Abstract:
Purpose The purpose of this paper is to extend previous studies to a hybrid analysis of three business improvement practices of Lean, Six Sigma and Lean Six Sigma (LSS) within last two decades and identify the research gaps and focusses in more comprehensive and robust classification framework. Design/methodology/approach A secondary data collection and a literature review were conducted to collect information about peer-reviewed journal articles under six dimensions of a tested classification framework. The frequency and distribution analysis was conducted followed by Pearson’s χ2 test to analyse any relationship between dimensions of framework in order to identify the gap. Findings Despite a relatively great deal of regular research outputs about Six Sigma, Lean and LSS, academic journal articles have been found mainly limited to a few industries, themes and countries. “General manufacturing”, “healthcare”, “automotive” and “electronic industries” as sectors; and “tools and techniques”, “benefits” and “success factors” as key themes have been mostly approached by LSS, Six Sigma and lean management articles. It was also found that there is still a great disparity amongst researchers and journals to publish about these three business improvement practices. Research limitations/implications The research publications for LSS, Six Sigma and lean management should have wider approach towards various manufacturing and service sectors, countries and journal publications. A greater level of research/enterprise activities has been found in relation to LSS and Six Sigma articles compared to lean management articles. Originality/value This research aims to identify the gaps in research publications during last two decades about three major business improvement practices in one package and through more comprehensive robust classification framework and also through comparative analysis.

Even though there is a high demand for clothing articles in the market, they do not meet quality standards. This can be appreciated after the clothes are washed and dried and the study revealed that over 20% of the items were returned each month. This occurs when the acquisition of raw materials is focused exclusively on the price. Thus, there are no requirements for compliance with any technical specifications that would enable the production process to be equipped with quality raw materials. Additionally, the absence of control over the quantity of consumables based on demand generates reprocessed inventory, thus damaging the fabric. Therefore, the proposed model was made through the Lean Six Sigma method to reduce waste and meet client requirements. The results were a decrease in returns by over 16.5% and an increase in productivity by 28.5%.

Abstract:
Although human trafficking was not a new concept, it gained increased attention across the United States and Canada in the first two decades of the 21st century. To better understand the Canadian anti-trafficking movement, this thesis analyzed the discourse on the topic in six local and national daily newspapers between 2008 and 2018. The goal of this thesis was to investigate the emergence of human trafficking as a social problem. Using social constructionism as a point of departure, a critical discourse analysis was conducted in NVivo of the quotes made by human trafficking experts in Canadian media. The results of this analysis suggest that an Unofficial Christian Coalition emerged in Canada, which – assisted by the media – led a moral crusade against human trafficking and pushed for the adoption of restrictive sex work legislation in Canada.

Abstract:
Overconstrained mechanisms with revolute joints occupy a special place in the big diversity of the articulated mechanisms because they don't follow the usual mobility criterion. They are composed of four, five or six bars in a single loop. If only one 4r and one 5r overconstrained mechanism exists, there exist many 6r oveconstrained mechanisms. Since the 19th century, these mechanisms were the subject of numerous publications exposing their geometrical and kinematical properties. In this memory, after a brief historic on the use of mechanisms over the time, a complete kinematics analysis of these mechanisms is presented. An original calculation method that confirms and spreads the already published results is used to find the loop closure equations, the input-output equation and the representative coordinate points of the mechanisms. An industrial application for the 6r overconstrained mechanisms is also presented. We show that it is possible to achieve a translator from the structure of a 6r overconstrained mechanism. The translator’s platform has the great advantage to present a big surface in the translation defined with good precision by three support points. A geometrical and kinematical study has been made for this translator, underlining the influence of geometrical parameters and the precision of platform displacements according to the different commands variables. A cad simulation and a prototype model have been made to verify the calculus and to validate the utility of this translator

Abstract:

