Abstract:
Abstract Background The SMSA (size, morphology, site, access) polyp scoring system is a method of stratifying the difficulty of polypectomy through assessment of four domains. The aim of this study was to evaluate the ability of SMSA to predict critical outcomes of endoscopic mucosal resection (EMR). Methods We retrospectively applied SMSA to a prospectively collected multicenter database of large colonic laterally spreading lesions (LSLs) ≥ 20 mm referred for EMR. Standard inject-and-resect EMR procedures were performed. The primary end points were correlation of SMSA level with technical success, adverse events, and endoscopic recurrence. Results 2675 lesions in 2675 patients (52.6 % male) underwent EMR. Failed single-session EMR occurred in 124 LSLs (4.6 %) and was predicted by the SMSA score (P < 0.001). Intraprocedural and clinically significant postendoscopic bleeding was significantly less common for SMSA 2 LSLs (odds ratio [OR] 0.36, P < 0.001 and OR 0.23, P < 0.01) and SMSA 3 LSLs (OR 0.41, P < 0.001 and OR 0.60, P = 0.05) compared with SMSA 4 lesions. Similarly, endoscopic recurrence at first surveillance was less likely among SMSA 2 (OR 0.19, P < 0.001) and SMSA 3 (OR 0.33, P < 0.001) lesions compared with SMSA 4 lesions. This also extended to second surveillance among SMSA 4 LSLs. Conclusion SMSA is a simple, readily applicable, clinical score that identifies a subgroup of patients who are at increased risk of failed EMR, adverse events, and adenoma recurrence at surveillance colonoscopy. This information may be useful for improving informed consent, planning endoscopy lists, and developing quality control measures for practitioners of EMR, with potential implications for EMR benchmarking and training.

Abstract:
Objective Primary SMSa treatment can be associated with hormonal control and tumor shrinkage in patients with GH-secreting pituitary adenomas. The aim of this study was to evaluate whether regular MRI follow-up was necessary in patients with acromegaly-treated and responsive to first-generation long-acting SMSa. Patients and methods In this retrospective monocentric study we included patients with GH/IGF-1 hypersecretion and pituitary adenomas with normal visual field, primarily treated with first-generation long-acting SMSa between 1995 and 2015 and regularly monitored (clinical evaluation, GH/IGF-1 levels and pituitary MRI) for at least 3 years. Results We included 83 patients (32 men and 51 women, mean age at diagnosis 50 ± 12 years) with mean GH = 19.3 ± 25.6 ng/mL, IGF-1 = 284 ± 110% ULN and pituitary adenoma height = 12.9 ± 4.7 mm. Mean follow-up was 8.9 ± 4.9 years in 36 controlled patients and 2.0 ± 1.6 years in 47 partial responders to SMSa alone. No significant increase in pituitary adenoma height was observed. Pituitary adenoma height decreased significantly in controlled patients (diagnosis: 11.9 ± 4.8 mm, SMSa: 9.6 ± 3.3 mm, P < 0.001), and in partially responders (diagnosis: 13.6 ± 4.5 mm, SMSa: 11.5 ± 4.5 mm, P < 0.001). Conclusion During SMSa treatment, no significant increase in GH-secreting adenoma size was observed. Primary SMSa treatment was associated with a significantly decrease in adenoma height in our population. Our cohort data suggest that regular MRI follow-up does not seem relevant in patients with acromegaly who are responsive to SMSa treatment.

With the traditional biomarkers medical treatment is therefore difficult to monitor with the traditional biomarkers. Additionally, Peg-v and LA-SMSA could be ideal combination since they have different mode of actions. We believe that the time has come to challenge the existing concepts of treatment and monitoring of patients with acromegaly.

