Abstract:
Objective: The mechanism underlying postnatal growth failure and catch-up growth in small-for-gestational-age (SGA) children is poorly understood. This study investigated the exosomal miRNA signature associated with catch-up growth in SGA children. Methods: In total, 16 SGA and 10 appropriate-for-gestational-age (AGA) children were included. Serum exosomal miRNA was analyzed using next-generation sequencing (NGS). Exosomal miRNA was profiled for five SGA children with catch-up growth (SGA-CU), six SGA children without CU growth (SGA-nCU), and five AGA children. Results: Exosomal miRNA profiles were clustered into three clear groups. The exosomal miRNA expression profiles of the SGA-nCU group differed from those of the SGA-CU and AGA groups. In all, 22 miRNAs were differentially expressed between SGA-nCU and AGA, 19 between SGA-nCU and SGA-CU, and only 6 between SGA-CU and AGA. In both SGA-nCU and SGA-CU, miR-874-3p was upregulated and miR-6126 was downregulated. Therefore, these two miRNAs could serve as biomarkers for SGA. Compared with SGA-CU and AGA, miR-30c-5p, miR-363-3p, miR-29a-3p, and miR-29c-3p were upregulated in SGA-nCU, while miR-629-5p and miR-23a-5p were downregulated. These six miRNAs could be associated with growth failure in SGA-nCU children. Conclusions: SGA children without CU have a distinct exosomal miRNA expression profile compared with AGA and SGA children with CU. Exosomal miRNAs could serve as novel biomarkers for CU.

Abstract:
Purpose: The aim of this study was to evaluate the relationship between thyroid hormone levels and clinical outcomes in preterm, small for gestational age (SGA) infants. Materials and Methods: The premature newborns (gestational age of ≤30 weeks) were divided into two groups as SGA and non-SGA. Thyroid stimulating hormone (TSH) and free thyroxine (fT4) levels, the frequency of congenital hypothyroidism (CH), demographic and clinical characteristics, morbidity and mortality rate were compared between the groups. Results: A total of 430 premature newborns, 72 in the SGA group and 358 in the non-SGA group were included. The frequency of CH, morbidity, demographic and clinical characteristics were similar between two groups. The mortality rate was higher in SGA (36.1%) than in non-SGA group (13.6%). Serum fT4 level was lower in SGA group (1.04±0.30 ng/dl) compared to the non-SGA group (1.24±0.33 ng/dl). The serum TSH level was higher in SGA group (9.91 ± 5.6 uIU/L) than in non-SGA group (6.6 ± 5.2 uIU/L). Conclusion: The frequency of thyroid dysfunction was higher in preterm SGA infants compared to non-SGA, which was due to transiently high TSH and low fT4 concentrations. Therefore, thyroid function tests should be monitored periodically in preterm and SGA infants.

Abstract Context: Low birth weight is associated with an increased risk of metabolic and cardiovascular diseases in adulthood. The development of insulin resistance (IR) seems to play a pivotal role; no data on the oxidant-antioxidant status are available in this risk group. Objective: This study is an assessment of oxidant-antioxidant status in prepubertal children born small for gestational age (SGA) in comparison to healthy controls and the relationship to IR. Design: This cross-sectional study compares indexes of IR and oxidant-antioxidant status in three different groups (SGA+, SGA−, controls), with analysis by post hoc and Pearson correlation. Setting: The study was conducted in the Academic Department of Pediatrics. Participants: A total of 19 SGA+ and 16 SGA− children were compared with 13 controls. Intervention: No intervention was used. Main Outcome Measures: Indexes of IR (glucose to insulin ratio, homeostasis model assessment of IR) were evaluated, and markers of oxidative stress (lag phase, malonildialdehyde, vitamin E) were measured. Results: Homeostasis model assessment of IR was significantly higher in SGA+ than SGA− children (1.32 ± 0.9 vs. 0.69 ± 0.47; P = 0.03) and controls (0.71 ± 0.37; P = 0.04). Glucose to insulin ratio was significantly lower in SGA+ than SGA− children (12.41 ± 5.01 vs. 26.54 ± 17.18; P = 0.02) and controls (26.96 ± 20.70; P = 0.04). Lag phase was significantly shorter in SGA+ than SGA− children (24.3 ± 4.38 vs. 35.59 ± 11.29 min; P = 0.003) and controls (45.28 ± 7.69 min; P = 0.0001) and in SGA− than controls (P = 0.01). Malonildialdehyde was significantly higher in SGA+ than SGA− children (0.79 ± 0.3 vs. 0.6 ± 0.1 nmol/mg; P = 0.03) and controls (0.36 ± 0.04 nmol/mg; P = 0.0001) and in SGA− children than controls (P = 0.02). Vitamin E was significantly reduced in SGA+ children than controls (27.54 ± 7.9 vs. 43.23 ± 11.32 μmol/liter; P = 0.002). Conclusion: Oxidative stress is present in both SGA+ and SGA− children, with a continuous alteration in relation to IR. Therefore, catch-up growth might exert the greatest influence in the development of future diseases.

