Crianças BE apresentaram diminuição no consumo dos seguintes nutrientes (mediana, percentil25/percentil75): carboidratos (g) [E=197,0(174,0/233,7) e BE=169,6 (154,6/200,1), p=0,04]; proteina (g) [E=53,1(42,5/60,5) e BE=36,8(32,5/56,3), p=0,04]; fibras (g) [E=12,0(9,8/13,9) e BE=36,8(32,5/56,3), p=0,01]; ferro (mg) [E=9,2(7,7/11,9) e BE=7,6(6,5/9,4), p=0,02]; e vitamina B12 (mg) [E=2,0(1,0/4,1) e BE=1,3(0,5/3,0), p=0,49], bem como menor consumo energetico (kcal) [E=1354,1(1214,6/1661,5) e BE=1198,4 (906,2/1351,4, p=0,02]. Também foi encontrada redução nos parâmetros antropométricos (valores médios ± desvio-padrão): peso corporal (kg) [E=29,5±1,0 e BE=20,7±0,7, p<0,001]; altura (cm) [E=133,7±1,0 e BE=118,8±0,9, p<0,001]; comprimento do tronco (cm) [E=68,3±0,5 e BE=60,7±0,9, p<0,001]; comprimento da perna (cm) [E=31,7±0,4 e BE=28,4±0,4, p<0,001]; circunferência da panturrilha (cm) [E=26,1±0,4 e BE=22,7±0,4, p<0,001]; e comprimento do pé (cm) [E=20,5±0,2 e BE=18,7±0,3, p<0,001]. A capacidade de produção de força voluntária (Nm) [E=19,8±1,3 e BE=13,1±0,9, p=0,01] e induzida (Nm) [E=3,3±0,3 e BE=1,9±0,2, p<0,01] foi diminuída nas crianças BE, , respectivamente, (E=73,2±4,6 e BE=57,7±4,0; p=0,04), inclusive após normalização com a circunferência da panturrilha (Nm m-1) (E=11,9±1,0 e BE=6,5±1,2; p=0,02). O atraso eletromecânico (ms) [E=9,8±0,1 e BE=10,6±0,3, p=0,006] durante a contração muscular foi maior nas crianças BE, assim como o índice de rigidez musculotendínea (MT), normalizado pelo torque (s-1) [E=69,1±4,2 e BE=87,2±4,4, p=0,01], que mensura a elasticidade muscular. Foi identificado aumento no índice de déficit de ativação muscular (% %-1) [E=0,26±0,02 e BE=0,46±0,04, p<0,001] no grupo BE e menor eficiência neuromuscular (Nm %-1) [E=3,6±0,3 e BE=2,3±0,5, p=0,03].

Full textOBJECTIVE: To conduct a comparative study between the arthroscopic technique (Tight - Rope® system with a single implant) and the mini - invasive technique (Twin Tail® System) for the treatment of acute acromioclavicular dislocations. MATERIALS AND METHODS: Acute acromioclavicular dislocations attended by doctors in the Arcos del Mar Menor Hospital during the period from January 2010 to May 2013 inclusive. During the patients follow up monitoring (between 1 and 4 years) all of them underwent stress radiographs (Zanca and Alexander projections) including the healthy side. All patients completed the Constant test (CS), the Simple Shoulder Test (SST), Arm Disabilities test, Shoulder and Hand test (DASH) as well as a specific test of the acromioclavicular joint known as the Acromioclavicular Joint Instability test ( ACJI ). The appearance of complications, instability and the statistical relationship between the various clinical and radiological parameters have all been recorded. RESULTS: Fifty-nine patients (45 men and 14 women) with an average age of 26.79 ± 10.26 have met the inclusion criteria and were evaluated (average follow-up period of 26.48 ± 12.28 months). Thirty have been operated on using the mini-invasive system (average operating time in minutes 80 ± 12.5) and twenty using the arthroscopic system (average operating time in minutes 45 ± 8.3). The right side was affected in 45.8% of cases with the most common cause of injury being sports accidents with 55.9% of cases, followed by traffic accidents with 30.5%. The most common Rockwood type has been the V type with 27 cases followed by type III with 26 cases. The average distance acromioclavicular on the healthy side was 3.88 ± 2.66 mm and 11.16 ± 6.24 mm on the pathological side. The average coracoclavicular distance on the healthy side was 8.22 ± 1.27 mm and 15.42 ± 4.85 mm on the pathological side. The average horizontal distance on the acromioclavicular healthy side was 1.59 ± 1.25 mm and 7.02 ± 4.1 mm on the pathological side. The arthroscopic system obtained values of 95.65 ± 5.25 in the CS test, 11.28 ± 0.67 in the SST test, 7.57 ± 5.24 in the DASH test and 88.65 ±12 in the ACJI test, offering a 55% degree of instability in the vertical plane and 72.4% in the horizontal plane. The mini-invasive system obtained values of 97.56 ± 3.16 in the CS test, 11.86 ± 0.34 in the SST test, 1.98 ± 3.17 in the DASH test and 94.16 ± 4.92 in the ACJI test, presenting a 26.6% degree of instability in the vertical plane and 56.6% in the horizontal plane. We have had to re-assess eleven patients (18.6%), ten of which after the arthroscopic system. The most common reason for re-assessment was the recurrence of the deformity and symptomatic osteoarthritis of the acromiclavicular joint. The most frequent complication was the appearance of calcifications (28.8%) followed by degenerative changes (18.8%). We found no relationship between the mechanism of production and sex with the onset of complications. We have detected that the left side is more complicated than the right side. There is a positive correlation between the vertical and horizontal displacement of the clavicle. 