Abstract:
Background and Objective. The aim of the current study was to compare the efficacy and safety of polypectomy by using rotary snare vs. traditional snare during colonoscopy. Methods. A single-center randomized controlled trial, which included consecutive participants who were ≥18 years old and detected with polyp(s) during routine colonoscopy between July and September 2018, was conducted. Participants with colorectal polyps were randomized to receive polypectomy using rotary snares or traditional snares. The primary outcome measure was the comparison of the average time of removing a polyp between those two groups. The secondary outcome measure was to compare the polyp resection time by using SMSA (size, morphology, site, and access) scores. Results. A total of two hundred participants were included in this study. Of them, 100 participants were randomly assigned to the rotary snare group (214 polyps) and the other 100 participants were randomly assigned to the traditional group (232 polyps). The mean resection time was significantly shorter in the rotary snare group than in the traditional snare group (24.41±18.14 seconds vs. 29.53±25.74 seconds, P=0.021). In the subgroup analysis, the resection time was also shorter in the rotary snare group than in the traditional snare group in SMSA level 1 (18.51±8.26 seconds vs.23.84±15.07 seconds, P=0.013) and in SMSA level 2 (25.03±15.32 seconds vs.29.15±24.82 seconds, P=0.009), respectively. Conclusion. Colorectal polyps could be removed more efficient by using rotary snares than by using traditional snares in SMSA level 1 and SMSA level 2.

Background Polyp assessment is multimodal and is vital prior to endoscopic mucosal resection. The size, morphology, site and access (SMSA) score has been validated in specialist endoscopic institutions. this study investigated the ability of this score to predict incomplete endoscopic resection of large colorectal polyps in a district general hospital Methods Consecutive patients undergoing endoscopic mucosal resection of large (≥ 20 mm) colorectal polyps at Worthing Hospital. Clinical, endoscopic and histological data were taken from prospective databases. The primary outcome of the study was to investigate the correlation of the SMSA score with incomplete endoscopic resection. Results Between February 2015 and August 2018, 114 patients underwent colorectal endoscopic mucosal resection. Of these, 67 (59%) were male. The median (interquartile range) age of the study population was 72 years (65–78 years). Some 17 lesions (15%) were pedunculated, 76 (67%) were sessile and 21 were (18%) flat; 84 polyps (77%) were located in the left colon/rectum, with the remainder in the right colon; 51 lesions (45%) were 20–30 mm, 27 (24%) were 30–40 mm and 36 (31%) were greater than 40 mm in diameter. When reclassified into the SMSA score, 9 of the polyps (8%) were level 2, 64 (56%) were level 3 and 41 (36%) were level 4. Incomplete resection was clinically diagnosed in 9/114 (8%). The SMSA score was positively correlated with incomplete endoscopic resection, but not with additional procedure usage, complications or advanced histology. Conclusions Many patients with large polyps can be managed outside of specialist units. This study has validated that the SMSA score was associated with incomplete endoscopic mucosal resection for large polyps in a district general hospital setting.

Based on the iterated statistically multiscale analysis (SMSA), we present the convergence of the equivalent mechanical parameters (effective moduli), obtain the error result, and prove the symmetric, positive and definite property of the equivalent mechanical parameters tensor computed by the finite element method. The numerical results show the proved results and illustrate that the SMSA-FE algorithm is a rational method for predicting the equivalent mechanical parameters of the composite material with multiscale random grains. In conclusion, we discuss the future work for the inhomogeneous composite material with multiscale random grains.

Medical treatment of acromegaly with long-acting somatostatin analogs (LA-SMSA) and the GH receptor antagonist, pegvisomant (PEGV), has made it possible to achieve normal serum IGF1 concentrations in a majority of patients with acromegaly. These two compounds, however, impact the GH–IGF1 axis differently, which challenges the traditional biochemical assessment of the therapeutic response. We postulate that LA-SMSA in certain patients normalizes serum IGF1 levels in the presence of elevated GH actions in extra-hepatic tissues. This may result in persistent disease activity for which we propose the term extra-hepatic acromegaly. PEGV, on the other hand, blocks systemic GH actions, which are not necessarily reliably reflected by serum IGF1 levels, and this treatment causes a further elevation of serum GH levels. Medical treatment is therefore difficult to monitor with the traditional biomarkers. Moreover, the different modes of actions of LA-SMSA and PEGV make it attractive to use the two drugs in combination. We believe that it is time to challenge the existing concepts of treatment and monitoring of patients with acromegaly.

