Abstract:
This book is designed to furnish Italian literature with an insight into the significance and the role of knowledge transfer, and in particular of technological brokerage. The idea is that, in the present-day world, dominated by a technology and knowledge available to an increasingly large number of people, enterprises are called upon to reconfigure the concept of innovation, expanding in even geographical terms the quest for solutions that aim at creating an exchange of interdisciplinary knowledge. To respond to the need for the dissemination of knowledge, collaboration between enterprises and the use of brokers appears to be the easiest solution. This can contribute to reducing the inefficacy of the markets and hence to facilitating the technological transactions. In this context the role of the brokers is fundamental in the knowledge markets in general, and in particular in that of technology, spawned by the need for an increasingly complex brokerage of knowledge, between applicant and user. In traditional markets, in effect, transactions can be conducted directly by the enterprises and may deal with current or future technology, but there is also the possibility of indirect transactions, involving the intermediation of specialised brokers. The emergence of these brokers is due to the frequent presence of structural gaps in the real markets which do not permit the normal flow of information: in practice, it is rare for every agent in a market to be connected with all the other agents that may important for him.

Abstract:
Civilian and military retrieval services commonly respond to mass casualty events and international disasters. It is necessary to adapt usual practices to achieve the most for many. The structures, systems, language, and discipline take on a military flavour in civilian disaster response. This brings some order to the chaos and facilitates multiagency cooperation. Triage, treatment, and transport must occur in unfavourable environments. This is exemplified in military scenarios where there is ongoing risk to casualties and retrieval teams. Medical care provided by retrieval teams will depend on risk and resources. Staged retrieval may be required. This is also the case with civilian international retrieval where the patient may be transferred to an intermediate destination facility for immediate care, before being repatriated to their country of origin. Also included, is a section on medical emergency response teams which provide a critical care response to deteriorating patients in a hospital ward setting.

Barr, Jane Ellen. Stoma therapy in palliative care. Oxford University Press, 2015. http://dx.doi.org/10.1093/med/9780199656097.003.0412.

Full text
Abstract:
Patients with ostomies, wounds, or incontinence in the setting of a serious or life-threatening illness experience numerous challenges, including distress related to pain and other symptoms, psychological disturbances, and family concerns. Expert management of these conditions and their many complications is an essential part of a comprehensive palliative plan of care. In many countries, nurse specialists with advanced training in the management of ostomies, wounds, or incontinence are available as consultants or as members of a specialist palliative care team. These professionals can improve health care and quality of life for selected patients across venues of care that include hospital, home, long-term care, hospice, and specialized settings. If a stoma nurse specialist is available, he or she may have a key role in directing decision-making and care management related to these problems, evaluating and controlling symptoms that cause patients and families suffering, and providing psychosocial and spiritual support.

Kahn, Jeremy M. The Role of Long-Term Ventilator Hospitals. Oxford University Press, 2014. http://dx.doi.org/10.1093/med/9780199653461.003.0004.

Full text
Abstract:
Long-term ventilator facilities play an increasingly important role in the care of chronically critically ill patients in the recovery phase of their acute illness. These hospitals can take several forms, depending on the country and health system, including step-down units within acute care hospitals and dedicated centres that specialize in weaning patients from prolonged mechanical ventilation. These hospitals may improve outcomes through increased clinical experience at applying protocolized weaning approaches and specialized, multidisciplinary, rehabilitation-focused care; they may also worsen outcomes by fragmenting the episode of acute care across multiple hospitals, leading to communication delays and hardship for families. Long-term ventilator facilities may also have important spillover effects, in that they free ICU beds in acute care hospitals to be filled with greater numbers of acute critically ill patients. Current evidence suggests that mortality of chronically critically ill patients is equivalent between acute care hospitals and specialized weaning centres; however, mechanical ventilation may be longer and cost of care higher in patients who remain in acute care hospitals. Given the rising incidence of prolonged mechanical ventilation and capacity constraints on acute care ICUs, long-term ventilator hospitals are likely to serve a key function in critical illness recovery.