Background: Small for gestational age (SGA) babies are at high risk of perinatal mortality. We aimed to determine the potential to reduce perinatal mortality by improving antenatal detection of SGA babies in Scotland. Methods: We conducted a retrospective population study of all singleton SGA babies born in the 15 Consultant-led maternity units in Scotland in a 3-month period (1st Dec 2014 to 28th Feb 2015 inclusive). Demographic and pregnancy outcome data were extracted from Scottish birth records for all pregnancies; case note review was performed for all SGA cases [defined as birthweight less than the 10th centile for their gestational age at delivery as defined by the appropriate sex-specific UK-WHO Child Growth Standards]. Results: The SGA rate in Scotland was 5.5% (673/12218; 95% confidence interval [CI] 5.1, 5.9) and 27.6% (186/673; 95% CI 24.3, 31.2) of SGA cases were identified prior to delivery. SGA was associated with 18.2% (12/66; 95% CI [10.1%, 30.0%) of all perinatal deaths. The majority (10/12, 83.3%) of SGA babies who died had been identified as SGA in the antenatal period. There was no difference in perinatal mortality whether SGA was detected or not (5.4% [10/186; 95% CI 2.8, 10.0] in the SGA detected group vs 0.4% [2/487 [95% CI 0.3, 2.2] in the non-detected group after adjusting for risk factors for SGA, gestation at delivery and birthweight centile (Adjusted odds ratio [AOR] 0.85 [95% CI 0.5, 1.5], p=0.556). Conclusions: Despite only around a quarter of SGA babies being identified antenatally, the potential to reduce perinatal mortality in the Scottish population by improving SGA detection is limited. Only a minority of perinatal deaths occurred in SGA babies; and in the majority of these SGA was detected antenatally.

Background: Hypoglycemia is one of the important problems encountered in small for gestational age (SGA) neonates. The incidence and risk factors associated with hypoglycemia in the first 48 hours of life in Small for Gestational Age (SGA) Neonates was investigated in this study.Methods: Capillary Blood Glucose was screened by glucostix at 1, 2, 6, 12, 24, and 48 hours of age in 100 SGA neonates fulfilling the inclusion and exclusion criteria.Results: The overall incidence of hypoglycemia in SGA neonates in the study was 24%. The incidence of hypoglycemia was significantly higher in preterm SGA neonates (37.50%) when compared to term SGA neonates (17.65%). The incidence of symptomatic hypoglycemia was 9%. Maximum incidence of hypoglycemia was noted at 2 hours of life. Of the 24 SGA neonates with hypoglycemia, 33.33% had one episode of hypoglycemia, 45.83% had two episodes of hypoglycemia, 12.50% had three episodes of hypoglycemia, 8.33% had four episodes of hypoglycemia. None of the SGA neonate had more than four episodes of hypoglycemia. Early initiation of enteral feeding was significantly associated with decreased incidence of hypoglycemia in SGA neonates. There was no significant correlation between incidence of hypoglycemia in SGA neonates and gender of the baby, parity of mother, mode of delivery and type of IUGR.Conclusions: It is recommended to monitor all SGA neonates especially preterm SGA for hypoglycemia and ensure early initiation of enteral feeding in all SGA neonates.

Length of stay (LOS) adalah masa rawat seorang pasien di rumah sakit dihitung sejakpasien masuk rumah sakit dan keluar rumah sakit, yang dipengaruhi oleh faktor usia, komorbiditas, hipermetabolisme,dan kegagalan organ serta defisiensi nutrisi. Status gizi merupakan salah satu komponenyang mempengaruhi biaya perawatan, lama hari perawatan, dan kualitas hidup. Salah satu cara penilaianstatus gizi adalah subjective global assessment (SGA) yang terdiri dari anamnesis dan pemeriksaan fisis yangmencerminkan perubahan metabolik dan fungsional.Tujuan. Penelitian ini bertujuan untuk menilai pengaruh status gizi berdasarkan SGA terhadap lama perawatanpasien rawat inap anak.Metode. Penelitian analitik observasional dengan rancangan kohort prospektif dilakukan selama periodeFebruari - Juni 2010 terhadap 311 pasien yang menderita infeksi akut usia 1 bulan 14 tahun yang dirawatdi ruang perawatan anak kelas III RSUP dr. Hasan Sadikin. Penilaian status gizi berdasarkan SGA dikelompokanmenjadi SGA A (gizi baik), SGA B (malnutrisi ringan + sedang, dan SGA C (malnutrisi berat).Hasil. Berdasarkan penilaian status gizi dengan SGA berturut-turut didapatkan SGA A, SGA B, dan SGAC sebesar 114 (36,7%), 98 (31,5%), dan 99 (31,8%). Dengan menggunakan uji chi square, didapatkanperbedaan lama perawatan yang bermakna (p<0,001) pada kelompok subjek SGA C dibandingkan kelompokSGA B dan SGA A. Berdasarkan analisis multivariat regresi logistik, kelompok SGA C berisiko 2,205 kalilebih tinggi untuk menjalani perawatan lebih lama (RR: 2,205; 95% CI: 1,151-4,227).Kesimpulan. Penelitian ini menunjukkan bahwa status gizi yang dinilai dengan SGA terbukti berpengaruhterhadap lama hari perawatan dan dapat dianjurkan untuk digunakan dalam penilaian status gizi.

ObjectiveSmall for gestational age (SGA) children are at risk of both later obesity and metabolic syndrome (MetS). However, it is unknown whether obesity or SGA status leads to MetS in these subjects. We hypothesized that overweight children with former SGA status had more present components of the MetS than overweight children with former appropriate for gestational age (AGA) status.MethodsWe analyzed 803 overweight children (4% SGA, mean age 11±0.1 years, body mass index (BMI) 27.3±0.2, SDS-BMI 2.32±0.02) concerning blood pressure, lipids, glucose, and insulin. Oral glucose tolerance tests (oGTT) were performed in all 35 former SGA children and 147 randomly chosen former non-SGA children.ResultsAfter adjustment for age, sex, pubertal stage, and BMI-SDS, former SGA status was significantly related to blood pressure, triglyceride, insulin, and 2 h glucose levels in oGTT. The MetS prevalence was more than doubled in overweight former SGA subjects (40% MetS) compared with overweight former AGA subjects (17% MetS). The corresponding adjusted odds ratio was 4.08 (95% confidence interval 1.48 to 11.22) for SGA compared with AGA children.ConclusionsOverweight former SGA children had an increased risk for the components of the MetS compared with overweight former AGA children. Therefore, SGA status seems to be a risk factor for the MetS independently of weight status. Particularly overweight children with former SGA status should be screened for the MetS.

