Abstract:
(1) Background: The prevalence of allergic respiratory diseases is still rising and efforts towards holistic treatments should be made. Although speleotherapy is widely applied in Europe to treat chronic airway diseases, the existing scientific evidence is rather low. Recreational winter exercise has been shown to improve allergic airway inflammation, but little is known about the combined effects of speleotherapy and recreational winter exercise. (2) Methods: In this clinical study we investigated the effects of winter exercise and speleotherapy on adults with allergic rhinitis and/or asthma. The speleotherapy group (n = 23) participated in a ten-day combined winter exercise and speleotherapy program and the exercise group (n = 18) joined a full-day winter sports program. The effects on allergic airway inflammation, quality of life, spirometry and cardiorespiratory fitness were assessed. (3) Results: No significant effects were found for fractional exhaled nitric oxide or nasal nitric oxide. Quality of life (p < 0.001 time effect) and allergic symptoms (p < 0.001 time effect) were improved in the speleotherapy and in the exercise group. (4) Conclusions: Winter exercise alone and winter exercise in combination with speleotherapy improve quality of life and allergic symptoms in adults with allergic rhinitis and/or asthma. Further studies are required to investigate the specific effects of speleotherapy. To our knowledge, this is the first investigation examining speleotherapy in combination with winter exercise. Recreational outdoor winter exercise and speleotherapy may be recommended for highly functioning patients with good disease control.

A total of 152 aerosol and spider web samples were collected: 96 spider’s webs in karst areas in 4 European countries (Czech Republic, France, Italy, and Slovakia), specifically from the surface environment (n = 44), photic zones of caves (n = 26), and inside (aphotic zones) of caves (n = 26), 56 Particulate Matter (PM) samples from the Sloupsko-Sosuvsky Cave System (speleotherapy facility; n = 21) and from aerosol collected from the nearby city of Brno (n = 35) in the Czech Republic. Nontuberculous mycobacteria (NTM) were isolated from 13 (13.5%) spider’s webs: 5 isolates of saprophytic NTM (Mycobacterium gordonae, M. kumamotonense, M. terrae, and M. terrae complex) and 6 isolates of potentially pathogenic NTM (M. avium ssp. hominissuis, M. fortuitum, M. intracellulare, M. peregrinum and M. triplex). NTM were not isolated from PM collected from cave with the speleotherapy facility although mycobacterial DNA was detected in 8 (14.3%) samples. Temperature (8.2 °C, range 8.0–8.4 °C) and relative humidity (94.7%, range 93.6–96.6%) of air in this cave were relatively constant. The average PM2.5 and PM10 mass concentration was 5.49 µg m−3 and 11.1 µg m−3. Analysed anions (i.e., F−, Cl−, NO2−, SO42−, PO43− and NO3−) originating largely from the burning of wood and coal for residential heating in nearby villages in the surrounding area. The air in the caves with speleotherapy facilities should be monitored with respect to NTM, PM and anions to ensure a safe environment.

As a result efficacy of the methods was evaluated and differentiated indications for their administration were developed with regards to characteristic features of the disease and co morbid status.

Abstract:
The revealed functional disorders in patients who have suffered pneumonia associated with the new coronavirus infection COVID-19 require long-term rehabilitation, in particular in outpatient environment. Aim. To evaluate the effectiveness of outpatient rehabilitation of patients who have suffered pneumonia associated with the new coronavirus infection COVID-19. Material and methods. The study included 72 patients after mild to moderate pneumonia associated with the new coronavirus infection COVID-19. The main group included 37 patients who underwent comprehensive rehabilitation, including physical therapy, massage, magnetic therapy, speleotherapy, cognitive behavioral therapy, and drug therapy. The comparison group (35 patients) received physical therapy, magnetic therapy and drug therapy. The effectiveness of therapy was assessed on the basis of standardized questionnaires and scales: Asthenia scale (MFI-20), Anxiety and Depression scale (HADS), mMRC and Borg dyspnea scales, clinical and laboratory examination data. The quality of life of patients in the dynamics of rehabilitation was assessed by the quality of life questionnaire (EQ-5D). Results and discussion. In the main group of patients, the effectiveness of rehabilitation was higher relative to the decrease in the severity of dyspnea, asthenia, normalization of lymphocyte levels. In this group, the quality of life improved more significantly on the scales of anxiety/depression and psychoemotional status. Conclusion. The rehabilitation program, which additionally included chest massage, speleotherapy, transcranial electrical stimulation (TES), showed an advantage in improving clinical and laboratory parameters and quality of life in patients who have suffered pneumonia associated with the new coronavirus infection COVID-19.

