Abstract:
This chapter discusses how island of Okinawa achieved the highest longevity in the world and why it then came to have high premature death rate in Japan. Okinawa returned to Japanese control in 1972 and for 20 years from 1975 to 1995 had the highest life expectancy and the highest number of centenarians per 100,000 population in the country. These days, Okinawa has a higher prevalence of obesity and higher mortality rates among 40-65 years-old than mainland Japan. This chapter seeks to understand the reasons for the health changes, from historical and cultural perspectives as example of social determinants of health.

This chapter begins the story of this book on the island of Okinawa, Japan, beginning at dawn on April 1, 1945. This point in time sparked the largest concentrated naval bombardment ever to cover a landing. The chapter charts the dramatic pace of events that followed during that month and beyond. What happened in Okinawa resulted in a change in leadership in Tokyo, which convinced diplomats in the United States that Japan was signaling its intent to end the war. However, the chapter argues, they were mistaken. With the Americans firmly on Okinawa and a new prime minister leading the country, Japan had reached a turning point yet was refusing to turn. Instead, the country continued its fatal path toward the end. The military continued to urge the people to endure sacrifice until they were, hopefully, to be rescued. However, as we now know, this was the beginning of the end.

The wartime occupation of Okinawa demonstrates the crucial role that considerations of race and ethnicity have on the conduct of military government. American military government planners recognized both the possible threat a population of 463,000 civilians might pose and the complexities of the relationship between Okinawa and Japan. Without losing sight of the impact that the civilians would have on military operations, planners from all services, including the Marines, analyzed the ethnicity of the Okinawans and how their cultural distinctiveness might inform their behavior. While the Marines’ policy prohibited further assessment of the population upon landing on the island, preliminary analyses provided the military leadership of all services with a more robust understanding of the battlefield that they faced and thus better prepared them to preserve military lives, safeguard American secrets, and win the battle. Acknowledgment of ethnic differences, done in a manner that seeks educative understanding, should hopefully foster cognizant policy that still supports military goals. An examination of the wartime occupation of Okinawa provides an example for effective military government programs now and in the future.

Roughly 35,000 years ago, hunting-fishing-gathering people occupied the Ryukyu Islands of Japan, a chain of small-sized islands in the western Pacific. There are Paleolithic sites scattered over most of the relatively large islands, thereby suggesting an extensive human dispersal over the sea at least 30,000 years ago. Recent morphological and genetic studies of the human fossils found in this area revealed that Paleolithic occupants might have an affinity with the modern and prehistoric populations of Southeast Asia. Recent excavation of Paleolithic sediments at Sakitari Cave, Okinawa Island, provided a variety of shell artifacts, including beads, scrapers, and fishhooks, and evidence of seasonal consumption of aquatic animals, especially freshwater crabs. The Paleolithic Ryukyu Islanders’ culture and lifestyle, which made use of unique resources, demonstrate behavioral adaptations to living on relatively small islands.

Since Okinawan’s integration into the Japanese nation as a prefecture in 1879, the Japanese government embarked on a program of propaganda and indoctrination to ensure loyalty in its new Okinawan subjects. As the Pacific War drew ever closer to the island of Okinawa, the Japanese government mandated that all civilians work for the war cause. The National Mobilization Act committed every resource toward supporting the war effort and every person prepared for war. Young Okinawans rallied to Japan’s cause with innocent fervor unmatched by their elders. The older population did not share the intensity of the children’s enthusiasm, but they still committed to serving the Japanese Empire as its subjects. In a practical manner, adult Okinawans prepared their families for the rough conditions that would result from a battle waged on their land. On the brink of the battle, Okinawans saw Japan as their country and felt compelled to protect it.

Although old age is readily recognizable, methods to define and measure the underlying biological processes are much less amenable to study. For this reason, life expectancy has been widely used as a surrogate measure of ageing, as well as to monitor economic progress at national and regional levels. It is generally acknowledged that lifespan is a constitutional feature of the human phenotype, and twin studies have indicated that 25–33 per cent of the variance in human longevity is genetic in origin. External factors including lifestyle can also exert a major influence, as illustrated by the current mean life expectancies of 79 and 86 years for males and females in Japan, whereas the comparable figures for Botswana are 35 and 33 years, respectively. The importance of genetic inheritance as a determinant of extended survival has been illustrated by population level studies in Okinawa, an island prefecture of southern Japan with a very high prevalence of long-lived individuals. On the island, the mortality rates of the male and female siblings of centenarians were approximately half those of birth cohort-matched, non-centenarian siblings. These findings parallel an earlier study of the family of Jeanne Calment, who died in France in 1997 aged 122 years. Of her 55 relatives, 24 per cent had lived to &gt;80 years compared to just 2 per cent of a matched control group. However, it remains unclear whether the enhanced lifespan of individuals who exhibit above average longevity is due to a slowing of the overall ageing process or is primarily associated with resistance to major life-threatening pathologies. The concept of an ‘allostatic load’, potentially involving the neuroendocrine, sympathetic nervous, immune and cardiovascular systems, and metabolic pathways, has been advanced to describe the lifetime costs of adapting to physical and psychological stresses. According to this hypothesis, while the actions of biological mediators of stress can be initially beneficial to health, chronic stimulation results in regulatory imbalance and subsequent pathophysiological changes. Empirical studies have indicated increased physiological dysregulation and functional decline at &gt;70 years of age, which would imply that predicted global increases in the numbers of older persons will be accompanied by disproportionately larger groups of individuals with major age-related pathologies.