Background. The small-for-gestational-age (SGA) in infants is related to an increased risk of developing Non-Communicable Diseases later in life. The Mediterranean diet (MD) is related to lower odds of being SGA. The study explored retrospectively the association between SGA, maternal MD adherence, lifestyle habits and other SGA risk factors during pregnancy. Methods. One hundred women (16–44 years) with a pregnancy at term were enrolled. Demographic data, parity, pre-gestational BMI, gestational weight gain, pregnancy-related diseases, and type of delivery were collected. The MD adherence (MEDI-LITE score ≥ 9), physical activity level, and smoking/alcohol consumption were registered. SGA neonates were diagnosed according to the neonatal growth curves. Results. Women were divided into “SGA group” vs. “non-SGA group”. The MD was adopted by 71% of women and its adherence was higher in the “non-SGA group” (p = 0.02). The prevalence of pregnancy-related diseases (gestational diabetes/pregnancy-induced hypertension) was higher in the “SGA group” (p = 0.01). The logistic regression showed that pregnancy-related diseases were the only independent risk factor for SGA. Conclusions. MD may indirectly reduce the risk of SGA since it prevents and exerts a positive effect on pregnancy-related diseases (e.g., gestational diabetes and hypertension). The small sample size of women in the SGA group of the study imposes a major limitation to the results and conclusions of this research, suggesting however that it is worthy of further investigation.

A gestão ambiental vem despertando interesse e ocupando maiores espaços em muitos segmentos por figurar entre os assuntos relevantes para a sociedade e fazer parte do espectro de discussões no âmbito das organizações, principalmente no que tange às implicações referentes à sobrevivência de organizações. O presente trabalho de investigação consiste na implementação de uma unidade de gestão ambiental no município de San Ignácio de Velasco, Santa Cruz de la Sierra, Bolívia, interpretando a legislação boliviana e descrevendo o funcionamento da gestão ambiental municipal para ajudar com as melhorias na qualidade de vida da população, com a preservação de seu meio ambiente. Os dados para a realização do estudo foram coletados através de fontes secundárias (documentos legais da empresa) e entrevistas não estruturadas aplicada junto aos funcionários do município. O estudo demonstrou que mesmo com as leis vigentes, como a Lei de Meio Ambiente, o município não possui uma forma adequada de fazer gestão ambiental, seja interpretando e entendendo melhor as leis ambientais e municipais, seja estruturando um modelo de gestão, o que dificulta o desenvolvimento de projetos, bem como o controle, a proteção e a conservação do meio ambiente e dos recursos naturais. Neste sentido, a pesquisa propõe a unidade ambiental para ser implementada e apresenta algumas reflexões sobre questões ecológicas, focalizando preceitos de ecologia que indicam novos caminhos para o Desenvolvimento Sustentável, como as tecnologias limpas e o sistema de gestão ambiental.

Resumo: No mundo dos negócios acontecem mudanças constantemente. Atrelada a estas mudanças está a sobrevivência organizacional, principalmente no que tange ao fator competitividade. Conseqüentemente, é fundamental que haja otimização do uso dos recursos disponíveis dentro de cada organização, tornando-o mais eficiente. Sendo assim, torna-se importante e necessária a integração máxima entre os diversos sistemas de gestão co-existentes dentro das organizações: gestão da qualidade, ambiental, da segurança e saúde ocupacional, financeira, de pessoas, estratégica, da responsabilidade social, entre outras. Com estes sistemas alinhados e integrados, as organizações tendem a maximizar sua eficiência com redução de custos, assegurando sua sobrevivência. Este trabalho, portanto, tem o objetivo de desenvolver e propor uma metodologia de avaliação da maturidade dos sistemas de gestão dentro das organizações, bem como do nível de integração entre eles, nas dimensões da qualidade: do produto, ambiental, da segurança e saúde ocupacional e ética e social. Com isto é possível identificar as oportunidades de melhorias na integração das diversas dimensões da gestão e de fato buscar a maximização da eficiência no uso dos recursos disponíveis
Abstract: In the business world changes happen constantly. Linked to these changes is the organization survival, mainly as for the competitiveness factor. Consequently, it is fundamental to have the otimization of the use of the resources available inside each organization, becoming it more efficient. So, it becomes important and necessary the maximum integration among the several management systems co-existant inside the organizations: quality management, environrnental, occupational health and safety, financial, people, strategic, social accountability, among others. With these systems aligned and integrated, the organizations tend to maximize their efficiency with cost reduction, assuring their survival. This paper, therefore, has the aim of developing and proposing a methodology for evaluation of the maturity of the management systems inside the organizations as well as the integration of them, in the dimensions of quality: of the product, environrnental, of the occupational health and safety and ethical and social. With this, it is possible to identify the opportunities for improvements in the integration of several management dimensions and, in fact, to search the maximization of the efficiency in the use of the available resources

