Abstract:
Objective This study examined the effect of using a variable-height staple construct containing three rows of staples with heights of 3.0, 3.5, and 4.0 mm (staple leg length, medial to lateral) versus standard three-row single-height staplers (with staple heights of either 3.5 or 4.8 mm) for pneumostasis in healthy porcine and canine lung parenchyma to determine whether a single stapler that uses variable staple heights could perform as well as, or better than, existing single-height stapling devices. The work presented here used healthy animal tissues, in lieu of diseased tissue, which is extremely difficult to obtain and quantify. Methods Briefly, fresh explanted porcine and canine trachea-lung blocs were used for all testing. Tissue thicknesses were measured with a custom-design spring-loaded caliper before stapling with control and test articles to ensure that the tissue was of “appropriate” thickness for the stapler size (staple height) selected (per manufacturer's instructions for use). All tissue measurements were comparable across each area of lung tested, and both test and control devices were fired into the same tissue thicknesses. After stapling, the lungs were submerged in water, insufflated, checked for air leaks at four discrete (increasing) pressures, and scored using a predetermined scale. Statistical analysis was performed for n = 26 (3.5-mm staples), n = 29 (4.8-mm staples), and n = 26 or 29 (paired to the standard group) for the variable-height stapler (3.0-, 3.5-, and 4.0-mm staples). Results The results demonstrated that the test article comprising three rows of variable-height staples provided comparable pneumostasis with the standard three-row single-height staplers (with staple heights of either 3.5 or 4.8 mm) under the test conditions described. Conclusions A novel test article containing three rows of staples with heights of 3.0, 3.5, and 4.0 mm (variable-height stapler) showed promising results when compared with standard commercially available single-height staplers, performing as well as or better than the standard single-height staplers. This work demonstrates important first steps to proving equivalent device performance, which might facilitate the use of a variable-height stapler in place of multiple single-height staplers.

Abstract:
Background: Laparoscopic appendectomy is the gold standard for treatment of appendicitis. Stapled closure of the appendiceal stump is commonly performed and has several advantages. Few prior cases have demonstrated complications from free staples left within the abdomen after the stapler has been fired.Case report: A 29-year-old female underwent laparoscopic appendectomy for acute uncomplicated appendicitis during which the appendix and mesoappendix were divided using laparoscopic gastrointestinal anastomosis (GIA) staplers. Her initial recovery was uncomplicated. She returned on postoperative day 17 with sharp mid-abdominal pain, obstipation, and emesis. Her abdomen was distended, and she had a mild leukocytosis. Computed tomography (CT) demonstrated twisted loops of dilated small bowel in the right lower quadrant with two transition points, suggestive of internal hernia with closed loop bowel obstruction. Diagnostic laparoscopy was performed through the three prior appendectomy incisions. An adhesion between the Veil of Treves and the mesentery of the ileum caused by a solitary free closed staple, remote from the staple lines, had caused an internal hernia. The hernia was reduced, and the small bowel was noted to have early ischemic discoloration. The adhesion was lysed by removing the staple from both structures. The compromised loops of bowel began to show peristaltic movement and color returned to normal, and the procedure was concluded without resection. She was discharged home the following day.Conclusions: Gastrointestinal staplers are commonly used due to ease of use and low complication rate. It is not uncommon to leave free staples in the abdomen as retrieval can be difficult and time consuming. Our case is only the second in the literature reporting an internal hernia with closed loop bowel obstruction as a complication of retained staples. Choosing the most appropriate size staple load to reduce the number of extra staples, removing free staples, or the use of an endoloop can prevent potentially devastating complications.

Use of the scanning electron microscope (SEM) for examination of tool marks, paricularly firing pin impressions of semi-automatic pistols, selected shotguns and rifles, is well documented. It was the intent of this investigation to apply SEM to staplers, in order to examine if the impressions on the shoe-ends of staples using a particular stapler possessed persisting defects with repeated tackings, and whether these defects can be used as “individual characteristics” in order to be able to identify the source of a staple on tacked papers from a list of eight staplers of different makes and models (Swingline® 747 and 27, ASCO® 40 and ACE® 102). The effects of pre-existing marks, which are usually existent as parallel striations on staples, and the different forms (100% round, 100% round with sharp points and chisel points) of staples on the resulting impression were examined by using different brands of staples (Swingline® standard No. S.F.-1, Swingline® R. W.-35, Bostitch® standard, and ACE® 2025).

