RésuméLe présent article décrit la constitution d’un corpus de français oral, baptisé les Récits du français québécois d’autrefois (RFQ), qui comprend des contes, des légendes et des entrevues recueillis auprès de locuteurs nés entre 1846 et 1895. Leur parler est celui du Québec rural du 19e siècle; il montre la variabilité inhérente et les structures vernaculaires caractéristiques des dialectes d’aujourd’hui. Nous examinons les avantages et les inconvénients de ces données diachroniques et montrons que, si elles sont utilisées judicieusement, elles représentent effectivement un stade antérieur du français québécois, avant son contact intense avec l’anglais. À ce titre, elles constituent un point de repère pour situer les traits linguistiques actuels. À partir d’une étude de cas de l’élision du ne, nous montrons comment une comparaison systématique entre les RFQ et le parler vernaculaire contemporain peut servir à repérer un changement linguistique, à le dater et à suivre son acheminement.

Abstract:
In a double-blind, two-period crossover study, 24 healthy volunteers were evaluated to establish the time of onset of action of activity of acrivastine in suppressing the weal and flare response to intradermally injected histamine. Volunteers received single doses of 8 mg acrivastine and placebo according to a fully randomized, balanced treatment plan. Acrivastine significantly ( P < 0.002) reduced the flare response induced by 0.4 μg histamine challenge 15 min after oral acrivastine dosing when compared with placebo. A significant ( P < 0.001) reduction of the weal response was noted at 25 min, although trends in this direction were already present at earlier time points. Dans d'une étude croisée à deux phase, réalisée en double aveugle et ayant porté sur 24 volontaires sains, on a tenté d'établir le moment du début de l'action de l'acrivastine dans la suppression de la réponse inflammatoire consécutive à l'injection intradermique d'histamine. Les volontaires ont reçu des doses uniques de 8 mg d'acrivastine et de placebo, selon un plan de traitement entièrement randomisé et équilibré. L'acrivastine a réduit significativement ( P < 0,002) la réponse de rubéfaction induite par 0,4 μg d'histamine 15 minutes après l'administration orale d'acrivastine, par rapport au placebo. Une réduction significative ( P < 0,001) de la réponse d'enflure a été notée à 25 minutes, bien qu'une tendance en ce sens ait déjà été observée à un stade plus précoce.

Background: Hypertrophic lupus is an uncommon clinical variant of chronic cutaneous lupus that remains a challenge to treat. A 45-year-old female day-care worker with long-standing lupus presented with hypertrophic cutaneous involvement on the dorsal hand, elbow, and toe of 6 years' duration. Treatmentsincluded monotherapy with either hydroxychloroquine or chloroquine and potent topical and intralesional steroid injection. Systemic chemotherapy with R–CHOP chemotherapy for stage IIA diffuse large B-cell lymphoma did not clear the skin findings. Objective: To review the clinical presentation and treatment of hypertrophic lupus. This report documents clinical improvement in refractory hypertrophic lupus with a regimen of acitretin and combination antimalarial therapy. Methods: The available published literature on the treatment of hypertrophic lupus was reviewed. There is limited published experience combining retinoids and antimalarials for the treatment of refractory lupus. Combination therapy with two antimalarials (ie, quinacrine with either hydroxychloroquine or chloroquine) provides therapeutic benefit for resistant cutaneous disease. In this case, triple therapy with two antimalarials and an oral retinoid achieved clinical clearance relatively rapidly, and this effect was maintained over a year. Results: This article reports successful treatment of refractory hypertrophic discoid lupus with combination therapy that included chloroquine 250 mg/d, quinacrine 100 mg/d, and actitretin 25 mg/d. Conclusion: This report suggests that combination therapy using two antimalarials and an oral retinoid is a consideration for refractory hypertrophic lupus, but further study is warranted. Contexte: Le lupus hypertrophique est une variante clinique rare du lupus cutané chronique, une affection difficile à traiter. Une travailleuse des services de garderie souffrant de lupus depuis longtemps a consulté pour la présence de lésions cutanées hypertrophiques sur le dos de la main, le coude et les orteils, et ce, depuis 6 ans. Plusieurs traitements ont été tentés, notamment l'hydroxychloroquine ou la chloroquine en monothérapie, de puissants médicaments topiques, et des injections intralésionnelles de stéroïde; la chimiothérapie R-CHOP par voie générale, pour les lymphomes diffus, à grandes cellules B, de stade IIA, n'est pas venu à bout des lésions cutanées. Objectif: L'étude avait pour objectif de passer en revue le tableau clinique et le traitement du lupus hypertrophique. Le présent rapport fait état d'une atténuation clinique du lupus hypertrophique réfractaire, par un traitement composé d'acitrétine et d'une association d'antipaludiques. Méthode: Nous avons passé en revue la documentation publiée, accessible sur le traitement du lupus hypertrophique. Les rapports publiés d'expériences associant les rétinoïdes et les antipaludiques dans le traitement du lupus réfractaire étaient peu nombreux. L'association de deux antipaludiques, soit la quinacrine avec l'hydroxychloroquine ou la chloroquine, procure un soulagement thérapeutique dans les cas de maladie cutanée rebelle. Dans le présent cas, la trithérapie, composée de deux antipaludiques et d'un rétinoïde oral, a produit, dans un temps relativement court, la disparition clinique des lésions, et l'effet s'est maintenu sur une période de 1 an. Résultats: Le présent rapport fait état du traitement réussi du lupus discoïde hypertrophique réfractaire, composé de chloroquine, 250 mg/j; de quinacrine, 100 mg/j, et d'acitrétine, 25 mg/j. Conclusion: D'après le rapport, le traitement d'association, composé de deux antipaludiques et d'un rétinoïde oral, peut être envisagé dans le lupus hypertrophique réfractaire, mais une étude approfondie s'impose.

