Full textThe presence of various sensory stimuli in the neonatal intensive care environment is a major determinant in the development of preterm infants. The impact of excess noise and light on the short, medium- and long-term development of the premature infant is documented by studies. Measures to optimize the care environment are implemented in neonatal units in the West and in other parts of the world, but they are not in Cameroon. However, this problem is more important in these units that the infant mortality rate is considerable. The research involved developing and implementing a nursing intervention protocol aimed at reducing noise and light in two neonatal units in Cameroon and assessing their acceptability and feasibility. To do this, the Sidani and Braden method taking place in four phases and using a mixed estimate was used. Three approaches to data collection are proposed by this method, namely experiential, empirical and theoretical approaches. Levine’s conservation care model and Mefford’s (2004) premature health promotion theory were used to situate the problems experienced by preterm infants from a nursing perspective and to suggest nursing interventions. For the first three phases of the study, the objective data were obtained by direct observation of the environment of the neonatal units and a measurement of the sound and light intensities on these units. The experiential data comes from individual and group interviews with nurses (n = 18), doctors (n = 11) and family members (n = 7). The empirical data are from a literature review and the theoretical data are from the study and the theoretical nursing literature. At the end of this stage, needs were identified, and an intervention protocol developed. Phase 4 consists of implementing the intervention protocol in the form of a pilot project. This phase also included the implementation of correctional measures on the two participating units and a set of interventions aimed at changing the behavior of the personnel of these units, and that of families and visitors in favor of protocol interventions. Based on the framework of Michie et al., Training Program and awareness sessions were offered, supported by various means (signs, flyers and reminders). Then, the implementation of the intervention protocol was carried out over a two-week period with the implementation of a cyclical light program on the participating units. The pilot project was carried out through the recruitment of 29 staff members (22 nurses and seven doctors). The results show that the correctional measures were implemented in full on one unit and partially in the other. The results show a high degree of acceptability and feasibility of such a protocol aimed at reducing noise and light on neonatal units in Cameroon. In conclusion, the study showed that local culture, lack of means and financial resources were not an absolute obstacle in terms of feasibility and acceptability and that a research program could better examine the conditions implantation, efficacy and effects on premature babies. Full textBackground: Collective competence (CC) is a dynamic combination of individual competence that bring superior results. This notion, little studied in nursing context, proves innovative in order to understand the performance of health care teams. Aim: To describe the implementation of the CC by nurses in a context of observation n a Quebec hospital emergency department. Method: Single study case combining documentary sources (n = 32), observations (n = 3) and interviews (n = 3) according to the critical incident technique. Results: An imbalance (eg instability, competence) leads to the combination of individual competence centered on the resolution to a higher quality of care. It causes secondary outcomes (competence development, increased collaboration). Various individual (competence), collective (collaboration, relationships), organizational (management, training) and technical (equipment, environment) factors influence it. Conclusion: The results highlight the significant contribution of the CC and the courses of action to improve the performance of health care teams.

Full textThe aim of this qualitative study was to describe the hospitalization experience of women with a high risk pregnancy on prolonged bedrest and their views of the prenatal care received. The research done on these two specific concepts is quite scarce as of today. Semi-structured individual interviews were conducted with eight French pregnant women who were placed on bedrest for an average of 30 days in a Quebec hospital. A phenomenological design (Giorgi 1997, 2009) was used for data analysis, from which 24 subthemes, seven themes, and two descriptive statements were obtained. The results of this study revealed the evolutionary and complex reality of mothers, who experience: 1) Going from an emotional state of shock to boredom, from chaos to reorganization and from a lack of knowledge to understanding; 2) Having to adapt to a new life with multiple restrictions, difficulties and fears; 3) Dealing with negative thoughts and emotions; 4) To continuously think of the future while living day-to-day. Women perceived their prenatal care as being fundamentally technical in nature. The care received was seen as being mostly technical, very routine and easy to accomplish according to the participants. They also point out patient requests occasionally forgotten and the absence of educational prenatal classes. The inconsistency in assigning the same nursing staff to the same hospitalized mothers is reported. Despite these unfortunate events or lacks in care, participants emphasize the presence of humanism, kindness and competency demonstrated by their caregivers. This qualitative study contributes to a deeper understanding of eight mother’s hospitalization experience and enhances the knowledge and awareness of caregivers caring for these women such as registered nurses. These clinical implications will help to humanize and improve the prenatal care of women on prolonged bedrest in a hospital setting.

Full textBackground: Evidence-based practice suggests a systematic monitoring of pain and level of consciousness for patients receiving palliative sedation by healthcare professionals. There is no measurement scale available to assess pain at end-of-life among those who are unable to communicate and receiving palliative sedation. An update of a systematic review identifies the CPOT as a good candidate to assess pain in this specific population. The RASS has only been validated once among patients receiving palliative care. The purpose of this thesis is to measure the psychometric qualities of the CPOT and the RASS within this population. Methods: A retrospective cohort study was conducted in a palliative care unit of Quebec City. The inclusion criteria were patients aged over18 years, unable to communicate, at the end-of-life (prognostic less than weeks) and receiving palliative sedation. The data concerning the CPOT, the RASS, and socio-demographic data were extracted from assessments made by nurses between December 2016 and January 2019. CPOT’s internal consistency was measured with Cronbach's alpha. The discriminant validity of the CPOT and the RASS was calculated by comparing the total scores before and after the administration of an analgesic. Results: 47 files were included in the study. All CPOT items obtained a moderate-to-acceptable Cronbach's alpha, except the Body movements item. The CPOT and the RASS have good discriminant validity, even if patients were strongly sedated (RASS between -4 and -5). Conclusion: Results suggest that the CPOT is homogeneous and capable of detecting the presence of pain in highly sedated patients. The RASS appears to be able to assess changes in the level of consciousness within this same population. However, more studies are needed to measure other psychometric qualities including the inter-rater reliability and the responsiveness of these scales.