This thesis focuses on the phenomenon of genericity in Russian, a language without articles, in comparison to English and Romance, languages with articles. The main goal of this research is to study how genericity is encoded in Russian. I propose that there is direct and indirect reference to kinds. The former is encoded in Russian by means of morphophonologically singular bare nominals, which are devoid of semantic and syntactic Number. Unlike English and Romance, where such NPs are preceded by a definite article, in Russian these nominals are semantically indefinite, but being singleton sets they are characterised by uniqueness. I argue that this uniqueness does not come as a result of an application of a semantic operator, it is ‘ontological’. I analyse definitional sentences in Russian as an environment where kind-referring NPs are found, showing that they are non-predicational copular sentences which express an identity/identificatiton relationship between two nominal concepts. Next, I study plural nominals which refer to sums of individuals that under certain circumstances may have a generic reference (indirect reference to kinds). Such nominals are bare in English and Russian, and definite in Romance languages. The source of genericity in this case is the type of predicate (kind-level for subjects and subject-experiencer for objects) or the type of sentence (characterising statement). I argue that, regardless of whether generic plurals are bare or overtly definite in a given language, they are characterised by maximality, identifiability and presupposition of existence, which makes them similar to nominals with a definite interpretation. They are different from NPs with a definite interpretation by the unboundedness of their domain: generic plurals cannot be restricted by spatiotemporal localisation or anaphoric anchoring. I propose that the 'definiteness effects' of generic nominals are encoded semantically by means of a definite article, as it is the case in Romance languages, or pragmatically by means of bare plural nominals in Russian (and possibly, in English).Further, I address a more general question of a possible semantic analysis of bare nominals in Russian, because genericity is only one of the interpretations they may have. So, it is highly important to understand not only how this interpretation is derived but also what it is derived from. I argue that Russian bare nominals are semantically indefinite and the other interpretations (definite and generic) they can be associated with are inferred pragmatically. Bare nominals in Russian do not give rise to a presupposition of uniqueness, and their perceived definiteness is either the result of 'ontological' uniqueness, topicality, or familiarity of the referent.

Abstract:
AbstractMargaret Ann Harvey was born on 18 May 1792 in St Peter Port, which is the capital city of Guernsey, the second-largest of the Channel Islands; and died there on 4 April 1903 at the reported age of 110. In this contribution, her exceptional age is thoroughly validated. Considering the data collected on her parents and siblings, there is no possibility of an erroneous linkage, as the name of Margaret and Ann appears only once in the birth records, her family’s birth intervals were narrow, and the dates of death of her siblings have been checked. As she did not have children, her name was not found in civil registration records after her marriage in 1823 until her death in 1903. This lack of records might have made it difficult to prove that the person who died at age 110 in 1903 was the same person who married in 1823 at age 30. Fortunately, she was enumerated in six successive censuses from 1851 to 1901, and a comparison of the ages reported in these censuses and her exact ages shows only minor deviations. Moreover, numerous letters and her numerous diaries help us to follow her life during that long period. Upon reaching age 100, she became famous in Guernsey. Thus, there are many photos of her and press articles about her life. These data support the reliability of the reported chronology of her life events, and thus allow us to validate this exceptional case. Accordingly, we can state that Margaret Ann Harvey Neve is the first documented female supercentenarian. As in the case of recently deceased supercentenarian Emma Morano, her life spanned three successive centuries – albeit one century earlier.

Abstract:
This chapter examines the Act of Six Articles, passed in 1539 by Henry VIII to enforce under heavy penalties the fundamental doctrines of the Church of England. In many respects, the Six Articles were a disaster for the reformers, affirming a traditionalist line on all the propositions Norfolk placed before Parliament. For one, heresy and treason became thoroughly conflated. The Six Articles were a setback for evangelicals, and a shot in the arm for conservatives, but they did not signal any fundamental repudiation of the path Henry had followed since 1532. The chapter analyses the ways that the Act of Six Articles not only reinforced existing heresy laws and reasserted traditional Catholic doctrine as the basis of faith for the English Church, but also determined the political fate of Thomas Cromwell, archbishop of Canterbury Thomas Cranmer, and the other reformist leaders.

