Full textObjective: According to the divergences of informations in the evaluation of the masticatory function as for the investigated aspects and the form of evaluation of them, as well as the need of standardization for comparison of the patients' masticatory function; it was necessary to elaborate a proposal of clinical evaluation of the masticatory function. Material and Method: A questionnaire was elaborated and sent by e-mail for some Speech Pathologists from several places from Brazil. This questionnaire had some questions about the formation and the professionals' performance and informations about the aspects that were investigated in the clinical history of the masticatory function and in the evaluation of this function as for the morphologic and functional aspects that were investigated and the form of evaluation of the masticatory function, besides the verification of other oral functions. Seventy questionnaires were answered and analyzed and they were used for the elaboration of the proposal of evaluation besides the analysis of the literature and experience of the Speech Pathologists of the Laboratory of Physiology at HRAC/USP. Results: From the analyses it was proposed the elaboration of a protocol of evaluation of the masticatory function, contemplating the following aspects: the test foods, the amount of the test foods, the teeth that bite the food, the mandibular movement, the initial side of the masticatory function, the masticatory laterality, the participation of the perioral muscles, the labial posture, if exists tremor, the contraction of muscles masseteres, the coordination of the mandibular movements, if there is a leak of food from the mouth, the trituration of the food with the tongue, the head's movement, the place of the trituration of the food, the breathing during the mastication, the noises in ATMs, the duration of masticatory function, the number of masticatory cycles, the formation of the bolus, if there is cough or choking and pain during the mastication, besides the test of masticatory efficiency. Final considerations: It was verified the lack of standardization of the evaluation of the masticatory function related to the aspects to be investigated, as well as the form of evaluation of them. For this reason, this work proposes a protocol for the clinical evaluation of the mastication and it will contribute with the Speech Pathology to bring criteria for the evaluation of the masticatory function.

Pacientes com paralisia cerebral se deparam com limitações funcionais motoras na mastigação, deglutição e fonação. É imprescindível compreender o comportamento da musculatura mastigatória para direcionar o diagnóstico e reabilitação oral desses pacientes. A eletromiografia de superfície é uma técnica diagnóstica empregada para a avaliação clínica da atividade muscular. O objetivo deste estudo foi avaliar a função motora oral de pacientes adultos com paralisia cerebral, mediante analises clínica e eletromiográfica. Foram selecionados 30 adultos com paralisia cerebral, de ambos os sexos, entre 20 e 59 anos, com capacidade de compreender e responder a comandos verbais. Após anamnese e exame clínico, a função motora da mandíbula, língua, lábios, palato mole e laringe, bem como expressões da mímica facial foram avaliados. De acordo com o escore alcançado por cada paciente ao realizar os movimentos orais, o comprometimento motor oral foi classificado nas intensidades leve, moderado e severo. O exame eletromiográfico da musculatura mastigatória foi realizado, bilateralmente, nos músculos masseter, temporal e supra-hióideos nas posições de repouso, isometria e abertura. Um transdutor de força e um goniômetro foram utilizados para a medição de força e abertura bucal máxima, respectivamente. A correlação entre os resultados clínicos e eletromiográficos foi avaliada pelo Teste de Friedman, Mann-Whitney e Wilcoxon. O grau de comprometimento motor oral foi moderado em 60% dos pacientes, severo em 36,66% e leve em 3,33%. As atividades elétromiográficas dos músculos estudados nas posições de repouso, isometria e abertura apresentaram respostas individuais, não se estabelecendo relação com os diferentes graus de comprometimento motor oral. O presente trabalho teve por objetivo avaliar o reposicionamento mandibular em pacientes assintomáticos e pacientes com sintomatologia dolorosa dos músculos mastigatórios. Com esta finalidade, foi utilizada uma amostra de 24 pacientes, divididos em 2 grupos (I e II). Para o grupo de casos (I), foram selecionados de forma aleatória 12 pacientes do sexo feminino, todos sintomáticos, totalmente dentados. Para o grupo controle (II), indivíduos do sexo feminino totalmente dentados, sem sinais e sintomas de desordem do sistema mastigatório. Para verificar o reposicionamento, após os modelos montados em máxima intercuspidação habitual (MIH) no articulador semi-ajustável, confeccionamos para cada paciente uma plataforma de registro, uma pua, um JIG de Lucia e uma placa oclusal miorrelaxante (POMR).Na plataforma de registro, realizaram-se marcações após o uso do Jig por quinze minutos e a POMR por sete e quinze dias , tendo como ponto de referência a MIH do Paciente. O que pode ser observado foi que o JIG por 15 minutos conseguiu promover um deslocamento mandibular posterior, igual ao uso da placa oclusal miorrelaxante por sete e quinze dias nos dois grupos. A média de deslocamento para o grupo I foi de 1,28mm (s=1.5), para o grupo II a média foi de 0.4mm (s=0.7), nível de significância de 95%.. Os resultados permitem concluir que não houve diferença significativa no reposicionamento final observado após o uso do JIG e da PMOR.
The focus of this study was to mensure the jaw replacement in assymptomatic pacient and pacient with masticatory muscles pain symptomatology. A sample composede of twenty four patients were divided in two groups I and II. The case group(I), was composed by twelve symptomatic, fully teeth , females individuals.The control group (II), was composed by twelve non symptomatic, fully teeth, females individuals. To check the jaw replacement, the cast was mounted on semi-adjustable articulator. Over the cast mounted were manufactured a horizontal plataform , a device worked, a jig of Lucia and a splint. The displacement of the position mandible after the use of the Jig (during 15 minutes) and after the used of splint ( 24 h/ day controlled 15 days). The distances between the marks were measured in a microscopy and the results were statistically analized. The use of the Jig( during 15 minutes), was able to reposit the jaw as the splint( 24 h/ day contolled 15 days) in both group. The replacement average to the group I was 1.28mm and for group II was 0.40mm. The result suggests that jaw replacement after using the Jig and the splint on the two group present similar behavior.

