Full textCentral Nervous System Tumours have high morbidity and mortality rates and, in Portugal, data about their main characteristics are scarce. The main objective of this work is to evaluate the time intervals since the presentations of these type of tumours until its diagnose and follow-up. The investigation consisted on a retrospective analysis of clinical files from patients of the Centro Hospitalar Cova da Beira with the diagnose of Central Nervous System tumor, selected accordingly to the International Classification of Diseases, 9th Revision, Clinical Modification, between 1st January of 2007 until 31st December of 2008 (n=57). The time intervals between presentation, first contact with healthcare system, diagnose, referral to specialized center and surgical procedure, and time intervals between the first therapeutic procedure and referral to Palliate Care and death were analyzed. 56% of cases were diagnosed in females and the age group 70-79 years registered the peak cases in male and female. Headaches and conscience changes are the most frequent presentations (17,5%, each). Glioblastoma multiforme is the most frequent primary tumour (16%) and about half of the tumors are secondary to systemic cancer. The systemic cancer that metastizes the most to the Central Nervous System is lung cancer (22%). Brain hemispheres are the preferred location of tumours (60%). About the time intervals, 18% of the patients presented to some kind of medical assistance in the first day of symptoms and 12% waited more than 30 days. 42% of the diagnoses were made within 8 days after the first contact with healthcare system. 54,4% of the patients were referred to specialized centers, of these 55% waited less than 8 days for referral. 35,5% of patients underwent some kind of surgical procedure, of these 63,6% waited less than 8 days after referral. 57,1% of the patients were referred to Palliative Care and 25% were referred within a month after the first therapeutic procedure. From the 65% of patients who have died, 8,1% passed away within a month after the first therapeutic procedure. Population aging as well as the development of the treatment for systemic cancer may explain the results obtained in relation to the most frequent types of tumours. The time intervals for referral are not too long; however, these results may be biased by the lack of information in the clinical files of the patients.

Lectinas sÃo proteÃnas que pertencem a um grupo heterogÃneo de molÃculas com capacidade de ligaÃÃo especÃfica e reversÃvel a carboidratos. Desde sua descoberta, as lectinas se tornaram importantes ferramentas para a investigaÃÃo de fenÃmenos como a adesÃo, a migraÃÃo e a proliferaÃÃo celular, em condiÃÃes normais e patolÃgicas. Durante o processo de diferenciaÃÃo, cÃlulas tumorais apresentam vÃrios graus de modificaÃÃo na expressÃo de glicoproteÃnas de membrana. Neste aspecto, a investigaÃÃo da estrutura da membrana tumoral pode ser compreendida como um mÃtodo sensÃvel e especÃfico de diagnÃstico. Portanto, o reconhecimento de marcadores capazes de identificar e quantificar estas caracterÃsticas, pode ser usado como ferramenta de diagnÃstico. Neste trabalho propomos um modelo experimental para detectar marcadores de membrana de tumores primÃrios do Sistema Nervoso Central usando lectinas vegetais (Con A e Con Br) acopladas à cromÃforos. Amostras de tumores foram obtidas de pacientes que tinham diagnÃstico clÃnico e radiolÃgico de tumores do Sistema Nervoso Central, apÃs cirurgias realizadas no ServiÃo de Neurologia da Santa Casa de MisericÃrdia de Sobral. As amostras foram processadas e investigadas com tÃcnicas imunohistoquÃmicas, usando lectinas vegetais acopladas à fluoresceÃna, e microscopia de fluorescÃncia. Os resultados mostraram que meningiomas e gliomas apresentaram um padrÃo diferente de interaÃÃo com Con Br-FITC e Con A-FITC quando comparados ao controle (BSA/FITC). Estes dados sugerem que as lectinas vegetais estudadas podem ser ferramentas Ãteis na identificaÃÃo de marcadores de membrana em tumores primÃrios do Sistema Nervoso Central.
Lectins are proteins that belong to a heterogeneous group of molecules capable of binding specifically and reversibly to carbohydrates. Since its discovery, lectins have become important tools for investigating phenomena such as adhesion, migration and cell proliferation in normal and pathological conditions. In the process of differentiation, tumor cells display different degrees of modification in the expression of membrane glycoproteins. In this respect, the investigation of membrane structure tumor can be understood as a sensitive and specific method of diagnosis. Therefore, the recognition of markers capable of identifying and quantifying these characteristics can be used as diagnostic tool. In this paper we propose an experimental model to detect markers of the membrane of primary tumors of the central nervous system using plant lectins (Con A and Con Br) attached to the chromophores. Tumor samples were obtained from patients who had clinical and radiological diagnosis of tumors of the central nervous system after surgeries performed at the Department of Neurology, Santa Casa de Misericordia de Sobral. The samples were processed and investigated with immunohistochemical techniques, using plant lectins coupled to fluorescein, and fluorescence microscopy. The results showed that meningiomas and gliomas showed a different pattern for interaction with Con Br-FITC and Con A-FITC compared to control (BSA / FITC). These data suggest that the studied plant lectins can be useful tools in identifying membrane markers in primary tumors of the central nervous system.

