and to 33% (33/99) with directed biopsies only. Overall, 21% (52/279) of the patients with the above PSA level had a prostatic tumour. The 10-sextant biopsy technique does not seem to be significantly superior to the classic 6-sextant approach.

Many texts stress the need for marine sextants to be held precisely vertical at the instant that the altitude of a heavenly body is measured. Several authors lay particular emphasis on the technique of ‘rocking’ the instrument in a small arc about the horizontal axis to obtain a good sight. Nobody, to the author's knowledge, however, has attempted to quantify the errors involved, so as to compare them with other errors inherent in determining celestial position lines. This paper sets out to address these issues and to pose the question: what level of accuracy of vertical alignment can reasonably be expected during marine sextant work at sea ?When a heavenly body is brought to tangency with the visible horizon it is particularly important to ensure that the sextant is held in a truly vertical position. To this end the instrument is rocked gently about the horizontal so that the image of the body describes a small arc in the observer's field of vision. As Bruce Bauer points out, tangency with the horizon must be achieved during the process of rocking and not a second or so after rocking has been discontinued. The altitude is recorded for the instant that the body kisses the visible horizon at the lowest point of the rocking arc, as in Fig. 2. The only other visual clue as to whether the sextant is vertical is provided by the right angle made by the vertical edge of the horizon glass mirror with the horizon. There may also be some input from the observer's sense of balance and his hand orientation.

282 Background: Magnetic resonance imaging (MRI) invisible tumors are a diagnostic challenge in prostate cancer due to the lack of ability to reliably monitor these lesions radiographically or pathologically. The progression and natural history of these lesions are unknown and outcomes over time are unclear. Methods: Men with multiparametric MRI of the prostate and MRI/Transrectal ultrasound (TRUS) fusion guided biopsy were assessed for the presence of MRI invisible tumors (MIT). An MIT is defined as cancer detected only on extended sextant biopsy and not visible on MRI. All men first underwent an MRI/TRUS fusion biopsy with tracked extended sextant biopsy which originally detected the MIT. The original biopsy needle course sampling these MITs was tracked and set as a future target using the MRI/TRUS fusion platform. Men subsequently underwent a combined MRI/TRUS fusion biopsy, systematic extended sextant biopsy, and a Targeted Tracked biopsy of the MIT (TT-MIT) that was recorded and tracked from the original biopsy. We describe the outcomes of tracking these MITs and compare the ability to monitor them with TT-MIT biopsy as opposed to systematic extended sextant biopsy. Results: 105 MITs were identified, 84 (80%) of which were originally Gleason 6 tumors. The median time between biopsies was 16.6 months. The overall cancer detection rate with TT-MIT was 77.4% compared to 59.7% using systematic extended sextant biopsy. Using TT-MIT, these invisible tumors showed higher Gleason scores in 16 (15.2%) tumors. When TT-MIT was compared to the systematic extended sextant biopsy sampling the corresponding location, it showed increased Gleason scores in 30 (28.6%) MITs while 58 (55.2%) showed concordant pathology and 17 (16.2%) showed less aggressive pathology. Conclusions: The ability to follow MRI invisible tumors suggests that these tumors are more effectively tracked using TT-MIT biopsy technique. These MITs change over time and 15% of them will upgrade to higher Gleason scores. Tracking these tumors with TT-MIT biopsy increases cancer detection rate compared to standard sextant biopsy and more accurately samples these MITs, unveiling a more aggressive pathology.

