RésuméLe présent article décrit la constitution d’un corpus de français oral, baptisé les Récits du français québécois d’autrefois (RFQ), qui comprend des contes, des légendes et des entrevues recueillis auprès de locuteurs nés entre 1846 et 1895. Leur parler est celui du Québec rural du 19e siècle; il montre la variabilité inhérente et les structures vernaculaires caractéristiques des dialectes d’aujourd’hui. Nous examinons les avantages et les inconvénients de ces données diachroniques et montrons que, si elles sont utilisées judicieusement, elles représentent effectivement un stade antérieur du français québécois, avant son contact intense avec l’anglais. À ce titre, elles constituent un point de repère pour situer les traits linguistiques actuels. À partir d’une étude de cas de l’élision du ne, nous montrons comment une comparaison systématique entre les RFQ et le parler vernaculaire contemporain peut servir à repérer un changement linguistique, à le dater et à suivre son acheminement.

Abstract:
In a double-blind, two-period crossover study, 24 healthy volunteers were evaluated to establish the time of onset of action of activity of acrivastine in suppressing the weal and flare response to intradermally injected histamine. Volunteers received single doses of 8 mg acrivastine and placebo according to a fully randomized, balanced treatment plan. Acrivastine significantly ( P < 0.002) reduced the flare response induced by 0.4 μg histamine challenge 15 min after oral acrivastine dosing when compared with placebo. A significant ( P < 0.001) reduction of the weal response was noted at 25 min, although trends in this direction were already present at earlier time points. Dans d'une étude croisée à deux phase, réalisée en double aveugle et ayant porté sur 24 volontaires sains, on a tenté d'établir le moment du début de l'action de l'acrivastine dans la suppression de la réponse inflammatoire consécutive à l'injection intradermique d'histamine. Les volontaires ont reçu des doses uniques de 8 mg d'acrivastine et de placebo, selon un plan de traitement entièrement randomisé et équilibré. L'acrivastine a réduit significativement ( P < 0,002) la réponse de rubéfaction induite par 0,4 μg d'histamine 15 minutes après l'administration orale d'acrivastine, par rapport au placebo. Une réduction significative ( P < 0,001) de la réponse d'enflure a été notée à 25 minutes, bien qu'une tendance en ce sens ait déjà été observée à un stade plus précoce.