As questões ambientais são cada vez mais relevantes para as organizações revelando implicações no seu desempenho económico e competitivo. A implementação de Sistemas de gestão Ambiental (SGA) como forma de regulação pró-ativa promove a melhoria do desempenho ambiental e baseia-se em instrumentos e metodologias de complexidade variada. A norma ISO 14001 enquanto referencial é aplicável a todo o tipo de organizações, independentemente da sua área de atuação ou dimensão e, para além de estabelecer os requisitos para o sistema, pode ainda servir de base à sua certificação. No contexto da melhoria das condições ambientais e desempenho das organizações, também os resíduos assumem um papel fundamental na Gestão Ambiental. O presente trabalho teve como objetivo a identificação e implementação de melhorias no SGA da Mistolin S.A, com principal incidência na Gestão de resíduos da organização. Para além disso, teve também como objetivo a identificação qualitativa das Matérias-primas perigosas envolvidas no processo de produção. A metodologia utilizada consistiu na análise e consulta dos documentos do SGA e outros registos internos, e no levantamento e observação dos processos de trabalho utilizados pela empresa. Foram estabelecidos contactos com órgãos de gestão de resíduos de forma a analisar (em termos de custo-benefício) e adotar soluções de gestão mais eficientes no contexto económico e ambiental. Foram identificadas oportunidades de melhoria ao nível do procedimento de gestão de resíduos, em particular na organização do parque de resíduos, na frequência e periodicidade de recolhas, distribuição e sistema de identificação de contentores. Para além disso, ficou clara a importância da sensibilização e da formação dos colaboradores no que respeita às questões ambientais no geral. Na análise das MPQ envolvidas, das 272 substâncias químicas constituintes dos produtos utilizados, identificaram-se 184 substâncias classificadas como perigosas para a saúde humana. A identificação destas substâncias consistiu na primeira fase do processo de avaliação da exposição a substâncias químicas, que continuará com a quantificação e verificação do cumprimento de Valores Limite de Emissão. Conclui-se que o SGA da Mistolin S.A., apesar do bom funcionamento atual poderá beneficiar pela introdução de algumas oportunidades de melhoria, a implementar no futuro, de forma a melhorar continuamente o seu desempenho.
Environmental issues are becoming more important to organizations and have important implications for their economic and competitive performance. The implementation of Environmental Management Systems (EMS), as a form of proactive regulation, promotes the improvement of environmental performance and is based on a complex variety of tools and methodologies. The ISO 14001 standard, as a framework, is applied to all types of organizations, independently of from their operational field or size and besides establishing the requirements for the system, it also provides a basis for certification. To improve the environmental performance and condition of organizations residues play a key role in environmental management. This present study aimed to identify and implement improvements in the EMS implemented at Mistolin SA with a focus on the organization's waste management. Furthermore, this work also aimed at the qualitative identification of hazardous raw materials involved in the production process. The methodology that was used consisted in the analysis and consultation of the EMS documents and other internal records, mapping and observation of working processes used by the company. Contact was made with waste management agencies in order to analyze (in terms of cost-benefit) and adopt more efficient management solutions in the economic and environmental contexts. Opportunities were identified in order to improve the waste management procedure, in particular in the organization of the waste park, in frequency, timing of collection, distribution and container identification system. Furthermore, the importance of awareness and employees training with regard to environmental issues in general became clear. During the Analysis of the Chemical Raw Materials involved, from the 272 chemical constituents of the products used, we identified 184 substances classified as harmful to human health. The identification of these substances consisted, in the first phase in the evaluation process about exposure to chemicals, which will continue with the quantification and verification of compliance of Emission Limit Values. In conclusion despite the good current operation the EMS at Mistolin SA could benefit from implementing some improvement suggestions in order to achieve in a near future continual improvement in its performance.