Resumo: Este estudo buscou identificar as possíveis relações dos padrões antropométricos nutricionais com as caracrerísticas morfológicas e funcionais do sistema mastigatório e níveis de biomarcadores salivares. Três estudos foram conduzidos, apresentados na forma de capítulos. As amostras foram compostas por sujeitos saudáveis, com idade entre 15 e 30 anos. Capítulo 1: Objetivou comparar a antropometria e a composição corporal e verificar se o padrão antropométrico nutricional estaria associado à disfunção temporomandibular (DTM). Foram avaliados 110 mulheres e 60 homens distribuídos em grupos controle, desordem muscular (MD) ou deslocamento de disco (DD), de acordo com o Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). As medidas antropométricas foram peso, altura e perímetros da cintura, abdomen e quadril. A composição corporal foi avaliada por meio da porcentagem de gordura corporal (BF%), e das massas adiposa, óssea, muscular e residual. A intensidade da dor (CPI) e os sintomas físicos não específicos incluíndo dor facial (NSPSP) foram considerados. Abstract: The aim of this study was to identify possible associations of nutritional anthropometric patterns with morphologic and functional characteristics of the masticatory system as well as with salivary biomarkers. Three studies were developed and reported in three chapters. Samples consisted of health subjects aged between 15 and 30 years. Chapter 1: To compare body characteristics and composition and to verify if nutritional anthropometric patterns are associated with temoromandibular dysfunctions (TMD). 110 females and 60 males were classified into Control, Muscle disorders (MD) or Disc displacements (DD) groups according to the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Anthropometric measurements recorded were weight, height and perimeters of waist, abdomen and hip. Body composition was evaluated with values of body fat percentage (BF%) as well as with values of fat, muscle, bone and residual mass. Full textThe stomatognathic system is a set of oral structures, organs and tissues that develop common functions, interdependent, and as a fundamental characteristic for the morphologic and functional development, especially the chewing, two important factors: the development and craniofacial muscle. Like any system, has features of its own, but is closely linked to the function of other systems like the nervous, circulatory, endocrine, and everyone in general because it is a separate unit from the rest of the body, but integrates the him. Several studies about the growth of the facial skeleton, however, little is known about the development of the muscular system of the head, in view of the difficulty associated mainly with methods of study. The muscles are developed, morphologically and functionally, in a planned and organized, but there are still many doubts and questions how that occur, particularly during the aging of the human being. This paper seeks evidence of muscle interaction in craniofacial development over the years and aims to analyze the electromyographic activity of masseter and temporal muscles of children, youth, adults and seniors, aiming to draw up criteria and parameters that can be considered normal in masticatory muscle development in relation to age and gender. The study focused aim to facilitate diagnostic and prognostic, bringing contributions to the quality and stability of treatments, as well as establishing patterns of normality.

Full textFibromyalgia syndrome is characterized by chronic and diffuse muscle pain in at least eleven points of the human body (tender points), morning stiffness, fatigue, sleep disturbances, extensive pain symptoms in the face, which is mainly manifested in the masticatory muscles, temporomandibular joint and ear region. The aim of this study was to analyze the masticatory (masseter and temporal) and cervical (sternocleidomastoid and trapezius) muscles in women with diagnosis of fibromyalgia syndrome. Sixty-eight women aged between 40 and 60 years participated in this research. They were divided into two distinct groups: FG: fibromyalgia syndrome (n = 34) and CG: healthy control (n = 34). The Myosystem-Br1 electromyography was used to analyze the EMG activity. The thicknesses of the masticatory and neck muscles were acquired by ultrasound Sonosite Titan. The maximum molar bite force was obtained using a digital force gauge Kratos. The data of EMG activity, muscles thickness and maximal molar bite force were tabulated, standardized and subjected to statistical analysis (SPSS 21.0). There were statistically significant differences (p<= 0.05) between FG and CG at all mandibular postural conditions, chewing, muscle thickness and molar bite force, with lower average data for FG. Based on the results of this research, the authors concluded that women with fibromyalgia syndrome have alterations in the stomatognathic system

The aim of the study was to investigate whether children with cleft lip and palate (CLP) have altered control of masticatory muscles. The muscular activity of 66 children with CLP (n=33) and without CLP (n=33), aged between six and 12 years was assessed during different tasks (rest, isometric, mastication) using electromyography. Amplitude of Muscle Activation (AMA) of the analyzed muscles was higher in children with CLP at rest (R) and inactive period (IP) (p <0.05), since the active period (AP) and isometric (I) was higher in children without CLP (p <0.05), except for the right masseter muscle in AP, which although less activation did not obtain significant results. Also found that the muscles of children with CLP remained active for longer than those without CLP (p <0.05) and length of the masticatory cycle was higher in children with CLP (p <0.05). The muscles of children with CLP in the R and IP of mastication are superior when compared to the control group. Already during the AP and I are reduced activation. Children with CLP have longer muscle activation during the chewing cycle and increased cycle time, which probably influences the occlusion, resulting in greater difficulty in chewing.
O objetivo do estudo foi investigar se crianças com fissura labiopalatal transforame incisivo unilateral (FLPU) possuem alterações no controle da musculatura mastigatória. A atividade muscular de 66 crianças sem fissura (n= 33) e com FLPU (n= 33), entre seis e 12 anos foi avaliada durante diferentes tarefas (repouso, isometria e mastigação) utilizando eletromiografia. A Amplitude de Ativação Muscular (AAM) dos músculos analisados foi maior nas crianças com FLPU no período de repouso (R) e período inativo (PI) (p <0,05). Já no período ativo (PA) e isometria (I) foi maior nas crianças sem FLP (p <0,05), com exceção do músculo masseter direito no PA, que apesar de menor ativação não obteve resultados significativos. Observou-se que os músculos das crianças com FLPU permaneceram ativos por mais tempo (p <0,05) e a duração do ciclo mastigatório foi superior nas crianças com FLPU (p <0,05). A atividade muscular da criança com FLPU no R e PI da mastigação é superior quando comparado ao grupo controle. Já durante o PA e I a ativação muscular é reduzida. Crianças com FLPU apresentam maior tempo de ativação muscular durante o ciclo mastigatório e maior duração do ciclo, o que provavelmente influência na oclusão, resultando em maior dificuldade na mastigação.