Background: Hypertrophic lupus is an uncommon clinical variant of chronic cutaneous lupus that remains a challenge to treat. A 45-year-old female day-care worker with long-standing lupus presented with hypertrophic cutaneous involvement on the dorsal hand, elbow, and toe of 6 years' duration. Treatmentsincluded monotherapy with either hydroxychloroquine or chloroquine and potent topical and intralesional steroid injection. Systemic chemotherapy with R–CHOP chemotherapy for stage IIA diffuse large B-cell lymphoma did not clear the skin findings. Objective: To review the clinical presentation and treatment of hypertrophic lupus. This report documents clinical improvement in refractory hypertrophic lupus with a regimen of acitretin and combination antimalarial therapy. Methods: The available published literature on the treatment of hypertrophic lupus was reviewed. There is limited published experience combining retinoids and antimalarials for the treatment of refractory lupus. Combination therapy with two antimalarials (ie, quinacrine with either hydroxychloroquine or chloroquine) provides therapeutic benefit for resistant cutaneous disease. In this case, triple therapy with two antimalarials and an oral retinoid achieved clinical clearance relatively rapidly, and this effect was maintained over a year. Results: This article reports successful treatment of refractory hypertrophic discoid lupus with combination therapy that included chloroquine 250 mg/d, quinacrine 100 mg/d, and actitretin 25 mg/d. Conclusion: This report suggests that combination therapy using two antimalarials and an oral retinoid is a consideration for refractory hypertrophic lupus, but further study is warranted. Contexte: Le lupus hypertrophique est une variante clinique rare du lupus cutané chronique, une affection difficile à traiter. Une travailleuse des services de garderie souffrant de lupus depuis longtemps a consulté pour la présence de lésions cutanées hypertrophiques sur le dos de la main, le coude et les orteils, et ce, depuis 6 ans. Plusieurs traitements ont été tentés, notamment l'hydroxychloroquine ou la chloroquine en monothérapie, de puissants médicaments topiques, et des injections intralésionnelles de stéroïde; la chimiothérapie R-CHOP par voie générale, pour les lymphomes diffus, à grandes cellules B, de stade IIA, n'est pas venu à bout des lésions cutanées. Objectif: L'étude avait pour objectif de passer en revue le tableau clinique et le traitement du lupus hypertrophique. Le présent rapport fait état d'une atténuation clinique du lupus hypertrophique réfractaire, par un traitement composé d'acitrétine et d'une association d'antipaludiques. Méthode: Nous avons passé en revue la documentation publiée, accessible sur le traitement du lupus hypertrophique. Les rapports publiés d'expériences associant les rétinoïdes et les antipaludiques dans le traitement du lupus réfractaire étaient peu nombreux. L'association de deux antipaludiques, soit la quinacrine avec l'hydroxychloroquine ou la chloroquine, procure un soulagement thérapeutique dans les cas de maladie cutanée rebelle. Dans le présent cas, la trithérapie, composée de deux antipaludiques et d'un rétinoïde oral, a produit, dans un temps relativement court, la disparition clinique des lésions, et l'effet s'est maintenu sur une période de 1 an. La mélancolie ou « folie calme » de Pinel a perdu depuis les années 80, son caractère de maladie autonome. Actuellement dans le DSM, elle désigne seulement une caractéristique des états dépressifs. Pourtant ce terme renvoie à un concept psychopathologique particulier dont la connaissance permet une meilleure compréhension de la maladie et une meilleure approche des patients. Nous mettrons ce concept à l’épreuve de la clinique chez trois patients ayant été hospitalisés pour un épisode mélancolique suite à une accusation de vol. Deux de nos patients ont présenté un syndrome délirant à thématique de persécution. Chez l’un d’eux, l’évolution s’est malheureusement faite vers le suicide. Nous essaierons à travers les vignettes cliniques de faire le point sur le rôle du narcissisme dans la psychopathologie de la mélancolie. En effet, chez nos trois patients, l’accusation de vol représente un événement traumatisant venant ébranler la relation à l’objet. Le moi appauvri en libido et fragile, s’identifie à l’objet frustrant afin de le préserver et le protéger de sa haine et de son activité destructrice. Il procède ainsi à une régression narcissique au stade oral tardif. Mais le moi ayant incorporé l’objet se trouve en conflit avec une autre instance qui est le surmoi. Ce dernier par sa fonction culpabilisante permet au mélancolique de se défendre contre l’incapacité du moi à être conforme à son idéal. Dans notre contexte culturel, le surmoi se trouve être plus collectif qu’individuel. Devant la perte de l’objet, l’introjection laisse la place à la projection de la haine sur un objet extérieur faisant émerger le délire de persécution. Enfin, quand l’identification à l’objet devient totale, les mouvements agressifs envers l’objet sont alors dirigés vers le moi aboutissant au non réparable qu’est le suicide.

