Abstract:
New tasks are being developed for pharmacy technicians in community practice. The objectives of this study were to (1) assess the willingness of community pharmacy technicians to perform new tasks, and (2) to identify factors affecting technicians in assuming new tasks in community pharmacy practice. An online survey asked about the respondent characteristics, involvement in pharmacy technician tasks, willingness to perform emerging pharmacy technician tasks, and influences on pharmacy technicians’ performance of emerging tasks. Descriptive statistics were calculated for all items. A total of 639 usable surveys from community pharmacy technicians were used in the analyses. The respondents reported a mean of 11.5 years working as a pharmacy technician, with 79.2% working full time. Technicians reported high willingness to perform four emerging tasks, moderate willingness for six tasks, and low willingness to perform two tasks. The low willingness tasks were administering a vaccination and drawing a blood sample with a finger stick. Four workplace influences on willingness to perform emerging tasks were insufficient staffing, insufficient time to complete additional tasks, employers not classifying technicians based on specialized skills, and usually feeling stress at work. It appears likely that pharmacy technicians will be willing to perform the new tasks needed to support the emerging patient care services in community pharmacies.

Abstract:
While pharmacy technician roles in some practice settings are expanding beyond the traditional dispensing activities to include advanced or specialized tasks such as immunization administration, medication history collection, and final product verification, these practices are not yet widespread. There are apparent barriers to expanding the role of pharmacy technicians, including inconsistency in the education, training, and certification requirements across the United States, and regulations that have not kept pace with the evolving role of pharmacy technicians. Every corner of the profession has an opportunity, and responsibility, to elevate pharmacy technicians in an effort to advance safety and better serve patients. Regulators can expand the responsibilities that may be delegated to technicians, professional organizations can bring pharmacy technicians into the fold, employers can build career ladders to allow for advancement, and individual pharmacists and pharmacy technicians can advocate and engage.

Computer Control of a 3 MV Van de Graaff AcceleratorThe development of accurate computer control of a 3 MV Van de Graaff accelerator operation is described. The developed system comprises the accelerator turn-on and turn-off procedures during a normal run, which includes the setting of the terminal voltage, ion source light up, beam focusing and control of ion beam current and energy during operation. In addition, the computer monitors the vacuum and is able to make a detail register of the most important events during a normal run. The computer control system uses a LabVIEW application for interaction with the operator and an I/O board that interfaces the computer and the accelerator system. For everyday operating conditions the control implemented is able to turn-on and off the machine in about the same time as a specialized technician. In addition, today more users can make experiments in the accelerator without the help of a specialized operator, which in turns increases the number of hours during which the accelerator can be used.

Abstract:
The introduction of digital imaging and diagnostics facilities has fundamentally changed radiology. Nevertheless, theory of digital image processing and analysis as well as their practical application are still only a subsidiary part in nowadays radiology technician curricula. This work focuses on the evaluation, to what extent the authors' simplified and standardized process model for applying image processing modules in generic domains is suited for radiographer students and medical staff, lacking deeper theoretical knowledge compared to physicians and imaging experts. The semi-automated image processing workflow thereby comprises region growing, live-wire segmentation and filtering steps, all available from MeVisLab prototyping framework. It is shown that the proposed imaging chain is highly applicable for analysis and facilitating medical diagnostics of arbitrary anatomical structures from tomographic data. After compact practical instruction, radiographer students are versed to achieve complex 3D analysis perfectly suited for quantitative analysis in clinical research typically only achievable by use of specialized software.

Abstract:
To ensure high performance of the agricultural machinery and equipment it is necessary to improve the system of technical service in agriculture through the creation of specialized service centers. Features of agricultural production are such that machinery and equipment implementing its processes are distributed throughout the agricultural enterprises, and mobile machines and units that form the basis of machine-tractor fleet, change their place of deployment as work progresses in certain areas. This situation complicates the maintenance (M) and current repairs (CR) of agricultural machinery and equipment, as their delivery of their service centers or check out of service technician with the necessary tools, equipment and materials directly to the operation of machines and equipment, or temporary storage sites are required. The result is more expensive M and CR and increased losses from downtime, of course these additional costs and losses must be minimized by the rational organization of processes of M and CR. Analytical dependences are got to determine the costs of the M or CR, taking into accounts the embodiment of losses from downtime of serviced machines and the method of its delivery to the service center. Selecting embodiment of M or CR is carried out on a minimum of these costs, for which the algorithm is proposed, comprising determining the costs of the options under consideration to perform M or TP and compare them to avoid irrational choices. These analytical dependence and calculation algorithm contribute to the improvement of the organization of technical services in agriculture.

