It is known that children feel and keep the memory of pain, that when untreated has consequences in their lifes, in short and long term. Pain relief and control should be a priority for all health professionals in all healthcare settings. Thus, it is fundamental to recognize, evaluate, prevent and treat pain in children, using non-pharmacological and/or pharmacological interventions. Nurses have a fundamental role in the management of painful processes, namely the Nursing Specialists in Child and Pediatric Health Nursing, who provide differentiated, advanced and safe care, which are promoters of the highest state of health and are based on evidence-based practice. The aim of this resport is to describ, analyze and discuss the knowledge obtain in the context of internship, which led to the development and acquisition of common and specific competences of specialist and master nurse.

The Child Health Care(CHC) havean important role in thepreventionof childhoodobesity and in the promotion ofchildren’s growth and development. The aimof this study was to illustrate thehealth promotion and preventativework of pediatricnurse’s against overweight and obesity in the CHC. Method: An interview study was conductedbased on a qualitative content analysis. The data collection was based on semi structured interviews with seven pediatric nursesworking at CHC:s.A qualitative content analysiswas used in the analysis process. Results: The result is dividedintofour categories based on the analysis: Assessment and follow-up of overweight and obesity, The good choices,Factors that concern the pediatric nurse’s work and The individual Child Care center.The pediatric nurse’shealth promotionand preventative work against overweight and obesity in children in the Child Health Care is based on promoting the good choices and creating a trustful relationship with the family. Conclusion: Hopefully thisstudy canhighlightthe pediatric nurse’shealth promotiveand preventative work against overweight and obesity in children.

Full textO presente documento, elaborado no âmbito do curso de Mestrado em Enfermagem com especialização em Enfermagem de Saúde Infantil e Pediátrica, tem como objetivo geral descrever e refletir acerca do percurso de aprendizagem realizado, que nos permitiu o desenvolvimento do projeto de intervenção “Continuidade de Cuidados em Pediatria: SER criança com doença crónica no domicílio- Uma Resposta Pediátrica”. A doença crónica tem um enorme impacto na criança/família, tendo que ser assumida como um enorme desafio e responsabilidade para as instituições de saúde, que deverão responder cabalmente às suas necessidades. Neste âmbito, o Enfermeiro Especialista em Enfermagem de Saúde Infantil e Pediátrica (EEESIP) ganha um papel de destaque. Percorridos diferentes contextos de cuidados à criança/jovem e família, foram delineados objetivos e desenvolvidas atividades, visando a obtenção de competências de Mestre, bem como competências específicas do EEESIP, seguindo a linha orientadora do projeto, o qual foi desenvolvido sob a metodologia de trabalho de projeto. Das experiências vivenciadas, emergiu um corpo de conhecimentos que contribuiu para a construção do perfil de competências de enfermeiro especialista e de Mestre em Enfermagem. Consideramos, assim, que atingimos os objetivos propostos.
The present document, elaborated in the scope of the Master's degree course in Nursing with specialization in Child and Pediatric Health Nursing, has a general objective to describe and reflect about the learning pathway that has enabled us to develop the intervention project "Continuity of Care in Pediatrics: Being a child with chronic illness at home - the Case Manager ". Chronic illness has a huge impact on the child / family, and must be assumed as a huge challenge and responsibility for health institutions, that must respond to their needs. In this context, nurses specializing in children's and pediatric health (EESIP) gain a prominent role. Through different contexts of child / youth care and family, objectives were defined and activities were developed, aiming at obtaining masters' competences, and also specific competences of EESIP, following the guideline of the project, that was developed under the methodology of the project work. From these experiences, emerged a body of knowledge that contributed to the construction of the skills profile of a specialist and Master nurse. We consider that we have achieved the proposed objectives.