ABSTRACT All available amino acid-requiring mutants of Aspergillus nidulans were found to be hypersensitive to MMS (methyl methanesulfonate) to various degrees. On MMS media, secondary mutations could be selected which suppress this MMS sensitivity but do not affect the requirement. Many such mutations were analyzed and found to be alleles of one gene, smsA (=suppressor of MMS sensitivity), which mapped distal on the right arm of chromosome V. This gene is more likely to be involved in general regulation of amino acid biosynthesis than MMS uptake, since a variety of pathway interactions were clearly modified by smsA suppressors in the absence of MMS.

Utilizando registros isotópicos (\'delta\' \'POT.18\'O e \'delta\'\'POT.13\'C) de alta resolução, razões de elementos-traço em relação ao cálcio e taxas de crescimento de espeleotemas, precisamente datados pelo método U-Th, foi feita uma reconstrução da intensidade do Sistema de Monções Sul-Americano (SMSA) nos últimos 1500 anos na região do município de Bonito, Mato Grosso do Sul. O estudo do sinal climático, interpretado por meio de variações geoquímicas dos espeleotemas, foi realizado com base em estudos e comparações com dados instr umentais de uma estação meteorológica de Bonito, com dados de monitoramentos do IAEA - GNIP de Campo Grande e Cuiabá e com informações de modelos computacionais que simulam a variação de \'delta\'\'POT.18\'O em relação à intensidade das monções na América do Sul, o qual e stá relacionado com variação regional de pluviosidade. Apesar destes dados mostrarem que o amount effect é um dos fatores determinantes na variação das razões isotópicas dos espeleotemas, outros fatores como o degree of rainout upstream e a fonte de umidade (Amazônia x oceano Atlântico sul) também devem ser considerados importantes. Assim, a interpretação das razões de \'delta\'\'POT.18\'O é atribuída à intensidade do sistema de monções e, desta forma, à atuação da Zona de Convergência do Atlântico Sul (ZACS), principalmente associada à propagação de chuvas da região Amazônica para o centro-oeste e sudeste brasileiro. O registro paleoclimático de Bonito mostrou excelente coerência com variados tipos de registros de outros locais da América do Sul, e também com dados do Hemisfério Norte, evidenciando a influência de eventos de escala secular como MCA e LIA na intensidade das monções na América do Sul. A boa correlação entre algumas razões de elementos-traço com a precipitação mostra que o \"amount effect\" não controla totalmente os registros isotópicos. Análises de estatísticas de séries temporais aplicadas aos registros isotópicos e aos registros de elementos-traço permitiram a observação de vários eventos de freqüêcia decadal a multidecadal influenciando as monções da América do Sul. Os principais destes eventos são a NAO (North Atlantic Oscillation) e a AMO (Atlantic Multidecadal Oscillation), os quais, pelas flutuações de TSM no Atlântico Norte, desencadeiam teleconexões que têm resultados no posicionamento da Zona de Convergência Intertropical (Intertropical Convergence Zone - ITCZ) e da ZACS.
High-resolution isotope records (\'delta\'\'POT.18\'O e \'delta\'\'POT.13\'C), growth rates, elemental ratios in precisely dated speleothems by U/Th method are used to reconstruct the intensity of the South American Monsoon System (SAMS) during the 1500 years in Bonito Region, Mato Grosso do Sul. The interpretations of geochemical variations in speleothems are based on comparative studies with instrumental climatic data and at isotope measurements in rain water at IAEA - GNIP stations in Campo Grande and Cuiabá, as well as from climate model studies using oxygen isotope ratios as tracers of monsoon rainfall intensity. Despite of these data suggest that the \"amount effect\" is a dominant factor controlling the isotope variations in preci pitation, the \"degree of rainout upstream\" is considered also important because the distal influence of SAMS and South Atlantic Convergence Zone (SACZ) along the moisture transport tra jectory from Amazon to SE Brazilian Coast. The other significant factor to isotope composition to precipitation, therefore to speleothems, is the difference in moisture source area from Amazon Region (summer monsoonal precipitation) and Atlantic Ocean (extratropical regime). Bonito\'s paleoclimate Record indicate very good coherence with other records in South America and Northern Hemisphere, particularly in the response to the global scale climate events such as Medieval Climate Anomaly (MCA) e Little Ice Age (LIA) on summer precipitation. In these events were evidenced dryer and wetter conditions, respectively, not just from the stable isotope ratios but also from Sr/Ca and Ba/Ca ratios. Time series statistical analysis in the isotope and trace -element records showed that the SAMS is largely influenced by decadal to multidecadal events. In this regard, the most influent climate modes are the NAO (North Atlantic Oscillation) and the AMO (Atlantic multi Decadal Oscillation), with significant influence on the precipitation variability. In both cases these changes in precipitation are resulted from teleconection with Atlantic Ocean, which impacts the location of ITCZ and consequently the SACZ activity in Central-West Brazil.