87.95% of patients have been satisfied or very satisfied with the results of their surgery. CONCLUSION: The mini - invasive treatment is better than the arthroscopic treatment of acute acromioclavicular dislocations since it achieves a more stable and long-lasting articular result upon the dislocation, better scores in the clinical tests and fewer complications and a lower reoperation rate.

Full textPlantar fasciitis is a common and painful degenerative condition. There are commonly used treatments, but the evidence base underpinning these treatment strategies is often unknown. Customized functional Orthosis and Extracorporeal Shock Waves (ESW) are evidence based. We want to determine the clinical and functional daily life activity benefits once the treatment has finished and along the three-month follow-up when associating both therapies, and to detect the main side effects. METHODS: Prospective and randomized study. Forty patients with plantar fasciitis were treated. Thirty were treated with customized foot orthosis associated to ESW. Ten were treated with ESW only. We measured both pain upon palpation in the internal calcaneal tuberosity and pain in the first footsteps after resting by means of the ten-point Analogic Visual Scale. Functional Daily Life Activities were measured by the FAAM Questionnaire (Foot and Ankle Ability Measure). All measures were acquired when therapies finished, with follow-ups one month and three months afterwards. RESULTS: Pain improved in all patients after three months of follow-up, although that improvement was only evident throughout those three months when both treatments were combined. Likewise, function improved only when both treatments were associated. No relevant adverse events occurred in either intervention group. Pain was the most frequently secondary event while ESW were administered, although it was not necessary to abandon treatment in any case. We detected one single case of Complex Regional Pain Syndrome. This event was unknown in the literature after treatment with ESW.

Full textTo check the security and assess the effectiveness, of the use of mesenchymal cells of human bone marrow (MSC) seeded and cultured on tricalcium phosphate porous matrix (FTC) associated with demineralized bone matrix (MOD) implanted in immunosuppressed mice NOD/SCID. Specific objectives: To study the acute and chronic toxicity of the implant (FTC+MSC+MOD) construct in muri-ne NOD/SCID model. To study the biodistribution of the implant (FTC+MSC+MOD) construct in murine NOD/SCID model. To evaluate the formation of bone tissue at the site of the (FTC+MSC+MOD) construct im-plant. To evaluate the cronology of bone tissue formation at the site of the (FTC+MSC+MOD) construct implant. Methodology Analytical prospective study with 30 NOD/SCID mice divided into 2 treatment groups was performed. In the first group of 25 mice, a construct of mesenchymal stem cells seeded and cultured on tricalcium phosphate porous matrix and associated with demineralized bone matrix (MSC+FTC+MOD) was implanted subcutaneously and intramuscularly In the second group of 5 mice, a construction of 0,9% saline, matrix porous tricalcium phosphate associated with demineralized bone matrix (SSF0,9%+FTC+MOD) was im-planted also subcutaneously and intramuscularly Male and female mice pairs of the first group were - sacrificed at 24 h, 48 h, 1 week and then 2,5,7 and 9 weeks after surgery. The second group of mice and the 11 remaining mice of the first group were sacrificed at 12 weeks. Acute toxicity was evaluated by the assessment of - weight, the animal welfare score, blood and biochemical analysis and histological studies of organs: liver, lung, spleen, brain, kidney, heart, gonads, bone marrow and bone of each mouse sacrificed in the first 48h after surgery. Chronic toxicity was evaluated by the same studies sacrificing 2 mice in the 1st, 2nd, 5th, 7th and 9th week, eleven mice in the 12 th