Full textCesareans are highly used and rising in many high-income countries. This overuse both increases costs and lowers quality of care. Identification of the various determinants of cesareans is thus important to optimize their use.We used a population-based retrospective cohort study that covers the deliveries of the 11 hospitals of the French district of Yvelines over the 2008-2014 period and conducted multilevel logit models while controlling for many relevant patient and hospital factors that are known to influence obstetric practices.We first studied the impact of supplementary private health insurance within the DRG-based financing of French hospitals. We found that although private hospitals are financed by a single public payer, like those in the public sector, they performed significantly more cesareans than public hospitals. This result is explained by additional payments covered by private insurance and charged by private but not public hospitals. Second, we investigated whether staffing levels of maternity units affect cesarean rates. We showed that high staffing levels for obstetricians and midwives were associated with lower cesarean rates. Finally, we examined the impact of prenatal care utilization on cesarean rates and then determined whether socioeconomic status affects the use of prenatal care and thereby influences the cesarean decision. We found that women who did not participate in prenatal education had an increased probability of cesareans compared to those who did. We further indicated that low socioeconomic women were less likely to participate in prenatal education while they were more likely to have cesareans

Full textBackground : Nursing workload assessment tools are widely used to determine nurse staffing requirements in the neonatal intensive care unit (NICU). However, several tools exist and it is unclear if they are equivalent. Objective : We aimed to compare three existing workload assessment tools and assess their association with mortality or morbidity among very preterm infants. Methods : Single-center retrospective cohort study of infants born <33 weeks and admitted to a 52-bed level 3 NICU in 2017 to 2018. Required nurse staffing was estimated for each shift using the Winnipeg Assessment of Neonatal Nursing Needs Tool (WANNNT) used as reference tool, the Quebec Provincial NICU Nursing Ratio (QPNNR), and the Canadian NICU Resource Utilization (CNRU). We evaluated correlation between tools using Pearson R. The association between NICU nursing provision ratio (actual number of nurses / recommended number of nurses per shift according to the tools used) during the first 24h, 7 days of hospitalization and whole hospital stay with mortality / morbidity was assessed using logistic regression models.Results : Median number of nurses required per shift using the WANNNT was 25.0 (interquartile range [IQR]:23.1–26.7). Correlation between WANNNT and QPNNR was high (r = 0.92, p < 0.0001), but the QPNNR underestimated the number of nurses per shift by 4.8 (IQR: 4.1–5.4). Correlation between WANNNT and CNRUwas moderate (r = 0.45, p < 0.0001). Nursing provision ratio during the first seven days of admission calculated using both WANNNT and QPNNR was associated with mortality/morbidity (adjusted odds ratio [aOR] (95% CI):0.92 (0.86-0.99); 0.94 (0.89-0.98), respectively). The association between nursing provision ratios for the first 24h and whole hospital stay and mortality/morbidity was not statistically significant. Conclusion : Lower nursing provision ratio during the first seven days of admission is associated with anincreased risk of mortality / morbidity in very preterm infants.

Full textThe tools used in the transformations of the production systems of goods and services in the industrial world to respond to market pressures around the 1970's have been applied since the past two decades in the health system in order to organizing more effectively and more efficiently healthcare for the populations. Facing an increase care demands; an aging population and a lack of qualified human resources in this field, health facilities managers are confronted with constraints that prevent them from responding adequately to the patient's expectations. The implementation of a Home Health care structure appears to be a fair alternative to traditional hospitalization. That helps to treat the patient in his/her environment/home, to reduce psychological pressure, to manage rationally the beds in the hospitals, and to avoid the risk of nosocomial infections, at the end to cut down on health care costs. However, the situation appears more critical in the developing countries where for many services, the current techniques in application to plan and to organize care deliveries are those used in the late nineteenth century in industrialized countries. The maternity ward is most affected by this lack of resources. In Haiti, 75% of births are home deliveries without any qualified medical assistance. The maternal mortality rate is 670 per 100,000.00, the highest in the American continent. In the other side, the infant mortality rate is 80 ? live births. While in the other countries located in the Caribbean as well as Haiti, such as the Dominican Republic, Cuba and Jamaica, there are respectively per 100,000.00 live births, maternal mortality rates of 150, 45 and 170; and infant mortality rates per 1,000.00 births alive, amounted to 27, 6 and 16. We need emergency responses to reduce the risk of death in the motherhood process. The goal of our work is to propose an alternative to the traditional maternity ward in order to prevent and to reduce maternal and infant deaths in the country. Indeed, a partial modeling of Haitian hospital system is carried out into two steps. At first, we address the problem of resources staffing based on engineering of conception. To quantify in optimal way the human resources that will be part of the "maternity home care assistance" team, we have applied the methodology ASCI (Analysis, Specification, Design, Implementation), originally designed at LIMOS laboratory at the University of Clermont Ferrand II by Michel Gourgand and Patrick Kellert Gourgand in the early 90s. With this methodology, we have developed a model of knowledge (with Petri nets) which allowed us to formalize the different knowledge of the system; some models of actions (using simulation with ARENA software) to assess the performance of the system; and a model of results that has provided indicators to intervene and to improve the system if necessary. And to optimize the results found, we have coupled our simulation with an optimization tool "OptQuest for ARENA". Second, the distributed control has enabled us to proceed to the coordination of planning for periodic assistance of the maternity home care. To achieve this goal, we have borrowed a tool of problem formulation applied in industrial engineering which is "MLCLSP (Multi-Level Capacitated Lot Sizing Problem). We have developed a mixed linear optimization algorithm that allows us to obtain optimal results for the planning of medical visits to pregnant women who had previously register to the platform for maternity home care assistance. The optimization of the algebraic model of decision is realized in the MIP solver (Mixted-Integer Programing) - CPLEX, implemented in GAMS (General Algebraic Modeling System)