Abstract:
A small Zenker's pouch is a major challenge for endoscopic staple-assisted esophagodiverticulostomy (ESED). This study tested the stapler dimensions so as to identify limitations they impose on ESED. Combining ESED with additional endoscopic suturing could extend the incision and consequently the myotomy. Zenker's diverticulum residual pouch measurements were performed with a previously reported latex glove model and in patients undergoing surgery. Two stapler designs were compared by measuring the residual pouch length for both the stock and modified staplers. One other stapler model cannot be modified without damaging the mechanism. The Endostitch was used to place an additional suture at the apex of the staple line, allowing cutting between the staples while leaving a closed distal incision. All three staplers suitable for ESED leave a residual pouch of 1.5 cm when unmodified. The modified anvil staplers gave a smaller residual pouch by 4 to 5 mm. An additional septal reduction can be accomplished by suturing the area distal to the staple line and incising the party wall beyond the stapler cut. The absolute amount of residual pouch with an additional myotomy is 3 mm. The combined staple-suture technique has proven relatively safe thus far. To provide a maximally efficient myotomy in a patient with a short pouch, the surgeon needs to be aware of the stapler differences. Modifications of the staplers may decrease the depth of the residual pouch, but may carry an added liability. A minimum residual pouch can be achieved with a combination of stapler and suture techniques, but is more technically demanding than the original ESED description.

Background: The objectives of the study were to study the operative time, the effect on wound healing, cosmetic results, patients acceptance and total cost with the use of sutures and staples.Methods: The study was conducted on 100 patients who were undergoing elective surgery from January 2016 to July 2017 in our institute. The patients were randomly selected to receive either suture or staple.Results: The study group included 50 patients who underwent wound closure by staplers and 50 patients underwent suturing. The commonest region of the surgical wounds was Mc Burneys site. The time taken for wound closure using staplers showed statistically significance difference over closure with suture, it took the stapler 4 times less duration to perform wound closure. The average cost of using stapler was higher than suturing. The appearance of the scar among the staple group was good in 90% of those who returned for follow-up at 1 month, 10% had average scar. The patients acceptance was better in staple group with less pain during removal as compared to suture group. P-value calculated using students unpaired T-test. P<0.0001 which was highly significant.Conclusions: Staples did not cause excess wound pain and allows saving in time with better cosmetic results.

Abstract:
Objective: Healing of laparotomy wounds is associated with number of factors. The aim of the study was to compare skin stapler and polypropylene sutures in closing laparotomy wounds. Materials and method: This randomized controlled trial study was conducted at the Department of Surgery, Sir Salimullah Medical College Mitford Hospital, Dhaka, Bangladesh. The study duration was 6 months, from July 2010 to December 2010. A total of 100 participants were included in the study, divide in two equal groups of 50 participants. The first group received interrupted suture with 2/0 polypropylene. The other group received skin staples. Results: The mean time for closure with suture and staple were 3.7±SD and 22.5±SD cm/min respectively (p- value<0.05). Rate of wound closure was calculated in cm per minute and materials were compared statistically in pairs by t- test. Wounds were closed five times faster with staples then suture (p<0.001). In relation to post-operative infection, 17 (34%) cases developed wound infection in sutured group and 15 (30%) patients in stapled group. Most of the infection occurred in the emergency contaminated cases. There was no significant association between wound infections and method of skin closure (p> 0.05, chi square= 3.15). Conclusions: Staples were well-liked by operators and resulted in substantial and worthwhile savings in time for wound closure.

Shape memory alloy (SMA) staples in nickel titanium with shape memory effect are effective for spinal growth control. This study was designed to evaluate the biomechanical properties of the staples and observe the stability of the fixed segments spine after the staples were implanted. According to the vertical distance of the vertebrae, SMA staples of 5, 6.5, and 8 mm were designed. The recovery stress of 24 SMA staples in three groups was measured. The pullout strength of SMA staples and stainless steel staples in each functional spinal unit was measured. Each of the six fresh specimens was divided into three conditions: normal, single staple, and double staples. Under each condition, the angle and torque of spinal movements in six directions were tested. Results show that the differences in recovery stress and maximum pullout strength between groups were statistically significant. In left and right bending, flextion, and extention, the stability of spine was decreased in conditions of single staple and double staples. Biomechanical function of SMA staples was superior to stainless steel staple. SMA staples have the function of hemiepiphyseal compression and kyphosis and scoliosis model of thoracic vertebrae in goat could be successfully created by the fusionless technique.