Abstract:
Sleep related disorders can severely disturb the quality of sleep. Among these disorders, obstructive sleep apnea (OSA) is highly prevalent and commonly undiagnosed. Polysomnography is considered to be the gold standard exam for OSA diagnosis. Even though this multi-parametric test provides highly accurate results, it is time consuming, labor-intensive, and expensive. A non-invasive and easy to self-assemble home monitoring device was developed to address these issues. The device can perform the OSA diagnosis at the patient’s home and a specialized technician is not required to supervise the process. An automatic scoring algorithm was developed to examine the blood oxygen saturation signal for a minute-by-minute OSA assessment. It was performed by analyzing statistical and frequency-based features that were fed to a classifier. Afterward, the ratio of the number of minutes classified as OSA to the time in bed in minutes was compared with a threshold for the global (subject-based) OSA diagnosis. The average accuracy, sensitivity, specificity, and area under the receiver operating characteristic curve for the minute-by-minute assessment were, respectively, 88%, 80%, 91%, and 0.86. The subject-based accuracy was 95%. The performance is in the same range as the best state of the art methods for the models based only on the blood oxygen saturation analysis. Therefore, the developed model has the potential to be employed in clinical analysis.

Abstract:
In recent years the profession of teaching has reached a maturity to the point of being considered a producer of own knowledge necessary to the practice. The teacher is no longer seen as a technician, but as an intellectual actor and the specialized literature advocates teaching as a profession, and it recognizes that the teacher has “knowledge base”, a set of skills that are developed during his teaching activity. The Pedagogical Content Knowledge (PCK) is a concept that seeks to represent the teachers’ professional knowledge and it has been widely used in the literature about teachers' knowledge. It has proved a fruitful model for investigations aimed to document the knowledge that makes one a good teacher. Despite the relevance that the PCK is, the consensus on what is the PCK is still far from being achieved and many researches propose different models and concepts for the PCK, sometimes conflicting. Many works brings the PCK and speak of it as if it were a clear concept and do not clarify which model / conception are using, which makes investigations on the PCK difficult and ends up attracting a lot of criticism. Thus, this study aims to look critically at the various models proposed in the literature most used and point differences and similarities so that an overview can be built with more insight and analyze their use and validity. The study will also present some ways to have access to PCK and the relation between PCK and teacher education is also discussed. Key words: knowledge base of teaching; pedagogical content knowledge; professional knowledge; teachers’ education; teachers’ knowledge.