Full textThis work has as purpose to investigate senses and meanings that the child with central nervous system tumor, which has already gone through oncological treatment, has of the disease and the visible body alterations due to the tumor and its treatment. This was aim because, in most cases, these children show visible sequelae on their bodies caused by the disease and its oncological treatment. In order to accomplish this work, it is used a bibliography research, over the areas of pediatric oncology, psychology and psychoanalysis, and a field research. Through one individual interview with eleven children, all of them patients from the pediatric oncology ambulatory from Santa Marcelina Hospital / São Paulo, is possible to reflect about some themes brought by the patients\' speeches. The interpretation of these themes is supported in concepts from Freud-lacanian psychoanalysis and embraces the angst, the mourning, the castration and the unconscious image of the body. Therefore, this work shows the importance of provide the child to build his own history of the disease to better deal with its effects. It yet reflects about the specificity of a psychoanalytic work on the multidisciplinaries groups that assists the patients outside the oncological treatment. Full textObjetivo: Determinar a epidemiologia dos tumores primários do sistema nervoso central em pacientes pediátricos numa instituição oncológica brasileira. Métodos: Analisamos retrospectivamente 741 prontuários (415 homens e 326 mulheres) de pacientes com idade inferior a 21 anos, com diagnóstico de tumor do SNC de acordo com os critérios da OMS (Organização Mundial da Saúde) no período entre 1989 - 2009. Distribuição por idade e sexo, topografia, sinais e sintomas clínicos, intervalo dos sintomas e classificação dos tumores foram avaliados. Pacientes com diagnóstico clínico/radiológico foram incluídos. Pacientes com idade superior a 22 anos ou que na triagem oncológica foram descartados neoplasia do SNC foram excluídos. Resultados: Setecentos e quarenta e um pacientes portadores de tumor do sistema nervoso central (SNC) foram revisados, sendo que 75% apresentavam diagnóstico histológico. O sexo masculino foi o mais prevalente (56%). Em crianças abaixo dos três anos de idade, o compartimento supratentorial predominou (51,3%). O astrocitoma foi o tumor mais freqüente (39,9%) seguido do meduloblastoma (14,0%), craniofaringioma (10,4%) e ependimoma (6,9%). A cefaléia foi o sintoma mais comum e o intervalo dos sintomas variou de 1 a 5010 dias. Aproximadamente 4% dos pacientes tinham síndromes genéticas associadas. Conclusões: Os autores analisaram uma série de pacientes pediátricos com tumores do sistema nervoso central em um hospital brasileiro de referência. Embora não seja um estudo populacional e possa ter viés seletivo, nossos dados podem ser representativos da nossa população.
Purpose: Central nervous system (CNS) tumors comprise the most frequent group of solid malignant tumors in pediatric patients, representing approximately 20% of all tumors in children under the age of 15. The objective of this study was to determine the epidemiology of primary tumors of the CNS in pediatric patients from a Brazilian oncology institute. Methods: We retrospectively analyzed 741 charts (415 males and 326 females) from patients under 21 years of age who were diagnosed with a CNS tumor. The analysis included patients from 1989 to 2009 and was completed using the World Health Organization criteria. We evaluated the distribution of age, gender, topography, clinical symptoms, symptom intervals and classification of the tumors. Patients with clinical/radiological diagnoses were included. Results: Seven hundred and forty-one patients with tumors in the CNS were reviewed, and 75% of the patients presented a histological diagnosis. Males (56%) were more prevalent than females. In children under the age of three, the supratentorial compartment was the predominant region involved (51,3%). Astrocytoma was the most frequent tumor type (39,9%), followed by medulloblastoma (14,0%), craniopharyngioma (10,4%) and ependymoma (6,9%). Headaches were the most common symptom, and the symptom intervals varied from 1 to 5.010 days. Approximately 4% of the patients had associated genetic syndromes. Conclusions: The authors analyzed a group of pediatric patients with CNS tumors in a Brazilian referral hospital. Although this was not a population study and selection bias may have occurred, our data may be representative of the Brazilian population.

Full textGlioblastoma multiforme (GBM) is the most malignant brain tumour and presents high proliferative, invasive and angiogenic capacities. In the present study the spatial distribution of molecules involved with these processes was analyzed in a rat GBM (C6) in vivo. Immunohistochemical analysis showed that tumours expressed extracellular matrix elements (collagens I, III and IV, fibronectin, tenascin C and R, vitronectin and proteoglycans), adhesion molecules (RHAMM, CD44 and integrins) and the proteolytic enzyme MMP-2. At the tumour borders and around the cells migrating along blood vessels these molecules seemed to organize in a pericellular pattern. The adhesion molecules analysis showed that RHAMM could have a pivotal role in hyaluronic acid recognition. Flt-1 and flk-1 expression by tumour cells suggests a role for these VEGF receptors not only in endotheliocyte metabolism, but also in tumour proliferation. All of the studied molecules are potential targets for anti-cancer therapies.

