AbstractIntroduction:Laryngopharyngeal reflux is perhaps the most extensively researched laryngology topic of the last decade. It has been suggested that some supraoesophageal symptoms, most notably asthma and laryngospasm, may be associated with night-time, or supine, reflux. The aim of this review was to assess the levels of evidence regarding a correlation between night-time reflux, snoring and apnoea.Methods:A Medline search was performed using the terms ‘reflux’, ‘night-time’, ‘larynx’, ‘gastroesophageal’, ‘laryngopharyngeal’, ‘sleep’, ‘apnoea’, ‘snoring’ and ‘ear nose throat’. The retrieved literature was reviewed, focusing on randomised and non-randomised, controlled, prospective trials. Papers on both paediatric and adult populations were included. Non-English language papers were excluded.Results:We found no randomised, controlled trials or meta-analyses addressing the possible correlation between reflux and snoring and/or apnoea.Conclusions:The role of night-time reflux in paediatric and adult snoring and apnoea is well described in the literature, but is based on poor levels of evidence from uncontrolled studies and case reports.

Abstract:
Background Sleep disorders in infants can cause developmental problems, suboptimal growth, behavioral disorders, fatigue, irritability, impulsiveness, and poor mother-infant bonding. Objective To evaluate possible risk factors for sleep disorders in infants. Methods This cross-sectional study was conducted in healthy infants aged 3-6 months. Subjects were selected using proportional random sampling from four different primary healthcare facilities in Manado, North Sulawesi. Their parents completed the Brief Infant Sleep Questionnaire. Sleep disorder was defined as the presence of one or more conditions including sleep duration less than 9 hours at night (from 19.00 until 07.00), waking up at night (from 22.00 until 06.00) more than 3 times, and more than 1 hour waking at night. Results Of 112 subjects, 58 (51.8%) were male. Subjects’ mean age was 4.21 (SD 0.829) months and 76 (67.86%) experienced sleep disorders. Sleep disorders had significant associations with low socioeconomic status (OR 17; 95%CI 3.8 to 75.8), middle school or lower maternal education (OR 44.5; 95%CI 9.8 to 202), non-supine sleeping position (OR 8.8; 95%CI 1.9 to 39.7), parental use of electronic devices (OR 156.2; 95%CI 35.1 to 692.9), and non-exclusive breastfeeding (OR 85.2; 95%CI 21.1 to 344.2). Correlative analyses also revealed that electronic media usage had the strongest association with sleep disorders, followed by breastfeeding pattern, maternal education, socioeconomic status, and sleeping position ( 0.839, 0.771, 0.624, 0.433, and 0.309, respectively). However, there were no significant correlations upon multivariate analysis. Conclusion Parental use of electronic media before sleeping is the strongest risk factor for sleep disorders among infants, followed by non-exclusive breastfeeding pattern, low maternal education, low socioeconomic status, and non-supine sleeping position. However, none of these correlations were significant upon multivariate analysis, this show that all these factors influence sleep together