Abstract:
This chapter explores the various issues in relation to arbitral awards under the CIETAC Rules. These issues include: the types of awards (Articles 49 and 50); the time period for rendering an award (Article 48); the making of an award (Article 49); the scrutiny of a draft award (Article 51); the allocation of costs (Article 52); correction of an award (Article 53); an additional award (Article 54); and performance of an award (Article 55). Arbitral awards are written decisions of an arbitral tribunal dealing with substantive matters in dispute and which have been submitted to arbitration. Under the CIETAC Rules, the arbitral tribunal may make the following types of awards: (1) partial award and interlocutory award; (2) consent award; and (3) final award. As set forth in Article 48.1, a final award shall be rendered within six months from the date on which the arbitral tribunal was constituted. Under Article 51, arbitrators are required to submit a draft award to the CIETAC for review before finalizing the award. The purpose of this scrutiny mechanism is to ensure the quality of arbitral awards.

Abstract:
Abstract Microstructure development is examined for a specialized spot weld that is used as a solid-state closure process for stainless steel tubing, referred to as pinch welding. In order to elucidate the microstructural evolution of the weld, a series of test welds were made at nominal conditions using both tubes, used in test articles and production like components. These pinch welds normally terminate after twelve cycles of a 60 Hz AC weld process. In this study, tubes with different thermal processing history were welded from one to twelve cycles and the microstructure and weld variables after each individual weld cycle number were characterized using radiography and optical metallography. Two etchants were used that highlighted different microstructural features. The study revealed that: (1) this type pinch weld is largely complete after about six cycles of 60 Hz AC current, half the weld time utilized; (2) the resistance, deformation, and closure length approach “steady state” conditions after six cycles; and (3) both oxalic and nitric acid electrolytic etchants are useful for highlighting specific microstructural attributes of type 304L stainless steel. Finally, two distinct microstructural regions can be identified for these welds: the edge of the weld which is driven by concentrated deformation, recrystallization and grain growth and the center region which is more typical of forge welding and micro-asperity breakdown followed by diffusion and grain-growth. The bond line of annealed tubes exhibits fewer indications and less contamination than that of the partially annealed and as-received cold worked tubes.

Abstract:
Since the final decades of the 20th century, university education has witnessed rapid growth. However, the quality of the education has varied a lot within the different programs around the world. How can we measure quality in university education? A number of institutions release annual global rankings of universities according to academic fields. One of the most respected lists, the Shanghai Academic Ranking of World Universities, uses a complex methodology to rank world universities, including six objective indicators, i.e. the number of alumni and staff winning Nobel Prizes and Fields Medals, the number of articles published in prestigious journals, the number of Highly Cited Researchers and the number of articles indexed in Science Citation Index - Expanded and Social Sciences Citation Index produced by Web of Science Group, and per capita performance. An overview of the annual list reveals the distance between universities, and offers a picture of global university education as a highly atomized system. How can we improve the homogenization of university education around the world? This is the central question of our research. In order to answer it, we focus on the classic tool used to evaluate students: exams. The aim of our paper is to establish a methodology with which to construct a database of world university exams by academic field. The database would be available to professors and students worldwide, and both categories could use it to contrast their level in a certain subject. In this sense, our proposal aims to achieve two objectives: 1) to maximize the effectiveness of exams as a measurement of students´ knowledge; 2) to use exams as a tool to homogenize education within universities in a certain academic field.

In 16th Annual International Conference RGCON. Thieme Medical and Scientific Publishers Private Ltd., 2016. http://dx.doi.org/10.1055/s-0039-1685331.