En el Perú hay una alta tasa de inseguridad ciudadana. El delito más frecuente es el robo de celulares, billeteras y dinero. En segundo lugar, existe el robo de vehículos. En este último caso, llama la atención que el 75% de los robos ocurran cuando están estacionados. Existen varios métodos de prevención, seguro de vehículos y GPS. ¿Pero qué pasa cuando un pueblo adquiere un segundo vehículo? Naturalmente, siempre tiene una incertidumbre acerca de no saber qué está pasando con su unidad mientras está estacionada. Actualmente, aproximadamente 16,000 vehículos usados se venden por mes en Lima. Nuestra idea de negocio nace como una respuesta a este mercado desatendido, ya que no tiene medidas de seguridad preventivas al alcance, ya que las compañías que comercializan el dispositivo GPS están dirigidas a vehículos nuevos y las compañías de seguros tienden a cobrar primas altas por el seguro de vehículos. Alert SMS es un servicio de geolocalización que está dirigido a usuarios que tienen un vehículo de segunda mano. Nuestro servicio ofrece alertas a través de SMS (mensajes de texto) cuando alguien abre las puertas, cuando el sistema eléctrico está encendido y permite bloquear el sistema eléctrico con un solo mensaje, todo a un precio asequible.
In Peru there is a high rate of citizen insecurity. The most frequent crime is the theft of cell phones, wallets and money. Second, there is vehicle theft. In the latter case, it is striking that 75% of theft happen when they are parked. There are several methods of prevention, vehicle insurance and GPS. But what happens when a people acquires a second vehicle? Naturally, you always have an uncertainty about not knowing what is going on with your unit while it is parked. Currently, approximately 16,000 used vehicles are sold per month in Lima. Our business idea is born as a response to this unattended market, since it does not have preventive security measures that are within reach, because the companies that commercialize the GPS device are aimed at new vehicles and insurance companies tend to charge high premiums for vehicle insurance. Alert SMS is a geolocation service that is aimed at users who have a second-hand vehicle. Our service offers alerts via SMS (text messages) when someone opens the doors, when the electrical system is turned on and allows to block the electrical system with just one message, all at an affordable price.