Full text
Abstract:
Definitions of ‘priest,’ ‘prophet,’ or ‘sorcerer’ must take account of the social setting of such religious specialists. Though most definitions are rooted in biblical traditions, ‘priest’ can serve as a useful umbrella term in a typology of terms of religious authorities, describing a person serving a community as the main specialist responsible for transmitting religious knowledge and establishing the relationship between individuals or the society as a whole and superhuman beings by performing rituals on behalf of the former and for the pleasure of the latter. This approach rejects a functional distinction between priests and sorcerers, given it embodies a contingent theological dichotomy between ‘religion’ and ‘magic.’ ‘Prophets’ are primarily performers of mantic techniques used to convey the gods’ messages to people, which in the Bible and the Qur’ān is mainly seen as conveying the ‘divine word.’ More generally, prophets also perform tasks similar to other priests and sorcerers.

Full text
Abstract:
Successfully weaning patients from prolonged mechanical ventilation requires the varied expertise of a dedicated multidisciplinary care team. Traditionally, this care was provided in acute care hospitals, increasingly these patients are transferred to specialized weaning centres. These may improve patient outcomes by concentrating weaning expertise in a low-acuity environment and implementing protocols for liberation from mechanical ventilation. However, these centres might also worsen patient outcomes because they typically offer less intense nurse and physician staffing compared with traditional intensive care units. Generally, the clinical evidence is mixed, with the best studies suggesting that weaning centres offer similar outcomes as acute care hospitals, but at lower costs. Health systems also might stand to gain from dedicated weaning centres, because they can release intensive care unit beds for more acutely-ill patients. Many gaps remain in our understanding of which patients should be transferred to dedicated weaning centres, the optimal timing of transfer, and the best approach to care for patients in this highly specialized setting.

Abstract:
Terms are pervasive in scientific and technical documents and their identification is a crucial issue for any application dealing with the analysis, understanding, generation, or translation of such documents. In particular, the ever-growing mass of specialized documentation available on-line, in industrial and governmental archives or in digital libraries, calls for advances in terminology processing for tasks such as information retrieval, cross-language querying, indexing of multimedia documents, translation aids, document routing and summarization, etc. This article presents a new domain of research and development in natural language processing (NLP) that is concerned with the representation, acquisition, and recognition of terms. It begins with presenting the basic notions about the concept of ‘terms’, ranging from the classical view, to the recent concepts. There are two main areas of research involving terminology in NLP, which are, term acquisition and term recognition. Finally, this article presents the recent advances and prospects in term acquisition and automatic indexing.

Abstract:
Soft tissue injuries of the hand are common and may present variously to therapists, general practice, emergency departments, or surgeons. Simple injuries can be treated without specialist input, but the majority of hand trauma should be referred to dedicated hand surgery centres. Diagnostic error and delayed specialist treatment may lead to poor outcomes and long-term disability. This chapter provides a detailed overview for non-specialists, as well as an aide memoire for hand surgeons and hand therapists. It includes the principles of general wound care, the management of nail injuries, the treatment of extravasation and high-pressure injection injuries, and gunshot wounds. We describe different levels of upper limb amputation, as well as microvascular reconstruction (both replantation and revascularization). We also cover thermal injuries (burns and frostbite), trauma-induced cold sensitivity, and factitious injury.

Abstract:
As most cancers occur in middle-aged or older adults, only a very small proportion of the overall radiotherapy workload relates to children and young people. As there is a wide spectrum of different cancer types in this age group, not only is paediatric cancer uncommon overall but each individual type is very rare. There are many ways in which to deliver radiotherapy, including advanced photon techniques, proton beam therapy, brachytherapy, and molecular radiotherapy. For these reasons, the care of children and young people requiring radiotherapy is limited to a small number of highly specialist centres. Delivery of high-quality paediatric radiotherapy requires a multiprofessional team including radiation or clinical oncologists, therapy radiographers, physicists, dosimetrists, anaesthetists, and play specialists. This team has to interact very closely with the wider paediatric and adolescent oncology multidisciplinary team, which includes oncologists, surgeons with different anatomical expertise, radiologists, and pathologists. Children, with their developing tissues and organs, are more susceptible to long-term radiation-induced complications, including second cancers, than adults. The art of paediatric radiotherapy, therefore, is to select treatment approaches which offer the maximum chance of cure while minimizing the risk of adverse effects. Careful teamwork, peer review of radiotherapy planning, and quality assurance within a clinical trial framework offer the best chances of achieving this balance. This book covers all these aspects, highlighting the need for highly specialist teams with the extensive knowledge and the broad skillset required to offer children and young people the best possible treatments.