Full textThe definition of reference values of healthy subjects is a fundamental parameter for the diagnosis in health. The aims of this study were to establish reference values based on standardized score for the orofacial myofunctional condition, for the orofacial strength and for the masticatory efficiency, and to analyze the relationships between them, after establishing a performance index for orofacial strength and masticatory efficiency. From a total of 316 subjects, 50 participants were selected, 21 men and 29 women, ranging from 18 to 40 years old, without temporomandibular disorder, no orofacial myofunctional disorder and presenting normal occlusion. Subjects were assessed using the Orofacial Myofunctional Evaluation Protocol with Scores (OMES) to investigate the orofacial myofunctional condition. The orofacial strength evaluated were maximal isometric bite forces on the right and left sides, maximal isometric cheeks strength on the right and left sides, maximal isometric tongue strength on the anterior and dorsum region and maximal isometric lips strength using an electronic dynamometer Kratos®. The values were recorded in Newton (N). The mean value of three trials of each orofacial strength was obtained. The masticatory efficiency was analyzed by colorimetric method with beads. The fuchsine concentration in micrograms per milliliter (µg/ml) was obtained. The masticatory efficiency was evaluated in three conditions: habitual, right and left unilateral chewing, for 20 seconds each one. For all variables, the Z scores were calculated to determine the reference values and cutoff values that differentiate the normal and abnormal subjects. The orofacial strength and masticatory efficiency values were transformed into the performance index, for 34 participants that were defined as normal subjects. The random error was calculated for all variables. Statistical analysis was performed using Spearman correlation test, using the MedCalc® program and the level of significance was p<0.05. The reference values (mean and standard deviation), established from the Z score, were 95.79 ± 3.74 for the orofacial myofunctional condition; 431.08 ± 138.01N for the maximal isometric bite force; 17.92 ± 7.44N for the maximal isometric cheeks strength; 9.45 ± 4.01N for the maximal isometric tongue strength in the anterior region; 13.44 ± 5.46N for the maximal isometric tongue strength in the dorsum region; 4.31 ± 1.48N for the maximal isometric lips strength; 0.72 ± 0.31µg/ml for the habitual masticatory efficiency; 0.71 ± 0.30µg/ml for the right masticatory efficiency, 0.78 ± 0.37µg/ml for the left masticatory efficiency. No correlations were found neither between the orofacial strength performance index and masticatory efficiency performance index (r=0.102, p>0.05), the orofacial strength performance index and orofacial myofunctional condition (r=-0.005, p>0.05), nor between the masticatory efficiency performance index and orofacial myofunctional condition (r=0.059, p>0.05). There was no relationship between the biomechanics orofacial and masticatory efficiency in this study.

Full textThe temporomandibular disorders (TMD) includes changes related to the temporomandibular joints (TMJ), masticatory muscles and other structures of the stomatognathic system presents itself as pain in the masticatory muscles, joints, cervical muscles, cuts and deviations in the movements of jaw joint sounds and patterns of inadequate occlusion. It is of major importance to study the masticatory muscles because of the great influence which it has in the stomatognathic system. The objective of this study was to analyze the thickness of the masseter and temporal bilaterally by ultrasonography in patients with TMD muscle, in order to set the standard for this thick muscle dysfunction. The results of this study will allow assessment of the stomatognathic system\'s functional point of view in individuals with TMD muscle and can guide the dentist in making diagnoses, prognoses and treatment plans, with gains in quality of life for their patients. Therefore it is concluded that at rest as in clenching in maximum intercuspation was a thinning of the left masseter, temporalis left and right for the group with muscular TMD. And with respect to gender there was a decrease in muscle thickness for both the masseter and temporalis muscle for in women.

Resumo: Este estudo teve como objetivo investigar o comportamento da atividade eletromiográfica durante a função mastigatória, em pacientes portadores de alterações funcionais do sistema estomatognático, submetidos à terapia por meio de aparelhos interoclusais planos. Para a realização das avaliações eletromiográficas foi utilizado o sistema de diagnóstico computadorizado K6 - I DIAGNOSTIC SYSTEM (Myo-Tronics, Inc.; Tukwila, W.A.; USA). Foram examinados e tratados vinte voluntários, com idade entre 18 e 53 anos, de ambos os sexos. As avaliações eletromiográficas para a verificação da atividade elétrica dos músculos masseter e temporal fascículo anterior, foram realizadas antes do tratamento e após 90, 120, e 150 dias de utilização dos aparelhos interoclusais planos, sendo que estes aparelhos receberam modificações nos dias 90 (guia em canino) e 120 (guia em grupo) respectivamente. Os dados coletados de cada fase do estudo foram submetidos a uma análise estatística através de um modelo de egressão linear simples. Após análise dos resultados obtidos, não se observou diferenças estatisticamente significativas para as três fases do estudo (p>0,05), porém pode-se observar que a atividade eletromiográfica dos músculos masseter e temporal anterior, tenderam ao equilíbrio bilateral no decorrer do tratamento
Abstract: This study investigated through computerized electromyography (EMG) the effectiveness of interocclusal appliances ín assessing patients with functional changes in the stomatognathic system. Twenty volunteers, aged 18-53 years, both genders, presenting with signs and symptoms of functional changes ín the stomatognathic system, were examined and treated. Electrical activity of the masseter and anterior temporalis muscles was measured and recorded before treatment and 90, 120, and 150 days postoperatively. Appliances were modified at day 90 (Canine guidance) and 120 (group guidance). Data were statistically analyzed using a simple linear regression mode!. A bilateral balance during treatment was obseryed for the EMG activity. of the masseter and anterior temporalis, under masticatory function; however; no statistically significant difference was observed.

