Chronic pain becomes a big medicine, economic and social problem that influences not just every person’s quality of life, disturbs normal social and personal life, family relations, but also public economic potential. Morbidity and incapacity to work in Lithuania meet global tendencies. Aim of the work: To evaluate the chronic pain of patients, their physical and emotional experiences and their knowledge about pain control. Goals of the work: 1. To find out the nature of pain and its intensity for patients who suffer from chronic pain. 2. To find out what methods patients use to decrease pain. 3. To evaluate how chronic pain influences everyday activities of a patient. 4. To evaluate how chronic pain influences emotional state of a patient. 5. To evaluate the knowledge of patients about the management of pain. Object of the research: Patients, who suffer from chronic pain and who are cured in VSI VUL “Santariskiu klinikos” anesthesiology, intensive therapy and pain treatment center. Methodology of the research: Questionnaire poll of patients is presented. 100 respondents were questioned. Results of the work: According to the data of the results, 85% suffered of the chronic pain. Full textThe main goal of the study was to examine the psychological factors of the connection between pain intensity and physical performance in rehabilitation patients. With this study had we explored changes in patients’ pain intensity and physical activity over time, evaluated their kinesiophobia level and pain coping strategies they were using in rehabilitation. Moreover we were trying to find out the relations among those variables. We had hypothesized there is a connection between pain intensity and physical performance in patients after hip replacement. Also we expected that pain coping strategies and kinesiophobia would interfere with pain intensity patients were experiencing and their physical activity in rehabilitation. The survey began after given permission to execute the research. The permission was given by administration of Vilnius Valakupiai rehabilitation center, Vilnius center of gerontology and rehabilitation, and sanatorium Pusyno kelias. The study was based on 41 patients in rehabilitation after hip replacement, comprising a total of 10 (24,4%) men and 31 (75,6%) women, age between 34–78 years (average age 62,9 years). Patients were investigated during their whole treatment period. Pain intensity and physical activity diaries were used to accomplish the research. Full textAbout 75% of the world population suffer of the back pain. This problem becomes more acute in developed countries with predominantly sedentary work. We see from the studies of the Ministry of Health that more and more young people complain of connective tissue and skeletal-muscle disorder, but there is no clear cause of back pain experienced because they are very much: they can be divided into six groups (spinal pathology, abdominal and oncological diseases, infections, muscle tearing apart and psychological reasons) and the risk factors that contribute to strengthen the back pain (overweight, poor posture, sedentary or heavy physical work, spinal trauma and deformity). The subject of this study - students back pain causes. The aim of the research is to identify causes of the students back pain. The next tasks have been realized in the work: to reveal the theoretical aspects of back pain; to ascertain the spread tendencies of the students back pain back pain; to ascertain the most common causes of the students back pain. Full textHeadache is a common symptom and complaint in pediatric practice. The etiology and pathogenesis of headache are still under discussion but interplay of physiological and environmental factors are considering. Headache significantly affect lives of children and their relatives and quality of life. Recurrent headache result in school absenteeism and decreased level of social activity. Headache have negative impact on relationships with peers and lead to learning outcomes. Fatigue is common symptom associated with pediatric headache. Recently the number of children who suffer headaches has increased significantly. Aim of the work was to evaluate the prevalence of headache among school age children, causal factors and characteristics of headache, quality of life and fatigue among school age children. To evaluate the changes of trunk functional condition before and after health training exercises among people with low back pain. 2. To evaluate the quantitative and qualitative rates pain rates before and after health training exercises among people with low back pain. 4. Full textSUMMARY Suffering the pain, people feel an unpleasant sensory and emotional experience, associated with actual or potential tissue damage. When the pain is untreated or treated inefficiently, the acute pain progress into the chronic pain which lasts for three or more months and becomes non adaptive, requiring for major psychological intervention. Trying to evaluate or alleviate the pain it is necessary to know how people understand the pain (cognitive factor – pain conception), as this subjective opinion is related with the selection of pain relief methods. There are two methods how to treat the chronic pain and they are as follows: traditional or conventional ones and alternative, but there is no common agreement how they are chosen, selected. So the aim of this paper was to clarify the pain conception of the chronic pain patients and to evaluate the relationship between the existing pain conception and chosen pain treatment methods. Used measures are: Beliefs about Pain Control Questionnaire, List of pain treatment methods, semi-structured interview to evaluate the pain conception. Data are analysed following the content analysis and on the basis of SPPS. The research results show, that people beliefs are important in analysing the treatment results, considering the potential pain reason (s), choosing the acceptable pain strategies