La première partie de cette étude analyse les caractéristiques des troubles du sommeil dans une approche comparative et développementale auprès d’une population d’enfants avec autisme et d’enfants sans trouble du développement, âgés entre 2 et 12 ans. Les résultats retrouvent une forte prévalence au sein de la population avec autisme. Les troubles du sommeil apparaissent intrinsèquement liés aux troubles autistiques avec un pic des émergences évalué aux alentours des 3-4 ans. Les difficultés de sommeil ne se lient pas significativement à un ensemble de facteurs pourtant trouvé corrélé au sein de la population contrôle (âge, durée, évènements familiaux, fratrie). Cette étude fait aussi le constat d’un manque dans le suivi et la prise en charge des difficultés de sommeil pour l’ensemble des deux groupes cliniques. La seconde partie de la recherche étudie les relations existantes entre les différents problèmes de sommeil (parasomnies, troubles respiratoires et dyssomnies). Elles sembleraient participer à l’entretien et au maintien des problèmes de sommeil chez l’enfant avec autisme, dès le plus jeune âge. La troisième partie analyse le rôle des habitudes et pratiques du sommeil dans la mise en place et le maintien des problèmes de sommeil chez l’enfant avec autisme. Les particularités de fonctionnement de l’enfant avec autisme semblent participer à l’entretien des troubles du sommeil
The first part of this study was interested in the characteristics of sleeping disorders in a comparative and developmental approach with a population of children with autism and healthy controls, aged 2-12 years. The results find a higher prevalence within the population with autism and would indicate that its can intrinsically bound to the autistic disorders with a peak of the emergences estimated around 3-4 years. The sleep disturbances are not significantly linked in a set of factors nevertheless found correlated within the population control (age, duration, family events, sibship). This study also makes the report of a lack in the follow-up and the coverage of the difficulties of sleep.The second part of the research was studied the existing relations between the sleep problems (parasomnias, respiratory disorders and dyssomnias) Its would seem to participate in the perseveration of the problems of sleep at the child with autism, from the youngest age. The thrid part analyses the role of the sleep habits in the sleep problems at the child with autism. The functionning of the child with autism seem to participate in the maintenance of sleep problems

Introdução :A enurese, as doenças respiratórias do sono e a asma são condições que comprometem a qualidade do sono da criança e podem ocorrer isoladamente ou associadas. A enurese é considerada um dos distúrbios do sono mais comuns em pediatria perdendo apenas para insônia. As doenças respiratórias do sono (DRS) como a apneia obstrutiva do sono são alterações comuns na infância e a sua associação com enurese já foi demonstrada. Anormalidades das vias aéreas superiores e inferiores podem coexistir segundo a teoria da “Via aérea unificada”. Crianças sibilantes têm maior incidência de ronco e apneia noturna. Admitindo-se que a asma tem associação com as DRS e que essas já foram correlacionadas com a enurese, questiona-se a possibilidade de associação entre asma e enurese. Objetivo: Avaliar a associação entre asma e enurese em escolares de 6 a 14 anos de Juiz de Fora. Métodos: De agosto 2012 a março 2015, foram aplicados 523 questionários por telefone a responsáveis de escolares de 6 a 14 anos de 16 escolas de Juiz de Fora, sorteadas aleatoriamente. Foram excluídos do estudo, pacientes com incontinência diurna, doença neurológica ou qualquer outra situação que possa cursar com enurese. Foram utilizados os questionários sobre sintomas de DRS (Tucson) e ISAAC (Internacional Study of Asthma and Allergies in Childhood) para avaliação das DRS e asma e uma anamnese estruturada para avaliar a enurese e suas características. Resultados: A prevalência de história de enurese de 6 a 14 anos foi de 15,87%[12,98-19,26%] IC95%. A média de idade foi de 9,42 ± 2,46 e a razão sexo masculino/feminino de 283/240. Enurese está associada a apneia relatada pelos pais OR = 5,34 [2,19-13,03] IC95% (p = 0,0002), a história de ter tido sibilância em qualquer momento da vida OR = 1,63 [1,01-2,64] IC95% (p = 0,0465) e asma atual OR = 2,33 [1,37-3,95] IC95% (p = 0,0017). Conclusão: Os resultados sugerem que assim como a apneia, a asma também tem associação com a enurese.
Introduction: Enuresis, sleeping respiratory problems and asthma are diseases that compromises the quality of sleep in children and may occur isolate or in association with each other. Enuresis is considered one of the most common sleeping disorders in pediatrics. Sleeping respiratory disorders (SRD) such as obstructive sleep apnea are common problems in infancy and association with enuresis has been demonstrated. According to the United Airway Concept, abnormalities of both superior and inferior airways may coexist. A sibilant toddler has a greater chance of having snoring and nocturnal apnea. Since asthma and SRD may be associated and SRD has an association with enuresis, we questioned the possibility of the association between asthma and enuresis. Objective: to evaluate possibility of the association between asthma and enuresis in children 6 to 14 years of age. Methods: Between August 2012 and March 2015, parents of children between 6 to 14 years of age from 16 different schools in our region were randomly chosen for interview. Children with non-monosymptomatic enuresis, neurological diseases were not included. A specific questionnaire for SRD (Tucson) and the ISAAC questionnaire (Internacional Study of Asthma and Allergies in Childhood) were used for the evaluation of SRD and Asthma and a structured questionnaire was applied to evaluate enuresis and its characteristics. Results: We evaluated 524 children, 283 males and 240 females, with mean age in years of 9.42 ±2.46. The overall prevalence of enuresis in this sample was 15.87% [12.98-19.26%] 95%CI. Enuresis is associated to apnea reported by parents OR = 5.34 [2.19 to 13.03] 95% CI (p = 0.0002), to history of wheezing at any time of life OR = 1,63[1, 01 to 2.64] 95% CI (p = 0.0465) and to have current asthma OR = 2.33 [1.37 to 3.95] 95% CI (p = 0.0017) Conclusions: These findings demonstrate that asthma, as well with sleeping respiratory disorders, is associated with primary nocturnal enuresis.