Lack of reinforcements is an existing drawback of 3D printed cementitious components, which is an urgent concern. A staple-inserting apparatus was developed and installed on a 3D printer and automatically fabricated 3D printed and staple-reinforced components with 98% successful insertion to achieve inner- and inter-reinforcement of the printed strips. The inserted staples inside the printed strips improved the compressive strength by 25% maximum owing to the inner locking effect by the staple pins, while the flexural strength did not increase because the scattered staples functioned separately. The staples over the strip interfaces remarkably increased the flexural stress by 46–120%. The inserted staples demonstrated a significant strip locking effect, but the unavoidable voids decreased the bonding between staples and the composite. The mechanical analysis concluded that the printing parameters considerably affected the reinforcing rate. The staple inserting technique proved the feasibility of automatic fabrication of fiber-reinforced and printed concrete structures.

We reviewed the use of compression staples made of the memory metal alloy nitinol for fixation in foot surgery. A retrospective study was performed of 31 feet in 27 patients who underwent arthrodesis or an osteotomy fixated using compression staples. OSStaples (BME, San Antonio, Texas) were used in 18 feet, and Memodyn staples (Telos Medical, Fallston, Maryland) were used in 13 feet. A total of 48 compression staples were implanted. The following procedures were performed and fixated using compression staples: 15 Akin osteotomies, 2 first metatarsal base epiphysiodeses, 3 first metatarsal–cuneiform fusions, 2 naviculocuneiform fusions, 3 calcaneocuboid fusions, 4 talonavicular fusions, 3 subtalar joint fusions, and 2 Evans osteotomies. In our clinical experience, compression staples provide an adequate source of internal fixation in foot surgery. Good bone apposition and stabilization and compression of the bone surfaces before staple fixation are important when using staple fixation to promote an optimal environment for bone healing. Limb asymmetry can, and often does, cause various health problems. Blount bone staples (clips) are used to correct such uneven growth. This article analyzes the performance of a biomechanical staple during bone (tibia) growth arrest. The staples considered in this study were made of 1.4441 stainless steel, the model of tibia consisted of two materials representing corticalis and spongiosis. Hooke’s law was used for modeling materials’ behaviors for finite element analysis (FEA). The maxima of stress and total staple displacement were evaluated using the finite element method and verification of the results, along with the determination of the maximum loading (growing) force that the staples are capable of withstanding, was performed experimentally. The presented method can be used to determine the safety and usability of staples for bone growth arrest. According to our results, the design of Blount staples considered in this paper is safe and suitable for orthopedic treatment.

O presente relatório resulta de um estágio curricular realizado na Staples Portugal, no departamento de Recrutamento e Seleção. O estágio consistiu na envolvência ativa em todas as fases do processo de R&S, com o intuito de pôr em prática os conhecimentos desenvolvidos ao longo do mestrado. Inicialmente, são enquadrados teoricamente os conceitos inerentes ao recrutamento, dando especial ênfase ao recrutamento externo e às técnicas que o caracterizam. Em seguida, procede-se à contextualização da empresa e à descrição das tarefas realizadas ao longo do estágio. Por fim, é exposto um balanço crítico do estágio, na qual são confrontadas as atividades desenvolvidas ao longo do estágio com o enquadramento teórico.
The present report is the final result of an internship carried at Staples Portugal, in the Recruitment and Selection department. The internship consisted in active involvement in all stages of the R&S process, with the objective of practically apply the knowledge attained during the Master's degree. Initially, the concepts inherent to recruitment are theoretically framed, giving special emphasis on external recruiting and the techniques that characterize it. Afterwards, the company is contextualized and the tasks carried out during the internship are described. To conclude, a critical assessment of the internship is presented, in which the activities developed during the internship and the theoretical framework are compared.