The increased masticatory muscle activity is considered a physical response to changes in emotional state. Stress and anxiety increase the myoelectric activity of the stomatognathic system muscles, with resulting parafunctions increase, as teeth clenching. Bruxism is a diurnal or nocturnal parafunctional activity including clenching or teeth grinding. Bruxism is strongly associated with the masticatory myofascial pain, a chronic facial pain of the masticatory muscles with possible blockage and limited mouth opening. The endocannabinoid system consists of cannabinoid receptors by the endogenous ligands (endocannabinoids) and proteins responsible for its metabolism. Endocannabinoids are involved in different physiological and pathological functions. The antinociceptive action of cannabinoids has been shown in several experimental models of pain, but there are no literature reports of cannabinoids involvement in nociception associated with muscle hyperactivity in parafunctional habits situations. The aim of this study was to study the masticatory muscle activity and nociception associated in emotional stress model in rats and the endocannabinoid system involvement. The study was approved by the Animal Research Ethics Committee of the Federal University of Ceará (N° 35/13). The communication box was used as stimulus instrument for emotional stress for 14 days. Subcutaneous electrodes were implanted in the masseter and temporal muscles region to evaluate the masticatory muscle activity and it was performed mechanical hyperalgesia test in these muscles during the emotional stress stimulation. Were also evaluated anxiety behavior parameters, body weight, adrenal glands gross weight and c-Fos expression and cannabinoid CB1 and CB2 receptors in the trigeminal ganglion and trigeminal spinal tract. Cannabinoid CB1 antagonist was administered and evaluated the nociceptive behavior. The expression of cannabinoid receptors in the masseter muscle was also assessed. The results showed a significant increase in stressed animals masticatory muscle activity on the seventh day of emotional stress stimulus. A significant reduction of masticatory muscles nociceptive threshold was observed, peaking on the eleventh day of emotional stress. The stressed animals showed anxiety behavior, with no differences in body weight or adrenal glands gross weight. The results also pointed that the stressed animals showed cytoplasmic vacuoles accumulation in the adrenal glands cortex, and increased expression of c-Fos and CB1 and CB2 receptors in the trigeminal ganglion and trigeminal spinal tract. The administration of the antagonist CB1 intensified the nociception on the fourth day of emotional stress. Cannabinoids CB1 and CB2 receptors have increased expression in the masseter muscle of the stressed animals. The data suggest that animals exposed to emotional stress have increased masticatory muscle activity and ‘brux-like’ behavior with a reduction in masticatory muscles nociceptive thresholds. Endocannabinoid system appears to be involved in the modulation of nociception induced emotional stress with increased expression of CB1 and CB2 receptors in the trigeminal ganglion, trigeminal spinal tract and masseter muscle.
O aumento da atividade muscular mastigatória é considerado uma resposta física de mudanças no estado emocional. O estresse e a ansiedade aumentam a atividade mioelétrica dos músculos do sistema estomatognático, com resultante aumento das parafunções, como apertamento dos dentes. O bruxismo é uma atividade parafuncional diurna ou noturna, incluindo apertamento ou ranger os dentes. Tem forte associação com a dor miofascial mastigatória, uma dor facial crônica dos músculos mastigatórios com possível bloqueio e limitação da abertura de boca. O sistema endocanabinoide é composto pelos receptores canabinoides, pelos ligantes endógenos (endocanabinoides) e proteínas responsáveis pelo seu metabolismo. Os endocanabinoides estão envolvidos em diferentes funções fisiológicas e patológicas, como imunomodulação, inflamação e analgesia. A ação antinociceptiva dos canabinoides tem sido demonstrada em vários modelos experimentais de dor, porém, não há relatos na literatura do envolvimento dos canabinoides na nocicepção associada à hiperatividade muscular em situações de hábitos parafuncionais. O objetivo da presente pesquisa foi estudar a atividade muscular mastigatória e a nocicepção associada em modelo de estresse emocional em ratos e o envolvimento do sistema endocanabinoide. O estudo foi aprovado pela Comissão de Ética em Pesquisa Animal da Universidade Federal do Ceará (nº 35/13). A caixa de comunicação foi utilizada como instrumento de estímulo do estresse emocional, durante 14 dias. Foram implantados eletrodos subcutâneos na região dos músculos masseter e temporal para avaliar a atividade muscular mastigatória e realizado teste de hipernocicepção mecânica nos referidos músculos durante o estímulo do estresse emocional. Foram avaliados também parâmetros de comportamento de ansiedade, peso corporal, peso bruto das glândulas adrenais e expressão de c-Fos e receptores canabinoides CB1 e CB2 no gânglio trigeminal e trato espinhal trigeminal. Foi administrado um antagonista canabinoide CB1 e avaliado o comportamento nociceptivo. A expressão dos receptores canabinoides no músculo masseter também foi avaliada. Os resultados demonstraram um aumento significativo da atividade muscular mastigatória nos animais estressados no sétimo dia de estímulo do estresse emocional. Observou-se uma redução significativa do limiar nociceptivo dos músculos mastigatórios, com pico no décimo primeiro dia do estresse emocional. Os animais estressados apresentaram comportamento de ansiedade, sem diferenças no peso corporal e no peso bruto das glândulas adrenais. Os resultados demonstraram ainda que os animais estressados apresentaram acúmulo de vacúolos citoplasmáticos no córtex das glândulas adrenais, e expressão aumentada de c-Fos e receptores CB1 e CB2 no gânglio trigeminal e trato espinhal trigeminal. A administração do antagonista CB1 intensificou a nocicepção no quarto dia de estresse emocional. Os receptores canabinoides CB1 e CB2 apresentam expressão aumentada no músculo masseter dos animais estressados. Os dados sugerem que animais expostos ao estresse emocional apresentaram atividade muscular mastigatória aumentada e comportamento bruxismo-símile, com redução nos limiares nociceptivos dos músculos mastigatórios. O sistema endocanabinoide parece estar envolvido na modulação da nocicepção induzida por estresse emocional com aumento da expressão dos receptores CB1 e CB2 nos gânglios trigeminais, no trato espinhal do trigêmeo e no músculo masseter.