Abstract:
Successful transitioning involves input from both paediatric and adult multidisciplinary teams with overlap between the two services and tailored to the needs of each individual. This includes varying the duration of the preparatory phase and the timing of transition and eventual transfer based on the chronological and developmental age, maturity, medical stability, and psychosocial issues of each individual patient. Buddy systems and peer support may aid smooth transitioning for some, with the promotion of patients attending a clinic with similar age groups, which can include the formation of a young adult clinic. Effective collaboration between children’s and adult services is required to obtain successful transition of adolescent patients. Moving from one paediatric hospital to different adult specialists in different hospitals requires excellent communication between various teams. Some staff members from each adult nephrology unit should specialize in adolescent medicine and construct their own transitional care pathways to ensure effective communication and collaboration with appropriate paediatric units and facilitate continuity of care with ongoing educational and social programmes.

Abstract:
This pocket handbook of paediatric neurology provides practical advice on the clinical approach, and ‘at a glance’ overviews and aides-memoire, to common and rare disorders and clinical scenarios. Precise and compact, the book includes many helpful tables (on aetiologies and differential diagnoses) and figures (e.g. innervations, neuroradiological anatomy). The book is divided into seven chapters: (1) Clinical approach, (2) Neurodiagnostic tools (both giving practical guidance on an orderly approach to how and why to perform specialist tests and how to interpret the results, along with introductions to neurophysiology and neuroradiology), (3) Signs and symptoms (offering a distinctive clinically oriented systematic approach), (4) Specific conditions, (5) ‘Real-life’ examples of consultations with other services, (6) Emergencies, and (7) A comprehensive, practically orientated Pharmacopeia. The Handbook’s contributors are trainees who have recently got to grips with the subject, and senior colleagues whose long teaching and clinical experience bring a fresh and pragmatic approach to everyday clinical situations This equips general and neurodevelopmental paediatricians with the knowledge they need to meet the neurological needs of the young people they see; provides an ideal introduction and essential reference for trainees on short-term child neurology attachments or subspecialty trainees in neurology, neurodisability, and developmental paediatrics. An international perspective includes North American emphases. The new edition has an expanded neuroimaging section and an increased emphasis on genetic aspects of neurological disease.

Abstract:
Recent research has provided important insights on the long-term outcomes of patients who develop acute kidney injury (AKI) in the setting of critical illness. Large epidemiologic studies have demonstrated compelling associations between episodes of AKI and progressive kidney disease and death, respectively, although such studies do not establish causality due to the potential for confounding. Whether AKI is intrinsically toxic or a mere by-product of serious comorbidities (e.g. prior chronic kidney disease, heart failure, diabetes), there is no doubt that AKI survivors are a high-risk group who would likely benefit from close post-discharge follow-up. Recent studies have shown that a minority of patients with AKI receive specialized nephrology follow-up after discharge, suggesting an opportunity for quality improvement. Emerging research is evaluating factors that predict chronic kidney disease, end-stage renal disease, and death among AKI survivors. This work will, it is hoped, suggest new targets for prevention and treatment, with the goal of enhancing the likelihood of recovery following AKI.

Abstract:
Chronic kidney disease (CKD) is associated with a variety of outcomes, some of which are directly and indirectly related to kidney disease, but which ultimately impact on patients’ quality of life and long-term outcomes. The events to which people with CKD are exposed ultimately determine their risk and prognosis of both progression to needing renal replacement therapy, or other morbidities and mortalities. The notion of competing risk is important. The five major outcomes of CKD are: progression of CKD, progression to ESRD (either dialysis or transplantation); death; cardiovascular events; infections; and hospitalizations. Where data is available, not only the risk of the specific outcome, but the factors which may predict those outcomes are described. Each section describes what is currently known about the frequency of the outcome, the limitations of that knowledge, the risk factors associated with outcome, and implications for care and future research. Available published literature often describes outcomes in CKD populations as if it is a homogenous group of patients. But it is well documented that outcomes in those with CKD differ depending on stage or severity, and whether they are or are not known to specialists. Where possible, each section ensures that the specific CKD cohort(s) from which the information is derived is clearly described.