Den industriella revolutionen kom med behovet av att bygga byggnader och broar, till en början förstod man inte vikten av armering och förstärkning av konstruktion. Men med tiden behövde man komma på någon strategi att göra konstruktionen starkare och mer hållbar, och det var då man började använda sig av förstärkning. I dagens moderna samhälle är det vanligare att bygga med betong, betong är det viktigaste byggmaterialet i världen. Det har flera egenskaper som garanterar en lång livslängd för konstruktionen; dess hållbarhet och styvhet är av störst intresse för att designa betongkonstruktioner. Andra egenskaper såsom täthet och bearbetbarhet är också viktiga. För att utnyttja betongegenskaperna korrekt måste nästan alla strukturer förstärkas på ett eller annat sätt. Därför har man under en lång period gjort stora forskningar på hur man kan göra betong starkt i draghållfasthet och inte bara i tryck. Moderna komplicerade konstruktionsformer kräver nya förstärkningsmetoder, komplexiteten i den nya strukturen både när det gäller höjd och bredd har nödvändiggjort behovet av nya metoder för betongarmering. En metod är användningen av järn-baserade minneslegeringar (Fe-SMA), genom forskning har det visat sig vara en effektiv metod för armering av stål. SMAs har unika egenskaper och det mest speciella är att det återgår till sin ursprungliga form även när det har deformerats. Som ett resultat av detta möjliggör det förstärkning av betong av vilken form och storlek som helst, de goda limegenskaperna hos betong och järn gör installationen komplett. Detta examensarbete resulterade att SMA har väldigt unika egenskaper som kommer att göra stora skillnader i byggbranschen. Detta gäller främst hos broar och är en väldigt aktuell metod som har förmågan att anpassa sig till den omgivande miljön. Utöver det kan den även justera sig själv för att säkerställa optimal och säker drift under normala och svåra belastnings villkor och det minimala kravet för underhåll.

För många med en inbrottscentral hemma eller på sitt företag  kan det vara intressant att i realtid kunna få information om statusförändringar. I vanliga fall kontaktas man av larmcentralen som man är ansluten till, men då oftast enbart vid allvarliga händelser som t.ex. larm eller sabotage och oftast med någon minuts fördröjning.I detta arbete visar jag hur man med enkla medel kan utöka funktionaliteten på sitt larmsystem genom att använda sig av en mikroprocessor och en mobiltelefon. Arbetet resulterar i en fungerande konceptuell enhet med förslag till enkla förändringar för utökad funktionalitet och säkerhet.

Abstract The technology has made it possible for you as a train traveller to be available in other places than stations and on the train. You now have the possibility to communicate by the phone or through the Internet to get information about the train you want to take. Development in mobile technology gives us new possibilities to communication. We want to find out whatever SMS would be a suitable approach to inform passengers about delays. The problem formulation is “If the technology is available to inform the passengers about delays in an more effective way than today, then why not use it” The “Kustpilen” that operates in Blekinge and Skåne is the case for our study. To obtain a broad perspective of the problem we will be in contact with travellers as well as with the organisations, which will be involved in a feasible SMS-service. To start with, we will gather information from different literatures to provide the reader an insight in the subject and try to obtain an image of how the technology of SMS is suited within the area of traffic information. The communication with the organisations will go through interviews. This way we are able to form a better understanding about existing problems for present system to take an SMS-service in use. The result demonstrates the travellers to be very positive to SMS. What concerns the travellers mostly is that the operators, which supply the service will not manage to provide the information on time. Regarding the organisations, there is a need for changes within the system at Banverket, which communicates the information about the traffic, for a well working SMS-service to be able to take in usage. Banverket is alone to have information about the train positioning, which makes Banverket an important part of the system.
Sammanfattning Tekniken har gjort det möjligt för dig som tågresenär att vara tillgänglig på andra platser än stationer och ombord på tåg. Du har en möjlighet att kommunicera via telefon eller Internet för att få tag i upplysningar rörande det tåg du avser resa med. Då utvecklingen inom mobil teknik ger oss nya möjligheter till kommunikation vill vi med detta kandidatarbete utreda om en SMS-tjänst skulle lämpa sig väl för ändamålet att upplysa resenärer om tåg är i tid eller otid. Problemformuleringen lyder ”Om tekniken finns till att informera resenärer angående tågförsening på ett effektivare sätt än vad som görs idag, varför används den då inte?” Kustpilen som trafikerar Blekinge och Skåne är föremålet för vår fallstudie. För att få en så bra helhetsbild som möjligt över problemet kommer vi att vara i kontakt med såväl resenärer som de verksamheter som skulle involveras i en möjlig SMS– tjänst. Till att börja med kommer vi att samla in litteratur för att ge läsaren en inblick i ämnet och försöka få en bild av hur SMS-tekniken lämpar sig för att användas inom området för trafikinformation 1 . Genom att fråga resenärer på Kustpilen hoppas vi få en uppfattning hur resenärerna ställer sig till en SMS-tjänst angående trafikinformation. Kontakten med verksamheterna kommer att ske i intervjuform för att på så vis bilda oss en bättre uppfattning om vilka problem det finns med nuvarande system för att ta en SMS–tjänst i bruk. Resultatet visar att resenärerna på Kustpilen är mycket positiva till SMS. Det som oroar resenärerna mest är att de som tillhandahåller tjänsten inte klarar av att ge informationen i tid. När det gäller verksamheterna så måste det till förändringar inom Banverkets system som förmedlar informationen om trafiken för att en väl fungerande SMS–tjänst skall kunna tas i bruk. Det är endast Banverket som har information om positionering av tågen. Vilket gör att Banverket är en viktig del i systemet.