Full textThis study estimated the interrater reliability and agreement of the somatosensory assessment performed at masseter and temporomandibular joint (TMJ) region in a group of healthy female and male participants. Forty healthy participants (20 men and 20 men) were evaluated in two sessions by two different examiners. Cold detection threshold (CDT), warm detection threshold (WDT), thermal sensory limen (TSL), cold pain threshold (CPT), heat pain threshold (HPT), mechanical detection threshold (MDT), mechanical pain threshold (MPT), wind-up ratio (WUR) and pressure pain threshold (PPT) were assessed on the skin overlying masseter body and TMJ. Mixed ANOVA, Intraclass Correlation Coefficients (ICC) and standard error of measurement (SEM) were applied to the data (=5%). Non-overlapping 95% confidence intervals (95% CI) of ICC values were considered significantly different. The ICC values of 77% of all QST measurements were considered fair to excellent (ICCs: 0.47-0.97). However, the reliability of WDT, TSL and HPT of masseter was significantly higher than TMJ, whereas the MDT reliability of TMJ was higher than masseter. In addition, the following combination of test/sites presented significantly lower ICCs for women: HPT, MDT of TMJ and MPT of both TMJ and masseter. Finally, the highest SEM values were presented for CPT and MPT. The overall somatosensory assessment of the masticatory structures performed by two examiners can be considered sufficiently reliable to discriminate participants. Nevertheless, possible site and sex influences on the reproducibility parameters should be take into account for an appropriate interpretation and clinical application.

Full textAmyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease that affects the motor neuron system preventing the skeletal striated muscles from functioning properly. The objective of this study was to analyze the masseter and temporal muscles of individuals with ALS by surface electromyography, masticatory function, and ultrasonography. Maximal molar bite force was also evaluated. Thirty individuals, aged 18 to 68 years old, were distributed into two groups: Amyotrophic lateral sclerosis group (mean age of 43.46 ± 3.67; n=15) and Control group (mean age of 43.33 ± 3.93 years; n = 15). A MyoSystem-I P84 electromyographer was used to evaluate the muscle activity in different conditions: rest, protrusion, right laterality, left laterality, tooth clenching in maximal voluntary contraction, with and without Parafilm M®. The efficiency of the masticatory cycle was analyzed by the integral of the signal envelope EMG during habitual and non-habitual mastication. Muscle thickness analyses were performed with a SonoSite NanoMax ultrasound tool. Acquisitions were made during rest, and maximal voluntary contraction. Maximal molar bite force (right and left sides) records were taken with a digital dynamometer Kratos. The electromyographic, ultrasound and maximal molar bite data were tabulated and submitted to statistical analysis using SPSS version 21.0 (Student t-test; p≤0.05). Statistically significant differences were found between the ALS group and controls during rest for the right (p = 0.03) and left masseter muscles (p = 0.03); protrusion for the left masseter (p=0.00) and right (p=000) and left temporal (p=0.03) muscles; during left laterality for the right (p=0.00), left masseter (p=0.00) and left temporal (p=0.00); and during nonhabitual mastication for the right masseter (p=0.00) and right temporal muscles (p=0.04). Based on the results obtained, the individuals with ALS showed different patterns of muscle activity and of masticatory function. Results differed for muscle thickness. In conclusion, ASL promoted functional changes in the stomatognathic system, including muscular hyperactivity and decreased masticatory efficiency and maximal molar bite force, which can affect the quality of life for individuals with ALS.

Full textThe stomatognathic system is a physiological and functional entity comprising a set of organs and tissues, whose biology and pathophysiology are absolutely interdependent. The tooth loss causes important changes in the masticatory system which affects bone, oral mucosa and muscle function. The masticatory performance deteriorates with tooth loss because it depends on the dentition, which includes the number of teeth present in functional occlusion, the number of posterior teeth in contact, the size of the functional areas of contact and degree of malocclusion. In order to have the rehabilitation of the stomatognathic system and consequently the masticatory function in patients with partial and / or complete tooth loss, important factors should be considered as muscle and prosthetic rehabilitation. Considering relevant the influence of the muscles in the rehabilitation of the stomatognathic system, the intention of this work was to evaluate the behavior of the masticatory muscles in different prosthetic rehabilitation. This study aims to analyze the electromyographic activity, the thickness of the masseter and temporal muscles, bite force and masticatory efficiency of adult individuals rehabilitated with different types of dental prosthesis, aiming to draw a pattern of muscle according to the different types of rehabilitation. These results allow full assessment of the stomatognathic system in terms of morphological and functional. The study focused aim to facilitate diagnosis and prognosis, bringing contributions to the quality and stability of treatments.