Full textL’objectif de notre thèse est de contribuer à une meilleure connaissance des déterminants d’une césarienne de qualité et de montrer comment en situation réelle (cas d’un district urbain au Burkina Faso) on peut agir sur ces déterminants pour améliorer la qualité des césariennes.
Dans le cadre d’un projet multidisciplinaire (santé publique, mobilisation politique et sociale, anthropologie) d’Amélioration de la QUalité et de l’Accès aux Soins Obstétricaux d’Urgence - le projet AQUASOU (2003-2006) - nous avons pu mettre en œuvre des activités visant à améliorer l’accès à une césarienne de qualité dans le district du Secteur 30) à Ouagadougou, Burkina Faso. Nous avons mené une étude Avant-Après et utilisé des méthodes d’évaluation mixtes quantitatives et qualitatives pour comprendre dans quelle mesure et comment ce type d’approche globale améliore la qualité de la césarienne. Nous avons utilisé le cadre d’analyse de Dujardin et Delvaux (1998) qui présente les différents déterminants de la césarienne pour organiser et structurer nos résultats. Cette expérience s’étant déroulée dans le cadre d’un projet pilote nous avons également évalué le degré de pérennité du projet AQUASOU quatre ans après sa clôture officielle et analysé sa diffusion au niveau région et national.
Le cadre d’analyse de la césarienne de qualité avec ses quatre piliers (Accès, Diagnostic, Procédure, Soins postopératoires) a permis d’aller au-delà de la simple évaluation de la qualité technique de l’acte césarienne. Il a structuré l’analyse des différentes barrières à l’accès à la césarienne comme par exemple l’acceptabilité des services par la population et le coût de la prise en charge.
L’analyse des discours des femmes césarisées a mis en lumière le sentiment de culpabilité des femmes d’avoir eu une césarienne - ne pas avoir été « une bonne mère » capable d’accoucher normalement. Les questionnements sur la récurrence de la césarienne pour les prochaines grossesses, les dépenses élevées à la charge du ménage, la fatigue physique et les complications médicales possibles après l’opération mettent la femme dans une situation de vulnérabilités plurielles au sein de son couple et de sa famille.
L’évaluation du système de partage des coûts pour les urgences obstétricales mis en place en 2005 dans le district du Secteur 30 a montré qu’il était possible de mobiliser les collectivités locales de la ville et des communes rurales pour la santé des femmes. La levée des barrières financières a pu bénéficier à la fois aux femmes du milieu urbain et rural mais l’écart d’utilisation des services entre le milieu de résidence n’a pas été comblé et cela confirme l’importance des barrières géographiques (distance, route impraticable pendant la saison des pluies, manque de moyen de transport) et socioculturelles.
L’étude sur le rôle des audits cliniques ou revues de cas dans l’amélioration de la qualité des soins a montré que les soignants avaient une bonne connaissance du but de l'audit et qu’ils classaient l'audit comme le premier facteur de changement dans leur pratique, comparé aux staffs matinaux, aux formations et aux guides cliniques. Cependant, l’institutionnalisation des audits se révèle difficile dans un contexte de manque de ressources qui affecte les conditions de travail et dans un environnement peu favorable à la remise en question de sa pratique professionnelle.
L’évaluation de la pérennité du projet pilote quatre ans après la fin du soutien financier et technique montre que les bénéfices pour la population sont toujours là en terme d’accessibilité à la césarienne :coûts directs pour les ménages de 5000 FCFA (US $ 9.8), qualité des soins maintenue avec une diminution de la mortalité périnatale précoce pour les accouchements par césarienne de 3,6% en 2004 à 1,8% en 2008.