Abstract:
Mental suffering costs the society amounts of money every year due to sick leave, suicide and general production loss. Posttraumatic stress disorder (PTSD) is one of the conditions that can affect anyone who has experienced a traumatic event.  This thesis examines the positive experiences and limitations of the treatment form Imagery rehearsal therapy for war veterans’ post-traumatic nightmares. The results show that this treatment reduces post-trauma nightmares both in frequency and in intensity. It also improves the quality of sleep and reduces PTSD- and depression symptoms. However, veterans with multiple traumas as a basis for their PTSD may need more sessions of imagery rehearsal therapy. The results also indicate that the treatment also can show positive results in the reduction of the nightmare frequency and intensity in individuals who suffer from other types of traumatic nightmares than the war veterans had.
Psykiskt lidande kostar samhället stora summor årligen genom sjukskrivningar, självmord och allmänt produktionsbortfall. Posttraumatiskt stressyndrom (PTSD) är ett av dessa sjukdomstillstånd och kan drabba vem som helst som upplevt en traumatisk händelse. Detta examensarbete undersöker de positiva erfarenheterna av och begränsningarna hos behandlingsformen imagery rehearsal therapy vid posttraumatiska mardrömmar hos krigsveteraner. Resultatet visar att denna behandlingsform minskar posttraumatiska mardrömmar både i frekvens och i intensitet. Den förbättrar även sömnkvalitén, samt minskar PTSD-och depressionssymptom. Dock, så kankrigsveteraner med flera trauman som grund för deras PTSD behöva fler sessioner av imagery rehearsal therapy. Resultaten visar också att behandlingen även kan visa positiva resultat i minskning av mardrömmars frekvens och intensitet hos individer som lider av andra typer av traumatiska mardrömmar än de krigsveteranerna hade.