Full textThis document is based on a placement of Pediatrics’ health care that took place between 19/04/2010 and 29/01/2011, in different caring contexts for child and family. This becomes with the purpose of showing the acquisition, development and integration of the several skills that will lead to a master’s degree in Nursing, in the main area of infant and child health care. Thus, as knowledge instrument, its primary purpose is to provide, in a global and coherent way, a synthetic and objective description of the work/activities undertaken in the different contexts of the placement. In order to achieve the aim and specific objectives of the placement, scientific, technical, cultural and human skills were developed as Nurse Specialist in Pediatric and Child Health in the areas of specialized care, care management and services, training and research. The placement took place at the Health Center of São Martinho do Bispo in Coimbra (Module I - Child Health) and at Santo André Hospital in Leiria (Module II- pediatric service and Module III - Pediatric Emergency and Unit of Pediatric and Neonatal Special Care. The methodology used was descriptive and reflexive. The critical and reflexive component is present throughout this document and it intends to convey the intent of the activities developed and performed. The meaning of the lived experiences and the enriching contribution has provided advanced and specialized nursing care, in a holistic perspective, to the child, adolescent, family and community. This work presents the acquisition, development and integration of skills/knowledge for advanced nursing care to newborns, children and adolescents, especially aimed at promoting the development and adaptation to health and disease processes. The focus of privileged nursing chosen was the safety promotion and accident prevention in children, transversal to the entire placement, for reasons connected with their social and professional relevance. Social, because childhood accidents, represent an important public health problem, despite of the development of the scientific-technological knowledge and the advances in medicine. Safety culture as it always existed. However, it is necessary to maintain this culture. It seems clear that to grow in safety it requires the development and consolidation of a social culture of security. Professional, because daily as a nurse practicing in a pediatric service, a significant number of children victims of accidents is faced. This is an emerging focus and highly sensitive to the intervention of a Specialist Nurse in Pediatric and Child Health, as a professional with knowledge, skills and abilities to a real understanding of the children´s growth and development, in their familiar and social environment. Children represent the future and ensure their safe growth and healthy development must be a primary concern of society in general and of health professionals in particular. So, at this stage of the placement, a multimedia presentation of transversal character was elaborated, entitled “Criança + Segura! Sem segurança ?? Sinto muito…”

Full textThis report aims to describe my learning process during the nursing internship course that began with the development of an individual project that aimed to "Pain in Child / Family" and aimed to contribute to the development of skills in scope of advanced nursing in child care / family with pain, through the relief and minimization of pain, within the tasks assigned to the Nurse Specialist in Pediatrics and Child Health. The knowledge concerning pain in children has been receiving more recognition and it is accepted that children learn early on the meaning of pain, however, some beliefs persist, often influencing child care. A number of interventions performed by everyone who cares for the child such as parents or health professionals, contribute decisively to the reduction and pain relief over invasive procedures. The theoretical principles underlying the work presented, was Katharine Kolcaba, and her Theory of Comfort (1994), which focuses on healthcare needs such as physical comfort, environmental, psycho-spiritual and cultural needs. The choice of this theory derives from the fact that pain can be interpreted and explained by the taxonomic structure of comfort defended by Kolcaba. The needs of the child / family, were essential in the initial evaluation, allowing me to diagnose the situation. Thus, planning of activities was defined with a base grounded in the child’s individual experience of health. I will highlight the development of the following activities within the primary health care strategies for pain relief before the early diagnosis and vaccination in the neonatal implementation of the "Wrapped Bath" and in the context of the implementation of emergency pediatric pain assessment and of non-pharmacological strategies for pain relief in children / family. In this formative process, I developed technical, scientific, moral, ethical and relational skills that allow me to care as a specialized advanced nurse for the child and family, contributing to my growth and development, aiming to reduce the pain and anxiety of the child in situations of invasive procedures and thus achieve excellence of care, and this, is what is expected of a Nurse Specialist in Pediatrics and Child Health

Full textThis report was the result of a reflection of the activities and skills acquired throughout the Internship Course inserted in Nursing Master of Pediatrics and Child Health. The overall objective for the Internship Course was "to provide nursing care for the child/family according to the Theory of Comfort by Katharine Kolcaba within the functions attributed to EESIP . Comfort is bound to nursing since its inception. In the pediatric context in Portugal, nurses have been developing a more holistic view of the binomial child / family. It is essential to abandon the concept that comfort is only a measure of pain relief but something positive, dynamic and complex. Nursing as a discipline of knowledge and practice, should base their interventions on methodological procedures. Given the comfort and the resulting state of nursing interventions, the Theory of Comfort Katharine Kolcaba (1994) presents a defined taxonomic structure, which allows applying the concept. Over the three modules occurring in the context InteRNhip: Community (Education Center for Disabled Children and Care Unit Custom Alcantara) hospitalization of the child / family with chronic illness (in Unit Special Care and Nutrition, Hospital Dona Estefania) and child care / family emergency (ER Pediatric and Neonatal Intensive Care Unit, Hospital Dona Estefania), was possible to demonstrate the applicability of this theory. Following a methodology, where used Surveys for gathering the needs and comfort of the child / family, along with a diagnosis of the situation of each location, all for shaping strategies in order to reach comfort as defined by Kolcaba. These results led to foster changes that sought to establish health-promoting behaviors in children and their family life. The promotion of comfort applied to child / family is an unselfish attitude, practical and satisfactory for those who are comforted and comfort, resulting in gains in health and contributing to the excellence of nursing care.