Por outro lado, a crescente concorrência global, sentida particularmente emPortugal, que possui uma economia pequena, obriga as empresas amelhorarem os seus índices de produtividade e competitividade. A articulação entre estes referenciais, aliada ao contexto actual caracterizadopor uma recessão profunda e generalizada nas economias é assim umenorme desafio para as indústrias portuguesas. É portanto, crucial para as empresas o reconhecimento de que bonsdesempenhos ambientais, alcançados através da introdução de práticasambientais, que proporcionem melhor eficiência de processos, redução dosconsumos de energia, matérias-primas e água, bem como minimização dotratamento de resíduos e efluentes, podem ser factores de aumento decompetitividade, de crescimento económico e de reconhecimento. Esta avaliação implicou numa fase inicial a caracterização dos Sistemas deGestão Ambiental e a caracterização do sector da Indústria Cerâmica,abordando em concreto o subsector da Cerâmica Estrutural, de forma à suacontextualização aos mais variados níveis. Numa segunda parte procedeu-se à descrição da implementação de umSistema de Gestão Ambiental numa empresa incluída no subsector daCerâmica Estrutural, a Preceram - Indústrias de Construção, SA. Foiefectuada a caracterização ambiental da empresa alvo de estudo, incluindo asMelhores Técnicas Disponíveis (MTD) já implementadas, identificados eavaliados aspectos ambientais, efectuado o enquadramento ambiental e aindaidentificadas oportunidades de melhoria e definidos objectivos e metas para aorganização. Conclui-se que o processo de implementação do SGA está intimamenterelacionado com a melhoria de desempenho ambiental e com o cumprimentode alguns dos requisitos legais da organização e que o mesmo é dificultadopela disponibilidade de recursos da empresa e pelo receio acrescido dedisponibilização de recursos face ao contexto económico actual. A resistênciainterna à mudança das práticas existentes constitui outro factor deconstrangimento e que deverá ser ultrapassado através de um maiorenvolvimento dos colaboradores. Constata-se ainda, que a obtenção da certificação ambiental poderá estarfortemente comprometida pelas dificuldades inerentes ao cumprimento dosvalores de emissão associados (VEA) às MTD estabelecidas no Documentode Referência de aplicação sectorial, e que foram adoptados aquando daemissão das Licenças Ambientais da organização.
Environmental protection is now seen globally. In this context, it should benoted the increase of legal requirements at the environmental level and theincrease of ecological awareness, that makes customers and consumers ingeneral, increasingly informed, more predisposed to buying and using environmentally sustainable products. On the other hand, the growing global competition, particularly felt in Portugal,which has a small economy, forces companies to improve their levels ofproductivity and competitiveness. The articulation among these issues, together with the current contextcharacterized by a deep and widespread recession in the economies is thus amajor challenge for Portuguese industries. Is therefore crucial for companies to recognize that good environmental performance, achieved through the introduction of environmental practices,which provide better process efficiency, reducing consumption of energy, rawmaterials and water as well as minimization of waste treatment and effluents,can give rise to increased competitiveness, economic growth and recognition. This dissertation evaluates the experience of implementing an EnvironmentalManagement System, taking as reference the ISO 14001:2004, in theCeramics Industry, a sector which is experiencing a particularly difficult phase in Portugal to the significant slowdown in the construction market. The aim is to check the intrinsic value of EMS as a tool for theoperationalization of sustainable development, as well as pass on theexperience of the sector in order to better implementation of these systems aseffective tools for this form of development. This evaluation resulted in an initial characterization of EnvironmentalManagement Systems and the characterization of the ceramic industry sector, addressing in particular the sub-sector of structural ceramics in order to contextualize it in their different levels. In a second part we proceeded to the description of the implementation of anEnvironmental Management System in a company included in the branch of structural ceramics, Preceram - Construction Industries SA. Was carried out to characterize the target company's environmental study, including the BestAvailable Techniques (BAT) have been implemented, identify and evaluateenvironmental aspects, environmental framework and also identifiedopportunities for improvement and set goals and targets for the organization. It is concluded that the process of EMS implementation is closely related tothe improvement of environmental performance and compliance with certain legal requirements of the organization and that it is hampered by theavailability of company resources and the fear of increased availability ofresources, given the current economic climate. The internal resistance to change existing practices is another constraining factor and should be overcome through greater involvement of employees. We noted also, that obtaining environmental certification may be greatlycompromised by the difficulties inherent in meeting the associated emission levels with (AEL) established in Reference Document on BAT applicationsector, which were adopted at the Environmental Permites issued for the organization.

INTRODUZIONE Negli ultimi anni sempre maggiore interesse è stato riservato allo studio dei bambini nati piccoli per l età gestazionale (SGA, Small for gestational age) ovvero quei soggetti che presentano peso e/o lunghezza alla nascita inferiore al 3° percentile o alle - 2 DS. Fino ad alcune decine di anni fa, tale categoria non era considerata a rischio, se non nel breve termine; oggi invece la letteratura scientifica è ricca di dati che indicano che i bambini nati SGA, rispetto ai nati di peso adeguato (AGA, Adeguate for gestational age) presentano una probabilità maggiore di sviluppare in età adulta iposomia, malattie cardiovascolari e metaboliche, quali ipertensione arteriosa, obesità, dislipidemia, insulino-resistenza, ridotta tolleranza glucidica e diabete di tipo II, identificando il quadro della sindrome metabolica o sindrome X, e problematiche neuropsicologiche quali disturbi dell attenzione e iperattività, deficit nell apprendimento, ridotta autostima e difficoltà di interazione sociale. Nonostante le numerose ricerche non risulta ancora chiara l eziologia che sottende l associazione tra basso peso alla nascita e patologie dell età adulta, anche se diverse sono state le ipotesi avanzate. Malgrado la maggior parte dei soggetti SGA manifesti una crescita di recupero, definita catch-up growth , entro il terzo anno di vita, circa un bambino su 10 non la esibirà presentando, quindi un quadro di iposomia. Dal momento che dai dati in letteratura si evince che il nascere piccoli per l età gestazionale comporta quindi un maggiore rischio di sviluppare sin dall infanzia problematiche di carattere endocrino-metabolico oltre al problema dell iposomia, risulta fondamentale nei bambini dismaturi effettuare accurati controlli al fine di attuare strategie di prevenzione e di riconoscere tempestivamente la presenza di eventuali complicanze. La presente trattazione si pone come fine quello di fornire un approfondimento sulla condizione dei bambini Small for gestational age, affrontando nella prima parte l analisi dei dati riportati in letteratura relativi la definizione, l epidemiologia e le problematiche correlate quali complicanze e possibilità di trattamento, e descrivendo, nella seconda parte, lo studio epidemiologico, osservazionale, da me condotto al fine di verificare le problematiche relativa alla bassa statura in una popolazione di bambini nati SGA del nostro territorio.