and pain control position or being everyday with their pain. Full textResearch of pain phenomena for patients with rheumatoid arthritis (RA) was done in this study. The introductory part of the study is dedicated to the review of pain phenomena. The attempt was made to reveal it‘s complexity. Biopsychosocial model of pain and problems of pain evaluation were discussed. Pain was reviewed both as independent experience and in relation with rheumatoid arthritis. Attempt was made to clearify how pain is experienced by concrete persons in their everyday life. Attention was paid to aspects of pain experience both common for all participants and unique. Group of ten male chronic pain patients participated in the research. Collected data was analysed using method suggested by A. Giorgi for phenomenological analysis of interview data. Preparation of ten unique situational definitions and final common combined definition of phenomena were done. Results of the study show that pain is life changing experience for patients with rheumatoid arthritis. It greatly affects mobility, efficiency and substantially reduces contacts with outer world. It was found that pain provokes different feelings and emotions of various intensity: uncertainty, anger, fear, loneliness, lack of self-confidence, envy, despair, anxiety, shame, sorrow, regret, blame, desolation, discontent. Full textThe aim of this research was to explore the particular aspects of patients’ adjustment to advanced cancer disease and to determine the relations between the ways of adjustment, cognitive schemata, life quality, psychopathology and the strategies of cancer and pain coping. We hypothesized that the ways cancer patients adjust to advanced cancer disease and chronic pain would be different and would include the acceptance of advanced cancer disease; we expected to determine the relations between adaptive cognitive schemata, better life quality and the active strategies of cancer and pain coping as well. We have investigated 38 cancer patients hospitalized at the Department of Radiotherapy and Chemotherapy of Vilnius University Oncology Hospital and at the Department of Hematology and Oncology of Vilnius University Hospital Santariškių Klinikos. Full textThe problem of pain is not new, it remains an important global issue, and a phenomenon of oligoanalgesia, unfortunately, is also common. Different researchers conducted studies and suggested different ways to eradi¬cate this phenomenon. However, the question why patients with acute abdominal pain are treated inadequately, even with clear evidence that the treatment will not cause any adverse effects so far remains unanswered. Researchers of this study have hypothesis that the well-established local tradition or people's attitudes can lead to the fact that evidence-based guidelines are not fully applied in practice. This study investigates clinical characteristics of pain in patients with acute undifferentiated abdominal pain and to identify the factors influencing effective management of acute abdominal pain in the ED at university hospital. Objectives of the study 1. To investigate clinical characteristics of pain in patients with acute undifferentiated abdominal pain upon arrival and departure from the ED at university hospital. 2. To evaluate the trends of acute abdominal pain relief in the ED at university hospital and to compare them with current guidelines. 3. To identify differences in subjective pain intensity rating between patients with abdominal pain and assessment of nurses and doctors. 4. Full textAim of the study – to evaluate patients attitudes towards barriers for control of oncologic pain. Objectives: to evaluate characteristics of oncologic patients: attitudes towards pain and its control, perception of pain intensity; communication with physician about pain control and pain intensity; adherence of analgesics use related with pain intensity; analgesics side effects impact on pain control; emotional conditions and influence on pain intensity. Methods. The study was quantitative analytical, including anonymous survey. The questionnaire comprised 6 subscales (all of them except HAD not used in Lithuania this far) and demographic set. Subscales included Brief Pain Inventory (BPI; short form), the Barriers Questionnaire-II, Modified Version of the Patients’ Perceived Involvement in Care Scale, Medication Adherence Report (MAR), and Hospital Anxiety and Depression Scale (HAD). For evaluation of sensoric disorders the questionnaire was constructed that consisted of questions concerning patient's refusal to get analgesic due to its side effects. The questionnaires were adapted and translated from English to Lithuanian. Full textProblem: Back pain – is the most popular problem in the middle age people, which is growing significantly. The main goal of treatment – to strengthen the trunk muscles. Research object: Low back pain in 22 − 45 years of age men and women. Aim: to evaluate the impact of physical therapy on back pain in adults. Tasks: 1. To evaluate intensity of back pain before and after physical therapy. 2. To compare intensity of back pain between men and women before and after physical therapy. 3. To evaluate life style of subjects. The study involved 10 subjects, including 5 men and 5 women. Physical therapy was applied for three months, four times a week for people experiencing back pain. Back pain intensity was evaluated by specific pain scales before and after the physical therapy sessions. Conclusions: 1. Intensity of back pain decreased significantly after physical therapy. 2. Before and after physical therapy intensity of back pain differed insignificantly between men and women. 3. All subjects (100%) worked a sedentary job, leisure time spent passively 50% of subjects, 70% of them felt that over the past year they experienced stress more than others, 60% of subjects had enough time to sleep, and the same amount of subjects woke up in the mornings feeling not well.