Full textRunning is described as an intense aerobic activity which currently involves millions of fans. Regular practice of this sport promotes changes in skeletal muscles striated fact which aroused interest in analyzing through the electromyographic ( EMG - Myosystem ) , its effect on the activity of the muscles of the stomatognathic system masseter and temporalis muscles in different positions mandible posture , plus its thickness by ultrasonography (SonoSite Titan portable Ultrasound Nationalized ) , as well as the maximum bite force using a digital dynamometer model IDDK Kratos - positioned in the region of the first molars . We evaluated 40 adults aged 18-65 years, of both genders without clinical alterations of the stomatognathic system divided into two groups: Group I Runners and Group II Control matched each subject. The average EMG were tabulated and subjected to statistical analysis using t Test (SPSS 19.0). The results showed significant differences (p < 0.05) in EMG activity for group of individuals practicing the street race occurring more functional muscle balance postures analyzed clinical. In the conditions in which this study was conducted the results indicate that the road race promoted significant changes in the masticatory system.

Full textHypertension is a multifactorial clinical condition characterized by high and sustained levels of blood pressure, associated with functional and / or structural organs and metabolic changes. The prevalence of hypertension in Brazil points an index above 30 % and this disease is considered a public health problem. The aim of this study was to evaluate the stomatognathic system in treated and controlled hypertensive individuals, compared to a control group using electromyographic analyzes, masseter and temporal thickness and maximum molar bite force. Data were tabulated and compared by t test (SPSS 19.0). It was found that individuals with hypertension had higher EMG activity in clinical conditions mandible posture, lower thickness of the temporal muscle and greater molar bite force maximum, right and left side (p< 0.05). Based on the results of this research can be concluded that the disease hypertension promoted functional changes of the masticatory system.

Full textObjective: to analyze the masticatory function in patients with long lasting arthrogenous temporomandibular disorders (TMD), by clinical evaluation, self-assessment of chewing difficulty and \"differential Lissajous EMG figure\", compared to control subjects and to verify the relationship between the variables obtained by these methods. Method: 55 patients with arthrogenous TMD, according to Research Diagnostic Criteria for TMD, Axis I (TMD group: 50 women and 5 men, mean age 32, SD 11.34 years) and 35 healthy subjects (group C: 31 women and 4 men, mean age 29, SD 8.57 years). The employed measures were: (a) self-evaluation of the signs and symptoms of TMD according to the validated protocol ProDTMmulti, (b) scale of chewing difficulty and (c) orofacial myofunctional evaluation, according to the validated protocol OMES, clinical analysis of mastication, according the protocol OMES-E, (d) surface electromyography (EMG) of temporal and masseter muscles (right and left), recorded during the performance of a standard test with chewing gum, analyzed by the \"differential Lissajous EMG figure\". The analysis of the interval data was performed by nonparametric statistics (Mann-Whitney test for unpaired samples). To continuous data, such as EMG, was applied the analysis of variance (ANOVA) with two variation factors (group and side). and the Student-t test for unpaired samples, for the Symmetry index. The significance level was P < 0.05. Results: The TMD group, compared with C group, showed greater severity of symptoms (mean 129.04, SD 82.04 x mean 2.91, SD 4.75) (P < 0.000), greater chewing difficulty (mean 44.98, SD 17.84 x 16.66 mean, SD 10.04) (P < 0.000), worse overall orofacial myofunctional condition (mean 85.96, SD 5.61 x mean 94.91, SD 4.74) (P < 0.000), worse scores of the masticatory type, considered separately (mean 7.05, SD 3.03 x mean 8,74, SD 1.84), (P <0.01), and increased time spent to chewing the food (mean 50.84, SD 11.50 x mean 41.53, SD 11.02) (P <0.001). In the EMG tests, the TMD group presented in the right chewing, greater incoordination than the left side, as well as the right and left chewing compared to the C, as verified by the mean values of the right Phase index (mean 93.21, 103.05 x 54.30 SD mean, SD 73.79) and left Phase index (mean 48.96, SD 36.36 x mean 31.27, SD 18.63) (P <0.01). The Symmetry index between right and left chewing sides was lower in the TMD group (mean 45.91, SD 29.86 x mean 68.52, SD 18.88) (P <0.0001), which also used greater muscle total activity, ie, rigth Impact (mean 1791.55, SD 1797.48 x mean 1229.89, SD 573.39), left Impact (mean 1762.98, SD 2097.67 x mean 1159.16, SD 591.01) (P < 0,01) to perform the function as well as increased activity in relation to percentage maximum voluntary contraction, ie, right Impact% (mean 89.61, SD 79.64 x mean 66.20, SD 25.30) and left Impact% (mean 89.09, SD 99.47 x mean 65.82, SD 33.39) (P < 0.05). The score of chewing difficulty was found to be significantly and negative correlated with the symmetry index (SMI) (r = - 0.25, P = 0.019) and AMIOFE total score (r = - 0.49, P < 0.000). Conclusion: TMD patients reported greater severity of articular symptoms and greater difficulty chewing. Clinically, a unilateral chewing pattern was observed, which may explain the lower rate of symmetry and the increased recruitment of muscles in the balancing side, which was reflected in Phase (angle). The clinical evaluation and the EMG are complementary and useful diagnostic methods for planning the rehabilitation of orofacial motor functions.