Full text
Abstract:
Introduction: Germ cell tumors of the ovary include all neoplasm derived from primordial germ cells of the embryonal gonad. Five percent of germ cell tumors are malignant, representing three to five per cent of all ovarian carcinomas of which pure primary non-gestational ovarian choriocarcinoma accounts for less than one per cent of ovarian tumors. Primary choriocarcinoma of ovary could be gestational or nongestational in origin. They pose diagnostic challenges in reproductive age group patients because of elevated human chorionic gonadotrophin (hCG). Non-gestational choriocarcinoma (NGCO) is resistant to single agent chemotherapy, requiring more aggressive combination chemotherapy post surgery. Due to the rarity of the disease, this article reviews the treatment protocol for NGCO. Methods: All the articles related to choriocarcinoma of ovary at Pubmed, Google scholarly article and Scopus were assessed and reviewed and their references were also reviewed and included in this article. Discussion: Clinical diagnosis of NGCO is often challenging because the clinical symptoms are often nonspecific and can mimic other, more common conditions that occur in young women, such as a hemorrhagic ovarian cyst, tuboovarian abscess, ovarian torsion, and ectopic pregnancy. The symptoms of vaginal bleeding, elevated hCG level, pelvic pain, and an adnexal mass often lead to incorrect diagnosis of ectopic pregnancy, threatened or incomplete abortion, cervical polyp, or other types of malignancy. Non-gestational choriocarcinomas have been found to be resistant to single agent chemotherapy, have a worse prognosis, and therefore require aggressive combination chemotherapy. Adjuvant chemotherapy with the EMA (etoposide 100mg/m2, methotrexate 100mg/m2, actinomycin-D 0.5mg) regimen may be given, for six to nine courses at seven days interval. Studies suggest that the disease responds well to the combination of surgery and postoperative adjuvant chemotherapy. However, long term effects of such therapy should be further studied with more cases. Conclusion: Because of the small number of patients with pure ovarian choriocarcinoma, a consensus on the treatment regimen including surgery and chemotherapy is lacking. Surgery with adjuvant combination chemotherapy is the standard treatment of choice.

Abstract:
The purpose of our critically appraised topic is to synthesize the highest-level evidence available regarding interventions for increasing confidence and mental health outcomes in caregivers taking loved ones home from inpatient rehabilitation. The final portfolio contains six research articles from peer-reviewed journals. Study designs include randomized control trials, a systematic review, and a pretest-posttest without a control group. All studies relate directly to the components of the PICO question. Four of the articles discussed both caregiver confidence and mental health while two articles discussed only mental health. There is strong evidence to support that in-person hands on training, in person discussion-based training, and/or virtual resources helped increase confidence in caregivers of patients. There is mixed evidence and only limited improvement to support mental health. The findings from this critically appraised topic will be used to draft new ideas for practice guidelines for addressing caregiver education and caregiver mental health in an inpatient rehabilitation facility.

Abstract:
Background/Purpose: Post-operative delirium leads to significant morbidity in elderly patients, yet there is no regimen to prevent POD. Opioid use in the elderly surgical population is of the most significant risk factors for developing POD. The purpose of this scoping review is to recognize that Dexmedetomidine mitigates cognitive dysfunction secondary to acute pain and the use of narcotic analgesia by decreasing the amount of norepinephrine (an excitatory neurotransmitter) released during times of stress. This mechanism of action also provides analgesia through decreased perception and modulation of pain. Methods: The authors developed eligibility criteria for inclusion of articles and performed a systematic search of several databases. Each of the authors initially selected five articles for inclusion in the scoping review. We created annotated literature tables for easy screening by co-authors. After reviewing the annotated literature table four articles were excluded, leaving 11 articles for inclusion in the scoping review. There were six level I meta-analysis/systematic reviews, four level II randomized clinical trials, and one level IV qualitative research article. Next, we created a data-charting form on Microsoft Word for extraction of data items and synthesis of results. Results: Two of the studies found no significant difference in POD between dexmedetomidine groups and control groups. The nine remaining studies noted decreases in the rate, duration, and risk of POD in the groups receiving dexmedetomidine either intraoperatively or postoperatively. Multiple studies found secondary benefits in addition to decreased POD, such as a reduction of tachycardia, hypertension, stroke, hypoxemia, and narcotic use. One study, however, found that the incidence of hypotension and bradycardia were increased among the elderly population. Implications for Nursing Practice: Surgery is a tremendous stressor in any age group, but especially the elderly population. It has been shown postoperative delirium occurs in 17-61% of major surgery procedures with 30-40% of the cases assumed to be preventable. Opioid administration in the elderly surgical population is one of the most significant risk factors for developing POD. With anesthesia practice already leaning towards opioid-free and opioid-limited anesthetic, the incorporation of dexmedetomidine could prove to be a valuable resource in both reducing opioid use and POD in the elderly surgical population. Although more research is needed, the current evidence is promising.

