Abstract:
During exercise water and electrolytes are lost in sweat. There is a large variation in both sweat rate and sweat composition and as a consequence sweat electrolyte loss can be large, especially for sodium, the primary cation in sweat. The loss of large amounts of sodium in sweat has been linked with hyponatraemia and muscle cramps. Sodium intake is encouraged in some athletes and in some exercise situations, which is in direct contrast to guidelines aimed at the general population aimed at reducing average sodium intakes to 2.4g of sodium per day (6g salt/day). Toxicity of arsenic species is dependent on chemical oxidation state. Inorganic arsenic in the trivalent state, arsenite or As(III), is more biologically active than pentavalent arsenic, arsenate or As(V), and is more toxic by most measures. One consequence of environmental exposure is peripheral vascular disease, which is primarily due to vascular changes, but toxicity to the erythrocyte has not been evaluated. The sodium pump (SP or Na+,K+-ATPase) is a membrane embedded protein complex that pumps 3 sodium ions out and 2 potassium ions into the cell per cycle and in so doing creates a cell membrane electrochemical potential. The membrane potential is critical for any functional cell. In the vasculature, reduction in the voltage potential causes vascular smooth muscle contraction and a narrowing of blood vessels (vasoconstriction) which can lead to increased blood pressure (hypertension). Although clinical prognosis can be improved by using glucocorticoid hormones to augment the ENaC-dependent removal of liquid from the lungs, we still require a better understanding of the underlying mechanism in order to improve treatments in the future.