OBJECTIVE: To determine the association among betel nut, betel quid or smoking alone, and betel quid combined with smoking in subjects reporting with oral sub mucous fibrosis, oral submucous fibrosis with malignant transformation in to cancer and oral squamous cell carcinoma. METHODOLOGY: An analytical cross sectional, multi centric study of n = 1009 cases collected through non-probability convenience sampling. These n=1009 subjects were subdivided into four groups: group 1, who consumed areca nut only; group 2, who chewed betel quid along with areca nut; group 3, who used betel quid and smoked; and group 4, who had no chewing habits history but were smokers. These changes were further confirmed with the help of biopsy reports of the subjects with OSMFCa and OSCC. Chi square test was performed to find out association of chewing habits with the progression of disease state. Level of significance was kept at p<0.05. RESULTS: The mean age of the sample (n=1009) were 42.79±1.31 years (range: 10-70 years). Statistically significant difference was (p<0.00) found among all four groups in terms of initiation, propagation and progression of oral squamous cell carcinoma. Furthermore, statistically insignificant difference (p=0.40) was found between group 2 and group 3 as similar number of cases (OSCC) was seen among them. CONCLUSION: Current study concluded that patients who have combined habits of chewing betel quid with areca nut and betel quid with smoking were at highest risk of initiation and progression of oral cancer. However, smoking cigarettes alone was the weakest risk factor. Haensel et Gretel dévorèrent une maison de sucreries : ils durent en assumer les conséquences. Pourquoi la gourmandise s’apparente-t-elle à une faute ? Pourquoi est-elle dénigrée alors que le plaisir, la jouissance et le désir du goût semblent agir tant le corps que le psychisme du sujet et ainsi interpeller la question de l’être entre éthique et esthétique ? Ethique : la gustation intègre une notion de transgression, mais ne peut se départir de l’autre, de la loi. Au sein de la réalité, la gourmandise laisse émerger du réel, ramène le sujet à la question de l’originaire, et concomitamment, élabore le surmoi. Esthétique : entre plaisir, jouissance immédiate, jouissance de l’être et de l’avoir et le principe de réalité, la gourmandise oscille, mais se situe au-delà. Elle est une jouissance autre. Elle est révélation d’un « étant » du sujet, d’une présence et expression de don, à soi, à l’autre, expérience et création d’un « être ». From within reality itself “gourmandise” allows the subject to transcend the real and confronts it with the issue if primal plenitude, while fashionning at the same time the superego. Aesthetics : ocillating between pleasure, instant gratification, possession of being, of having and the reality principle, “gourmandise” lies beyond all these. It is another kind of delectation. It is at one and the same time the revelation of an existent subject, of a presence, and also the incarnation of a gift to onself and the other ; the experience of and creation of an indeterminate “being”. A close reading of fairytales and myths will help to develop and elucidate our reflexions

L’objectif de ces travaux est de connaître et évaluer les caractéristiques épidémiologiques des cancers de la cavité orale et de l’oropharynx. Ces deux localisations partagent des facteurs de risque en commun, et sont de fait souvent regroupées dans les études épidémiologiques. Cependant, la découverte de facteurs de risque spécifiques, telle l’infection par le virus du papillome humain pour les cancers de l’oropharynx, nous conduit à fournir des taux d’incidence spécifiques avec la classification anatomique de ces cancers. En réorganisant les données disponibles dans la base des données du Centre International de Recherche sur le Cancer, nous avons recherché les cas incidents au niveau mondial et recalculé les taux d’incidence dans les registres de 60 pays, pendant la période 1998-2002. La classification TNM n’est pas disponible dans les bases de données du CIRC. Nous avons identifié et contacté les registres du cancer qui ont déclaré son recueil. Cela nous a permis de créer et structurer une base des données innovante et inédite, dont les informations ont été analysées par rapport à la qualité. Finalement nous avons comparé la distribution de stades précoces et avancés dans 8 pays. Les résultats montrent que l’incidence des cancers de la cavité buccale et de l’oropharynx est très hétérogène au niveau mondial par rapport à la sous localisation des tumeurs, à l’âge d’incidence, au ratio homme/femme et au stade clinique
Oral cavity and oropharynx cancers : International incidence and TNM classification in population-based cancer registries The aim of this work was to know and to evaluate the epidemiological patterns of oral cavity and ororpharynx cancers. These topographies share some common risk factors and they are often grouped in epidemiological studies. However, the implication of the human papilloma virus in oropharyngeal tumors lead us to provide incidence rates according to the anatomical classification of these tumors. We reorganized the incidence data available at the International Agency for Research on Cancer, for the period 1998-2002. Incidence rates were calculated for oral cavity and oropharynx cancers separately for 60 countries. As the TNM classification is not available on the IARC database we contacted the cancer registries that declared to abstract and collect it. Based on their data we created and structure a new, innovative and quality controlled. Finally, we compared the TNM stage distribution among 8 countries. The results show that the oral cavity and oropharynx cancers have a very heterogeneous distribution in the studied registries concerning tumor sub-sites, age of incidence, male to female ratio and clinical stage