Bakgrund och problem: Ett styrmedel företag kan använda för att nå sina mål är belöningssystem. Utformningen av belöningssystemet samt att det tydligt motiverar de anställda är två viktiga aspekter. Tillsammans är belöningssystem samt motivation två välstuderade ämnen inom vissa branscher. För mer monotona arbeten som lagerverksamhet saknas dock detta arbete. Av denna anledning är det av intresse att studera hur lagerpersonal upplever att de motiveras av belöningssystemet.    Syfte: Syftet med studien är att beskriva fallföretagets belöningssystem samt utreda hur de anställda upplever belöningssystemet och huruvida det motiverar dem. Om det empiriska materialet visar på svagheter i belöningssystemet presenteras förslag på förändringar.    Metod: Studien har genomförts i form av en fallstudie på fallföretaget. Flertalet respondenter har intervjuats genom semi-strukturerade intervjuer. För att stärka studiens kvalitet har flertalet teoretiska källor använts.     Slutsats: Fallföretaget har idag uppsatta mål som de anställda arbetar mot samt ett utvecklat belöningssystem. Det som saknas för att belöningssystemet ska vara en motivationsfaktor är kopplingen mellan uppnådda mål och belöningen för detta.Det empiriska resultatet visar att gemenskapen är det som gjort att de anställda stannat kvar på företaget. Gemenskapen och grundlönen är även de största motivationsfaktorerna.
Background: One of the control mechanisms the companies can use to reach their objectives is a reward system. The parts included in the reward system and that it motivates the employees are two important aspects. The two subjects, reward system and motivation, are very well studied for certain branches. For more monotonous works such as warehouse operations, that is not the case. Therefore,it is interesting to study how warehouse employees experience if and in what way they are motivated by the reward system.    Purpose: The purpose of this study is to describe the case company’s reward system but also examine how the employees are experiencing the system and to find out how it motives them. If the empirical data shows that there are weaknesses in the reward system, suggestions for improvements will be given.    Method: The paper has been conducted in a case study form. More than one respondent has been interviewed, by semi-structured interviews. To increase the quality of the study different theoretical sources have been used.    Conclusions: The employees at the case company are working towards achieving objectives conducted by the company and the company has a well-developed reward system. However, what is missing is the connection between the objectives and the reward to make the reward system a factor of motivation. The empirical data shows that the feeling kinship among the employees is the reason why they have stayed at the company. The kinship and the salary is also the most important factors of motivation.