Full textAdministrative work is often perceived as a burden by hospital nurses, who have the impression that the accumulation of paperwork prevents them from spending more time with patients. And yet, precious few descriptions exist of this forsaken aspect of nursing work, of these under-valued responsibilities. Existing research about the nursing profession focuses on the evolution of clinical care, which often contributes to obfuscating an important part of the nurse’s workday. Thus, this thesis seeks to open the black box of nurses’ administrative activities throughout various care contexts in two countries: France and the United States. An ethnographic study provided the basis for this research, involving nearly 50 nurses and 700 hours of observation. The results allowed for the identification of 6 categories of administrative and organizational activities, called DOA (Documentation and Organizational Activities). These DOA are symptomatic of an adaptation to new complexities of care, and manifest themselves in three ways: the reporting of indicators and the monitoring of caregiving, organizational activities, and the involvement in the establishment’s institutional life. The ambiguities and contradictions revealed by this thesis demonstrate that perceptions of administrative work are indeed quite varied. If the valorization of nursing work through patient care is legitimate, so is complaining about what distances caregivers from it. This kind of complaint, echoed by the profession, is not always experienced as such in practice. This thesis invites hospital managers and nursing leaders to take into account these different perceptions in their ideal vision of the profession

Full textBecoming a mother is the yearning of many women, even though in Douala, they continue to heavily pay with their very lives the act of childbirth. Paradoxically, the reality about what they go through while being pregnant, and the personnel assigned to manage them remains unrecognized or ignored by the general public. In order to understand this phenomenon, we carried out an enquiry based on observations and discussions with pregnant women, the medical/paramedical personnel, and the relatives of the parturient from March 2008 to December 2010, in five health institutions in the town of Douala. This research takes into account the various elements of discussion to rebuild the anthropological context generated by it, and of which it is also the product. It enables us to understand the contradiction between the valorisation of the parturient status, and the behavioural abnormalities during parturition. As a result, a concordance problem arises between the culturally marked attitudes, and the health norms. Though being vulnerable and aware of the conditions that favour a happy end of the pregnancy, the women still begin prenatal consultations late

Full textAll over the world providing a solid education to nurses is a necessity. In order to face the changes, a lot of countries are conceiving postgraduate programs. Around the world, training in management of nursing managers has become a necessity. To provide nursing leaders abilities to cope with change, many countries have developed training graduate programs in educational administration for the nursing services. Several programs that are being developed in this direction are more or less valued. Several authors believe that evaluation of these programs would better appreciate the achievement of educational goals and ultimately, improve training and management practices. To contribute to the development of new skills that will surely meet the training needs and assist in real change, we propose to make this study which aims to evaluate the training program administration and teaching of the National School of Health and Social Development (ENDSS) of Senegal. This assessment takes into account the views of graduates, teachers, program designers and directions of hospitals. This is a retrospective evaluation research type. A questionnaire survey and interview was conducted with graduates, teachers, program designers and management head of the hospitals. Also, databases of students graduating between 2003 and 2009 were consulted to collect the scores on the certification exam, the number of admission and enrollment and demographic information. This assessment focuses on the major components of the training program Administration and teaching of ENDSS are evaluated through the systemic model of evaluation of educational program of Stufflebeam commonly known as CIPP model, which is retained as the reference of this study. The results of the analysis based on 70 questionnaires and 37 interviews. In order to answer the research questions, the data are analyzed and interpreted using indicators and according to defined criteria. The process of analyzing the results was carried out in three stages. First, data from questionnaires and interviews provided by graduates, teachers and program designers are analyzed as well as the academic record of students enrolled in the program. Second, the review focused on questionnaire data collected from directions of hospitals. Finally, the opinions of all respondents on improvements to the program are analyzed to account for their recommendations thereon. Throughout the analysis of results, data from questionnaires and interviews are presented at a time. The verdict on the effectiveness of the training program Administration and teaching of ENDSS is determined by analysis of indicators related to four types of evaluation proposed by Stufflebeam (1981). Graduates and program designers believe that training is adequate for the exercise of the functions of head of nursing services. Consistency between the concepts of the module Management of health services and the exercise of the function of chief of nursing services is considered stronger in respect to several notions, by graduates, teachers and program designers. Graduates, as well as teachers believe in the majority, provided that supervision is inadequate. More than half of graduates and a majority of program designers believe that the material and didactic resources in the library do not facilitate learning the matter. However, students do well in their school leaving examination, particularly in the last four years reviewed. Teachers, graduates and program designers are satisfied with the training program Administration and teaching of ENDSS. Directions of hospitals are shared on the idea that students are prepared for function of chief of nursing services. In addition, as most of the directions of hospitals is not responding to the strategy of the hospital reform. They unanimously say that they were not involved in the process of developing the program and agree that, in general, the training received by the head of nursing services is quality. Unanimously, the directions of hospitals felt that the concepts should be added or improved in the training program administration and teaching-ENDSS. Interpretation of the data reveals the complexity surrounding the importance of balancing the number of hours to devote to the theory and practice in a training program. In addition, results on knowledge gain express a high need for development of further education or the content of notions Health Information System and Introduction to Computers, Introduction to Management, Program elaboration and Hospital administration. An appropriate choice of land where the student could achieve the learning objectives, a selection of the appropriate period for the course, and a signifier coaching, would be a profitable course for the student. We believe that this research has opened avenues for research perspectives related to professional development of future leaders of the nursing department. Full textIn order to reduce high level of maternal mortality rate estimated in 2005 at 401/100 000 live births, and following task shifting innovative strategies already initiated in the country to increase access to emergency obstetric care in Senegal, an alternative training program has been implemented for general medical doctors working in remote areas, to be effective obstetricians gynecologists. Adapted from the normal curriculum, this program combines distance learning using new information and communication technology, practical training in university facilities, coaching and mentoring by university teachers in learners’ heath centers, on line evaluations and also formal annual evaluations such as the normal program. The program has been implemented in two districts in Senegal by Obstetrics and Gynecology Unit of Dakar University. This midterm evaluation analyzes the program implementation and effects using case study with qualitative and quantitative data. Results show a positive evolution of emergency obstetric care indicators compared with two control districts, an increased level of trainees’ knowledge and skills, and also satisfaction of trainees, trainers and beneficiaries. However, some weaknesses have been identified in the program implementation particularly regarding implementation of new training innovations, coordination and sustainability by Ministry of Health.