Abstract:
La mélancolie ou « folie calme » de Pinel a perdu depuis les années 80, son caractère de maladie autonome. Actuellement dans le DSM, elle désigne seulement une caractéristique des états dépressifs. Pourtant ce terme renvoie à un concept psychopathologique particulier dont la connaissance permet une meilleure compréhension de la maladie et une meilleure approche des patients. Nous mettrons ce concept à l’épreuve de la clinique chez trois patients ayant été hospitalisés pour un épisode mélancolique suite à une accusation de vol. Deux de nos patients ont présenté un syndrome délirant à thématique de persécution. Chez l’un d’eux, l’évolution s’est malheureusement faite vers le suicide. Nous essaierons à travers les vignettes cliniques de faire le point sur le rôle du narcissisme dans la psychopathologie de la mélancolie. En effet, chez nos trois patients, l’accusation de vol représente un événement traumatisant venant ébranler la relation à l’objet. Le moi appauvri en libido et fragile, s’identifie à l’objet frustrant afin de le préserver et le protéger de sa haine et de son activité destructrice. Il procède ainsi à une régression narcissique au stade oral tardif. Mais le moi ayant incorporé l’objet se trouve en conflit avec une autre instance qui est le surmoi. Ce dernier par sa fonction culpabilisante permet au mélancolique de se défendre contre l’incapacité du moi à être conforme à son idéal. Dans notre contexte culturel, le surmoi se trouve être plus collectif qu’individuel. Devant la perte de l’objet, l’introjection laisse la place à la projection de la haine sur un objet extérieur faisant émerger le délire de persécution. Enfin, quand l’identification à l’objet devient totale, les mouvements agressifs envers l’objet sont alors dirigés vers le moi aboutissant au non réparable qu’est le suicide.

OBJECTIVE: To determine the association among betel nut, betel quid or smoking alone, and betel quid combined with smoking in subjects reporting with oral sub mucous fibrosis, oral submucous fibrosis with malignant transformation in to cancer and oral squamous cell carcinoma. METHODOLOGY: An analytical cross sectional, multi centric study of n = 1009 cases collected through non-probability convenience sampling. These n=1009 subjects were subdivided into four groups: group 1, who consumed areca nut only; group 2, who chewed betel quid along with areca nut; group 3, who used betel quid and smoked; and group 4, who had no chewing habits history but were smokers. These changes were further confirmed with the help of biopsy reports of the subjects with OSMFCa and OSCC. Chi square test was performed to find out association of chewing habits with the progression of disease state. Level of significance was kept at p<0.05. RESULTS: The mean age of the sample (n=1009) were 42.79±1.31 years (range: 10-70 years). Statistically significant difference was (p<0.00) found among all four groups in terms of initiation, propagation and progression of oral squamous cell carcinoma. Furthermore, statistically insignificant difference (p=0.40) was found between group 2 and group 3 as similar number of cases (OSCC) was seen among them. CONCLUSION: Current study concluded that patients who have combined habits of chewing betel quid with areca nut and betel quid with smoking were at highest risk of initiation and progression of oral cancer. However, smoking cigarettes alone was the weakest risk factor. Aim. Obtaining stage results, pre-test questionnaire oral health’s assessment in patients with chronic kidney disease (CKD) – chronic renal failure (CRF), with or without nephritic syndrome, with or without initiated dialysis treatment. Material and method: the study was conducted by applying the questionnaire (pre-test) on a group of 30 patients – 15 patients in the target and 15 patients in the control group. Recorded data have been used for statistical analysis. Group distribution – gender: equal, 50% each group; age: average (years): 60.6 years old on target group and 62.6 years old on control group. IRC average time period (years) has been: 8.55 years on target group (women – 8.12 years and man – 9.04 years). On both groups the number of non-smoking patient has been higher then smoking patients. 9 patients on target group (5 women and 4 men) were framed on nephritic syndrome (average period of 16.5 years). Dialysis patients: 7 (4 women and 3 men), with an average period of 3 years. 67% of patients (target group) were in the range of 15-60 ml/min glomerular filtration rate. On the control group 30% of patients were between the values of 90-120 ml/min (or higher) and 50% in the range of 60-90 ml/min. Results and conclusions. In terms of oral health status of patients with CKD it was found that 50% of them are partially edentulous on more than a half-dental arcade, and 60% have no dentures.

