Full textThe aim of this literature review was to examine if there is scientific evidence that massage relieves pain. The study focused on cancer- and back pain and the use of massage as a nursing intervention and alternative/complementary method for pain relief under these specific conditions. The systematic method by Goodman was used in a modified version. The review is based on nine scientific articles where the result shows that there is insufficient scientific evidence for the use of massage as an alternative/complementary method for pain relief among patients with cancer- and back pain. Massage can, however, possibly be used as a short term nursing intervention to cancerpatients with pain. Further research about massage as a method for relieving pain is required if it is to be used within healthcare in the future.

Full textPostoperativ smärta upplevs av många patienter trots att det finns effektiv smärtlindring tillgänglig. Infiltrationsanestesi är en metod då lokalanestetika injiceras i operationsområdet och därmed blockerar överföringen av smärtimpulserna till centrala nervsystemet. Syftet var att undersöka om infiltrationsanestesi vid bukkirurgi minskar den postoperativa smärtan. 16 randomiserade-dubbelblind studier analyserades och gav information om läkemedlets betydelse, administreringstillfället och det kirurgiska bukingreppets betydelse. 13 av 16 studier visade på en reducering av den postoperativa smärtan hos patienterna med variation från den första postoperativa timman upp till 10 dagar. Metoden att infiltrera lokalanestetika en och en halv timma före operationen med fortsättning under, i slutet och sex timmar efter den första infiltrationen gavs visade på längst uppmätt postoperativ smärtlindring. Kunskaper om anestesiformer och smärtbedömning krävs av sjuksköterskan för att i god tid möta upp med ytterligare smärtlindring. För att kunna ge förslag på evidensbaserad sjukvård och därmed minska patienternas lidande krävs noggrannare analys av variationerna i metoderna och ett ökat samarbete mellan olika yrkeskategorier.

Full textA burn injury is a very painful experience. It is not only in the acute state, when the injury occurs, pain is as most intense and unbearable. Depending on the severity and the size of the burn, painful dressing changes with debridement sessions and later rehabilitation are frequent elements in the care of the burn patient. The aim of this literature review was to investigate which complementary non pharmaceutical methods nurses can implement to optimize pain relief during treatment and rehabilitation of burned patients. This literary review uses nine quantitative articles audited by the authors. Results: Interventions that are presented in our study are Virtual Reality, massage-music- and video intervention, patient controlled dressing changes and sensory focusing. These interventions are presented in three different themes distraction, relaxation and control. All the methods showed positive results on the burned patients’ pain perception to varying extent.

Full textPain is one of the most frequent reason for patient´s seeking emergency care. Pain management is a crucial task in nursing due to the fact that pain is a subjective experience. This study aimed to illuminate what affects nursing care of adult patient´s seeking emergency care because of pain. Through a systematic literature search relevant research were identified, reviewed and analyzed. The results presents three themes; Nurse´s pain assessment, Nurse´s perceptions and priorities and Nurse´s working conditions. The results indicates that a majority of nurse´s underestimate patient´s pain. Nurse´s individual perception of pain has a large impact on nursing care regarding pain management. Priority of pain as a phenomenon is essential for pain management. Nurse´s define themselves as patients advocates but because of the emergency department environment they feel unable to provide satisfactorily care for patient´s in pain. Organizational support and teamwork is a presumption to enable best-practice in nursing pain management. Further research regarding patient´s perceptions of nursing care in pain management is required to enhance nurse´s understanding and awareness of pain and pain management.

Full textThe purpose of this thesis is to examine the changes in price and availability for OTC-pharmaceuticals before and after the deregulation of the Swedish pharmacy monopoly. In order to carry out the examination a quantitative approach is used and data has been collected by phone calls and observations. The framework is theories on the subject of market structure, deregulation, the marketing mix, price strategies and availability. The thesis has concluded that before the deregulation the price continually increased but the opening hours were limited. After the monopoly ceased the price has marginally increased and the number of opening hours per week has extended. Different retailers are after the deregulation able to sell OTC-pharmaceuticals and there is a variety among them regarding price and opening hours, which enables the customers to make choices between them. The study found a weak positive correlation between price and opening hours. The strongest correlation was found among Petrol stations and Convenience stores, which showed high price and flexible opening hours.