week, and the remaining 5 of the second group. Biodistribution of the construct was analysed by RT-PCR test of 2 proteins, actin and the β2microglobulin of liver, lung, spleen, brain, kidney, heart, gonads, bone marrow and pe-ripheral blood from each mouse sacrificed. Effectiveness was evaluated at implanted site by qualitative and quantitative studies Im-munohistochemistry with anti-osteocalcin antibodies and histological studies with hema-toxylin-eosin, Masson's trichrome were performed. Conclusions Construct (FTC+MSC+MOD) implanted via subcutaneous or intramuscular has not pre-sented any signs of acute or chronic toxicity in the mouse model NOD / SCID. Human mesenchymal stem cells have not been found in any organ analyzed at murine model of Construct (FTC+MSC+MOD) implanted subcutaneously or intramuscularly . These have remained at the implant site. No statistically significant differences were observed between the osteogenic power of the construct MSC + FTC + MOD. However, it has definetely been found between subcutane-ous and intramuscular implant. Structured bone formation is observed from week 7 in the subcutaneous implant and from week 9 in the intramuscular implant.
Molina, Prats Patricia. "Efecto de la apigenina potásica sobre un modelo de mucositis oral inducida por 5-fluorouracilo en hámster." Doctoral thesis, Universidad de Murcia, 2015. http://hdl.handle.net/10803/312157.
Full textINTRODUCTION Mucositis is an important side affect of cancer therapy, occurring in nearly 20% to 40% of patients receiving conventional chemotherapy, 80% of those under high-dose chemotherapy as conditioning for hematopoietic stem cell transplantation and nearly all patients receiving head and neck radiation therapy. Tissue repair complexity may be influenced by different factors. Naturally occurring flavonoids, like apigenin, have been shown to possess anti-inflammatory properties related to their antioxidant and free radical scavenging ability. PURPOSE General objective: - The aim of this study was to evaluate the effect of administration of apigenin potassium or dexamethasone on an animal model of oral mucositis induced by 5-fluorouracil chemotherapy. Specific objectives: - Replicate the experimental model of oral mucositis in hamsters by the systemic application of 5-fluorouracil chemotherapy. - Evaluate clinically the developed oral lesions and its evolution with topical administration of potassium apigenin or dexamethasone, and study the changes at different times of sacrifice (5, 7, 10 and 14 days). - Analyse by anatomopathological study the area of oral mucositis, and evaluate oral changes at 5, 7,10 and 14 days of application potassium apigenin or dexamethasone. MATERIALS AND METHODS Seventy-two male Syrian hamster were divided into three groups, and 72 samples were obtained: control (50% acetic acid + 5-FU); 50% acetic acid + 5-FU + Potassium Apigenin (KA) and 50% acetic acid + 5-FU + Dexametasone. Six animals from each group were sacrificed 5, 7, 10 and 14 days after inducing the mucositis; ulcers were assessed by imaging analysis using a scoring system (Cho, 2006). The number of inflammatory cells in the ulcerated region was quantified in all periods through histomorphometric analysis (H&E). RESULTS The healing process of the control group was slower than that of KA and Dexametasone-treated groups. There was significant difference between KA and control group in the 10-day period. Lower quantity of inflammatory cells in the KA treated group in comparison to control group in the 7- and 10-day periods was observed (p<0.05). No statistically significant difference was verified among the groups in 5- and 14-day periods. CONCLUSIONS After having established the objectives and the results analised, the conclusions of this study are: 1. Application of 5-fluorouracil and acetic acid resulted, in all animals of the study, in the occurrence of oral mucositis in the treated area, confirming the suitability of the used animal model. 2. Treatment with potassium apigenin caused the improvement of ulcerative lesions compared to the control group,in both the clinical assessment and the histomorphometry, where there were statistically significant differences on tenth day. 3. The effect of apigenin potassium in the model used was similar and even superior in some ways and times, to the steroid dexamethasone, suggesting that it could be a useful natural origin agent for the treatment of oral mucositis.