Abstract:
L'agressivité excessive chez les jeunes enfants est un problème important en santé mentale, entraînant des dysfonctionnements en famille et en collectivité. Les objectifs de cette étude sont de : a) spécifier les troubles cliniques chez 70 jeunes enfants âgés de moins de 5 ans, orientés pour de l'agressivité physique excessive ainsi que leur profil développemental (tempérament, langage et développement psychomoteur) ; b) cerner les caractéristiques des attitudes éducatives parentales et celles des interactions maternelles et les articuler avec les profils développementaux des enfants ; c) comparer ces données avec celles recueillies auprès d'une population de 80 jeunes enfants typiques du même âge. Les résultats montrent que 60% des enfants cliniques (Age moyen = 3,5 ans) présentent un trouble de la régulation sensorielle. Les enfants cliniques manifestent fréquemment des troubles du sommeil concomitants et un retard de langage. Leur tempérament est caractérisé par une impulsivité importante et des difficultés de contrôle inhibiteur. Les attitudes éducatives parentales sont à la fois laxistes, surréactives et hostiles. Les interactions mère-enfant semblent perturbées et ceci, dès le plus jeune âge. Les résultats sont discutés en termes de stratégies thérapeutiques adaptées qui peuvent être proposées pour aider ces jeunes enfants et leurs parents
Excessive aggression in young children is of major concern in the field of child mental health. They often disturb family and social life. The first aim of this study is to specify the clinical troubles and the developmental profile (temperament, language and psychomotor development) of 70 preschool children under 5 years referred for excessive physical aggression. The second aim is to define parenting characteristics and mother-child interactions and to articulate them with the children's developmental profile. The third aim is to compare these data with a typical population of the same age. The results show that 60 % of the clinical children (Average age = 3,5 years) present a disorder of the sensory processing. The clinical children show frequently concomitant sleeping disorders and delay of language. Their temperament is characterized by an important impulsiveness and difficulties of effortful control. Parenting is characterized by laxness, overreactivity and hostility. Children with excessive aggression evolve in disrupted mother-infant interactions from the youngest age. The results are discussed in terms of their implications for the therapeutic interventions that may be designed to help these young children and their family

Abstract Purposes: This study aims to explore the prevalence of musculoskeletal discomfort and sleep disorder, and the work-related quality of life (QOL) scores among hotel room attendants. The purposes of this study also include the investigations of work characteristics in relation to musculoskeletal discomfort (MSD), sleep disorder, and work-related QOL; as well as the investigation of associations of MSD with both sleep disorder and work-related QOL. Study subjects: Between July and December, 2008, a convenient sample of a total of 495 room attendants from 30 restaurants and hotels, posted by the Tourism Bureau of the Ministry, were visited and solicited to complete a questionnaire. All study participants were the room attendants in duty at the time of visit. Methods: The study used a cross-sectional research design. We used a structured questionnaire to collect personal characteristics and the information on sleep disorders. The modified Nordic Musculoskeletal Disorders questionnaire and the WHOQOL-BRIEF Taiwan version questionnaire were also used to assess study participant’s MSD and work-related QOL (the score for individual domain ranged from 5 to 20 while the overall QOL score was in a range of 20-80, a higher score is indicative of a higher QOL), respectively. We first calculated the body part-specific prevalence of MSD and sleep disorder, and the mean work-related QOL and their 95% confidence intervals. We then used logistic regression model to analyze (1) the associations of work characteristics with MSD and sleep disorder, and (2) the relationship between MSD and sleep disorder. The general linear regression model was employed to analyze (1) the relationship between work characteristics and work-related QOL, and (2) the association of MSD with work-related QOL. We used SPSS 16.0 for the analyses, and the statistical significance level was set at α=0.05. Result: 1. The results showed that the body parts with severer self-assessed discomfort included shoulder (47.9%), waist (46.3%) and neck (38.6%). It also revealed that the rate of difficulty in initiating sleep (DIS) was 54.2%, the rate of difficulty in maintaining sleep (DMS) was 45.5%, and the rate of early morning awaking (EMA) was estimated at 50.1%. Additionally, the QOL score for the physiological, psychological, social, environmental domains, as well as for the overall QOL were 15.02.1, 13.82.6, 14.12.1, 13.72.4 and 56.67.7 respectively. 2. Multivariate regression analysis revealed that “monthly work hours” was significantly associated with MSD at neck, upper back, and elbow, and it was also significantly associated with both environmental domain QOL and odds of DIS. In addition, “length of life-time employment” was significantly associated with leg and ankle discomfort, and it was also significantly associated with the QOL score of social domain. Moreover, “taking a break during work” was significantly and positively associated with physiological domain QOL, while “doing housework after work” was significantly and positively associated with upper arm discomfort. 3. The attendants with neck discomfort were associated with a significantly higher risk for DIS (odds ratio (OR)=2.24) and EMA (OR=2.01). Waist (OR=1.58) and ankle/foot (OR=1.96) discomfort were both significantly associated with a higher odds of DIS. On the other hand, the thigh discomfort was associated with a significantly reduced OR (0.45) of DIS, and the subjects with upper arm discomfort had significantly reduced OR of EMA (OR=0.49). 4. The study participants with neck, waist, or ankle/foot discomfort all had significantly lower overall QOL with a magnitude of 2.30, 1.51, and 2.43 points. Additionally, the study participants with neck discomfort also had significantly lower QOL scores for the physiological (0.74 points), psychological (0.80 points), and environmental (0.61 points) domains. The waist discomfort was also observed to be associated with a significantly lower QOL score for the physiological (0.50 points) and psychological (0.56 points) domains. Moreover, the ankle discomfort was significantly associated with a reduced QOL score for physiological, psychological, and social domains with a magnitude of 0.61, 0.73, and 0.65 points, respectively. Conclusion: Compared to figures for other workers, data of this study indicated a higher prevalence of MSD in restaurant/hotel room attendants in Taiwan, and such elevated prevalence of MSD at some body parts was found to be positively associated with sleep disorder, and was negatively and significantly related to work-related QOL (especially physiological and psychological domains), suggesting that the restaurant / hotel industry or the occupational health authority should pay more attention to the MSD problem in those room attendants. Through appropriate arrangement of working hours and provision of adequate rest, it can effectively reduce the occurrence of MSD. Because the room attendants reported a lower psychological and environmental QOL scores, future research should also be carried out to investigate the potential influences of work environment and psychological well-being on the health of room attendants in order to enhance the their overall work-related QOL.