Full textIntroduction: Pain is a complex and subjective experience, it is hard for any other else than the patient to understand it. All patients that undergo surgery will experience some sort of postoperative pain and therefore it is important to understand what pain is and what the patient goes through. Insufficient pain management could lead to postoperative complications that could be life threatening and increases the cost to society. Aim: The aim of this study was to examine the patient’s experiences of postoperative pain management. Methods: This study used a descriptive literature review. The searches were performed in the databases PubMed and CINAHL. Results: Results show that patients who were well-informed both pre- and postoperatively about the pain management were the most satisfied with their experience. Good communication and good treatment heightened the well-being of patients. Negative aspects could be communication difficulties and troubles describing the pain, lack of information and overstrained staff. Conclusion: The majority of patients were satisfied with their postoperative pain management but there were different things that had a negative influence on their experience. Improvements can be made, especially in the staffs’ individual behavior and in the organization as such.

Full textThe aim was to examine possible obstacles of postoperative pain alleviation of children 6-12 years old in a literature study and examine what kind of postoperative pain alleviation methods is used on children 6-12 years old. The literature study is based on eleven articles that was quality examined by the authors. In the result we present, factors that affect children’s postoperative pain alleviation in categories. The nurses participation in the child’s postoperative pain alleviation, the parent’s participation in the child’s postoperative pain alleviation, the children’s experience of postoperative pain alleviation and obstacles for optimal pain alleviation. The result of the literature study shows that it is still difficult to give enough pain alleviation when there are a large number of obstacles for postoperative pain alleviation, for instance nurses ignorance, defective prescriptions, lack of time and nurses background factors. The results also show that parents took a big part in their children’s hospital stay and pain alleviation.

Health and Society, Department of Nursing, 2010.A nurse daily meets patients who suffer from pain and has a key role in assessing this pain as well as in evaluating the treatment plan. The aim of this study was to describe some nurses’ experiences and emotional coping of not being able to relieve the patient’s pain, from an advanced beginner- expert nurse perspective. For this study Giorgi’s descriptive phenomenological approach was used. A total of four in depth interviews was conducted with nurses working at a hospital ward. Patricia Benner’s theory of advanced beginners and expert nurses was used to give a deeper understanding of the nurse’s experiences. The result show that nurses experience feelings of frustration and powerlessness when not being able to relieve a patient’s pain. Coping with these feelings needs clinical knowledge as well as a good atmosphere on the ward. Due to their lack of clinical knowledge advanced beginners has a bigger need for support from colleagues. Full textPain in various forms is the main reason for seeking care. To be advertent to changes in patient condition it is important for the patient to get as good care as possible. Previous studies have shown that despite improving professionals´ skills in the subject of pain through information and education, patients' pain prevalence continues to be high. The purpose of this study was to portray patients' views concerning pain perception, pain assessment, pain management, and follow-up of pain treatment. The study was conducted in two different hospitals in the south of Sweden. The results were categorized and presented in tables. The result was that 20% - 50% of patients had pain but were satisfied with pain relief that they had received. More research in this area is needed to identify factors that make pain prevalence among patients still high.

Full textBackground: Each year about 23,000 people in Sweden are burned so badly that they require care. The pain resulting from treatment of the burns is usually very intense and stressful for the patient. Aim: To investigate how different coping strategies can have an analgesic effect during dressing changes of patients suffering from burns. Method: A literature review with a systematic approach. The result is based on 17 scientific articles and one licentiate dissertation. Result: Distraction, relaxation and manageability emerged as the central findings at the various coping strategies. Depending on what strategy the patient uses vary the emphasis between the findings. Conclusion: Pain intensity, activation and manageability were key factors that can affect copingabilities and thus pain relief. In order to help patients cope with their situation in an adequate manner, it is important for nurses to become aware of the importance of these factors.