Bakgrund: Högre grad av automation i lager har blivit en viktig och tilltagande trend. Lager är en viktig komponent i de flesta flöden och essentiellt i strävandet av önskade servicenivåer då de ofta verkar i det avslutande stadiet i flöden där ordern förbereds och levereras till kund. Den ständigt växande konkurrensen på marknaden har dock lett till att lagerverksamheter ständigt måste arbeta med kontinuerlig förbättring. Genom eliminering av icke värdeskapande moment skapas förutsättningar för att uppnå effektivare lager och överlag en högre kvalitet i lagerverksamheten. Kvaliteten som erbjuds samt kvalitetens betydelse för kunden är det som utgör den totala kvalitetsnivån, eventuella avvikelser från denna nivå betraktas som brister. Syfte: Syftet med studien är att genom kartläggning beskriva hur påfyllnads- och plockprocessen på den automatiserade delen av Staples lager utförs. Vidare ämnar studien identifiera kvalitetsbrister som uppstår i påfyllnadsprocessen och plockprocessen. Dessutom ämnar studien undersöka de bakomliggande orsakerna för kvalitetsbristerna och ge förslag avseende hur påfyllnads- och plockprocessen kan förbättras. Metod: Studien är en kvaliativ fallstudie på Staples. Datainsamlingen utfördes genom semi-struktuerade intervjuer, direkta observartioner och fokusgrupper. Inledningsvis utfördes processkartläggning för påfyllnads- och plockprocessen, varpå kvalitetsbrister i de båda processerna identifierades. Därefter utarbetades orsakerna bakom kvalitetsbristerna och förbättringsförslag presenterades. Slutsatser: Ett flertal kvalitetsbrister i påfyllnads- och plockprocessen identifierades. Samtliga identifierade kvalitetsbrister kunde härledas till de tre främst förekommande kvalitetsbristerna Staples tillämpar på lagret: Saknas i kolli, Felplock: artikel eller antal och Dålig nedpackning. Bakomliggande orsaker till dessa var avvikande arbetssätt på ZtZ, att produkter faller ur kartong eller skadas på transportbandet och fellevererat till I-iiiPoint. Dessutom var brister i kvalitetsarbetet orsakade av högt produktivitetsfokus, otydliga orsakskoder och bristfällig kostnadsmedvetenhet avseende bristande kvalitet förekommande i alla tre. Genom att faställa mål för kvalitetsförbättring, grundade i identifierade kvalitetsbrister och personalens kompetenser ökar möjligheten för att de ska realiseras och förankras bättre hos personal. För att uppnå kvalitetsförbättring bör ledarskap som uttrycker betydelsen av kvalitet i såväl anvisningar som handling fastställas. Genom utbildning kan personalens förståelse öka avseende deras påverkan på kvalitet och koppling mellan olika aktiviteter och resultera i ökat kvalitetsansvar. Abstract:
In mountainous terrain, landscape can influences the thermal state of snow. Snow temperature and mass flux have been calculated using an energy balance model, Radtherm/RT, to account for the effects of topography and meteorological conditions. For a specific location, a terrain model is defined and contains a connected assemblage of elements or facets. Each element has a specified terrain type with assigned thermal properties. Meteorological data are applied, and a one dimensional energy balance is calculated for each element. This energy balance includes conduction, convection, radiation, and latent heat; however, the calculation of radiation is unique. Taking into account topography, global position, and time, the model is used to calculate incoming solar radiation for each element as well as reflected short wave radiation and the exchange of long wave radiation between terrain surfaces. Light detection and ranging topographic data with a one meter resolution were used to create separate models (on the order of 10 4 m2) for two slopes in southwest Montana. Meteorological data were collected at these two slopes as well as a third location having a relatively unobstructed view of the sky. The results for elements in different locations and under different meteorological conditions were compared. Readily available USGS topographic data with a 30 meter resolution were used to create a model (on the order of 10 6 m2) containing both slopes. For this model of a much larger scale, surface temperatures and mass flux were again calculated and compared with results for the slope scale models. Incoming long wave radiation from the atmosphere only was found to be critical input data for accurate temperature calculations. The set value for albedo also had a major effect. When suitable long wave data and good estimations of albedo were used, snow surface temperature was calculated with accuracies on the order of several degrees. Additionally, when surface hoar deposition and growth was observed and reasonable temperature results were achieved, calculated values of mass flux were consistently positive. In one instance, observed variations in surface hoar growth across a slope matched calculated variations in mass flux across the same slope.

Metod:                        Vid genomförandet av studien tillämpades en fallstudiedesign. Det empiriska materialet samlades in genom intervjuer utförda på Staples Sweden AB. Slutsats:                      Vid tillämpningen av modellen som utvecklades i studien framkom att distributionslösningen ska utgöras av direktleveranser, konsoliderade direkleveranser, ”cross-docking”, ”cross-docking” med buffert och traditionell lagerhållning. Orderkostnaden skiljde sig mellan distributionssätten där ”cross-docking” var billigast och direktleveransvarianterna var dyrast.

The problem is that no one in the company knows how such a variety of items should be managed and how the structure of order costs would change. Based on these problems, the task for the study arose. Purpose:                      The thesis aims to develop a model for the selection and design of distribution methods for proposing a solution that can manage one million articles. It also aims to identify the structure of order cost for the proposed solution through a cost analysis. Methodology:             The study was conducted by applying a case study design. The empirical data were collected through interviews conducted at the Staples Sweden AB. Conclusion:                 In the application of the developed model showed that the distribution solution should consist of direct shipping, consolidated direct shipping, cross-docking, cross-docking with buffer and traditional warehousing. Order cost differed between distribution where "cross-docking" showed to be the cheapest and direct shipping variants were the most expensive.