Abstract:
Aim. Pregnancy-induced hypertension (PIH) and intrauterine growth restriction (IUGR) are both multisystemic disorders of pregnancy that cause perinatal morbidity and mortality. Recently, researchers focused on the role of oxidative stress (OS) as a pathophysiological mechanism in the development of these pathologies. The aim of this study was to compare OS in placental-related pathologies (PIH and IUGR) and uncomplicated pregnancies. We also investigated which salivary OS markers reflect systemic oxidative status and which only reflect the state of the oral cavity. Material and Methods. A total of 104 pregnant women (n=104; 27 with PIH, 30 with IUGR, and 47 controls) were evaluated. Malondialdehyde (MDA), total antioxidant capacity (ORAC), aldehyde dehydrogenase (ALDH), and activity of glutathione peroxidase (GPx) and glutathione transferase (GST) in plasma/whole blood and/or saliva were analysed. Dietary nutrient intake was calculated using a Semiquantitative Food Frequency Questionnaire (SFFQ). Oral health was assessed to eliminate patients with bleeding, severe periodontitis, and other dental pathologies. Results. In the IUGR group, increased concentration of ORAC was observed both in saliva and plasma. Also, lower plasma levels of MDA in IUGR compared to the control group was detected. No sign of oxidative stress was confirmed in the PIH group. The examined groups did not differ regarding diet and markers of inflammation. ORAC in saliva was correlated with its level in plasma. No such correlations for MDA were observed. In the IUGR group, there were no differences in OS markers in plasma, but there was a lower ALDH level in the blood compared to the control group. It confirms OS occurrence in IUGR. In IUGR, a higher activity of salivary ALDH was probably due to worse oral health. Conclusion. Oxidative stress differs between IUGR and PIH groups: the presence of oxidative stress was confirmed only in the IUGR group. Salivary ORAC can be used to estimate ORAC in plasma. The activity of salivary ALDH reflects the state of the oral cavity.

ABSTRACT Saliva plays a vital role in oral health as patients strive to maintain a healthy dentition throughout their lives. It is nature's primary defense mechanism for the oral environment, and is particularly important for protecting exposed tooth surfaces. While internal protection for dentin comes from odontoblasts and the dental pulp, the body's external protection for enamel comes from saliva. The noninvasive nature of salivary testing has made it an effective alternative to blood and urine testing and home testing kits have made it possible for people to monitor their own health using this diagnostic medium. Abstract:
The specific function of phosphatidylserine (PS) in the context of the development of a hypercoagulable state among individuals with oral squamous cell carcinoma (OSCC) is uncertain. The goal of this study was therefore to assess the exposure of PS on microparticles (MPs) as well as on endothelial and blood cells and to assess procoagulant activity (PCA) as a function of the stage of OSCC progression. We recruited patients with OSCC ( n = 63) as well as healthy controls ( n = 26) to participate in this study. PS exposure was then assessed via confocal microscopy and flow cytometry, revealing that patients with stage III/IV OSCC exhibited higher frequencies of PS-exposing blood cells, MPs, and serum-cultured endothelial cells (ECs) than did patients with stage I/II OSCC or healthy controls. When we conducted functional coagulation assays, we discovered that PS+blood cells, MPs, and serum-cultured ECs from patients with stage III/IV OSCC mediated more rapid coagulation and more substantial production of FXa, thrombin, and fibrin as compared with controls. When samples were treated with the PS antagonist lactadherin, this resulted in an 80% disruption of PCA. Strikingly, when pre- and postoperative samples were compared from patients with stage III/IV OSCC undergoing resective surgery, PCA was significantly reduced in the postoperative samples. After stimulating ECs with inflammatory cytokines, we found by confocal microscopy that they expose PS on their cell membranes, thus generating FVa and FXa binding sites and mediating the formation of fibrin. Together our findings provide evidence that PS+blood cells and MPs are important mediators of the development of a hypercoagulable and prothrombotic state among individuals afflicted by advanced-stage OSCC. As such, a PS blockade may be a viable therapeutic strategy for treating such patients.