Full textSmärta är förenat med ett lidande, och sjuksköterskan har ett stort ansvar vid smärtbedömning och smärtbehandling av patienter. Trots dagens väl utvecklade metoder och preparat för smärtlindring behandlas inte patienters smärta på ett adekvat sätt, bland annat på grund av negativa attityder hos sjuksköterskan när det gäller narkotikaklassade läkemedel. För att åskådliggöra hinder hos sjuksköterskan att administrera opioider till patienter med smärta genomfördes en systematisk litteraturstudie baserad på 18 originalartiklar. Resultatet visade att opiofobi hos både patient och sjuksköterska kunde verka som en barriär. Bristande kunskaper angående smärtbedömning och opioid som preparat var ett hinder för tillräcklig smärtlindring Ett bra samarbete mellan sjuksköterska och läkare var av stor vikt för att uppnå goda resultat vid smärtbehandling. Det är viktigt att sjuksköterskan håller sig ajour med de behandlingsmöjligheter som finns inom smärtområdet för att förbättra vårdkvaliteten. Riktlinjer för smärtbehandling är inte bara ett stöd för sjuksköterskan utan de förbättrar även patientsäkerheten. Patientens upplevelser av sjuksköterskans smärtbedömning kan vara intressant att forska vidare om i framtiden.

Smärtbedömning kan försvåras av exempelvis brist på kommunikation hos vårdpersonalen och attityder hos vårdpersonal och patienter. Vid utvärderingen av effektiviteten av omvårdnaden är smärtlindring och smärthantering viktiga begrepp. Smärtupplevelser är subjektiva och unika för varje patient. Därför krävs individuell omvårdnad vid postoperativ smärta. Syfte: Syftet med studien var att belysa patienters erfarenheter av postoperativ smärta och smärtlindring. Metod: Studien genomfördes som en litteraturstudie med granskning och sammanställning av tidigare forskning. Resultat: Postoperativ smärta bör bedömas regelbundet. Patienterna upplevde att deras smärta underskattades av sjuksköterskorna. Delaktighet och preoperativ information minskade patienternas upplevelse av postoperativ smärta. Miljön upplevdes som viktig vid postoperativ smärta. Slutsats: Postoperativ smärta är en individuell upplevelse. God smärtlindring och omvårdnad kan leda till färre komplikationer. Fortsatta forskningar och kontinuerliga uppföljningar, av patienternas upplevelse av postoperativ smärta, behövs för att upprätthålla omvårdnadskvaliteten.

Full textBackground: Postoperative pain is a subjective experience that is a global, ethical and socio-economic problem. All patients’ experience pain and analgesia in different ways and pain is at the same time considered by health professionals to be the greatest obstacle for postoperative wellbeing. Purpose: The aim was to describe patients’ experience of pain and analgesia in postoperative care. Method: A literature study with ten qualitative studies via PubMed and CINAHL. Analysis method: inductive content analysis where three categories were identified: Information, personal treatment and communication and Other factors that affects pain. Result: Most participants experienced pain in a more or less high extent. A few experienced the time in postoperative care completely painless. Adequate information before and after surgery was experienced as positive in the pain management, whilst inadequate or contradictory information contributed to uncertainty and worse pain management. Good personal treatment and good communication with the nursing staff was experienced to relieve suffering, whilst bad personal treatment and bad communication inhibited the participants from asking for analgesia. Not getting enough pain relief and not getting pain relief in time was perceived as a flaw. At the same time, pain relief could cause both fatigue and nausea. Participation in the patients’ own self-care was individual and perceived as important so some and as a burden to others. Participants living with chronic orthopedic pain were able to experience the postoperative pain as less severe than they imagined and had good coping strategies for pain. Others who lived with chronic pain experienced the postoperative pain as worse than expected. Conclusion: Pain in the postoperative care pathway is a subjective and multifaceted experience that affects each individual in different ways. How the pain is experienced can be both enhanced and reduced by a number of factors. Inadequate post-operative pain is a common occurrence today. The nurse and the healthcare as a whole therefore have a responsibility to continuously ensure competence about pain, which is unique to the individual, the phenomenon of pain, in the postoperative care pathway and to contribute to more knowledge in the subject through further research for relieved suffering in postoperative care.