Meningsskapande är ett forskningsområde som många forskare inom organisationsstudier har kommit att intressera sig för. Ett annat vanligt förekommande forskningsområde är hållbarhet. Hållbarhet är även något som många företag har blivit intresserade av att fokusera på. Dock finns det inte mycket tidigare forskning om meningsskapande kring hållbarhet i organisationer. Vi ville därför, genom en kvalitativ studie, öka förståelsen för det meningsskapande som sker kring hållbarhetsarbete i företag. Denna studie studerar hållbarhet i ett sammanhang av förändring för att se det meningsskapande som då sker. Detta resulterar i uppsatsens fokus; vad skapas det för mening kring hållbarhet och hur skapas den. För att möjliggöra denna studie har vi intervjuat åtta medarbetare och hållbarhetschefen på Staples. Hållbarhetschefen presenterade i sin intervju hur arbetet kring hållbarhet ser ut och vad Staples har för mål och visioner kring det. I de intervjuer som genomfördes med medarbetare fick de öppet prata och resonera kring hur de själva upplevde och förstod hållbarhetsarbetet. Efter en tolkning utifrån de nio respondenterna kunde vi finna att den etablerade förståelsen för hållbarhetsarbetet på Staples skiljde sig åt mellan medarbetarna. Vissa upplevde att det var svårtolkat, medan andra upplevde det som klart och tydligt.
Sensemaking is a research area that many researchers in organization studies have found interest in. Another common area for research is sustainability. Sustainability is also a subject that many companies have become interested in. However, there is not much previous research on sensemaking about sustainability in organizations. Therefore, we wanted to with a qualitative study increase the understanding of the sensemaking that occurs around sustainability in companies. This study studies sustainability in a context of change to see the meaning that is created. This results in the focus of the study; what kind of meaning is created about sustainability and how is it created. To enable this study, we have interviewed eight employees and the sustainability manager at Staples. In the interview with the sustainability manager, she presented the work that Staples does around sustainability and their goals and visions around it. In the interviews that were carried out with the employees, they talked openly about how they understood and experienced the sustainability work. After an interpretation, based on the nine respondents, we were able to find that the understanding of Staples’ sustainability work was differentiated between the employees. Some of them thought it was difficult to interpret, while others saw it as obvious and clearly.

Minimally invasive surgery (MIS) has become the standard approach for an increasing number and variety of procedures. Designing devices for such surgeries presents many challenges and must address efficiency, accuracy, and ease of use. The complexity of a device’s design likely influences its performance in real life situations. Therefore, identifying the complexity and potential for failures of a device is crucial in the early stage of design in order to ensure the effectiveness and safety of the final product. A complexity measure is explored utilizing design variables such as the maximum number of connections, number of total elements, and number of unique elements within a device. Reverse engineering of medical devices has been completed to begin understanding such complexity variables. The overall objective of this research is to determine the correlation between a medical device’s complexity measure and its failure modes. The nature and frequency of problems associated with various surgical medical devices must be characterized. This paper is an initial investigation and focuses on surgical stapling devices for MIS. The analysis pertains strictly to surgical staplers that simultaneously staple and transect tissue with a design that allows insertion through small incisions via a trocar, wound protector and retractor, or direct insertion. Adverse event reports involving minimally invasive surgical staplers have been retrieved from the U.S. Food and Drug Administration (FDA) Manufacturer and User Facility Device Experience (MAUDE) database from January 2006 – January 2016 and examined to determine trends in the characterization of device problems and prevalence of such problems. A total of 13,312 reports are included in the analysis. 106 events resulted in death, 3234 resulted in injury, and 9972 involved a device malfunction. A yearly analysis has been conducted analyzing the trends in event type (death, injury, and malfunction) and device brands involved in the reports over the past decade. A sample of reports was taken in order to perform a detailed analysis of the event descriptions. The reports are categorized by phase and description of failure modes associated with surgical stapler use. The phases of use in which failures occur have been identified as packaging, reload, articulation, application, firing, cutting, removal, and staple line. FDA recall information associated with these devices was also investigated. An extensive study regarding adverse events reported to the FDA associated with surgical staplers has not been completed since 2004 to the authors’ knowledge, nor a study investigating this specific category of surgical stapling devices. These devices are constantly evolving in regards to their design features, and their application is expanding to more wideranging open and MIS procedures. Despite the prevalence of minimally invasive surgical stapler use, any incident of failure may put a patient’s health and safety at risk. Malformed staples as a result of the firing phase, removal issues, and leaking staple lines were the main contributors to surgical stapler failure in the adverse event reports analyzed. Bariatric and thoracic surgery accounted for the majority of procedure types identified within the reports. The range of procedures in the analysis verifies the expansion of surgical stapler use and application. Various failure modes can be attributed to user error; however, the FDA recall information associated with these devices indicates that device failure shares responsibility. The results of this work contribute to the awareness of both surgical stapling device designers and users, and the importance of such must be heavily emphasized in order to prevent future complications in the field.