Full textHeart failure (HF) is characterized by the heart’s reduced capacity to pump, resulting in symptoms causing respiratory problems and reduced quality of life. HF patients are advised to adopt self-care behaviours to prevent frequent episodes of decompensation. While medication and behaviour change play a major role in maintaining physiological stability, patient adherence to self-care recommendations is not optimal. The goal of this pilot study was to evaluate a Motivational Intervention following the Stages of Change (MISC) on HF patients’ self-care behaviours. To guide the MISC, the situation-specific theory of heart failure self-care (Riegel & Dickson, 2008) was retained as well as Bédard et al. (2006) model of intervention, a combination of Motivational interviewing (MI) (Miller & Rollnick, 2006) and Transtheoretical model (TTM) (Prochaska & DiClemente, 1984). This was a randomized experimental pilot project (pre and post-test, with a control group) (n=15/group). Patients in the control group received usual care from the HF clinic while patients from the experimental group (EG) received 3 MISC interventions. Data were collected at one month post-randomization by a blinded research assistant. The effect of the intervention was assessed on five self-care outcomes using analysis of covariance models: self-care maintenance and management (general and specific to HF), confidence in self-care (general and specific to HF) and conviction. Acceptability and feasibility were assessed. Significant results were obtained regarding the confidence in performing self-care behaviours specific to HF. A majority of patient in the EG did progress in the stages of change. The results suggest that a MISC intervention has potential to improve self-care. A larger study is needed to evaluate the effects of the MISC intervention in a randomized controlled trial.

Full textSeveral studies have shown that many patients undergoing a cardiac surgery suffer from moderate to severe pain. These levels of pain may be explained by individual barriers toward pain relief. Knowledge acquired on pain-related cognitions has not been translated into new approaches. The aim of the doctoral project was 1) to develop an intervention to facilitate the self-management of postoperative pain after cardiac surgery and, 2) to assess its the acceptability and feasibility and to investigate its efficacy potential on pain relief in patients undergoing cardiac surgery. The development of SOULAGE-TAVIE included four steps based on clinical, empirical and theoretical knowledge and inspired by diverse methodological considerations. SOULAGE-TAVIE consists in a 30-minute computer-tailored preoperative educational session about postoperative pain management, tailored according to an algorithm. Two short reinforcements are provided in person. The evaluation of SOULAGE-TAVIE consisted in a pilot randomized controlled trial including 60 participants assigned to the experimental group (EG: SOULAGE-TAVIE) and the control group (CG: usual care including educational pamphlet). Data were collected at the time of admission and across Day 1 to 7 after surgery. The results provide preliminary support for the acceptability and feasibility of a tailored and virtual intervention. The results revealed that patients of the EG did not experience less intense pain but they reported significantly less pain interference when breathing/coughing, exhibited fewer pain-related barriers, and consumed more opioid medication. This pilot study provides promising results to support the potential benefits of this new web-tailored approach. Patient empowerment is complementary yet crucial in the current context of care and may contribute to improve pain relief.

In front of this increase, it is relevant to assess the effects of an educational intervention with people with T2DM their sense self-efficacy and self-care capacity to make their most optimal therapeutic adherence. This study use an experimental design: before / after 3 months. Adherence was assessed using the HbA1c <7% and the SDSCA for self-care behaviors of DMSES for self-efficacy. The formed sample: 71 65 GE and GC. GE has improved the level of self-care behaviors, the implementation of recommendations, the feeling of self-efficacy and HbA1c. As a nurse education promotes adhesion among T2DM. This study could contribute to the development of nursing knowledge and renewal of clinical practices.

Full textThe purpose of this study was to evaluate from a constructivist perspective, partnering with the stakeholders , the usefulness of nursing interventions inspired from The Best Practice Guidelines in Bereavement Care (BPGBC) in a first line context, during the follow up a couple whom had experienced a prenatal lost in the last six months. A case study design was used along with The Fourth Evaluation’s methodological approach designed by Guba and Lincoln (1989). An experienced nurse in the care of bereaved families, used the BPGBC to guide her interventions in five therapeutic meetings with the couple, followed by four interviews with the stakeholders. The interventions considered the most helpful and the less useful were identified. The gathering of the data and their analysis followed an interactive and ongoing process. The results support the pertinence of the nursing interventions inspired from the BPGBC, as well as the specific contribution of the nursing profession in the bereavement work. Implementing the BPGBC becomes a realistic goal when the managers and the clinical professionals are made aware of the issues encompassed by the grieving population. The nursing profession could incorporate the BPGBC in its training curriculum, in the research field and in the care offered to the bereaved population in different care settings.