Full textBackground: Fibromyalgia is a chronic, widespread disease which mostly revolves around pain. 3-5 % of the world population is estimated to be suffering from the disease. The nurse’s mission is to help patients to recover and to improve their health. Pain relief is a vital part of this process. Since the disease has no cure, the treatment is instead focused to ease the symptoms. There are many patients that seek pain relief from non-pharmaceutical treatments. Aim: To study if TENS and/or acupuncture have pain relieving effects in patients suffering from fibromyalgia. Do any of the methods a reduction on VAS? Do any of the methods have a larger reduction?Method: The study was a quantitative literature review. 11 articles were found on literature databases (e.g. PubMed and CINAHL). The quality and nature of these articles were assessed through a template. Result: Eight of the chosen articles regarded acupuncture as a treatment option and the rest regarded TENS. The study showed that both acupuncture and TENS caused a reduction on the VAS. The decrease was dependent on the duration of the treatment and was proven to be nonsignificant if treated longer than 12 weeks.Conclusion: Acupuncture caused a larger reduction on the VAS than TENS. Though more articles regarding acupuncture as a treatment were used and these studies were performed on larger populations than the studies regarding TENS. Thus, the nurse may not recommend acupuncture over TENS as a treatment option for fibromyalgia.

Full textBakgrund: Att förebygga, bedöma och behandla smärta är centralt i anestesisjuksköterskans arbete. Barns upplevelser och uttryckssätt vid smärta skiljer sig mycket från vuxnas. Det är därför en stor utmaning för anestesisjuksköterskor att tolka barns uttryck och känslor för att förstå barnet och kunna erbjuda trygghet och smärtlindring. Forskning har visat att barns postoperativa smärta underbehandlas, trots att kunskaperna om barns smärta ökat. Genom att belysa anestesisjuksköterskans upplevelser kan förståelsen för vad som bidrar till en god respektive mindre god postoperativ smärtlindring av barn öka.
Syfte: Syftet med denna studie var att beskriva anestesisjuksköterskors erfarenheter i samband med postoperativ smärtlindring av barn.
Metod: Semistrukturerade intervjuer genomfördes med åtta anestesisjuksköterskor från ett svenskt länssjukhus i mellersta Norrland. Insamlad data analyserades med manifest innehållsanalys.
Resultat: Utifrån analysen framkom tre kategorier och sju subkategorier. De tre kategorierna var: Att skapa kontakt är en utmaning, Helhetstänkande och Rutiner. Det var en utmaning för anestesisjuksköterskan att skapa kontakt med ett barn som varit sövd. Det var viktigt att alla personalgrupper samt föräldrar samarbetade, och den egna erfarenheten var den viktigaste källan till kunskap samt att det fanns brister gällande skriftliga rutinbeskrivningar.Diskussion: Det är viktigt att det finns tydliga rutiner som underlättar smärtlindringen av barn postoperativt och barn bör bedöma den egna smärtan i så stor utsträckning som möjligt. Det var viktigt att ha en god planering t.ex. att påbörja smärtlindringen tidigt. Stress och oro innan operation kunde ge effekter långt tid efter. Föräldrarna var viktiga och skall involveras i vården för att vara ett stöd för sitt barn. Barn kan reagera kraftigt på anestesiläkemedel och därför bör de väckas långsamt och under lugna förhållanden.
Background: To prevent, assess and treat pain is central to anesthesia nurses work. Children's experiences and expression of pain are very different from adults. It is therefore a major challenge for anesthesia nurses to interpret children's expressions and emotions in order to understand the child, and to offer security and pain relief. Research has shown that children's postoperative pain is undertreated, despite that the knowledge of children's pain has increased. By highlighting anesthesia nurses’ experiences the understanding may increase concerning factors that contributes to a good or not so good postoperative pain relief in children.
Objective: The purpose of this study was to describe anesthesia nurses' experiences when caring for children with a need of postoperative pain relief.
Method: Semi-structured interviews were carried out with eight anesthesia nurses at a county hospital in the north central part of Sweden. Collected data was analyzed with a manifest content analysis.
Results: From the analysis three categories and seven sub-categories emerged. The three categories were: To create communication is a challenge, Holistic thinking and Routines. It was a challenge for the anesthesia nurses to communicate with a child that has been put to sleep. It was also of importance that all healthcare staff and parents cooperated. The anesthesia nurses own experiences was the main source of knowledge and there was a shortage of written routine descriptions.Discussion: It is of importance to have clear procedures to facilitate pain relief of children postoperatively and children should assess their own pain as much as possible. To have a good planning e.g. to initiate pain relief before the operation was also of importance. Stress and anxiety before surgery may have an impact long time after the treatment was over. Parents are important and should be involved in the care so they can support their child. Children may react strongly to the anesthesia drugs and therefore they should be brought back slowly postoperatively and in calm conditions.