Daar word gefokus op die langtermyneffekte van die kindermolestering met spesifieke verwysing na die effek op die • fisiese gedrag van die volwassene; • die aard van die seksuele verhoudings van die volwassene; en • die aard van die interpersoonlike verhoudings van die volwassene. Tydens hierdie bespreking word die oorlewingstrategiee, wat die volwassene aanwend om met die l angtermyneffekte te oorl eef, duidel i k belig. Die he l i ngsproses waardeur 'n vol wassene vol gens sy of haar ei e pas beweeg ten einde die effek van die molestering te verwerk, word volledig bespreek. Die teorie is toegepas in die navorsing om die uitwerking van die kindermolestering op volwassenes te beskryf asook enkele gebeure (events) van die helingsproses waardeur 'n volwassene beweeg het. Gevolgtrekkings en aanbevelings word geformuleer ten einde die bruikbaarheid van hierdie studie in die praktyk aan te dui.
In this study the world of the adult survivor of sexua 1 abuse is described in terms of the eco-systemic approach, as well as certain constructions within the approach. These constructions served as a base on which the researcher construed further constructions of the adult survivor of sexual abuse. The study focuses on the 1 ong term effects of the child abuse with specific reference to the effect on • the physical behaviour of the adult; • the nature of the sexual relationships of the adult; and • the nature of the interpersonal relationships of the adult. The strategies of the adult in surviving with these long term effects are also highlighted in the discussion. The healing process through which the adult survivor passes in healing the sexual trauma according to his or her own pace, is fully discussed. In the empirical research the theory is applied to describe the effect of sexual abuse on the lives of adults as well as on certain events of the healing process. Conclusions and recommendations are formulated to indicate the usefulness of this study for the field.