Darbo tikslas Ištirti mažų pagal nėštumo trukmę vaikų postnatalinį augimo pobūdį, hormonų profilį bei nustatyti prenatalinius ir ankstyvuosius postnatalinius veiksnius įtakojančius vaikų augimą ir metabolinius pokyčius iki brendimo Darbo uždaviniai 1. Ištirti mažų pagal nėštumo trukmę vaikų postnatalinio augimo pobūdį pirmaisiais 6 gyvenimo metais, lyginant juos su to paties amžiaus vaikais, gimusiais atitinkančio nėštumo trukmei svorio ir ūgio. 2. Įvertinti tėvų ir naujagimių antropometrinių rodiklių bei hormonų profilio gimimo metu įtaką mažų pagal nėštumo trukmę vaikų augimui nuo gimimo iki 6 metų. 3. 6 metų mažiems ir atitinkantiems nėštumo trukmę vaikams ištirti hormonų profilį (insulinoidinį augimo faktorių–I (IGF–I), IGF–I surišantį baltymą–3 (IGFBP–3) leptiną, adiponektiną), jautrumą insulinui bei lipidų profilį. 4. Nustatyti gimimo svorio ir ankstyvojo postnatalinio augimo įtaką vaikų nutukimo, atsparumo insulinui bei dislipidemijos rizikai iki brendimo. Mažiems pagal nėštumo trukmę vaikams anksti nustatyti pradiniai metaboliniai pakitimai (sumažėjęs jautrumas insulinui, padidėjęs leptino ir sumažėjęs adiponektino kiekis, dislipidemijos požymiai, gliukozės tolerancijos sutrikimas), žemaūgiškumas, polinkis nutukimui gali suteikti savalaikę informaciją bei galimybę pritaikyti ankstyvas profilaktines priemones nutukimui bei 2 tipo cukrinio diabeto, širdies ir kraujagyslių ligų išsivystymui vyresniame amžiuje.
Aim The present study aims to explore the postnatal growth pattern and hormonal profile in small for gestational age children and to identify prenatal and early postnatal risk factors of short stature and postnatal metabolic changes in early prepubertal age. Objectives: 1. To explore the postnatal growth pattern of children born small for gestational age during the first six years of life in comparison with children of appropriate for gestational age birth size. 2. To evaluate the impact of parental axiology, neonatal anthropometry and hormonal status at birth on the early postnatal growth of small for gestational age children. 3. To investigate hormonal profile (insulin like growth factor-1, insulin like growth factor binding protein-3, adiponectin and leptin), insulin sensitivity and lipid profile in six-year-old small and appropriate for gestational age children. 4. To evaluate the influence of birth size and postnatal growth pattern on the risk of obesity, insulin resistance and dyslipidemia in small for gestational age children during the late prepubertal period.

Este trabalho tem por objetivo analisar as formas de implantação de um sistema de gerenciamento ambiental (SGA) na empresa Jornal A Notícia, bem como os procedimentos utilizados para a certificação ambiental. Para nortear o trabalho utilizaram-se os seguintes objetivos específicos: identificar os procedimentos de implantação do SGA no Jornal A Notícia; caracterizar as etapas para a certificação ambiental (NBR ISO 14001) no Jornal A Notícia. Para desenvolver a pesquisa foi efetuado um estudo de caso, sendo que a metodologia utilizada foi a qualitativa, exploratória, descritiva, onde foram realizados estudos por meio de livros, jornais, revistas e meios eletrônicos. Como resultado do trabalho, se puderam observar quais são as etapas necessárias para a adoção e implantação de um SGA de acordo com a Norma ISO 14001 e também os diversos benefícios auferidos por essa empresa ao adotarem e se certificarem pela norma ambiental.

O presente trabalho tem como objetivo verificar a existência de Educação Ambiental/treinamento dentro do SGA em duas empresas do ramo cerâmico no sul de Santa Catarina. Destas, a primeira apresenta a certificação ISO série 14000; a segunda está em processo de certificação. Para alcançar esse objetivo, desenvolveu-se um estudo exploratório através de questionários estruturados em quatro níveis de análises: i) formação sócio - econômica e cultural; ii) meio ambiente e empresa; iii) controle da qualidade ambiental na empresa e iv) mudanças de comportamento. Como ponto fundamental, cabe destacar a dimensão da educação Ambiental/treinamento nas referidas empresas, quando se considera que o conhecimento ambiental está relacionado também com as informações recebidas nestes ambientes. Nesse sentido, foi possível constatar que as empresas que apresentam o SGA estão mais comprometidas com a educação/treinamento. Cada referência, no entanto, contribui à mudanças de comportamento no âmbito das empresas, onde baseado nos dados coletados, é possível constatar que os programas de treinamento também podem favorecer e se constituir em valioso instrumento para informar, sensibilizar e estimular a qualidade ambiental.

O compromisso das organizações com as obrigações decretadas ao nível do ambiente, segurança e qualidade emancipa a implementação de Sistemas de Gestão e o processo de auditar. Neste trabalho, decorrente do estágio curricular no âmbito do Mestrado em Engenharia do Ambiente, está evidenciada a importância dos Sistemas de Gestão Ambiental e as razões preponderantes para que sejam auditados. O trabalho pretendeu auditar e avaliar a conformidade do sistema de gestão ambiental, de acordo com a NP EN ISO 14001:2004, de uma indústria de fundição de metais, Funfrap -. Fundição Portuguesa SA, a fim de identificar oportunidades de melhoria no desempenho ambiental da organização. No âmbito do estágio foram ainda realizadas outras atividades, incluindo campanhas de sensibilização, inventário de resíduos sólidos, ferramentas da metodologia World Class Manufacturing, formação sobre a separação de resíduos nos postos de trabalho, acompanhamento de um ensaio na estação de tratamento de águas residuais industriais e apoio para o desenvolvimento do relatório ambiental anual. Da auditoria interna identificaram-se e corrigiram-se nove não conformidades menores e cinco observações. Da auditoria externa foram identificadas apenas duas não conformidades, nenhuma delas relacionadas com as detetadas anteriormente. Além deste resultado foram sugeridas algumas medidas para alcançar um melhor desempenho ambiental da organização.
The organizations’ commitment with the obligations imposed at the level of environment, safety and quality, emancipates the implementation of management systems and audit process. This report, resulting from the internship for master degree in environmental engineering, showed the importance of environmental management systems and the prevailing reasons to be audited. The aim of this work was to audit and assess the conformity of the environmental management system, according to NP EN ISO 14001:2004, of a metals casting industry, Funfrap – Fundição Portuguesa SA., in order to identify opportunities for environmental performance improvement. Under the internship other activities were carried out, including awareness campaign, solid wastes inventory, tools from the World Class Manufacturing methodology, training on waste separation at workstations, an experiment on the industrial wastewater treatment plant and support to the development of the annual environmental report. From the internal audit nine minor non-conformities and five observations were identified and corrected. As a result, during the external audit occurred only two minor nonconformities were identified but not related with the previous one. Apart of this result some measures have been suggested to achieve a better environmental performance in the organization.