Abstract The necessity of new approaches for structural synthesis of mechanical systems, which directly identify a limited number of structures that carry a potential for solving the technical problems and meet the specific requirements for the design of mechanical systems, mainly for specialised robots, is specified. The identified five types of kinematic chains (primary, parallel, secondary, additional and subsidiary), with different functionality, allow the synthesis of manipulators structures according to the defined goal tasks and specific functional requirements for hybrid systems, in particular specialised industrial robots. Two mechatronic approaches for functional structural synthesis of mechanical systems of industrial robots, where the main manipulation mechanism is a path generator, were introduced. A limited number of structures that meets the set of objectives and technical requirements to design mechanisms, is directly determined. Emphasis is placed on the tasks of passive (mechanical) control of manipulation systems associated with specialized robotics. Abstract:
Abstract INTRODUCTION While hematopoietic stem cell transplantation has become the standard of care for many patients in developed countries, the significant economic investment, infrastructure, and healthcare provider training that are required has prohibited it from being widely adapted. We describe our experience in developing a transplant program at Dhaka Medical College and Hospital (DMCH) in Bangladesh via a partnership with health care providers at the Massachusetts General Hospital (MGH). In 2011, the Minister of Health in Bangladesh became interested in establishing a transplant program at Dhaka Medical College and Hospital (DMCH). DMCH was established in 1946 and has 2300 beds located in two buildings (DMCH-1 with 1800 beds and DMCH-2 with 500 beds). The Bangladesh government and AK Khan Healthcare trust, an interested non-governmental organization (NGO), provided funding for the endeavor. Despite available funding, we faced significant challenges due to limitations in experienced healthcare personnel. METHODS The DMCH transplantation unit was built on the top floor of DMCH-2 and includes 5 patient rooms, an apheresis area, the hematopathology and general hematology labs, and also includes rooms for leukemia patients who are not currently undergoing transplantation. Three hematologists, two apheresis technicians, one lab technician, and 1 pharmacist from DMCH participated in an exchange program during which they each spent 2-3 months at MGH observing transplantation practices. Additionally, several physicians from MGH traveled to Bangladesh and spent several weeks training DMCH physicians in diagnostics, peripheral blood stem cell collection, transfusion medicine, and management of potential complications. The training of Bangladeshi nurses was an extensive undertaking. In brief, 25 nurses from DMCH were selected to take part in a novel enhanced specialized nurses training program (ESPNTP) run by nurses from MGH, which is described in detail in a separate report. This intensive 12-month program included 3 months of training in English language, 3 months of fundamental nursing training, and 6 months of specialized training in HSCT. Nurses continue to train on an ongoing basis via an advanced curriculum taught by MGH nursing staff. RESULTS The first transplant was performed on March 10, 2014. Since that time, DMCH has performed 14 autologous stem cell transplants (9 multiple myeloma, 2 diffuse large B-cell lymphoma, acute myelogenous leukemia, Hodgkin's lymphoma, and peripheral T-cell lymphoma). Baseline characteristics, transplant protocol, and summary of results are shown in (Table 1). All patients engrafted successfully (range 9-16 days). Remission was achieved in all cases, and so far there has only been one case of relapsed disease in a patient with multiple myeloma. Significant complications include mucositis (grade II-IV: 12), febrile neutropenia (8), bacteremia (S. epi: 2; P. aeurginosa: 2, MRSA/K. pneumonia: 1), one case of acinetobacter pneumonia, and one case of cytomegalovirus reactivation. DISCUSSION We attribute a large part of the program's ability to successfully perform its first transplants within 3 years of conception due not only to available financial support but also logistical and personnel support from collaboration with an established transplant center. A major goal of the current program is to ultimately establish a transplant center capable of performing allogeneic stem cell transplantation, as this would enable treatment of patients with thalassemia, the most common genetic disorder in the country that carries a high burden on the economy. We hope that our experience will encourage more partnership and collaboration between transplant programs in developed countries and those countries who currently lack expertise but could benefit from having this therapeutic modality available. Abstract:
Introduction. Overweight and obesity are leading risk factors for metabolic syndrome (MS). From 20 to 35% of Russian people have this condition, depending on their age. MS is a precursor of cardiovascular disease, diabetes mellitus, diabetic nephropathy, and nonalcoholic steatohepatitis. Specialized foods (SFs) with hypocholesteremic effects are an important component of the diet therapy for MS patients. Creating local SFs to optimize the nutritional status of MS patients and prevent related diseases is a highly promising area of research. The aim of our study was to develop the formulation and technology of SFs and evaluate their effectiveness in MS treatment. Study objects and methods. The objects of the study were food ingredients and SFs. Safety indicators and micronutrient contents were determined by standard methods, whereas nutritional and energy values and amino acid contents were determined by calculation. Results and discussion. Based on medical requirements, we selected functional ingredients and developed a formulation and technology of SFs with an optimized protein, fat, and carbohydrate composition. The formulation included essential micronutrients and biologically active substances with a desirable physiological effect. Clinical trials involved 15 MS patients aged from 27 to 59. For two weeks, they had a low-calorie standard diet with one serving of SFs in the form of a drink instead of a second breakfast. The patients showed a significant improvement in anthropometric indicators. Blood serum tests revealed decreased contents of total cholesterol (by 16.9%), low-density lipoprotein cholesterol (by 15.3%), and triglycerides (by 27.9%). Conclusion. We developed technical specifications and produced a pilot batch of SFs. The trials showed an improvement of lipid metabolism in the MS patients who were taking SFs as part of their diet therapy.