The main purpose of this study was to explore the effect of Pingshuai training intervention for eight weeks on the functional fitness and sleep quality on Middle-aged female with sleep disorder in the community. The object of the study were the students of the Senior Career Center of Sanzhong District, New Taipei City. A total of 203 students from the Learning Center were selected. After screening, a total of 80 people participated in the research activities. The participants are 60-77 years old. The average age was 68.33 years for the experimental group and 68.68 years for the control group. The participants joined Pingshuai practice for 45 minutes two times a week. The results were tested after eight weeks of training with Pingshuai. The score of the Pittsburgh Sleep Quality Table improved from 13.30 to 5.36, while the control group did not change much. In terms of functional fitness, there were significant differences except for the static balance and Upper limb muscle endurance. The conclusion was that Pingshuai training has a significant improvement on the sleep quality and functional fitness on Middle-aged female with sleep disorder in the community.

Sleep apnea and other sleeping disorders impair health and quality of life. Polysomnography is the primary method for diagnosis, but involves cost and utilization of medical resources, which limit access for potential patients. The clinical environment and sensors of polysomnography hinder typical sleep patterns in many individuals, thus degrading the analysis. Sensors suitable for at-home monitoring of sleep have recently become available. At-home monitoring of sleep may improve diagnosis due to increased familiarity for sleeping and ability for multiple sleep sessions, as well as lowering the cost. However, more robust algorithms would be needed to partially compensate for the less controlled conditions and sensor systems. A mat with a grid of force sensors has become available. This study was developing a state machine algorithm to analyze the activity at multiple force sensors of a mat while the subject was lying in supine position on the mat and undertaking natural, rhythmic respiration. The algorithm monitored the subset of active sensors to detect potential respiratory cycles. The similarity of the timing of the detected cycles between different sensors was used to determine the overall pattern of respiratory activity for the subject. Reliable detection of timing for respiratory cycles would be useful for detection of sleep apnea events.

Degeneration of the articulating surfaces and pain associated with temporomandibular joint (TMJ) dysfunction are the primary symptoms of TMJ disorders (TMDs), where normal life activities such as eating, talking, and even sleeping may be drastically impaired [1–3]. To accelerate the discovery of effective therapeutic interventions for the treatment of TMD pain, we have been establishing a novel non-invasive approach for objectively assessing the presence of joint hypersensitivity. Our approach to identify chronic joint pain is based on evidence that all of the etiological factors associated with TMD pain implicate remodeling and degeneration of the joint in response to alterations in motion and loading. The injury model used for this study was a reversible, mechanical model through splint placement on the molars. It is hypothesized that arthrokinematic analysis will identify a specific pattern of functional changes that constitute a signature for the presence of irreversible damage.

Abstract Sleep disorders impair the quality of life for many individuals, but often go undiagnosed and untreated due to the cost and sleep-disturbing aggravation of polysomnography, the clinical sleep test. Simpler sleep monitoring systems that could be used at home may provide useful health information. A 2D grid of force sensors within a mat beneath the thorax of a sleeping subject has been reported to enable monitoring of respiration during sleep. A physical model of a thorax over such a 2D grid of force sensors may enable more tests and perturbations of parameters than could be done using only human subjects. The purpose of this project was to develop and test a physical model of a thorax undergoing volume changes, and measuring the changes in force by a grid of force sensors under the model. A prototype system was developed. Early testing shows promise for being able to monitor the changes in force as volume of the model changes. More development and testing are required toward development of improved algorithms and systems for sleep monitoring mats.

The instability of modern social conditions, informational and emotional richness of learning contribute to the emergence of negative states of fear, uncertainty, aggression, and provokes psychosomatic disorders of students. In order to satisfy the requirements of society, it is necessary to have a high level of development of the ability to control oneself, allowing self-regulation of the mental state within certain social frameworks. In everyday life, students use sets of techniques developed in individual experience that allow them to cope with negative conditions, enter the educational rhythm or relieve stress. Therefore, it is relevant to study spontaneously developing methods and techniques for self-regulation of states that help prevent the negative dynamics of the current state and provide an acceptable level of adaptation. In this regard, the article aims at studying ordinary ways of self-regulation of negative states of students and their relationship with the productivity of cognitive processes and individual cognitive styles. According to the results of the study, we found that typical negative states of students are fatigue, laziness, frustration. The most frequent methods of their regulation are communication, music, walking, sleeping. The relationship between the productive, stylistic characteristics of cognitive processes and the choice of self-regulation methods is shown. Thus, respondents with a high level of imagination and heuristic cognitive style choose a method of communication. Differences in the methods of self-regulation and experienced states in boys and girls were revealed. The results can be of interest to teachers and practical psychologists in education.