Full textThe purpose of this research was to develop, field-test and evaluate the effects of a reflective practice intervention (RPI) with nurses who care for hospitalized elders. Based on the theory of Human Caring put forth by Watson (1979) and on the Model for Structured Reflection developed by Johns (2006), the study used a mixed design. A qualitative action-research approach was used for the development and testing of the RPI. The evaluation of the intervention used a qualitative approach and a quantitative quasi-experimental approach with a comparison group (CG). Upon completion of the RPI, the nurses were invited to identify the benefits of the intervention on their skills and nursing knowledge and on their perception of the RP as a way of improving their professional practice. Three hypotheses were advanced: following this RPI and compared with the CG, nurses in the experimental group (EG) would improve their attitudes and knowledge regarding elders, identify a larger number of nursing interventions adapted to the hospitalized elder clientele, and achieve a higher level of reflection. The study was carried out with 43 university-hospital nurses (EG = 22; CG = 21). The RPI spanned a 22-week period. It included eight thematic workshops each 75 minutes long, delivered at three-week intervals, combined with reading assignments and individual exercises. The RPI focused on three themes central to elder hospitalization: medication, mobilization, and discharge planning. Qualitative and quantitative data were collected through questionnaires, both open-ended (vignettes, reflective essays) and standardized (Kogan’s Attitudes Toward Old People Scale, Palmore’s Facts on Aging Quiz), completed pre- and post intervention by both groups. Nurses form the EG also completed a questionnaire on the RP experience and some took part in focus groups. A thematic content analysis was carried out on the qualitative data using the approach proposed by Miles and Huberman (2003), and Wilcoxon rank tests were run on the quantitative data. Results show that the nurses developed different skills and abilities required for a RP, including introspection, openness to others, and critical analysis. The participants recognized that the RPI allowed them, among other things, to develop empiric, ethical, esthetic, personal and emancipatory knowledge. Moreover, they saw the RPI as a means of improving professional practice, particularly thanks to its potential for countering the routinization of care. Furthermore, compared with their CG counterparts, the EG nurses significantly improved their attitudes and knowledge regarding elders. However, no significant inter-group differences emerged following the RPI in terms of either the number of interventions adapted to situations experienced by hospitalized elders or the level of reflection achieved. Research hypotheses were partly supported. This study demonstrates the potential of a RPI as an innovative professional development approach that valorizes the experience of nurses while allowing them to change their attitudes towards elders in a positive fashion and to adjust their knowledge according to the needs of this frail and vulnerable clientele. Possible avenues of pursuit are proposed for clinical practice, training, management and research.

Full textNursing interventions that target motivation to adopt healthy behaviors, such as brief motivational interviewing (MI), can help reduce cardiovascular risk factors. While face-to-face MI training lacks accessibility, e-learning use for MI training is promising because of the flexibility it offers. The objective was to assess the feasibility, acceptability and preliminary effect of a web-based e-learning platform for brief MI (MOTIV@CŒUR) on cardiovascular nurses’ clinical use and perceived skill in brief MI. A single group pre-post pilot study was conducted to evaluate MOTIV@CŒUR with nurses working in a coronary care unit. The web-based e-learning platform consists of two sessions with a total duration of 50 minutes based on videos of nurse-patient interactions. In each session, a theoretical introduction of brief MI is followed by role playing based on real life clinical situations in which a nurse practitioner evaluates patients’ motivation to change, and intervenes according to the principles of brief MI. The clinical situations target smoking, medication adherence, physical activity and diet. Nurses were asked to complete both training sessions online within 20 days. Data on feasibility, acceptability and preliminary effects (perceived skills in brief MI and self-reported clinical use of conviction and confidence interventions) were collected at 30 days (± 5 days) following the first session. We enrolled 27 women and 4 men (mean age 37 ± 9) in March 2016. Twenty-four out of 31 participants (77%) completed both sessions in less than 20 days, and at one month, 28 had completed at least one session. The training was found highly acceptable, with information quality, perceived ease of use, and system quality scoring the highest. The score of self-reported clinical use of confidence interventions was higher after the two sessions than before the sessions (P = .032). While all scores increased from baseline, other results were not statistically significant. In conclusion, the implementation of a web-based e-learning platform for brief MI is both feasible and acceptable among cardiovascular care nurses. Moreover, it can have a positive effect on self-reported clinical use of confidence interventions towards health behaviour change.