Colorectal surgery is widely performed to re-establish the bowel, the part of which is removed because of diverticulitis, intestinal polyps, inflammatory bowel and cancer. Stapled anastomosis is a highly standardized technique and considered the standard of care. However, complications like leakage, stenosis and postoperative bleeding cannot be completely avoided [1,2]. There is limited understanding of the impact of staplers on tissue and basic mechanics of tissue stapling. According to the previous research, compression, staple height, tissue thickness, tissue compressibility, and tissue type have an effect on the patient outcomes [3]. Hence, we conducted this prospective study to analyze the effect of staple height on tissue damage and compression pressure. An attempt was made to determine the relationship between the tissue height and the optimal closed staple height.

Abstract Surgery has changed a great deal in the past decade. The change is denoted by the term “minimally invasive surgery”. The essence of minimally invasive surgery is the use of technology to enable the surgeon to perform his therapeutic tasks in ways which are less physically and physiologically harmful to the patient. At present, the technologies principally in use are imaging technologies (chiefly video and x-ray) coupled with some very basic mechanical devices which allow the surgeon to manipulate organs in various body cavities without opening the body cavity. There is still a long way to go in achieving the full potential of minimally invasive surgery. MEMS technologies can definitely play a role here. I envision the main use for MEMS technology to be in allowing us to design surgical instruments which incorporate various kinds of sensors. Placing micro sensors on the instruments will restore for the surgeon the tactile immediacy of operating with his hands directly on the tissue, instead of through the intermediary of an insensitive and awkward mechanical device. The sensor types which will probably be useful for surgical instruments fall into two overall categories: the first category is sensors which give the surgeon some force feedback as he goes about the job of placing various types of tissue fasteners (staples or sutures). Many surgical complications arise because there is insufficient or inaccurate force feedback to regulate the closing pressure of staples or sutures. Excessive closing pressures can result in tissue necrosis while insufficient pressures may result in leakage through the staple/suture line. The other category of sensors would be various chemo sensors, which can detect the pH, oxygen and carbon dioxide levels in tissue, as well as other chemical entities of clinical importance such as lactic acid.

For the treatment of scoliosis, traditional instrumentations such as Cotrel-Dubousset are very rigid and effect spine growth ([1] & [2]). Recently, spinal fusion-less instrumentation has been clinically introduced. These instrumentations are more spring-like (compliant). Such instrumentations include multiple level staples, vertical expanding prosthesis, and changeable rod system. However, whether the instrumentation is rigid or compliant, the growth of the spine is effected. The actual level of instrument stiffness that affects growth is not known nor well quantified. The goal of this study was to examine the effect of the level of instrumentation stiffness on the simulation of growth of the pig spine by using finite element analysis (FEA).

Whilst its focus has primarily been its humanitarian mandate, prior to the Taliban takeover, WFP had been working to see how its strategic outcomes in the 2018 Country Strategic Plan (CSP) were aligned with viable peace and development efforts nationally. The Taliban takeover has accelerated an already deteriorating humanitarian crisis. Drought in 2021 had left many in the west of the country in need of humanitarian assistance. The collapse of international support, and the freezing of the banking system and assets held overseas has exacerbated already very high levels of poverty and threatened the price of staples in the market. This has necessarily focused all external efforts on the humanitarian response. Despite the severity of the humanitarian situation, WFP is keen not to entirely neglect development and peace aspects, knowing that both are essential to the future of Afghanistan. This short note sets out the likely medium-term policy framework and some considerations for WFP in navigating this. This Policy Note should be read in conjunction with the longer document Theories of Change for WFP Afghanistan’s Contribution to the Triple Nexus: Final Report (Sida and Nelis 2022).

Abstract:
We study the relationship between rising prices of tortillas---the Mexican staple par excellence---and household food insecurity between 2008 and 2014, a period in which global food prices experienced dramatic increases. The use of a unique combination of household-level data and official state-level information on prices allows us exploit signi cant variation in prices across the Mexican states. Since households cannot be tracked across time, we follow Deaton (1985) by constructing a series of pseudo-panels to control for time- invariant unobserved heterogeneity and measurement error. The regression estimates suggest that increasing tortilla prices affected food insecurity rates in Mexico. More speci cally, households with children or those in the second or third income quintile are more likely to be affected.