Abstract:
Abstract Introduction Patients with sleep apnea are often prescribed positive airway pressure (PAP) treatment. Some patients have difficulty consistently using a PAP machine due to problems such as air leak, mask discomfort, and dry mouth. The purpose of this project is to evaluate the satisfaction and efficacy of a PAP Troubleshooting Clinic lead by a RN and a respiratory technician (RT). This clinic seeks to improve PAP compliance, increase PAP tolerability and increase PAP treatment efficacy Methods The PAP Troubleshooting Clinic consists of a RN and RT with specialized knowledge about sleep apnea and PAP treatment. During a 30 or 60-minute clinic visit, these providers review the patient’s experience with PAP, assess mask fit, review PAP data and PAP settings, and recommend treatment adjustments for the primary Sleep Clinic provider’s consideration. Outcomes assessment includes satisfaction (a telephone survey one week after the visit) and efficacy (30-day data on overall use, mask leak, and days with ≥ 4 hours of use downloaded from PAP machines). Results To date, 58 patients have received care in the clinic and 56 (96.5%) patients reported they were satisfied/highly satisfied with the PAP Troubleshooting Clinic. From the sample, forty-three (74.1%) patients were compliant with mask use over 30 days. Twenty-four (55.81%) showed a greater than 10% reduction in mask leak after intervention. Ten patients (23.3%) achieved an improvement with a 10% increase in number of days with > than 4 hours of PAP use. Conclusion Patients and Sleep Clinic providers are very satisfied with the PAP Troubleshooting Clinic. Patients are referred to this clinic because they have significant issues with PAP usage and are at high risk of discontinuing use. Patients find the clinic helpful and encouraging, while sleep medicine physicians and APPs appreciate the assistance in helping patients succeed with PAP, especially during the 30-day time period where PAP compliance receives scrutiny from clinics and payers. Support A PAP Troubleshooting Clinic is an effective way to improve patient PAP Use, Mask Leak, Prolonged Use and Patient Satisfaction. Importantly, this new clinical model offers a valuable alternative to provide patients with the appropriate level of care.

Abstract:
Problem statement. Analysis of traditional methods for predicting the states of complex organizational and technical systems for power structures has shown that these methods are not always acceptable for solving decision support problems.Objective. Determining the causes of forecasting failures, developing a new approach to the design of decision support systems for organizational and technical systems of a particular purpose.Results. The system analysis of the class of specialized organizational and technical systems showed that the systems under consideration are non-linear. Traditional forecasting methods do not take into account the curvilinearity of systems. It is proposed to use simulation modeling, one of the staging methods, to simulate the change of system and environment states. Programs that implement simulation modeling include system software layouts and simulation programs. Simulation mockups, layouts, and simulation programs are defined in more detail. The concept of closure software is introduced. It is shown that the simulation modeling programs provide closure. The scheme of decisionmaking in competition, which is implemented by simulation modeling programs, is given.Practical implications. Examples of the use of simulation modeling in system development are given. It is shown that the used approach of simulation modeling application not only provides the software closure but also reduces the cost of creating a simulation model of the system operation and allows creating it in the process of software system development.

The rapid burst of electronic databases in the recent decades has led to a boom in specialised terminology meant to describe the structure and way of functioning of these databases. Some of this terminology was borrowed from “common language” vocabulary but used in a specialized technical language. This decision to use existing terms in a new sense, rather than coining new terms for the new concepts, was a questionable one as in some cases this terminology may be misleading, at least in certain contexts. This is the case, in our opinion, with the use of terms expressing genealogical relationships to express hierarchical taxonomic relationships in databases.

Abstract The article offers a practical approach to the skills a present-day translator needs in order to create high-quality translations. Although a lot of theories can be found regarding the skills of a translator, it is worth checking the reality, which is the primary aim of this article. After a short introduction about the standard skills, we look into the subtitling of an episode from a TV series. Our presupposition is that a subtitler has to combine all sorts of information from different fields effectively in order to maintain quality, including general and specific knowledge of the subject matter. Furthermore, the particular environment of subtitling may contain certain pitfalls, such as the technical know-how. layout, and constraints deriving from the nature of subtitling. We can draw the conclusion that a well-prepared translator can successfully handle the technical challenges of multimedia translation of whatsoever type.