With the latest initiative of the government to develop a high speed passenger rail system in the United States the first and most important strategic transportation goal is to “Ensure safe and efficient transportation choices. A key element of safe railroad operation is to address the issue of fatigue among railroad operating employees and how to fight it. In this paper, we are presenting a novel approach to estimating fatigue levels of train conductors by analyzing the speech signal in the communication between the conductor and dispatch. We extract vocal indicators of fatigue from the speech signal and use Fuzzy Logic to generate an estimate of the mental state of the train conductor. Previous research has shown that sleeping disorders, reduced hours of rest and disrupted circadian rhythms lead to significantly increased fatigue levels which manifest themselves in alterations of speech patterns as compared to alert states of mind. To make a decision about the level of fatigue, we are proposing a Fuzzy Logic algorithm which combines inputs such as word production rate and speech intensity to generate a Fatigue Quotient at any moment in time when speech is present. The computation of the Fatigue Quotient relies on a rule base which draws from existing knowledge about fatigue indicators and their relation to the level of fatigue of the subject. For this project, the rule base and the membership functions associated with it were derived from real time testing and the subsequent tuning of parameters to refine the detection of changes in patterns. It was successfully shown that Fuzzy Logic can be implemented to estimate alertness levels from speech metrics in real-time and that the membership functions for this purpose can be found empirically through iterative testing. Furthermore, this study has proven that the framework to run such an analysis continuously as a monitoring function in locomotive cabins is feasible and can be realized with relatively inexpensive hardware.

In this paper, we are presenting a novel approach to estimate fatigue levels of train conductors, by analyzing the speech signal. An independent neural network joined with a Markov Model, will output the probability density, which illustrates the likelihood of the result of the first step to be accurate. Vigilance research has shown that, for most operators engaged in attention-intensive and monotonous tasks, retaining a constant level of alertness is almost impossible. Sleeping disorders, reduced hours of rest and disrupted circadian rhythms amplify this effect and lead to significantly increased fatigue levels. Increased fatigue levels manifest themselves in alterations of speech metrics, as compared to alert states of mind. To make a decision about the level of fatigue, we are proposing an alertness estimation system which uses speech metrics to generate a fatigue quotient indicative of the fatigue level. A speech pre-processor extracts metrics such as speech duration, word production rate and speech intensity from a continuous speech signal and uses a Fuzzy Logic algorithm to generate the fatigue quotient at any moment in time when speech is present. However, the nature of human interaction introduces levels of uncertainty, which make fatigue level recognition difficult. In other words, even with a perfectly trained neural network and Fuzzy Logic algorithm, we cannot make definite conclusions about the level of alertness. The reason being, that there is no guarantee that the estimated level of alertness is robust for a certain amount of time and didn’t come from drinking half a cup of coffee. Moreover, coming up with a perfect model of speech-fatigue (i.e. input-output) for humans, to train the Fuzzy algorithm is almost impossible. For this reason the study of “Risk and Uncertainty” is an integral part of this research. Motivated by the distinction between “risk” (randomness that can be fully captured by probability and statistics) and “uncertainty” (all other types of randomness), we propose a fine taxonomy: fully reducible, partially reducible, and irreducible uncertainty, that can explain some of the key differences between long term alertness and a short term change of state that makes the operator alert. An experimental study is conducted where a hyper articulated speech signal with three different levels of simulated fatigue is analyzed by the algorithm and a probability density function is assigned to the fatigue quotient to take the risk and uncertainty into account and make the overall result more reliable.