Full textBackground: Studies show that in pediatric care, nurses underestimate children's pain and perform pain assessments too seldom.Aim: To investigate how pain measurements were performed in a postoperative pediatric care unit, how pain assessment scales were employed, and how children experienced the pain relief. Furthermore the study aimed to investigate how children's age influenced the above mentioned factors. Method: A quantitative cross-sectional study based on a survey and a consecutive sample. 30 children, undergoing surgery, aged 5-17 answered questionnaires postoperatively. The children answered the questionnaires themselves or with support from their caretakers. Results: The pain assessment scales Visual Analoge Scale (VAS) and Faces Pain Scale-revised (FPS-r) were rarely used (3% each), while Numeric Rate Scale (NRS) was frequently used (78%). Only 20% of the children received preoperative information about pain assessment scales. In general the children were pleased with how often they were asked about their pain (73%) and the majority (80%) thought they always (43%) or often (37%) received enough help with their pain. No significant difference was found regarding age. Conclusion: The study shows that NRS was frequently used, and that the children felt content with how often personnel asked about their pain and the help they received. Preoperative information about pain assessment scales was an area for improvement. The study highlights the important subject of children’s pain from a child's perspective and could be used to make improvements in children's pain measurement and treatment.

Full textBackground: Pain is a complex, subjective and individual experience and can therefore not be compared between patients. One of the duties for the nurse is to relieve pain and to show respect. Patients with pain and coexisting opioid-dependency creates challenges concerning pain relief for the nurse but also the patient where communication is a factor for optimal care.Aim: The aim of the literature review is to examine pain relief and opioid-dependency from the nurse and patient perspective.Method: A literature review with a qualitative design using twelve articles.Result: The nurse experienced the patient group as “difficult” because of the fear in maintaining the patient addiction. Different opinions regarding treatment goals created conflicts between the patient and the nurse. Education was something that the nurses wished for in order to more easily treat the patient group. The support from colleagues gave an increased feeling of safety for the nurses in the caring work. Other findings were that the patients often experienced frustration and felt mistreated due to prejudices from the nurses. Mutual respect and the importance of communication helped creating trust.Conclusion: The nurses experienced a lack of education regarding the patient group. Prejudices was determined to how the nurses trust and treat the patient. Teamwork helps to optimize care and the aim of building trust nurses need to create a partnership with the patient. The patients wish to be treated with respect and that the nurses look beyond the addiction.Keywords: Experiences, nurse, opioid dependency, pain, pain relief, stigma.

Syftet med litteraturstudien var att beskriva sjuksköterskans kunskap samt attityd till
barn med postoperativ smärta. Metoden som författarna använt sig utav var sökningar
i databaserna Academic Search Elite, Med Line samt manuellt.
Till litteraturstudien användes 14 vetenskapliga förgranskade vetenskapliga artiklar.
Med hjälp utav Benner och Wrubels omvårdnads teori tydliggörs sjuksköterskans roll.
Resultatet visade att sjuksköterskans kunskap, attityd samt smärtlindring till barn med
postoperativ smärta var undermålig samt bristfällig. Även kommunikationen mellan
sjuksköterska samt förälder var bristfällig. Smärtskattningsinstrument existerar men
endast en liten del av sjuksköterskorna hade kunskap, god attityd eller tid till att
använda dem Författarna anser att föreliggande studie är av betydelse för att skapa en
debatt samt föra upp ämnet i ljuset.

Full textBackground: Literature reveals that undiagnosed and untreated pain among older persons with dementia are common occurrences. This is in contrast to the principles of equal value among human beings, the dignity of the individual, and the alleviation of human suffering. There are many types of dementia, and there exists a belief that persons with dementia feel no pain. A common consequence of dementia is inability to communicate or locate the origin of pain. Selfassessment and observational pain scales are frequently not used. Aim: The aim of this literature review was to shed light upon the challenges associated with pain and the practice of pain relief in the care of older adults with dementia. Method: This is literature review study, where ten quantitative scientific articles have been analyzed using manifest context analysis. The results of the articles were sorted into four categories, each with subcategories, that describe the challenges associated with pain and the practice of pain relief. Results: The following result was obtained. Older persons with dementia often received inadequate pain relief, and the risk for under treatment increased with increasing level of cognitive impairment. Under treatment of pain was common even when pain assessment scales were used. Persons with assumed vascular and Alzheimer's dementia often stated that they felt more pain and less pain, respectively, than older adults with no cognitive impairment did. A group of persons with dementia were shown to experience no anticipatory effect of analgesics. Conclusion: Under treatment of pain was a common occurrence among older persons with dementia and more research is needed regarding how to estimate pain in persons with cognitive impairment.