Abstract:
A teacher mastering a new paradigm needs professional support. In this case, it is recommended to create collective pedagogical action, in which all reflexive functions are performed by a teacher together with colleagues and educational technicians specialized in activity-based educational methods. The specific task of educational technicians is to follow semantic guidelines and normative frameworks of the above-mentioned pedagogical action. The reflexive position of educational technicians in relation to teaching means is that they should rely on activity-based learning and the corresponding principles of organizing and managing educational processes while supporting teachers. Thus, it is necessary to state that activity-based learning with its foundations, objectives, and methods has not acquired a comprehensive form for its popularization among school teachers. Currently, this paradigm is represented by a set of theoretical constructs from classical Russian psychology and a description of different educational experiments. The prerequisites for the training of educational technicians are associated with the application of the activity-based approach, which is the most developed in general education, for the training of educational technicians specialized in activity-based learning at institutions of higher education, including through the organization of quasi-professional activities. The study aims at considering various aspects of training educational technicians at institutions of higher education, including the specifics of the interim assessment. The authors of the article examine the corresponding literature on activity-based formats of professional training, as well as methods of forming educational events and outlining their content. The results obtained in the course of the study offer possible solutions to creating favorable conditions for the experimental and productive actions of future educational technicians. The novelty of the study is determined by a new approach to the training of educational technicians, which includes educational games having both diagnostic and reflexive potential.

Abstract:
The issue of terminographic gaps in specialized discourses has always concerned the researchers and readers alike. However, due to the interlingual nature of such a technical issue, the need for interdisciplinary collaboration between translation and terminography seems to be in prospect. For such a reciprocation scheme to come into practical effect, the present study has aimed to conduct a translational-terminographic concerto by putting a specialized English text to the test of Persian translation. This has been done to answer the question if a translator is required to provide for any terminological gap once all attempts at finding the corresponding terminological items have failed. In this pursuit, certain workable criteria for terminographic proposition via translation have been discussed. As such, the practical phase of this study concerns itself with addressing the issue of Persian terminological gaps in a language-related metadiscoursal field and consequently detecting the problem zones of non-equivalence in a specialised text carefully selected for translation. Ultimately, a list of Persian terminological items constructed on the basis of the proposed translational-cum-terminographical scheme is compiled to address the identified terminological gaps in the target metadiscourse under study.

Abstract:
The present article features the organizational and methodological aspects of developing profession-oriented didactic resources in English, i.e. textbooks or teaching manuals, for students of technical specialties in order to improve their work-related competencies. The authors see the interaction between the department of foreign languages and the technical departments as an organizational and methodological dominant. They outline the development principles, content, types of exercises, and various activities. The new generation profession-oriented textbooks, created by the joint efforts of linguists and technicians, contain elements of a computer-based learning environment in its lingua-didactic apparatus. This contributes to a more efficient mastering of the content. Such textbooks follow the methodology of Content and Language Integrated Learning rather than that of English for Specific Purposes. The new manuals are highly specialized, which encourages students to pose questions. In addition, they contain creative cognitive tasks that improve special knowledge in the technical sphere.

Abstract:
Currently, there are two main options for training agricultural specialists in the Russian Federation. The first option is studying in a college (technical school) or mastering university programs through bachelor's, specialist and master's programs. The second way is to form a stepwise organisation of continuous agricultural education: a specialised general education school, a college (technical school), a university, and an institution of additional professional education. Insufficient attention to the change and development of new social statuses by students makes it difficult to fully disclose the educational potential of the system of continuous agricultural education. Thus, it is necessary to organize pedagogical support for students' adaptation to current changing academic situations. It is also essential to consider lifelong agricultural education not only as advanced training in the workplace, but also as a transition from specialized training in a secondary general education school to secondary agricultural vocational education, and then to the development of programs of higher agricultural education and subsequent regular, professional development throughout the entire period professional activity.