The tadpole of Hypsiboas atlanticus (Caramaschi & Velosa, 1996) is described from the municipality of Maceió, State of Alagoas, Brazil. At stage 36 the larvae have an overall elliptical body in lateral and dorsal views, oral disc anteroventral, spiracular tube sinistral, and labial tooth row formula 2(1,2)/3(1). The oral disc is surrounded, almost completely (anterior medial gap present) by a single row of marginal papillae. Described tadpoles of the H. punctatus species group can be differentiated by a combined disc oral features. Additional descriptions of H. punctatus (Schneider, 1799) tadpoles from populations throughout South America may be helpful in determining the status of these populations.

ABSTRACT BACKGROUND AND AIMS Activin, a member of the transforming growth factor beta protein family, was originally isolated from gonadal fluids and stimulates the release of pituitary follicle-stimulating hormone (FSH). Activin has numerous functions in both normal and neoplastic cells. Although it was suggested that gonadal tissue is the primary site of activin production, several extragonadal sources have subsequently been identified, including human thyrocytes. METHODS In our study, serum levels of activin A were measured by chemiluminescence, and the expression of receptors of activin A in thyroid tissue was detected by immunohistochemistry in female patients with thyroid papillary cancer [stage I (n = 60), stage II (n = 60), stage III (n = 60), stage IV (n = 60)] and in normal controls (n = 60). RESULTS The serum levels of activin A were no significantly different between patients with thyroid papillary cancer and normal controls as well as different stages. But the expressions of receptors of activin A were greater in patients with thyroid papillary cancer than in normal controls. And the positive rates of receptor expression were significantly more in stage III and IV than in stage I and II. CONCLUSION In conclusion, this study demonstrates that serum levels of activin A undergo no significant changes when thyroid papillary cancer occurs. The thyroid gland is not the predominant source of activin A. The expressions of receptors of activin A were significantly greater in patients with thyroid papillary cancer than in normal controls. And the positive rates of receptor expressions were associated with the severity of cancer. Because activin A may exert negative action on thyrocyte proliferation, it is conceivable that the increase in the receptor of activin A in thyroid papillary cancer might represent a counteracting mechanism.

Abstract Objective: to evaluate the self-perceived oral health status of elderly persons and its relation to self-care practices, the use of public oral health services, denture use, dental complaints and impact on everyday activities. Method: The 95 subjects of this quantitative, analytical and cross-sectional study were from Fortaleza, a state capital in the northeast of Brazil. They were aged 60 years or over and were mentally capacitated according to the Mini-Mental State Examination. The study parameters included gender, age group, race/ethnic background, level of schooling, household income, self-care practices, use and need for dentures, dental complaints, impact on everyday activities, access to oral health services, and access to information. The outcome parameter was self-perceived oral health. To verify the association between the study parameters and the outcome parameter, prevalence ratios were calculated and submitted to the Chi-squared test, the Fisher's exact test, the Mann-Whitney test and multivariate regression analysis. Result: self-perceived oral health was described as good/excellent significantly more often by women than by men (p=0.044). Oral health had a negative impact on everyday activities among nearly one third of the sample (n=29; 30.5%). The mean and median values of dental complaints and impact on everyday activities were significantly lower for subjects reporting good/excellent oral health than for subjects reporting poor/fair oral health. Conclusion: it is expected that these results will strengthen oral health care for elderly persons, in order to maintain their quality of life during this stage of life.

Changes in dental plaque, oral microbial flora, and local oral immunity may be important in the development or exacerbation of disease in critically ill patients, trauma patients, adults with chronic obstructive pulmonary disease, and frail elderly. Inasmuch as oral health potentially can be influenced by nursing interventions, nursing research in this area can contribute greatly to improved patient outcomes in these diverse populations. The authors’ research teams have conducted several federally funded projects focused on oral health and have developed synergy in research methods. A unifying theme for these research projects is the measurement of oral health. Standardized measures of components of oral health are available and applicable across populations, and their uses and relationship to nursing research and patient outcomes will be discussed.

