研究背景<br>耐多藥結核病(MDR-TB)在世界範圍內的廣泛傳播阻礙了結核病的有效控制。世界衛生組織推薦了一系列提高耐藥結核病人治療依從性和治愈率的社會支持政策,包括直接面試下服藥(DOT)、經濟支持、健康教育和心理支持。但是目前還不清楚這些社會支持政策能否促進耐多藥結核病的有效治療。為填補這一領域的研究空白,我們首先針對DOT這一促進治療依從性的主要社會支持政策與耐多藥結核病人治療結果之間的關係做了系統綜述和Meta分析。為了進一步驗證不同社會支持政策對耐多藥病人治療結果的影響,我們在中國浙江省对全球基金支持的國家耐多藥結核病控制項目(NMTP-GF)中納入的病人開展了回顧性隊列研究。<br>研究方法<br>在開展系統綜述和meta分析時,由兩名研究員在常用的電子文獻數據庫中搜索1970年1月至2015年12月期間發表的相關英文文章并提取關鍵信息。Meta分析的結果採用隨機效果模型分析并報告了合併的成功治療率以及95%置信區間(CI)。隊列研究的病人入選條件是:2009年1月1日至2012年6月30日期間開始治療的耐多藥結核病人。採用定量和定性相結合的研究方法收集數據。定量數據來自所有納入病人的病案以及面對面訪談的病人問卷調查。結果採用多元回歸分析方法檢驗社會支持、依從性和治療結果的影響因素并採用路徑分析驗證治療依從性的中介效應。定性數據通過分別對10名治療督導員(通常為社區衛