When selecting world scientific serials for certain disciplines in the natural and technical sciences, the indicators of selected publications citation in specialized journals and the specialized journals citation in the selected publications are used. The study was conducted within the topics “Energy security and energy conservation, energy efficient technologies and engineering». Total citation in individual specialized journals was calculated along with the “discipline impact factor” - an indicator similar to the impact factor having in its numerator the value of citation of journals in the selected specialized journals rather than the whole range of journals being indexed by JournalCitationReports®. The serials were also selected by their total citation of the selected specialized journals. The indicator similar to the “discipline impact factor” is calculated as the fraction of the number of the serials’ links to the specialized journals publications in “the citation window” made by the serials being assessed on the specialized journals publication within the “publication window” and the citing publications in “the citation window”. In all the cases, the citation window equals one year, and the publication window equals “5 + 1” years, i.e. five years prior to the year when the links are being recorded.

Abstract:
The article analyses characteristic features of the formation of system of secondary specialized library institutions in Ukraine in the 1920s — 1950s, as well as various professional courses and secondary technical schools that trained library specialists. The purpose of this article is to reveal the distinctive features, the main factors and stages of formation and development of secondary specialized library education in the Ukrainian SSR in the 1920s — 1950s, to characterize the varieties, features of creation and development of technical schools, which trained library specialists of mid-level management, especially library technical schools. The author used both general scientific research methods (description, comparison, analogy, deduction, induction, analysis, etc.) and historical (historical-comparative, historical-typological, historical-diachronic, chronological) methods. The author proves that in the second half of the 1930s, the country developed the system of secondary specialized library education, consisting both of training courses, retraining of specialists and also of a number of special educational institutions — various types of technical schools and other institutions where librarians were trained. In particular, there was formed the network of special library technical schools. Based on the analysis of historical, socio-political, cultural and other social factors, the author proposes periodization of formation and development of secondary special educational institutions, which trained library workers. Training of specialists in library technical schools and other secondary institutions became the basis for the formation of human resource of librarians of mid-level qualification for the different types of libraries (mainly for mass). The results and conclusions of this work can serve as a source material for further research on the history of secondary special library education; the material can be used in the special course lectures on the history of library education.

Abstract:
The aim of the article is to evaluate production efficiency and its determinants of specialised dairy farming among the EU regions. In the most of European regions, there is a relatively high significance of small specialised farms including dairy farms. The DEAVRS method (data envelopment analysis with variable returns to scale) reveals efficient and inefficient regions including the scale efficiency. In the next step, the two-samplet-test determines differences of economic and structural indicators between efficient and inefficient regions. The research reveals that substitution of labour by capital/contract work explains the variability of the farm net value added per AWU (annual work unit) income indicator by more than 30%. The significant economic determinants of production efficiency in specialised dairy farming are farm size, herd size, crop output per hectare, productivity of energy, and capital (atα=0.01). Specialised dairy farms in efficient regions have significantly higher farm net value added per AWU than inefficient regions. Agricultural enterprises in inefficient regions have a more extensive structure and produce more noncommodity output (public goods). Specialised dairy farms in efficient regions have a slightly higher milk yield, specific livestock costs of feed, bedding, and veterinary services per livestock unit.

Abstract:
Background and Aim: The objective of this study was to describe the manual fabrication of a transparent facial pressure mask for treating facial deformities. The mask combines the use of a silicone inner liner and mechanical pressure in the facial region. Technique: A negative mold is formed by covering the face with plaster. Manipulation of soft tissue is a crucial part in this process. After hardening and removal of the negative mold, the positive mold is formed and dried. Next a rolled silicone sheet is placed over the positive mold in a vacuum environment. Subsequently, the silicones are vulcanized. Then the rigid outside of the mask is created. The silicone inner liner and outside shell are then affixed. Discussion: This described technique results in accurate facemasks with precise fitting. During therapy, the mask is adjusted multiple times to keep excellent fit, as remodeling of scars and deformities takes place. Clinical relevance: Facemasks are a reputable therapeutic modality to reduce excessive facial scarring. They require excellent fitting to give pleasing results. To provide a better understanding of facemask therapy, this study describes the manual fabrication technique.