Full textSince the beginning of the 2000s, Emergency Medical Services has had rapid development in several areas. The possibility to administer pain relief has been developed and today's ambulances are well-equipped with a variety of pain relief preparations to choose from. This places great demands on the specialist nurse who cares for the patient. In 2015, new medicines were introduced alongside with a, for the ambulance service, new route of administration: Intranasal. A large proportion of the patients in the ambulance service are suffering from acute pain and are therefore in need of adequate pain relief. Sufentanil is a strong opioid that has properties to be the optimal intranasal drug for pain management. However, there are few studies on intranasal pain management in a pre-hospital context. In addition, no previous studies have compared the effect of intranasally administered sufentanil between children, adults and the elderly. The aim of the study was to describe the use of intranasally administered sufentanil in the pre hospital environment. More specifically we studied: the degree of pain relief, the occurrence of side effects, the establishment of peripheral lines, the environment in which the patient was and also the experience of the staff. The authors analyzed evaluation protocol witch had been filled in by nurses, who had administered intranasal sufentanil. The protocols were divided into three groups based on age. The groups where children, adults and elderly. Quantitative approach where used and data were analyzed according to ANOVA and chi2test depending on the data grade. Both nominal and ordinal data were included in the analysis. Data presented in visual analogue scale (VAS) before pain management and at arrival at emergency department (ED), dose and incidence of adverse effects were analyzed and compared between the groups. The results showed that all groups receive adequate pain relief, which was defined as three points or lower measured by visual analogue scale [VAS] on handoff at the emergency department [ED]. Children received significantly higher doses compared to adults and elderly and they also scored a lower VAS at arrival to the ED. Respiratory depression occurred in two cases in the elderly group. The most common adverse effects were nausea and vomiting except in children where sedation was the dominant adverse effect. In children significantly lower number of peripheral lines were established compared to adults and elderly. Nurses appreciated the effect of the intervention as good but with greater contented in the children. The conclusion was that pre-hospital administration of sufentanil appears to provide a good pain relief within the treatment target, the proportion of adverse reactions is not greater than in previous studies. On the other hand, our results showed that children to a certain extent become drowsy in treatment with intranasal sufentanil.

Full textAbstract Background: Complications still occur in connection to moderate and severe postoperative pain. The nurse anaesthetic´s is completely responsible for the patient care perioperative. An overall assessment of the patient all together determine the postoperative need of analgesic. The perioperative dialogue is a method which gives the nurse anaesthetic´s a sense of meaningfulness of the work and creates a perception of safety for the patient. The authors identified the absence of guidelines to make an individual overall assessment when managing postoperative pain. Aim: The aim of the study was to elucidate the nurse anaesthetic´s planning, implement and evaluate postoperative analgesic during total anaesthesia along the time-phases pre-, intra and postoperative and the impact of the perioperative dialogue. Method: A qualitative study was conducted with ten semistructured interviews. A qualitative content analysis was made with an inductive approach. Result: The result is presented in four head categories and eleven sub-categories that illuminate the informants’ experiences. Preoperative undertakings - Feeling responsibility for planning, Create a care-relation, Respect the patients emotional state. Intraoperative steps – Constant evaluation of treatment, Adapt the treatment continuously, Independent responsibility. Postoperative expectations – trying to obtain feedback, Avoid outbreak of pain. Perioperative organizational aspects – Adjustment along work situation, Absence of the perioperative dialogue. Conclusion: The postoperative pain management is a complex matter that demand consideration of several aspects to achieve a good outcome. The perioperative dialogue is not prioritised. There is a lack of consensus among nurse anaesthetic´s managing postoperative pain. Developing and increasing cooperation along the entire chain of care is necessary to manage the issue of postoperative pain.