Full textIntroduction: The aim of this study is to asses if an adequate cancer patients teaching can affect patient quality of life and self care. Material and methods: The study was performed in University hospital of Lithuania. Adult cancer patients with cancer pain score ≥ 4 were randomly assigned into to two groups: I group – patients (n=30) who had received adequate teaching about cancer pain management and educational material about cancer pain was provided. Teaching course lasted 20 – 25 minutes, II (control) group – patients who had received standard care. The anonymous questionnaires were distributed to bough patients groups a day before firs group education. Second time patients completed questionnaires a month after first group education. There were used questionnaires EORTC QLQ – 30, supplementary 21 questions questionnaire that contained questions about pain assessment, knowledge about opioids, concern about side effects, fear of addiction and tolerance, questions containing sociodemographic and disease related information. For assessing pain tolerance numerical 0 – 10 scale was used. Results: The results of the study show that following patients teaching mean score of QLQ increased from 35.3 to 58.9 points (p<0.05). Pain mean intensity in group I decreased from 7.01 to 3.1 (p< 0.05). Pain intensity correlated with QLQ (r= - 0.64). There were no significant changes in control group. Full textNeck and upper limb pain is frequently reported by computer workers. It is becoming a common problem as more people are using computers and so more and more people are complaining of pain in the neck and upper limbs, and headaches. Aims: To identify the efficacy of physiotherapy in order to relieve headache, chronic neck and shoulder pain. This research programme was concerned with people that spend the largest part of their working day using computers. Methods: The present study was designed to quantify the typical level of pain, and then to test the efficacy of physiotherapy intervention. Sixty eight workers answered the questionnaire. Only twenty five computer workers were asked to partcipate in the study, all of whom had reported headaches and had frequent or long standing neck and upper limb symptoms. Participants were asked to fill out a modified questionnaire detailing their pain before the intervention and after. The range of various neck and arm movements of the participants were then measured using goniometry and the results recorded. The group was then taught various exercises and required to take regular breaks for 12 weeks. Results: The pain was found to have been effectively relieved. Physical exercise intervention resulted in a statistically significant (p<0,05) decrease in the intensity of headaches and neck pain (from 92% to 48%). The investigation last 4 weeks, where patients were provided with 12 procedures of remedial massage.The pain was evaluated under visual evaluation scale of pain intensity within the analysis, SF-36 questionnaire of life quality was used as well as Roland-Moriss questionnaire “Pain in the bottom part of dorsum and disorder of life activity”, questionnaire of hospital anxiety and depression (HAD). Results.Being carried out the study and processed with the data by mean of SPSS program, the obtained results statically credibly differed.The pain has decreased from 4,67 points to 0,96 points.The quality of life has improved statically in all clauses.Statistically positive changes were noticed while evaluating functional and emotional condition of the patients. Full textTheoretical low back pain reduction, muscle strengthening program evaluation analysis was performed in the paper. The object of research: impact of rate of conducted therapeutic exercises and endurance training on reduction of low back pains. The aim: to investigate and assess the rate of muscle exercise performance and efficiency of endurance exercise complex in reducing low back pains. Hypothesis of the study claims that the trunk muscle endurance is more important factor in preventing back pains, compared with the rate of muscle movements. At the same time feeling of the subjects should improve. Questionnaire survey and testing were used to examine and evaluate the performance rate of muscle exercises and endurance exercise complex effectiveness in reducing low back pains. The sample consisted of 14 people (two groups of seven people, four women and three men, who felt pain in the low back part, but they have not been diagnosed with hernia). The study, which lasted a month, was carried out in VsI Šiauliai Rehabilitation Center. The pain of low back that is induced by structural and functional changes is multifactorial pathology that