Full textIn Western societies, the increase in life expectancy and the phenomenon of demographic investment are generating an increase in older sectors. It is therefore a priority to research new ways of mitigating the suffering of elderly people, which can limit the quality of life through fatigue and cognitive impairment, amongst others. The aim of this paper is to analyze the effect of L-Carnitine on fatigue and cognitive function in a group of institutionalized elderly. We selected thirty-three elderly in a geriatric institution, which were distributed in two homogeneous groups, to receive placebo or L-Carnitine at a dose of two grams per day for four months. We performed three evaluations: at the beginning, the second month and the fourth month. Plasma levels of L-Carnitine (total and free) were determined, and the following assessment questionnaires were applied: the Wessely-Powell questionnaire on the nature of fatigue, the Krupp questionnaire on fatigue severity and the Pfeiffer questionnaire on cognitive status. In addition, we decided to include the Katz index for functional assessment and a short test for geriatric depression screening. We found that L-Carnitine deficiency was present almost a third of our population. The final free L-Carnitine plasma levels only increased significantly in males in the experimental group, with respect to men in the placebo group. The total L-Carnitine values were also increased in the male experimental group as well as female placebo group. With regard to fatigue, the L-carnitine group women were the only ones that reduced the nature of fatigue, (but not statistically significant), and that maintained values in the severity of fatigue, whilst the other groups worsened. A trend towards improved cognitive functions was observed in in the L-Carnitine group, against a clear significant worsening in the case of the placebo group women. Functional assessment remained unchanged in all groups throughout the study, and depression screening showed a significant worsening in the placebo group, with a slight improvement in the male experimental group. We conclude that carnitine deficiency is a prevalent situation in the elderly, oral administration is well tolerated and that it could reduce both the degree of fatigue, as happened with women, such as cognitive impairment

Full textABSTRACT The main contribution of this study is the characterization of the state of health of a collective with spina bifida, with an age range between 1 and 12 years. Objectives: It was considered the typification of the diet, by the description of the macro and micronutrients intake, to compare subsequently the diet of the infant-juvenile population affected by spina bifida with the recommendations given for the Spanish infant-juvenile population; and finally to know the perception and personal experiences of fathers and mothers about the disability of children with spina bifida in the Region of Murcia. Methodology: This descriptive transversal study had two sampling periods: May-July 2009, May-July 2011. The number of participants was 19 in the 1st sampling, belonging to AMUPHEB; and 12 in the 2nd, belonging to associations of spina bifida all over Spain. In both cases the parents were given the dietary history of 15 days, Kreceplus test for food habits, Kreceplus test for physical exercise and questionnaire of sociodemographic characteristics, made ad-hoc by the research staff. The intake of macro and micronutrients were determined and the percentage of individuals according to their ages that fulfilled such daily intake recommendations. Regarding the physical activity, the largest proportions were calculated, by age, of number of hours of physical exercise per day as well as the time spent daily with the television and video game console/s. In order to determine the food habits, 7 items were assigned for the food consumption, as well as 4 for the breakfast and 4 for the preferences; calculating the number of subjects according to their age who fulfil the condition. From these results it was diagnosed the proportion of individuals for each group which was in each nutritional level (low, medium, high). Finally, in the test of sociocultural characteristics, it was determined the percentage and the proportions of individuals who lived in the same home and place of residence. Lastly a study of qualitative design was carried out (May 2014) in order to explore the perceptions, expectations and experiences about the feeding, the physical exercise, stigmatization and perception of weight, by an interview in depth. 14 children with spina bifida from the Region of Murcia took part (ages 8-18); delimiting such sampling by information saturation. The collection of data was done by an interviewer previously trained, establishing a series of open questions by means of a semi-structured interview; they were registered and transcribed literally. They were characterized according to the subjacent categories stablished and the systematization on the analysis was carried out in several stages. Conclusions: It is concluded that the infant-juvenile population with spina bifida from 1 to 12 years, object of this study, does not comply with the nutritional parameter of the Mediterranean Diet. It presents low consumption of fresh fruit, fresh vegetables as well as cereals and legumes; high consumption of processed baked goods, candies and sweets. The daily consumption of energy, carbohydrates and fibre was lower than the recommended amount; high on proteins. The dairy intake of folic acid and iodine were anomalously low, being anomalously high the sodium one. Practically, the totality of the population presents overweight and/or obesity to a greater or lesser degree. Regarding the sociodemographic characteristics, most of the population lived in rural municipalities and the predominant family model was non-extended nuclear family. The totality of the population exhibited a sedentary pattern, devoting a great amount of their free time to sedentary leisure and doing less than 2 daily hours to physical activities. The fundamental characteristic of our sample is that they suffer continuous hospitalizations and medical visits to different specialists, to treat the diverse functional complications that are inherent to their condition. As a consequence they present high rates of school absenteeism, taking part in less school and out-of-school activities, feeling rejected and excluded. They presented problems and difficulties in socialization, solitary conducts and social isolation. The experience of the parents is that the school and urban physical environment are not sufficiently adapted to the needs of children with this physical disability and there is scarce social awareness. They require social sensitivity of their condition (spina bifida) and full inclusion in society without any restriction. Key words: spina bifida, split spine, infant-juvenile, test Kreceplus, daily intake, food consumption frequency, nutritional pattern, Mediterranean diet, passive leisure, sedentarism, rural, non-extended nuclear family, socialization problems, solitude, social isolation, school absenteeism, social awareness, adapted spaces.