According to the Union for International Cancer Control (UICC)/ American Joint Committee on Cancer (AJCC), staging system for the locally advanced laryngeal cancer generally denotes stage III or IV, stage III being represented by T3 or N1 tumors and the non-metastatic stage IV including N2-N3 or T4 tumors. The main therapeutic goals are local control and survival, but also the functional organ preservation (speech, swallowing and airway patency), if possible. To achieve these objectives, the management should be established by a multidisciplinary tumor board, based on the analysis of patient-specific factors (age, performance status, comorbidities, and psychosocial support), cancer topography and staging, but also the physician expertise and the availability of rehabilitation services. Regarding the larynx preservation, there are two major therapeutic strategies: total laryngectomy (associated with adjuvant radio- and chemotherapy) and larynx preservation strategy, which includes neoadjuvant chemotherapy followed by exclusive radiotherapy or concurrent radio-chemotherapy or radio-biotherapy. Total laryngectomy can be performed by open surgery or, in order to avoid a wide surgical field and reduce the local morbidity, by transoral techniques. After laryngectomy, the recurrence can be local, at the resection site, nodal, at cervical lymph nodes, or distal, the lung being the most common site of recurrence as a distant metastasis. To improve locoregional control and survival, adjuvant treatments are proposed, including radiotherapy, chemo- and biotherapy.

The objective of this study was to improve the solubility of poorly water-soluble drugs by pharmaceutical cocrystal engineering techniques and select the best pharmaceutical forms with high solubility and solubilized formulations for progress from the early discovery stage toward the clinical stage. Several pharmaceutical cocrystals of TAK-020, a Bruton tyrosine kinase inhibitor, were newly discovered in the screening based on the solid grinding method and the slurry method, considering thermodynamic factors that dominate cocrystal formation. TAK-020/gentisic acid cocrystal (TAK-020/GA CC) was selected based on a physicochemical property of enhanced dissolution rate. TAK-020/GA CC was proven to be a reliable cocrystal formation with a definitive stoichiometric ratio by a variety of analytical techniques—pKa calculation, solid-state nuclear magnetic resonance, and single X-ray structure analysis from the view of regulation. Furthermore, its absorption was remarkable and beyond those achieved in currently existing solubilized formulation techniques, such as nanocrystal, amorphous solid dispersion, and lipid-based formulation, in dog pharmacokinetic studies. TAK-020/GA CC was the best drug form, which might lead to good pharmacological effects with regard to enhanced absorption and development by physicochemical characterization. Through the trials of solid-state optimization from early drug discovery to pharmaceutical drug development, the cocrystals can be an effective option for achieving solubilization applicable in the pharmaceutical industry.

Background: Health is generally regarded as a very high good, and oral health may substantially affect the quality of life of patients. Oral health–related quality of life has usually been investigated by means of disease-specific descriptive instruments, such as the Oral Health Impact Profile and the General Oral Health Assessment Index. These instruments, however, do not enable a comparison of oral health–related quality of life with other medical diseases. Economic methods, such as the time trade-off technique, enable a comparison of the impact of oral health with other medical diseases and thus provide a means to build a bridge in quality-of-life assessments between medicine and dentistry. Methods: We included in our study a total of 58 patients who received a complete denture in our clinic in the last 10 y (between January 2001 and May 2012) and who were ≥65 y old. Patient preferences for the edentulous and poorest imaginable oral health state were assessed via the time trade-off method. Results: Edentulous patients rated their current oral health state as 0.73 (SD, 0.25) and the poorest oral health state as 0.43 (SD, 0.33) on a scale between 0 (death) and 1 (best possible health state). These results are comparable to patient preferences for other serious diseases, such as breast cancer (0.75), asymptomatic HIV infection (0.69), depression (0.44), and osteoarthritis of the hip (0.44). Conclusion: In conclusion, our results suggest that oral health may substantially affect quality of life no less than other medical diseases. Knowledge Transfer Statement: Health is generally considered the highest good of humankind. In the present article, we show that oral health substantially affects quality of life. In particular, we show that loss of teeth (i.e., being edentulous) reduces quality of life no less than other systemic diseases. Treatment modalities for the edentulous patient may therefore substantially improve the patient’s well-being and should be a research priority.