La présente recherche s'intéresse à l'étude des précurseurs corporels et interactifs de la communication et du langage chez les bébés présentant des contraintes alimentaires importantes (liées à diverses affections pédiatriques). Le but est de vérifier si ces troubles précoces ont, ou non, une incidence sur la mise en place du langage et du style alimentaire. Il s'agit d'une étude ouverte, longitudinale, de l'âge de 3 mois jusqu'à 4 ans. La méthodologie inclut l'enregistrement audio-visuel mensuel des bébés avec leurs mères, tous les mois (pendant environ 10 minutes), ainsi qu'un film des interactions pendant le repas. Images et sons sont ensuite analysés par une équipe de cliniciens, de mathématiciens, et de statisticiens. L'évaluation inclut, par ailleurs, l'analyse du développement psychomoteur, du langage et d'éventuels troubles alimentaires. Différents cliniciens de la petite enfance relatent que la non-utilisation de la bouche pour l'alimentation, dans le cas de pathologies obligeant à mettre en place une nutrition entérale, (par voie parentérale ou par gastrostomie), donnerait souvent lieu à des difficultés de réalimentation, accompagnées parfois de troubles du langage. Nous avons constaté que le développement langagier n'est pas empêché quand la fonction alimentaire per os est mise à mal. Ceci voudrait dire que les processus concernant le stade oral - processus de prise en soi de ce qui vient du monde extérieur - peuvent être préservés même quand la bouche ne peut pas fonctionner de façon satisfaisante pour l'alimentation. D'autres réflexions concernant les différents mécanismes anorexigènes ont été également envisagées au cours de la présente recherche
This research concerns the language bodily and interactive precursors' development of babies suffering from disabled orality (paediatric diseases that prevents or strongly interfere with mouth feeding during the first months after birth) verifying if it might interfere with setting up of language. It's an open longitudinal study of children from 3 months until 4 years old. It consists on the videotaping of babies from 3 to 9 months with their mothers and/or fathers, every month, through a simple protocol of 10 minutes, and the videotaping of a feeding of the baby by its mother. The further image digital and clinical analysis is made by a team of clinicians, mathematicians and statisticians. The evaluation of this group of children includes the analysis of their psychomotor and language development, and of eventual feeding disorders. Clinical experience has shown that permanently putting aside the use of the mouth as the feeding route when we are obliged to use a non-oral one (such as nasogastric tube or gastrostomy), often engenders feeding difficulties when time for weaning comes, and is sometimes accompanied by delayed or impaired language development. We have seen that language development is not necessarily disturbed when the alimentary function (per os) is restrained. Which means that processes involved with the oral phase can be preserved even when the mouth cannot satisfactory function for feeding. Other thoughts concerning different anorexigenic mechanisms were also considered during the present study

Abstract:
Despite much progress, the infection of oral biomaterials by bacterial and fungal cells remains an important problem in the clinic, affecting millions of patients worldwide. Although biofilm formation comprises a series of stages, the initial cell-surface interaction is crucial in determining infection of the biomaterial surface. By employing single-cell force spectroscopy (SCFS) and nanoindentation with the atomic force microscope (AFM), the biophysics of the bacteria-biomaterial surface interaction has been characterised for Streptococcus sanguinis, Staphylococcus aureus and Candida albicans. Initially, the development and optimisation of a protocol to harvest and immobilise living bacterial and fungal cells for AFM experimentation is described. In a next step, SCFS was utilised to explore the influence of implant surface nanotopography on the colonisation of S. aureus, utilising an in vitro polycarbonate implant model. Although nanotopography was not found to influence bacterial elasticity, it did increase the adhesion of S. aureus to the surface at early time points. Subsequently, the interaction between clinically relevant titanium (Ti) implant substrates and S. aureus and S. sanguinis cells was studied, which demonstrated strain-dependent differences in the unbinding patterns observed in AFM experiments. Worm-like chain (WLC) modelling of unbinding events was used to predict the length of the bacterial adhesins involved in the Ti-bacteria interaction, which were found to be different for S. aureus and S. sanguinis. Finally, the attachment of C. albicans to acrylic surfaces at the single-cell level was explored with AFM. C. albicans was found to exhibit morphology-dependent adhesion onto acrylic, with adhesion being increased in hyphal tubes compared to yeast cells. Also, experiments suggest a potential correlation between strain virulence and increased adhesion to surfaces. Future work should focus on utilising this in vitro AFM model to explore novel antiadhesive and antimicrobial approaches at the single-cell level.