They’re responsible for the act of caring, incumbent on them to accompany child/youngster and their families throughout health/disease process, providing humanized care, anchored in the respect for individuality of the child/youngster and families included, meeting their health needs. Integrated on the research line of “necessities in nursing care in specific populations”, this project has as its purpose to contribute, through the capacitation of the nursing team, for the improvement of humanized care towards the child/youngster and family, in a situation of special complexity with the support of the Project Methodology, having been implemented during the Final Internship. It is also presented in this report a reflective analysis regarding the trajectory accomplished, acquisition and development of capabilities of Master and Nurse Specialist in Pediatric and Infant Care Health nursing. The creation of this project, as allowed us to conclude that humanized care is a facilitating strategy for accepting the disease process and adhering of therapeutic regimen, promoting an effective therapeutic relationship between nurse-child/young-family.

Health Literacy has an important role in improving health status. However, it cannot be dissociated from general literacy, being this the premise that guided our training path, as well as our health intervention project – "Stories to Grow in Love and Health", which aimed to promote literacy and shared reading, as well to promote health literacy in a playful-pedagogical way, with family involvement. The project has been proven to be a leading factor for moments of shared reading, consequently promoting health literacy and increased gains in Child Health

Full textAntimicrobial resistance is a growing problem of public health, which contributes to the prevalence of infections associated with health care and which are more prevalent in the pediatric age, being infection control fundamental and imperative for the safety and quality of care. In the line of research on "Safety and Quality of Life", and through the adoption of the methodology of project work, interventions were made adapted to the needs and particularities of four distinct contexts, with the aim of contributing to the promotion of effective infection control in pediatrics. For effective infection control, it is concluded that it is essential to raise awareness among professionals about the problem and to encourage parental participation, through the establishment of a true partnership of care, in the fulfillment of the necessary measures. The interventions focused on solving specific context problems, which proved to be important and motivating for nurses. The entire training course is articulated throughout the report, as well as the reflection on its contribution to the acquisition of the master competences, common and specific of the specialist nurse in child and pediatric health nursing.

Full textThe present document presents as an element of evaluation of the Curricular Unit of Report of the Master Course in Nursing with Specialization in Nursing of Child and Pediatric Health of the Escola de Enfermagem-Porto, Universidade Católica Portuguesa. The Pediatric and Child Health Internship began on May 2, 2017 and ended on January 16, 2018. It has a total of 30 ECTS, comprising 750 hours of work, of which 540 are contact and 210 individual work. Structurally this Unit incorporates three Modules: Module I - Context of primary health care developed in Community Health Care Unit and Personalized Health Care Unit; Module II - Medicine and Surgery Services in the care of children with acute or chronic illness; Module III - Emergency and Neonatology Services. The elaboration of this report describes the process of acquisition of specialized competences taking care of the four domains of Nursing: Caring Care; Management; Training and Research. Based on the Nursing areas of intervention, the objectives for the Internship were outlined and the activities for its scope outlined, using a critical-reflexive analysis of the specialized skills learning course. This critical-reflexive analysis focuses on caring for the child and family as a target unit of care, and focuses attention areas related to Nursing assistance in Child and Pediatric Health, where the transition to parenting, the care, attachment and mother-child relationship, breastfeeding and control of pain in children. It reflects the development of learning by reconciling the theoretical knowledge of the Master Course with the contributions of my professional and personal experience. The Methodology used to elaborate this work was the descriptive methodology, based on the bibliographical research on the current scientific evidence.

Full textThis report is the culmination of a long journey of traineeship as part of the Master Course of Child Health and Pediatrics,of the Instituto de Ciências da Saúde held at the Universidade Católica Portuguesa. The training modules were carried out in different contexts, particularly in community resources, Primary Health Care, Pediatric Emergency Room and Neonatology. The transversal theme of the route was the Health Promotion in Children with Diabetes; however it has suffered adjustments according to the needs of the different contexts. The choice of theme resulted from a combination of factors, including personal interests and state's diagnostics performed. Over the years, the definition and problematic of health promotion has been discussed by several entities, verifying their relevance to health gains. On the other hand, it has been observed a significant increase in childhood diabetes worldwide, especially type 2. This increase in diabetes is associated with hereditary factors, the increasing urbanization, habits and unhealthy lifestyles. The EESIP has a key role to intervene in this issue of public health, developing strategies to promote health by preventing the illness from appearing or preventing the development of!associated complications. In this sense, objectives were outlined and activities were developed, for the promotion of child health in this context, namely health education sessions, information leaflets and pair formation. Thus contributing to a better management of the disease, a better control of glycosylated hemoglobin (HbA1c) and for the prevention of late complications of diabetes and therefore more health gains. The! internship! had as base the theoretical model of Nola Pender’s Health Promotion, complemented with the conceptual model of care focused on the child, who sees the dyad child/family as the center of the interventions. The partnership model of Casey’s cares, which stresses the importance of parents as partners in providing care to the child, and is one of the main strategies to achieve a maximum level of personal health status. The methodology used incorporated a constant thoughtful and critical analysis on the professional practice, the scientific evidence and the situations experienced throughout the internships, allowing, along path, to identify problems and make decisions, implementing targeted interventions with applicability in different contexts. This way, I conclude that the defined objectives and activities developed were relevant and achieved, contributing to the acquisition of specialized skills and to improve the quality of care provided to the child and family