Full textResumo: Grande parte dos pacientes reabilitados com próteses totais convencionais é insatisfeita devido a problemas com retenção e estabilidade, principalmente quando é usuária de próteses totais mandibulares. Uma opção de tratamento satisfatório é a utilização de próteses totais removíveis suportadas por implantes (overdentures), conectadas por meio de diferentes sistemas de encaixe. O objetivo neste estudo foi avaliar o efeito de dois diferentes sistemas de encaixes (O'ring e Locator) para overdenture mandibular retida por um implante sobre a autopercepção da saúde bucal, impacto da saúde bucal na qualidade de vida, força máxima de mordida e a eficiência mastigatória. Este estudo foi do tipo cross-over, composto por 28 participantes distribuídos igualmente em dois grupos de acordo com os critérios de inclusão e exclusão. Para avaliar o impacto da saúde bucal na qualidade de vida e a autopercepção da saúde bucal foram utilizados os questionários Ohip-Edent e GOHAI, respectivamente. Para a avaliação da força de mordida, utilizou-se um gnatodinamômetro, e em relação a eficiência mastigatória, utilizou-se cubos de silicone de condensação denso e amêndoas, mastigados por 30 ciclos, 40 ciclos e até o paciente se sentir “pronto para engolir”. A análise estatística dos dados foi realizada com os seguintes testes: Friedman e Wilcoxon (para os questionários utilizando uma média para as pontuações), t-student, análise de variância a dois fatores para medidas repetidas e Correlação de Pearson ... Abstract: Most patients rehabilitated with conventional dentures feel dissatisfied due to problems with retention and stability, especially when they are user of mandibular conventional dentures. A satisfactory option of treatment is the use of removable total dentures supported by implants (overdentures), connected through different attachments systems. The aim this study was to evaluate with two different attachments systems (O-ring and Locator) for mandibular overdenture retained by a single implant on the self-perception of oral health, quality of life impact, maximum bite force, and masticatory efficiency. To evaluete impact of oral health on quality of life and self-perceived oral health were using the OHIP-Edent and GOHAI questionnaires, respectively. To avaluated bite force was using a gnathodynamometer, and with respect to masticatory efficiency, was using a dense condensation silicone cubes and almonds with 30 cycles, 40 cycles until the patient feel "ready to swallow". Statistical analysis of data was performed with tests subsequent: Friedman and Wilcoxon (to questionnaires using a mean to the scores), t-student, two-way ANOVA test for repeated measures and Pearson's Correlation (maximum bite force and masticatory efficiency). Full textSleep bruxism is a serious oromotor activity that influences quality of life, predisposes to pathophysiological imbalance of stomatognathic system, and is mainly characterized by grinding and clenching of teeth, wear of tooth surfaces and muscular fatigue. The present study analyzed masseter and temporalis muscles by surface electromyography, masticatory function, and ultrasonography of individuals with sleep bruxism, diagnosed by polysomnography, and classified according to severity by BiteStrip®. Maximum molar bite force and quality of life were also evaluated. Ninety individuals, aged 18 to 45 years old, were distributed into two groups: Sleep Bruxism (mean age of 30.58 ± 6.78 years; n = 45) and Control (mean age of 29.44 ± 7.88 years; n = 45). Sleep Bruxism Group included individuals with mild (n = 18), moderate (n = 3) and severe (n = 24) levels. Wireless electromyography Delsys Trigno TM was used to evaluate myoelectric activity in different conditions: rest, right laterality, left laterality, protrusion, tooth clenching in maximal voluntary contraction, and tooth clenching with Parafilm M®. Masticatory function was evaluated by the efficiency of masticatory cycles, integrating the linear envelope of electromyographic signal in habitual and non-habitual mastication. Muscle thickness was measured by ultrasound imaging using SonoSite NanoMaxx in clinical conditions of rest and clenching in maximal voluntary contraction. Kratos digital dynamometer was used to analyze right and left maximal molar bite force. Quality of life was assessed by OHIP-14Br questionnaire. Electromyographic data, muscle thickness and bite force were tabulated and submitted to statistical analysis (SPSS 21.0) and was used student t test for independent samples with significance level of 5% and confidence interval of 95%. Quality of life was examined using U-Mann Whitney test (p ≤ 0.05) for comparison of each question, subscales and sum of all OHIP-14Br questionnaire. Pearson linear correlation (p ≤ 0.05) was used to correlate muscle thickness at rest and at maximal voluntary contraction with right and left molar bite forces. Statistically significant difference was found between Sleep Bruxism and Control Groups in habitual mastication of peanuts, for left temporal muscle (p = 0.04), and in habitual mastication of raisins, for right masseter muscle (p = 0.04). There was no statistically significant difference between groups for muscle thickness and maximum bite force. Molar bite force (right and left) showed positive correlation with thickness of masticatory muscles, at rest and at maximal voluntary contraction, with statistically significant difference (p ≤ 0.05). Quality of life impact in oral cavity showed statistically significant differences in \"mouth pain\" question (p = 0.00), \"physical pain\" subscale (p = 0.04) and total sum of OHIP-14Br (p = 0.05). Therefore, sleep bruxism altered electromyographic activity, masticatory function and quality of life, but did not change masticatory muscle thickness and maximum molar bite force

Full textThe Duchenne Muscular Dystrophy is caused by a change in the locus xp21 of chromosome X, essentially in the gene (dys) that encodes a protein called dystrophin. The bearers of the disease present clinical picture of loss of muscle strength in a progressive way, starting on the lower limbs. This cross-sectional observational study has to analyze the pattern of electromyographic activity, masticatory efficiency, muscle thickness and the strength of molar bite maximum of individuals with Duchenne Muscular Dystrophy. Were selected 40 individuals of the male gender with age between 04 to 15 years, with a mean age of 10 ± 4 years, divided into two groups matched for age, weight and height. GI - carriers of Duchenne Muscular Dystrophy (n= 20). G2 - Control: healthy individuals (n= 20). The individuals were submitted to the evaluation of the stomatognathic system by surface electromyography during the clinical conditions that required postural control of the jaw, and chewing muscles fatigue, masseter and temporal sternocleidomastoid, ultrasonography of the muscles mentioned in the clinical condition of rest and habitual maximum intercuspation and assessing the strength of molar bite maximum on the first molars. The statistical analysis was performed by SPSS 21.0 using the t test (p< 0.05). In electromyographic evaluation it was found that changes in patterns of muscular activity in clinical conditions related to the posture of the mandible. In masticatory efficiency it was observed higher EMG activity for the control group during the mastication of Parafilm, of the grape raisins and the peanuts. On muscle fatigue was noted frequently elevated values of median for individuals with Duchenne muscular dystrophy. In the evaluation of muscle thickness, in individuals with Duchenne dystrophy muscular, we found higher muscle thickness for all assessed muscles during rest and maximum intercuspation usual, except for the masseter muscles in condition of habitual maximum intercuspation. On the strength of molar bite maximum note-if smaller forces us bearers of dystrophy. It is concluded that there are no changes of the stomatognathic system in muscle activity, masticatory efficiency, muscle thickness and strength of maximum molar bite procreate by Duchenne muscular dystrophy.