The fundamental basis of the project concentrates upon an interactive manoeuvre involving Modern Continental Philosophy and the Postmodern Visual Arts. The primary components that structure the thesis conduct a Deleuzoguattarian "process” of action to produce a series of mechanisms designed to “open-up" a space in which to manifest a range of interpretations/translations that follow the developmentary trajectory of designated specific areas of art production. The primary aims concern the advance of the action to communicate an innovative/original set of expositions with a view to both “animate" and enhance these designated perspectives. The structural framework of the project tracks through “passages'/lines of flight” that occur/result from the impact of the Deleuzoguattarian mechanisms upon chosen material - "Introduction" (Primary) sets forth some guiding themes (how to think knowledge without the presumption of the neutral interchangeability of the subject?). “Tours of the Black Clock" (Dark) elaborates the Nietzschean background that informs the philosophical basis of the complete project. “Electrolite” (Spin the Black Circle), “Where Angels Play" (In a Network of Lines that Intersect), “Not to Touch the Earth” (Elephant Stone) “Across” (Out of Time) are sections designed to trace a number of alternative pathways through the major texts of the Deleuzoguattarian enterprise. “Becoming X" (Impact) initiates a secondary movement of experimentation. “Animal Nitrate" (The Image of Chance) effects “an encounter" with the work of painter Francis Bacon via the Deleuze text(s) “Logique de la Sensation" (1981), “Time and Again" (New Damage) introduces and articulates a number of instances from Deleuzian film-theory “The Fountainhead” (Vision Machines) is the main section of the project which evolves a detailed process analysing the paintings of American painter David Salle “The Mirror of Enigmas" (Numbers in the Dark) traces related Deleuzeoguattarian themes through the films of David Lynch, maintaining as a background an examination of Postmodern American culture/society. “Breaking into Heaven" (Elegia) returns to the more specific analysis of the late paintings of David Salle and uses the mechanics elaborated in Deleuze's book “Le Pli”. “Final Cut”/”Last Exit”/”Memorial Beach" is an attempt at a series of concluding remarks within the project divisions are resolved within a comprehensive Deleuzoguattarian structural system - evolving to be primarily defined in terms of two “active" processes/movements which engage theory and explanation, practice and application respectively, being therefore both a philosophical /academic debate while incorporating a high-level of “creative" experimentation. The first sections of the project detail an extensive series of explanatory “pathways/journeys” through the texts of Deleuze and Guattari with a view to indicating measures that can be deployed/utilized in a Deleuzoguattarian process of art/cultural analysis. Constructing the architectural pattern/”anatomy” of a series of interpretative surfaces to reflect the dimensions of the cultural material approached. The sections in the second part of the project enact a number of experimental designs, dispatching Deleuzoguattarian mechanisms in the body of the chosen cultural material. An interpretative surface is never under the influence of a single differentiated flow it constitutes the mutual boundary of numerous adjacent flows. The same interpretative surface is therefore available to be understood in as many different ways as there are adjacent flows along its fractural surface. This project is essentially a speculative enterprise, a philosophical experiment whose aim is to enhance, enlarge and intensify our “knowledge" of the area defined, through a synthesis, here labelled as a “Deleuzoguattarian process of Art analysis”. For Deleuze, thought creates what it thinks, as perception creates what it perceives (and therefore does not relate to it). The philosophical concept does not refer to the lived, but consists through its own creation, in setting up an event that surveys ("survole“) the whole of the lived no less than every state of affairs Deleuzoguattarian philosophy does not establish a relationship “between” philosophy and other disciplines, other ways of making sure, but eliminates this relationship to the advantage of a type of “greater philosophy” Thought/Philosophy thinks the sufficient reason of the actual in this manner. For Nietzsche, whose enterprise constitutes (I have suggested) the base/departure for the Deleuzoguattarian program, “the world" we know lies in our interpretation of it. However, the task of interpretation is not to discover “truth" but to “create" it. According to Nietzsche, there is no “world-in-itself, no unconditioned and stable entities that await our discovery, but a dynamic and turbulent “becoming" in which the forms we perceive are not separate from our interpretations of them Because we are ourselves part of this becoming, there is no vantage point from which it is possible to gain the absolute/unconditioned knowledge that we have imbued with the idea of “truth”. The exercise merely becomes self-reflective. However, despite this skepticism, Nietzsche realizes that the game of truth is unavoidable, so therefore he, and In consequence we, departing/initiating the pathways through this project, have to cast in the role of the philosopher who knows what he invents and invents while he knows. In Postmodern Art no style dominates. Alternatively we experience endless improvisations and variations on themes, parody/playfulness. Postmodern artists are unabashedly eclectic and call attention to it, combining traditions borrowing from rituals and myths. All the world's cultural symbols are now available in the public domain - “SANTA CLAUS IS ON THE CROSS". The Deleuzoguattarian process of art delineated is a play with the game of “truth” and not an explanation of a “whole" a description of the network of the "dynamic”/fluid nature of our transitional relationship to it. It is the flow of energy that encompasses what Deleuze/Nietzsche refer to as “the whole" A flow invokes the dynamic/fluid nature of becoming, while energy/activity implies a potentiality - the inherent capacity and growth actuated by the will to power. The flows of energy are manifested into differentiated flows within flows, “Ad infinitum” and powered by the parameters of state space - a potential i.e field defined by the characteristics of the subject flows associated Nothing exists in isolation Differentiation exists only via relations to other flows in a dynamic/non linear network of resistances.

Cette thèse étudie ce vaste mouvement de découverte et d'inventaire destiné à l'élaboration d'un savoir sur l'espace ainsi que la production d'un discours géographique et son articulation avec les objectifs politiques, laissant entrevoir l'existence de décalages. D'abord livresque, ce savoir provient ensuite des reconnaissances conduites sur le terrain par des explorateurs et des militaires à l'aide de pratiques spécifiques imposées par les conditions du parcours. L'entreprise de connaissance évolue enfin avec la mise en place du Protectorat français et pose la question de la création d'une géographie marocaine, au moment où est achevée la carte de reconnaissance du pays.

Background: The optimal extent of the prostate biopsy remains controversial. There is a need to avoid detection of insignificant cancer but not to miss significant and curable tumors. In alternative treatments of prostate cancer, repeated sextant biopsies are used to estimate the response. The aim of this study was to investigate the reliability of a repeated systematic sextant biopsy as the standard biopsy technique in patients with significant tumors which are being considered for curative treatment. Methods: Systematic sextant biopsy was performed in vitro in 92 radical prostatectomy specimens. Of these patients, 81 (88.0%) had palpable lesions. Results: Of the 92 investigated patients, 70 (76.1%) had potentially curable pT2-3pN0 prostate cancers. In these patients, the cancer was detected only in 72.9% of cases by a repeated in vitro biopsy. In the pT2 tumors, there was a detection rate of only 66.7%. Conclusions: This study underlines the fact that a considerable number of significant and potentially curable tumors remain undetected by the conventional sextant biopsy.