Clinical care for major depressive disorder (MDD) would be greatly improved if we had reliable clinical predictors of individual antidepressant treatment outcome. While, at the present time, no biomarkers have sufficiently proven utility to be ready for clinical application, several neuroimaging modalities have shown promise for such development. Attempts to combine the recently developed modality of resting-state functional Magnetic Resonance Imaging (rs-fMRI) with pharmacological challenges to explore the impact of antidepressants on resting-state brain connectivity have just begun (McCabe et al., 2011a, McCabe et al., 2011b). The aim of the current study was to investigate the effects of a single dose of the SSRI (selective serotonin reuptake inhibitor) escitalopram on resting-state functional connectivity in health.

AbstractOral squamous cell carcinoma is the third most common cancer in Indian subcontinent affecting people with lower socioeconomic status. Due to inadequate screening facilities and lack of awareness among individuals most of the oral cancer cases are detected at an advanced stage. As early stage oral squamous cell carcinoma patients can be treated with single modality treatment (surgery or radical radiotherapy), multimodality regimen (surgery followed by concurrent chemoradiation) is adopted for high risk advanced stage cancers with multiple adverse features like extra nodal extension, lymphovascular invasion and perineural spread. The chapter outlines the principles of adjunctive therapy in oral cancer patients with special reference to different techniques, indications of radiotherapy and role of chemotherapeutic regimes in improving the overall survival of advanced stage oral cancer patients.

Introduction: Uterine sarcoma accounts for nearly 3% of all uterine malignancies. They have 4 major pathology includes endometrial stromal sarcoma high grade, ESS low grade, uterine leiomyosarcoma (uLMS) and undifferentiated uterine sarcoma (UUS). Recent WHO classification 2014, recognizes low grade ESS and high grade ESS as distinct entity. They differ from endometrial carcinoma in their aggressive nature and poor prognosis. We review our database and found total 44 eligible patient treated at our institute. Materials and Methods: Its retrospective analysis of computer based database of our institute from January 2009 to December 2015. We analyzed demographic, pathological, treatment and survival data. Results: Total 44 patient treated for uterine sarcoma at our institute. Among these 16 were operated at our institute during study period. Here we reporting results of operated patients at our institute. The histological diagnosis LMS (5/16), ESS-L (4/16), MMMT (3/16), UUS (3/16) and ESS-H (1/16). Stage distribution was stage I, (6/16) stage II, (5/16) stage III, (3/16) stage IV, (0/16) and unknown stage (2/16). Two patients underwent completion surgery for outside myomectomy. The adjuvant treatment was CT in 3/16, CT with RT in 7/16, HT in 4/16 and one lost to follow up with one was put on observation. Median follow up is 30 month with 14 patients alive and one lost to follow up. At last follow up 4 patients alive with metastatic disease and 10 patients alive with no evidence of disease. Conclusion: Uterine sarcoma are uncommon disease with

A purposeful approach has been taken to match teaching pedagogies (techniques), learning experiences and assessment methods to various types of student learning in undergraduate aerospace propulsion courses at junior-level at the United States Air Force Academy and at the senior-level at Oklahoma State University. Prior studies in the scholarship of teaching and learning have shown the benefits of matching assessment methods, as well as teaching pedagogies and learning experiences, to the types of student learning associated with desired educational outcomes. Literature suggests that the best method for teaching and assessing student cognitive learning is through explanation and presentation. Oral assessments have been implemented at the Air Force Academy and Oklahoma State University to evaluate student cognitive learning in undergraduate aerospace propulsion and power courses. An oral midterm exam was developed to assess student acquisition of subject matter knowledge and understanding of fundamental concepts, the type of learning occurring early in course lesson sequences. End-of-semester design project poster sessions and presentations were used as summative oral assessments of student creative thinking, decision making, and professional judgement. Conversely, two written midterm exams and a final exam primarily focused on assessing student problem solving skills and less on comprehensive knowledge. Oral assessments also served as reflective thinking experiences that reinforced student learning. Student feedback on oral assessment methods was collected through surveys conducted after each assessment. Survey results not only revealed the effectiveness of using oral assessments but also how to improve their design and implementation, including the use of information technology and broader curricular employment.