Full textOlder persons occupy a growing proportion of the beds in Quebec short-stay hospitals. Many of these persons present acute confusional state (ACS), or delirium, in the course of their hospitalization. The nursing care that they require is complex but studies show that continuing nursing education takes scant account of this reality. The approaches used in these studies have focused above all on the acquisition of knowledge and technical skills and have neglected the creative, relational, critical, reflexive and ethical aspects essential to delivering quality nursing care. In addition, these studies contain little information relative to the design of pedagogical interventions and their evaluation. Against this background, a study was undertaken aimed at field-testing and qualitatively evaluating the process and results of a pedagogical intervention for nurses who care for hospitalized older persons with ACS. The focus was more specifically on identifying the facilitating and constraining conditions to learning, the aspects most useful to practice, the different types of knowledge expressed, and the care outcomes perceived by participants. Inspiration for the design and evaluation of the intervention was drawn from the narrative pedagogy approach developed by Diekelmann (2001) and the various patterns of knowing knowledge defined by Chinn and Kramer (2008). The narrative pedagogy at the root of this intervention consists essentially of describing the lived care experiences of nurses and creating a learning environment conducive to group interpretation of these experiences based on theoretical and empirical data. To achieve the objectives, a case study design was used. Purposive sampling allowed selecting participants working all three work shifts, possessing different levels of training, boasting nursing experience ranging from eight months to 36 years, varying in age from 23 to 64 years. The sample, composed of 15 nurses who worked on cardiac and orthopedic surgery units and who often cared for persons with ACS, was divided into three equal groups of five. The pedagogical intervention comprised four days of training offered at an interval of three weeks for a total duration of 12 weeks. On each of these days, participants had to complete a written reflexive exercise regarding a lived care situation involving a person with ACS and then, in the context of a group discussion, share, interpret and question these situations by drawing connections with available theoretical and empirical data on ACS. A triangulation of data collection methods, including the investigator’s field notes, the participants’ written reflexive exercises, questionnaires completed by the participants after each day of training, and individual interview with each participant carried out by an outside interviewer at the end of the intervention, allowed describing the field-testing of the intervention and qualitatively evaluating its process and results. The qualitative intra and inter-participant data was then subjected to content analysis. Field-testing of the intervention highlighted the importance of taking account of the varied needs of the participants and to adjust the pedagogical intervention from one group to the next, particularly with respect to the theoretical and empirical content on ACS. Process evaluation underscored that the intervention was facilitated by the attitudes and the diversity of experiences of the participants, as well as by the use of real care situations, which allowed integrating theory in practice. Access to new tools for assessing persons with ACS was perceived to be particularly useful by the participants. As for the results evaluation, it allowed revealing various types of knowledge, including of the empirical, ethical and esthetic sort, but also some personal and emancipatory knowledge expressed by the participants. Moreover, the participants performed more in-depth assessments of persons with ACS, reduced or avoided the use of physical control measures on these persons, and involved families more in care. The evaluation also allowed documenting some of the effects perceived by the participants on the physical and psychological well-being of care recipients and their families. Persons with ACS were reassured, calmer and soothed, among other things, while families were less worried and more involved in care. The study’s results evidence the applicability of a narrative pedagogy intervention based on a nursing frame of reference and its utility in continuing education in the field of care. The study opens up the possibility of transferring the intervention to other populations of nurses who tend to clienteles with complex needs, particularly in geriatric, oncological and palliative care. It is proposed that future studies evaluate the intervention on a larger sample and explore its effects on the persons cared for and their families.

Full textPatients with an implantable cardiac defibrillator (ICD) decrease their risk of mortality related to life threatening arrhythmias. However, multiple concerns about this device exist, including ICD shocks, which may result in increased anxiety and a perceived limitation in performing everyday activities. These emotional reactions and behavioral changes may have a negative impact on the acceptance of the device. The purpose of this randomized pilot study with a group control (n=15 / group), was to verify the feasibility and acceptability of an individualized nursing intervention aiming to decrease anxiety, improve performance in everyday activities and device acceptance in patients with newly implanted ICDs. Based on Human Caring theory and a cognitive behavioral approach, this intervention addressed each patient’s unique concerns. From the identified concerns, the nurse intervened focussing on the patient’s dysfunctional beliefs that can lead to anxiety and avoidance behavior. After randomization, the first encounter for the experimental group (EG) was face-to-face before hospital discharge. Subsequently, two encounters were made via telephone, 7 and 14 days after discharge. The results support the feasibility and acceptability of the study design and the intervention evaluated. In addition, they emphasize the potential of the intervention to reduce symptoms of anxiety among the participants of the EG. The results of this pilot study may provide directions for future research and help guide clinical practice.

Full textThe use of an assessment tool for delirium detection (ATDD) is important to intervene as quickly and as efficiently as possible. The aim of the study was to evaluate the efficacy of a knowledge transfer tailored intervention (KT-TI) to optimize the conform use of an ATDD by nurses. The intervention was based on barriers and facilitators to the use of an ATDD. The barriers identified with the questionnaire (n=30) revealed a need for knowledge and an improvement of nursing skills. One of the barriers identified in focus groups (n=4) was the lack of knowledge on delirium causes and specific nursing interventions to favour accordingly. The chosen KT activities were a clinical capsule on delirium manifestations as evaluated by the ATDD, and a pocket sized information card on possible nursing interventions. The rate of conform use of an ATDD was evaluated in pre and post KT intervention (pre-post design) with a review of 242 patient’s medical charts (pre n=121; post n=121). No significant difference was noted between pre and post KT-TI (p > 0.99). An explanation for this resides in the fact that the observed rate of use of an ATDD by nurses was already high (> 85%) in pre KT-TI on two work shifts. The KT intervention based on barriers and facilitators to change was appreciated by nurses and it may have the potential to promote evidence-based practice.