doesn’t have a unique treatment opinion. Nowadays the main task of medicine is to control the pain effectively and to restore disordered functions. In order to realize such task different treatment means are invoked, such as medicaments, surgery and etc. Nowadays preferential treatment is appointed to rehabilitation, especially physiotherapy. The dominant research opinion proposes that physical therapy in the water is very effective treatment of the pathological low back pain. It is mentioned a lot of methods used in the water, but there isn’t excluded any advantages of one or another method. The main task of this research is to define the influence of two different physical therapy methods in water to the low back pain with radiculopathy. There is estimated influence of different physical therapy methods to the functional ability and straight leg test results. That study main aim was to determine kinesiotaping influence for pregnant women low back pain reduction. Study object was kinesiotaping influence for pregnant women low back pain. Full textPain is a very serious problem in the 21st century. More and more young people complain of backache. Because of reducing amount of physical activities, incorrect posture, backache is one of the most common diseases. A hipothesy is that exercises in water will improve spine flexibility and children posture will improve as well. A research was made in LKKA swimming pool. 12 children – 10 boys and 2 girls – took part in this research. The average age of those who were researched was 10,88 ± 2,10 years, the average weight 46,2 ± 1,54 kg and height 156,9 ± 2,04 cm. They were in physiotherapy in water for 8 months 1 hour twise a week. Aquatic therapy consisted of posture, stretch, relaxing exercises and exercises for waist muscles. Posture, spine flexibility, waist muscles static endurance and particularities of lifestyle of the researched people was evaluated. All indicators are mentioned three times (in the beginning, in the middle and at the end of physiotherapy in water). The purpose is to find spine flexibility, muscle endurance and posture changes of kids who were in physiotherapy in water. Full textThe purpose of the work was to analyze the links of life quality of the persons, who have diseases of large joints, with the indexes of disease’s gravity (pain, limited mobility). In order to assess the life quality the questionnaire was used for the research, which was prepared on the basis of the questionnaire of World Health Organization Quality of Life (WHOQOL – 100) and adapted by A. Goštautas (Kaunas VMU Department of Psychology) and N. Goštautaite - Midttun (Vilnius University, Department of Psychiatry, 2000). In order to assess the mobility the Barthel scale was used, while the intensity of pain was assessed with the help of verbal pain scale. During the research 90 patients with the diseases of large joints in Abromiskes rehabilitation hospital were examined: 57 men and 33 women. According to the results, the biggest dominants of the life quality in the physical area, area of social relations and independence are among the men and women, whose degree of the disease of large joints is light. Lower evaluation is encountered among men and women with moderate gravity of the disease, and the lowest evaluation in the aforementioned areas is seen among men and women, whose degree of the disease is heavy. Full textThe aim of the research: to examine the impact of Quality of Life (QoL) of patients with RA after Arthritis Self – Management program. There where 72 persons of participants with RA: 58 women & 14 men. Their mean age was: 60,03 and 64,28 years. The first group (24 participants) attended Arthritis Self – Management program; the second group of 24 RA patients had treatment at hospital, and the third group of 24 respondents were without impact. In the analysis were used methodics: demographic questionnaire and Questionnaire SF-36 for Quality of Life. First the participants were asked to complete the questionnaires before research. The second stage took place after six months: the participants completed Questionnaire SF-36. Conclusions: the moust of changes after six months where in scores by participants after Arthritis Self – Management program. The scores of physycal function; role-physical; general health; vitality; social function; role-emotion and mental health were highiest by participants of Arthritis Self – Management program. Bodily pain was moust decreased by attenders of Arthritis Self – Management program. It is obvious that participating Arthritis Self – Management program increase controle of RA symptoms and affects health - related physical, psychological and social spheres.