No planeamento de um Campus universitário, é necessário ter em consideração determinadas variáveis de modo a garantir que no futuro, o desempenho ambiental da instituição seja optimizado com a implementação de um Sistema de Gestão Ambiental. O presente trabalho foca o caso do Campus da Faculdade de Ciências e Tecnologia da Universidade Nova de Lisboa encontrando-se dividido em três partes. Na primeira parte, em que é efectuada uma revisão da literatura, faz-se uma abordagem aos conceitos de Campus, planeamento urbano e Sistemas de Gestão Ambiental. Incluem-se também referências à legislação e regulamentação Portuguesa em relação ao planeamento, aos instrumentos de ordenamento do território e as normas relativas à implementação de Sistemas de Gestão Ambiental. É igualmente analisado um exemplo internacional relativo à implementação e resultados de um Sistema de Gestão Ambiental num Campus e é feita uma introdução das características gerais do caso em estudo – Faculdade de Ciências e Tecnologia da Universidade Nova de Lisboa. Na segunda parte, dedicada à análise do historial dos estudos de planeamento realizados para o Campus da Faculdade de Ciências e Tecnologia, começa-se por recorrer à compilação e análise documental dos vários estudos e projectos de planeamento realizados para o Campus. São nesta parte analisados três estudos, o ―Estudo de Implementação da FCT‖, ―Plano Director da FCT/UNL‖ e o ―Plano Urbanístico da FCT‖. Com base nestes estudos e na revisão bibliográfica foi possível definir variáveis que devem ser consideradas durante a elaboração de estudos de planeamento, pois têm impacte no desempenho ambiental do Campus. Assim, na terceira parte, é apresentado o estado actual do Campus do caso de estudo. Em seguida procede-se a uma comparação entre o planeado e o executado, procurando pontos de conflito de modo a perceber os impactes ambientais e sociais que daí advêm analisando-se essencialmente as variáveis propostas anteriormente. Depois desta análise, determinaram-se metas e objectivos que devem ser seguidos em futuros planos analisando a influência que o planeamento tem na implementação de um Sistema de Gestão Ambiental. A finalizar, apresentam-se as conclusões e propõem-se futuros desenvolvimentos.

Abstract:
Steroidal glycoalkaloids (SGAs) are secondary metabolites being part of the plant defense response. The two major SGAs in cultivated potato (Solanum tuberosum) are α-chaconine and α-solanine, which exhibit strong cellular lytic properties and inhibit acetylcholinesterase activity, and are poisonous at high concentrations for humans. As SGAs are not destroyed during cooking and frying commercial cultivars have been bred to contain low levels, and their content in tubers should not exceed 20 mg/100 g fresh weight. However, environmental factors can increase tuber SGA content above the safe level. The focus of the proposed research was to apply genomic approaches to identify candidate genes that control potato SGA content in order to develop tools for potato improvement by marker-assisted selection and/or transgenic approaches. To this end, the objectives of the proposal included identification of genes, metabolic intermediates and allelic variations in the potato SGAbiosynthetic pathway. The SGAs are biosynthesized by the sterol branch of the mevalonic acid/isoprenoid pathway. Transgenic potato plants that overexpress 3-hydroxy-3-methylglutaryl-CoA reductase 1 (HMG1) or squalene synthase 1 (SQS1), key enzymes of the mevalonic acid/isoprenoid pathway, exhibited elevated levels of solanine and chaconine as well as induced expression of genes downstream the pathway. These results suggest of coordinated regulation of isoprenoid (primary) metabolism and SGA secondary metabolism. The transgenic plants were further used to identify new SGA-related candidate genes by cDNA-AFLP approach and a novel glycosyltransferase was isolated. In addition, genes involved in phytosterol biosynthesis may have dual role and synthesize defense-related steroidal metabolites, such as SGAs, via lanosterol pathway. Potato lanosterol synthase sequence (LAS) was isolated and used to prepare transgenic plants with overexpressing and silencing constructs. Plants are currently being analyzed for SGA content. The dynamics of SGA accumulation in the various organs of a potato species with high SGA content gave insights into the general regulation of SGA abundance. Leaf SGA levels in S. chacoense were 10 to 20-fold greater than those of S. tuberosum. The leptines, SGAs with strong antifeedant properties against Colorado potato beetles, were present in all aerial tissues except for early and mid-developmental stages of above ground stolons, and accounted for the high SGA content of S. chacoense. These results indicate the presence of regulatory mechanisms in most tissues except in stolons that limit the levels of α-solanine and α-chaconine and confine leptine accumulation to the aerial tissues. The genomes of cultivated and wild potato contain a 4-member gene family coding for SQS. Three orthologs were cloned as cDNAs from S. chacoense and heterologously expressed in E. coli. Squalene accumulated in all E. coli lines transformed with each of the three gene constructs. Differential transcript abundance in various organs and amino acid sequence differences in the conserved domains of three isoenzymes indicate subfunctionalization of SQS activity and triterpene/sterol metabolism. Because S. chacoense and S. phureja differ so greatly for presence and accumulation of SGAs, we selected four candidate genes from different points along the biosynthetic pathway to determine if chcor phuspecific alleles were associated with SGA expression in a segregating interspecific diploid population. For two of the four genes (HMG2 and SGT2) F2 plants with chcalleles expressed significantly greater total SGAs compared with heterozygotes and those with phualleles. Although there are other determinants of SGA biosynthesis and composition in potato, the ability of allelic states at two genes to affect SGA levels confirms some of the above transgenic work where chcalleles at two other loci altered SGA expression in Desiree. Present results reveal new opportunities to manipulate triterpene/sterol biosynthesis in more targeted ways with the objective of altering SGA content for both human health concerns and natural pesticide content without disrupting the essential metabolism and function of the phytosterol component of the membranes and the growth regulating brassinosteroids.