Full textSpinocerebellar ataxia is a neurological disorder manifested in motor functions with loss of coordination of movements, nystagmus, disorders in postural control, gait and dysphagia. This study aimed to analyze the electromyographic activity, masticatory efficiency, temporal and masseter muscle thickness and maximal molar bite force of individuals with ataxia and individuals who did not present neurological impairments. We selected 28 individuals with the average age of 45± 3 years, of both genders, distributed in two matched groups: with ataxia (n=14; AG) and without the disease, control (n=14; CG). The temporal muscles and masseters were evaluated bilaterally with the following tools: surface electromyography in clinical conditions that required postural control of the mandible and mastication; ultrasonography, in the conditions of rest and maximum habitual intercuspation and evaluation of the maximum molar bite force (right and left). The results of normalized electromyography, ultrasonography and maximum molar bite force were analyzed statistically by t-test for independent samples considering value p<=0.05. The results showed a higher electromyographic activity of the temporal muscles and masseters in all postural conditions of the mandible for the AG group, with a significant difference for the right temporal (p=0.01) and left temporal muscle (p=0.05) on protrusion; right laterality (p=0.02) and left laterality (p=0.01). In chewing, the AG group presented lower efficiency of the masticatory cycles, with a significant difference for all muscles evaluated (p=0.00). Regarding muscle thickness, a greater thickness was observed in the temporal and masseter muscles for the AG group in all clinical conditions, except for the right masseter muscle at rest, which presented values similar to the CG, with no statistically significant difference. Regarding the maximum molar bite force, AG presented lower average on both sides with a significant statistical difference (p<=0.05). This study suggests that the stomatognathic system of individuals with ataxia presents morphological and functional alterations, mainly in the electromyographic activity, masticatory efficiency and maximum molar bite force.

Full textINTRODUCTION: Individuals with dentofacial abnormalities present facial alteration - in the vertical and/or anterior posterior plane -, with consequent unstable occlusion and poor dental intercuspation. These alterations can cause impairments in the functions of the orofacial myofunctional system, and functional adaptations are usually observed for each type of deformity. The aim of this thesis was to conduct a randomized clinical trial that the relevance variable was the functional response of the mastication after the rehabilitative experimental program in patients submitted to orthognathic surgery. METHODS: All patients underwent to the orthognathic surgery, in a period of 12 months, which agreed to participate of this study. Patients were randomized assigned into two groups: experimental group (EG = 6) and group instruction (GI = 5). A control group consisted of individuals with normal dental occlusion (CG = 6). The EG patients underwent a structured treatment protocol, in six sessions, with technical focus was on the function of mastication. Patients in the IG were submitted to two supervised sessions for instruction related to the look out of mastication (standard treatment). The variables adopted for the comparison groups were obtained by clinical evaluation and by assessment of surface electromyography (sEMG) that was: orofacial mobility, masticatory/deglutition clinical performance, jaw movements and muscle activation (electrical activity) of the masticatory muscles. These parameters were evaluated pre-and postoperative long-term (12 months). The subjects in the CG were also tested in two distinct moments, with a one year interval. RESULTS: One year after orthognathic surgery, EG showed a significant improvement in orofacial mobility (p=0.001), masticatory/deglutition clinical performance (p=0.001), jaw lateralization (p=0.001) and jaw protrusion (p=0.011). Both groups of patients presented no significant differences for the sEMG results. The CG presented no significant variations in performance for any of the investigated variables when comparing the different time points. CONCLUSION: The results suggest that the rehabilitative experimental program demonstrated a positive result. No adverse effects were identified. There was adherence to the treatment. The sample size, although relevant to the area, requires careful about the power of generalization of the results.

Full textA toxina botulínica deriva da bactéria Clostridium botulinum e inibe a libertação de acetilcolina nos terminais nervosos motores, o que leva a uma diminuição da contração muscular. Por esse motivo, tem-se mostrado uma versátil ferramenta terapêutica com variadas aplicações clínicas nas mais diversas áreas da Medicina. A disfunção temporomandibular é o termo que descreve qualquer desordem musculoesquelética na ATM, músculos mastigadores ou estruturas associadas à cabeça e ao pescoço com etiologia multifatorial. Na disfunção temporomandibular, o sintoma mais referenciado é a dor. Pacientes que sofram desta doença de uma maneira crónica apontam sinais de depressão, distúrbios no sono, falta de produtividade e alterações nas relações interpessoais e sociais. Por afetar a qualidade de vida dos nossos pacientes, este é um assunto de interesse onde deve ser encorajada a adoção de tratamentos conservativos e reversíveis como é o caso da terapia com toxina botulínica. Esta monografia foca as características da toxina botulínica e as vantagens e desvantagens da sua utilização, bem como alguns dos estudos já realizados nesta área. Tem também como objetivo perceber se é uma alternativa eficaz comparativamente a outras terapias já conhecidas e utilizadas até então. Embora os resultados dos vários estudos que relacionam a toxina botulínica com disfunções temporomandibulares sejam promissores, não existe consenso nos resultados apresentados. Assim propõe-se que sejam realizados mais estudos, principalmente ensaios clínicos controlados e aleatórios.