Full textGliomas are tumors of the central nervous system (CNS) that evolve from glial cells. The most common and most aggressive form of these tumors is known as glioblastoma multiforme (GBM). The biological aggressiveness of GBM is associated with its rapid growth and lack of apoptosis. Its poor prognosis is strongly associated with the difficulty of treating these cells as they are resistant to chemotherapy and radiotherapy. The gene expression of the enzymes cyclooxygenase-1 (COX-1), cyclooxygenase-2 (COX-2), microsomal prostaglandin E synthase-1 (mPGES-1), microsomal prostaglandin E synthase-2 (mPGES-2), cytosolic prostaglandin E synthase (cPGES) and the products of the activity of these enzymes, including prostaglandin E1 (PGE1) and prostaglandin E2 (PGE2), are directly related to the malignancy of gliomas. PGE1 and PGE2 can act in an autocrine and paracrine manner, by interacting with their target cells via binding to cell surface receptors that are linked to G-proteins. These receptors are known as EP receptors and are divided into four subtypes: EP1, EP2, EP3 and EP4; each of which activates distinct intracellular signaling pathways. Therefore, this study aimed to analyze, in vitro, the role of PGE1, PGE2 and their receptors in the proliferation and apoptosis of human glioma and the expression of COX-1, COX-2, mPGES-1, mPGES-2 and cPGES.

Resumo: Os gangliogliomas (GGG) são tumores raros do Sistema Nervoso Central (SNC) correspondendo a cerca de 1,3% das neoplasias intracranianas. Geralmente se manifestam por crises convulsivas de difícil manejo medicamentoso. A localização mais frequente é o lobo temporal seguido do lobo frontal. Sua localização infratentorial é extremamente rara com o relato, até o ano de 2007, de apenas 30 casos na literatura mundial. Aparentemente não há relato de caso semelhante na literatura brasileira. A presente tese tem como objetivo a descrição de um desses raros casos operados no Hospital das clínicas da Unicamp com ampla discussão sobre os aspectos clínicos, radiológicos, patológicos e do tratamento desses tumores nessa localização. Com isso visamos propor uma sequência racional na condução desses casos tão peculiares na prática neurocirúrgica
Abstract: Gangliogliomas are rare central nervous system tumors, corresponding to 1,3% of all intracranial tumors. Usually they manifest clinically as epilepsy refractory to medical treatment. The most common location are the temporal lobes, followed by the frontal lobes. The infratentorial location is extremely rare with only 30 cases reported until 2007. Apparently there is no such report on the brazilian medical literature. In the present thesis our objective is to describe one of these rare cases operated at our institution with a broad discussion over the clinical, radiological, pathological and the treatment of these tumors. W e are proposing a rational sequence on how to manage these cases, wich are so rare on neurosurgical practice

Full textThis work aimed at evaluating the risk of death by leukemia, brain tumor and amyotrophic lateral sclerosis in adults in relation to magnetic field exposure, in the city of São Paulo. A population-based case-control study was held, and 6224 subjects were enrolled. Cases and controls were selected from the PROAIM database, and were matched by sex, age and district of residence. Cases were all deaths ocurred between 2001 and 2005 by the specific causes, ocurring in adults 40 years and older, living in the city of São Paulo. Controls were selected from all other deaths ocurred at the same period. Exposure to magnetic fields was accessed according to the distance of the dwelling to the closest transmission line (TL). The risk of death by leukemia was elevated within the subjects living closest to the TL, with an adjusted OR of 1,8 (CI 95%: 0,8 - 4,3) and 2,5 (CI 95%: 1,0 - 7,2) for people living within 50 m and within 50 to 100 m away from the TL, respectively, in relation to people living at 400 m or further.

Full textIntroduction: Gliomas are the most frequent brain tumors, in patients with Central Nervous system (NCS) malignancies, being the Glioblastoma Multiforme the most aggressive and lethal of all. Despite current multimodality treatment efforts, the prognosis for GBM patients remains poor. New therapeutic strategies that target these pathways to improve the treatment of malignant gliomas are needed. Combination of therapies with synergistic effects in the cellular signaling pathways of cancer could potentiate the anti-tumor effect of monotherapy alone. Methods: U87 and A172 cell lines were treated with the anti-EGFR Thyrphostin AG1478, the Histone Deacetylase inhibitor (HDACi) Sodyum Butyrate (NaB), or combination of both, for 72 hours. The cellular proliferation in short and in a long time was measured through the trypan-blue assay on neubauer chamber, the influence on the cell cycle and the capability of form colonies was evaluated by nuclear staining with propidium iodide and clonogenic assay respectively. Results: We found that combined treatment with AG1478 and NaB, are able to reduce the viability and proliferation in U-87. Conclusion: Our work show that combined inhibition of both epidermal growth factor receptor and histone deacetylases was able to reduce cell proliferation in GBM cell lines. This reduction was considerably significant in U-87 cell lines when compared with individual treatments. Further studies should be performed to discover the possible crosstalk between the signaling pathways of both targets in GBM.