Full textThis report aims to describe and analyze the personal and professional experience and the competences of the candidate to Specialist Nurse, following the completion of internships in children health and pediatric nursing. The internships were designed to develop the trainee’s general and specific competences in children/youth health nursing. Another aim was to promote and support the development of Family Centered Care (FCC) practices in healthcare facilities. The clinical practice – Module I “Child Health” was held at Centro de Educação para o Cidadão Deficiente (CECD, Mira Sintra) and at Direcção-Geral da Saúde. The clinical practice – Module III was held at a Pediatric Emmergency Department and at a Neonatal Care Unit from two hospitals in Lisbon. The description and analysis of the training experiences are structured according to the stages of health planning. The Report also identifies and discusses the specific and general competences of child health nursing developed by the trainee within the following domains: (1) professional/ethical and legal responsibility; (2) quality improvement; (3) management of care; (4) development of professional learning; (5) conception of nursing care for the child/young and family. In the domain of research, the Report describes two descriptive-correlational studies performed during the practice clinic. These studies aimed to: (1) identify the nurses’ perception about the practices of FCC; and (2) analize the factors that influence these perceptions. The results of the study suggest that nurses identify organizational and individual difficulties in the implementation of FCC. The Report is concluded with a discussion of the implications of the results of the studies for nursing research and practice, as well as for the development and implementation of FCC practices. The discussion also focuses on the complexity of current contexts of child/youth and family health care, and the resulting need to consider the development of competences of specialist nurses as a continuing learning process

Full textThe Internship Report presents the course of professional development undertaken during the Internship, consisting of two modules (I and III) in two different health care institutions: “Emergir”- Family Health Unit and Dona Estefânia Hospital. The reason of choosing – Promotion of Child Safety- as the action plan area was based on the daily reflections carried out under the professional exercise as well as the analysis of various health indicators. The accidents are a major cause of mortality, morbidity and temporary or permanent disability, resulting in personal, familiar, social and economic costs. Nowadays it is considered a serious public health problem. The main objective of the Internship was to promote awareness in the children/youth person and family, nurses and other health professionals about the importance of child safety and injuries prevention. In this context, the Nurse Specialist in Child and Youth Health Nursing plays an active role as a promoter and educator of health, changes in behavior and adoption of best practices by the child/youth person and family, improving the quality of life and influencing positively the health indicators of the country.

Full textThis report comes from the testimony of the trainee in the process of transition from a general to a specialized care for children. This process occurred not only due to the experiences offered by her career as a nurse, but also from learning acquired in the course of the Internship Master's degree in Nursing Nature Professional Specialisation in Child Health Nursing and Pediatrics, sponsored by the Institute of Health Sciences Portuguese Catholic University of Lisbon. The report presents not only theoretical and conceptual foundations, but also a report concerning the way the trainee assumed the central problematic of child maltreatment in different contexts of action, displayed in three modules. Thus, were developed by the intervener, actions with the assumed main goal: "Developing skills in specialist nursing care for children at risk and / or danger" The first module, took place at Casa da Encosta in Cascais and at a Community Care Unit in Damaia, and stood the preparation of the "Script Supervisor of Domiciliary Visit" and planning the pioneer project "Adolescent Mothers" as interventions at the primary and secondary level in child maltreatment prevention. In Pediatric Service of Hospital Professor Doutor Fernando Fonseca E.P.E., second module, was structured a “Risk Sheet ", as a contribution to secondary and tertiary level prevention, in situations of admitted minors at risk or danger of abuse. And in the Intensive and Special Care Neonatal Unit of the same hospital, third module, was conducted a secondary prevention level through the dissemination and public awareness of the International Day of Prematurity and elaboration of the "Manual of Support and Community Resources". Therefore, the author tried to contribute to early intervention in family by promoting protective factors for a healthy child development, through risk and child vulnerability limitation of violence. The reader may follow the route of the trainee in search of the quality in caring for the child and family in contexts of risk and/or danger. You can also discover the reality of activities planned, developed and implemented during the process of specialization.