Objective: Our aim was to determine the influence of pulmonary rehabilitation conducted in therapeutic salt mine chambers on the functional fitness of older adults. Methods: The study included 22 individuals of age >65 years with chronic respiratory conditions. The patients underwent the Fullerton test before and after a 3-week outpatient pulmonary rehabilitation in the “Wieliczka” Salt Mine Health Resort. Results: After the rehabilitation stay, the results showed statistically significant improvements within five of the six parameters evaluated. In the Arm Curl, the mean number of repetitions within 30 s increased from 14.55 ± 3.63 to 16.68 ± 3.83 and in the Chair Stand from 11.86 ± 2.55 to 14.41 ± 2.95. Beneficial changes were observed in the Back Scratch, but without statistical significance. In Sit and Reach results increased from -2.3 ± 11.11cm to 2.14 ± 9.19 cm. Time for performing the 8-Foot Up and Go decreased from 6.63 ± 1.27 s to 5.8 ± 0.86 s and in 2-Minute Step results increased from 88.27 ± 20.64 to 96.55 ± 16.38 repetitions. Conclusion: Functional fitness of examined older adults with pulmonary disorders has increased after a rehabilitation and treatment stay in underground salt mine chambers. Chronic obstructive pulmonary disease (COPD) is a progressive disease characterized by dyspnea and chronic cough. The main risk factor is cigarette smoking, but there are other ones implicated in the COPD etiology such as air pollution, childhood asthma, aging, chemical exposure, dietary factors, and genetic predisposition. Besides, COPD is associated with several comorbidities that influence prognostic and management, like asthma, lung cancer, obstructive sleep apnea, cardiovascular disease, metabolic syndrome, and depression or anxiety. The management is multidisciplinary and its role is to ease symptoms, prevent complications, slow disease progression, and improve the quality of life. In the last years, many alternative techniques have been implemented such as speleotherapy, halotherapy, muscular training, neuromuscular electrostimulation, acupuncture, thermotherapy, and music therapy. Abstract:
The childhood incidence of bronchial asthma is the most common chronic pathology in children. Due to the influence of many factors,as well as due to the widespread prevalence, the problem is becoming particularly relevant today. In recent years, a frequently usedconcept of medical rehabilitation is the so-called “multidisciplinary approach”, which also consists in the implementation of a personalizedapproach. Aim. To develop a comprehensive targeted program for the medical rehabilitation of children with bronchial asthma, to study the effectivenessof a comprehensive targeted program selected taking into account the individual characteristics of the child. Material and methods. There was developed an individual targeted program of medical rehabilitation was developed, in accordancewith the procedures and standards of medical care, taking into account indications and contraindications, taking into account a multidisciplinaryapproach, and rehabilitation methods, namely, such as: climatotherapy, physiotherapy, speleotherapy, aromatherapy,inhalation therapy, oxygen therapy, hypoxytherapy, mechanotherapy, psychocorrection, massage, physical therapy, phytotherapy.The patients were divided into three groups by randomization, without reference to the treatment method or any other factor. Allthree groups of patients received a basic program of medical rehabilitation, which included magnetotherapy, dry carbon dioxidebaths,speleotherapy and physical therapy. Results. Positive indicators of clinical and laboratory results and subjective data as a resultof rehabilitation were obtained in all patients from all 3 groups. In children with bronchial asthma of mixed genesis, as a result of complextreatment, a significant improvement was noted. These results allow us to speak about the significant effectiveness of individuallyselected targeted medical rehabilitation programs in patients with bronchial asthma. Conclusion. As a result of the analytical work carried out according to the results of the study, it can be concluded that the use of anindividual rehabilitation complex of procedures and therapeutic techniques in children’s patients with bronchial asthma is effective.The implementation of these methods in a complex improves the condition and well-being of patients according to a subjective feeling,and also allows us to reliably speak about the improvement of the condition according to clinical and diagnostic indicators. It ispossible to recommend the introduction of the use of these programs in the medical rehabilitation of this group of patients. Drawingup targeted programs for use in children’s patients with a history of bronchial asthma is an important link in medical rehabilitation,as it allows each patient to conduct an individually selected targeted rehabilitation program in the conditions of the department ofmedical rehabilitation for patients with somatic diseases.