Full textThe aim of this study was to evaluate clinical characteristics of actinic keratosis and the efficacy of photodynamic treatment with different red light doses. The diagnosis of AK is based upon its typical clinical aspects. However, there are data stating that diagnosing single AKs clinically is not precise and that there are even interobserver disagreements between dermatologists and dermatopathologists. In this study we assessed the clinical peculiarities of AK and its correlation with histological characteristics. We assessed the sensitivity and specificity of AK clinical diagnosis based on histopathological analysis. In 0.025% to 20% cases AK transforms into squamous cell carcinoma. It is impossible to predict which AK will spread from the epithelium into the deeper layers of the skin. Therefore, treatment of actinic keratosis is recommended in all cases. Topical photodynamic treatment is safe and effective therapy for AK but pain is the limitation of the procedure. We performed a prospective within-patient study and evaluated the clinical pecularities of actinic keratosis after photodynamic treatment. We assessed the factors that influence pain during the treatment. We assessed the efficacy of photodynamic treatment with different red light doses for actinic keratosis after two-years follow-up. Factors that influence the recurrence of AK were evaluated. Follow-up data for two years was used to formulate the practical recommendations of the study.

Full textBack pain is a common problem and one of enormous social, psychologic, and economic burden. 15 – 20% of adult population experience back pain at least once in a year, and 70 – 80% experience back pain at least once in the lifetime. Meanwhile 5 – 10% suffers from chronic back pain. Back pain afflicts about 40% of working population and is a major cause of disability in the adult working population. Aquatic therapy is an effective way for controling back pain and muscle – sceletal features. The objective of this research is to define the effects of aquatic therapy on back pain, back mobility, posture and trunk muscle endurance. In order to obtain the objective we came up with the next targets: to measure the variation of posture, back mobility, trunk muscle, back pain when applying aquatic therapy and to estimate and assess the lifestyle of the participants. We hypothesized that aquatic therapy will reduce back pain, increase back mobility, trunk muscle endurance and improve posture. 14 people with back pain participated in the research (5 men and 9 women) with the average age of 32±6,9 years. Aquatic therapy sessions were held twice a week in the swimming pool of Lithuanian Academy of Physical Education. It took 45 minutes for a session. Full textA lumbar pain is a relevant problem of modern athletes. The causes of these pains and the ways of avoidance and prevention of them have been analysed in many sports branches. In boxing, however, not much has been done regarding spine pains. The study on the cause of pain and its prevention when exercising boxing would enable to reduce the possibility of pain and ensure full range preparation of an athlete. Before initiating the study, a problem question was formulated: is it possible to improve the mobility of the spine and avoid or, at least, diminish low back pains by applying stretching exercises in the boxing sport branch. Problem: rheumatoid arthritis induces joints’ pain, reduces range of movement, and makes human’s physical condition worse. Pain, joints’ stiffness and reduced range of movement depend on progression of disease. Serious consideration in this work was paid in reducing pain and stiffness and increasing range of movement after applying cryotherapy. Hypothesis: applying of cryotherapy should reduce joints’ stiffness which is caused by rheumatoid arthritis, increase range of movement and reduce pain. The aim of this study was to assess efficiency of cryotherapy in rheumatoid arthritis. The tasks: 1. To evaluate joints’ pain before and after applying cryotherapy; 2. To evaluate range of movement before and after cryotherapy; 3. To evaluate morning stiffness before and after cryotherapy; 4. To evaluate joints’ pain, range of movement and morning stiffness in relation to the number of applied cryotherapy procedures. The tasks set were as follows: 1. To estimate the influence of prior eccentric-concentric load on the changes of cardiorespiratory parameters during moderate and high intensity exercises. 2. To compare sex differences in the influence of prior eccentric-concentric load on the changes of cardiorespiratory parameters and effect of delayed onset muscle soreness during various intensity exercises. Materials and methods. The subjects were ten healthy female 20,9 (0,5) years and eight healthy male 20,8 (1,9) years volunteered to participate in this study. The first test was used to measure their maximal oxygen uptake during an incremental exercise (3 min was 20 W and then 2 W/ 5s at 70 rev/min) to exhaustion on the cycle ergometer Ergoline-800 (Germany). Depending of in last 15 s on load output was determinate max. Both first and second ventilatory thresholds (VeS1, VeS2) were determinate by pulmonary ventilation (Ve) and depending of Ve/ and Ve/VCO2 equivalents on load output.