Abstract:
This project aims to appraise evidence of the effectiveness of various practices on reducing stress, anxiety, and depression among parents of infants in the neonatal intensive care unit (NICU). The project contains six research articles from both national and international journals. Study designs include one meta-analysis, one randomized controlled trial, one small scale randomized controlled trial, one prospective phase lag cohort study, on pretest-posttest study, and one mixed-methods pretest-posttest study. Recommendations for effective interventions were based on best evidence discovered through quality appraisal and study outcomes. All interventions, except for educational programs and Kangaroo Care, resulted in a statistically significant reduction of either stress, anxiety, and/ or depression. Family centered care and mindfulness-based intervention reduced all barriers of interest. There is strong and high-quality evidence for the effect of Cognitive Behavioral Therapy on depression, moderate evidence for the effect of activity-based group therapy on anxiety, and promising evidence for the effect of HUG Your Baby on stress.

Abstract:
Background. Timely discharge of hospitalized patients can prevent patient harm, improve patient satisfaction and quality of life, and reduce costs. Numerous strategies have been tested to improve the efficiency and safety of patient recovery and discharge, but hospitals continue to face challenges. Purpose. This Technical Brief aimed to identify and synthesize current knowledge and emerging concepts regarding systematic strategies that hospitals and health systems can implement to reduce length of stay (LOS), with emphasis on medically complex or vulnerable patients at high risk for prolonged LOS due to clinical, social, or economic barriers to timely discharge. Methods. We conducted a structured search for published and unpublished studies and conducted interviews with Key Informants representing vulnerable patients, hospitals, health systems, and clinicians. The interviews provided guidance on our research protocol, search strategy, and analysis. Due to the large and diverse evidence base, we limited our evaluation to systematic reviews of interventions to decrease hospital LOS for patients at potentially higher risk for delayed discharge; primary research studies were not included, and searches were restricted to reviews published since 2010. We cataloged the characteristics of relevant interventions and assessed evidence of their effectiveness. Findings. Our searches yielded 4,364 potential studies. After screening, we included 19 systematic reviews reported in 20 articles. The reviews described eight strategies for reducing LOS: discharge planning; geriatric assessment or consultation; medication management; clinical pathways; inter- or multidisciplinary care; case management; hospitalist services; and telehealth. All reviews included adult patients, and two reviews also included children. Interventions were frequently designed for older (often frail) patients or patients with chronic illness. One review included pregnant women at high risk for premature delivery. No reviews focused on factors linking patient vulnerability with social determinants of health. The reviews reported few details about hospital setting, context, or resources associated with the interventions studied. Evidence for effectiveness of interventions was generally not robust and often inconsistent—for example, we identified six reviews of discharge planning; three found no effect on LOS, two found LOS decreased, and one reported an increase. Many reviews also reported patient readmission rates and mortality but with similarly inconsistent results. Conclusions. A broad range of strategies have been employed to reduce LOS, but rigorous systematic reviews have not consistently demonstrated effectiveness within medically complex, high-risk, and vulnerable populations. Health system leaders, researchers, and policymakers must collaborate to address these needs.