Full textIn Quebec, nearly 25 000 people are affected by Parkinson’s disease (PD). They are, for the most part, elders cared for by their spouse. At the moderate stage, PD alters the couples health and quality of life. This stage of the illness offers opportunities for dyadic interventions, as these couples experiment growing losses requiring numerous adjustments. Nevertheless, no study had yet explored their intervention needs during this transition and few interventions to support them have been the object of evaluative studies. Based on transition theory by Meleis and colleagues (2000) and on family systems approach by Wright and Leahey (2009), the purpose of this study was to develop, implement and evaluate a dyadic intervention oriented towards elderly couples living with PD at the moderate stage. To achieve this goal, a qualitative design and a participative approach were privileged. The Intervention Mapping framework by Bartholomew and colleagues (2006) and the analytic methods developed by Miles and Huberman (2003) were used to conceptualize and evaluate the intervention. The study took place in an ambulatory clinic specialized in PD. Ten couples and four health professionals collaborated to the intervention development. Three new couples tested and evaluated the intervention. The dyadic intervention consists of seven sessions of 90 minutes, held every second week. The themes of the sessions and the intervention methods and strategies are based on the couples needs and goals, as well as on specific theories and empirical findings. The intervention is oriented towards dyads concerns, health promotion, problem solving, access to resources, communication, and role adjustments. The study results showed the feasibility, the acceptability and the utility of the intervention. The main benefits observed by the dyads are the adoption of health behaviors, the finding of solutions adapted to various situations and favorable to both partners, the skills to get help and services, and the increased feelings of mastery, mutual support, pleasure and hope. This study can illuminate nurses involved in different domains of practice and interested in developing and evaluating dyadic interventions that are ecologically and theoretically based.

Full textSince the enactment of Bill 90 in 2002, the Nurses Act (section 36) gives nurses greater autonomy by recognizing their competency and responsibility in clinical assessment, featured in this Act as the corner stone of the nursing profession (Pellerin, 2009; OIIQ, 2010). Assessment constitutes a fundamental step in the clinical assessment process (Dillon, 2007; Doyon, Brûlé & Cloutier, 2002; Pellerin, 2009; Weber & Kelley, 2007). The emergency nurses have a major responsibility with regard to patient assessment. In fact, they work with patients of all ages that have various health problems that can be acute and sometimes urgent, and they must intervene, according to their clinical assessment, before a diagnosis has been established or the patient has seen a doctor (Newberry & Criddle, 2005). Yet, despite the importance now conferred by law on clinical assessment, this role has yet to be accepted as an integral part of clinical nursing. The aim of this qualitative descriptive study is to explore the perceptions of nurses, nursing managers and clinical resource nurses with regard to assessment. The clinical standard of the Emergency Nurses Association for patient assessment (1999) is the frame of reference for this study. Five 60-minute individual semi-structured interviews and a 90-minute discussion group were conducted with emergency nurses. A second discussion group included two nursing managers and two emergency clinical resource nurses. The data collected were analysed using the approach proposed by Miles and Huberman (2003). The results obtained show that nurses view patient assessment as a fundamental part of their emergency responsibilities that influences the improvement of the quality and security of the care given. However, the results also revealed that their understanding of assessment varies, and that this practice is random and largely influenced by the biomedical model. Several factors influence nursing practice regarding patient assessment, in particular: individual notions of assessment and the role of nurses, the relations and interactions between members of the nursing team and other professionals, as well as the support of the organisation and the nursing managers. This last is a leading factor in the context of the emergency department and shows that the support of the nursing managers and clinical resource nurses has an important impact on practice in clinical settings. This study proposes further areas of study focused on academic training, continuing professional education, nursing management and inter-professional collaboration that could facilitate the actualisation of nurses’ roles concerning patient assessment to improve the quality of care provided in the emergency department.

Full textSince 2000, the professional licensure exam of the Ordre des infirmières et infirmiers du Québec (OIIQ) is comprised of two parts; a written exam and a practical exam in the form of an objectively structured clinical examination (OSCE). Various establishments have created preparation programs in order to help candidates study for the exam; however, these programs have never been evaluated. The goal of this study was to qualitatively assess a preparation program for the professional licensure exam. Focus groups were held with candidates who had participated in a preparation program. The groups explored the program’s impact on the candidates’ experience during the professional licensure exam, as perceived by the candidates themselves. The data analysis was performed based on the method of analysis by analytical questioning of Paillé and Mucchielli (2008). The second-generation competency-based approach developed by Goudreau, Pepin, Dubois, Boyer, Larue and Legault (2009) was used as a framework for this study. Analysis of the interviews identified candidates’ expectations with regards to the preparation program and its impact on their experience during the professional licensure exam. The program allowed candidates to assess their strengths and weaknesses in order to guide their study sessions, while reducing the stress linked to the fear of the unknown and increasing their motivation and confidence. The OSCE and the feedback provided after OSCE are the activities perceived as being the most relevant, both by the candidates and from a competency approach perspective. On the other hand, the written exam was perceived as being less relevant for the exam preparation and less relevant to competency development. Suggestions to improve the preparation program were gathered from the interviews with the participants and recommendations for management, practice, education and research in nursing science were made as a result of this study.