The article aims to discuss the main features of teaching reading in the lessons of RFL (Russian as a foreign language) in the Chinese audience. Foreign language reading is significant speech activity because it is the basis of developing other speech activities. The article analyses the phonetical and methodological barriers, which may meet Chinese students while improving their reading skills, and provide some methods of removing the difficulties in reading. When teaching Russian to Chinese students, especially outside the linguistic environment, teachers do not pay due attention to practicing various types of reading, and, consequently, automating verbal forecasting. In the methodological literature, to overcome oral speech problems, the need to focus on spontaneous oral speech is noted, directly related to reading as a resource for expanding vocabulary and grammatical structures. Using system analysis, comparison, typology, theoretical and methodological forecasting, the work compares the most popular types of reading in China and Russia when teaching international students, analyzes the advantages and disadvantages of using these types of reading. Based on Chinese students’ most frequent difficulties, a developed balanced type text is proposed that considers the most problematic phonetic cases. Also, a system of various exercises is proposed, aimed at developing the skills of oral speech. The exercises and use of phonetically balanced texts might contribute to the further enrichment of RFL methodology. Protein C (PC) activity and antigen levels have been related to clotting activities of factors VII and X during the induction and withdrawal periods of oral anticoagulant treatment. Both factor VII and PC activities fell rapidly during a gradual induction regime of nicoumalone in six consecutive patients but factor VII showed a more rapid and much more marked depression than PC. In contrast reductions in factor X were much slower. PC antigen although depressed rapidly at the initiation of treatment did not subsequently fall to the same degree as PC activity, The ratio of activity to antigen became progressively smaller.In six further serial patients discontinued from long-term treatment with nicoumalone (mean duration 12-6 months) there was a reversal of the pattern, but with two important differences. Firstly, there was evidence of an excessive rise (“rebound”) of factor VII compared with the steady state levels in these patients; and secondly there was an unexpectedly slow return of PC activity and antigen to normal levels after the oral anticoagulant was withdrawn (levels were still below normal on day 4). Factor X also showed a slow rate of increase, similar to PC activity recovery. These observations lend support to gradual withdrawal of oral anticoagulants after a period of long-term administration. The results suggest that after discontinuation of long-term oral anticoagulants patients may have increased coagulability up to four days.

As the fibrin clot may play a role in the intravascular metastatic spread of breast cancer, we have treated women with advanced breast cancer with oral anticoagulants. After mastectomy, 11 consentient women with postsurgical stage Nl (6 patients), N2 (3 patients) and N3 (2 patients) and without evidence of distantmetastasis have been treated with acenocoumarol (INR 2-4,5) for 4-22 months (mean 12 months). All the patients received adjuvant therapy (radiotherapy, polichemiotherapy and/or tamoxifen) in conformity with theclassic indications. FibrinopeptideA plasmatic level, checked in everyone on stable anticoagulation, was always lower than 2 ng/ml (mean 0,78 ng/ml). No major bleeding occurred, except a copious hematuria caused by overdosage. Until now all the 11 women are alive. Two of them stopped anticoagulant, one because of hemorrhagic cystitis, the other because of awareness of visceral metastases; all the other 9 women arebeing treated now. Three patients(2N2 and 1 N3) have evidence of visceral metastases; no sign of relapsehas been observed by serial instrumental and laboratory examinations inthe other 8 women. A control group of 13 patients (9 N2 and 4 N3)was compared with the group of anticoagulated patients in more advanced stages (2 N2 and 3 N3):The follow-up of the anticoagulatedNl patients is too brief as yet forcomparative evaluation. Our preliminary data seem to indicate an useful role of oral anticoagulants inbreast cancer and oblige us to prolong investigation.