I detta arbete har vi studerat tjänsten SMS-betalning. Vi har fokuserat på att utvärdera och beskriva tjänstens användningsområde samt dess styrkor och svagheter. Utifrån företaget Mobill, vars affärsidé är att utveckla en betalningstjänst via SMS har vi fått en bild av hur ett företag kan satsa på denna teknik. I en studie med konsumenterna har vi visat den generella bilden av inställningen till betalningstjänsten, detta för att vidare kunna utvärdera betalningssystemet. Dessa studier har båda en kvalitativ karaktär. Våra undersökningar har visat att betalningstjänstens främsta användningsområde är för mindre betalningar, så kallade mikrobetalningar, vid betalning av framför allt parkeringsavgifter, biljetter för transport, events och vägtullar. Konsumenternas attityd till betaltjänsten är positiv men fortfarande saknas det något för att en bredare användning ska utveckla sig. Vår studie visar dessutom att SMS-betalningen för med sig en stödtjänst nämligen SMS-biljetten, där SMS-systemet både ersätter betalningen av en biljett samt biljetten i sig. Något som kan ses som positivt och tala för tjänstens fortsatta tillväxt. Hotbilden är dock signifikant och utgörs av konkurrerande tjänster som kan substituera SMS-betalningen genom nyare, säkrare och mer effektiva betalningslösningar med mobiltelefonen som hjälpmedel. Om SMS-betalningens existens är hotad är dock mer tveksamt, möjligen på en längre sikt där även SMS-funktionen som kommunikations medel blir föråldrad och utbytt.


In this thesis we have made a study of SMS-payment. Our focus has been to describe and analyze the possible uses of the service as well as their strengths and weaknesses. Through the business of Mobill whose idea is to develop a payment method via SMS, we have obtained a picture of how a company invests in this technology. Our research has shown that the SMS-payments main area of use is for the so called micro payments, when paying parking fees, tickets for transportation, events or road tax. The attitude of the consumers is positive in general but there is still something missing that will enable a broader use of the service. Furthermore our study shows that the service also enables the customer to get an electronic ticket when paying tickets by SMS. This is a factor that might provide the payment method with more chance of success and further growth. However there is a significant threat to the service, provided by their competitors. These competitors are developing mobile payment systems that can be considered newer, safer and more effective than the SMS-payment and might end up being a substitution. Yet it is more questionable whether the existence of the SMS-payment is threatened. Perhaps in a long term view where even the SMS-function as a means of communicating will be considered old and ultimately end up being replaced.
