Abstract:
Drug addiction is associated with a range of medical and social problems, not least the negative side effects experienced by those who take drugs. The long-term effects are particularly concerning and can be difficult to treat. Changes in the behaviour of long-term drug users can include impulsivity and irrationality, which can negatively impact on society. Dr Kyoji Okita, Integrative Brain Imaging Center (IBIC), National Center of Neurology and Psychiatry, Japan, is working to shed light on the neurological biochemistry behind drug addiction, as this aspect is little understood. In his work, he is collaborating with experts and he intends to apply the knowledge he and his collaborators uncover to real-world treatments. A particular focus for Okita is the use of positron emission tomography (PET) to explore the dynamics behind psychological and addiction biology. Currently, he is investigating the potential of a drug called Istradefylline as a treatment for methamphetamine use disorder. The drug is currently used as part of the treatment for Parkinson's disease and Okita wants to decipher if administering Istradefylline to people addicted to methamphetamines can help increase their dopamine sensitivity and, in the process, reduce their reliance on the drug, as well as the psychological side effects. If found to be effective, Istradefylline could form a key part of a treatment programme for addiction.

Abstract:
Introduction This study investigated variables associated with methamphetamine-related deaths in Thailand. Methods This study used data obtained from methamphetamine-related autopsy cases over a six-year period from 2011 to 2016. From the data available during this period, considered variables included: demographic, toxicological and histopathological profiles. Methamphetamine blood concentration calculations and myoglobin immunostainings in kidney samples were also carried out. Statistical analysis and tests of significance were conducted using a paired-sample t-test, adopting a p-value of 0.05. Results A total of 61 methamphetamine-related cases were reviewed. Of several pathological findings, cardiovascular pathological findings were the most common. Cases were divided into a non-trauma group ( n = 19; 31.15%) and a trauma group ( n = 42; 68.85%), and it was found that methamphetamine blood concentrations of non-trauma cases were largely in therapeutic ranges. The differences between methamphetamine concentrations of trauma and non-trauma groups were not statistically significant ( p > 0.05). Immunostainings for myoglobin in kidney samples were positive in two non-trauma cases, which is suggestive of methamphetamine-induced rhabdomyolysis. Conclusions Methamphetamine intoxication causes cardiac toxicity and can cause death. However, methamphetamine quantitation, autopsy findings and scene investigations are considered altogether in determination of cause of death due to many factors such as drug tolerances. Myoglobin immunostaining was found to be a useful tool in determining cause of death.

There is increasing interest in the role of mindfulness and mindfulness-based interventions to optimize recovery from a substance use disorder (SUD). However, relatively little is known about the theory-based psychological and social pathways whereby mindfulness could have beneficial effects for managing a chronic, relapsing SUD. Informed by Revised Stress and Coping Theory, the present cross-sectional study examined affective, cognitive, and social pathways whereby mindfulness is associated with lower methamphetamine craving. A total of 161 HIV-positive, methamphetamine-using sexual minority men completed a screening visit for a randomized controlled trial. Using a hybrid structural equation model, we examined pathways whereby mindfulness is associated with lower methamphetamine craving. We found that greater mindfulness was directly associated with lower negative affect and higher positive affect as well as indirectly associated with less methamphetamine craving. Interestingly, the indirect association between mindfulness and methamphetamine craving appeared to be uniquely attributable to positive affect. Only positive affect was indirectly associated with lower methamphetamine craving via higher positive re-appraisal coping and greater self-efficacy for managing triggers for methamphetamine use. Methamphetamine craving was supported by moderate associations with greater substance use severity and more frequent methamphetamine use. These findings support the role of mindfulness in cultivating positive affect, which could be crucial to build the capacity of individuals to manage methamphetamine craving as a chronic stressor that threatens recovery from SUD.

This study investigated the relationship between Chinese drug users’ perceived social support and their intentions for choosing abstinence. Both bivariate and multivariate analyses were performed with 3,239 drug users, especially heroin (427) and methamphetamine (890) users. All participants were from multiple Chinese compulsory drug treatment institutions. Perceived support from friends was the factor that showed a strongest positive relationship with Chinese drug users’ abstinence intentions. The positive influence from family support on the willingness to seek and choose abstinence was not as great as scholars have suggested, which challenges previous research findings. Methamphetamine users had a significantly lower chance of pursuing abstinence, but their abstinence intentions were positively related to perceived support from friends. Comparatively, all types of perceived social support had no significant influence on heroin users’ abstinence intentions. This study highlights that, as China’s methamphetamine-addicted population rapidly expands, tailored treatment, especially facilitating positive peer support, is urgently needed.

Abstract:
The term ‘amphetamine’ refers to a class of synthetic drugs which includes methamphetamine. The latter is a globally popular drug of abuse which induces euphoria, affecting cognitive/psychomotor performance and sleep. It also provokes risk taking and violent behaviour. The central effects of methamphetamine are due to the overproduction of neurotransmitters, resulting in high levels of dopamine. In recent years, there have been significant increases in cases of methamphetamine abuse in North and South America, Australia and Asia due to its ready availability and low cost. The following review examines changing trends in methamphetamine use and problems that arise diagnostically in medico-legal cases in determining the significance of post-mortem blood levels, the relationship of these to ante-mortem levels, the possible effects on physical and psychological behaviours and the possible contribution of the drug to a lethal episode.

Purpose The purpose of this paper is to document the experience of current and former methamphetamine users on their crystal meth use patterns and on their use of services related to their chemsex practice. Design/methodology/approach For qualitative component, two focus groups were formed with nine current users of methamphetamine and eight former users. Thematic analysis was performed to know their experiences. Findings All participants were already engaging in chemsex with various substances before they first started using methamphetamine. Methamphetamine use led some to slam (methamphetamine or mephedrone injection). Some participants report that their sexual experiences were intensified early in their chemsex practice. They reported feeling more confident with their partners, feeling like they are sexually attractive and overcoming their barriers to sexuality. The intensification of methamphetamine use and, in particular, injection change the positive perception of sexual life. Thus, for some participants, substance use takes more space and their sexual experiences become less satisfactory. Practical implications Participants report the services that address the phenomenon of chemsex are still scarce in Quebec province. In addition, the few services available aim to relearn a sober sexuality. However, the mourning of the positive aspects of chemsex on sexual experiences seems still very little discussed. Greater consideration of positive chemsex experiences is needed in services that address this issue. Originality/value This project documented the perception of pleasure related to sexual practices among regular methamphetamine users. Their perception of pleasure will help develop services adapted to their reality.