Objective. To assess the comparative benefits and harms across three airway management approaches (bag valve mask [BVM], supraglottic airway [SGA], and endotracheal intubation [ETI]) by emergency medical services in the prehospital setting, and how the benefits and harms differ based on patient characteristics, techniques, and devices. Data sources. We searched electronic citation databases (Ovid® MEDLINE®, CINAHL®, the Cochrane Central Register of Controlled Trials, the Cochrane Database of Systematic Reviews, and Scopus®) from 1990 to September 2020 and reference lists, and posted a Federal Register notice request for data. Review methods. Review methods followed Agency for Healthcare Research and Quality Evidence-based Practice Center Program methods guidance. Using pre-established criteria, studies were selected and dual reviewed, data were abstracted, and studies were evaluated for risk of bias. Meta-analyses using profile-likelihood random effects models were conducted when data were available from studies reporting on similar outcomes, with analyses stratified by study design, emergency type, and age. We qualitatively synthesized results when meta-analysis was not indicated. Strength of evidence (SOE) was assessed for primary outcomes (survival, neurological function, return of spontaneous circulation [ROSC], and successful advanced airway insertion [for SGA and ETI only]). Results. We included 99 studies (22 randomized controlled trials and 77 observational studies) involving 630,397 patients. Overall, we found few differences in primary outcomes when airway management approaches were compared. • For survival, there was moderate SOE for findings of no difference for BVM versus ETI in adult and mixed-age cardiac arrest patients. There was low SOE for no difference in these patients for BVM versus SGA and SGA versus ETI. There was low SOE for all three comparisons in pediatric cardiac arrest patients, and low SOE in adult trauma patients when BVM was compared with ETI. • For neurological function, there was moderate SOE for no difference for BVM compared with ETI in adults with cardiac arrest. There was low SOE for no difference in pediatric cardiac arrest for BVM versus ETI and SGA versus ETI. In adults with cardiac arrest, neurological function was better for BVM and ETI compared with SGA (both low SOE). • ROSC was applicable only in cardiac arrest. For adults, there was low SOE that ROSC was more frequent with SGA compared with ETI, and no difference for BVM versus SGA or BVM versus ETI. In pediatric patients there was low SOE of no difference for BVM versus ETI and SGA versus ETI. • For successful advanced airway insertion, low SOE supported better first-pass success with SGA in adult and pediatric cardiac arrest patients and adult patients in studies that mixed emergency types. Low SOE also supported no difference for first-pass success in adult medical patients. For overall success, there was moderate SOE of no difference for adults with cardiac arrest, medical, and mixed emergency types. • While harms were not always measured or reported, moderate SOE supported all available findings. There were no differences in harms for BVM versus SGA or ETI. When SGA was compared with ETI, there were no differences for aspiration, oral/airway trauma, and regurgitation; SGA was better for multiple insertion attempts; and ETI was better for inadequate ventilation. Conclusions. The most common findings, across emergency types and age groups, were of no differences in primary outcomes when prehospital airway management approaches were compared. As most of the included studies were observational, these findings may reflect study design and methodological limitations. Due to the dynamic nature of the prehospital environment, the results are susceptible to indication and survival biases as well as confounding; however, the current evidence does not favor more invasive airway approaches. No conclusion was supported by high SOE for any comparison and patient group. This supports the need for high-quality randomized controlled trials designed to account for the variability and dynamic nature of prehospital airway management to advance and inform clinical practice as well as emergency medical services education and policy, and to improve patient-centered outcomes.

The aim of this project was to develop an impact framework for the Science Gallery Network (SGN). This work was commissioned by the Science Gallery International (SGI). The SGN has eight member organisations across four continents: Dublin, London, Melbourne, Bengaluru, Detroit, Rotterdam, Atlanta and Berlin. Whilst the network consistently sees unprecedented levels of accomplishment by its members, a testimony to their capacity, innovation and vision, the SGN does not have a systematic way to measure and monitor this impact. An impact framework that can assist with understanding and reporting the value of this impact will provide important recognition that the SGN has achieved what it sets out to do— bringing science, art, technology and design together to deliver world-class educational and cultural experiences for young people. This report details the robust consultation approach that was undertaken by the University of Melbourne’s project team—one that included a desktop review, focus group discussions, surveys and interviews—to ensure multiple perspectives were gathered on what could be considered a multi-faceted concept. The desktop review provided a thorough review and an environmental scan of the impact literature and its measurement. In addition, the focus group discussions and interviews provided a rich understanding of what ‘good impact’ means for the SGN and the implications of this to the measurement of impact outcomes. Five key recommendations are provided and summarised. Note that these key recommendations should be taken as a point of departure for further in-depth consultation throughout the wider SGN.