Full textAnatomo-radiological correlation of human temporomandibular joint: Evaluation of the relationship of the lateral pterygoid muscle with different joint elements Introduction: Temporomandibular joint (TMJ) is subject to multiple human clinical studies because is a complex anatomical region and suffers Temporomandibular dysfunction. There is controversy about the morphology and function of the joint not only from its embryological origin, but also by the characteristics of its different elements. From a clinical point of view, it is essential to know the morphology of this articulation, for which is often used imaging techniques such as computed tomography (CT) and especially magnetic resonance imaging (MRI). Projections most used are those in the sagittal, axial and coronal planes and also projections on special planes (oblique-sagittal and oblique-coronal). Plastination techniques are an excellent tool for displaying as much anatomical detail and allow the correlation with CT images and MRI. The aims of this thesis are to evaluate the morphological characteristics of the various elements of the TMJ, especially the insertion of the lateral pterygoid muscle in the joint, and its radiological correlation and assess the usefulness of different MRI projections. Material and Methods: Twenty-four human TMJ from eighteen individuals have been used. MRI was performed in 7 of them in the axial, oblique-sagittal (perpendicular to the long axis of the condyle and parallel to the fibres of the upper bundle of the lateral pterygoid muscle) and oblique-coronal. Four of them were cut in the same planes as MRI and the sections were plastinated (technical E12). Other 3 different TMJ joints were studied by CT in the same planes as MRI. Later they were cut and processed histologically using hematoxylin-eosin, orcein and Masson trichrome staining. The remaining pieces (17) were dissected. Results and conclusions: The findings shows that sectional anatomical study has displayed that there are two different capsules. By E12 plastination technique it has been identified new joint recesses: posterosuperior and superomedial. In the sections in the oblique-coronal plane the greater thickness of the medial part of the disc is appreciated. The different techniques used in all sections, and mainly in the oblique-sagittal plane, have allowed to observe that the upper fascicle of the lateral pterygoid muscle is inserted in the anterior region of the upper and lower sheets of the capsule, in the front of the underside of the disc, in the condyle and the pterygoid fovea. Fibres of both bundles converge in a single tendon to be inserted laterally into the pterygoid fovea, while the rest of the lower fascicle of the lateral pterygoid muscle is inserted into the neck and mandibular branch, descending until the vicinity of the lingula. The different techniques used in the oblique-sagittal and oblique-coronal planes have shown to be a multipennate muscle. The oblique-sagittal plane, parallel to the fibres of the lateral pterygoid muscle, allows more accurately assess the morphology and insertion of the muscle in the different elements of the TMJ. Histological and E12 plastination techniques with MRI images in the different sections have made it possible to appreciate the pterygoid venous plexus and how it is between the two sheets of capsule, along the periphery of the disc with mutual relationships with the fibres of insertion of the lateral pterygoid muscle. It is important performing imaging studies in both oblique-coronal plane and in oblique-sagittal planes, (especially those parallel to the lateral pterygoid muscle fibres) because they provide complete visualization on the details of the anterior part of the TMJ.