Abstract:
Abstract The paper deals with a methodology proposed for measuring the concentration of air ions in the environment of speleotherapeutic caves, and with the implementation of the AK-UTEE-v2 ionmeter. Speleotherapy, in the context of its general definition, is the medical therapy that utilizes the climate of selected caves to treat patients with health problems such as asthma. These spaces are characterized by the presence of high air humidity and they make extreme demands on the execution of the measuring device, the Gerdien tube (GT in the following) in particular, and on the amplifier electronics. The result is an automated measuring system using a GT with low-volume air flow, enabling long-term measuring of air ion concentration and determination of spectral ion characteristics. Interesting from the instrumentation viewpoint are the GT design, active shielding, and execution of the electrometric amplifier. A specific method for the calculation of spectral ion characteristics and the mode of automatic calibration were proposed and a procedure of automatic measurement in the absence of attendants was set up. The measuring system is designed for studying and long-term monitoring of the concentration of light negative ions in dependence on climatic conditions and on the mobility of ions occurring in the cave.

Abstract Currently, there is no consensus on the salt caves aero ionic environment effects mechanism on the human body, however, there is physiotherapy a separate type, based on such exposure health-improving effect, known as speleotherapy. To recreate the salt cave environment aero ionic composition, a halo chamber is used. To reduce their cost, recreating the air environment methods by contacting flowing air means with carnallite rock finely dispersed powder and dry and wet aerosols generators have been developed. When implementing these methods, both the powder obtained directly by crushing the rock and the powder obtained by salt solutions evaporation drying can be used. The study aim is to reveal finely dispersed salt particles morphometric properties obtained by carnallite solutions spray drying based on different dispersion raw materials. It has been established fine salt particles those morphometric properties obtained by spray drying of carnallite solutions based on different dispersion raw materials differ in average size, sphericity factor, topography character and chemical composition. Particles formed by solution evaporation on the macro disperse raw materials (3-5 mm) basis have larger size, smaller fractional range, more compact shape and relief characterized by multifractality. At the same time, solution-based particles from macro disperse raw materials contain more impurities. Particles a distinguishing characteristic from solution-based lumpy raw materials is the potassium cluster distribution on the surface.

Abstract:
Introduction. Management of patients with chronic pathology requires development of long-term programs with organic combinations of medicamental and non-medicamental influences. Haloaerosoltherapy is a group inhalation of rock salt aerosol with concentration of more than 2-3mg/m3 and certain dispersion (with presence of large-, medium- and small-grained aerosol) aboveground. Material and method. On the basis of literature data review and results of own researches a comprehensive description of haloaerosoltherapy, its mechanisms, available technologies, indications and effectiveness at pulmonary pathology was given. Results and discussion. Basis of haloaerosol curative influence is hyperosmolar stimulus that improves drainage function of bronchi and provides sanitizing effect, which causes further changes at local and systemic levels. This justifies haloaerosoltherapy usage as method of rehabilitation treatment. Comparison of haloaerosoltherapy and other methods of halotherapy (use of halite), in particular, "salt rooms (caves)" was made. It has been proven that "salt rooms" can be used only as spa procedure. Conclusions. Application of term "halotherapy" to all methods that use halite or underground treatment is incorrect. This determines necessity for stop using this term. It is necessary to differentiate clearly applied methods of treatment using terms that reflect the essence of therapeutic effect. Haloaerosoltherapy should be considered a method of respiratory rehabilitation with a proven mechanism of action and effectiveness. Haloaerosol is obtained using specific devices - halogenegenerators. The effectiveness of haloaerosoltherapy immediately after the course of treatment and in remote period is testified. Abstract:
Speleotherapy (ST) is a non-drug method of treatment which uses the microclimate of underground spaces. Such spaces may be represented by karst caves, salt mines, rock-salt and metal mine workings. The specific environment of underground spaces is the main factor affecting the patient's body. Speleoenvironment is formed by the combination of medical and health-improving components. Microfine salt aerosol is the main component of the speleoenvironment. It is based on the light negative ions arising at high levels of air ionization. The other components of the speleoenvironment are the constancy of the microclimate, temperature background, humidity, air flow rate. Another important factors are the absence of allergens and low bacterial contamination. Shielding effect ot the underground space, protecting from the exposure to radio-magnetic radiation is also mentioned. The main stages of the formation and development of the ST method in the world practice are also discussed in the article. Various types of speleological hospitals and the peculiarities of their geographic location are described. The factors of the speleoenvironment are presented. These factors affect the patient's body if the method is used under various conditions.
